Smart textile structures such as sensor yarns provide real possibility for in situ structural health monitoring of textile reinforced thermoplastic composites. In this work thermal properties of E-glass/polypropylene (GF/ PP) and E-glass/poly(N,N’-hexamethylene adipamide) (GF/PA66) sensor yarns based on conductive polymer complex [3,4(ethylenedioxy)thiophene]-compl-poly(4-vinylbenzenesulfonic acid) (PEDOT-compl-PSS) and related composites were studied. Thermogravimetric analysis (TGA), microscale combustion calorimetry (MCC) and limiting oxygen index (LOI) methods were used to detect thermal behaviour of these structures and effect of coatings applied. According to TGA, GF/PP sensor yarn started to decompose at higher temperature, 345 °C, and showed higher pyrolysis residue, 28 %, compared to GF/PA66 sensor yarn that started to decompose at 316 °C and had lower pyrolysis residue, 23 % . The MCC showed that Heat Release Rate peaks of GF/PP sensor yarn, 341 W/g, and GF/PA66 sensor yarn, 348 W/g, occurred at similar Heat Release Temperature, ~ 430 °C. The additional peak, 51 W/g, was detected for GF/PP sensor yarn at 493 °C. Finally, LOI 22 and LOI 23 were detected only for GF/PP and GF/PA66 composites with integrated sensor yarns.

One hundred and thirty-one pregnant, parous Border Leicester x Merino ewes at pasture were supplemented with a mixture of lupin and oat grain for a minimum of 3 weeks prepartum, and an average of 1 week post-partum in a lamb production experiment. Supplement was group-fed daily at a rate of 400 g/ewe. Pasture and grain intake were determined on 1 day, at an average 7 days prepartum. On an individual ewe basis, there was considerable variation in supplement intake with 11% of ewes consuming <200 g/day and 10% consuming >700 g/day. Significantly (P<0.01) more supplement but less pasture (P<0.05) was eaten by twin-bearing ewes (440 � 23 g) than single-bearing ewes (340 � 36 g). Total feed intake was similar with less variation than in supplement or pasture intakes. Results suggest lower pasture intakes in twin-bearing ewes can be compensated by higher grain intakes which means that separation of single- and twin-bearing ewes may not always be warranted in a supplementation program.

The Internet impacts students' lives and career paths. Excessive use may correlate with tobacco and alcohol consumption despite its benefits. The present study aimed to identify the relationship between gratification of Internet usage, problems in Internet usage, alcohol consumption, and drug abuse among Malaysian youth. The quantitative method was applied by using a survey questionnaire. A total of 440 students from the University Putra Malaysia (32.3% males, 67.7% females) were selected using a stratified random sampling method. The majority of the respondents (93.2%) were single. The students ranged from 23 to 40, with the majority of the younger group (78.3%) in the age range of 18-23. No link was found between Internet use gratification and alcohol/drug usage. No correlation was found between the Internet, alcohol, and drug problems. The association between age and alcohol usage was not significant. However, this relationship for drug abuse was substantial and positive. No significant differences were found in the mean values for the respondents' fields of study and alcohol or drug usage. The findings of the independent t-test illustrated that there were significant differences between marital status and alcohol consumption. Still, there were no significant mean differences between marital status and drug usage.

Abstract. In August 2021, a historic heatwave was recorded in Greece which resulted in extreme wildfire events that strongly affected the air quality over the city of Athens. Saharan dust was also transferred over Greece on certain days of the same period due to the prevailing southern winds. The impact of these events on air quality and surface solar radiation is investigated in this study. Event characterization based on active and passive remote sensing instrumentation has been performed. The study shows that significantly increased levels of air pollution were recorded from the end of July to the first week of August. The smoke led to unusually high aerosol optical depth (AOD) values (up to 3.6 at 500 nm), high Ångström exponent (AE) (up to 2.4 at 440–870 nm), and a strong and negative dependence of single-scattering albedo (SSA) on wavelength that was observed to decrease from 0.93 at 440 nm to 0.86 at 1020 nm, while the dust event led to high AOD (up to 0.7 at 500 nm), low AE (up to 0.9 at 440–870 nm), and a positive dependence of SSA on wavelength that was observed to increase from 0.89 at 440 nm to 0.95 at 1020. Furthermore, the smoke plume was also detected over the PANhellenic GEophysical observatory of Antikythera on 7 August, which is about 240 km away from Athens. Increased AOD values (up to ∼ 0.90 at 500 nm) associated with a high fine-mode AOD (up to ∼ 0.85 at 500 nm) and decrease in SSA with wavelength suggested the dominance of fine biomass burning aerosols. The impact of dust and smoke on solar irradiance revealed significant differences in the spectral dependence of the attenuation caused by the two different aerosol types. The attenuation of solar irradiance in the ultraviolet (UV-B) spectrum was found to be much lower in the case of dust compared to smoke for similar AOD500 values. Differences were less pronounced in the near-infrared and visible spectral regions. The large AODs during the wildfires resulted in a decrease in the noon UV index by up to 53 %, as well as in the daily effective doses for the production of vitamin D (up to 50 %), in the daily photosynthetically active radiation (up to 21 %) and in the daily global horizontal irradiance (up to 17 %), with serious implications for health, agriculture, and energy. This study highlights the wider impacts of wildfires that are part of the wider problem for Mediterranean countries, whose frequency is predicted to increase in view of the projected increasing occurrence of summer heatwaves.

Objectives:The risk of subsequent fractures is double the risk of having a first fracture. We analysed whether this risk is constant or not over time.Methods:A population-based study in 4140 postmenopausal women, aged between 50 and 90 years, on radiographic confirmed clinical fractures from menopause onwards analysed by Cox regression.Results:A total of 924 (22%) women had a first fracture and 243 (26% of 924) a subsequent fracture. Of all first fractures, 4% occurred in each year from menopause onwards, while after a first fracture 23% of all subsequent fractures occurred within 1 year and 54% within 5 years.When calculated from time of first fracture, the relative risk (RR) of subsequent fracture was 2.1 (95% CI 1.7 to 2.6) and remained increased over 15 years. When calculated for specific time intervals after a first fracture, the RR was 5.3 (95% CI 4.0 to 6.6) within 1 year, 2.8 (95% CI 2.0 to 3.6) within 2–5 years, 1.4 (95% CI 1.0 to 1.8) within 6–10 years and 0.41 (95% CI 0.29 to 0.53) after >10 years.Conclusions:From menopause onwards, clinical fractures cluster in time, indicating the need for early action to prevent subsequent fractures.

346 Background: AFP is elevated in 70% of HCC and is associated with poor prognosis. The role of AFP as a biomarker of response to systemic treatments has not been established, though small, retrospective studies show association between AFP decline and survival on sorafenib. The relationship between AFP changes and response to immune checkpoint inhibitors (CPI) has not been reported. This study examines AFP changes on treatment for association with outcomes on first-line (1L) SOR and any subsequent CPI in a contemporary, multicenter U.S. population. Methods: Design: Multicenter retrospective case series. Key eligibility: Received 1L SOR or SOR-based combination for advanced HCC; ≥ 1 post-treatment AFP value available; enrolled on IRB-approved registry. Objectives: associate AFP changes within 3 months of start of treatment with overall survival (OS) and time on treatment (TOT) on 1L SOR and any subsequent CPI; associate baseline AFP with OS and TOT for SOR and CPI; relate baseline AFP and changes on treatment to clinical covariates. Results: 152 patients were identified from two centers. Baseline characteristics: M/F 132/20; HBV/HCV/nonviral 40/71/41; Child Pugh A/B 128/23; BCLC A/B/C: 4/15/133. Baseline AFP < 20/ ≥ 400: 43/59. 43 received CPI after SOR. See Table. Baseline AFP was not related to TOT or OS for SOR or CPI. Multivariable analyses for AFP with clinical covariates will be presented. Conclusions: This series is the largest multicenter analysis of AFP response to systemic therapy with SOR and CPI. AFP decrease and increase within the first 3 months of treatment with SOR and CPI were inversely and significantly associated with median OS. AFP warrants further study in randomized cohorts as a biomarker of response to systemic therapy in HCC, now with multiple treatment options available at progression.[Table: see text]

This study proposes a novel technique for computing cloud feedbacks using histograms of cloud fraction as a joint function of cloud-top pressure (CTP) and optical depth (τ). These histograms were generated by the International Satellite Cloud Climatology Project (ISCCP) simulator that was incorporated into doubled-CO2 simulations from 11 global climate models in the Cloud Feedback Model Intercomparison Project. The authors use a radiative transfer model to compute top of atmosphere flux sensitivities to cloud fraction perturbations in each bin of the histogram for each month and latitude. Multiplying these cloud radiative kernels with histograms of modeled cloud fraction changes at each grid point per unit of global warming produces an estimate of cloud feedback. Spatial structures and globally integrated cloud feedbacks computed in this manner agree remarkably well with the adjusted change in cloud radiative forcing. The global and annual mean model-simulated cloud feedback is dominated by contributions from medium thickness (3.6 < τ ≤ 23) cloud changes, but thick (τ > 23) cloud changes cause the rapid transition of cloud feedback values from positive in midlatitudes to negative poleward of 50°S and 70°N. High (CTP ≤ 440 hPa) cloud changes are the dominant contributor to longwave (LW) cloud feedback, but because their LW and shortwave (SW) impacts are in opposition, they contribute less to the net cloud feedback than do the positive contributions from low (CTP > 680 hPa) cloud changes. Midlevel (440 < CTP ≤ 680 hPa) cloud changes cause positive SW cloud feedbacks that are 80% as large as those due to low clouds. Finally, high cloud changes induce wider ranges of LW and SW cloud feedbacks across models than do low clouds.

The chemical composition of essential oils isolated from immortelle (Helicrysum italicum subsp. italicum) collected in Herzegovina during five different periods, was investigated by GC/MS analysis. The main compounds were a-pinene (15.7 %) and γ-curcumene (12.8 %), followed by 4,6,9-trimethyldec-8-en-3,5-dione (8.7 %), neryl acetate (6.9 %), limonene (6.4 %) and β-selinene (5.3 %). In total, 69 components were identified whose share changed over the vegetative cycle. Antioxidant activity of methanolic extracts of immortelle were determined according to DPPH (IC50 = 23–34 μg/mL) and FRAP (29 μg/mL is equivalent to 1.1‒2.2 mM Fe2+) methods. Acetylcholinesterase inhibitory potential, investigated by modified Ellman’s assay and determined as IC50 values, were 340–440 μg/mL for methanol extracts and 135 μg/mL for essential oil. Metanolic extracts showed strong antioxidant activity and potential to inhibit AChE. Essential oil possesses complex chemical composition, inhibition activity of AChE and weak antioxidant capacity.

The breeding ecology of the red squirrel (Tamiasciurus hudsonicus) was studied in a mixed conifer – hardwood forest in southern Québec between June 1978 and October 1980. There were two mating seasons per year, one in March–April and one in June–July. The average size of 12 litters was 4.2 young (4.6 in 1979 and 4.0 in 1980). The sex ratio (female: male) of young was 23:25. The earliest recorded age at first successful mating was 10 months. In 1979, nearly all females, including yearlings, bred successfully in the spring and 50% of them raised a second litter. In 1980, only adult females produced young in the spring and four of six yearlings reproduced in the summer. A maximum of 29% of all females could have produced two litters in 1980. Differences in birth rate between the two years may be explained by a lower quality and (or) quantity of food resources at mating time rather than a drop in food supply available the previous fall.

O principal objetivo deste estudo foi verificar as alterações odontológicas mais freqüentes encontradas em jogadores de futebol. Num levantamento realizado pela Seção de Odontologia da Associação Portuguesa de Desportos-SP, 400 prontuários de futebolistas, correspondentes a um período de seis anos, foram avaliados. Todos eram do sexo masculino, sendo 353 amadores e 47 profissionais, com média de idade de 16,5 ± 2,1 anos (variação de 9 a 20) e 24,7 ± 3,6 anos (variação de 21 a 34), respectivamente, pertencentes ao quadro de atletas do Departamento de Futebol. Os equipamentos utilizados durante os procedimentos foram: espelho clínico e explorador nº 5 da marca Duflex em um consultório modelo MLX Plus da marca Funk. As radiografias oclusais foram realizadas com filmes da marca Kodak no equipamento de raios-X modelo Spectro II da marca Dabi Atlante. As seguintes alterações e os resultados encontrados nos futebolistas amadores foram: 199 casos (56%) de tartarectomia, 283 de cáries (71%), 61 de gengivites (17%), 51 de endodontias (14%), 51 de pulpites (14%), 33 de abscessos (9%), 21 de fístulas (6%) e 10 de aftas (3%). Nos casos de ortodontia foram verificados: diastema em 12 atletas (3,5%), apinhados em 35 (10%), heterotópicos em 36 (10,2%) e empactados em 4 (1%). Foram realizadas 78 exodontias (22%), 41 obturações (11,5%) e 72 próteses (20%). A profilaxia realizada atingiu 148 casos (42%). Os jogadores profissionais apresentaram os seguintes resultados: 17 casos de tartarectomia (36%), 32 cáries (68%), 14 gengivites (30%), 11 endodontias (23%), 6 pulpites 13%), nenhum abscesso, 2 fístulas e aftas (4%), respectivamente. Foram verificados 6 casos de ortodontia, subdivididos em 3 apinhados e heterotópicos (6%), respectivamente. Foram realizadas 24 exodontias (51%), 6 obturações (13%) e 23 próteses (49%). Conclusão: os resultados verificados neste estudo demonstraram a importância da odontologia, como medida preventiva e/ou corretiva, em jogadores de futebol. É importante salientar que uma dentição inadequada pode gerar desde alterações gastrintestinais até lesões músculo-esqueléticas, dificultando a participação do atleta em treinamentos e jogos competitivos.

The heterotopically transplanted rat heart (TH) undergoes rapid muscle atrophy and a concurrent shift from alpha- to beta-myosin heavy chain (MHC) by 1 wk after surgery. In the current experiments, TH were continuously paced (420 beats/min) for 1 wk beginning 24 h after surgery or for 1 wk beginning 14 days after surgery to determine the role of increased heart rate in preventing or reversing cardiac atrophy. Left ventricular (LV) wet weight (283 vs. 256 mg paced vs. nonpaced) and protein content (32 vs. 23 mg paced vs. nonpaced, P < 0.05) were significantly elevated in TH paced 1 wk after surgery but were unchanged (211 vs. 198 mg and 24 vs. 23 mg LV wet wt and protein content, respectively) in TH paced 2 wk after surgery. Total cardiac protein synthesis in the TH paced immediately after surgery was increased compared with the corresponding nonpaced hearts (5.6 vs. 4.0 mg.mg LV wet wt-1.day-1, P < 0.05), while in the TH, where pacing was initiated 2 wk after surgery, it was unchanged (3.6 vs. 3.7 mg.mg LV wet wt-1.day-1). Fractional synthesis rate was elevated in TH and was not altered by pacing. Pacing the TH also attenuated the shift in alpha-MHC in the first 7 days after surgery but did not reverse the shift 2 wk later. The increase in protein synthesis combined with an unchanged fractional synthesis rate suggests that pacing attenuates cardiac mass by decreasing protein degradation and that once the atrophic process is established, neither synthesis rate nor isomyosin shift can be altered by continuous pacing.

522 Background: HDCT plus PBSCT is effective salvage therapy for relapsed mGCT but has potential toxicity which can be more pronounced in older patients. We report survival and toxicity outcomes in pts with relapsed mGCT age ≥ 40 at time of HDCT. Methods: 440 consecutive pts with relapsed mGCT were treated with HDCT and PBSCT with tandem cycles at Indiana University (IU) between 2004-2017 per our previous reported regimen (N Engl J Med 2007; 357: 340-8). Kaplan-Meier methods were used for progression free survival (PFS) analysis. Results: 110 pts were age ≥ 40 while 330 pts were age < 40. Among pts age ≥ 40, median AFP was 6.6 (range, 1-2,709) and median hCG was 5.3 (range, 1-42, 453). Of the 110 pts age ≥ 40, 75 had complete remission without relapse during a median follow-up of 23 months. There were 3 treatment-related deaths. Conclusions: HDCT plus PBSCT is safe and effective salvage therapy in pts age ≥ 40 with relapsed mGCT. [Table: see text]

The structural changes effect on functional properties of ribbon shaped samples of the Fe81B13Si4C2 amorphous alloy during annealing process was investigated in this paper. Differential scanning calorimetry method has shown that this alloy crystallizes in one stage, in temperature range from room temperature up to 700?C. Structural relaxation process was investigated by sensitive dilatation method in nonisothermal and isothermal conditions. It has been shown that structural relaxation process occurs in two stages by measuring thermal expansion at constant temperatures of t1=420?C, t2 = 440?C and t3 = 460?C. The first stage is characterized by linear logarithmic dependence of thermal expansion upon time at constant temperature. The second stage of structural relaxation process is characterized by linear dependence of isothermal expansion upon the square root of process time. These results imply that the first stage of structural relaxation process is a rapid kinetic process, while the second stage of structural relaxation process is a slow diffusion process. The rate constants k11 = 2,27?10- 3 s-1, k12 = 2,79?10-3 s-1, k13 = 3,6?10-3 s-1, k21 = 0,67?10-4 s-1, k22 = 3,72?10-4 s-1, k23 = 21,53?10-4 s-1 and activation energies E1 = 48,64 kJ/mol and E2 = 366, 23 kJ/mol were determined for both stages of structural relaxation process. The distinct correlation between structural relaxation process and magnetic susceptibility relative change was determined by thermomagnetic measurements. It has been shown that magnetic susceptibility can be increased by up to 80%, by convenient annealings after structural relaxation process, at magnetic field intensity of 8 kA/m.

Propagation of Winecups [Callirhoe involucrata (Torrey & A. Gray)] for use as a landscape ornamental has been impeded by a lack of understanding of the seed dormancy and a practical method for overcoming it. As with many members of the Malvaceae family, C. involucrata produces hard seed. In the populations tested, it accounted for 90% of an average sample. Impermeability, however, is not the only limiting factor to germination. Three disparate populations of seed, representing two different collection years have been investigated using moist pre-chilling, boiling water, leaching, gibberellic acid, hydrogen peroxide and mechanical and chemical scarification methods. Scarifying in concentrated sulfuric acid stimulates germination of some seed fractions and causes embryonic damage in others, suggesting variation in seed coat thickness. Similar results were obtained using a pressurized air-scarifier; the hard seed coat of some seed fractions were precisely scarified while others were physically damaged using the same psi/time treatment. Placing seed in boiling water increases germination from 4%, 7%, and 18 % to 23%, 25%, and 77% in the three populations, respectively. Leaching for 24/48 h in cold (18 °C) aerated water or for 24 h in warm (40 °C) aerated water showed only a minor increase over the control. Pre-chilling at 5 °C for 30, 60, and 90 days showed no improvement over the control. Gibberellic acid-soaked blotters improved germination at 400 ppm to 20%, 10%, and 41%; at 500 ppm germination was reduced. Soaking seed for 24 h in a 3% concentration of hydrogen peroxide did not effect germination; at a 30% concentration germination was reduced. The considerable variation in seed dormancy expression may be a function of differences in environmental factors during development or seed age.

BackgroundSafety is a driving factor when selecting treatment, but the risks should be balanced against the treatment benefits, relative to alternative treatment options. In JAKis, recent evidence of increased safety risks in older patients with cardiovascular (CV) risk factors, have led to concerns. However, an increased risk among certain patients might still be offset by an increasing benefit in the same patient segment, and vice versa. A full understanding of the JAKi benefit-risk profile thus requires a better understanding of the relative (JAKi vs other treatments) benefits in those patients where their relative safety profile may be worse.ObjectivesTo assess the observed and adjusted relative effectiveness (remission and response) of JAKis (and non-TNFi bDMARDs) when compared to TNFi, as used in clinical practice, and to assess whether the predicted effectiveness of either drug class is modified by age and the presence of CV risk factors.MethodsRA patients initiating treatment with a JAKi, non-TNFi, or TNFi 2016-2021 were followed using the Swedish Rheumatology Quality Register. Good EULAR response and remission (CDAI<=2.8) at 6 months were assessed at return visits (within 90 to 270 days after treatment initiation). Non-responder imputation was implemented where treatment ended prior to 210 days after initiation.Crude probabilities of outcomes were presented by age and having ≥1 CV risk factor (Y/N). Linear regression estimated the risk difference (versus TNFi) in age and CV risk groups, adjusted for sex, baseline DAS28 (response) and CDAI (remission), and line of therapy. Probability of outcome was predicted from logistic regression models, fitted to allow effect modification of treatment cohort by age and CV risk score (ERS-RA score), adjusted for sex, baseline DAS28 (response) and CDAI (remission), and line of therapy.Results10,309 treatment episodes contributed data on response at 6 months, and 12,016 episodes with data on remission. Overall, 21% achieved good EULAR response and 11% reached CDAI remission (Table 1).Crude observed risks overall indicated superior response and remission with TNFi versus JAKi (Table 1). Adjusted risk differences suggested superior outcomes in JAKi versus TNFi, overall, and in those with no CV risk for CDAI remission. Patterns of predicted response and remission varied across age and CV ERS-RA score (Figure 1), with JAKis having superior effectiveness overall once adjusting for line of therapy.ConclusionAge and CV risk may modify the absolute and relative effectiveness of JAKis vs. TNFi. Wheat is the second most important cereal crop and plays a key role in food and nutritional security. The study examines the impact of elevated carbon dioxide and temperature under limited irrigations on wheat crop using crop growth simulation models under sub temperate climate. The Info Crop model was validated using the historical data generated by the research trials of All India Coordinated Research Improvement Project at Palampur, Himachal Pradesh. The model was run for 20 years from 1991 to 2010 for Palampur weather station. The results revealed 3.6 to 4.0 percent and 1.7 to 7.5 percent increase in simulated crop yield with 420 and 470 ppm CO2 respectively. The elevated maximum and minimum temperature by 1 and 20C with 370 ppm carbon dioxide reduced the anthesis period by 5-7 and 9-11 days respectively over the sowing window of November 15th to December 30th. Similarly, days to maturity also reduced by 4-5 and 7-8 days with 1 and 20C rise in temperature respectively. The simulated crop yield showed increase by 17.9 to 63.0 and 33.2 to 133.4 percent with 1 and 20C rise in temperature at 370 ppm CO2 under limited irrigations. The simulated grain yield at 420 ppm CO2 showed an increase of 23 to 69.7 percent with 10C and 39.5 to 123.5 percent with 20C whereas at 470 ppm CO2 level the increase was 27.9 to 76.1 at 10C and 46.4 to 133.0 percent with 20C rise in maximum and minimum temperatures respectively. Hence, simulated results of elevated temperature and CO2 levels proved to be beneficial in rabi wheat with adaptations strategy of limited irrigations under sub temperate climate of North Western Himalayas

Background: Choriocarcinoma is a malignant trophoblastic tumor that can degrade the uterine basement membrane to facilitate local and distant metastasis. Matrix metalloproteinase-2 (MMP-2) is a vital proteinase produced by trophoblasts and can cleave type IV collagen in the basement membrane of the uterine epithelium. In addition, E-cadherin controls cell adhesion, which is associated with tumorprogression, and loss of E-cadherin function is associated with metastasis. Vitamin A analogues, including all-trans retinoic acid (ATRA), exhibit anticancer properties, including metastasis inhibition.This study aimed to investigate the ability of ATRA to prevent trophoblast invasion and metastasis in choriocarcinoma by assessing MMP-2 and E-cadherin activity. Methods: This study administered various doses of ATRA to the BeWo (American Type Culture Collection CCL-98) choriocarcinoma cell line and assessed MMP-2 and E-cadherin activity through flow cytometry. The research was performed at the Biomedical Laboratory of the Faculty of Medicine of Universitas Brawijaya Malang, East Java, Indonesia. Results: BeWo cells were exposed to 0, 50, 100, 200, 400, or 800 μg/mL ATRA. MMP-2 was downregulated after ATRA treatment. The greatest reduction in MMP-2 level was observed after 200 μg/mL ATRA treatment, but this result was not statistically significant (P = 0.550). However, ATRA treatment significantly reduced the number of BeWo cells with low E-cadherin levels (P = 0.012). Conclusion: ATRA downregulates MMP-2 and upregulates E-cadherin. The immediate, 1 day and 14 days skin reaction was determined in 23 female SLE patients and 23 age-matched controls after standardized exposure to ultraviolet light of the UV-A wavelengths (320-440 nm). Eighteen of the patients and 12 of the controls were photosensitive by history. Eight separate sites on the buttocks were exposed to UV-A light, four sites at doses between 42 and 252 kJ/m2 and four sites with longpass filters (320, 345, 360 and 375 nm). The reactions on test sites were graded by two independent observers unaware of given doses or filter location. All patients and controls reacted with immediate erythema irrespectively of the presence or absence of photosensitivity. After 1 day, 39% of controls and 78% of patients had erythema and the reactions were more pronounced to longwave UV-A light (>320 nm) in the patients (P < 0.001). After 14 days, six patients, but no control, had persistent erythema (P = 0.04). Interestingly, three of the four patients without anamnestic photosensitivity did not react on days 1 or 14, while the pattern seen in the controls on day 1 was totally unpredictable with regard to reported photosensitivity. These findings strongly suggest that a considerable proportion of SLE patients show pathological skin reactions to physiological doses of longwave UV-A and not only the far more studied shortwave UV-B (290-320 nm). The responsible chromatophore for the UV-A reaction is not known.

BackgroundDepression and anxiety cause severe loss of quality of life for patients with systemic lupus erythematosus. The causes and factors that contribute to these psychological manifestations in lupus are difficult to disentangle.ObjectivesThe aim of this study was to determine factors associated with anxiety and depression in Asians systemic lupus erythematosus patients (pts) from Kazakhstan and Kirghizstan.Methods1060 SLE pts who fulfilled SLICC 2012 criteria were enrolled into the study. The SLEDAI 2K index activity, SLICC damage index, depressive and anxiety symptoms were assessed by the Hospital Anxiety and Depression (HAD) scale (0-21 points).Results1060 Lupus pts were studied, respectively. The pts were predominantly female (92%) and of indigenous nationality (Kazakh 10%,/ Kirghiz 54%/Russian 36%)with a mean±SD age 34,6±11,8 years. The mean disease duration (Мe) was 4 [1;9], the disease activity (SLEDAI 2K) 13,23±8,74, SLICC damage index 1,36±0,82. More than half of the pts (619 [58.3%]) had abnormal points by HADS scale. 1060 pts were divided into two groups. The first group included 619 SLE pts with abnormal HADS (HADS-A ≥ 8 points; HADS-D ≥ 8 points; HADS-D ≥ 8 points and HADS-A ≥ 8 points). The second one included 441 SLE pts with normal HADS. The most common were anxiety and depression (HADS-D ≥ 8 points and HADS-T ≥ 8 points) - in 346 (32.6%) pts, the second most common was anxiety disorder (HADS-T ≥ 8 points) - in 230 (22%) pts, the third - depressive disorder (HADS-D ≥ 8 points) - in 43 (4%). Such factors as age of the pts, age of SLE onset, duration of illness, SLICC damage index, SLEDAI 2 К, and dosage of corticosteroids. The pts with abnormal HADS were older (36,04±12,11 versus 29,94±11,49 years; p=0,003), with less disease duration (3,1 [0,9;8] versus 4,7 [1,5; 10]; p=0,01), with disease onset at a more later age (31,55±11,86 versus 26,43±11,39 years; p=0,0002), with higher SLE activity by SLEDAI 2 K (14,78±8,59 versus 12,22±8,30; p=0,01). Background:Psoriatic arthritis (PsA) is characterised by musculoskeletal symptoms, and patients (pts) with PsA usually experience psoriasis concurrently. Real-world data reflecting impact of skin symptoms on PsA disease burden are limited.Objectives:Analyse effectiveness of ustekinumab (UST) and tumour necrosis factor inhibitor (TNFi) therapy on extent of skin involvement, and the impact this has on PsA disease burden and drug persistence.Methods:PsABio (NCT02627768) is a prospective, observational study of 1st/2nd/3rd-line UST or TNFi treatment in PsA in 8 European countries. Extent of skin involvement was categorised as body surface area (BSA): clear/almost clear; <3% but not clear/almost clear; 3–10%; or >10%. Pt-reported disease impact was evaluated by PsAID-12, including assessment of two skin-related domains (D): D3 (skin problems, including itching) and D10 (embarrassment and/or shame because of appearance). Estimated persistence at 1 year was assessed across baseline (BL) BSA categories.Results:At BL, significantly more pts receiving UST than TNFi had BSA >10% (Figure 1). BL disease impact (PsAID-12) was worse in pts with BSA >10% than <3% in D3, D10 and total (non-overlapping 95% CIs suggest significance) (Table 1). BSA improved from BL to 1 year with both treatments. At 1 year, 64% of pts in both groups had clear/almost clear skin and only 3% had BSA >10% (Figure 1). At 1 year, both treatments significantly reduced disease impact (PsAID-12 total), and D3 and D10 scores, irrespective of BL BSA category, but most markedly in pts with higher BL BSA (Table 1). Worse BL psoriasis was generally associated with longer persistence for both treatments; however, at 1 year, pts with BSA >10% had significantly shorter persistence with TNFi (mean [95% CI]: 361 [336; 387] days) than with UST (410 [394; 426] days).Conclusion:In PsA, interleukin-12/23 inhibition (UST) and TNFi therapy in routine care rapidly and substantially reduced extent of skin involvement and related disease impact. Pts with highest BL skin involvement had significantly longer drug persistence with UST than with TNFi. IntroductionBASHH guidance recommends screening for STIs up to every 3 months for individuals at risk of HIV. Conversely, commissioning pressures aim to reduce inappropriate attendances. We describe below the characteristics and outcomes of frequent attenders at our service.MethodsNotes review of individuals with 4 or more new or re-book attendance episodes at a central London sexual health service between 1stApril 2015 and 31stMarch 2016.Results170 individuals received more than 4 new episodes of clinical care in a year; 145(85%) were male, 136 MSM. 21(12%) were female, 4(2.4%) transgender. 23(14%) of the patients were HIV positive, all MSM. Median age was 31 years. Median number of sexual partners in preceding 3 months was 6. 75(44%) disclosed chemsex activity in the preceding month.In the 12 months from April 2015, there were 442 new STIs in this population, an average of 2.6 per patient: 346 STI diagnoses were in the 147 HIV-negative individuals and 96 in the 23 HIV-positive individuals. In HIV-negatives, the diagnosis was a rectal bacterial STI in 36% and syphilis in 7%. 206 courses of PEP were prescribed; 25 individuals received 4 or more PEP courses. There were 5 new diagnoses of blood borne virus infections; 2 hepatitis C, both in HIV positive MSM, and 3 HIV.DiscussionThe majority of frequent attenders at our clinic had indicators of high risk sexual behaviour. The high number of STIs and PEP prescriptions implies that the frequent attendances are appropriate in this patient population.

The effects of exogenous insulin-like growth factor-I (IGF-I) on colonic adaptation were examined in male Sprague-Dawley rats (n = 60, 225-275 mg) after either a 60% small bowel and cecal resection (RX) or mid-small bowel transection with reanastomosis (TX). Animals received a 7-day treatment with either IGF-I (2.4 mg.kg-1.day-1) or vehicle (V; 0.1 mol/l acetic acid). Body weight decreased significantly after resection (-25.6 +/- 4.0 g; P < 0.05 vs. TX/V). IGF-I treatment significantly reduced weight loss after resection (-12.4 +/- 3.8 g; P < 0.01 vs. RX/V) and induced significant weight gain after transection (15.6 +/- 4.0 g; P < 0.05 vs. TX/V). Plasma IGF-I decreased with resection (526 +/- 41 TX/V vs. 344 +/- 17 ng/ml RX/V; P < 0.01). IGF-I treatment significantly increased plasma IGF-I levels (805 +/- 100 ng/ml TX/IGF, 677 +/- 56 ng/ml RX/IGF). After resection, IGF-I treatment significantly increased colonic mucosal weight, DNA, protein content, and crypt depth when compared with resection alone (P < 0.05). Colonic water absorption, measured by an in vivo [3H]polyethylene glycol assay, was significantly increased by IGF-I treatment in resected animals (399 +/- 23 RX/IGF vs. 306 +/- 32 microliter.cm-1.h-1 RX/V; P < 0.05). Resection resulted in increased steady-state colonic IGF-I mRNA (182% of TX/V; P < 0.01) without significantly affecting IGF-I receptor mRNA expression. Regulation of IGF binding protein (BP)-3 and -4 was discoordinate, with IGFBP-3 mRNA tending to decrease with resection (67% of TX/V; P is not significant) and IGFBP-4 increasing significantly (191% of TX/V; P < 0.05). An important role for IGF-I in colonic adaptation after massive intestinal resection is indicated by 1) significantly enhanced colonic mucosal growth and water absorption with IGF-I treatment and 2) postresection upregulation of colonic IGF-I mRNA and alteration of IGFBP-3 and IGFBP-4 mRNA expression.

ABSTRACTThree randomized-block experiments involving 87 British Friesian, 18 Simmental × Friesian and 18 Hereford × Friesian, artificially reared calves (mean initial live weight 47 kg and age 9 days), were carried out to examine the effects of the quantity of milk substitute consumed on the lifetime performance of beef cattle. The three treatments were (1) 400 g milk powder per head daily, weaned at 42 days of age (2) ad libitum intake of milk powder, weaned at 42 days and (3) ad libitum intake of milk powder, weaned at 60 days. All calves were individually penned and fed until after weaning; those on treatment 1 were bucket fed twice daily, while those on treatments 2 and 3 sucked the milk from 25·1 containers through artificial teats. They were offered a pelleted, barley/soya-bean meal concentrate (197 g crude protein per kg dry matter (DM)) ad libitum from purchase until intake reached a maximum of 2·7 kg per head daily, and well preserved grass silage ad libitum from 6 weeks of age. Three commercial, skimmed milk-based, acidified milk substitutes (242 and 187 g crude protein and oil per kg respectively) were used. Intakes of milk substitute, concentrate DM and silage DM per calf to 12 weeks of age for treatments 1 to 3 respectively were as follows: 13·0, 39·7 and 54·8 (s.e. 1·67) kg; 88, 73 and 58 (s.e. 1·5) kg and 9·4, 10·6 and 11·2 (s.e. 0·23) kg. Live-weight gains to 12 weeks of age and carcass weights adjusted to 19 months of age were 0·71, 0·82 and 0·84 (s.e. 0·026) kg/day and 335, 340 and 341 (s.e. 3·8) kg. It is concluded that increasing the input of milk substitute above 400 g per head daily until 6 weeks of age produced only a small and uneconomic increase in the lifetime performance of beef cattle.

Abstract Abstract 5014 Exposure to toxic compounds and pesticides leads to an increased risk to develop Multiple Myeloma (MM). The metabolism and the excretion of xenobiotics are mediated by the enzymes and transporters acting in the detoxifying/elimination process. The nuclear receptors NR1I2 (or PXR) and NR1I3 (or CAR) act as xenosensor activating the detoxifying/elimination process in response to the intracellular levels of xenobiotics. It has been hypothesized that part of the individual variability in drug metabolism efficiency could be due to the genetic variations within these regulator genes affecting their expression and/or function. To investigate the impact of genetic variation within these genes on MM susceptibility, we selected and genotyped 10 tag Single Nucleotide Polymorphisms (SNPs) in the PXR gene and 7 tag SNPs in the CAR gene in 627 MM cases (320 males and 307 females) and 883 (459 males and 424 females) controls from different European populations. All the SNPs were in Hardy-Weinberg equilibrium (p>0.001), with the exception of the PXR SNP rs2461818 that was therefore excluded from the analysis. We found no association of any of the genotyped SNPs with MM risk. In the same way, haplotype distribution showed no differences between cases and controls. This was the first comprehensive investigation of genetic variation in xenobiotic regulators genes PXR and CAR in relation to MM risk and our data suggest that common variants in these genes have no impact in modifying MM risk. Evaluation of 12 Swiss chard varieties (Beta vulgaris ssp. cicla L.) in field experiments was focused on the observation of morphological characteristics, yield and nutritional quality of leaves and stalks. The following mean parameters of all varieties were found: 86% field germination, plant weight 346 g, plant height 506 mm, stalk width 23 mm, 10 leaves per plant, and yield 35 t/ha. Mean content of vitamin C was 307 and 72 mg/kg of fresh matter (f.m.) in leaves and stalks, respectively. Potassium content was 4,198 and 4,848 mg, sodium amount was 2,101 and 966 mg, calcium content was 481 and 310 mg and finally magnesium content represented the levels of 361 and 113 mg/kg of f.m., always in leaves and stalks. Significant effects of plant height and plant leaf number on total plant weight were found. Plants with the mean weight of 400 g formed the widest stalks. In addition, a negative effect of higher yield on vitamin C and Ca stalk content was detected. An opposite effect was determined in Na and partially in Mg stalk content that simultaneously increased with higher yield levels. The variety Lucullus is still comparable with newer varieties such as Gator, Z&uuml;rcher Gelber andCharlotte, which achieved good results among all tested varieties.

The importance of the Atlantic walrus ivory trade for the colonization, peak, and collapse of the medieval Norse colonies on Greenland has been extensively debated. Nevertheless, no studies have directly traced medieval European ivory back to distinct Arctic populations of walrus. Analysing the entire mitogenomes of 37 archaeological specimens from Europe, Svalbard, and Greenland, we here discover that Atlantic walrus comprises two monophyletic mitochondrial (MT) clades, which diverged between 23 400 and 251 120 years ago. Our improved genomic resolution allows us to reinterpret the geographical distribution of partial MT data from 306 modern and nineteenth-century specimens, finding that one of these clades was exclusively accessible to Greenlanders. With this discovery, we ascertain the biological origin of 23 archaeological specimens from Europe (most dated between 900 and 1400 CE). These results reveal a significant shift in trade from an early, predominantly eastern source towards a near exclusive representation of Greenland ivory. Our study provides empirical evidence for how this remote Arctic resource was progressively integrated into a medieval pan-European trade network, contributing to both the resilience and vulnerability of Norse Greenland society.

This study investigates the design an electric motor propelling hubless wheel mounted on an electric bicycle (e-bike) by numerical analysis. The motor is part of an in-wheel system that uses a permanent magnet synchronous motor (PMSM) to drive the hubless wheel. This paper presents an optimized PMSM design and compares various torque-related parameters, such as cogging torque, cogging torque total harmonic distortion (THD), and ripple torque. The comparison is based on a motor design that examines the effect of different factors, including the number of windings, slot openings, permanent magnet span, and magnet thickness. The motor was modeled and analyzed with variations in the number of windings (21, 23, 25), slot openings (2.4 mm, 3.0 mm, 3.6 mm), permanent magnet span (23 mm, 25 mm, 27 mm), and permanent magnet thickness (3.5 mm, 4.0 mm, 4.5 mm). Our findings suggest that increasing the permanent magnet’s thickness leads to a higher magnetic flux density. Additionally, when the permanent magnet span exceeds 25 mm, there is a sharp increase in cogging torque THD. Based on these results, a slot opening of 3.0 mm and the use of 25 windings are considered suitable for a hubless e-bike.

Translation of Buddhist texts from Sanskrit into Chinese was of great importance for spreading and emergence of Buddhism in China. And yet the history of these texts translation has not yet received a sufficiently complete and comprehensive coverage in the Russian literature. The relevance of studying this topic is determined by importance of studying the centuries-long process of translating Buddhist literature into Chinese for understanding the history of Buddhism in China. It was one of the most important factors in transformation and Sinification of Buddhism in the country. This article is devoted to the least known period in the history of Buddhist texts translation, i.e. the period of Nanbeichao (Southern and Northern Kingdoms, 420–589). This period is of particular importance for the history of Buddhism in China, since it was then that social, cultural, and religious fodder had developed for emergence of schools of Chinese Buddhism, which marked the definitive formation of Buddhism in China. The article is to study the translation activities of Buddhist missionaries in the Southern and Northern Kingdoms. Therefore, it sets several tasks: to analyze the process of translating Buddhist literature in both kingdoms; to determine the continuity and differences between translations of the specified period and those of the previous eras; and to assess their historical significance. The solution to these problems rests on the method of systematic and comparative analysis. The article shows that, despite the difference in political, economic, and social situations in both kingdoms, translation activities did not differ much. They followed translation traditions of the Eastern Jin period (316–420). Much attention was paid to translation of the Yogachara school literature, reflecting the development of Buddhism in India. Attention was also paid to translation of the Mahayana texts, which played a huge role in the formation of philosophical and soteriological principles of Chinese Buddhism. The research has introduced into scientific use some little-known translations of Buddhist sources and named their translators. It has been revealed that, in the period under review, the greatest importance was attached to repeated translations, which indicated the further improvement of translation technique. It has been determined that re-translations played an important role in the shaping of doctrines of the schools of Chinese Buddhism. The study concludes that the history of translation of Buddhist texts in the Nanbeichao period is a valuable source for studying not only history of Buddhism of the period, but also social and political history of medieval China. It can clarify many issues related to adaptation of the Indian Buddhism values to the Chinese cultural environment.

Three-dimensional (3D) printing is a powerful tool that enables the printing of almost unlimited geometry in a few hours, from a virtual design to a real structure. In this paper, we present a micro-electromechanical energy harvester that utilized a 3D printed micromechanical structure combined with a miniature permanent magnet and a microelectronic coil towards a hybrid electromagnetic vibrational hybrid energy harvester. Various micromechanical structure geometries were designed, printed, and tested. The characteristic dimensions of the springs were from 200 μm to 400 μm and the total volume of the devices was below 1 cm3. The resonant frequencies (95–340 Hz range), as well as bandwidths (6–23 Hz range), for the developed prototypes were determined. The maximal generated output power was almost 24 μW with a power density up to almost 600 μW/cm3.

The Allergen Bank was established to give dermatologists easy access to special test materials in order to make early diagnoses of special cases of allergic contact dermatitis. The Allergen Bank comprises a computer system to register several hundred contact allergens in appropriate patch test concentrations available at the allergy laboratory and the patch test results. At the request of dermatologists in practice for Allergen Bank may supply special contact allergens for aimed patch testing of contact dermatitis patients. The organization of the Allergen Bank and the procedure of its use are described. During its first 23 months 28 dermatologists asked for 2,209 allergen samples for testing of 386 patients, an average of 6 allergens per patient and 14 patients per dermatologist. A total number of 164 positive reactions have been registered, and 440 of the 540 allergens have been in use. One third of the positive reactions were caused by the 16 most frequently ordered allergens, which amounted to 340 allergen samples. The allergens included plant chemicals, acrylates, animal feed additives, fragrance chemicals and preservatives. Selected allergens were investigated for stability during handling and shipping under varying conditions relevant to the function of the Allergen Bank. The possible inhomogeneity of petrolatum based allergen preparations is discussed in relation to diagnostic patch testing.

Para isso, foi conduzido um experimento no Campus de Engenharia e Ciências Agrárias da Universidade Federal de Alagoas. O delineamento foi em blocos casualizados, com quatro tratamentos e cinco repetições (E1= 28/05/14, E2-11/06/14, E3-25/06/14 e E4- 23/07/14). A primeira época de plantio foi a que apresentou maior altura do dossel (2,25 m), índice de área foliar (4,0 m2 m-2) e rendimento agrícola (8,03 t ha-1). A E4 apresentou a menor altura do dossel (0,9 m) e a menor produtividade agrícola foi observada na E3 (5,9 t ha-1). Isso aconteceu porque com base no balanço hídrico da cultura, quando o plantio é realizado até o primeiro decêndio de junho há uma melhor distribuição da chuva o que maximiza o potencial da cultura. For this, an experiment was conducted at the Campus of Engineering and Agricultural Sciences of the Federal University of Alagoas. The statistical design was randomized blocks, with four treatments and five replications (E1 = 28/05/14, E2-11 / 06/14, E3-25 / 06/14, and E4- 23/07/14). The first planting season was the one with the highest canopy height (2.25 m), leaf area index (4.0 m2 m-2) and agricultural yield (8.03 t ha-1). The E4 had the lowest canopy height (0.9 m) and the lowest agricultural yield was observed in the E3 (5.9 t ha-1). This happened because based on the water balance of the crop, when planting is carried out until the first ten days of June, there is a better distribution of rain, which maximizes the crop's potential. Abstract Abstract 4001 Background: Low serum albumin level is known to be an adverse prognostic factor in patients with malignancies such as multiple myeloma. We previously reported that severe hypoalbuminemia (&lt;3.0 g/dl) at day +90 post allogeneic hematopoietic stem cell transplant (AHCT) was an independent predictor of non-relapse and overall mortality in patients with acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS) (Kharfan-Dabaja et al Biol Blood Marrow Transplant. 2010 Jul). In this study we examined prognostic value of serum albumin level in patients with MDS. Methods: Data were analyzed from the Moffitt Cancer Center (MCC) MDS database with chart review verification. The primary objective was to examine the role of serum albumin at time of presentation to MCC as a prognostic marker for overall survival (OS). Patients were divided into 3 groups of serum albumin levels (≤ 3.5, 3.6–4.0 and &gt; 4.0 g/dl). The Kaplan–Meier method was used to estimate median OS. The log rank test was used to compare Kaplan–Meier survival estimates between two groups. Cox proportional hazards regression was used for multivariable analysis. Results: Between January 2001 and December 2009, 844 patients were captured by the MCC MDS database. The median age was 69 years. MDS subtypes were coded as refractory anemia (RA) (n=98;12%), refractory anemia with ring sideroblasts (RARS) (n=76;9%), del(5q) (n=20;2.4%), refractory cytopenia with multi-lineage dysplasia (RCMD) (n=96;11%), refractory anemia with excess blasts (RAEB) (n=255;30%), therapy related MDS (n=22;2.6%), and MDS-NOS (n=275; 33%). The distribution of IPSS risk groups was: 18.7% Low risk, 42.9% Intermediate-1 (Int-1), 19.9% Int-2, 5.3% High risk, and 13.2% unknown. Baseline characteristics for the three patient groups defined by serum albumin level are summarized in (Table-1). There was no difference in red blood cell transfusion dependency (RBC-TD) rate between the 3 groups (p=0.21). The median OS for all patients was 36 months (95% confidence interval (CI) 31.5–40.5 mo). Age, IPSS risk group, RBC-TD, Serum ferritin were statistically significant prognostic factors in univariable analysis. The median OS was 19 mo (95%CI= 14.9–23.1 mo), 35 mo (95%CI= 28.7–41.3 mo), and 53 mo (95%CI= 44.7–61.3 mo) for patients with serum albumin levels ≤ 3.5 g/dl, 3.6–4.0 g/dl, &gt; 4.0 g/dl, respectively. (Figure-1) (p= &lt;0.005). After adjustment for age, RBC-TD, OS was statistically significantly inferior among MDS patients with lower serum albumin (Hazard Ratio (HR) = 0.79.; 95%CI= 0.69–0.90; p= 0.001), and higher-risk IPSS group (HR=1.67; 95%CI=1.48-1.87; p= &lt;0.005). The overall rate of AML transformation was 29.2%. Rate of AML transformation was higher in patients with lower serum albumin, 38% in patients with serum albumin ≤ 3.5 g/dl, 30% for patients 3.6–4.0 g/dl, and 23% in patients with serum albumin &gt; 4.0 g/dl (p-value 0.005). Among patients in the Low/Int-1 IPSS risk group, the median OS was 28 mo (95%CI=15.7-40.3 mo), 48 mo (95%CI=38.8-58.0 mo), and 60 mo (95%CI=47.6-72.4 mo) for patients with serum albumin levels ≤ 3.5 g/dl, 3.6–4.0 g/dl and &gt; 4.0 g/dl, respectively (p=0.003). Among patients in the Int-2/High IPSS risk group, the median OS was 16 mo (95%CI 13.3–15.7 mo), 22 mo (95%CI 18.0–26.0 mo), and 21 mo (95%CI 8.8–33.2 mo) respectively for patients with serum albumin levels ≤ 3.5 g/dl, 3.6–4.0 g/dl and &gt; 4.0 g/dl, respectively p=0.03). Conclusion: In this retrospective analysis of a large single institution MDS database, serum albumin is found to be an independent prognostic factor for OS and AML transformation in MDS patients. The prognostic power of low serum albumin was greatest among patients with Low/Int-1 IPSS risk group, but remained an independent variable across all risk groups. Serum albumin may also be a surrogate marker of general health, co- morbidities, and performance status. Disclosures: No relevant conflicts of interest to declare.

<p><strong>Background and Objective</strong>: The Coronavirus disease 2019 (COVID-19) pandemic has affected more than four million people in 213 countries. Health care workers (HCWs) who deal with COVID-19 patients have a high chance of getting infected. They have got valid concerns regarding their own health as well as the impact it can have on the wellbeing of their family and acquaintances. The current study aims to assess the prevalence of psychological problems among HCWs during the COVID-19 pandemic. <strong>Methods:</strong> A cross-sectional study was conducted among HCWs of Wah Medical College and Pakistan Ordnance Factories (POF) Hospital Wah Cantt from 1st May 2020 to 7th May 2020 after an ethical approval by ethical review board. A total of 340 HCWs participated by filling an online questionairre; questions on demographic profile and validated Depression, Anxiety and Stress Scale&ndash;21 (DASS-21) score were included. Frequency of depression, anxiety and stress and mean DASS-21 scores between house officers and all other healthcare workers and between Medicine and allied department and other departments was assessed. Data was analysed using Statistical Package for the Social Sciences (SPSS)-23. P-value &le; 0.05 was considered statistically significant. <strong>Results:</strong> Among 340 HCWs majority i.e. 66.8% were less than 35 years of age and the mean age of participants was 40.65 &plusmn; 11.64 years. There was a female proponderance with a F:M ratio of 1.7:1. Most (55.3%) of the participants were single and there was a high percentage of house officers with 45% among all. The overall prevalence of depressiom, anxiety and stress among all HCWs was 32.9%, 25% and 23.5% respectively. The depression, anxiety and stress among house officers than other HCWs and higher prevalence in Medicine and Allied Departments than other departments. <strong>Conclusion:</strong> As the global threat of COVID-19 continues to emerge, it is important not to underestimate the psychological impact of this pandemic on HCWs. Interventions and support to especially vulnerable groups of HCWs will help improve psychological health, which in turn will help improve the quality of patient care.</p>

This work describes the three-dimensional survey of “Ex Stazione Frigorifera Specializzata”: initially used for agricultural storage, during the years it was allocated to different uses until the complete neglect. The historical relevance and the architectural heritage that this building represents has brought the start of a recent renovation project and functional restoration. <br><br> In this regard it was necessary a global 3-D survey that was based on the application and integration of different geomatic methodologies (mainly terrestrial laser scanner, classical topography, and GNSS). <br><br> The acquisitions of point clouds was performed using different laser scanners: with time of flight (TOF) and phase shift technologies for the distance measurements. The topographic reference network, needed for scans alignment in the same system, was measured with a total station. For the complete survey of the building, 122 scans were acquired and 346 targets were measured from 79 vertices of the reference network. Moreover, 3 vertices were measured with GNSS methodology in order to georeference the network. For the detail survey of machine room were executed 14 scans with 23 targets. <br><br> The 3-D global model of the building have less than one centimeter of error in the alignment (for the machine room the error in alignment is not greater than 6 mm) and was used to extract products such as longitudinal and transversal sections, plans, architectural perspectives, virtual scans. <br><br> A complete spatial knowledge of the building is obtained from the processed data, providing basic information for restoration project, structural analysis, industrial and architectural heritage valorization.

341 Background: Sorafenib (sorf.) has become the standard treatment for Hepatocellular Carcinoma (HCC), demonstrating an increase in patients (pts) median survival from 7.9 to 10.7 months (m) (Llovett, NEJM 2008). Abou-Alfa added doxorubicin to sorf. observing a median survival of 13.7 m vs 6.7 m. for doxorubicin + placebo, (JAMA. 2010). Since most of our HCC pts were cirrhotic and 8/12 HCV +, and 2/12 HBV only +, with cirrhosis and thrombopenia we chose to add capecitabine (cap) to sorf in a phase II trial, anticipating better tolerance. Primary objectives were to assess safety and tolerability of the combination while secondary objectives included estimations of PFS, OST and documentation of disease control rate (DCR = CR+PR+SD). With relaxation of eligibility criteria C-P B-7 pts and ≥ 50,000 platelets were allowed. Methods: Sorf dose was 400 mg BID with dose adjustments for sorf related hand foot syndrome (HFS). Starting cap dose was 850mg/m2 BID x 7 Q 14 and adjusted as tolerated to 650mg/m2 BID or to 500 mg/m2 BID x 7 Q 14.. The lowest dose used was 250 mg/m2; BID (500 mg BID) for 5 days Q14. Patients: Only 12 of about 30 assessed patients met eligibility criteria, and were recruited between Nov. 2, 2009 and Sep. 13, 2011.The protocol was reviewed and approved by the WIRB and all participants signed an IRB approved consent form. Results: Preliminary Results: Eight of 12 patients are alive, survivals ranging from 2.5 to 23 m. One patient has an unmaintained radiological CR continuing for 23 months with an AFP decrease from 139 to 2.9. Another patient had a PR and an AFP decrease from 355 to 3.2 ng/mL. She died in PR at 13 months from an UGI bleed. Grade 3 and 4 toxicities were limited to the skin, and included HFS, in 4/12, skin ulcers at sites of prior osteomyelitis (1/12); requiring treatment cessation, and fatigue in 4 /12. Conclusions: With meticulous attention to cap dose adjustments as dictated by HFS this combination can be tolerated longitudinally in these frail pts that are not candidates for adding more aggressive agents to sorf. The objective radiological responses corroborated by AFP decreases in this very small cohort suggest that the findings are real, and warrant expansion of the trial.

Abstract Abstract 228 Haploidentical hematopoietic stem cell transplantation (haplo-HSCT) is an important alternative option for patients with hematological malignancies who need urgent transplant but without HLA matched either sibling or unrelated donor (Dao-Pei Lu et al., Blood 2006; 107:3065). Each patient usually has several haploidentical family members who could be selected as a donor. To determine the principal of donor selection among all available related haploidentical candidates, the clinical outcomes of a large series of haplo-HSCT in our hospital are analyzed. From April 2002 to April 2010, consecutive 440 patients with hematological malignancies who underwent haplo-HSCT were included. The median age of patients was 23 (3-59) years old. The diagnosis included AML (39.8%), ALL (35.9%), MDS (3.6%), CML (16.1%), and others (4.6%). Transplants at CR1 or CML-CP1, ≥CR2 or CML-CP2/AP, and advanced disease (refractory/relapsed acute leukemia or CML-BC) were 33.4%, 40.9% and 25.7%. HLA mismatched at 1, 2, 3 loci was 13.2 %, 27.5%, 59.3%, respectively. Transplants in sex-matched donor-recipient pair, female donor to male recipient, and male donor to female recipient were 55.1%, 33.0% and 13.9%. Major clinical characteristics among these three arms were similar. All patients received unmanipulated combined marrow and peripheral blood stem cells for transplant after BUCy2/CyTBI plus ATG conditioning. Fludarabine was substituted for cyclophosphamide in some patients due to impaired organ function. Prophylaxis and treatment of GVHD were reported previously. Steady hematopoietic reconstitution was seen in 98.6% of recipients. The cumulative incidences of grade II to IV acute GVHD and chronic GVHD were 32.6%, 61.3%, respectively. 100-day mortality was 10.5%. With the median follow-up of 32 (3-99) months, 2-year and 5-year overall survival (OS) rates for patients who were in different disease status were 57.9% and 52.9%. Univariate and multivariate analysis all showed that disease status before transplant, CD34+ cell dosage infused and sex-matched or not between donor and recipient but not HLA disparity and age were pivotal impact factors on survival. Two-year OS of transplants in sex-matched donor-recipient pair, female donor to male recipient, male donor to female recipient were 65.5%, 55.3%, 37.6, respectively (p=0.000). No significant differences were found on OS of transplants among haploidentical donors from sibling or parent or offspring or other relatives. In conclusion, our clinical results from a large series of transplants demonstrate that haplo-HSCT in sex-matched donor-recipient pair has survival advantage. Therefore, in haplo-HSCT, sex-matched donor should be the first choice, if not available, then female donor to male recipient could be the second option. 2515 Background: LY573636 -sodium (hereafter referred to as LY573636 ) is a novel anti-cancer compound that induces apoptosis by a mitochondrial-mediated mechanism. Methods: A phase I study was conducted to determine the maximum tolerated dose (MTD) and pharmacokinetic (PK) profile of LY573636 administered as an intravenous infusion over 2 hours every 21 days. Results: 53 patients (pts) were enrolled on study, 16 males, 37 females, with a median age of 58 (range: 18–83). LY573636 was dose-escalated using a flat dosing paradigm in the first 34 patients (range 100 mg to 2400 mg). PK analysis of this cohort revealed that LY573636 had a low total plasma clearance (CL = 0.02L/hr) and terminal elimination half-life of approximately 340 hours due to high-albumin binding (∼99%). The dose limiting toxicity (DLT) for the flat dose escalation was bone marrow (BM) suppression, which included CTC (ver2) grade 3/4 thrombocytopenia/neutropenia. In some patients, significant BM suppression occurred in cycle 2 and later, attributed to the accumulation of albumin-bound drug with repeated flat dosing. Based on these findings, the dose-finding strategy was modified to an idealized dosing paradigm which calculated LY573636 doses based on pt lean body weight, and used a loading/maintenance dose regimen to achieve a specific Cmax target value. All 3 pts in the first 400 μg/mL Cmax cohort completed at least 2 cycles and no DLTs were reported. Of the 16 pts enrolled to the 420 μg/mL targeted dose group: 1 pt reported a DLT, a transient grade 3 elevation of ALT/AST which did not recur in subsequent cycles; 8/16 pts received 4 or more cycles prior to disease progression and 2 remain on treatment. Overall, 23/53 pts had stable disease (SD) after 2 or more cycles. Seven pts received >8 cycles (SD > 6 mos), including heavily pre-treated pts with ovarian cancer, non-small cell lung cancer, soft tissue sarcoma, and thymoma. Conclusions: LY573636 is a novel anti-cancer compound with a unique mechanism of action. The most common DLT was BM suppression. The individualized, target Cmax dose of 420 μg/mL has been taken forward into phase II clinical trials. No significant financial relationships to disclose.

Background:Australian/Canadian Hand OA Index (AUSCAN), Michigan Hand Outcomes Questionnaire (MHQ), Functional Index of Hand OA (FIHOA) and visual analogue scale (VAS) are frequently used instruments to measure pain and function in hand OA research. MCII and PASS are useful to interpret results of patient reported outcomes.Objectives:To estimate MCII and PASS for these instruments using anchor-based methods.Methods:Hand OA patients participating in a six-week randomised placebo-controlled trial with prednisolone (RCT; NTR5263) and those attending the two-year follow-up visit of the observational Hand OSTeoArthritiS cohort completed AUSCAN subscales pain and function, MHQ subscales pain, activities of daily living (ADL) and overall function, FIHOA and 100mm VAS pain. RCT participants were asked to indicate whether they changed compared to baseline (improved/no change/worse) and to rate the importance of improvement (very much/moderately/slightly/not at all). MCII was defined as the minimal improvement in symptoms achieved by 75% of participants who stated a slight/moderate improvement during the RCT, calculated as the 75th percentile of the distribution of change scores from baseline in this group. Absolute and relative percentage change were evaluated. For MCII direction of effect was unified, so positive values indicate worse symptoms and vice versa. Participants from both studies rated satisfaction with their state of health (acceptable/unacceptable). PASS was defined as the minimal score considered acceptable for 75% of participants, calculated as the 75th percentile of the distribution of scores in participants who rated their health ‘acceptable’.Results:Demographics of the RCT (n=92, mean age 63.9, 79% women) and cohort (n=383, 60.9 years, 84% women) participants were typical for hand OA. RCT participants were more symptomatic (e.g. mean [SD] VAS pain 54.0 [20.5] versus 35.2 [19.1]). Of the function instruments, only AUSCAN had a credible MCII (relative percentage improvement 9.8%), while the (positive) MCII values for FIHOA and MHQ subscales would indicate that worsening was rated as functional improvement (table 1). MCII was negative (corresponding to improvement) for all pain instruments, with relative percentage change around 25% for VAS and MHQ, compared to only 2% for AUSCAN. PASS values of all instruments were comparable in the two populations. Most instruments had a PASS around 50% of the possible maximum score, except for MHQ ADL, in which higher is better and a relatively high PASS is thus indicative of a floor effect (table 1).Conclusion:The only function instrument with an acceptable threshold for MCII was AUSCAN function, while for pain MHQ and VAS performed better than AUSCAN. В Кировской области скотоводство является ведущей отраслью животноводства. Выявлена положительная динамика продуктивных качеств коров с увеличением возраста их первого отела от 26 до 28 мес. Большое значение для определения уровня молочной продуктивности имеет возраст коровы к первому отелу. Эта проблема приобретает особую актуальность при совершенствовании черно-пестрой породы голштинским скотом, который отличается высокой скороспелостью. Наибольший удой за 305 дней первой лактации установлен у коров-первотелок с живой массой 431 – 500 кг, что превысило на 773 кг (12,8 %, P > 0,95) коров с живой массой 340 – 380 кг, и у коров первотелок, осемененных в возрасте 18 – 22 мес., что превысило на 537 кг (9,7 %, P > 0,95) коров, осемененных в возрасте до 15 мес. Подобные результаты были получены по второй и третьей лактациям. Достоверная разница (P > 0,95) составила по 2-й лактации 713 кг (10,4 %), по 3-й лактации — 752 кг (9,8 %). Сделан вывод, что молочная продуктивность голштинизированных коров черно-пестрой породы зависит от возраста первого плодотворного осеменения и от живой массы. Следует проводить первое плодотворное осеменение коров черно-пестрой породы с прилитием крови быков голштинской породы в возрасте 16 – 17 мес. с живой массой 380 – 400 кг.

ABSTRACT We present a pipeline based on a random forest classifier for the identification of high column density clouds of neutral hydrogen (i.e. the Lyman limit systems, LLSs) in absorption within large spectroscopic surveys of z ≳ 3 quasars. We test the performance of this method on mock quasar spectra that reproduce the expected data quality of the Dark Energy Spectroscopic Instrument and the WHT (William Herschel Telescope) Enhanced Area Velocity Explorer surveys, finding ${\gtrsim}90{{\ \rm per\ cent}}$ completeness and purity for $N_{\rm H\,\rm{\small I}} \gtrsim 10^{17.2}~\rm cm^{-2}$ LLSs against quasars of g &lt; 23 mag at z ≈ 3.5–3.7. After training and applying our method on 10 000 quasar spectra at z ≈ 3.5–4.0 from the Sloan Digital Sky Survey (Data Release 16), we identify ≈6600 LLSs with $N_{\rm H\,\rm{\small I}} \gtrsim 10^{17.5}~\rm cm^{-2}$ between z ≈ 3.1 and 4.0 with a completeness and purity of ${\gtrsim}90{{\ \rm per\ cent}}$ for the classification of LLSs. Using this sample, we measure a number of LLSs per unit redshift of ℓ(z) = 2.32 ± 0.08 at z = [3.3, 3.6]. We also present results on the performance of random forest for the measurement of the LLS redshifts and H i column densities, and for the identification of broad absorption line quasars.

Abstract Background Epstein-Barr virus (EBV)-associated nasopharyngeal carcinoma (NPC) in endemic regions may have undetectable plasma EBV DNA. Methods We prospectively recruited 518 patients with non-metastatic NPC and measured their pre-treatment plasma EBV DNA. The stage distribution and prognosis between pre-treatment plasma EBV DNA-negative (0–20 copies/ml) and EBV DNA-positive (>20 copies/ml) patients following radical treatment were compared. Results Seventy-eight patients (15.1%) were plasma EBV DNA-negative, and 62 in this subset (12.0%) had 0 copy/ml. Only 23/78 (29.5%) plasma EBV DNA-negative patients with advanced NPC (stage III-IVA) had strong EBV encoded RNA (EBER) positivity (score 3) in their tumours compared to 342/440 (77.7%) EBV DNA-positive patients of the same stages (p < 0.001). Though EBV DNA-negative patients had more early-stage disease (p < 0.001) and smaller volumes of the primary tumour and the positive neck nodes (p < 0.001), they had similar 5-year overall survival and cancer-specific survival to those EBV DNA-positive counterparts by stage. Similar results were also seen when plasma EBV DNA cut-off was set at 0 copy/ml. Conclusions Patients with low-volume NPC may not be identified by plasma/serum tumour markers and caution should be taken in its utility as a screening tool for NPC even in endemic regions. Abstract Introduction Varied presentations of vitamin B12 (B12) deficiency include hematologic (megaloblastic anemia) and neurologic (myelopathy, peripheral neuropathy). Delayed recognition and treatment may have serious consequences. Assessing B12 status can be challenging due to diverse laboratory assays and thresholds, resulting in a wide range of reported prevalence of B12 deficiency from 3-26%. Comorbidities may also spuriously elevate or suppress B12 levels. Methylmalonic acid (MMA) level, often used as a companion test to assess for B12 deficiency, may also be affected by differing laboratory thresholds and other medical comorbidities, such as renal dysfunction. Our institutional approach to B12 assessment, termed the pernicious anemia (PA) cascade, consists of reflexive testing to 1) document low B12 stores (B12 levels with or without MMA), and 2) diagnose pernicious anemia (intrinsic factor binding antibody [IFBA] with or without gastrin assay). In this study, our objectives were: (1) assess the usefulness of our B12 testing algorithm and associated laboratory thresholds for identifying B12 deficiency, and (2) evaluate clinical follow-up of test results as part of a quality project. Methods In this retrospective study, adult (age ≥18 years) inpatients who had B12 testing from 2007 to 2013 were identified. Medical records for those patients with B12 levels 150-400 ng/L were reviewed. Data abstracted included patient demographics, indication for B12 testing, B12 levels, results for MMA, IFBA, gastrin, serum creatinine, complete blood count, and clinical outpatient follow-up. The PA cascade algorithm is as follows: 1) for B12 <150 ng/L, IFBA is performed; 2) if IFBA is negative or indeterminate, then gastrin is performed; 3) for B12 150-400 ng/L, MMA is performed; 4) if MMA is >0.40 nmol/mL, then IFBA is performed. Reference range for B12 is 180-914 ng/L. Patients were determined to be B12 deficient if B12 level was <180 ng/L, or if B12 was 150-400 ng/L with elevated MMA and macrocytosis in the absence of significant renal dysfunction, defined as serum creatinine ≥2.0 mg/dL. Results Records of 326 patients (150 female) were reviewed. Indications for B12 testing were: hematologic, 175 patients (53.7%); neurologic, 127 patients (39.0 %), psychiatric, 18 patients (5.5%); other, 4 patients (1.2%). Fifty-nine of 326 (18.1%) patients were B12 deficient, of whom 17 of 59 (28.8%) had PA. There were 54 cases of elevated MMA, >0.40 nmol/mL, of which 30 cases had mildly elevated MMA, >0.40 nmol/mL and ≤0.60 nmol/mL, considered to be the MMA "gray zone". Thirty-two of the total 326 patients had serum creatinine ≥2.0 mg/dL, of which 23 patients also had elevated MMA. Elevated MMA in 13 of these 23 patients was more likely due to renal dysfunction than to B12 deficiency. B12 testing results were not mentioned in hospital discharge summaries for 173 patients, including 8 patients with B12 deficiency. For 22 B12-deficient patients, there was no documentation of B12 replacement. Conclusions The PA cascade detected B12 deficiency in a significant number of inpatients with low-normal B12 levels that might have otherwise been missed; 28.8% of this group had PA for whom long-term B12 replacement is indicated. As treatment for B12 deficiency is inexpensive, non-toxic, and non-invasive, it is reasonable to evaluate B12 status by cascade testing which may provide additional diagnostic data beyond B12 level. Notable clinical practice findings from this study include the lack of B12 results in 173 of 326 discharge summaries, and no record of B12 supplementation in 22 of 59 patients with B12 deficiency. These gaps in documentation and follow-up may be associated with the relatively long turnaround time of MMA testing. Studi pendahuluan di Rumah Sakit Dr. R. Ismoyo Kota Kendari diperoleh data pada tahun 2017 dari 796 ibu bersalin, yang bersalin dengan tindakan sectio caesarea berjumlah 342 orang (42,96%). Tahun 2018 dari 779 ibu bersalin, yang bersalin dengan tindakan sectio caesarea berjumlah 307 orang (39,40%). Tahun 2019 (Januari – Juni) dari 404 ibu bersalin, yang bersalin dengan tindakan sectio caesarea berjumlah 128 orang (31,68%). Untuk mengetahui karakteristik ibu dengan persalinan sectio caesarea di Rumah Sakit Dr. R. Ismoyo Kota Kendari tahun 2019. maka di gunakan metode penelitian Jenis penelitian deskriptif dengan populasi adalah semua ibu bersalin dengan tindakan sectio caesarea yang berjumlah 128 orang pada bulan Januari-Juni tahun 2019. Sampel sebanyak 57 orang dengan teknik pengambilan sampel menggunakan simple random sampling. Kategori umur terdapat 9 orang (15,8%) yang risiko tinggi dan 48 orang (84,2%) umur risiko rendah. Kategori paritas terdapat 31 orang (54,4%) yang risiko tinggi dan 26 orang (45,6%) umur risiko rendah. Kategori KPD terdapat 34 orang (59,6%) yang KPD dan 23 orang (40,4%) yang tidak KPD. Kategori makrosomia terdapat 30 orang (52,6%) yang makrosomia dan 27 orang (47,4%) yang tidak makrosomia.
