ImportanceEarly in the COVID-19 pandemic, states implemented temporary changes allowing physicians without a license in their state to provide care to their residents. There is an ongoing debate at both the federal and state levels on whether to change licensure rules permanently to facilitate out-of-state telemedicine use.ObjectiveTo describe out-of-state telemedicine use during the pandemic.Design, Setting, and ParticipantsThis cross-sectional study of telemedicine visits included all patients with traditional Medicare from January through June 2021.Main Outcomes and MeasuresTelemedicine visits from January through June 2021 where the patient’s home address and the physician’s practice address were in different states.ResultsIn describing which patients and specialties were using out-of-state telemedicine, we focused on the period between January to June 2021. We chose this period because it was after the turmoil of the early pandemic, when vaccines became widely available and the health care system had stabilized, but before many of the temporary licensing regulations began to lapse by mid-2021. In the first half of 2021, there were 8 392 092 patients with a telemedicine visit and, of these, 422 547 (5.0%) had 1 or more out-of-state telemedicine visits. Those who lived in a county close to a state border (within 15 miles) accounted for 57.2% of all out-of-state telemedicine visits. Among the out-of-state visits in this time period, 64.3% were with a primary care or mental health clinician. For 62.6% of all out-of-state visits, a prior in-person visit occurred between the same patient and clinician between March 2019 and the visit. The demographics and conditions treated were similar for within-state and out-of-state telemedicine visits, with several notable exceptions. Among those with a telemedicine visit, people in rural communities were more likely to receive out-of-state telemedicine care (33.8% vs 21.0%), and there was high of out-of-state telemedicine use for cancer care (9.8% of all telemedicine visits for cancer care).Conclusions and RelevanceThe findings of this cross-sectional study suggest that licensure restrictions of out-of-state telemedicine would have had the largest effect on patients who lived near a state border, those in rural locales, and those who received primary care or mental health treatment.

Abstract Background: The number of times a patient will seek acute care in the emergency department or require hospitalization for sickle cell related illness has not been described in a population-based manner. Twenty years ago, rates of acute care visits for 3,578 patients who were part of the Cooperative Study of Sickle Cell Disease were reported, eloquently describing patterns of acute care utilization for people followed at select centers. In that study, only 1% of patients had more than six visits per year and 5% of the population (who made three to 10 visits per year) accounted for almost one-third of all visits. The objective of this study is to describe the emergency department and hospital utilization for patients with sickle cell related conditions over a two- year period. This study will be the first to provide a complete assessment of the utilization patterns of patients with sickle cell disease, one that is multi-state, inclusive of all ages, all insurance types, and includes patients that are followed at community, academic and tertiary care centers. Methods: We conducted a retrospective cohort study using 2005 and 2006 State Emergency Department and State Inpatient Databases that include encrypted personlevel identifiers to allow linkage of record level information. The data are from the Healthcare Cost and Utilization Project (HCUP), a Federal-State-Industry partnership sponsored by the Agency for Healthcare Research and Quality. Data for all sickle cell related emergency department visits and hospitalizations within the following seven states (Arizona, California, Florida, Missouri, South Carolina, Tennessee, and New York) were extracted for each patient. To be eligible, a patient had at least one sickle cell specific visit, defined as a visit with a principal diagnosis of sickle cell crisis or a secondary diagnosis of sickle cell disease with a principal diagnosis that was sickle cell related (e.g. pneumonia, stroke, fever). All sickle cell related visits were linked by unique personal identifiers, thus clustering visits by patient and allowing population-based statewide assessments of utilization. An emergency department visit on the same day as an inpatient hospitalization was counted only as an inpatient hospitalization to avoid over counting care-seeking visits. The distribution of acute care visits for each patient (presented as numbers of emergency department visits and hospitalizations over the two-year period) was determined for the entire cohort, then stratified as child versus adult. Results: A total of 24,668 patients with sickle cell disease made 86,535 acute care visits during the two-year study period, 33,520 (38.7%) were emergency department visits and 53,015 (61.3%) were inpatient visits. Of the 24,668 patients, 8,895 (36.1%) were less than 18 years of age; 15,773 were adults. 52.8% of the entire cohort made one visit in the two year period. 1,320 (5.4%) patients had more than 12 visits over the two-year time period; 3,210 (13.0%) made 6–20 visits over two years, and accounted for 31,752 (36.7%) acute care visits. An additional 579 (2.4%) patients had more than 20 visits over two years, accumulating 18,701 (21.6%) acute care visits. Children were less likely to have more than 12 visits over the two years (1.9%) compared to adults aged 18–45 (8.1%) and were also less likely to be in the high utilization group of 6–20 visits over two years (9.9% of children compared to 15.8% of those 18–45 years old). Conclusions: A significant proportion of patients with sickle cell disease seek acute care multiple times in an emergency department setting or through hospitalization. Our population-based study demonstrates an increased proportion of high utilizers compared to previous work, especially among adult patients. Our findings likely reflect the difference in healthcare utilization in the broader community as compared to that within a cooperative study in academic settings. It suggests that some patients, adults in particular, may have limited access to urgent care in a primary care setting and would benefit from better access and more aggressive preventive care. Further work on patterns of and reasons for utilization, especially emergency department care, in this high-utilizer group, would be helpful in targeting and improving overall care for these patients.

This article argues that state visits are highly symbolic political performances by analyzing state visits to Berlin in the 1950s and 1960s. The article concentrates on how state visits blended in the Cold War's culture of suspicion and political avowal. Special emphasis is placed on the role of mass media and on the guests' reactions and behavior. State visits to Berlin illuminate the heavy performative and emotional burden placed on all participants. Being aware of the possibilities for self-presentation offered by state visits, West German officials incorporated state visitors into their symbolic battle for reunification. A visit to Berlin with extensive media coverage was, therefore, of prime importance for the German hosts. Despite their sophisticated visualization strategies, total control of events was impossible. Some visitors did not want to play their allotted role and avoided certain sites in Berlin, refused to be accompanied by journalists or cancelled their trips altogether.

ImportanceThe use of nonphysician practitioners (NPPs) in the emergency department (ED) continues to expand, yet little is known about associations between NPPs and ED imaging use.ObjectiveTo investigate whether the state share of ED visits for which an NPP was the clinician of record is associated with imaging studies ordered, given that state NPP share is associated with state-level NPP scopes of practice.Design, Setting, and ParticipantsThis cross-sectional study compared diagnostic imaging ordering patterns associated with ED visits based on 2005-2020 Medicare claims for a nationally representative 5% sample of fee-for-service beneficiaries. For all 50 states and the District of Columbia, the state NPP share of ED visits by year was used to represent state-specific practice patterns for NPPs and physicians and how those patterns have evolved over time. The analysis controlled for patient demographic characteristics, Charlson Comorbidity Index scores, ED visit severity, year, and principal diagnosis.ExposuresThe share of ED visits in each state in each year (state share) for which an NPP was the evaluation and management clinician.Main Outcomes and MeasuresThe main outcomes were the number and modality of imaging studies associated with ED visits. Analyses were by logistic regression and generalized linear model with γ-distribution and log-link function.ResultsAmong 16 922 274 ED visits, 60.0% involved women, and patients’ mean (SD) age was 70.3 (16.1) years. The share of all ED visits with an NPP as the clinician increased from 6.1% in 2005 to 16.6% in 2020. Compared with no NPPs, the presence of NPPs in the ED was associated with 5.3% (95% CI, 5.1%-5.5%) more imaging studies per ED visit, including a 3.4% (95% CI, 3.2%-3.5%) greater likelihood of any imaging order per ED visit and 2.2% (95% CI, 2.0%-2.3%) more imaging studies ordered per visit involving imaging.Conclusions and RelevanceIn this study, use of NPPs in the ED was associated with higher imaging use compared with the use of only physicians in the ED. Although expanded use of NPPs in the ED may improve patient access, the costs and radiation exposure associated with more imaging warrants additional study.

Objective. Emergency departments (EDs) are a critical source of medical care in the U.S. Information is sparse concerning infectious disease visits among Medicaid entitlement enrollees nationwide. The objective of this study was to describe infectious diseases in terms of Medicaid/State Children's Health Insurance Program (SCHIP) as an expected source of payment. Methods. Data for 2003 from the National Hospital Ambulatory Medical Care Survey (NHAMCS) were analyzed for infectious disease visits. NHAMCS is a national probability sample survey of visits to hospital EDs and outpatient departments of nonfederal, short-stay, and general hospitals in the U.S. Data are collected annually and are weighted to generate national estimates. Results. Nationally in 2003, an estimated 21.6 million visits were made to hospital EDs for infectious diseases (rate = 76 visits/1,000 people). Medicaid/SCHIP was the expected source of payment for an estimated 6.7 million infectious disease-related visits (rate = 200 visits/1,000 people covered by Medicaid). Children aged <15 years made 39% of visits nationwide (nationwide rate = 139 visits/1,000 people). Of Medicaid visits, 63% were made by children <15 years of age (Medicaid enrollees rate = 255 visits/1,000 people). The rate of visits for Medicaid enrollees was comparable for females and males (198 visits vs. 201/1,000 people). The rate of visits for black Medicaid enrollees was 33% higher than for white Medicaid enrollees (255 vs. 192 visits/1,000 people). Upper respiratory tract infection (URTI) is the most frequent infectious condition recorded at ED visits. An estimated 47% of ED visits with an expected pay source of Medicaid relate to URTIs (93 visits/1,000 people), compared with 38% of ED visits in general (29 visits/1,000 people). Conclusion. Medicaid enrollee-specific ED visit rates for infectious diseases were higher by age group, gender, race, and region, compared with national rates. Because approximately half of visits relate to URTIs for a Medicaid payment group, URTIs should form the basis for development of appropriate control strategies.

Abstract Introduction: According to the Commission on Cancer's 2012 program standards, patients diagnosed with cancer may experience psychological issues that can interfere with patient treatment plans and adversely affect outcomes. To address these issues, the Commission developed the following guidelines to accurately determine patient distress levels: 1) patients with cancer are offered screening for distress at least 1 time during a pivotal medical visit, 2) the mode of administration for the distress screening is to be determined by the program, and 3) facilities select the tool to be administered to screen for distress with preference being given to standardized, validated instruments. To meet this standard and plan future work in distress reduction for stem cell transplant recipients, the St. Louis University Blood and Marrow Transplant Program began implementing The State Trait Anxiety Inventory (STAI) with patients during pivotal medical visits. The STAI is a psychological inventory based on a 4-point Likert scale and consists of 40 questions on a self-report basis. The STAI differentiates between the temporary condition of "state anxiety" and the long-standing quality of "trait anxiety." The essential qualities evaluated by the STAI scale are feelings of apprehension, tension, nervousness, stress, and worry. Scores on the STAI scale increase in response to physical danger and psychological stress and decrease as a result of relaxation training. Average scores for working, male adults are 35.72 (state) and 34.89 (trait). Average scores for working, female adults are 35.20 (state) and 34.79 (trait). After implementing the STAI, it was realized that these screenings could be de-identified and analyzed in groups to determine if patterns emerged regarding patients' perceived anxiety levels throughout the bone marrow transplantation process. Method: The study team received Institutional Review Board approval to perform a retrospective examination of STAIs completed by patients throughout the bone marrow transplantation process at the St. Louis University Blood and Marrow Transplant Program from 03/11/2104 through 06/24/2014. A total of 30 inventories were collected, de-identified, and categorized by the following medical visits: arrival visit (the patient's first visit to the Blood and Marrow Transplant clinic), data review visit (the visit to review transplant related testing and sign consents), start of preparative regimen visit, day 0 visit, day +30 bone marrow biopsy visit for allogenic transplantation, day +30 bone marrow biopsy result visit for allogenic transplantation, and day +100 visit for auto transplantation. Averages for each medical visit category were determined by finding the mean score. Category averages were then compared to determine if a particular pivotal medical visit caused patients to experience an overall increase in anxiety level. Results: Results from the study indicate that patients experience the highest levels of anxiety during the early medical visits of the bone marrow transplantation process. Average state anxiety scores were 46 during the arrival visits, 41 during the data review visits, and 44 during the start of preparative regimen visits. Average trait anxiety scores were 38 during the arrival visits, 45 during the data review visits, and 39 during the start of preparative regimen visits. During the day 0 visits, patients' state anxiety scores decreased to an average of 36 and trait anxiety scores decreased to an average of 35. Day +30 and day +100 visits demonstrated even further decreases in state and trait anxiety scores. Conclusion: The surprising finding of this study was that patients demonstrated a higher level of distress in the period leading up to the transplant which gradually decreased once the preparative regimen was administered. The sample size for this study was small and could possibly skew results. However, this study does provide a starting basis for future study in bone marrow transplant recipient distress, and larger, multi-site studies are being planned to ensure the accuracy of the patterns, which emerged from this study. If patterns could be accurately identified and predicted, the study team may also be able to develop future studies to primitively lower patients' anxiety levels early in the bone marrow transplantation process and thus improve outcomes. A participação de empresários em visitas oficiais realizadas por Chefes de Estado ou de Governo é uma forma das entidades oficiais promoverem a internacionalização e as exportações das empresas. Existe um vasto corpo de literatura sobre estudos que se debruçaram sobre a promoção das exportações e sobre a avaliação de programas de promoção das exportações. Contudo, até à data, parece não ter sido avaliada a utilidade das visitas oficiais como meio de promoção das exportações. Nesse sentido, este estudo é um contributo para a literatura teórica e empírica. É apresentada uma base teórica para fundamentar a utilidade das visitas oficiais, elaborada a partir da literatura existente sobre a promoção das exportações, programas de promoção das exportações e missões comerciais. A nível empírico, este estudo aferiu a percepção dos empresários portugueses acerca da utilidade das visitas oficiais, através da realização de um inquérito aos empresários que participaram em 12 visitas oficiais organizadas pelos Chefes de Estado e de Governo, entre 2005 e 2008. Adicionalmente, o estudo propõe um modelo multivariável que analisa em que medida as características estruturais das empresas, e, as características associadas à visita oficial, podem afectar o modo como os empresários avaliam a utilidade das visitas oficiais para a concretização de determinados objectivos. Os resultados sugerem que a qualidade dos contactos e das informações obtidas durante a visita é um factor estatisticamente significativo e contribui positivamente para todos os objectivos avaliados. Os resultados revelam ainda, que factores como a dimensão, o capital estrangeiro, a intensidade exportadora, a intensidade de inovação, a experiência exportadora no mercado visitado, a existência de contactos ou negócios prévios, ou o facto de ser a primeira vez que a empresa visita o mercado, tendem a afectar o modo como as empresas avaliam a utilidade das visitas oficiais para a realização de determinados objectivos.
The participation of businessmen in official visits done by Heads of State or Government is a way found by official authorities to promote exports and the internationalization of the firms. There is a large body of literature about export promotion and the evaluation of export promotion programs. However, to date, it seems has not been evaluated the usefulness of official visits as a way to promote firms exports. This study provides a theoretical and empirical contribution to the literature. The study presents a theoretical basis to substantiate the usefulness of official visits found in the literature about export promotion, export promotion programs and trade missions. A survey was done questioning Portuguese businessmen who had participated in 12 official visits to evaluate the usefulness of those visits. Additionally, the study proposes a multivariable model to examine how firm and visit caractheristics do affect, or not, the firms evaluation about official visits. The estimated results suggest that the quality of contacts and information gained during the visit positively affect the impact over a set of objectives. The results also reveal that factors such as firm s size, foreign ownership, export intensity, R&D intensity, experience in the market, the existence of previous contacts or business, or first visit to the market affect the firms evaluation about some objectives of official visits.

A nível empírico, este estudo aferiu a percepção dos empresários portugueses acerca da utilidade das visitas oficiais, através da realização de um inquérito aos empresários que participaram em 12 visitas oficiais organizadas pelos Chefes de Estado e de Governo, entre 2005 e 2008. Adicionalmente, o estudo propõe um modelo multivariável que analisa em que medida as características estruturais das empresas, e, as características associadas à visita oficial, podem afectar o modo como os empresários avaliam a utilidade das visitas oficiais para a concretização de determinados objectivos. Os resultados sugerem que a qualidade dos contactos e das informações obtidas durante a visita é um factor estatisticamente significativo e contribui positivamente para todos os objectivos avaliados. Os resultados revelam ainda, que factores como a dimensão, o capital estrangeiro, a intensidade exportadora, a intensidade de inovação, a experiência exportadora no mercado visitado, a existência de contactos ou negócios prévios, ou o facto de ser a primeira vez que a empresa visita o mercado, tendem a afectar o modo como as empresas avaliam a utilidade das visitas oficiais para a realização de determinados objectivos.
The participation of businessmen in official visits done by Heads of State or Government is a way found by official authorities to promote exports and the internationalization of the firms. There is a large body of literature about export promotion and the evaluation of export promotion programs. However, to date, it seems has not been evaluated the usefulness of official visits as a way to promote firms exports. This study provides a theoretical and empirical contribution to the literature. The study presents a theoretical basis to substantiate the usefulness of official visits found in the literature about export promotion, export promotion programs and trade missions. A survey was done questioning Portuguese businessmen who had participated in 12 official visits to evaluate the usefulness of those visits. Additionally, the study proposes a multivariable model to examine how firm and visit caractheristics do affect, or not, the firms evaluation about official visits. The estimated results suggest that the quality of contacts and information gained during the visit positively affect the impact over a set of objectives. The results also reveal that factors such as firm s size, foreign ownership, export intensity, R&D intensity, experience in the market, the existence of previous contacts or business, or first visit to the market affect the firms evaluation about some objectives of official visits.

Abstract:
The main objective of this thesis is to apply ideas and techniques from modern control theory, especially from robust state estimation and model validation, to various important problems in computer vision. Robust model validation is used in texture recognition where new approaches for classifying texture samples and segmenting textured images are developed. Also, a new model validation approach to motion primitive recognition is demonstrated by considering the motion segmentation problem for a mobile wheeled robot. A new approach to image inpainting based on robust state estimation is proposed where the implementation presented here concerns with recovering corrupted frames in video sequences. Another application addressed in this thesis based on robust state estimation is video-based tracking. A new tracking system is proposed to follow connected regions in video frames representing the objects in consideration. The system accommodates tracking multiple objects and is designed to be robust towards occlusions. To demonstrate the performance of the proposed solutions, examples are provided where the developed methods are applied to various gray-scale images, colored images, gray-scale videos and colored videos. In addition, a new algorithm is introduced for motion estimation via inverse polynomial interpolation. Motion estimation plays a primary role within the video-based tracking system proposed in this thesis. The proposed motion estimation algorithm is also applied to medical image sequences. Motion estimation results presented in this thesis include pairs of images from a echocardiography video and a robot-assisted surgery video.

The Covid-19 pandemic has left an impact on the whole world and all economic activities without exception. There are already indications of the consequences, and they will be fully perceptible only after the pandemic is over, which still seems far away. One of the general characteristics of tourism is vulnerability during periods of crisis which everyone globally has to face. Since the beginning of 2020, and especially since March, a drastic drop in tourist travels on the tourism market of the European Union, or its member states, is obvious. The goal of this paper is to define a model which will be able to predict changes in tourist visits based on their movements in previous periods. The analysis includes all European Union member states and changes in the number of tourist visits on a monthly basis during 2020. The model will also enable future predictions, which is very important for tourism policy carriers, as well as for the tourism offer which can adapt to expectations.

Deep learning-based predictive models, leveraging Electronic Health Records (EHR), are receiving increasing attention in healthcare. An effective representation of a patient's EHR should hierarchically encompass both the temporal relationships between historical visits and medical events, and the inherent structural information within these elements. Existing patient representation methods can be roughly categorized into sequential representation and graphical representation. The sequential representation methods focus only on the temporal relationships among longitudinal visits. On the other hand, the graphical representation approaches, while adept at extracting the graph-structured relationships between various medical events, fall short in effectively integrate temporal information. To capture both types of information, we model a patient's EHR as a novel temporal heterogeneous graph. This graph includes historical visits nodes and medical events nodes. It propagates structured information from medical event nodes to visit nodes and utilizes time-aware visit nodes to capture changes in the patient's health status. Furthermore, we introduce a novel temporal graph transformer (TRANS) that integrates temporal edge features, global positional encoding, and local structural encoding into heterogeneous graph convolution, capturing both temporal and structural information. We validate the effectiveness of TRANS through extensive experiments on three real-world datasets. The results show that our proposed approach achieves state-of-the-art performance.

Medication recommendation is an important healthcare application. It is commonly formulated as a temporal prediction task. Hence, most existing works only utilize longitudinal electronic health records (EHRs) from a small number of patients with multiple visits ignoring a large number of patients with a single visit (selection bias). Moreover, important hierarchical knowledge such as diagnosis hierarchy is not leveraged in the representation learning process. Despite the success of deep learning techniques in computational phenotyping, most previous approaches have two limitations: task-oriented representation and ignoring hierarchies of medical codes. To address these challenges, we propose G-BERT, a new model to combine the power of Graph Neural Networks (GNNs) and BERT (Bidirectional Encoder Representations from Transformers) for medical code representation and medication recommendation. We use GNNs to represent the internal hierarchical structures of medical codes. Then we integrate the GNN representation into a transformer-based visit encoder and pre-train it on EHR data from patients only with a single visit. The pre-trained visit encoder and representation are then fine-tuned for downstream predictive tasks on longitudinal EHRs from patients with multiple visits. G-BERT is the first to bring the language model pre-training schema into the healthcare domain and it achieved state-of-the-art performance on the medication recommendation task.

This paper discusses the results of a project funded by the California Energy Commission Public Interest Energy Research (PIER) to complete an independent assessment of the energy savings, environmental improvements and water conservation capabilities of emerging non-chemical water treatment technologies. The project was completed by a team from California State University at Sacramento and included a technical review of the emerging technologies and a detailed assessment of the emerging non-chemical water treatment technology. Clearwater Systems, Corp. The research was focused on gathering information from industrial field customers who had purchased and installed these systems and had actual experience with their operational characteristics from several months to several years. The team completed a telephone survey with approximately 15 end user customers and made site visits to ten sites. Some limited independent water testing was also completed. The results of these phone surveys and site visits were consolidated and placed in an interim report. Even though only a small number of end user customers were actually surveyed or visited, the research indicated that several hundred systems have been successfully installed in California and throughout the United States. The emerging technologies provide nonchemical treatment for cooling tower and evaporative condenser system water. All the information collected and results derived from this effort will be made available to the public later this year in the form of a PIER Technical Report. A Project Advisory Committee that included representatives from CalEPA, the Energy Commission PIER Program and local utilities supported this team. The National Park Service (NPS) manages the Nation’s most iconic destinations that attract millions of visitors from across the Nation and around the world. Trip-related spending by NPS visitors generates and supports economic activity within park gateway communities. This report summarizes the annual economic contribution analysis that measures how NPS visitor spending cycles through local economies, generating business sales and supporting jobs and income. In 2020, the National Park System received over 237 million recreation visits (down 28% from 2019). Visitors to national parks spent an estimated $14.5 billion in local gateway regions (down 31% from 2019). The estimated contribution of this spending to the national economy was 234,000 jobs, $9.7 billion in labor income, $16.7 billion in value added, and $28.6 billion in economic output. The lodging sector saw the highest direct effects, with $5 billion in economic output directly contributed to this sector nationally. The restaurants sector saw the next greatest effects, with $3 billion in economic output directly contributed to this sector nationally. Results from the Visitor Spending Effects report series are available online via an interactive tool. Users can view year-by-year trend data and explore current year visitor spending, jobs, labor income, value added, and economic output effects by sector for national, state, and local economies. Education remains crucial for socioeconomic development and is linked to improved quality of life. In Nigeria, basic education has remained poor and is characterised by unhealthy attributes, including low quality infrastructure and a lack of effective management of primary and secondary schools. Access to education is a massive issue—according to the United Nations, there are currently about 10.5 million out of school children in Nigeria, and 1 in every 5 of the world’s out-of-school-children lives in Nigeria despite the fact that primary education in Nigeria is free. A considerable divide exists between the northern and southern regions of Nigeria, with the southern region performing better across most education metrics. That said, many children in southern Nigeria also do not go to school. In Nigeria’s South West Zone, 2016 data from the Nigerian Federal Ministry of Education reveals that Lagos State has the highest number of out of school children with more than 560,000 children aged 6-11 not going to school. In the South South Zone, Rivers State has the highest number of out-of-school children; more than 900,000 children aged 6-11 are not able to access education in this state. In Enugu State in the South East Zone, there are more than 340,000 children who do not have access to schooling (2016 is the most recent year high-quality data is available—these numbers have likely increased due to the impacts of COVID-19). As part of its political economy research project, the RISE Nigeria team conducted surveys of education stakeholders in Enugu State including teachers, parents, school administrators, youth leaders, religious leaders, and others in December 2020. The team also visited 10 schools in Nkanu West Local Government Area (LGA), Nsukka LGA, and Udi LGA to speak to administrators and teachers, and assess conditions. It then held three RISE Education Summits, in which RISE team members facilitated dialogues between stakeholders and political leaders about improving education policies and outcomes in Enugu. These types of interactions are rare in Nigeria and have the potential to impact the education sector by increasing local demand for quality education and government accountability in providing it. Inputs from the surveys in the LGAs determined the education sector issues included in the agenda for the meeting, which political leaders were able to see in advance. The Summits culminated with the presentation of a social contract, which the team hopes will aid stakeholders in the education sector in monitoring the government’s progress on education priorities. This article draws on stakeholder surveys and conversations, insights from the Education Summits, school visits, and secondary data to provide an overview of educational challenges in Enugu State with a focus on basic education. It then seeks to highlight potential solutions to these problems based on local stakeholders’ insights from the surveys and the outcomes of the Education Summits.

Statistics are presented on physician practices as well as patient and visit characteristics using data collected in the 2019 National Ambulatory Medical Care Survey (NAMCS). NAMCS is an annual nationally representative sample survey of visits to nonfederal office-based patient care physicians, excluding anesthesiologists, radiologists, and pathologists.

Over the past two decades, the Inter-American Development Bank (hereafter "the Bank") has gained considerable experience with regard to the promotion of sustainable tourism activities in Brazil. These include ecotourism in the Amazon region, but particularly in the less developed Northeast. This has entailed a valuable learning process with respect to the environmental impacts and management challenges associated with tourism development and investments to support it, leading to a significant evolution in the Bank's approach over time. As a result, much of what the Bank is currently doing in connection with its tourism development projects in Brazil can be characterized as good practice. This is especially the case both in terms of its strategic planning approach at the state and local levels, as well as the ways in which environmental considerations and components have been integrated, or mainstreamed, into overall project and subproject design and implementation. Based on a review of pertinent documents; interviews with Bank staff and selected borrowers; and field visits to two of the four active Prodetur projects (in the states of Ceara and Pernambuco), the purpose of this paper is to summarize these features of the Bank's approach to sustainable tourism in Brazil and to briefly describe how this approach has evolved over time. It will also identify a few areas where further improvements would be useful.

Based on the proposal by the European Council, more than 25 heads of state and the World Health Organization (WHO) support development of an international treaty on pandemics, that planned to be negotiated under the auspices of WHO, will be presented to the World Health Assembly in May 2021. Given that the treaty alone is not enough to ensure compliance, triggers for a high-level political response is required. To this end, to inform the design of a support system, we explored institutional mechanismsi with a mandate to review compliance with key international agreements in their signatory countries and conduct independent country investigations in a manner that manages sovereign considerations. Based on our review, there is no single global mechanism that could serve as a model in its own right. There is, however, potential to combine aspects of existing mechanisms to support a strong, enforceable treaty. These aspects include: • Periodic review - based on the model of human rights treaties, with independent experts as the authorized monitoring body to ensure the independence. If made obligatory, the review could support compliance with the treaty. • On-site investigations - based on the model by the Committee on Prevention of Torture according to which visits cannot be blocked by state parties. • Non-negotiable design principles - including accountability; independence; transparency and data sharing; speed; emphasis on capabilities; and incentives. • Technical support - WHO can provide countries with technical assistance, tools, monitoring, and assessment to enhance emergency preparedness and response.

Abstract:
In 2021, recreation visits to National Park Service (NPS) sites rebounded from the COVID-19 pandemic-driven low visitation of 2020 and climbed to 297,115,406 recreation visits. This is an increase of 60 million recreation visits (+25.3%) from 2020 and a decrease of 30 million recreation visits (-9.3%) from 2019. Recreation visitor hours were 1,356,657,749 – a 28.6% increase from 2020 and a 5.1% decrease from 2019. Total overnight stays followed a similar pattern with 12,745,455 overnight stays – up 4.7 million (+58.5%) from 2020 and down 1.1 million (-8%) from 2019. Five parks were added to the reporting system in 2021: Alagnak Wild River in Alaska, Camp Nelson National Monument in Kentucky, Medgar and Myrlie Evers Home National Monument in Mississippi, Tule Springs Fossil Beds National Monument in Nevada, and World War I Memorial in Washington, D.C. These parks were responsible for over 629,000 recreation visits in 2021. Factors influencing visits to National Park System units in 2021 include: continuing closures and limited capacities due to COVID-19 mitigation at some parks, temporary closures for wildland fires in 2021 (eleven parks), severe regional smoke/haze from ongoing wildland fires throughout the summer and early autumn affecting parks in the western half and northern tier of states in the continental U.S., two hurricanes in 2021 – both in August – impacted visitation: Hurricane Henri caused temporary closures of some parks in the northeast and Hurricane Ida caused temporary closures of parks along the Gulf Coast and generated some heavy flooding in the northeast, hurricanes and wildland fires in previous years resulting in lingering closures, most notably Hurricanes Irma and Maria in 2017, the Carr and Woolsey Fires in 2018, Hurricane Dorian in 2019, the Caldwell, Cameron Peak, East Troublesome, and Woodward Fires in 2020, and Hurricane Sally in 2020. Forty-four parks set a record for recreation visits in 2021 and 6 parks broke a record they set in 2020. See Appendix A for a list of record parks. The number of reporting units with over 10 million recreation visits was the same as in recent years (3 parks) and 73 parks had over 1 million recreation visits. Twenty-five percent of total recreation visits occurred in the top 8 parks and fifty percent of total visitation occurred in the top 25 parks. Several parks passed annual visitation milestones including Capulin Volcano NM which passed 100,000 annual recreation visits for the first time, Big Bend NP and Devils Tower NM which each passed 500,000 annual recreation visits for the first time, and Zion NP which passed 5 million visits for the first time. Other parks passed milestones for accumulated recreation visits including Hamilton Grange NMEM (1968-2021) and Palo Alto Battlefield NHP (2003-2021) each passing 1 million total recreation visits, Voyageurs NP (1976-2021) passing 10 million total recreation visits, and Hot Springs NP (1904-2021) passing 100 million total recreation visits. Population center designations were updated in 2021 to reflect overlap of park boundaries with statistical areas from the 2020 U.S. Census. Many population center changes reflect increases in local population as indicated by parks changing from rural to outlying or from outlying to suburban. Other changes reflect increasing complexity in population density as parks changed from a single designation, such as rural or suburban, to a mixed designation. See the Definitions section for population center definitions and Table B.1 for previous and updated population center designations by park. In the pages that follow, a series of tables and figures display visitor use data for calendar year 2021. By documenting these visits across the National Park System, the NPS Statistical Abstract offers a historical record of visitor use in parks and provides NPS staff and partners with a useful tool for effective management and planning. Water quality monitoring in National Park Service units of the Northern Colorado Plateau Network (NCPN) is made possible through partnerships between the National Park Service Inventory & Monitoring Division, individual park units, the U.S. Geological Survey, and the Utah Department of Environmental Quality. This report evaluates water quality data from site visits at 42 different locations within and around eight park units in Utah and Colorado from October 1, 2018 through September 30, 2022. Data are compared to state water quality standards for the purpose of providing information to park managers about potential water quality problems. Parks included for evaluation are Arches National Park (NP), Bryce Canyon NP, Canyonlands NP, Capitol Reef NP, Dinosaur National Monument (NM), Hovenweep NM, Timpanogos Cave NM, and Zion NP. Evaluation of water quality parameters relative to state water quality standards indicated that 21,644 (96.8%) of the 22,356 total designated beneficial-use evaluations completed for the period covered in this report met state water quality standards. The most common parameters that did not meet a standard include fecal indicator bacteria (Escherichia coli), water temperature, and total dissolved solids (TDS). While TDS can be an indicator of pollution, in NCPN parks, it mostly occurs downstream of rock outcrops that naturally increase TDS in streams. Phosphorus concentrations were often greater than acceptable thresholds but were rarely associated with indicators of impairment such as algal blooms, fish kills, or low dissolved oxygen. Sites monitored in Arches NP, Bryce Canyon NP, Capitol Reef NP, Dinosaur NM, Hovenweep NM, and Zion NP all had occurrences when fecal indicator bacteria concentrations were greater than associated state standards. State-coordinated plans to reduce waste contamination are in place for the North Fork Virgin River (Zion NP) and the Fremont River (Capitol Reef NP). The plans have resulted in a decrease in the number of chronic and acute standard violations at Zion. Elevated water temperatures occurred at sites in Canyonlands NP, Capitol Reef NP, and Zion NP. Water temperature is strongly correlated with air temperature in surface waters across the Colorado Plateau. Additional issues of management concern include low dissolved oxygen in Salt Wash at Wolfe Ranch (Arches NP) and Square Tower Spring (Hovenweep NM), as well as selenium in the Colorado River (Arches NP and Canyonlands NP). State-coordinated plans to reduce selenium concentrations in the Upper Colorado River basin are in place.
