Full textCD4 T-cells are necessary for the establishment of an efficient anti-cancer response by providing help for the priming and persistence of tumor-specific cytotoxic T lymphocytes. However, the prognosis of patients can be differently affected, depending on CD4 T-cell subtypes infiltrating the tumor (TIL). Epigenetics take an important part in : i) the regulation of CD4 T-cells polarization, ii) plasticity towards other subsets and iii) maturation. It has been shown before that epigenetic modulators could regulate CD4 T-cells differentiation. Thus, the treatment of CD4 T-cells by epigenetic therapies can be used as a strategy to improve immunotherapy. Our results confirmed the presence of Th17-cells in expanded T-cells issued from liver metastases of colorectal cancer. Moreover, the number of expanded Th17-cells among total TIL was inversely correlated with patients' survival. We then attempt to evaluate the effect of epigenetic regulators on both CD4 T-cells differentiation and function. A screening of a bank of epidrugs allowed us to identify a Sirtuin 1 activator, Agrimol B, that might downregulate proliferation, cytokine secretion and CCR6 expression on CD4 T-cells. Our results thus indicated that Agrimol B might regulate the migration and the functionality of Th17-cells from healthy donors. Mechanistically, the inhibition of caspases by our molecule could prevent the cleavage of Sirtuin 1 and thus maintain its activity compared to control conditions. Together, this work aims to uncover the role of epigenetics in the regulation of CD4 T-cell subsets in order to potentiate the effectiveness of cancer immunotherapy

Full textChronic myeloid leukemia (CML) is a myeloproliferative disorder and is characterized by the presence of a Philadelphia chromosome (Phl) encoding the BCR-ABL fusion protein with constitutive tyrosine kinase activity. The treatment by Tyrosine Kinase lnhibitors (ITK) is not curative. The problem today is ta target leukemic stem cells (HSCs) to prevent relapse of patients after stopping treatment with TKI and permanently eradicate the disease. Studies have identified the interleukin-1 accessory protein receptor (IL-lRAP) as a potential target for killing CSL (BCR-ABL1 positive) in CML or in acute myeloid leukemia (AML). We therefore propose to develop a cell therapy approach targeting IL-lRAP, using T cells (LT) redirected by a CAR (Chimeric Antigen Receptor) in the treatment of CML and AML. We produced a specific anti-lL-1RAP murine monoclonal antibody (mAb) called # A3C3. We tested the specificity of the antibody # A3C3 in flow cytometry, in Western blot, by immunohistochemistry or confocal microcopy, on positive human cell lines IL-1RAP (KU812) or IL-1RAP negative (Raji). Moreover, by using this antibody, we have demonstrated by immunohistochemistry, on 20 different normal tissues that our antibody does not recognize or very few non-specific (off-target) targets. Finally, we demonstrated that this antibody is able to detect positive IL-1RAP CSLs in primary bone marrow or peripheral blood samples of LMC patients at diagnosis or after ITK treatment. By molecular biology, we have sequenced and identified rearrangements of the heavy (lgH) and light {lgL) chains coding region for hypervariable regions of# A3C3. The coding sequence for the "single chain variable fragment" (scFv), lin king the 2 lgH and lgl chains, was cloned into a 3rd generation lentiviral skeleton securised by a suicide gene, inducible caspase 9 (iCASP9). We produced CART-cells and LT-mock ex-vivo. The transduction efficiency is measured by CD3 + / CD19 + expression (82% on average, n = 5), in flow cytometry. The functionality of the suicide cassette iCaspase9/AP1903 on CART-cells after 48h treatment with AP1903 was tested. We have shown the death of more than 90% of CART-cells (7-AAD labeling in flow cytometry). We then performed in-vitro functional tests of CART-cells against LMC lines naturally expressing IL-1RAP or against transfected lines to express the membrane form of IL-1RAP. After co-culture of CART-cells in the presence of IL-1RAP + target cells, we have shown that they proliferate (CFSE test), that they secrete interferon y and inflammatory cytokines (intracellular labeling and Cytokines Bead Array assay). They are able to degranulate (CD107a & b labeling). Finally, by 7 AAD labeling, we demonstrated the cytotoxicity of CART-cells against IL1-RAP + cells. Finally, we showed the effectiveness of CART-cells in vivo, in a tumor xenograft model in NSG immunodeficiency mice engrafted by the K562-IL1RAP+/Luciferase+ line. There is a decrease in the tumor load 4 days after injection of CART-cells, until complete disappearance.This proof of concept suggests alternative therapeutic perspectives in the treatment of CML or AML, which should be optimized in murine models (sequence and injection numbers, cell numbers, associations with ITKS or other chemotherapies) in order to Phase I clinical trial, to demonstrate the feasibility and safety of the approach to consider a demonstration of efficacy in human. Securing by the suicide cassette makes it possible to consider the use of CART-cells in an autologous or allogeneic situation (Donor Lymphocytes infusion, DU)

Full textRecent advances in immunology have now validated the concept of cancer immunosurveillance and the leading role of adaptative T cell immunity. Until a few years ago, antitumor CD8 T cell responses have been the most studied due to their direct cytotoxic activity on tumor cells. On the other hand, study of antitumor CD4 T cell responses are even more challenging because of the heterogeneity and plasticity of the various CD4 T cells subpopulations described. Among them, CD4 T helper type-1 cells (Th1), mainly characterized by the production of IFN, control the activation of antitumor cellular immunity. Thus, stimulation of specific CD4 Th1 cells may have a major interest for the development of anticancer immunotherapies. During this research thesis, we characterized novel HLA class II epitopes derived from a relevant tumor antigen, telomerase (TERT), and studied their capacities to stimulate specific CD4 Th1 cell responses. Using a method based on predictive immunology, we identified 4 peptides derived from TERT, referred as « Universal Cancer Peptides » (UCPs), enable to bind the most commonly found HLA-DR alleles in human. Using HLA-A2/HLA-DR1 transgenic mouse model, we first evaluated the in vivo immunogenicity of these peptides. Immunization of mice with UCPs induces high avidity specific CD4 T cells. The study of their polarization showed that UCP-specific CD4 T cells do not produce IL-4, -5, -10 or -17 cytokines, excluding a Th1, Treg or Th17 differentiation. In contrast, we measured high amount of IFN and IL-2 which characterize a Th1 pattern. The study of helper role allow us to demonstrate that CD8 peptide-based vaccinations in presence of UCPs enhance the efficacy of tumor specific CTL responses. Indeed, the intensity of these responses is strongly correlated with that of UCP-specific CD4 T cells induced in vivo. Furthermore, the stimulation of UCP-specific CD4 T cells promotes activation and IL-12 release by dendritic cells through a mechanism that involves IFN, GM-CSF and CD40L. We also demonstrated the antitumor efficacy of UCPs during a therapeutic vaccination in mice, as well as their capacity to foster the recruitment of specific CD8 T cells at the tumor site. In addition, the presence of naturally occurring UCP-specific CD4 T cell responses was found in different types of cancers such as leukemia, lung, colorectal or renal cancers. A study conducted in a cohort of 84 metastatic lung cancer patients revealed a synergistic effect of spontaneous UCP-specific CD4 Th1 and chemotherapy-treatment. Altogether, this study provides further evidences that stimulation of antitumor CD4 Th1 cells is a powerful method to improve cancer vaccines and also highlights the interest of TERT-derived UCPs for the innovative monitoring of antitumor CD4 T cell responses
