AbstractObjectivePinterest (San Francisco, CA) and Instagram (Menlo Park, CA) are 2 popular photo-sharing social media platforms among young individuals. We assessed differences between Instagram and Pinterest in relaying photographic information regarding Zika virus. Specifically, we investigated whether the percentage of Zika-virus-related photos with Spanish or Portuguese texts embedded therein was higher for Instagram than for Pinterest and whether the contents of Zika-virus-related photos shared on Pinterest were different from those shared on Instagram.MethodsWe retrieved and manually coded 616 Pinterest (key words: “zika” AND “virus”) and 616 Instagram (hashtag: #zikavirus) photos.ResultsAmong the manually coded samples, 47% (290/616) of Pinterest photos and 23% (144/616) of Instagram photos were relevant to Zika virus. Words were embedded in 57% (164/290) of relevant Pinterest photos and all 144 relevant Instagram photos. Among the photos with embedded words, photos in Spanish or Portuguese were more prevalent on Instagram (77/144, 53%) than on Pinterest (14/164, 9%). There were more Zika-virus-related photos on Instagram than on Pinterest pertinent to Zika virus prevention (59/144, 41%, versus 41/290, 14%; P<0.0001), the effects of Zika virus on pregnancy (27/144, 19%, versus 32/290, 11%; P=0.04), and Zika-virus-associated deaths (4/144, 2%, versus 0/290, 0%; P=0.01).ConclusionsPinterest and Instagram are similar platforms for Zika virus prevention communication. Abstract This research was to investigate speech acts of Borish Jonshon’s speech concern in the illocutionary act and the use of speech act analysis by Hymes (2014). The speech was held in Prime Minister’s office and Borish Johnson has used a national TV address at 8.30 p.m. The data of this research taken from the script and speech video of the U.K. Prime Minister named Borish Johnson on 23 March 2020 which talked about Covid-19. The research applied descriptive qualitative as a method and hold in Yule’s speech act theory. Based on the analysis, the researcher was found and analyze 222 utterances. Those are consist of, 36% representative, 33% declarative, 16% directive, 9% expressive, and 6% commissive. As the result was representative is the highest use of the illocutionary aspect found in this research. It can be seen that Borish Johnson hoped that his audiences would follow what he said to reduce the coronavirus disease - 19. Keywords: Pragmatics, Speech Act, Covid-19

In 2011, universal lipid screening was recommended for children aged 9 to 11 years; the impact of this recommendation on the lipid clinic setting is unknown. We examined the rate of primary and secondary dyslipidemia diagnoses in a lipid clinic before (2010-2011) and after (2012-2015) the guideline recommendation. We conducted a retrospective study of new clinic patients aged 0 to 20 years seen between April 2010 and April 2015. Chi-square testing was applied. The 345 subjects were 58% males; 48% ≥13 years; 56% Hispanic; and 59% obese. There was no difference in the rate of dyslipidemia diagnoses between periods (before: primary 23%, secondary 73%, no dyslipidemia 4% vs after: 22%, 72%, 6%, respectively; P = .616). There was no significant difference between periods in subject demographics for the total sample, but among those with primary dyslipidemia, pre- to post-guideline percentage of subjects with public insurance decreased (71% to 39%; P = .006). Additional strategies to increase identification of children with dyslipidemia are needed.

Echeveria is a polyploid genus with a wide diversity of species and morphologies. The number of species registered for Echeveria is approximately 170; many of them are native to Mexico. This genus is of special interest in cytogenetic research because it has a variety of chromosome numbers and ploidy levels. Additionally, there are no studies concerning nuclear DNA content and the extent of endopolyploidy. This work aims to investigate the cytogenetic characteristics of 23 species of Echeveria collected in 9 states of Mexico, analyzing 2n chromosome numbers, ploidy level, nuclear DNA content, and endopolyploidy levels. Chromosome numbers were obtained from root tips. DNA content was obtained from the leaf parenchyma, which was processed according to the two-step protocol with Otto solutions and propidium iodide as fluorochrome, and then analyzed by flow cytometry. From the 23 species of Echeveria analyzed, 16 species lacked previous reports of 2n chromosome numbers. The 2n chromosome numbers found and analyzed in this research for Echeveria species ranged from 24 to 270. The range of 2C nuclear DNA amounts ranged from 1.26 pg in E. catorce to 7.70 pg in E. roseiflora, while the 1C values were 616 Mbp and 753 Mbp, respectively, for the same species. However, differences in the level of endopolyploidy nuclei were found, corresponding to 4 endocycles (8C, 16C, 32C and 64C) in E. olivacea, E. catorce, E. juarezensis and E. perezcalixii. In contrast, E. longiflora presented 3 endocycles (8C, 16C and 32C) and E. roseiflora presented 2 endocycles (8C and 16C). It has been suggested that polyploidization and diploidization processes, together with the presence of endopolyploidy, allowed Echeveria species to adapt and colonize new adverse environments.

ABSTRAK Tujuan dilangsungkannya sebuah penelitian ini ialah untuk mengkaji pengaruh kesadaran halal dan label halal terhadap niat beli mie instan Korea pada remaja di Sumenep melalui sikap. Metode penelitian yang digunakan yaitu kuantitatif dengan menggunakan data primer. Sampel yang digunakan yaitu 100 responden yang belum pernah membeli mie instan Korea. Pengujian menggunakan analisis SEM-PLS dengan konstruk yang menunjukkan semua konstruk mempunyai hasil yang valid, sehingga dilanjutkan pengujian pada hubungan antar variabelnya. Secara langsung kesadaran halal berpengaruh signifikan terhadap sikap, namun kesadaran halal tidak berpengaruh signifikan terhadap niat beli. Label halal berpengaruh secara signifikan t atas niat beli, namun label halal tidak berpengaruh signifikan terhadap sikap, dan sikap berpengaruh secara signifikan atas niat beli. Secara tidak langsung, kesadaran halal berpengaruh signifikan terhadap niat beli jika melalui sikap, sedangkan label halal tidak berpengaruh signifikan terhadap niat beli jika melalui sikap. Penelitian ini hanya berfokus terhadap remaja generasi Z usia 13-23 tahun di Sumenep yang belum pernah membeli mie instan Korea. Adapun saran pada penelitian ini adalah remaja generasi Z di Sumenep diharapkan lebih memperhatikan dan peduli terhadap kehalalan suatu produk yang dikonsumsi, dan bagi peneliti selanjutnya diharapkan meneliti hal serupa dengan mengganti objek dan tempat penelitian. Kata Kunci: Kesadaran Halal, Label Halal, Niat Beli, Sikap. ABSTRACT The purpose of this research is to examine the effect of halal awareness and halal label on the intention to buy Korean instant noodles in adolescents in Sumenep through attitudes. The research method used is quantitative using primary data. The sample used is 100 respondents who have never bought Korean instant noodles. The test uses SEM-PLS analysis with constructs that show all constructs have valid results, so that testing continues on the relationship between the variables. Halal awareness directly has a significant effect on attitudes, but halal awareness has no significant effect on purchase intention. Halal labels have a significant effect on purchase intention, but halal labels have no significant effect on attitudes, and attitudes have a significant effect on purchase intentions. Indirectly, halal awareness has a significant effect on purchase intention if it is through attitude, while the halal label has no significant effect on purchase intention if it is through attitude. This study only focuses on Generation Z teenagers aged 13-23 years in Sumenep who have never bought Korean instant noodles. The suggestion in this study is that Generation Z teenagers in Sumenep are expected to pay more attention and care about the halalness of a product that is consumed, and for future researchers it is expected to examine similar things by changing the object and place of research. 86 Background: In NSABP B-32 5611 women with invasive breast cancer were randomly assigned to SLNB plus axillary dissection or SLNB alone. Permanent pathologic assessment of SLNs at participating sites was designed to identify all macrometastases >2 mm (slicing SLNs at approximately 2.0 mm intervals, embedding all slices in paraffin tissue blocks, and examining one H&E-stained slide from each block). Routine use of IHC or deeper sectioning was prohibited. SLN paraffin tissue blocks from negative SLNs were later evaluated centrally for occult metastases deeper in the blocks. In this report we examine the effect of this detailed assessment on the FNR of SLNB. Methods: Routine and cytokeratin IHC stains were used at two widely spaced additional levels. Occult metastases were detected in 616 of 3884 pts (15.9%) with initially negative SLNs; 69.8% had isolated tumor-cells, 27.9% micrometastases, and 2.3% macrometastases (Weaver D, et al: N Engl J Med, 2011). Results: Previously reported FNR in B-32 was 9.8% (75 of 766 pts). Information on additional pathologic assessment was available on 72 of these 75 pts and revealed occult metastases in 23 pts (31.9%). Of those 23 pts, 14 had isolated tumor cells (61%), 7 had micrometastases (30%), and 2 had macrometastases (9%) in the SLN. Including the information from the additional pathologic assessment, the FNR of SLNB in B-32 was reduced to 6.4% (49 of 763 cases). This 35% reduction in FNR was statistically significant (p< 0.001). Among the 23 FN SLNs with occult metastases, the number of positive non-SLNs was 1 in 16 pts, 2 in 2 pts, 3 in 2 pts, 4 in 2 pts, and 6 in 1 pt. Conclusions: In the B-32 trial, more detailed assessment of the SLNs with deeper sectioning and IHC staining would have significantly reduced the FNR of SLNB by about one-third. However, this reduction would have come at the expense of a 16% increase in the rate of axillary dissection by taking occult metastases into account. ImportanceNeonatal hypoglycemia is common, but its association with later neurodevelopment is uncertain.ObjectiveTo examine associations between neonatal hypoglycemia and neurocognitive outcomes at corrected age 2 years.Design, Setting, and ParticipantsExploratory cohort analysis of the Hypoglycaemia Prevention With Oral Dextrose (hPOD) randomized clinical trial was conducted. The trial recruited participants from January 9, 2015, to May 5, 2019, with follow-up between January 26, 2017, and July 31, 2021. Infants were recruited from 9 maternity hospitals in New Zealand and assessed at home or in a research clinic. Children born late preterm and at term at risk of neonatal hypoglycemia but without evidence of acute or imminent illness in the first hour after birth were screened and treated to maintain blood glucose concentrations greater than or equal to 47 mg/dL.ExposuresHypoglycemia was defined as any blood glucose concentration less than 47 mg/dL, recurrent as 3 or more episodes, and severe as less than 36 mg/dL.Main Outcomes and MeasuresNeurologic examination and tests of development (Bayley III) and executive function. The primary outcome was neurosensory impairment (any of the following: blindness, deafness, cerebral palsy, developmental delay, or executive function total score worse than 1.5 SD below the mean).ResultsA total of 1197 of 1321 (91%) eligible children were assessed at a mean of corrected age 24 months; 616 (52%) were male. Compared with the normoglycemia group, children who experienced hypoglycemia were more likely to have neurosensory impairment (111 [23%] vs 125 [18%]; adjusted risk ratio [aRR], 1.28; 95% CI, 1.01-1.60), particularly if they experienced severe episodes (30 [28%] vs 125 [18%]; aRR, 1.68; 95% CI, 1.20-2.36), but not recurrent episodes (12 [19%] vs 125 [18%]; aRR, 1.06; 95% CI, 0.63-1.80). The risk of cognitive, language, or motor delay was similar between groups, but children who experienced hypoglycemia had lower Bayley-III composite cognitive (adjusted mean difference [aMD], −1.48; 95% CI, −2.79 to −0.18) and motor scores (aMD, −2.05; 95% CI, −3.30 to −0.79).Conclusions and RelevanceIn children born at risk of hypoglycemia but otherwise well, those who experienced neonatal hypoglycemia were more likely to have neurosensory impairment at corrected age 2 years, with higher risks after severe episodes. Further research is required to determine causality.

Blast disease is considered as a major limiting factor in the global rice production because of its wide distribution and destructiveness and it has been causing significant yield loss in all rice growing areas of Nepal. Host resistance is the most desirable means of managing blast, especially in developing countries. Considering the importance of this disease field screening experiment was conducted to identify resistant rice genotypes against this disease. A total of 314 and 346 rice genotypes with resistant (Sabitri) and susceptible checks (Sankarika) were evaluated under epyphytotic conditions during 2016 and 2017 summer seasons at Rampur, respectively. During 2016 disease severity varied from 1 to 9 and only five genotypes; Sabitri, IR 12L 110, WAS122-IDSA14-WASB-FKRI, IR 10F 559and IR 10F 616 were resistant, 30 moderately resistant, 150 susceptible and 129 highly susceptible against blast disease. Similarly during 2016 out of 346 genotypes, 23 resistant as ARIZE SWIFT GOLD, IR95784-21-1-1-2, NR2169-10-4-1-1-1-1-1-2, NR2169-10-2-3-1-1-1-1-1, NR2181-165-1-1-1-1-1-1-1, NR2167-48-5-1-2-1-1, NR2171-2-1-1-3-1-1-2, NR2170-5-5-1-6-1-1-3-1, NR2170-31-1-1-5-1-1-1-1, NR2167-41-1-1-3-1, NR2172-34-1-1-1-1-1-1-1, Sabitri, IR82589-B-B-114-3, IR79913-B-238-3-3, IR93823-36, IR08L 152, IR82589-B-B-51-4, IR09F 434, IR55423-01, IR94391-131-353-19-B-1-1-1-1-1, NR2154-8-1-1-1-1-1, NR 2124-43-3-1-1-1, NR2160-68-1-1-1-1-1., 72 moderately resistant, 191 susceptible and 155 were highly susceptible. Background:polyarteritis nodosa (PAN) is a primary systemic vasculitis that is becoming a rare disease in part by the decrease in hepatitis B virus (HBV) infection due to widespread vaccination. It is characterized by a full vast constellation of nonspecific clinical manifestations, which sometimes delays and makes it difficult to diagnose. Still, muscle involvement is a feature that could guide the clinician.Objectives:to describe the main clinical and laboratory characteristics of patients with PAN and to confirm the frequency of muscle involvement.Methods:retrospective cross-sectional descriptive study of 23 adult patients diagnosed with PAN between January 2011 and December 2018 in two high complexity hospitals in Medellin-Colombia.Results:twenty-three patients met ACR 1990 classification criteria for PAN, 52% were men with a median age of 51 (IR 36-60), 78.3% were newly diagnosed, and only two patients (8.7%) had HBV infection. General symptoms (found in 95% of the patients), cutaneous (82%), and articular (56%) were the most frequent manifestations. Among systemic symptoms, myalgia, especially calf pain, was the most common characteristic (78.3%), followed by weight loss (73.9%), fatigue (69.3%), and fever (59.3%). Laboratory findings and severity scores are shown in the table. Angiography was performed in 27.3% of patients, finding splanchnic (renal, hepatic and splenic) microaneurysms (17.4%), stenosis (13%), and renal infarction (4.3%). Fourteen patients (61%) had at least one positive biopsy documenting medium-sized artery vasculitis, mainly skin, muscle, nerve, or both; 9 (39%) had normal or inconclusive biopsy findings. All patients received high daily doses of prednisolone (50 ± 16 mg); 52.2% required cyclophosphamide, 30.4% azathioprine, 17.4% methotrexate, 8.7% rituximab, 4.3% dapsone and 4.3% plasmapheresis; acetylsalicylic acid was given to half of the patients and only one required antiviral therapy for HBV. With treatment, 87% improved; 22.7% had an infection, and 8.7% of patients died.Conclusion:myalgia was the main characteristics of our PAN patients, especially in calves, and its presence in patients with other general, skin or articular symptoms should raise the suspect of this vasculitis.References:[1]Karadag O, Jayne DJ. Abstract ‘Chance ends some governments. . . . The electoral calendar terminates other executives. . . . Still other governments collapse as a result of deliberate party choice’ (Mershon 2002: 143). Understanding the sources of cabinet durability1 and the dynamics of cabinet termination in parliamentary systems of government is ‘a very important substantive concern for political science’ (Laver 2003: 23). After all, low levels of cabinet duration may have implications for the extent to which executives are able to dominate legislatures (Lijphart 1999: 129); for ministerial efficiency vis-à-vis the bureaucracy (Huber and Lupia 2000); for a political system’s and government’s policy performance (Huber 1998); and for democratic regime legitimacy (Linz 1978: 66–9; Warwick 1994: 5). In post-war Western Europe, cabinet durations have varied considerably within and across countries (see Table 10.1). There is a prolific tradition of empirical and theoretical work in comparative political science seeking to explain such variations (see the survey in Laver 2003). Nevertheless, progress has been hampered, firstly, by a lack of integration of theoretical and empirical work (Mershon 2002: 187; Laver 2003: 38–9).

El análisis de la educación financiera y su impacto en la gestión de organizaciones requiere de actualización y transformación constante en el contexto. La presente labor investigativa tuvo como objetivo principal demostrar la relación existente entre la educación financiera y la gestión organizacional de comunidades campesinas en la provincia de Tarma. Con un estudio orientado por un enfoque cuantitativo, de tipo básico, a nivel correlacional y con un diseño no experimental. La muestra fue un estudio censal a 115 directivos de 23 comunidades campesinas de la provincia de Tarma. Se empleó la técnica de encuesta y como instrumento, dos cuestionarios: Encuesta de percepciones, conocimientos y habilidades financieras propuesto por Avendaño et al. (2020) y Encuesta de Gestión Organizacional de Echevarria (2017). Con coeficientes de validez de contenido de 0,82 y 0,84 respectivamente y nivel de confiabilidad Omega de McDonald de 0,864 y 0,987. Los resultados ponen en evidencia que existe correlación entre educación financiera y gestión organizacional (Tau b de Kendall = ,016) el resultado es significativo (p valor <0,01). Se concluye que cuanto mayor es el nivel de educación financiera mejor es la calidad de la gestión organizacional en las comunidades campesinas.

Este estudo objetiva identificar o nível de conhecimento e aplicação da educação financeira por idosos endividados na cidade do Rio de Janeiro, RJ. A pesquisa possui abordagem qualitativa, quantitativa e descritiva. Como instrumento de pesquisa foi aplicado um questionário contendo oito questões, e 138 respondentes foram validados. A amostra é composta por idosos: de 60 a 65 anos (12%), de 66 a 70 anos (25%), de 71 a 75 anos (38%), e acima de 76 anos (25%). Os resultados demostram que cerca de 62% atribuem seu nível de endividamento ao excesso de consumo, sendo que 49% reconhecem que controlam apenas às vezes seus gastos. A principal fonte de conhecimentos para administrar receitas e gastos para 23% dos idosos foi a escola/faculdade, e para 37% foi Internet, jornais e outros. Além disso, 75% dos idosos entrevistados participam de maneira significativa no orçamento doméstico. Sendo que 44% dos idosos vivem em casa com até 3 pessoas, 30% com 4 ou 5 pessoas, 18% com mais de 5 pessoas, e apenas 8% com até 2 pessoas. A maioria, 83%, possui endividamento com condições de pagamento, e 9% admitem não terem condições de quitar suas dívidas. Conclui-se ser fundamental a educação financeira de idosos, considerados vulneráveis, na prevenção do excesso de endividamento e inadimplência.

Minoru Isobe of Nagoya University extended (Synlett 2009, 1157) conjugate addition–intramolecular epoxide opening to substrates such as 4, leading to the cyclobutane 6 with high diastereocontrol. In the course of a total synthesis of (+)-brefeldin A, Jinsung Tae of Yonsei University established (Synlett 2009, 1303) conditions for the trans-selective cyclization of 7 to 8. Cyclization with TiCl4 gave the alternative cis diastereomer. Several methods have been put forward for the conversion of carbohydrate derivatives to carbocycles. Yeun-Mi Tsai of the National Taiwan University found (Tetrahedron Lett . 2009, 50, 3805) that acyl silanes such as 9 cyclized efficiently under free radical conditions, leading to the silyl ether 10. In influence maximization (IM), the goal is to find a set of seed nodes in a social network that maximizes the influence spread. While most IM problems focus on classical influence cascades (e.g., Independent Cascade and Linear Threshold) which assume individual influence cascade probability is independent of the number of neighbors, recent studies by sociologists show that many influence cascades follow a pattern called complex contagion (CC), where influence cascade probability is much higher when more neighbors are influenced. Nonetheless, there are very limited studies for complex contagion influence maximization (CCIM) problems. This is partly because CC is non-submodular, the solution of which has been an open challenge. In this study, we propose the first reinforcement learning (RL) approach to CCIM. We find that a key obstacle in applying existing RL approaches to CCIM is the reward sparseness issue, which comes from two distinct sources. We then design a new RL algorithm that uses the CCIM problem structure to address the issue. Empirical results show that our approach achieves the state-of-the-art performance on 9 real-world networks.

Abstract ASME Section XI Case N-666, Weld Overlay of Class 1, 2, and 3 Socket Welded Connections, was developed for repair of non-isolable leaking socket welds where the mode of failure is determined to be fatigue cracking attributed to mechanical vibration [1]. The case was approved by ASME on July 23, 2002 and first published in the 2004 Code Cases: Nuclear Components, Supplement 9. Case N-666 and N-666-1 have served the industry well, allowing repair of leaking socket welds where the system cannot be readily drained and isolated for cutout and full pipe replacement. This has provided significant benefit to the nuclear industry by allowing the repair to be performed online without the need to shut the plant down, avoiding significant loss in power generation. This paper provides the technical basis for proposed changes to Case N-666-1 that will expand the scope of socket welds that can be repair by weld overlay. The revision will expand the case to include overlay repair of dissimilar metal socket welded joints and will remove the 0.30 wt% maximum carbon content limitation when the overlay repair is installed with water backing. Finally, the revision will also address the conditions imposed by the NRC in Regulatory Guide 1.147 Revision 19.

The improvement of thermal efficiency by increasing the operating temperature and pressure of boilers has recently led to the development of new creep-resistant steels. For 9–12% Cr steels, grade T/P91, developed at the end of the seventies by CE (Combustion Engineering) and ORNL (Oak Ridge National Laboratories) [1] in the USA, marked the starting point for these developments. With its excellent mechanical characteristics at elevated temperatures and good workability, it was rapidly adopted worldwide for erecting new power generators. The tube design temperature is limited to around 610°C (1130°F) inside the combustion chamber. This limit depends on such factors as heat flow and corrosion. Steam lines in P91 are generally operated at temperatures below or slightly above 600°C (1112°F). More recent developments to produce new grades such as T/P911 and T/P92, have improved mechanical properties at high temperatures, in particular an increase in creep strength of 10 to 30% in 100,000 hrs at 600°C (1112°F). This makes it possible to reduce the wall thickness of the pipes and consequently improve their behavior to thermal fatigue. The new grades T/P23 and T/P24 are well suited for boiler components working at lower temperatures. While these grades were initially developed for manufacturing the water wall panels of Ultra Super Critical Boilers (USCB), they are also used for the superheaters and reheaters of conventional boilers and Heat Recovery Steam Generators (HRSG) in combined cycles. In addition to their excellent workability, they have the advantage to be used without post-weld heat treatment in case of thin wall tubes. Furthermore, owing to the good creep properties, they can be used to replace P22 and for some applications even P91, with the advantage of lower costs. This paper sets out the characteristics of these new grades and gives the main recommendations for processing them.

Abstract High temperature regions in the upper sections of the advanced ultrasupercritical (AUSC) boilers are exposed to temperatures higher than traditional supercritical (SC) boilers and require high strength materials. Use of modified 9-12% Cr materials such as T91 and T92, while meeting the strength requirements, are still under research stage for large-scale fabrication of the membrane walls for several reasons, such as required post weld heat treatment PWHT (ASME Code) or hardness limits on as-welded structures (European codes). The main objective of this paper is to explore alternate tubing materials that do not require a PWHT in the high temperature sections of the AUSC boiler membrane walls. Composite bimetallic tubing with high strength cladding, applied by weld overlay or co-extrusion that may meet the requirement of high operating temperature and high overall strength, is addressed through an alternate design criterion. Bimetallic tubes can replace the single metal tubes made from 9-12% Cr materials. The bimetallic tube is assumed to be fabricated from Grade 23 steel (base tubes) with Alloy 617 overlaid. The alternate design method is based on an iterative analytical solution for the through-wall heat transfer and stresses in a composite tube with temperatures and strength variations of both the materials considered in detail. A number of different analyses were performed using the proposed analytical approach, methodology verified through benchmark solutions and then applied to the membrane wall configurations.

Actimetry has been shown to be useful for non-parametric analysis of biological activity/rest rhythms. We verified the pattern of activity and rest of 5 women from the city of Maringá/Pr during 30 days. We applied a sociocultural questionnaire (CONEP: 4,032,221). A (31), B (46), C (54), D (55), E (61) years. Total sleep time for A was between 6:00 am and 2:02 pm, with an average of 11 awakenings. For B, 5:10 am and 9:14 am, with an average of 21 awakenings. Participant C, 5:23 am and 10:16 am, with an average of 10 awakenings. For D, 6:02 am and 11:38 am, with an average of 16 awakenings. Participant E, 5:16 am and 8:14 am, with an average of 10 awakenings. All of them presented circadian rhythm and irregularities in sleeping and waking up during the week, with an increase in the number of hours slept on the weekends. They reported worsening sleep quality during the pandemic. When asked about the state of anxiety in relation to the moment, on a scale from zero to ten, participants A, B, C and D answered 9 and E answered 8. In relation to fear of possible health and financial risks, A, C and D scored 8, B and E scored 6 for health. A, B and C scored 9, E scored 6 and D scored 8 for the financier. We can infer that sleep suffers environmental/social influences. Actions that lead to selfknowledge can contribute to the recognition of sleep changes, allowing control and monitoring of real situations of external conflict.

Se hace necesario implementar programas, para imputados Duales con el fin de lograr una inserción real a la sociedad.

ЦЕЛЬ: оценить влияние вариабельности гликемии у больных сахарным диабетом 2 типа (СД2) во время стационарного лечения по поводу острого инфаркта миокарда (ОИМ) на прогноз. МАТЕРИАЛЫ И МЕТОДЫ: в одноцентровое исследование включались больные СД2, госпитализиро- ванные в Региональный сосудистый центр «Городской клинической больницы №13» г. Нижнего Новгорода по поводу ОИМ, подвергнутые чрескожному коронарному вмешательству (ЧКВ) за период с января по ав- густ 2021 года. Всего включен 161 пациент. Средний возраст по группе составил 66±9 лет. Медиана длитель- ности СД2 - 7 [1; 10] лет. Медиана количества измерений гликемии в течение госпитализации составила 21 [14; 29] раз. Гликемический контроль оценивался по показателям средней гликемии, стандартному от- клонению (SD), коэффициенту вариации (Coefficient of Variation, CV), который определялся как отношение SD гликемии к показателю средней гликемии в течение госпитализации. Под низкой вариабельностью гликемии понимали утвержденное рекомендациями целевое значение CV ≤36% Отдалённые исходы оценивались через 12 месяцев после выписки из стационара. Статистическая обработка проводилась в программе Statistica 10.0. РЕЗУЛЬТАТЫ: летальные исходы в исследуемой когорте зафиксированы в 11 случаях (7%). В течение 12 месяцев после выписки наступили еще 12 смертельных исходов (8%), то есть за весь период наблюдения 23 случая. Выжившие пациенты имели лучшие показатели средней гликемии 9,4±1,7 vs 12,1±2,5 ммоль/л (p<0,001), SD гликемии 2,9±1,1 vs 4,2±1,5 ммоль/л (p< 0,001), CV 29,9±9 vs 36,3±14,2% (p=0,01, t-тест). Среди пациентов с благоприятным исходом низкую вариабельность (CV≤36%) во время стационарного этапа лечения ОИМ имели 107 (78%) против 11 (48%) умерших, (p=0,009, χ2 Pearson). При проведении много- факторного анализа CV≤36% достоверно ассоциируется со снижением риска наступления сердечно-со- судистой смерти в течение 12 месяцев после выписки по поводу ОИМ (ОР 0,29; 95% ДИ 0,09-0,89, р=0,03, Cox regression). ВЫВОДЫ: у больных СД2 во время стационарного лечения по поводу ОИМ низкая вариабельность гликемии (СV≤36%) достоверно ассоциируется со снижением риска наступления летального исхода в те- чение года после выписки.

INTRODUCCION El pronóstico y evolución en el Trastorno por Uso de Alcohol (TUA) y Trastorno Mental (TM) está condicionada por la multifactorialidad: falta de adherencia, recaídas, y complicaciones orgánicas. OBJETIVOS Identificar patología psiquiátrica concomitante. Analizar las tasas de adherencia a consulta y tratamiento. Correlacionar patología psiquiátrica con índices de retención. METODO Estudio descriptivo observacional longitudinal. Referencia muestral: 77 pacientes con TUA de 472 incluidos en Plan de Microeliminación del VHC de San Miguel Adicciones (2017 a 2020). Variables de estudio: sociodemográficas, clínicas y de consumo e indicadores de cumplimiento y retención. RESULTADOS Muestra: 62% hombres y 38% mujeres (34 y 70 años). Inicio del consumo de alcohol entre 15 y 18 años. Presentan Patología Dual 69% mujeres y 42% hombres. 85% de pacientes con TUA presentan un solo TM concomitante (95% hombres, 80% mujeres). Patología psiquiátrica predominante: Trastorno Depresivo 22% (H:53%;M:47%), Trastorno de Ansiedad 7% (H:60%;M:40%), Trastorno Ansioso Depresivo 8% (H:34%;M:66%). Destacar la prevalencia de un 18% de mujeres con Trastorno Psicótico. El 58% de mujeres han realizado uno o más Gesto Autolítico. Acuden por Iniciativa Propia 49%, derivación MAP (25%), Especializada 9%. Trastorno Depresivo: 82% de adherencia a consulta, 47% de retención superior a 6 meses. Trastorno de Ansiedad (20%) y 60% de retención entre 6 y 12 meses. CONCLUSIONES Mujeres con TUA presentan más comorbilidad psiquiátrica. La prevalencia de más de un Trastorno Mental es superior en mujeres. El Trastorno Depresivo es el de mayor incidencia con TUA. Escasa derivación desde los Servicios Especializados. References 1. INTRODUCCION La Patología Dual (PD) en el consumo de alcohol condiciona el pronóstico, adherencia y retención en la intervención ambulatoria. OBJETIVOS Conocer prevalencia de patología dual. Determina tasas de adherencia a consulta y tratamiento. Analizar correlación PD cumplimiento y efectividad terapéutica. METODO Estudio descriptivo observacional longitudinal. Referencia muestral: 77 pacientes con TUA de 472 incluidos en Plan de Microeliminación del VHC de San Miguel Adicciones (2017 a 2020). Variables de estudio: sociodemográficas, clínicas y de consumo e indicadores de cumplimiento y retención. RESULTADOS Muestra: 62% hombres y 38% mujeres (34 y 70 años). 52% Patología Dual (69%M; 42%H). Patologías psiquiátricas: 22% trastorno depresivo (53%H;47%M), 7% ansiedad (60%H;40%M), 8% síndrome ansioso depresivo (34%H;66%M). Tentativas autolíticas 16%, (42%H;58%M), 7% más de un trastorno psiquiátrico. Acuden por Iniciativa Propia 49%, derivación MAP (25%), Especializada 9%. La retención es superior en los pacientes que no tienen PD (>12 meses). Menor índice de retención en pacientes con trastorno psiquiátrico 40%. Trastorno Depresivo: 82% de adherencia a consulta (AC), 47% de retención (R) > 6 meses, 29% abandona (A) y 24% alta terapéutica (AT). Trastorno de Ansiedad: 20%(AC), 60% (R) >6 meses, 40% (A), 0% (AT). Trastorno Ansioso-Depresivo: 10% (AC), 67%(R) > 6 meses, 50% (A) y 33% (AT). CONCLUSIONES Predominio de trastornos afectivos: depresión, ansiedad. Diferencia en el factor género en TUA y PD. Prevalencia de más de un Trastorno Mental superior en mujeres. Objective The aim of this living systematic review is to provide a summary of the effects of remdesivir in the of people with acute covid-19. Methods Eligible studies were randomised trials evaluating the effect of remdesivir versus placebo or no treatment. We conducted searches in COVID-19 L·OVE (Living OVerview of Evidence) platform, a system that performs regular searches in databases, trial registries, preprint servers and websites relevant to COVID-19. All the searches covered the period until 06/23/2023. No date or language restrictions were applied. Two reviewers independently evaluated potentially eligible studies according to predefined selection criteria, and extracted data on study characteristics, methods, outcomes, and risk of bias, using a predesigned, standardised form. We performed meta-analyses using random-effect models and assessed overall certainty in evidence using the GRADE approach. Results Our search strategy yielded 897 references. 10 randomized controlled trials reported in 15 references met our inclusion criteria and were included in the review. The relative risk of mortality was 0.91 (95% CI 0.83 - 1.00). This means remdesivir reduced the risk of mortality in 9%, compared with placebo or no treatment. In the studies identified in this review 760 people not receiving remdesivir out of 5372 presented the outcome Mortality (141 per 1000) versus 732 out of 5790 in the group that did receive it, with a risk difference of -12.22 per 1000 (from -24 to 0). In other words, -12.22 per 1000 (from -24 to 0) people did not develop the outcome because of the intervention. We assessed the certainty of the evidence for this outcome as low. The estimated absolute effect, -12.22 per 1000 (from -24 to 0), falls in the range of an effect of small magnitude. The relative risk of mechanical ventilation requirement was 0.62 (95% CI 0.40 - 0.94). This means placebo or no treatment reduced the risk of mechanical ventilation requirement by 38%, compared with .
