Abstract:
Ten seminal papers on disorders of the neuromuscular junction are described, covering historical aspects, recent advances in immunological, biological, and genetic researches, and future perspectives. Early descriptions of myasthenia gravis (MG) date back to the seventeenth century, and MG acquired its name in the nineteenth century. The first symptomatic treatment with cholinesterase inhibitors was reported in 1934, leading to the development of modern immunological therapies. Following the discovery of anti-MuSK (muscle-specific tyrosine kinase) antibody in 2001, MG is currently classified into three categories: AChR-positive, MuSK-positive, and dual-seronegative. Lambert-Eaton myasthenic syndrome was recognized in 1956, followed by the discovery of antibodies to voltage-gated calcium channels in the pre-synaptic membrane, facilitating diagnosis and improving the understanding of the pathophysiological mechanisms. Since the late twentieth century, many types of congenital myasthenic syndromes with pre-synaptic, synaptic, and post-synaptic defects have been identified, and a classification based on molecular genetics is in evolution.

Abstract:
This article examines developments in electromagnetism and field physics during the early nineteenth century, when electricity had become a fully respected area of research. It begins with a discussion of the ‘Volta’s pile’, an apparatus developed by Alessandro Volta, along with mathematical approaches to electricity and Hans-Christian Ørsted’s discovery of electromagnetism. It then reviews the work of André-Marie Ampère and the Biot–Savart law, introduced by Jean Baptiste Biot and Félix Savart; developments in electrodynamics during the period 1821–1826; and Michael Faraday’s research initiative in electromagnetism, and especially electromagnetic induction and the electrotonic state. It also looks at three important developments in electromagnetism during the 1820s: galvanometers, electromagnets, and Arago’s effect. Finally, it describes Lenz’s law, electromagnetic generators, the electromagnetic telegraph, the Faraday effect, diamagnetism, and the question of polarity as well as the role of mathematics in Faraday’s theories.

Abstract:
Arrhythmogenic cardiomyopathy is a heart muscle disease clinically characterized by life-threatening ventricular arrhythmias and pathologically by an acquired and progressive dystrophy of the ventricular myocardium with fibrofatty replacement. The clinical manifestations of arrhythmogenic cardiomyopathy vary according to the ‘phenotypic’ stage of the underlying disease process. Since there is no ‘gold standard’ to reach the diagnosis of arrhythmogenic cardiomyopathy, multiple categories of diagnostic information have been combined. Different diagnostic categories include right ventricular morphofunctional abnormalities (by echocardiography and/or angiography and/or cardiovascular magnetic resonance imaging), histopathological features on endomyocardial biopsy, electrocardiogram, arrhythmias, and family history, including genetics. The diagnostic criteria were revised in 2010 to improve diagnostic sensitivity, but with the important prerequisite of maintaining diagnostic specificity. Quantitative parameters have been put forward and abnormalities are defined based on the comparison with normal subject data. A definite diagnosis of arrhythmogenic cardiomyopathy is achieved when two major, or one major and two minor, or four minor criteria from different categories are met. The main differential diagnoses are idiopathic right ventricular outflow tract tachycardia, myocarditis, sarcoidosis, dilated cardiomyopathy, right ventricular infarction, congenital heart diseases with right ventricular overload, and athlete’s heart. Among diagnostic tools, contrast-enhanced cardiovascular magnetic resonance is playing a major role in detecting subepicardial-midmural left ventricular free wall involvement, even preceding morphofunctional abnormalities. Moreover, electroanatomical mapping is an invasive tool able to detect early right ventricular free wall involvement in terms of low-voltage areas. Both techniques are increasingly used in the diagnostic work-up although are not yet part of diagnostic criteria.

Voltaire was a poet first and foremost, in his own eyes as well as in the eyes of his contemporaries, and throughout his long life he never stopped writing verse. ‘The Epicurean poet’ explains how Epicureanism shaped Voltaire’s thinking and how he took a paradoxical stance in the Quarrel of the Ancients and Moderns. It describes his early epic poem La Henriade and his libertine verse such as La Pucelle, a bawdy mock epic poem about Joan of Arc. It also discusses his poem Stances à Mme Du Châtelet, composed in 1741, which typifies Voltaire’s lyric voice at its most persuasive.
