Abstract:
This body of work sought to explore kinematic impairments which may underlie falls incidences during turning following stroke and review the evidence for the effectiveness of interventions aimed at improving aspects of locomotor coordination which are key to controlling turning. A systematic review of the literature identified insufficient homogeneity of high quality evidence to determine if task specific locomotor practice interventions are effective in improving aspects of gait coordination which are key to the controlling turning. The review highlighted a need for a better understanding of the nature of coordination deficits in functional walking tasks, such as turning, after stroke. In order to provide a base of knowledge regarding abnormalities in the coordination of locomotor patterns during turning while walking, two experimental studies were undertaken. The studies employed analysis of full-body kinematics during turns made under pre-planned and reactive conditions as well as turns of different magnitudes and those made by participants with and without a falls history. Findings from Study 1 showed a strong trend for participants with stroke (in particular those with lesions involving the basal ganglia) to initiate pre-planned turns later than their age-match counterpart. Turns made in response to an external cue were made in a similar manner to healthy controls. Results from study 2 indicate that while participants with stroke and falls history took significantly longer to turn, all other aspects of the movement pattern were similar to healthy controls and non-fallers. Therefore, incidences of falls during turning following stroke may not be due to impaired movement patterns alone. On this basis, we suggest that rehabilitation efforts and further studies should address the interplay of impaired movement production with other factors such as attention.

Abstract:
The emergence of increasingly complex problems, combined with growing concerns for the environment, is fuelling the demand for more innovative and sustainable products, services and systems. Whole system design is one approach that aims to integrate social, economic and environmental phenomena into a comprehensive design solution. The approach encourages the development of partnerships between actors from a variety of different backgrounds, disciplines and sectors to develop an innovative, sustainable and optimised solution at a whole system level. However, there is limited research concerning the integrative process that actors are required to follow in order to reach such a solution. The aim of this study was to gain improved knowledge and understanding surrounding the process of whole system design and to identify those factors that influence its success. This was achieved in two phases; firstly a longitudinal case study was undertaken which followed the process of whole system design from beginning to end. 22 design and progress meetings were observed, 18 interviews were carried out and a multitude of relevant documentation was analysed. This resulted in the identification of 10 themes. The second phase of the research aimed to validate initial findings by conducting 5 smaller cases and interviewing 11 individually selected experts. The study ultimately produced 8 confirmed themes, 68 individual findings and 37 factors that enable and inhibit the process of whole system design. As a result of this study, an improved knowledge and understanding surrounding the process of whole system design has been presented. In particular, findings have been provided concerning: the development of partnerships, the pertinence of human and non-human interaction, the requirement of individual characteristics, enhanced understanding of purpose and process, the necessary alignment of individual and organisational motivation, the necessity of sense making activities, the role of a facilitator and the need for integration, each of these within the context of whole system design. The framework of these findings provides a novel contribution to knowledge within the context of whole system design.

The health insurance has become complementary to the National Health Care system in Portugal. In the last years, the increase of this insurance has been considerable. Despite the health concerns of Portuguese citizens, related to better life quality, medical technology and others, the ageing of Portuguese population is a reality to be well thought-out. Regarding this fact, the whole life health insurance is an important product to be developed. In this dissertation, it is presented an approach to the calculation of the level premiums for the whole life health insurance in order to fulfil the Portuguese insurer’s market requests. A private health insurance company with a historical data of ten years provided the statistics used for this calculation. The levelled insurance premiums were calculated on the basis of the risk involved and according to the principle of equivalence. This means that regarding the period insured, the total of premiums should match the total of the benefits.

Abstract:
Intensiv träning resulterar iskada i skelettmuskulaturen. Symptomen som idrottare uppvisar efter träning är svullnad, smärta och nedsatt muskelfunktion. Användningen av Whole Body Cryotherapy(WBC) har uppmärksammats och används för att återhämta sig från de symptom som orsakas vid Delayed Onset Muscle Soreness(DOMS). Syftet med denna litteraturstudie var att se vilken effekt WBC har på DOMS. Litteratursökningen ägde rum i PubMed, Cinahl och Google Scholars databaser. Utfallsvariabler som testade för DOMS var smärta registrerat med Visual Analog Scale(VAS), Maximal genererad kraftkapacitet(MKK) och blodprover som testade för inflammatoriska ämnen CreatinCinase(CK) och inflammatoriska cytokiner. Litteratursökningen resulterade i nio artiklar. Fem av de totalt nio studierna som hade smärta som en utfallsvariabel visade en minskad smärta för WBC-guppen jämfört med kontrollgruppen. Studier som undersökte MKK var motsägelsefulla. Tre av sex studier som undersökte MKK visade en signifikant skillnad på WBC gruppen, samtidigt visade de tre resterande studierna ingen effekt alls. Studier som haft CK och inflammatoriska cytokiner som utfallsvariabel visade ingen effekt. Sammanfattningsvis visade det sig att WBC har liten och ingen effekt på DOMS. Området saknar fortfarande forskning och detta område behöver undersökas mer innan WBC kan rekommenderas som en effektiv återhämtningsmetod mot DOMS, och hittills har metoden visar liten eller ingen effekt på dessa utfallsvariabler
