Abstract:

Abstract Background Gastrostomy placement is the preferred means of long-term enteral feeding for patients who cannot eat by mouth. During laparoscopic gastrostomy, it is standard to perform gastropexy, apposing visceral and parietal peritoneum. In some settings, due to altered anatomy from prior surgery, direct apposition of the stomach to the abdominal wall is not possible. This study reports a series of cases where laparoscopic gastrostomy was performed via a Witzel approach without gastropexy. Methods A retrospective chart review was performed of all patients at a tertiary academic medical center who underwent Witzel gastrostomy without gastropexy over a 3-year period. In each case, an 18-French tube was placed into the fundus of the stomach and secured with a purse-string suture. A 5-cm serosalized Witzel tunnel was created around the tube using running silk suture. No gastropexy was performed. Results Six patients underwent 7 Witzel gastrostomy procedures. In three cases, patients had undergone prior major upper abdominal surgery where adhesive disease prevented gastropexy. In the other four cases, the patients had undergone prior gastric bypass with antecolic antegastric position of the roux limb. No patient suffered leak of gastric contents into the peritoneum, and there were no postoperative complications or mortality related to the gastrostomy. Conclusion In cases where enteral access is necessary, and where the stomach cannot reach the anterior abdominal wall for gastropexy due to prior surgeries, a Witzel gastrostomy without gastropexy is a safe option which resulted in no morbidity or mortality in our series.

Abstract:

Background. Acute esophageal necrosis is a rare and potentially lethal entity. The pathogenesis is multifactorial, generally presenting with symptoms of upper gastrointestinal bleeding. We present a case that presents atypically with initial esophageal perforation. Case presentation. A 46-year-old man with a history of alcoholism and cocaine use, an active smoker, and a ruptured celiac trunk aneurysm treated by embolization, who, after acute chest and epigastric pain, is diagnosed with a Stanford B thoracoabdominal aortic dissection, being repaired endovascularly by placing an aortic endoprosthesis. Due to clinical suspicion of mesenteric ischemia complicated with esophageal/gastric perforation, a postoperative tomography was performed, revealing perforation of the esophagus distal to the left pleura and ischemic cholecystitis. Transhiatal esophagectomy, cervical esophagostomy, Witzel-type decompressive gastrostomy, Witzel-type feeding jejunostomy, classic cholecystectomy, and mediastinum drainage were performed. During the postoperative period, the patient remained in critical condition, dying as a result of hypoxic encephalopathy. The histopathological study reported acute transmural esophageal ischemia. Discussion. Tissue hypoperfusion plays a dominant role in the pathogenesis of acute esophageal necrosis. Esophageal perforation is a serious complication and can occur in the early stages, with esophagectomy and deferred digestive reconstruction being the appropriate treatment. Conclusion. Ischemia is a fundamental mechanism of acute esophageal necrosis; its diagnosis must always be established in the various complications that may occur in patients with hemodynamic compromise, in order to obtain a timely treatment.

Abstract:

Гастростомия является вынужденной операцией у больных, страдающих дисфагией. Пациенты не имеют возможности самостоятельно сравнить различные способы гастростомии, поэтому невозможно ориентироваться на их субъективную оценку. Для выбора способа гастростомии необходимы объективные критерии сравнения. Размеры площади стенки желудка, используемые при данном вмешательстве, влияют на объем и деформацию желудка, поэтому могут быть использованы как критерий сравнения. Цель статьи - сравнить различные способы гастростомии по объективным критериям. Для сравнения наиболее распространенных способов данного вмешательства проведен анализ техники выполнения операции. С помощью законов геометрии вычислены используемые для формирования свищей размеры стенки желудка без учета швов для гастропексии. При пункционной гастростомии площадь используемой стенки желудка соответствует площади внутренней прижимной пластинки (бампера). Для гастростомической трубки для чрескожной эндоскопической гастростомии 15 Fr с диаметром внутреннего бампера 2,3 см площадь используемой стенки желудка равна 4,2 см². При использовании для расчетов гастростомической трубки диаметром 1,0 см для гастростомии по Witzel необходимо около 31 см² стенки желудка, а для гастростомии по Stamm-Senn-Kader - 38,5 см² стенки желудка. Пункционные способы гастростомии являются оптимальными для пациентов, не требующих дополнительных вмешательств в брюшной полости. При невозможности пункционной гастростомии целесообразно использовать прижимную гастростомию через минилапаротомию, которая менее травматична, чем традиционные операции. Лапаротомные и лапароскопические гастростомии с формированием свища из стенки желудка показаны при необходимости дополнительных диагностических и лечебных вмешательств в брюшной полости. При этом целесообразнее применять гастростомии по Witzel или хоботкового типа (по Depage или другие), при которых используется передняя стенка желудка меньшей площади и происходит меньшая деформация желудка по сравнению с гастростомией по Stamm-Senn-Kader. Gastrostomy is a forced operation in patients suffering from dysphagia. Patients do not have the opportunity to independently compare different methods of gastrostomy, so it is impossible to focus on their subjective assessment. Objective criteria for comparison are needed to select a gastrostomy technique. The dimensions of the stomach wall area used in gastrostomy affect the volume and deformation of the stomach, and therefore can be used as a comparison criterion. The aim of the article is to compare different methods of gastrostomy according to objective criteria. To compare the most common methods of gastrostomy, an analysis of the operation technique was carried out. Using the laws of geometry, the dimensions of the stomach wall used for the formation of fistulas were calculated without taking into account the sutures for gastropexy. With puncture gastrostomy, the area of the stomach wall used corresponds to the area of the internal pressure plate (bumper). For a 15 Fr percutaneous endoscopic gastrostomy tube with an internal bumper diameter of 2.3 cm, the area of the stomach wall used is 4.2 cm². When using a gastrostomy tube with a diameter of 1.0 cm for calculations, Witzel gastrostomy requires about 31 cm² of the stomach wall, and for Stamm-Senn-Kader gastrostomy - 38.5 cm² of the stomach wall. Puncture methods of gastrostomy are optimal for patients who do not require additional interventions in the abdominal cavity. If puncture gastrostomy is not possible, it is advisable to use pressure gastrostomy through minilaparotomy, which is less traumatic than traditional operations. Laparotomic and laparoscopic gastrostomies with the formation of a fistula from the stomach wall are indicated if additional diagnostic and therapeutic interventions in the abdominal cavity are necessary. In this case, it is more expedient to use gastrostomy according to Witzel or proboscis type (according to Depage or others), in which the anterior wall of the stomach of a smaller area is used and there is less deformation of the stomach compared to gastrostomy according to Stamm-Senn-Kader.

Abstract:

Abstract Daily interpersonal stressors, such as arguments with spouses, are predictors for increased negative affect within the literature (Witzel & Stawski, 2021). Emotion work, or activities done to promote another’s positive emotional state, has been noted to have mixed effects on health and well-being (Umberson et al., 2020) and emotion work is more prevalent in times of increased stress (Rao, 2017). Moreover, with old age, individuals experience fewer daily interpersonal stressors (Witzel & Stawski, 2021). Using the 2015 wave of the Health and Relationships Study (Ncouples=756, Mage=48.22, Rangeage=35-65, Mmaritalduration=8.52; Umberson, 2015), we explored 1.) how daily arguments with spouses within diverse marriages were related to negative affect across 10 days and 2.) whether age and emotion work moderate these associations. HARP is a daily diary study exploring marriage and health in same-sex and different-sex marriages. Preliminary results using multilevel models suggest that individuals’ negative affect significantly increased on days they reported spousal arguments. The interaction between spousal arguments and emotion work was significant (p<.05); on days when individuals reported more emotion work, negative affective reactivity associated with spousal arguments was higher, compared to days when less emotion work was reported. No significant age differences were found. Discussion will focus on the import of spousal arguments and emotion work for shaping daily well-being, and their relevance throughout adulthood, midlife, and older age. Future work should explore how being the recipient of emotion work may buffer or exacerbate the associations between daily arguments and negative affect and potential gendered patterning therein.

Buscando elencar e destrinchar os discursos de uma violência legitimada por parte do governo Witzel desde o início do seu mandato, tecemos uma trajetória do discurso do medo e da violência contra determinados grupos sociais, que na elaboração das situações sociais são estigmatizados e, consequentemente, criminalizados desde o período pós-abolição até os dias atuais. Concluímos que o periódico carioca naturalizou as ações do atirador como necessárias para estabelecer a “ordem”.

Abstract:

A new jejunostomy tube placement technique is described and compared to traditional methods. The interlocking box technique was compared to a simple purse-string with jejunopexy and an inverting serosal tunnel (Witzel technique) with jejunopexy. Procedures performed in fresh canine cadavers were fluid pressure tested following tube removal. Mean+/-standard deviation (SD) intraluminal pressure necessary to induce leakage at the jejunopexy site of the interlocking box group (87.63+/-40.56 cm H2O) was significantly greater (p less than 0.001) than the simple purse-string (43.17+/-31.69 cm H2O) and serosal tunnel (46.33+/-23.60 cm H2O) groups. Wie geht die verbandliche Jugendarbeit mit der Tatsache um, dass die Verbände in der Regel wertegebundene Organisationen sind, dass die Teilnehmerinnen und Teilnehmer heute aber normativ durchaus heterogen sind? Diese Frage wirf Marc Witzel im dritten Beitrag auf. Aktuelle gesellschaftliche Diskurse sehen kulturelle Heterogenität als Normalität. Eine Form des Umgangs mit dieser Heterogenität sei die „interkulturelle Öffnung“ der Verbände, stellt der Autor fest. Der Beitrag stellt – gestützt auf empirische Erkenntnisse – dar, welche Herausforderung sich dadurch für die verbandliche Jugendarbeit ergeben. Gezeigt wird, inwiefern die Auseinandersetzung mit interkultureller Heterogenität als eine Auseinandersetzung um die Teilhabe an einer Gemeinschaft verstanden werden kann. Die Befunde werden am Beispiel eines evangelischen Jugendverbandes diskutiert. Gezeigt wird, wie die partizipative Herstellung von verbandlicher Gemeinschaft im Alltag funktioniert.

Tendo em vista que no debate político-eleitoral o interesse é triunfar sobre o adversário, a imagem de si, prévia ao discurso ou propriamente nele desenvolvida, muitas vezes determina o sucesso da adesão de um auditório e o consequente sucesso de um projeto de dizer empreendido com certa finalidade. A proposta de estudo é analisar a tensão decorrente do jogo de imagens projetadas no debate político-eleitoral/2018, dando enfoque às situações de luta discursiva da etapa de segundo turno das eleições ao cargo de Governador do Estado do Rio de Janeiro. Natural orifice transluminal endoscopic surgery (NOTES) was applied for the first time by K. Witzel in 2008, introducing the transoral endoscopic resection of the thyroid gland in human cadavers and living pigs. In 2016, A. Anuwong published the first case series using the vestibular approach in humans. We report the case of a 37-year-old female patient with a mass in the anterior aspect of the neck which appeared two months before consultation. The thyroid ultrasound reported multinodular goiter. The fine-needle aspiration (FNA) biopsy reported hyperplastic follicular nodule in both lobes. The patient underwent transoral endoscopic thyroidectomy by vestibular approach. Operative time was 180 minutes and blood loss was 20 mL. Pain was minimal and hospital stay was 36 hours. We report our first experience with total thyroidectomy in a patient with benign thyroid nodules.

Die Ökonomisierung und Beschleunigung hat besondere Konsequenzen für die Soziale Arbeit. Rationalisierungstendenzen wirken tief in die Praxis hinein und beeinflussen dabei auch die Art der Erkenntnisbildung Sozialer Arbeit. Ein identifizierendes Denken dominiert die professionellen Hilfen, was zum positivistischen Zwang zur Antwort und zum Verlust offener Fragen führt. Jedoch mahnt uns die Psychoanalyse auch mit dem Unbewussten, dass Ungewissheiten die Arbeit mit Menschen immer begleiten und Momente des Nicht-Wissens sowie des Nicht-Verstehens unumgänglich sind. In seinem Beitrag reflektiert Hauke Witzel die Ungewissheiten Sozialer Arbeit als Negative und diskutiert einen Ansatz, die Grenzen professioneller Praxis nicht aus der unmittelbaren Hilfe auszuschließen. Anhand von Fallbeispielen aus der aufsuchenden Psychoanalyse des Sigmund-Freud-Instituts in einer Erstaufnahmeeinrichtung illustriert der Autor einen selbstreflexiven Zugang zur Praxis, der die Negativität zum Ausgangspunkt eines konkreten beziehungsorientierten Angebots für Geflüchtete werden lässt.

O presente artigo objetiva analisar um possível deslocamento da noção weberiana de Estado Moderno, quanto ao uso e a função da violência, tomando como objeto a gestão de Wilson Witzel, ex-governador do Rio de Janeiro. A partir da análise de seu discurso de posse, discorremos como ele se apropria de uma linguagem profundamente religiosa e utópica para ascender ao cargo, sem apresentar um plano político técnico-burocrático, especialmente no que tange a segurança pública. O uso da linguagem religiosa e da imagem de servo de Deus à serviço dos “cidadãos de bem”, operacionalizada de forma violenta e a partir de fundamentos neoliberais, acionados como justificativas das políticas implementadas, operaram como deslocamentos daquilo que conhecemos como Estado Moderno e o uso e função da violência, vitimando principalmente os pobres, no território favelizado. O método utilizado para o desenvolvimento deste conteúdo foi pesquisa bibliográfica.

Zusammenfassung. Sportler und Sportlerinnen, die ihre gewohnten Leistungen nicht in den Wettkampf transportieren können, werden in der Sportwelt als Trainingsweltmeister bezeichnet. Die vorliegende empirische Studie untersucht das Phänomen des Trainingsweltmeisters aus Sicht der betroffenen Athleten. Mittels des problemzentrierten Interviews nach Witzel (2000 ) wurden sechs Leistungssportler fünf unterschiedlicher Sportarten (Bogenschießen, Schwimmen, Judo, Handball, Wasserspringen) auf Bundes- und Landeskaderniveau befragt. Die Auswertung der Interviews erfolgte anhand einer Kombination von qualitativen Auswertungsmethoden und mündete in einem gegenstandsbegründeten Modell zum Trainingsweltmeister, unter anderem wurden Kodierverfahren der Grounded Theory verwendet. Die Ergebnisse zeigen, dass Trainingsweltmeister in der Wettkampfsituation spezifische Ängste empfinden, die mit einem ausgeprägten Bedürfnis nach Bindung zusammenhängen. Dieses Bindungsbedürfnis steht im Konflikt mit ihrer eigentlichen Aufgabenstellung, andere im Wettkampf zu besiegen. Wird diesem Konflikt und den spezifischen Bedürfnissen der Sportler vom Umfeld nicht Rechnung getragen, führen die Ängste im Wettkampf zu einer Diskrepanz zwischen Trainings- und Wettkampfleistungen.

Zusammenfassung: Zur Analyse und zum Vergleich der in Witzen vermittelten Bilder der Geschlechter wurden 453 Frauenwitze, 586 Blondinenwitze und 858 Männerwitze nach den Prinzipien der qualitativen Inhaltsanalyse interpretiert und kategorisiert. Das Witzmaterial wurde neueren Buchveröffentlichungen sowie Witzsammlungen aus dem Internet entnommen, wobei nur Witze berücksichtigt wurden, in denen jemand ausschließlich aufgrund seiner Geschlechtszugehörigkeit zum Witzobjekt wird. Als auffälligster Befund konnte erhoben werden, dass in den Witzen über Frauen (insbesondere in den Blondinenwitzen) das Männerwunschbild einer attraktiven, sexuell potenten und willigen Frau entworfen wird, während in den Witzen über Männer der Mann als überflüssiges und sexuell impotentes Wesen mit kümmerlichen Sexualorganen erscheint. Diskutiert wird der Bezug zur Geschlechtsstereotypforschung.

Complete gastroduodenal transaction following blunt abdominal trauma in a road side accident is a rare and potentially life-threatening condition. The course of management and choice of surgical intervention depends on clinical presentation; site, size, nature of defect; amount of tissue and blood loss. In this case report, the clinical presentation, diagnostic evaluation, surgical management and postoperative course of an 18-year adult male patient who presented to the emergency room with complaints of generalized abdominal pain, vomiting and signs of peritonitis following a road side accident suggesting bowel injury. The patient was diagnosed with complete gastroduodenal transaction following blunt abdominal trauma based on radiological imaging and intraoperative findings. The patient underwent emergency laparotomy and primary repair of gastroduodenal transaction with Witzel jejunostomy for better outcome. This case report underscores the early recognition, diagnosis and prompt surgical intervention in cases of complete gastroduodenal transaction following blunt abdominal trauma to achieve favourable patient outcomes.

Der Beitrag berichtet Ergebnisse einer empirischen Studie zum Berufseinstieg von Erzieherinnen und Kindheitspädagoginnen in das Arbeitsfeld Kita. Empirische Erkenntnisse hierzu erscheinen im Sinne einer gelingenden beruflichen Einmündung beider Berufsgruppen bedeutsam. Sie geben zudem darüber Auskunft, inwieweit sich die neue Berufsgruppe der Kindheitspädagoginnen im noch weitgehend akademikerfreien Arbeitsfeld der Kindertageseinrichtungen etablieren kann und will. Untersucht wird aus vergleichender Perspektive, wie Erzieherinnen und Kindheitspädagoginnen mit und ohne Erzieherausbildung ihre Berufswahl begründen, wie sie ihren Berufseinstieg nach eigener Einschätzung subjektiv bewältigt haben und wie sie ihn im Nachhinein bewerten. Die Datenbasis bilden 28 leitfadengestützte Interviews. Basierend auf dem ARB-Modell nach Witzel (2001), das zwischen Aspirationen, Realisationen und Bilanzierungen unterscheidet, wurden die Aussagen der Berufseinsteigerinnen mittels fallweiser inhaltsanalytischer Auswertung und anschließender quantitativer Clusterbildung zu Typologien verdichtet. Ermittelt wurden spezifische Begründungs-, Bewältigungs- und Bewertungstypologien mit je unterschiedlichen Verteilungen der drei Berufsgruppen. Die Ergebnisse werden vor dem Hintergrund einer noch ausstehenden Etablierung der relativ neuen Berufsgruppe der Kindheitspädagoginnen im Arbeitsfeld Kita diskutiert.

Introduction: Feeding jejunostomy is a rather safe procedure. Major complications are rare, but can be serious, nevertheless. Case Report: We report a case of a 57-year-old male, diagnosed with epidermoid carcinoma of the larynx. The patient received 3 cures of induction chemotherapy before being lost to follow-up. One year later, the patient presented with significant dysphagia and benefited from a feeding jejunostomy utilizing the Witzel technique. Postoperative course was marked by abdominal tenderness, and hemodynamic and respiratory impairment. Lab tests showed a continuous elevation of markers of infection. Computed tomography (CT) scan revealed an extradigestive distal end of the jejunostomy tube, associated with a voluminous pneumoperitoneum, predominant around the tube end, keeping with a small bowel perforation. The evolution was marked by respiratory fatigue, and hemodynamic instability. The patient, unfortunately died subsequently, despite maximal supportive measures. Conclusion: Computed tomography (CT) scan is a helpful tool for the diagnosis of feeding jejunostomy’s major complications besides physical examination and biology. Limiting the indications of this surgical procedure and being attentive to surgical details may help reduce the risk of complications.

As a result of the global interest of the cultural heritage sites lay great significance on the residents' role in safeguarding the cultural heritage. Due to this interest, the importance of safeguarding the cultural heritage sites become the most important element of the development plans. The purpose of this paper is to explore the importance of safeguarding the cultural heritage sites, the essential role of local resident and the government to safeguard their heritage and culture. Also, to review the perceptions and opinions of 40 respondents living in the investigated site, who were raised by the presence of these cultural sites, through the method of problem-centred interviews according to witzel (1982), which can summarize as: brief questionnaire, dialogue guideline, audio recording, and postscript. The results of the study highlighted the crucial need of involving and participation the local residents in safeguarding their cultural heritage. Besides, the need for greater public awareness and education in safeguarding cultural heritage, the government should involve the local residents in the planning and implementation projects which attend to safeguard the city’s cultural heritage.

Zusammenfassung Zielsetzung In der vorgestellten explorativen Datenerhebung werden der Bedarf und mögliche Herausforderungen hinsichtlich eines strukturierten Ausfallmanagements in einem Klinikverbund erläutert, was eine Alternative zur Nutzung von Leiharbeit darstellen kann. Methodik Zur Erhebung der Daten wurden leitfadengestützte Interviews nach Witzel (1985) mit verschiedenen Stakeholdern geführt. Anschließend wurden diese nach der qualitativen Inhaltsanalyse nach Mayring (2016) ausgewertet. Ergebnisse Berufsgruppend übergreifend wurde große Skepsis gegenüber der Forschungsidee eines strukturierten Ausfallkonzepts in einem Klinikverbund geäußert. Es wurde ein Bedarf gesehen, dass die Gesamtanzahl von Pflegekräften erhöht werden muss, um eine Patientenbetreuung 24 Stunden gewährleisten zu können. Darüber hinaus wurden zahlreiche Herausforderungen beschrieben. Schlussfolgerung Die benannten Herausforderungen spiegeln die aktuelle Situation im Hinblick auf den demografischen Wandel, den Fachkräftemangel und der bestehenden Unzufriedenheit verschiedener Berufsgruppen im deutschen Gesundheitswesen wider. Positive Effekte der Zusammenarbeit in Klinikverbünden wurden auf Grund etablierter Wettbewerbsgedanken nicht erkannt. Ebenso die Möglichkeit mit Hilfe von Organisations- und Personalentwicklungsmaßnahmen oder einem passgenauen Change Management, die Situation in der Pflege zu verbessern. Es zeigt sich, dass ein Generationsmanagement, Kommunikation und Transparenz sowie Innovationsbestreben fehlen, jedoch unerlässlich sind.

AbstractIntraday variability in compact flat-spectrum radio sources has been intensively studied in recent years. For most IDV events the apparent brightness temperatures derived from the observed timescales are in the range of Tb,app ~ 1016−18 K. For extremely rapid variations, Tb,app can reach up to ~ 1021 K (e.g. Kedziora-Chudczer et al., 1997). Refractive interstellar scintillation may be the most likely extrinsic mechanism (Rickett et al., 1995; Qian, 1994a; Qian, 1994b). Especially for the case of extreme Tb,app (> 1018 K) RISS may be dominant (Dennet-Thorpe and de Bruyn, 2000). However, some IDV events with Tapp ~ 1017−18 K show evidence for an intrinsic origin e.g. the correlated radio-optical intraday variations observed in the BL Lac object 0716+714 (Wagner and Witzel, 1995, Qian et al, 1996). It seems important to distinguish between IDV which is a phenomenon intrinsic to the compact radio sources and IDV which is primarily due to RISS. Multifrequency polarization and VLB I observations would be most helpful (Gabuzda and Kochanev, 1997).
