Abstract:
In a globalized scenario where widespread international travel allows viral agents to migrate from endemic to non-endemic areas, health care providers and critical care specialists must be able to readily recognize a suspected case of viral haemorrhagic fever (VHF). Early suspicion is pivotal for improving patient outcome and to ensure that appropriate biosafety measures be applied. VHFs are acute febrile illnesses marked by coagulation disorders and organ specific syndromes. VHFs represent a great medical challenge because diseases are associated with a high mortality rate and many VHFs have the potential for person-to-person transmission (Filoviruses, Arenavioruses, and Bunyaviroses). Dengue is the most frequent haemorrhagic viral disease and re-emergent infection in the world and, due to its public health relevance, severe dengue will receive special attention in this chapter. The diagnosis of VHFs is made by detecting specific antibodies, viral antigens (ELISA) and viral nucleic acid (RT-PCR) on blood samples. Supportive care is the cornerstone in the treatment of VHFs. Ribavirin should be started as soon as a case of VHF is suspected and discontinued if a diagnosis of Filovirus or Flavivirus infection is established. Adjunctive antimicrobial therapy is usually implemented to treat co-existing or secondary infections. Antimalarial treatment should also be initiated if a malaria test (thick blood films) is not quickly available and/or reliable and patients travel history is compatible. It is always recommended to apply appropriate biosafety measures and notify local infection control unit and state and national authorities.

In Between Gaia and Ground Elizabeth A. Povinelli theorizes the climatic, environmental, viral, and social catastrophe present as an ancestral catastrophe through which that Indigenous and colonized peoples have been suffering for centuries. In this way, the violence and philosophies the West relies on now threaten the West itself. Engaging with the work of Glissant, Deleuze and Guattari, Césaire, and Arendt, Povinelli highlights four axioms of existence—the entanglement of existence, the unequal distribution of power, the collapse of the event as essential to political thought, and the legacies of racial and colonial histories. She traces these axioms' inspiration in anticolonial struggles against the dispossession and extraction that have ruined the lived conditions for many on the planet. By examining the dynamic and unfolding forms of late liberal violence, Povinelli attends to a vital set of questions about changing environmental conditions, the legacies of violence, and the limits of inherited Western social theory. Between Gaia and Ground also includes a glossary of the keywords and concepts that Povinelli has developed throughout her work.

Infections of the nasal cavity, sinuses, pharynx, epiglottis, and larynx are termed upper respiratory tracts infections. These include acute coryza, pertussis, sinusitis, pharyngitis, tonsillitis, epiglottitis, laryngitis, laryngotracheobronchitis, and influenza. Rhinoviruses and coronaviruses account for the majority of acute coryzal illnesses. Acute sinusitis (<4 weeks duration) is also usually viral in origin. About 70% of pharyngitis and tonsillitis cases are viral in etiology. Haemophilus influenzae (Type B) is responsible for most cases of epiglottitis. Acute laryngitis and laryngotracheobronchitis are usually caused by human parainfluenza viruses. This chapter focuses on upper respiratory tract infections, including their etiology, symptoms, demographics, natural history, complications, diagnosis, prognosis, and treatment.

Abstract:
Encephalitis is an inflammation of the brain parenchyma, typically due to a viral infection. Pure encephalitis will lack the signs and symptoms of meningeal irritation (eg, stiff neck and photophobia). New-onset seizures, cognitive deficits, new psychiatric symptoms, lethargy/coma, cranial nerve abnormalities, or movement disorders should alert the clinician to possible encephalitis. It is important to question the patient about foreign travel, immunocompromised state, and potential exposures. Empiric treatment for presumed viral encephalitis is with the antiviral acyclovir. Empiric broad-spectrum antibiotics are also typically given to cover for possible bacterial meningitis. If there are signs of elevated intracranial pressure (ICP), neurosurgical consultation should be obtained for possible decompressive craniotomy. Standard therapy for ICP (ie, hyperventilation, steroids, mannitol, hypertonic saline, and elevation of the head of the bed) should also be considered. The most concerning complication of encephalitis is the development of life-threatening cerebral edema with resultant brainstem compression and herniation.

This chapter explores the various manifestations of Saudi influence in Yemen’s extreme north, particularly with regard to the role of Saudi patronage politics in protection of the controversial and vulnerable border between the two countries. The beginnings of Saudi patronage policy can be traced back to the Saudi-Yemeni War of 1934 and the Treaty of Ṭāʾif, which resolved it. Since 1934 tribal elites in the Yemeni borderlands played a vital role in securing the common border. By considering the boundary problem through the lens of the borderland tribes, this chapter focuses on the influence of Saudi patronage politics in the area, the mutual interdependencies between Saudi boundary policy and the emergence of the Houthi conflict, and the vital role that tribes and tribal elites played in this process.

India’s engagement with the institutions and norms of global governance has evolved significantly since independence in 1947. This chapter traces the evolution—beginning with early engagement with international organizations under Nehru, to the waning of its enthusiasm for multilateralism in the 1960s and 1970s, and its struggle for greater voice and recognition internationally in the twenty-first century. Through the prism of its quest for a permanent seat in the UN Security Council, its approach to climate change negotiations, global economic diplomacy, and its engagement with global norms, this chapter traces India’s rise as a vital player in the rebalancing of international relations in a multipolar world. However, despite its tremendous progress, some ongoing challenges continue to constrain India’s meaningful participation in global governance at times. The chapter concludes with an assessment of India’s contribution to global governance and its prospects as a stakeholder and shareholder on the global stage.

The essential trace mineral selenium is a component of antioxidant enzymes and proteins that regulate thyroid hormone metabolism and immune function. Thyroid metabolism is sensitive to selenium in the diet, and selenium deficiency can exacerbate the effects of iodine deficiency. Selenium deficiency is also associated with enhanced viral virulence and is a risk factor for perinatal transmission of HIV. Several selenium-dependent proteins are important in male fertility. Most pregnant women in developed countries will be able to meet selenium requirements through their usual diet, but because some foods differ widely in selenium content according to their geographical sources, a variety of foods should be eaten to ensure adequate intake and to avoid toxicity. Taking selenium supplements in addition to adequate dietary intake is not recommended, as the safe intake range is narrow.

The estimation of arterial oxygen saturation by pulse oximetry and arterial carbon dioxide tension by capnography are vital monitoring techniques in critical care medicine, particularly during intubation, ventilation and transport. Equivalent continuous information is not otherwise available. It is important to understand the principles of measurement and limitations, for safe use and error detection. PETCO2 and oxygen saturation should be regularly checked against PaCO2 and co-oximeter SO2 obtained from the blood gas machine. The PECO2 trace informs endotracheal tube placement, ventilation, and blood flow to the lungs. It is essential their principles of estimation, the information gained and the traps in interpretation are understood.

This chapter traces the changing representation of the balance-of-payments ‘problem’ from the 1940s through the post-war period, and the reasons for its virtual disappearance as an important narrative about the economy in the last quarter of the twentieth century. The balance of payments is an especially striking case of both the malleability and partiality of narratives deployed to seek to persuade the public of the importance of an economic policy issue. This chapter also shows the possibility of long-standing economic ‘problems’, once deemed vital to the national economy’s management, disappearing from view as new ways of looking at economic issues, linked to changing institutional arrangements and doctrines, come to the fore.

Optimization techniques have played a vital role in improving the performance (i.e., input speed and accuracy) of soft keyboards. This chapter introduces the challenges, methodologies, and results of keyboard performance optimization. Leveraging the robust human motor control phenomena manifested in text entry, we used the Metropolis random walk algorithm, and Pareto multi-objective optimization method to optimize the keyboard layout and a soft keyboard decoder. The optimization led to layouts that shorten finger travel distance and improve the input speed as well as accuracy over the Qwerty layout, and a soft keyboard decoder with improved correction and completion ability.

This chapter relays in broad terms the long history of European American settlement and subsequent Latin American migration—particularly undertaken by Mexicans—to the U.S. Heartland. It gives particular attention to the capitalist-led development during the second half of the nineteenth century as the United States sought to build itself into not only a formidable industrial power but also a world power. It traces the vital role that immigrant workers—and specifically Mexican laborers—have played in this process despite their often being treated as second-class citizens. An appreciation of this history provides one with a clear sense of the neocolonial aspirations of U.S. enterprise—both governmental and commercial—as well as the many contradictory and timeworn Anglo rationalizations that exploit Mexican workers in the United States today.

Abstract:
This chapter examines the construction of vehicular space in Latin literary texts, identifying two significant aspects of Roman transport’s representation. First is the set-piece depiction of roadway encounters, articulated as a physical run-in of wayfarers with clashing modes of transport. Although the moralizing thrust of such portraits is to inveigh against lavish transportation while extolling simple travel, their more powerful function is to assert the ability of (particular) mobile parties to transcend transit’s physicality. Second is such scenes’ frequent problematization of physical agency, drivers and passengers being caught between demeaning hands-on participation in travel’s means and excessively passive forms of conveyance. The aim throughout is to move beyond a view of such articulations as merely diatribes against luxurious travel, and to excavate how the underlying instrumentality of transportation—its emphatic ‘by-way-of-ness’—is rhetorically constituted, and indeed a vital part of the Roman production of space.

Abstract:
Interrupting Capitalism: Catholic Social Thought and the Economy traces the history of Catholic thinking about economic life from the Second World War to the present, from the perspective of a “theology of interruption.” God interrupts history through the Incarnation and the continued mission of the church, but in an increasingly pluralist society, Christians must learn to see God’s presence in the religious other and in the poor and vulnerable, as well. Faithful to God’s call, the church’s social teaching provides a way for Christians to interrupt capitalism, to live out economic life faithfully in the midst of the global economy. The book traces official Catholic social teaching on the economy from the Second Vatican Council to Pope Francis, but also looks at four important voices in recent Catholic theological reflection on economic life: liberation theology, progressive Catholicism, neoconservative Catholicism, and communitarianism. Of the four, the communitarian perspective appeals to those aspects of the church’s social tradition that are most vital to living out the Christian vocation in contemporary American economic life. The book also turns to critical realism and institutional economics—two perspectives from the social sciences that can help the Catholic social tradition understand the global economy, including the relationship between local practices and broader social structures and institutions.
