AbstractFrom 1971 to 1981, 16752 women were delivered at the University Women's Clinic in Heidelberg; 228 of these were twin pregnancies (1.36%). The relationships between sex, gestational age, birthweight, mortality, and fetal outcome were investigated. For the reason of grading, so-called χ2 curves are used. These curves show the strength and the course of relationship between two parameters. Mortality of the twin (8.3%) depended on birthweight and on sex of the infant, but not on the mode of delivery. All twins were ‘small-for-date’ (25th weight percentile of single births). Starting at the 35th week of gestation, the increase in weight decelerated additionally. So, immaturity as well as twin-related factors led to the high mortality rate for twins. Fetal outcome was evaluated in respect to the birthweight and to the delivery interval.

Objective To determine the successful decannulation rate with a published pediatric tracheostomy decannulation protocol. Study Design Case series with chart review. Setting A single tertiary care institution. Subjects and Methods A chart review was performed for patients aged ≤5 years who underwent tracheostomy. Extracted data included demographic data, indication for tracheostomy, age at tracheostomy and decannulation, comorbidities, and surgical complications. Records were searched for documentation of early decannulation failure (within 1 month of decannulation) or late failure (within 1 year). Results Forty patients with a tracheostomy aged ≤5 years underwent attempted decannulation during the 11-year study period. Seventeen patients were excluded from the study for documentation of nonprotocol decannulation. The final study population of 23 patients underwent a total of 27 decannulations, 26 of which were performed by protocol. Of the 26 protocol decannulations, 22 were successful, for a failure rate of 15%. Conclusion Twenty-six protocol decannulations were attempted among 23 patients, 4 of which were unsuccessful for an overall failure rate of 15%. This result is consistent with rates reported in other published decannulation protocols. We believe that our protocol minimizes resource utilization in its use of pulse oximetry over polysomnography, while maximizing patient safety and success through the use of capping trials for very young and very small pediatric patients.

To situate the Aboriginal Studies program I am designing, I would need to say that in a Year 11 course in Religious Education at a Catholic Girls’ Independent (non-systemic) High School in Brisbane, the Semester 2 area is Morality and Justice and involves a consideration of personal decision making, understanding stages of moral development, individual/personal moral issues and social, moral and justice issues. (I should add that the school is almost totally non-Aboriginal although it is multi-ethnic to the extent that it runs ESL classes at each year level).

Background: This report utilizes 11 years (2003-2013) of clinical records of patients for a retrospective evaluation of the effectiveness of various combinations of diagnostic methods, treatment options and surgical procedures to try to determine the optimal combinations to improve the rate of success for the treatment of septic wrist arthritis. Methods: Analysis of records of 40 patients treated for septic wrist arthritis in our hospital involving records of physical examination, full blood analysis, biochemical, microbial profiles, type of surgical intervention, length of stay, number of surgical interventions to resolution and the rate of morbidity and mortality. The patients were subdivided into 2 groups, consisting of 6 (mortality) in one group and 34 (non-mortality) in the other. The various parameters as listed above were compared for differences. The patient records included those from immunocompromised elderly patients, with other existing medical complications such as gout, pseudogout, cellulitis and arthritic flare that made accurate diagnosis of septic wrist arthritis challenging. Results: There is a trend showing better success in the treatment of septic wrist arthritis among patients without co-morbid medical problems. Due to the unavoidable delays in microbial identification, it was noted that escalation of antibiotics should be adopted especially for the immunocompromised patients. Conclusions: Staphylococcus aureus, the most common microbial pathogen in our findings, point to the need to adopt a lower threshold for escalation of antibiotics for immunocompromised patients with slow reversal of infection in order to reduce morbidity, mortality, number of surgeries and length of post-operative stay.
