Abstract:

Successful veterinary treatment of animals requires owner adherence with a prescribed treatment plan. The aim of our study was to evaluate and compare the level of adherence of the owners of patients presented for behavioural, cardiological, urological, and oncological problems. At the end of the first examination, each owner completed a questionnaire. Then, the owners were called four times to fill out another questionnaire over the phone. With regard to the first questionnaire, statistically significant data concern behavioral medicine and cardiology. In the first area the owner’s worry decreases during the follow-up and the number of owners who would give away the animal increases. In cardiology, owners who think that the pathology harms their animal’s quality of life decreased significantly over time. With regard to the 9 additional follow-up questions, in behavioural medicine and urology the owner’s discomfort resulting from the animal’s pathology significantly decreases over time. Assessment of adherence appears to be an optimal instrument in identifying the positive factors and the difficulties encountered by owners during the application of a treatment protocol.

Abstract:

Objective: To determine the frequency of encrustations of ureteral stents in a tertiary care hospital of Pakistan. Patients and Methods: In this study we included 150 patients of Urology Unit of Ibn e Sina Hospital and Research Institute Multan, Pakistan in whom ureteral stents implantation was done. The study duration was April-2020 to July-2020. Patients with ureteral stents having renal stone, ureteric stone, ureteral stricture, pelviureteric junction obstruction (PUJO), trauma or malignant disease as primary pathology and duration of implantation >2 weeks to <2 years were included. Results: Mean age of patients included in this study was 46.46±12.26 years. Mean duration of stent placement was 11.96±6.01 weeks. There were 97 (64.7%) male and 53 (35.3%) female patients. There were 72 (48%) patients with renal stone disease, 32 (21.33%) patients with ureteric stone, 11 (7.33%) patients with pyeloplasty, 22 (14.67%) patients with partial nephrectomy and 13 (8.67%) patients with obstruction sec. to malignancy were included in this study. Stent encrustation was diagnosed 52 (34.67%) patients. Conclusion: After implantation, ureteral stent encrustation is still a cause for concern for urologists. Because its encrustation management may involve additional urologic procedures. In present study, stent encrustation was found in 62 (32.29%) patients. Keywords: Double-J stents, stent encrustation.

Abstract:

Background: Normally female urethral stricture (FUS) is uncommon and underdiagnosed condition. It is raising a diagnostic challenge for the physicians. It is the one of the rarely known urological entity. Urethral dilatation is the traditionally used treatment for urethral stricture (FUS). The female urethroplasty have shown the promising outcomes. Objective: The objective of the study was to compare the outcome of the dorsal onlay urethoplasty FU obtained by using buccal mucosal graft and vaginal wall graft. The FU is more effective treatment for female urethral stricture (FUS) as compared to the repeated dilatation. Study design: It is a retrospective study with the statistical approach, conducted at Urology Department, Pakistan Institute of Medical Sciences Islamabad from June 2021 to November 2021. Material and Methods: The women who underwent the dorsal onlay urethropasty at the urology department of the hospital were included in the study. The outcomes of BMG and VWG were compared. The re-intervention was marked as failure. The association symptom score, postvoid residual urine (PVR), cystourethrogram, cystoscopy and uretheral calibration were included in the assessment. Operating time, catheter time, etiology, location, length, prior interventions and suprapubic cystostomy were the other parameters that was also reported. For the statistical analysis the Mann-Whitney test, t-test and proportion test was performed. Results: The calculated mean age of the patients included in the group was 48 years. Its range was between the 26 to 76 years. The women visited the urology department of the hospital from June 2021 to June 2022. The average follow-up was for 26 months. The AUA symptoms changes from 22 to 6, Qmax from 4ml/s to 26ml/s and RVR from 185ml to 7ml. The operating time was the only parameter showed the difference, while other remained the same. The 94% was the overall urethral patency rate. Conclusion: For the substitution of dorsal onlay urethroplasty the use of VWG and BMG is highly recommended. It is an efficient and reliable treatment method. There are very low chances of complication and it has an easy protocol. Apparently, no specific change was observed in the FUS treatment by BMG and VWG. Abstract:

Objective: To compare the outcomes of percutaneous nephrolithotomy (PCNL) in patients with renal stones with high (≥700) versus low (<700) HU. Patients and Methods: A total of 200 patients having age 18 to 60 years, with diagnosis of unilateral renal stone disease, having stone size 2 to 4 cm in diameter on non-contrast computed tomography (NCCT) of Urology Unit of Ibn e Sina Hospital and Research Institute Multan were included. The study duration was May-2021 to July-2021. NCCT was done pre-operatively in every patient to measure Hounsfield units (HU). Post-procedural X-ray KUB was done 2 weeks after the surgical procedure to determine the stone clearance in every patient. Results: Mean age was 45.79±11.17 Years. Mean duration of disease of patients included in this study was 8.78±8.59 months. Mean stone size in patients included in this study was 3.11±0.61 cm. On comparison of stone clearance rate between the patients having high-HU versus low-HU, stone clearance was found in 61 (72.6%) patients having low HU value and in 105 (90.5%) patients having high HU value (p-value 0.001). Conclusion: HU-value ≥700 is a significant predictor of success rate of percutaneous nephrolithotomy (PCNL) in patients having renal stone disease. Abstract:

This report draws on a unique, longitudinal dataset of over 250,000 San Francisco Bay Area residents to examine residential instability—including moving, crowding, and financial health—in the Bay Area during the pandemic. Our research finds a substantial decrease in moving during the pandemic, particularly for residents of extremely low socioeconomic status (SES). At the same time, we report a concerning rise in residents living in crowded conditions and experiencing declining credit scores. These trends suggest that COVID-19 rent relief programs and eviction moratoria may be successful in reducing displacement; however, alternative strategies may be necessary to address other forms of residential instability, like crowding, especially in Black and low-income neighborhoods. This report concludes with recommendations to address residential instability in the Bay Area.

Abstract:

Приводятся результаты флористического анализа, выполненного в мае – августе 2021 г. на территории ООПТ «Чубовская степь» Кинельского района Самарской области. Данный объект исследования включает в себя 158 видов сосудистых растений, которые принадлежат к 2 отделам, 30 семействам и 104 родам. Большая часть видов относится к травам (109 видов). Среди экологических групп доминирующую роль в сложении флоры играют ксерофиты (78 видов) и мезофиты (48 видов). Фитоценотический анализ показал наличие 9 фитоценотипов, наиболее многочисленнную группу составляют степные (52 вида), лесостепные (33 вида) и горно-степные (27 видов) растения. По хозяйственно-ценным группам лидируют кормовые (61 вид), медоносные (55 видов) и лекарственные (49 видов) растения. Зафиксировано произрастание 25 редких видов. Выявлен 31 вид раритетных растений.

Abstract:

The article presents an analysis of the activities of the Department of Emergency Advisory Medical Care and Medical Evacuation of the Territorial Center of Disaster Medicine of the Republic of Crimea for the period 2019–2021 in the issues of sanitary and aviation evacuation of patients, as well as victims in emergency situations. In general, over the past three years, 207–238 departures have been carried out annually with the transportation of 203–258 patients by one full-time aviation resuscitation team of the Territorial Center of Disaster Medicine with the involvement of other specialists, if necessary. For the first time, the feature of professional activity in the «flight crew — aviation medical team» system is revealed. The training of the aircraft crew (pilot, navigator) on the basics of medical evacuation is proposed. Also, in order to optimize the activities of the specialists in aviation medical teams during the flight, it is recommended to conduct instructor trainings and simulators on the rules of operation of on-board medical equipment on the ground.

Abstract:

Unintended pregnancy is one of the major issues affecting reproductive health in Nigeria. In Isiala Ngwa North Local Government Area of Abia State, Nigeria, we noticed that one in every five pregnancies is unplanned. Many women who have mistimed pregnancies resort to abortions, which are essentially performed in dangerous conditions. Therefore, we sought to understand from 210 young married couples how effective voluntary family planning services are to curb the problem of unplanned pregnancies in Abia State’s Isiala Ngwa North Local Government Area. The framework provided by contraceptive decision theory was helpful in the thematic analysis of the data. Findings reveal voluntary family planning is effective at averting pregnancies, but there are limited contraceptive options available for individuals. Our findings indicated white-coria and osu-oji (forms of traditional contraceptives) as efficacious in preventing pregnancy. Recommendations on the effectiveness of family planning services were made.

Abstract:

Аt least in the last decade and a half the EU has put strict priorities on decarbonisation and developmennt of carbonfree technologies (mostly based on renewable energy resources) in its energy policy. Achieving this goal consists a challenge for many member-states from Central and Eastern Europe (CEU). That formed a tendency for this countries to defend nuclear power as a tool of decarbonisation. So this countries becomes a "nuclear fl ank" of the EU and has itd infl uences on forming outlines of energy and climate policy of the Union. The article describes the origin of nuclear power sector in the countries of the region and basic challenges for the sector during postsocialist transit and EU accesion. This challenges were overcome in building partnership with the players of world nuclear industry. In the same time lasting dependency from russian (soviet) nuclear technologies and general distrust of the EU toward the nuclear projects fi nanced by Russia formed an obstacle for several ambitious projects. Countries of the CEU also coordinated their positions on the role of nuclear sector in the decarbonisation and tried to fi nd poweful allies within the EU (France) to defend their stance.

Abstract:

Background: Chronic kidney disease (CKD) affects both dogs and cats, mainly elderly animals, due to tubulointerstitial inflammation associated with the increase of fibrosis through the excess deposition of extracellular matrix (ECM) which leads to decrease glomerular filtration. Many different underlying renal diseases can affect the kidneys of dogs such as congenital or acquired in origin. Therefore, the main objective of this transversal study was to evaluate the epidemiology through clinical and laboratory evaluation of 225 client-owned dogs with CKD.Materials, Methods & Results: Complete blood count (CBC), urinalysis, and biochemical profile were retrospectively selected and evaluated from 225 client-owned dogs with CKD of both sexes, different ages, and breeds from the patient population of the Nephrology and Urology Small Animal Service of the Teaching Hospital of the School of Veterinary Medicine and Animal Science - São Paulo State University from 2011 to 2017. All dogs were divided in groups according to the International Renal Interest Society (IRIS) CKD grading and statistical analysis was performed according to Kruskal-Wallis non-parametric test complemented with Dunn's multiple comparisons test, and analysis of variance for the model with a factor complemented with the test of multiple comparisons of Tukey. In this retrospective study, we observed that most dogs in all groups were elderly (≥ 9 years old). CBC demonstrated lower RBC’s (P < 0.005), hemoglobin (P < 0.001), hematocrit (Ht%) [P < 0.001] at the highest stage of the disease. However, urinary specific gravity (USG) did not demonstrate significant differences between the disease stages, but urinary protein: creatinine ratio (UPC) was statistically different (P < 0.01) between IRIS-CKD stages 1 and 4. Furthermore, serum phosphate concentrations demonstrated significantly higher levels in dogs at IRI-CKD stage 4 compared with IRIS-CKD stage 3 (P < 0.001).Discussion: The analysis of 225 dogs with CKD showed that 130 animals were elderly, older than 9 years, and according to previous studies, 15% of dogs over 10 years of age are diagnosed of CKD, presenting significant morbidity and mortality. Laboratory findings such as the presence of non-regenerative anemia is expected in dogs with CKD. In our study, the degree of anemia corresponded with the stage of the disease, similarly to serum creatinine concentrations. Another important laboratory finding in diagnosing CKD is the early detection of the kidney's abilities in concentrating its tubular filtrate. In this retrospective study, isosthenuria was not significant due to all dogs presented CKD as criteria of inclusion, especially those without azotemia, although, proteinuria was reported in 90% of the population investigated. Electrolyte imbalances are also expected in CKD. However, despite serum sodium, potassium, and total calcium did not demonstrate significant results, serum phosphate had its significance between IRIS-CKD stage 3 and stage 4. Hence, despite the age factor of most dogs in all groups, with the association of laboratory results such as serum creatinine, serum phosphorus, ionized calcium, erythrogram, isosthenuria, SBP, and the degree of proteinuria, it was possible to perform early diagnosis of CKD even in dogs with IRIS-CKD Stage 1 in a stable and hydrated patient. With the proper diagnostic, staging and substaging according to IRIS guidelines, these parameters can be monitored, predicting longevity and good quality of life, or progression of the disease with a more reserved prognosis.

Sexually transmitted infections (STIs) still continue to be a major public health problem globally, affecting the quality of life as well as causing serious morbidity and mortality. STIs have a direct and significant impact on reproductive and child health. They can directly cause infertility, cancers as well as pregnancy complications, and indirectly affects individual economies. The information on the number of PW found to be sero positive for Syphilis among women attending antenatal care in India and number of babies diagnosed with Congenital Syphilis is collected from HMIS electronic records of MoHFW, Government of India with key objective to know about the impact of covid-19 era i.e., 2020 and 2021 on prevalence of syphilis by comparing it with the pre pandemic era of 2018 and 2019. I collected sero positivity data for Syphilis among women attending antenatal care in India and number of babies diagnosed with Congenital Syphilis across 36 states and union territories of India including all health facilities viz. public, private, rural, urban health facilities. The data is collected, observed and analyzed with Microsoft office software. Among 254705.2 averages per month number of PW (pregnant women) tested using POC test for Syphilis, 2144.4 averages per month were sero positive for Syphilis giving a sero-prevalence of 0.84 % in 2021(Jan to May). Of 175579.75 averages per month number of PW tested using POC test for Syphilis, 855.58 averages per month were sero positive for Syphilis giving a sero-prevalence of 0.49 % in 2020(Jan to Dec). Of 53313.66 averages per month number of PW tested using POC test for Syphilis, 419.33 averages per month were sero positive for Syphilis giving a sero-prevalence of 0.79 % in 2019 (Jan to Dec). Of 11364.75averages per month number of PW tested using POC test for Syphilis, 339.08 averages per month were sero positive for Syphilis giving a sero-prevalence of 2.98 % in 2018 (Jan to Dec).

ستعرض هذه الدراسة بمنهجيةٍ مقارنةٍ لموضوع عدول المستفيد عن عقد المشاركة بالوقت في النظام السعودي وذلك في أربعة مباحث، يستهلها تمهيد في مفهوم المشاركة بالوقت. يتناول المبحث الأول ماهية خيار العدول عن العقد، فيعرِّفه ويكْشِف عن مسوغاته، ومن أهمها توفير الحماية الكافية للمستفيد من خلال ضمان حصول رضاه التَّام القائِم على التبصُّر والتروِّي في أمر التعاقد، ومعرفة تبعاته. ثم نحدِّد خصائص العدول؛ فهو خيار مؤقت ومجاني مصدره القانون، وهو أخيراً حقٌّ تقديري للمستفيد. ثم نُبيِّن الطبيعة القانونية لخيار العدول، وأنه رخصة منحها القانون استثناءً لضمان حماية فعَّالة للمستفيد. يكشف المبحث الثاني عن الأساس القانوني لخيار الرجوع عن العقد، وفيه حديث عن الاتجاهات الفقهية المختلفة، نختمها برأينا في ترجيح فكرة العقد غير اللازم، وهي فكرة ذات جذور إسلامية تتسق مع النظام القانوني الذي تتبناه المملكة. ويستعرض المبحث الثالث ضوابط ممارسة خيار العدول، إذ يجب أن يقع على عقد المشاركة بالوقت، وعلى وفق الشكل الذي تطلبه المُنظِّم. ثم يجب ممارسة العدول خلال المهلة المحددة. يبيِّن المبحث الرابع آثار ممارسة حق العدول، بالنسبة للمستفيد أولا ثمَّ بالنسبة للمنشأة.

Abstract:

Mortar is considered one of the construction materials in great demand in the world, obtaining materials for its preparation require the exploitation of a large part of renewable and nonrenewable resources. In order to dissipate the consumption of natural aggregates, the reuse of different materials that are normally discarded has been studied; glass is an important option for the preparation of mortar. Therefore, the objective of the present investigation was to design a mortar for masonry incorporating crushed glass. The mixing design was made for dosages of 1: 3.5, 1: 4, 1: 5 and 1: 6 with substitution of 5%, 10%, 15%, 20%, 25% and 30% of fine aggregate by crushed glass; tested at the age of 7, 14, 21 and 28 days. The fluidity of the mortar in fresh state and the mechanical properties such as the resistance to compression and bending of the mortar, resistance to adhesion by bending in masonry piles, resistance to axial compression of piles and resistance to diagonal compression in walls were evaluated. of bricklayer. The highest resistance was obtained for the dosage of 1: 3.5 with 30% as the optimum percentage of substitution, reaching a higher resistance compared to the standard mortar; for the dosage of 1: 4 the optimum percentage was 25%; for 1: 5 it was determined with 20%; and finally, 10% was obtained as the best percentage for the 1: 6 dosages. From these values it was determined that, once the optimal percentages were reached, the strength of the mortar decreases as the substitution increases. The results obtained allowed to conclude that the crushed glass significantly influences the mortar properties.

يدرس البحث حديث أَبِي الدَّرْدَاء  "فضل إصلاح ذات البَيْن" برواية أبي داود السجستاني، وهو حديث فيه حثٌّ على إصلاح ذات البَيْن واجتناب الإفساد فيها، وقد اعتمدت في دراسته المنهج التحليلي، بهدف الوقوف على أهم مسائل الإسناد والمتن، والتعرف على منهج أبي داود في الحديث، واستنباط أهم الدروس والأحكام المستفادة منه، وقد قسَّمت البحث إلى مبحثين: الأول الدراسة التحليلية لمسائل الإسناد، والثاني الدراسة التحليلية لمسائل المتن، وضمنّت كل مبحث مطالب عدّة، وخلُصت إلى أهم النتائج والتوصيات؛ ومن النتائج: يُعدُّ إصلاح ذات البين من أفضل الطاعات المتعدية بيّنما يُعدُّ إفساد ذات البين من أعظم المعاصي المتعدية، إنَّ إصلاح ذات البَيْن سبب للاعتصام بحبل الله تعالى، بيّنما فساد ذات البَيْن ثلْمَة في الدين، وإنَّ المشتغل في إصلاح ذات البَيْن واجتناب فسادها ينال درجة فوق ما يناله الصائمُ القائمُ المُشتغلُ بِخُوَيْصَةِ نفسه، ومن التوصيات: الاهتمام بالدراسات التحليلية لبعض الأحاديث التي تلامس الواقع وتعالج مسائل اجتماعية.

هذا البحث يتناول موضوعاً، -إن لم يكن أهم- فهو من أهم المواضيع التي خاض فيها الباحثون والفلاسفة قديماً وحديثاً، واختار الباحث له عنواناً ( مصادر العلم والمعرفة بحقائق الوجود – دراسة قرآنية- ) ، وقد عالج الباحث فيه في المبحث الأول مصادر العلوم والمعارف في القرآن الكريم، وهي أنواع، منها ما يعتمد على وسائل بشرية، وهي (العقل، السمع، البصر، اللمس، الذوق، الشم)، ومنها ما لا يعتمد على القدرات البشرية، وهي (الهداية الفطرية والإلهام الرباني)، ونوع ثالث: وهو (الوحي الإلهي) الذي يأتي من الله سبحانه عن طريق الأنبياء. وفي المبحث الثاني: عقد الباحث مقارنات بين نظريتين (نظرة الإسلام والنظرة المادية) لمصادر العلم والمعرفة في القرآن الكريم. وفي نهاية البحث خرج الباحث بنتائج أهمها: أن النظرية الإسلامية والنظرية المادية تتفقان على مصادر العلم والمعرفة البشرية من حيث الأصل –مع خلاف في النظرة إلى هذه المصادر-، باستثناء مصدر (الوحي الإلهي)، الذي تنكره النظرية المادية من أساسه، بينما تعتبره النظرية الإسلامية أهم المصادر وأرقاها.

Thyroid disease commonly affects nervous system which can be easily recognized when there are frank signs of thyroid dysfunction. The clinical spectrum of hypothyroidism ranges from sub-clinical hypothyroidism to frank myxoedema coma in severe hypothyroidism. It is necessary to consider thyroid diseases in differential diagnosis when neurological manifestation express itself as unexplained encephalopathy, progressive cerebellar degeneration, polyneuropathy, psychosis or progressive cognitive decline and even dementia. Due to protean non-specic diverse manifestations, hypothyroidism may remain undiagnosed for many years hence proactive search and early replacement therapy helps in better therapeutic outcome. Congenital Hypothyroidism is potentially preventable and correctable form of metabolic cerebral disease and mental retardation necessitating neonatal screening for early detection and treatment.

تناولت الدراسة أثر السياق القرآني في انفرادات سورة البقرة مع نفسها، فالقرآن الكريم انفرد بألفاظ خاصة ببعض السور القرآنية للمتشابهات اللفظية تميزها عن غيرها من السور الأخرى دون الوقوع في الخلل والتباين، ثمَّ الحديث عن تعريف انفرادات السورة لغةً واصطلاحًا، وأهمية سياق الآية في فهم معاني القرآن، ثُّمَّ الدراسة التطبيقية لانفرادات سورة البقرة مع نفسها مع حصر المواضع القرآنية فيها، ودراستها، وتوجيهها، فلكل انفراد مقصد يختلف عن مقصد ذكره في موضع آخر للمُشابه له. فيأتي كل لفظ مناسب لما يجاوره من ألفاظ، فلكل مقام ما يناسبه، ولكل حال ما يلائمها.

Abstract:

Necrotizing fasciitis (NF) is an entity that is part of the spectrum of necrotizing skin and soft tissue infections (NSTI), characterized by extensive and rapidly progressive tissue destruction, accompanied by signs of sepsis. It is classied as polymicrobial (type I) and monomicrobial (type II). The incidence is estimated at 0.3-15 cases per 100,000 inhabitants. Early signs of the disease include swelling, pain, and erythema. Early diagnosis is imperative, which is why prediction scales have been created, with LRINEC being the most widely used. Treatment has two fundamental pillars: early surgical debridement to control the septic focus and broad-spectrum antibiotic therapy. However, despite advances in the diagnosis and treatment of FN, its mortality continues to be around 25-35%.
