Abstract:

Introduction Synthetic bone grafts are often used to achieve a well-consolidated fusion mass in spinal fusion procedures. These bone grafts function as scaffolds, and ideally support cell function and facilitate protein binding. Objective The aim was to characterize an electrospun, synthetic bone void filler (Reb) for its bone morphogenetic protein (BMP)-2 release properties and support of human mesenchymal stem cell (hMSC) function in vitro, and its efficacy in promoting BMP-2-/bone marrow aspirate-(BMA)-mediated posterolateral spinal fusion (PLF) in vivo. Methods BMP-2 release kinetics from Reb versus standard absorbable collagen sponge (ACS) was determined. hMSC adhesion and proliferation on Reb was tested using cell counting, fluorescence microscopy and MTS. Cell osteogenic differentiation was quantified via cellular alkaline phosphatase (ALP) activity. For in vivo analysis, 18 Lewis rats were treated during PLF surgery with the following groups: (I) Reb + BMA, (II) Reb + BMA + BMP-2 and (III) BMA. A safe, minimally effective dose of BMP-2 was used. Fusion consolidation was followed for 3 months using radiography and micro-CT. After sacrifice, fusion rate and biomechanical stiffness was determined using manual palpation, biomechanical tests and histology. Results In vitro, BMP-2 release kinetics were similar between Reb versus ACS. MSC proliferation and differentiation were increased in the presence of Reb. At 3 months post-surgery, fusion rates were 29% (group I), 100% (group II), and 0% (group III). Biomechanical stiffness was higher in group II versus I. Micro-CT showed an increased bone volume and connectivity density in group II. Trabecular thickness was increased in group I versus II. H&E staining showed newly formed bone in group II only. Conclusions Reb possesses a high protein binding affinity and promotes hMSC function. Combination with BMA and minimal dose BMP-2 allowed for 100% bone fusion in vivo. This data suggests that a minimally effective dose of BMP-2 can be used when combined with Reb.

Abstract:

The aim of this study was to compare the bone regeneration ability of particle and block bones, acting as bone scaffolds, with recombinant human bone morphogenetic protein (rhBMP)-2 and evaluate them as rhBMP-2 carriers. Demineralized bovine bone particles, blocks, and rhBMP-2 were grafted into the subperiosteal space of a rat calvarial bone, and the rats were randomly divided into four groups: particle, block, P (particle)+BMP, and B (block)+BMP groups. The bone volume of the B+BMP group was significantly higher than that of the other groups (p < 0.00), with no significant difference in bone mineral density. The average adipose tissue volume of the B+BMP group was higher than that of the P+BMP group, although the difference was not significant. Adipose tissue formation was observed in the rhBMP-2 application group. Histologically, the particle and B+BMP groups showed higher formation of a new bone. However, adipose tissue and void spaces were also formed, especially in the B+BMP group. Hence, despite the formation of a large central void space, rhBMP-2 could be effectively used with block bone scaffolds and showed excellent new bone formation. Further studies are required to evaluate the changes in adipose tissue.

Abstract:

Les bétons à hautes performances (BHP) sont des bétons qui présentent des résistances mécaniques élevées (> 60 MPa) et une micro texture très dense (faible porosité), qui empêche la pénétration des agents agressifs. Dans le contexte de ce travail une étude expérimentale a été faite à fin d’élaborer un béton BHP à base des matériaux locaux disponible dans la région de Béchar, dont le but de réduire le cout de BHP. Le deuxième objectif de ce travail est de voir l’influence du rapport E/C (0.3, 0.35, 0.4) sur le comportement rhéologique et physico-mécanique (la maniabilité, porosité, résistances à la compression, résistance à la flexion, l’absorption d’eau et la masse volumique) de BHP élaboré à base de pouzzolane naturelle de la région de Boukais (Bechar), et voir aussi l’effet de la pouzzolane naturelle qui a utilisé comme une substitution partielle dans le ciment portland (10% et 20%) sur le comportement physico-mécanique de BHP.

Abstract:

This article examines the relationship between media consumption and voting choices in India in the context of increasing exposure to media, both traditional and new. Using media exposure-related data from the Centre for the Study of Developing Societies’ (CSDS) national election surveys since 1996, it makes a few key assertions. Historically, there has been a positive relationship between traditional news media exposure and voting for the BJP, i.e., the greater the voters’ TV and newspaper consumption, the higher their support for the BJP. With respect to the Congress party, the tendency has generally been the opposite. This pattern now extends to social media exposure as well. However, this impact of media exposure on voting preferences that’s visible at the broad level weakens considerably when slicing the data further by socio-demographic factors. It has been found that the trend of the BJP’s vote share rising with increasing exposure to media is not consistent across the categories of age, education, and caste during many elections. If there is one election, however, that bucked this trend, it was the 2014 Lok Sabha election. The article argues that the BJP was successful in recognizing this and capitalized on it through an extensive media and communications campaign in 2014.

The research community has already reported the unwanted characteristics of BGP like low integrity and slow convergence through theoretichal analyzes andd empirical measurements. Simulations allow for more realistic and flexible experiments than the theoretichal approach and also lower costs than the measurements in real life environments. The first part of this work describe theoretically characteristics and problems related to BGP nd also expectations of the today Internet users to real time applications (like VoIP). The second part concentrates in identifying and implementing of the elements for creating an integrated simulation environment for evaluating dhe effects of slowconvergence of BGP in these applications. At last it is evaluated the created environement through some small scale simmulations that try to model the now days Internet Structure.

Abstract:

There is a current clinical need for the development of bone void fillers and bioactive bone graft substitutes. The use of mesenchymal stem cells (MSCs) that are seeded into 3D scaffolds and induce bone generation in the event of MSCs osteogenic differentiation is highly promising. Since calcium ions and phosphates promote the osteogenic differentiation of MSCs, the use of the calcium complexes of phosphate-containing polymers is highly prospective in the development of osteogenic scaffolds. Calcium poly(ethylene phosphate)s (PEP-Ca) appear to be potentially suitable candidates primarily because of PEP’s biodegradability. In a series of experiments with human adipose-tissue-derived multipotent mesenchymal stem cells (ADSCs), we demonstrated that PEP-Ca are non-toxic and give rise to osteogenesis gene marker, bone morphogenetic protein 2 (BMP-2) and mineralization of the intercellular matrix. Owing to the synthetic availability of poly(ethylene phosphoric acid) block copolymers, these results hold out the possibility for the development of promising new polymer composites for orthopaedic and maxillofacial surgery.

Abstract:

This study attempted to examine the EFL students’ Basic English Pronunciation (BEP) at the first semester of the English study program, the State University of Timor. It was aimed at finding out the mastery of the students’ ability towards the Basic English Pronunciation (BEP), to what extent do the students still find difficulties to pronounce the basic English words, and to what level do the students' mastery level of pronunciation categorized? The method applied in this research was a descriptive quantitative method that employed the syntax of mathematical operation to investigate the properties of data collection (Walliman, 2005). The instrument was a list of 24 numbers of groups of Voice Consonants and Unvoiced Consonants of Basic English Pronunciation taken from “Improve Your English Pronunciation and Learn over 500 Commonly Mispronounced Words”, meanwhile, 26 students were chosen from the freshmen students of English Study Program (ESP) to be the subject of this present study. The result showed that the students’ ability in pronouncing the Basic English words was fair. Particularly, the students mostly had the ‘Enough’ level of ability at the four elements of supra-segmental as such: stress, intonation, voice quality and gesture. Specifically, at this ‘Enough level’, there were 16 students (61.5%) of ‘Stress’ element, 17 students (65.3%) of ‘Intonation’ element, 13 students (50.0%) of ‘Voice Quality’ and ‘Gestures’ as part of Segmental element was 16 students (61.5%). Meanwhile, at the ‘Rhythm’ element, most students (53.8% = 14 students) showed the ‘Less’ ability in this element and only 12 (46.1%) categorized at the level of ‘Enough’’. Otherwise, there were no students in the ‘Bad’ level category. However, few students still encountered difficulties in Segmental element (Voice Quality) and Supra-segmental parts such as words’ stressing, intonation, rhythm, and gestures respectively.

Abstract:

Due to the lack of macroporosity in current available Calcium Phosphate cement used in osteoarticular surgery, Micro and Macroporous Biphasic CaP Cement (MCPC™) was developed. The MCPC™ concept was the association of a settable and a fast resorbable matrix and a sieved fraction of microporous biphasic calcium phosphate (BCP) granules, recognized for the high osteoconductive and osteogenic properties. During the resorption of the matrix, a porous structure is created and the osteoconductive effect of the granules promotes the bone ingrowth. A goat preclinical study was realized to evaluate the efficacy of MCPC™ for C3 and C4 vertebral body filling defects during 6 months. Bone remodelling was evidenced demonstrating bone ingrowth at the expense of the cement and surrounding the residual BCP granules. Bone trabeculae were observed coming from the spongious bone to the implant site. Human vertebral body filling cases demonstrated the biocompatibility and the safety of MCPC™ for bone reconstruction. Results of this study demonstrated the importance of special combination of calcium phosphate granules in the MCPC™ to provide macroporosity and scaffolding for newly formed bone.

Traditional IP networks use a hop-by-pop principle for transmitting traffic. This leads to aggregation of heterogeneous traffic on links in different parts of the network, which causes considerable possible growth of congestion and leaves the network with both unbalanced use of resources and link failure in congested parts. To support a growing number of users and multiple classes of applications with different performance requirements and characteristics, service providers have been forced to adapt to new technologies. Researchers have found that conventional IP packet forwarding is not suitable for applications such as VOIP and video conferencing, which are currently in huge demand. In addition to offer a general provision of MPLS technology, architecture, operation method and features, we will consider the Border Gateway Protocol (BGP) “Black hole” issue that results in the inability of the network to transfer traffic between some end points, and how MPLS help us avoid this problem and even optimize network operation and resources utilization.

Abstract:

Background: Operation room (OR) nurses are at a high risk for bloodborne pathogen (BBP) exposures because they are constantly in contact with blood and body fluid (BBF) through surgical sites, and they frequently use sharp surgical instruments and sutures during surgeries. We explored occupational experiences with BBP exposures among OR nurses in South Korea. Methods: With an institutional review board’s approval, this qualitative research was performed with 12 OR nurses who had worked for >3 months and had experienced BBP exposures. Based on the main research question “How is the experience of BBP exposure among OR nurses?” the semistructured questions for in-depth interview were prepared. Narrative data were collected through 1-hour individual interviews from June to September 2020 and were analyzed using a thematical analysis method. Results: The average age of the participants was 34.4 years with an average 9.75 years of clinical experience. The main theme extracted was “The nurses are alerted to their safety after experiencing the aftereffects and emotional trauma from BBP exposures,” with 4 subthemes and 14 concepts. The first subtheme, “OR nurses risking exposure to BBF,” included (1) hurried doctors and hasty nurses; (2) sharp surgical instruments everywhere; (3) deprioritized self-protection due to ongoing surgery; (4) inattentive to BBF risk; and (5) uncomfortable, foggy goggles and receded safety devices. The second subtheme, “BBP exposures occurred in a flash,” included (1) sharp injury occurred in a split second; (2) temporarily treat sharp wounds while hiding frightened feelings; and (3) OR nurses concentrated on surgery by suppressing anxiety and sharp wounds. The third subtheme, “Burdened time that could be overwhelmed alone,” included (1) BBP exposures moments that I wish to reverse; (2) anger over dangerous environments and the turmoil of anxiety; (3) exhausted body and facial discoloration due to taking postexposure prophylaxis; and (4) exposure to BBP that I want to hide from family and friends. The fourth subtheme, “Voice for everyone’s safety,” included (1) establishing a safety culture, which requires everyone’s efforts, and (2) necessity of practical resources for decreasing BBP exposures. Conclusions: The participating OR nurses felt that they were working with a high risk to BBF exposures and called for institutional interventions to reduce the risks, including surgeons’ attentive collaboration. Korean hospitals should make greater efforts to establish safety culture in ORs and to provide repeated, tailored education to prevent BBF exposures. This article presents the capability approach as an alternative conceptual and normative framework to assess the impact and relevance of existing labour market regulations. In this perspective, what matters is not GDP growth or the overall employment rate, but the development of people’s real freedom to choose a job or an activity they have reason to value. The two key notions of ‘capability for work’ and ‘capability for voice’ are presented in detail, as well as the way to use them as a framework for scientific analysis and empirical research. The article strongly emphasizes the significant difference that the use of such tools makes when it comes to assessing the impact and relevance of collective regulations in the field of work. Cet article présente l’approche par les capacités comme un cadre conceptuel et normatif alternatif pour évaluer l’impact et la pertinence des réglementations existantes du marché du travail. Dans cette perspective, ce qui importe, ce n’est pas la croissance du PIB ou le taux global d’emploi, mais le développement de la liberté effective des individus de choisir un emploi ou une activité qu’ils ont des raisons d’apprécier. Les deux notions essentielles de « capacité à exercer un travail » et de « capacité à faire entendre sa voix » sont présentées en détail, de même que la manière d’utiliser ces notions comme cadre d’analyse scientifique et de recherche empirique. L’article souligne fortement la différence considérable qu’entraîne l’utilisation de tels outils quand il s’agit d’évaluer l’impact et la pertinence des réglementations collectives dans le domaine du travail. In diesem Beitrag wird der Ansatz der Verwirklichungschancen als alternativer konzeptueller und normativer Rahmen vorgeschlagen, um die Auswirkungen und die Relevanz der bestehenden Arbeitsmarktvorschriften zu bewerten. Unter diesem Gesichtspunkt ist nicht das BIP-Wachstum oder die Gesamtbeschäftigungsrate von Bedeutung, sondern die Entwicklung der realen Freiheit der Menschen, eine Arbeit oder Tätigkeit zu wählen, die sie wertschätzen. Die beiden zentralen Konzepte der “capability for work” (Verwirklichungschance im Arbeitsleben) und der “capability for voice” (Chance, für Arbeitnehmerbelange einzutreten) werden ausführlich beschrieben, und es wird erklärt, wie diese als Rahmen für die wissenschaftliche Analyse und empirische Forschungsarbeiten dienen. Fitur Traffic Engineering pada MPLS dapat melakukan perpindahan pada link trafik yang mengalami congestion, sehingga link dapat dipindahkan pada link yang kosong. Teknologi MPLS VPN-TE menjadi solusi untuk meningkatkan keamanan dan pemilihan rute terbaik dalam suatu jaringan. Integrated Service merupakan salah satu model QoS untuk masalah pengontrolan bandwidth end-to-end pada suatu jaringan yang diperlukan oleh teknologi MPLS VPN-TE untuk kestabilan jaringan. Open IMS Core merupakan server layanan multimedia yang digunakan pada teknologi MPLS VPN-TE dengan mempertimbangkan Quality Of Service pada layanan multimedia. Penggunaan protocol BGP merupakan jenis peroutingan yang dapat melakukan pertukaran informasi routing dengan memetakan tabel IP network antar Autonomous System (AS) dengan memberikan peningkatan QoS pada suatu jaringan.Pada Penelitian ini diimplementasikan teknologi MPLS VPN TE dengan menggunakan router mikrotik. Dari hasil pengujian menunjukan bahwa teknologi MPLS VPN dengan penambahan fitur Traffic Engineering didapatkan hasil perbaikan delay sebesar 27,44% untuk voip,11,14% untuk video call. Untuk parameter throughput mengalami perbaikan sebesar 6,02 % untuk voip, 56,6% untuk video call dan jitter mendapatkan hasil < 1 ms.

Abstract:

Individuals have been found to make judgments on a person’s personality, income, education, and employment in as little as 30 seconds after listening to their voice. This study investigates the perceptions of Cuban and Peninsular Spanish varieties by native Cuban and Peninsular Spanish speakers, second language (L2) Spanish learners, and monolingual English speakers, to analyze whether (i) these groups differ in their ability to recognize these varieties, and (ii) whether there is stigma attributed to either accent. The study consisted of 5 Cuban (Havana) and 5 Peninsular (Madrid) voices which were disguised and rated by 20 adult listeners. The methodology included the administration of a Bilingual Language Profile (BLP) questionnaire and a survey intended to gauge at listeners’ perceptions. Results revealed that listeners do make unconscious assumptions on an individual’s voice, as the Peninsular variety was often attributed to a higher educational level (84% SP, 48% CU), income (60% SP, 40% CU), and was more closely associated with a CEO position (32%), while the Cuban voice was associated with being funnier and slightly more intelligible. Furthermore, native Cuban listeners were found to outperform all other groups in their ability to correctly categorize the accents with an accuracy rate of 92%.

Abstract Automatic voice pathology detection and classification plays an important role in the diagnosis and prevention of voice disorders. To accurately describe the pronunciation characteristics of patients with dysarthria and improve the effect of pathological voice detection, this study proposes a pathological voice detection method based on a multi-modal network structure. First, speech signals and electroglottography (EGG) signals are mapped from the time domain to the frequency domain spectrogram via a short-time Fourier transform (STFT). The Mel filter bank acts on the spectrogram to enhance the signal’s harmonics and denoise. Second, a pre-trained convolutional neural network (CNN) is used as the backbone network to extract sound state features and vocal cord vibration features from the two signals. To obtain a better classification effect, the fused features are input into the long short-term memory (LSTM) network for voice feature selection and enhancement. The proposed system achieves 95.73% for accuracy with 96.10% F1-score and 96.73% recall using the Saarbrucken Voice Database (SVD); thus, enabling a new method for pathological speech detection.
