Zusammenfassung Ziel: In der vorliegenden Pilotstudie wurde untersucht, ob charakteristische Veränderungen der Dopamintransportfunktion und der D2-Bin-dungskapazität bei Patienten mit der klinischen Diagnose eines idiopathischen Parkinson-Syndroms (IPS) mit der SPET nachgewiesen werden können und ob sich diese Veränderungen von jenen unterscheiden, die mit sekundärem Parkinson-Syndrom (SPS) einhergehen. Methode: Bei 16 Patienten wurden SPET-Studien (Doppelkopfkamera) sowohl mit I-123-ß-CIT als auch mit I-123-IBZM 20-24 h p.i. bzw. 2 h p.i. durchgeführt (darunter 6 Pt. mit IPS, 6 Pt. mit SPS außer M. Wilson und 4 Pt. mit M. Wilson). Die Auswertung der ß-CIT-Studien erfolgte auf der Grundlage der Aktivitätsrelationen zwischen Basalganglien (BG) und Cerebellum (CER), die der IBZM-Studien unter Verwendung von BG und medialem frontalen Kortex (MFC). Ergebnisse: Die Relation BG/CER für 1-123-ß-CIT betrug 3,04 ± 0,83 bei IPS und 7,73 ± 3,28 bei SPS ohne Morbus Wilson (p< 0,05). Die Relation BG/MFC für I-123-IBZM war bei IPS jeweils kontralateral zur Symptomatik erhöht (MW = 1,75 ± 0,12 gegenüber 1,61 ± 0,16). Die entsprechenden Werte bei SPS waren gegenüber denen bei IPS nur mäßig, aber signifikant vermindert (1,58 ± 0,24 vs. 1,68 ± 0,16). Bei den Patienten mit M. Wilson war die Relation BG/MFC für IBZM zum Teil erheblich vermindert (bis 1,29), während die I-123-ß-CIT-Aufnahme mit BG/CER-Relationen von 8,00 ± 2,90 gegenüber der von Patienten mit SPS (p < 0,01) nur gering reduziert war. Schlußfolgerung: Es wird gefolgert, daß die beiden verwendeten Radiopharmaka zwar tendenziell die bei den untersuchten Krankheitsbildern erwarteten neurochemischen Veränderungen darzustellen in der Lage sind, daß aber eine Differenzierung zwischen IPS und SPS eher mit I-123-ß-CIT gelingt als mit I-123-IBZM.

ABSTRACT Times of higher paleolake levels in Mono Lake basin correspond to higher abundances of authigenic minerals such as calcite and Mg-smectite in the Wilson Creek Formation, the lake sediments exposed around the modern lake that represent the persistent wetter conditions of the last glacial cycle. It has been suggested that precipitation of these minerals in Mono Lake is controlled by the flux of water (surface and ground), which replenishes Ca2+ and Mg2+ ions in the lake. This water is subsequently depleted due to the high rates of evaporation in the Mono Basin, resulting in precipitation of calcite and Mg-smectite mineral phases. Thermodynamic evaporation models starting with Sierra Nevada spring water can simulate the chemical composition of Mono Lake remarkably well. These models do not, however, consider the mixing of freshwaters in the lake that is hypothesized to result in precipitation of calcite and Mg-smectite. Here, we present the results of our empirical evaporation and mixing (E&M) model using simple thermodynamic approaches. Although this model is highly simplified, it provides a valuable test of the hypothesis.

Chapter two tells the story of how the Allies began establishing a theater of war in North Africa. Shipping space was limited, so the Allies began establishing vehicle assembly plants in North Africa, a remarkable story. The Allies established a formal communications zone and began experimenting with various approaches to command and control, particularly for theater administrative support. Allied Force Headquarters (AFHQ) established three base sections under the command of BG Arthur Wilson, MG Tom Larkin, and BG Arthur Pence. MG Everett Hughes served as Deputy Theater Commander. The Services of Supply was critically short of manpower, a situation exasperated be the inclusion of free French forces. The rearming of ~300,000 Free French forces was a major undertaking with political ramifications. The chapter concludes with a description of British and German logistics in North Africa. Rommel had his own supply challenges, to include insufficient supplies, shipping interdictions, and lack of an effective theater distribution system.

Het is druk op het voetbalcomplex van Magreb ’90. De twee voetbalvelden en de kantine liggen er mooi bij. Naast de kantine staat een hele grote tent waar we kunnen aanschuiven voor het ontbijt. Dat zijn we niet gewend bij Nederlandse verenigingen, maar gezellig is het wel. Het wemelt van de kinderen, ouders en trainers. Het is een bonte verzameling van spelers: van de blonde spelers van JSV en Houten, tot de Marokkaanse jeugd van vv Hoograven, VVOO en Magreb zelf. De organisatoren van Magreb, die pas sinds vijf jaar jeugdelftallen hebben, voelen zich trots dat al deze clubs wilden komen. Het toernooi verloopt voorspoedig, alles loopt op rolletjes. De kinderen genieten. Aan het eind van de dag zijn de organisatoren zeer tevreden. De dag is prima verlopen. Winst en verlies op het veld waren belangrijk. Maar de belangrijkste winst is deze: ‘Ook de Nederlandse mensen waren enthousiast’, zegt de leiding van het toernooi. En daar mag je als ‘Marokkaanse’ vereniging best trots op zijn. Het doet mij denken aan de uitspraak van de Marokkaanse kinderen van Hoograven als er iets fout gaat, zoals het niet aanwezig zijn van ouders voor het vervoer. ‘Dit is een neppe vereniging.’ Voor organiseren geldt soms een Nederlandse maat.

Abstract:
Introduction: Wilson’s disease (WD) is a rare autosomal recessive disorder resulting in hepatic and neurological symptoms. Uncommon presentations are prone to diagnostic and treatment delay. Objectives: Describe an atypical case of WD and improve clinicians awareness. Case report: A 19 years-old male presented to us with seizures and upper limb tremor in the past three years, evolving to torsion-like movements in limbs and trunk. He was unable to open his eyes and had severe dysphagia. There was no consanguinity, perinatal complications or neurodevelopmental delay. Examination revealed generalized dystonia, blepharospasm, retrocollis, dysarthria, trismus, right hemiparesis and marked muscle wasting; there was a gray halo around both irises. The patient was bedridden and severely dependent. Magnetic resonance imaging (MRI) showed T2/FLAIR (T2-weightedFluid-Attenuated Inversion Recovery) hyperintense lesions in the left hemisphere, basal ganglia and cerebellar peduncles. Laboratory workup revealed low ceruloplasmin levels (< 8 mg/dL; range: 16–45), serum copper of 44.2 mcg/dL (range: 70–140). A diffuse increased echogenicity was described in liver ultrasound. The patient had no clinical or laboratorial evidence of liver damage. Due to presumed Kayser-Fleisher rings, movement disorders and MRI findings, WD was suspected. Genetic test revealed two variants in the ATB7B gene, probably a compound heterozygosity. Having a final Leipzig score of 7, WD diagnosis was established. Conclusion: WD may present with cortico-subcortical involvement. Hepatic subclinical involvement may delay WD diagnosis. It is important to have a high index of suspicion for WD in any movement disorder in young people. In order to prevent treatment delay and severe neurologic sequel, neurologists should be aware about atypical clinical and radiological presentations of this treatable disease.
