Abstract:
Primary mediastinal B-cell lymphoma (PMBCL) is a relatively rare lymphoma subtype affecting mainly seen in young adults with female predominance. It constitutes approximately 2–4% of all non-Hodgkin’s lymphomas (NHLs). No risk factors for this type have been identified but it may be related to 5533 C>A mutation in the MLL gene. Its molecular signature and clinical features resemble classical Hodgkin’s lymphoma. PMBCL belongs to a group of aggressive diffuse large B-cell lymphomas. 2008 WHO classification distinguishes this lymphoma as a separate entity due to its specific clinical features and pathological features. Gene expression profile studies showed that it shares common features with classical Hodgkin’s lymphoma. The optimal chemotherapy for this lymphoma subtype has not been established. Furthermore, no convincing data are supporting the use of radiotherapy. Relatively low patient numbers are the main obstacle in conducting randomised prospective trials. Hence, therapeutic decisions have been based mainly on retrospective studies.

Abstract This study explores the education of young Polish female refugees in postwar Britain, and the implications of the establishment of the Committee for the Education of Poles, a body brought into being on April 1, 1947, as a consequence of the passage of the Polish Resettlement Bill in March 1947 (the first ever British legislation dealing with mass immigration). It examines the newly introduced grammar schools for girls, the challenges raised for these new educational establishments, and the strategies introduced by the British authorities in order to cope with the growing numbers of Polish adolescents entering the country after World War II. Based on archival records and supported by oral interviews, the sample case studies presented herein highlight one way in which the young Polish community was rehabilitated in exile at the start of their path towards civic integration. They highlight the key importance of education to that pathway. The studies presented here provide an insight into the experiences of, and challenges faced by, young Polish women during the Process of their education and resettlement. Thanks to extant school reports, it has been possible to ascertain not only the practical and financial problems that impacted the schools presented in the paper, which operated under the auspices of the Committee for the Education of Poles, but also their slow, but steady improvement over time. The paper provides clear evidence that it was thanks to the Committee that a significant proportion of the younger generation of Poles managed to acquire the qualifications needed to secure their first jobs and ultimately a career in industry or business and so play a useful part in the economic life of Great Britain and her overseas territories.

The development of late-onset Alzheimer's disease (LOAD) is under strong genetic control and there is great interest in the genetic variants that confer increased risk. The Alzheimer's disease risk gene, growth factor receptor bound protein 2-associated protein (GAB2), has been shown to provide a 1.27–1.51 increased odds of developing LOAD for rs7101429 major allele carriers, in case-control analysis. GAB2 is expressed across the brain throughout life, and its role in LOAD pathology is well understood. Recent studies have begun to examine the effect of genetic variation in the GAB2 gene on differences in the brain. However, the effect of GAB2 on the young adult brain has yet to be considered. Here we found a significant association between the GAB2 gene and morphological brain differences in 755 young adult twins (469 females) (M = 23.1, SD = 3.1 years), using a gene-based test with principal components regression (PCReg). Detectable differences in brain morphology are therefore associated with variation in the GAB2 gene, even in young adults, long before the typical age of onset of Alzheimer's disease.

In the public health sector and the field of medicine, the popularity of data mining and its usage in knowledge discovery and databases (KDD) are rising. The growing popularity of data mining has discovered innovative healthcare links to support decision making. For this reason, there is a great possibility to better diagnose patient’s diseases and maintain the quality of healthcare services in hospitals. So, there is an urgent need to make disease diagnosis possible by discovering the hidden patterns from the patients’ history information in developing countries. This work is a step towards how to use the extracted knowledge to enhance the quality of healthcare facilities. In this paper, we have proposed a web-centered hospital information management system (HIMS) that identifies frequent patterns from the data with eye disorder patients using the association rule-based Apriori data mining technique. The proposed framework has the capability to overcome all the key issues and problems in the current hospital information management system regarding data analysis and reporting services. For this purpose, data were collected from more than 1000 university students (China citizens) both online and manually (printed questionnaire). After applying the Apriori algorithm on the collected data, we revealed that almost 140 individuals out of 1035 had myopia (near-sighted disorder), at current age of 22 years, and that there were no male patients found with myopia. We concluded that their clinical relevance and utility can generate favorable results from prospective clinical studies by mapping out the habits or lifestyles that potentially lead to fatal diseases. In the future, we plan to extend this work to fully automate HIMS to help practitioners to diagnose the reasons of various diseases by extracting patient lifestyle patterns.

Abstract:
Purpose The purpose of this paper is to discuss architecture that produces spatial experience with which children and young adults are able to interact, giving them a great sense of positive energy that translates into actual healing. This type of architecture is few in number but can create a transition toward sustainable healing. Design/methodology/approach Spatial experience was evident in the architectural practice of the ancient world. Back then architects considered what moods should they give to spaces that best suit their functions. In our contemporary world, this trail of thinking is replaced by architecture that do not connect with the user’s psychology. The paper will prove that there are few architects today who are willing to exert an effort in providing the right moods for their buildings with a sustainable vibe. The paper will discuss this point by taking four examples of architecture specialized in healing young adults and children. Findings Through analysis of the case studies, the paper reveals the importance of spatial experience approach in producing meaningful architecture that connects with the user. The paper shows that it is through this approach that important moments of architectural history was made as well as the works of famous architects of our times. Originality/value The research redefines how should we look at architectural history through spatial experience analysis. It also gives us an insight into how architects become famous today through their unique design process that continue to be successful and admired by ordinary users not just specialists. The research is not limited to this paper, but currently expanding to include other case studies of different building types.

Sudden cardiac death (SCD) in adolescents and young adults is a major traumatic event for families and communities. In these cases, it is not uncommon to have a negative autopsy with structurally and histologically normal heart. Such SCD cases are generally attributed to channelopathies, which include long QT syndrome, short QT syndrome, Brugada syndrome, and catecholaminergic polymorphic ventricular tachycardia. Our understanding of the causes for SCDs has changed significantly with the advancements in molecular and genetic studies, where many mutations are now known to be associated with certain channelopathies. Postmortem analysis provides great value in informing decision-making with regard to screening tests and prophylactic measures that should be taken to prevent sudden death in first degree relatives of the decedent. As this is a rapidly advancing field, our ability to identify genetic mutations has surpassed our ability to interpret them. This led to a unique challenge in genetic testing called variants of unknown significance (VUS). VUSs present a diagnostic dilemma and uncertainty for clinicians and patients with regard to next steps. Caution should be exercised when interpreting VUSs since misinterpretation can result in mismanagement of patients and their families. A case of a young adult man with drowning as his proximate cause of death is presented in circumstances where cardiac genetic testing was indicated and undertaken. Eight VUSs in genes implicated in inheritable cardiac dysfunction were identified and the interpretation of VUSs in this scenario is discussed.

IntroductionAdolescent transitions to adulthood are a vulnerable phase for the development of mental illnesses. Additionally, there are often disruptions in psychiatric care delivery during the transition phase, potentially leading to a considerable treatment delay with a high risk of early chronification. Thus, the health care system and professionals in both child and adolescent psychiatry and adult psychiatry should be given greater consideration to the transition phase.ObjectivesThe aim of the project ProTransition is the development of an online course for health care professionals to give in-depth knowledge of “transition psychiatry”, practical guidance and to sensitize them for the special challenges and needs of young adults with mental illness.MethodsThe online-course is being developed at the Department of Child and Adolescent Psychiatry/ Psychotherapy, Ulm and is expected to start in May 2021. It comprises e.g. special psychopathology of emerging adulthood, clinical interventions for adolescents with mental illness or legal aspects. An innovative and multi-didactical approach with specialized texts, case-studies, online-chats and interviews with experts and young people is applied. Additionally, user satisfaction with the online course will be evaluated.ResultsOn the basis of the gained experiences, ideas for new transition-psychiatric treatment models will be derived. The accompanying research will point out the status quo and the course-related increasing knowledge of health care professionals regarding transition psychiatry. First results are expected in November 2021.ConclusionsAs transition psychiatry is facing great difficulties and challenges, professionals should be adequately educated. E-Learning offers a flexible and low-level approach to reach a broad target group.

Coronary arteriovenous fistula (CAF) is a rare form of congenital heart disease. However, it is the most common type of congenital anomaly of the coronary arteries. [1] When there is the connection between the coronary artery and the chambers of the heart, it is called a coronary chamber fistula. The fistula may also be between a coronary artery and another adjacent vessel from the pulmonary or systemic circulation. An open fistula provides low-resistance flow by directing blood from an artery into a vein, heart chamber, or other low-pressure vessel such as the pulmonary artery. Patients with CAF may develop symptoms at birth or later in life, depending on the type of fistula and the presence of collateral circulation. Studies have reported an association between ventricular arrhythmias and sudden cardiac death syndromes in young adults and athletes with certain types of coronary anomalies, such as anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) [2-5]. The most common symptom is a myocardial ischemia. The purpose of this article is to present a clinical case of endovascular treatment of coronary pulmonary fistula. As a result of a modern diagnostic methods, such as CT angiography with three-dimensional reconstruction, it is not difficult to assess the degree and nature of the pathology. Having assessed the tactics, modern doctors are able to cope with coronary arteriovenous fistulas with great success using minimally invasive X-ray endovascular technologies.

An epidemic of suicide is spreading in the world, which, according to UNICEF, has become the third most common cause of death for young people aged 15-19. Typically, one school suicide affects about 135 people around the suicide, leading to suicide clusters, mental disorders, and other dangerous consequences. At present, Ukraine has developed a scientifically sound set of tools for diagnosing suicidal ideation, early intervention, and crisis intervention in case of suicide attempts, in particular in educational institutions. At the same time, fasting as psychological support for affected groups and individuals remains the most minor developed part of the suicide prevention system in educational institutions. The article aims to analyze foreign models (protocols) of suicide and determine the directions of development of the domestic model of post-support of the staff of the educational institution in which the suicide (bullycide) or his attempt occurred. Protocols of preventive measures developed and applied as models in five countries are analyzed: Australia, Great Britain, Belgium, Canada, USA. According to the results of the analysis, invariant (which are in all protocols) and variable (which are in some, but not in other protocols) components of the structure are distinguished. Invariant components of postvention protocols are creating a crisis team responsible for crisis response, early development, and creation of crisis response routes, early training of relevant specialists, chronological regulation of postvention process, normalization of behavior, standardization of behavior, communication, and social space organization. Variable components primarily reflect the community’s cultural, socio-economic, political-psychological specifics for which the protocol is developed. Creating a suicide protocol (bullycide) adapted to domestic realities or trying to commit it in an educational institution will reduce the destructive impact of a suicide episode on people at risk, including the likelihood of suicide clusters, retraumatization of the immediate environment, etc.; will contribute to the faster normalization of the socio-psychological climate in the educational institution and its return to normal operating conditions

Abstract:
Multiple sclerosis (MS) is one of the most common diseases of the central nervous system (CNS), one of the biggest puzzles in medicine. A number of drugs and therapies have been developed that affect the symptoms or change the severity and frequency of the seizures. Multiple sclerosis is of great social and economic importance to the world, due to the great invasion at early age. It affects the "young adults" in their creatively active period - between 10 and 50 years of age. Contrary to most allegations that pain is absent in MS patients, this is not the case. In the first periods of the disease, pain is almost absent, apart from the periodic manifestation of cramps, perceptions and all types of pain in the body, which at night interfere with sleep, provoke chronic fatigue and irritability (as they themselves have other mechanisms leading to the event their). However, some studies show that pain persists more frequently in women than in men. In most cases, disabling and immobilizing patients is exalted by their attempts to avoid pain. Physiotherapy is a basic tool for maintaining and improving the general condition of the patient. The study involved 22 patients with relapsing-remitting MS. The survey was conducted on the territory of Sofia - NSA "Vasil Levski", in the period from 01.09.2013 to 30.10.2015. The pain syndrome was evaluated in multiple sclerosis patients, at the beginning, the 10th procedure and the last procedure. For each patient in the experimental group, 20 procedures were performed within 10 weeks. The duration of the individual activity was 1 hour. The procedures for hippotherapy were a total of 16, each lasting 30 minutes. After the completion of the 10-week Physiotherapy program, each of the patients continues the hippotherapy courses on the Khan Asparuh horse base. The therapeutic program we have prepared and tested includes the following: Exercise with a large therapeutic ball (fitball); proprioceptive neuromuscular facilitation (PNF); myofascial techniques (MFT); autogenous training (AT); hippo therapy (HT). During the study, the subsequent results showed a significant decrease in pain in the tested group. At the end of the study, the improvement of the indicators from the initial to the final study were seen. The statistical significance is less than or equal to 0.01, with the possibility of error being minimal. From the conclusions, it can be said that the methodology thus performed brings great positives in terms of pain reduction in women with multiple sclerosis who participated in the study.

Abstract:
Sociologists are generally in agreement that the closing decades of the twentieth century involved striking changes in the landscape against which British young people grew up. Transformations in education and the labour market had the potential to dramatically alter and re-shape patterns of social inequality. This thesis addresses the importance of family effects upon educational attainment, early career prospects and, in turn, the post-16 trajectories of young adults against the contextual changes of this period. Recently, youth researchers have been keen to argue that we are continuing to progress towards a ‘post-modern era’, which centres on the ‘individualisation’ or ‘detraditionalisation’ arguments of Beck and Giddens; where structural factors, such as gender and social class are diminishing as the defining elements of the pathway a young person will take. In this study, the British Household Panel Survey (BHPS), a contemporary source of longitudinal data from the early 1990s onwards, is used to demonstrate a lack of evidence of detraditionalisation, or the weakening of structural factors in determining the outcomes of young people. To the contrary, the gap between those from advantaged and less advantaged backgrounds remains wide. Furthermore, this research augments and extends previous studies of educational and early labour market outcomes by providing more comprehensive and integrated statistical analyses of household, family and parental effects, using techniques for longitudinal data analysis which give insight into patterns of social inequality being replicated in current contexts. Evidence using 17 years of longitudinal panel data indicate that, over time, family effects on school attainment and early labour market outcomes remain strong.

This study explores teacher's knowledge and practice of empowering young children as learners. Empowerment is a complex and multifaceted construct, and a recurring theme in early childhood literature. This study took place in four early childhood settings in Australia and the United Kingdom. The research was conducted using qualitative methodology, primarily with the use of video-taped observations and stimulated-recall teacher interviews. Findings indicate that the teachers enacted their knowledge and practice of empowerment. However, empowerment was interpreted differently by each teacher. The current study found links existed between teachers' knowledge and practice and their pedogogic orientation.

Abstract The notion that childhood provides the basis of the adult’s intellectual and moral status is extremely old. However, with the exception of studies of height growth, the idea that the foundation of adult health is prepared in the early years of life has been slower to develop. In Britain the findings of the Boer War recruiting medical examinations (MacKenzie & Matthew, 1904) and the Interdepartmental Committee on Physical Deterioration (British Parliamentary Papers, 1904) raised anxieties about the health of the nation’s young people. Although the outcome was of great value in terms of the public health care of mothers and infants (Dwork, 1987), the implications for epidemiologic studies of etiology were recognized much more slowly.

Among the actions planned to achieve the goal of the European Green Deal to become climate neutral by 2050 is accelerating the transition to sustainable and smart mobility [1]. The action indicates the need to put the user at the center and to reduce emissions by 90% by 2050 through the development of multimodal, automated, and connected mobility using clean and sustainable energy sources.While the transition to electric cars enables the reduction of fossil fuel consumption and urban pollution, on the other hand it does not alleviate urban traffic and parking problems, and presents a major challenge regarding the recycling of end-of-life vehicles and batteries.For this reasons micro-mobility solutions, which include a wide range of light vehicles such as e-bikes and e-scooters (shared and personal), represent an alternative with great potential capable of contributing to the creation of healthy and smart environments, balancing a low demand for physical effort with a substantial environmental benefit [2]. However, many studies highlight how these solutions are currently attractive primarily to a younger target [3] and within specific usage scenarios related to the tourism and recreational context [3] and last-mile connections for commuters [2]. Due to reduced physical performance, in fact, ease of travel by bicycle can be an insurmountable barrier for older users [4]. The same barrier also recurs in the case of e-scooters, for which skepticism and risk perception are even higher, mainly due to the balance control skills that the vehicle requires when being used [3], as well as road safety issues and inadequate infrastructure for micro-mobility.This means that a large share of users remain excluded from the solutions currently available, with a strong negative impact on the development of sustainable mobility, considering the fact that current demographic trends highlight the progressive increase in the number of elderly citizens in many countries around the world. Promoting micro-mobility as the most efficient urban and peri-urban mobility system in terms of emissions, energy and material consumption therefore implies a paradigm shift in the design of light vehicles, with a design for inclusion approach that overcomes the main issues limiting their deployment and leads to new functional solutions to better meet the needs of older user groups [5]. The paper presents some research results in this topic, which, starting from the identification of emerging needs and trends in urban and peri-urban mobility of older users with different lifestyles, investigates new scenarios of use, aimed at the inclusion of a wide range of users, in the development of light vehicles and the study of potential offered by artificial intelligence to make vehicles and people autonomous.With the aim of improving people's ability to move autonomously, sustainably and inclusively, three design solutions of light vehicles for people and goods, inclusive and unisex, were developed, all oriented at promoting an active and healthy lifestyle, and improving safety, independence and efficiency in user mobility at urban level. The projects presented in this paper were selected among the concepts that emerged within various Design Sprint sessions held with young designers and in one case developed for participation in a Horizon-CL5-2023-D5-01 project.
