Resumen
Abstract The main theoretical contribution of the paper is the description of two classes of multigraded ideals named after Cartwright and Sturmfels and the study of their surprising properties. Among other things we prove that these classes of ideals have very special multigraded generic initial ideals and are closed under several operations including arbitrary multigraded hyperplane sections. As a main application we describe the universal Gröbner basis of the ideal of maximal minors and the ideal of 2-minors of a multigraded matrix of linear forms generalizing earlier results of various authors including Bernstein, Sturmfels, Zelevinsky, and Boocher.

Resumen
LetX=(Xij)be anm(1)bym(2)matrix whose entriesXij,1≤i≤m(1),1≤j≤m(2); are indeterminates over a fieldK. LetK[X]be the polynomial ring in thesem(1)m(2)variables overK. A part of the second fundamental theorem of Invariant Theory says that the idealI[p+1]inK[X], generated by(p+1)by(p+1)minors ofXis prime. More generally in [1], Abhyankar defines an idealI[p+a]inK[X], generated by different size minors ofXand not only proves its primeness but also calculates the Hilbert function as well as the Hilbert polynomial of this ideal. The said Hilbert polynomial is completely determined by certain integer valued functionsFD(m,p,a). In this paper we prove some important properties of these integer valued functions.

Resumen
AbstractThis paper takes a new look at ideals generated by 2×2 minors of 2×3 matrices whose entries are powers of three elements not necessarily forming a regular sequence. A special case of this is the ideals determining monomial curves in three-dimensional space, which were studied by Herzog. In the broader context studied here, these ideals are identified as Northcott ideals in the sense of Vasconcelos, and so their liaison properties are displayed. It is shown that they are set-theoretically complete intersections, revisiting the work of Bresinsky and of Valla. Even when the three elements are taken to be variables in a polynomial ring in three variables over a field, this point of view gives a larger class of ideals than just the defining ideals of monomial curves. We then characterize when the ideals in this larger class are prime, we show that they are usually radical and, using the theory of multiplicities, we give upper bounds on the number of their minimal prime ideals, one of these primes being a uniquely determined prime ideal of definition of a monomial curve. Finally, we provide examples of characteristic-dependent minimal prime and primary structures for these ideals.

We show that the defining ideal of every monomial curve in the affine or projective three-dimensional space can be set-theoretically defined by three binomial equations, two of which set-theoretically define a determinantal ideal generated by the 2-minors of a 2 × 3 matrix with monomial entries.

Resumen
Consider the affine space consisting of pairs of matrices ( A , B ) (A,B) of fixed size, and its closed subvariety given by the rank conditions rank ⁡ A ≤ a , rank ⁡ B ≤ b \operatorname {rank} A \leq a, \, \operatorname {rank} B \leq b , and rank ⁡ ( A ⋅ B ) ≤ c \operatorname {rank} (A\cdot B) \leq c , for three non-negative integers a , b , c a,b,c . These varieties are precisely the orbit closures of representations for the equioriented A 3 \mathbb {A}_3 quiver. In this paper we construct the (equivariant) minimal free resolutions of the defining ideals of such varieties. We show how this problem is equivalent to determining the cohomology groups of the tensor product of two Schur functors of tautological bundles on a 2-step flag variety. We provide several techniques for the determination of these groups, which is of independent interest.

Resumen
Bangladesh is a Muslim-majority society with more than 163 million people. Most Bangladeshis hold the ideals of Islamic norms and values which is manifest in all sorts of socio-cultural behaviour. In reference to such values, the tradition of legitimizing child marriage in Bangladesh is the issue that needs to be addressed in a holistic yet rigorous approach. Currently Bangladesh ranks 4th in the world and 1st in Asia in terms of child marriage. Recently the Child Marriage Restraint Act 1929 has been abolished and it has been replaced by the Act of 2017 preserving article 2 of the previous statute, the legal age for marriage for a boy 21 and for a girl 18. This Act adds article 19 which legalizes minors (below 18 years of age) to be married off with the consent of the parents/guardians at the presence of a magistrate under “special circumstances” deemed with securing the best interest for them. The law artfully coincides with the Muslim Marriage Law which allows participants of 15 years and above to get legally married and as such contradicts the international law and the Act of 2017 itself. In the West intimate relationships including extra-marital cohabitation before reaching 18 years of age are culturally accepted. In contrast, such extra-marital and intimate relationships are strictly prohibited in Muslim-majority societies, which are dearly adhered in Bangladeshi Muslim culture. This study examines how the religious cultural and socio-economic realities influence child marriage practice in Bangladesh. Along with secondary documents, we interviewed 22 individuals including the Deputy Commissioner, the District Women and Children Affairs Officer, elected Union Parishad Chairman and Members, Social Workers, married couples and their parents/guardians at Manikganj district. In addition, we also conducted a mass survey with 62 randomly selected participants, and a voluntary online survey where the opinion of another 53 young students were collected to find broad opinion. We also collected stories of how marriages take place at the rural, urban and sub-urban areas in Bangladesh. The study has revealed that Bangladeshis does not support marriage at early ages but socio-economic reality often pushes poor into getting their children married at early ages. Many view that the special provision may encourage child marriage in the country. This study suggests that the government of Bangladesh should redefine public policy in regard to finding a middle ground between Islamic ethics and international values by exploring isomorphic mimicry and other socio-culturally accepted measures with a view to abolishing child marriage successfully.

Resumen
Every child has the right to survival in terms of the nation and state, children are the future of the nation and the next generation of the ideals of the nation so that they have the right to survive, develop, grow, participate and are entitled to protection from acts of violence and discrimination as well as civil rights and freedoms. The high rate of exploitation of minors in Pekanbaru is one of the things that makes us pay attention to the splendor of the development side of the city of Pekanbaru which leads to civility. The black side of exploitation of underage children makes as citizens we are obliged to socialize the existence of the Child Protection Act, especially on the threat of criminal acts of employing children. We can also socialize human rights to parents. Early prevention at the family level can reduce the effects of children becoming victims of exploitation. There is also a formulation of the problem raised in this research which is "What is the background of the formation of the exploitation of basic age children in Pekanbaru City?". The research procedure used in this research is a qualitative method by collecting information by associating as many as 15 informants, namely 7 key informants and 8 informants whose results from the study were analyzed using the Atlas application system. IT is useful for classifying information experienced in the field. The results that the author has are the presence of actors who are motivated to carry out exploitation, appropriate targets for exploitation and the lack of supervision regarding the exploitation of these children. And the aspects behind the formation of exploitation of minors are economic aspects, coercion from parents, culture and the absence of care. In order to minimize this problem, it is hoped that the related service and the Civil Service Police Unit will carry out patrols more often and residents will participate in overcoming these problems.

ObjectiveTo evaluate the clinical utility of individualising dual antiplatelet therapy (DAPT) after percutaneous coronary intervention (PCI) in an all-comers population, including ST-elevation myocardial infarction (STEMI) patients.SettingTertiary care single centre registry.Participants1008 consecutive PCI patients with stent implantation, without exclusion criteria.InterventionPeri-interventional individualisation of DAPT, guided by multiple electrode aggregometry (MEA), to overcome high on-treatment platelet reactivity (HPR) to ADP-induced (≥50 U) and arachidonic acid (AA)-induced aggregation (>35 U).Outcome measuresThe primary efficacy end point was definite stent thrombosis (ST) at 30 days. The primary safety end point was thrombolysis in myocardial infarction (TIMI) major and minor bleeding. Secondary end points were probable ST, myocardial infarction, cardiovascular death and the combined end point: major cardiac adverse event (MACE).Results53% of patients presented with acute coronary syndrome (9% STEMI, 44% non-ST-elevation). HPR to ADP after 600 mg clopidogrel loading occurred in 30% of patients (73±19 U vs 28±11 U; p<0.001) and was treated by prasugrel or ticagrelor (73%), or clopidogrel (27%) reloading (22±12 U; p<0.001). HPR to ADP after prasugrel loading occurred in 2% of patients (82±26 U vs 19±10 U; p<0.001) and was treated with ticagrelor (34±15 U; p=0.02). HPR to AA occurred in 9% of patients with a significant higher proportion in patients with HPR to ADP (22% vs 4%, p<0.001) and was treated with aspirin reloading. Definite ST occurred in 0.09% of patients (n=1); probable ST, myocardial infarction, cardiovascular death and MACE occurred in 0.19% (n=2), 0.09% (n=1) and 1.8% (n=18) of patients. TIMI major and minor bleeding did not differ between patients without HPR and individualised patients (2.6% for both).ConclusionsIndividualisation of DAPT with MEA minimises early thrombotic events in an all-comers PCI population to an unreported degree without increasing bleeding. A randomised multicentre trial utilising MEA seems warranted.Trial registration numberhttp://www.clinicaltrials.gov; NCT01515345.

Resumen
Cereal grains are the most important source of the world’s total food and also the major accumulator of toxic arsenic. Arsenic distributes in the environment via contaminated ground water which is used as drinking water and irrigated in the crop field. Chronic arsenic toxicity can be lethal for liver, kidney, bladder, lung, skin. Skin hyperpigmentation and hyperkeratosis are common exposure related phenotypes. Therefore, it's high time to produce ideal cereals that must contain a lower amount of arsenic and ensure safety for all kinds of lives. Ensuring safe food is one of the objectives of sustainable development goal-2 (SDG-2). With this view in mind, the Plant Breeding Division of BARI currently working with nine minor kinds of cereal included Barley, Foxtail millet, Proso-millet, Finger-millet, Pearl-millet, Buckwheat, Oat, Quinoa, and BARI-Sorghum 1 were undertaken for biochemical analysis of arsenic on their leaves. The biochemical analysis of these nine kinds of minor cerealsshowed negligible amount of arsenic on leaves. Further analysis in other location in Bangladesh might bring the overall picture of arsenic uptake in minor cereals.

The title compound, [Au(C9H10NOS)(C18H15P)], features a near linear P—Au—S arrangement defined by phosphane P and thiolate S atoms with the minor distortion from the ideal [P—Au—S is 177.61 (2)°] being traced in part to the close intramolecular approach of an O atom [Au...O = 3.040 (2) Å]. The packing features supramolecular layers lying parallel to (011) sustained by a combination of C—H...π and π–π [inter-centroid distance = 3.8033 (17) Å] interactions. The molecular structure and packing are compared with those determined for a previously reported hemi-methanol solvate [Kuanet al.(2008).CrystEngComm,10, 548–564]. Relatively minor differences are noted in the conformations of the rings in the Au-containing molecules. A Hirshfeld surface analysis confirms the similarity in the packing with the most notable differences relating to the formation of C—H...S contacts between the constituents of the solvate.

Let Kq[X] = Oq(M(m, n)) be the quantization of the ring of regular functions on m × n matrices and Iq (X) be the ideal generated by the 2 × 2 quantum minors of the matrix X=(Xij)l≤i≤m, I≤j≤n of generators of Kq[X]. The residue class ring Rq(X) = Kq[X]/Iq(X) (a quantum analogue of determinantal rings) is shown to be an integral domain and a maximal order in its divisionring of fractions. For the proof we use a general lemma concerning maximalorders that we first establish. This lemma actually applies widely to prime factors of quantum algebras. We also prove that, if the parameter isnot a root of unity, all the prime factors of the uniparameter quantum space are maximal orders in their division ring of fractions.

Resumen
Is pornography within the coverage of the First Amendment? A familiar argument claims that it is not. This argument reasons that (1) the free speech principle protects the communication of ideas, which appeal to the reason (the major premise); (2) pornography communicates no ideas and appeals to the passions rather than the reason (the minor premise); (3) therefore pornography is not protected by the free speech principle. This argument has been specified in different ways by different writers. The most prominent and careful of these are Frederick Schauer and John Finnis. Both founder on the attempt to distinguish pornography from art, which both would protect. If art, film, and literature should be protected, then this protection should extend to the pornographic subsets of these genres.

Abstract Introduction Febrile neutropenia is one of the major causes of morbidity, treatment interruptions and mortality during oncological treatment and bone marrow transplant (BMT). The time from fever to initiation of empiric antibiotics therapy (AET) is related to the outcome of patients. Ideally, patients should receive EAT in less than one hour after fever. However, many patients fail to receive EAT in less than 1 hour due to intrahospitalar delays, and may have impact in patient outcomes. Therefore, institutional policies to ensure ideal management of these patients are recommended and fever-to-patient antibiotic delivery is one of quality control measure in our Oncology and BMT Unit. Methods We implemented a nurse-based program to reduce the time from first fever or clinical EAT indication to EAT infusion. A senior nurse was in charge of implementing the program in the Oncohematological and Bone Marrow Transplantation Unit. Several actions were implemented: (1) Data collection for comparative studies, (2) Team education (pharmacy, medical and nurses) through lectures and (3) Daily data verification to verify the accuracy of data registered in medical files; (4) Survey about patient characteristics included in this study. All patients in the unit were daily censored for neutropenia, and neutropenic patients were followed for fever or need for ATB initiation. Patients who initiated EAT on the emergency room (ER) or day clinic were excluded from the analysis. For all patients, time from EAT indication and EAT infusion was collected. Data was then classified in 4 categories: (1) Time to EAT <1 hour; (2) Time to EAT >1 hour; (3) Major data inconsistency (defined when EAT indication registered time was later than EAT infusion registered time); (4) Minor data inconsistency (defined when time to EAT infusion was registered at the same time of EAT indication). Twenty medical files were retrospectively selected for comparison. Results We present the results of the first three months after the program implementation. In the retrospective group, only 35% (7/20) of patients received EAT in less than 1 hour, 15% (3/20) of patients received EAT in more than 1 hour, and inconsistencies were seen in 50% of medical files, including 40% of minor and 10% major inconsistencies. After program implementation (n=17), the percentage of patients receiving EAT in less than 1 hour was 82% (14/17), 6% of patients received EAT >1 hour (1/17), and 12% (2/17) of patients had minor inconsistencies in registered time. No major inconsistencies were observed after the program. For good practicing analysis, we grouped patients in two groups: Ideal (EAT <1 hour) and Not Ideal (EAT>1 hour and inconsistencies in medical file registration). With this approach, we showed that Ideal group improved from 35% to 82%, and the Not Ideal group declined from 65% to 18% (p=0.007). Moreover, the median time to EAT decreased from 60 minutes (range 30-240) to 30 minutes (range 8-66) (p=0.01) Conclusions The implementation of a nurse-based program significantly increased the number of patients receiving ATB in <1 hour in an Oncohematological and Bone Marrow Transplant Unit after only 3 months. Moreover, the time to ATB initiation was significantly decreased with this policy. Our findings indicate that, despite the nursing staff recognize the importance of febrile neutropenia, monitoring process, education and constant communication are necessary for an effective treatment and for improvement the patient care. Further implementation of this program in the day clinic and ER are planned and a survey about patient education of neutropenic infections will be implemented. Disclosures: No relevant conflicts of interest to declare.

Resumen
Phase formation in the system Bi–Sr–Cu–O has been examined as a function of composition, temperature, ambient atmosphere, cooling history, and annealing time. Ceramic processing and melt crystallization techniques were used. For the ceramic materials (using Bi2O3, SrCO3, and CuO) processed at 700 °C in air the Bi2Sr2CuO6 composition (221) crystallizes to a mixture of CuO, SrCO3, and the rhombohedral Bi2O3 · xSrO solid solution. At 800–830 °C in air for short durations (5 min to 2 h) the reacted products consist principally of the ideal 221 phase with minor amounts of CuO. For longer reaction times (2–400 h) the reacted products consist of the ideal 221-type structure with c = 24.64 Å and a = 3.804 Å, a “collapsed” 221 structure with c = 23.6 Å, and CuO. With increasing reaction time the “collapsed” 221 phase grows gradually at the expense of the ideal 221 phase. The “collapsed” 221 phase is not an oxycarbonate and appears to be a distinct ternary compound near the 221 composition, with a layered structure having a 1 Å smaller stacking repeat. The ideal 221 phase is a solid solution with variable Sr content. With decreasing Sr in the starting mixture [2 to 1.25 atoms per formula unit (afu)] we observe the following: (1) the formation of the “collapsed” 221 structure is inhibited; (2) for the ideal 221 phase the c-cell dimension decreases significantly (0.2 Å) and the a-cell dimension increases slightly (0.02 Å); (3) the low temperature resistivity behavior changes from superconducting with Tc onset of 6 K for Sr>1.5 afu to semiconducting for Sr > 1.5 afu; (4) the positions of the superlattice peaks around the (001) reflections become more incommensurate with respect to the parent structure. Rapid quenching (<5 s) from temperatures near the melting point (900 °C) can raise the superconducting Tc onset to 9 K. Independent of the cell variation with Sr content, quenching causes the c-cell dimension to expand by 0.03 Å on average while the a-cell dimension remains invariant. A small number of oxygen vacancies are quenched in from high temperature, and presumably originate in the Bi2O2 layer. As grown from the melt, crystals of the ideal 221 phase exhibit semiconducting behavior at low temperature; but with an additional high-temperature anneal in oxygen, metallic resistivity is restored with a superconducting onset near 5 K. Ca doping does not increase Tc in the ideal 221 phase. La and Y substitution occurs for Sr in the ideal 221 phase and ruins superconductivity.

Resumen
In order to essentially improve interfacial transition zone between cement paste and aggregate, the concept and ideal model of function aggregate are developed. Function aggregate is composed of a porous matrix with high strength and low water absorption, and a coating layer which can hydrate in cement paste. It can improve ITZ between aggregate and cement paste and control curing range of cement paste. Based on optimization of raw materials and processing techniques, high strength aggregate with low absorption and minor C2S and C4AF phase in coating layer is acquired. Results indicate that cordierite is formed in aggregate when MgO content reaches 2%, and it shows that the presence of cordierite minimizes the potential for micro-crack formation, thus decreases water absorption.

Resumen
The aim of the paper is to answer the question: how should one write about masters? It is a question about the narrative strategies of authors writing about masters. The presented analysis is based on five examples: (1) John A. Hall’s Ernest Gellner: An Intellectual Biography, (2) Anita Burdman Feferman and Solomon Feferman’s Alfred Tarski: Life and Logic, (3) Edmund Leach’s Lévi-Strauss, (4) Andrzej Walicki’s Idee i ludzie. Próba autobiografii [Ideas and People. An Attempt at an Autobiography], and (5) Dialogues by Roman Jakobson and Krystyna Pomorska. Each text presents different rhetorical devices, authorial relations to the master, and academic aims. The paper concludes with a critical comparison of the five examples (with the addition of some other minor cases also discussed in the paper).

Resumen
The title compound, [dicyclohexyl(sulfanylidene)-λ5-phosphanyl]methanol, Cy2P(=S)CH2OH (1), was obtained from the reaction between [Cy2P(CH2OH)2]Cl with one molar equivalent of NaOH and an excess of elemental sulfur (powdered). Characterization was by a single-crystal X-ray structure determination as well as IR, and 1D-NMR (1H, 13C{1H}, 31P{1H}), and 2D-NMR (DEPT-135 and HSQC) spectroscopy, ESI mass spectrometry, and elemental analysis. X-ray crystallography on Compound 1 shows the phosphorus atom to be tetrahedrally coordinated within a non-symmetric C3S donor set but with relatively minor distortions from the ideal geometry. In the molecular packing, hydroxyl-O–H⋯S(thione) hydrogen bonds led to supramolecular helical chains along the b-axis direction.

Prasminiai žodžiai: nudegimo sunkumas, nudegimo žaizdos patogenai, išskirti sukėlėjai Analysis of burn patients and the isolated pathogens Rokas Bagdonas, Algimantas Tamelis, Rytis Rimdeika, Mindaugas KiudelisKaunas Medical University, Clinic of Surgery,Eivenių str. 2, LT-50009, Kaunas, LithuaniaE-mail: rbagdonas@hotmail.com Background / objective The major challenge for a burn team is infection, which is known to cause over 50% of burn deaths. Burns become infected, because the environment at the site of the wound is ideal for the proliferation of infecting organisms. This study, approved by the regional Ethics Committee, analyzes the features of burned patients and the rates of pathogens isolated from burn wounds. Patients and methods We studied 2246 burn patients (mean age 27 years) admitted to the tertiary academic hospital in 1997–2002. The differentiation of the severity of burn injury was based on the scheme of the American Burn Association (ABA). 2462 surface swabs for microbiological analysis were taken from all 2246 patients. The wound area was swabbed with an alginate swab and cultured in 5% blood and MacConkey agar. Results There were 1447 (74%) men and 799 (26%) women (p < 0.001), age range 2–47 years. There were 1261 patients (56%, p < 0.05) with minor, 522 with moderate and 463 with major burn injuries. 2130 swabs (86.5%) out of 2462 burn wound surface swabs were positive. Out of 2130 isolates positive for pathogenic bacterial culture, there were 1110 (52.1%) isolates positive for Staphylococcus aureus infection. The rate of MRSA was 23.4% (498 isolates). Conclusions Young male patients mostly have a minor burn injury. Burn wounds are most commonly infected with S. aureus. MRSA is still the main hospital pathogen in burns. Keywords: severity of the burn injury, burn swabs, isolated pathogens

Resumen
Aalto University has been able to create a fruitful frame for activitiesenhancing interaction between mathematics, art and architecture. One of the most recent highlights of this progress was a student exhibition titled `'IN TRANSITION -- Mathematics and Art' [12] at Espoo Cultural Centre [9].The exhibition was further extended to the Aalto Math&Arts exhibition inShanghai ([3],[25]). In this paper we describe our long-lasting open-mindedcollaboration to build a minor in Mathematics and Arts that is useful fromfreshmen to PhD students across the conventional barriers between disciplines. A dialogue between scientific and artistic practices break clichés related to mathematics by bringing deep phenomena in the field to the level of human experience. Challenges and future scenarios are discussed broadly. Some ideas about the present state of Aalto Math and Arts can be found in[2].

Resumen
Abstract. The beginning of the new Millenia brings new social challenges adjoint with the antisocial behavior of the youth. Crime rates are clearly on the rise among the young people, and their crimes in turn significantly worsen the criminal situation in the society as a whole. The trend can be also observed in the raise of drug abuse among the younger generation, that provokes even more criminal acts. Minors are dragged into the criminal business (such as drug trafficking etc.), nudged to the side of different sexual deviances. Considering this, knowledge, skills and abilities in the field of deviantology among the specialist working with minors are especially relevant and important today. Teachers and psychologist are the part of that cohort; therefore, we talk about their professional ability to inhibit the growth of antisocial behavior among the minors. The goal of the study was to assess the level of expertise in the field of deviant behavior of minors among the future educators and psychologists, to research the student’s opinions on deviant persons and identify their social and professional responsibility in the sphere of pedagogical and psychological prevention of deviancy. Methods. Methodology of the research is based on the subject-oriented approach to the study of a person, which explores the person traits introspectively, considering the person as a carrier of an individual worldview, a certain system of individual meanings and reasons. That approach supposes that persons behavior tends to be repetitive in the most different situations happening as a result of the intervening and interplaying objective and subjective factors. In order to achieve the chosen goal, the authors have used a method of survey. They surveyed 144 pedagogic and psychology juniors age 20 – 25. The sample consisted of the students who not only possessed a basic level knowledge but also a personal opinion on the questions of deviant behavior. Results. The authors have found the following: 1) the cognitive component shows that students have relevant notions on deviant behavior, its predictors and age characteristics; 2) the affective component is more differentiated and heterogeneous then a cognitive one; 3) conative component lets us suppose that school is a key factor in minors’ socialization, but lacks powers to prevent and correct the deviant behavior of kids and young people. Scientific novelty. The achieved results show the necessity to provide psychology and pedagogic students the knowledge in the field of deviantology. The work widens our ideas on cognitive, affective and conative components of their learning and professional competence. Empirical significance. The findings of the work point us to the importance of the additional curriculum hours dedicated to the deviant behavior studies, choosing the most effective educational methods and technics in that matter, as well as offering psychological trainings raising stress resilience, forming self-regulation and constructive communication in complex life situations.

Resumen
BackgroundPolicies aimed at diverting care from EDs to alternative services have not been successful in reducing ED attendances and have contributed to confusion for service users when making care-seeking decisions. It is important that service users are at the heart of decision making to ensure new services meet the needs of those who will be accessing them. In this study, service users were encouraged to think freely about the desirable qualities of an ideal urgent and emergency care (UEC) system.MethodsFrom September to February 2019, an open inductive methodology was used to conduct focus groups with service users who had used UK UEC services within the previous year. Service users that had contact with NHS111, ambulance service, General Practice out-of-hours, minor injuries unit, walk-in centre or ED were purposively sampled and stratified into the following groups: (1) 18–45 years; (2)≥75 years; (3) adults with young children; (4) adults with long-term conditions. Focus groups were structured around experiences of accessing UEC services and perspectives of an ‘ideal’ UEC system.Results30 service users took part in the study, across four focus groups. The ideal UEC system centred around three themes: a simplified UEC system (easier to understand and a single-point of access); more ‘joined-up’ UEC services and better communication between health staff and patients.ConclusionDesirable qualities of an ideal UEC system from a service user perspective related to simplifying access for example, through a single point of access system where health professionals decide the appropriate service required and improving continuity of care through better integration of UEC services. Service users value reassurance and communication from health professionals about care pathways and care choices, and this helps service users feel more in control of their healthcare journey.

Resumen
AbstractIn his ethnomusicological writings and lectures, Béla Bartók describes folk music as ‘a natural product, just like the various forms of animal and vegetable life’ and elaborates this view, going on to describe a collection of developmental processes modelled explicitly on biological evolution. In this article, I characterize Bartók's evolutionary model by laying bare the taxonomies and genealogies inherent in his classificatory system. Then, through an analysis of the fifth of hisEight Improvisations on Hungarian Folk Songs(1920), I suggest the outlines of a method for interpreting and analysing Bartók's music engendered from this evolutionary model, a method that involves the elaboration of two ideas: (1) a conceptual shift from a relatively historically static major/minor tonality to a multivalent, ‘evolving’ tonality, and (2) the reconception of motives or themes as having no single original forms, but rather as being related genetically, as somehow evolving in their own right.

Resumen
Abstract The crystal structure of polylithionite-1M from Darai-Pioz, (K0.97Na0.03Rb0.01)Σ1.01(Li2.04Al0.84 Ti4+0.09Fe3+0.03)Σ3.00(Si3.98Al0.02)O10[F1.68(OH)0.33]Σ2, a 5.1974(4), b 8.9753(6), c 10.0556(7) Å, β 100.454(1)°, V 461.30(6) Å3, space group C2, Z = 2, was refined to R1 = 1.99% using MoKα X-radiation. In the space group C2, there are three octahedrally coordinated M sites in the 1M mica structure: the M(1) site is occupied by Li+ and minor vacancy that is likely locally associated with Ti4+ at the M(2) site; the M(2) site is occupied dominantly by Al3+, with other minor divalent to tetravalent cations; the M(3) site is completely occupied by Li+. In the space group C2, the structure is completely ordered. Each non-bridging O2– ion is surrounded by an ordered arrangement of 2Li+ + Al3+ + Si4+ with an incident bond-valence sum of 1.95 vu (valence units). The F– ion is coordinated by Li+ + Li+ + Al3+ with an incident bond-valence sum of 0.84 vu (values around F– generally tend to be lower than ideal). Thus, the valence-sum rule is satisfied, both long range and short range. In the space group C2/m, there is long-range order but not short-range order. There are three different short-range arrangements, one of which has bond-valence deficiencies of 0.38 and 0.49 vu around the non-bridging O2– ion and the F– ion, destabilizing the structure relative to the more ordered arrangement of the C2 structure, which conforms more closely to the valence-sum rule. The drive to lower the symmetry in polylithionite-1M from C2/m to C2 comes from the short-range bond-valence requirements of the structure.

Resumen
Background: More than 2% of the general population experience suicidal ideas each year and a large number of them will attempt suicide. Evidence-based therapeutic options to manage suicidal crisis are currently limited. Objectives: The aim of this study was to overview the findings on the use of ketamine and esketamine for the treatment of suicidal ideas and acts. Design: Systematic review. Data Sources and methods: PubMed, article references, and Clinicaltrials.gov up to June 30, 2022. Meta-analyses published within the last 2 years were also reviewed. Results: We identified 12 randomized controlled trials with reduction of suicidal ideation as the primary objective and 14 trials as secondary objectives. Intravenous racemic ketamine was superior to control drugs (placebo or midazolam) within the first 72 h, in spite of large placebo effects. Adverse events were minor and transient. In contrast, intranasal esketamine did not differ from placebo in large-scale studies. Limitations, clinical considerations, and opportunities for future research include the following points: large placebo effects when studying suicidal ideation reduction; small concerns about blinding quality due to dissociative effects; no studies on the risk/prevention of suicidal acts and mortality; lack of studies beyond affective disorders; no studies in adolescents and older people; lack of knowledge of long-term side effects, notably liability for abuse; no robust predictive markers; limited understanding of the mechanisms of ketamine on suicidal ideas; need for improved assessment of suicidal ideation in clinical trials; need for studies in outpatient settings, emergency room, and liaison consultation; need for research on ketamine administration; limited knowledge on the positive and negative effects of concomitant treatments. Conclusion: Overall, there is compelling evidence for a favorable short-term benefit-risk balance with intravenous racemic ketamine but not intranasal esketamine. The place of ketamine will have to be defined within a multimodal care strategy for suicidal patients. Caution remains necessary for clinical use, and pharmacovigilance will be essential.

Resumen
Aim. Optimization of the surgical tactics in hepatic echinococcosis.Material and methods. From 2009 to 2021, 56 patients with hepatic echinococcosis have been operated: 6 patients underwent PAIR, 7 – open echinococcectomy, 18 – pericystectomy (“ideal echinococcectomy”) and 16 – anatomical hepatectomy (S2/3, S6/7 bisegmentectomy) or atypical resection. In 7 cases hemigepatectomy has been performed, in 2 – combined interventions have been performed for echinococcosis relapse of the abdominal cavity.Results. The duration of minor hepatic resection was shorter than pericystectomy and major hepatic resection. The smallest blood loss was noted in patients who underwent open echinococcectomy, anatomical and atypical hepatectomy. No complications after PAIR were noticed. The lowest incidence of postoperative complications, including biliary complications, was found after hepatic resection interventions. The lenths of hospital stay after hepatic resections was significantly shorter compared to ecinococcectomy and pericystectomy. The reccurence of hepatic echinococcosis relapse was detected in 4 (66.7%) patients after PAIR and 2 (28.6%) patients after ecinococcectomy.Conclusion. Organ-preserving techniques should be a priority in the surgical treatment of hepatic echinococcosis. The best results in terms of the early postoperative complications frequency in the hepatic echinococcosis treatment were demonstrated by minor hepatic resections. Open echinococcectomy should be performed only for central location of large cysts and (or) their massive contact with the liver vessels. Minimally invasive resection technologies demonstrate optimal short- and long-term results. Major hepatectomy should be performed very selectively and only in those cases when they have advantages over other methods, and their implementation does not lead to a loss of more than 20% of healthy hepatic parenchyma. The best long-term results showed pericystectomy and hepatectomy in comparison with echinococcectomy.

Resumen
SynopsisPteridophytes with subterranean sexual stages exist in five families. Their prothalli grow underground without light, nourished by fungi, and are rounded, massive, and without chlorophyll. Various authors have suggested that such gametophytes may inhibit outbreeding ability and that minor variations may therefore form in colonies through inbreeding. This paper questions these ideas. The subterranean condition is believed to be suited to fields, grassy meadows, and other habitats subject to seasonal drying. The parenchymatous thallus makes possible water storage. Water can flow between some particles. Sperm may be carried by mass flow in various directions and can evidently swim as much as 2 cm. Close clumping of underground gametophytes appears to be no less common than in surficial gametophytes and evidence exists that hybridisation occurs. No evidence has been detected that outbreeding is inhibited, and we conclude that it is successful and that cross-fertilisation is no less common among underground gametophytes than among surface gametophytes. Numerous examples are listed from Lycopodiales, Psilotales, and Ophioglossales.

Resumen
Introduction. Subvesical bile duct (SVBD) injury is a secondary major cause of minor bile duct injury after laparoscopic cholecystectomy (LC). However, this injury is usually not recognized intraoperatively, but postoperatively. Case Report. Case 1: the patient was an 84-year-old female, preoperatively diagnosed with acute cholecystitis. During LC, a tiny hole in the gallbladder fossa from which bile juice oozing was confirmed. Suturing was performed laparoscopically. Case 2: the patient was an 81-year-old male, preoperatively diagnosed with cholelithiasis. Because of a previous history of gastrectomy, laparoscopic adhesiolysis around the gallbladder was performed. During dissection, a small amount of bile was oozing from the surface of the liver adjacent to the gallbladder fossa. Suturing was performed laparoscopically. Conclusion. If a small amount of bile juice was detected, meticulous observation not only around the cystic duct stump but also the gallbladder fossa should be performed. Simultaneous laparoscopic suturing was feasible, and an ideal procedure against SVBD injury developed during LC.

Resumen
AbstractEndoskarns formed where a swarm of diorite dykes have intruded calcite marble at Redcap Creek include an inner melilite-dominated, a wollastonite-dominated, and an outer massive tilleyite zone in contact with marble. The massive tilleyite is unusual in that it is coarse-grained (prisms 2–15 cm in length) and its deep grey colour contrasts with the lighter coloured varieties described elsewhere. A chemical analysis gives a formula close to ideal, with only minor substitution of Al, Ti, and Mg. Refined unit cell parameters are in close agreement with those quoted in the literature. The skarns have clearly formed by transport of Si, Mg, Fe, Al, and Ti from the igneous rocks, and Ca in the reverse direction from the marble. Activity diagrams derived from experimental data are most useful in interpreting the zonal sequence of endoskarns, and preliminary results suggest mass transfer at low Xco2 and temperature of the order of 800°C or higher for the formation of the massive tilleyite.

"Can Lateralization of Reverse Shoulder Arthroplasty Improve Active External Rotation in Patients with Preoperative Fatty Infiltration of the Infraspinatus and Teres Minor?" Resumen
(1) Background: Postoperative recovery of external rotation after reverse shoulder arthroplasty (RSA) has been reported despite nonfunctional external rotator muscles. Thus, this study aimed to clinically determine the ideal prosthetic design allowing external rotation recovery in such a cohort. (2) Methods: A monocentric comparative study was retrospectively performed on patients who had primary RSA between June 2013 and February 2018 with a significant preoperative fatty infiltration of the infraspinatus and teres minor. Two groups were formed with patients with a lateral humerus/lateral glenoid 145° onlay RSA—the onlay group (OG), and a medial humerus/lateral glenoid 155° inlay RSA—the inlay group (IG). Patients were matched 1:1 by age, gender, indication, preoperative range of motion (ROM), and Constant score. The ROM and Constant scores were assessed preoperatively and at a minimum follow-up of two years. (3) Results: Forty-seven patients have been included (23 in OG and 24 in IG). Postoperative external rotation increased significantly in the OG only (p = 0.049), and its postoperative value was significantly greater than that of the IG by 11.1° (p = 0.028). (4) Conclusion: The use of a lateralized humeral stem with a low neck-shaft angle resulted in significantly improved external rotation compared to a medialized humeral 155° stem, even in cases of severe fatty infiltration of the infraspinatus and teres minor. Humeral lateralization and a low neck-shaft angle should be favored when planning an RSA in a patient without a functional posterior rotator cuff.

Resumen
Representative, actively collected surveillance data on asymptomatic SARS-CoV-2 infections in primary schoolchildren remain scarce. We evaluated the feasibility of a saliva mass screening concept and assessed infectious activity in primary schools. During a 10-week period from 3 March to 21 May 2021, schoolchildren and staff from 17 primary schools in Munich participated in the sentinel surveillance, cohort study. Participants were tested using the Salivette® system, testing was supervised by trained school staff, and samples were processed via reverse transcription quantitative polymerase chain reaction (RT-qPCR). We included 4433 participants: 3752 children (median age, 8 [range, 6–13] years; 1926 girls [51%]) and 681 staff members (median age, 41 [range, 14–71] years; 592 women [87%]). In total, 23,905 samples were processed (4640 from staff), with participants representing 8.3% of all primary schoolchildren in Munich. Only eight cases were detected: Five out of 3752 participating children (0.13%) and three out of 681 staff members (0.44%). There were no secondary cases. In conclusion, supervised Salivette® self-sampling was feasible, reliable, and safe and thus constituted an ideal method for SARS-CoV-2 mass screenings in primary schoolchildren. Our findings suggest that infectious activity among asymptomatic primary schoolchildren and staff was low. Primary schools appear to continue to play a minor role in the spread of SARS-CoV-2 despite high community incidence rates.

Resumen
17525 Background: Follicular dendritic cells (FDC) support the survival of follicular lymphoma (FL). TNFα is overexpressed by FL cells and TNFα plays a central role in the development and maintenance of normal FDCs. TNFα may be an ideal target for therapy due to its pleotropic effects on FDCs. Inhibition of TNFα is possible with the decoy receptor, etanercept. Methods: Patients with relapsed/refractory FL received 8 weeks of etanercept, 25 mg SC on day 1 and 4 of each week. Patients with any response or stable disease (SD) received 16 more weeks of etanercept at the same dose/schedule. 7 patients enrolled from April 2002 to September 2005. Median age was 63. All patients had stage III/IV disease and had received multiple chemotherapy regimens (median 3); 2 had prior autologous stem cell transplant. FDG-PET was performed at baseline and after 8 (scan 1, n = 7 patients) and 24 weeks on therapy (scan 2, n = 3 patients). Maximum standardized uptake values (SUVmax) were measured in up to 3 lesions per patient (n = 17 lesions for scan 1; 7 lesions for scan 2). The summation SUVmax of all lesions was calculated at each time point. The percentage (%) change in SUVmax in individual lesions and the summed SUVmax for each patient was calculated relative to baseline. The % metabolic response (mR) was assessed using EORTC thresholds for % SUVmax change (mPR ≤ −25% < mSD < +25% ≤ PD). Results: All patients completed at least 8 weeks of etanercept. 2 patients completed 24 weeks. 5 patients had minor or mixed responses. At the 8 week evaluation 5 patients had SD and 2 had progressive disease (PD). Of the 5 with SD, 2 progressed at 9 and 12 weeks of therapy and 3 progressed by 24 weeks. PET scan 1 showed mPR in 5/7 pts, SD in 2/7 and no PD. PET scan 2 showed mPR in 2/3 pts, mSD in 1/3 and no mPD.All patients are alive at a median of 20+ months after therapy. 1 grade 3 toxicity (lymphopenia) and 3 grade 1/2 toxicities (rhinitis/URI and 2 injection site reactions) were reported. Conclusions: Etanercept was well tolerated and minor clinical responses were observed. By EORTC criteria for metabolic response, mPR occurred in 5/7 pts, mSD in 2/7 and there was no mPD. The significant number of metabolic PR’s suggest that targeting the microenvironment with agents like etanercept may be a novel treatment approach for FL. No significant financial relationships to disclose.

Resumen
Introduction. The author analyzes the features of the historical memory of the American Civil War in American pop culture as a form of historical memory that dominates in the consumer society. Methods and materials. The problems of imagination and invention of images of the civil war are analyzed in the contexts of cultural history, as well as intellectual history and the history of ideas. Analysis. The author presumes that 1) the historical memory of the U.S. Civil War in modern American identity is heterogeneous; 2) various forms of culture, including “high”, “mass” and “popular”, actualize the images of the civil war; 3) “high” culture became the first form of imagination of the civil war in American identity; 4) in the 20th century, various forms of consumer society culture (“mass” and “popular”) assimilated images of the civil war; 5) “popular” culture presents a “transitional” (from “high” to “mass”) version of the memory of the war. Results. Analyzing the transformation of the Civil War memory in the pseudo-documentary film “C.S.A.: The Confederate States of America” (directed by Kevin Willmott), the author presumes that 1) popular culture reduced the civil war to a minor event in the history of the Confederation; 2) the film offers an alternative version of American historical memory, actualizing the possible trajectories of the “invention” of images of the civil war in the reality where South won; 3) the film is integrated into the intellectual history and archeology of American mass-cult ideas; 4) some texts by American intellectuals belonging to the discourse of alternative history, became cultural stimuli for the project of Kevin Willmott; 5) the transformation of the memory of the Civil War in American pop-culture actualizes the crisis of memory in society, which wasn’t successful in its attempts to overcome the historical trauma of racism and racial discrimination.
