Abstract:

Bakgrund: Verbal och icke verbal kommunikation är ett utbyte av information eller känslor mellan två personer. Inom omvårdnadsarbetet är kommunikation ett viktigt redskap att använda sig av som sjuksköterska. Syfte: Att belysa verbal och icke verbal kommunikation mellan sjuksköterska och patient vid det akuta omhändertagandet av patienten. Metod: En allmän litteraturstudie med systematisk artikelsökning samt innehållsanalys av artiklarna. Resultat: Det finns både positiva och negativa sätt att kommunicera. Sjuksköterskorna var väl medvetna om vikten av god kommunikation, men trots detta visade det sig att det fanns påtagliga brister i omvårdnaden vid det akuta omhändertagandet. Sjuksköterskans sätt att kommunicera påverkades av många faktorer, bland annat patientens tillstånd och bakgrund samt störande faktorer som tidsbrist och telefon. Slutsats: Det tycks finnas ett behov av utökad utbildning och träning inom kommunikation för samtliga som arbetar inom omvårdnad. Dessutom bör forskningen inom kommunikation utökas.

Abstract:
This investigation examined the impact of a potent source of social influence—the placebo—on verbal and nonverbal pain expression. Placebos exert a powerful influence on pain in both clinical and laboratory settings; nevertheless, the placebo remains a poorly understood phenomenon. In the present study the placebo was employed as a social influence designed to alter subjects' expectations or beliefs concerning their response to a noxious event. Furthermore, an attempt was made to classically condition a placebo response in accordance with the conditioning theory of placebo. The pain stimulus consisted of 500-millisecond electrical shocks. The placebo was an inert white cream described as an effective and quick-acting local anaesthetic. To gain a broad assessment of verbal and nonverbal pain expression, two self-report measures and an objective behavioural measure—facial expression—were used. Self-report comprised the "Sensory" and "Unpleasantness" ratio scales (derived from Gracely, McGrath, & Dubner, 1978). Facial expressiveness was coded using the Facial Action Coding System (FACS) developed by Ekman and Friesen (1978). Psychological interventions have been said to primarily alter patients' affective response to pain, thus it was expected that the greatest impact of the placebo would be reflected by the Unpleasantness Scale. The Sensory ratings and facial activity were expected to reflect less of a change. Sex differences were analyzed but were not expected because the pain stimulus intensity was individualized. Anxiety ratings taken before the trials with placebo were expected to be lower than anxiety ratings before the no-placebo trials; this was expected to be positively related to a placebo response (defined as lower pain expression during placebo trials than during no-placebo trials). In the baseline phase, subjects were tested with and without the placebo. They received the expectation manipulation that the analgesic cream would act as a local anaesthetic. Next, to condition a placebo response, in the positive conditioning group shocks administered with placebo cream were surreptitiously decreased; in the negative conditioning group shocks were increased; and in the baseline (control) group shocks remained at baseline levels. A final return-to-baseline phase tested the conditioned placebo response. A significant placebo response was found to be most clearly represented by the self-report measures and less by the facial activity. Control group subjects, who were exposed only to the verbal directive that they were receiving an analgesic cream, demonstrated a clear placebo response on both the Unpleasantness and Sensory self-report scales. Thus both self-report measures reflected a substantial placebo effect. Nonverbal expression reflected an effect only with half of the control group—the males—who exhibited significantly less facial activity during the placebo trials compared to the no-placebo trials. However, neither of the two groups exposed to manipulated shock levels demonstrated a significant conditioned placebo effect on any dependent measure. There was a modest relationship between facial activity and self-report. Self-reported anxiety was somewhat predictive of the degree to which subjects rated placebo trials as less painful than no-placebo trials, but the direction of this relationship was opposite to that hypothesized. In brief, subjects led to expect an analgesic cream, but not exposed to the conditioning manipulation, reliably exhibited a placebo response on both self-report scales; the males in this group also exhibited a placebo response as measured by facial activity. No support was found for the conditioning theory of placebo effects.

Abstract:
Objective: To explore the utility of cognitive tests, that did not rely on spoken language from participants for decision making about eligibility for treatment with Anti-cholinesterase Inhibitors (AchI). Methods: A cross-sectional design was used. Data was collected from 20 participants, aged 65 to 90 years (M, 77.6: SD, 7.2), with dementia, or cognitive difficulties. The sample comprised of eleven males and nine females. The Mini-Mental State Examination (MMSE), and six alternative cognitive tests were administered, these were; the Rey Complex Figure Test (RCFT), Ravens Colour Progressive Matrices (RCPM), Symbol Digit Modalities Test (SDMT), Brixton Test, Clock Drawing Test (CDT), and Colour Trails Test (CTT). Results: There was statistically significant correlations between the MMSE and the following cognitive tests: RCFT visual construction subtest(r= .609; P<.006), the RCFT recognition subtest (r= .496; P<.031), RCPM (r= .452; p<.045), the SDMT (r=.670; P<.001), the CTT 1 (Rho=-.576; P<.012) and the CDT (r=-.577; P<.008). The area under the ROC curve values were as follows: RCFT visual construction (0.750, 95%, CI .524 - .976), RCFT recognition memory (0.801, 95%, CI .590 – 1.012), RCPM (0.573, 95%, CI 0.298 – 0.848), SDMT (0.708, 95%, CI 0.469 – 0.947), CTT1 (0.818, 95%, CI 0.610 – 1.027) and the CDT (0.734, 95%, CI 0.479 – 0.990). Cut-off scores with adequate sensitivity and specificity were identified for all the above measures apart from the RCPM, which had predictive accuracy that was equal to chance. Conclusions: Cognitive tests which do not require spoken language have utility in differentiating between those who are and are not eligible for treatment with AchI, as defined by the cut-off (<20) on the MMSE in the National Institute for Clinical Excellence guidelines (NICE, 2007).

Abstract:
This study examines specific speech forms that comprise physicians' language use, and motives for use. A coding system combining work on verbal immediacy and conversational involvement was used to assess the language of 19 physicians during 58 videotaped interactions with patients. Physicians were found to use more nonimmediate than immediate speech. Information-giving was positively related to use of nonimmediate speech. Use of implicit nonimmediacy was positively related to physicians' perceptions of the medical community's consensus regarding a patient's condition and recommendations for treatment. Experience was positively related to use of spatial nonimmediacy and automatic phrases. Gender and experience interact to predict use of temporal, implicit, and qualified nonimmediacy. Inexperienced males used the least of these forms of speech, while experienced males used the most. No relationship was found between use of nonimmediate speech and patients' understanding, satisfaction, or met expectations. Implicit nonimmediacy was directly related to patients' behavioral intent to comply. Findings are reviewed for implications to both Communication and Medicine.

Data on adult mortality are very limited in sub-Saharan Africa where only small proportions of deaths occur in health facilities. In such settings, ascertainment of causes of death from data obtained from relatives or associates of the deceased through interviews in surveys or longitudinal surveillance systems appears to be an attractive option. This technique, known as verbal autopsy (VA) is based on the assumption that important causes of death have distinctive symptoms and signs, and these can be recognised, remembered and reported by lay respondents, and that based on the reported information causes of death can be reached. The existing experience of VA for adult death is limited mainly to maternal deaths and the validity of VA for adult death is unknown. We developed a VA questionnaire, mortality classification system and "expert opinion" based algorithms for reaching diagnoses for adult deaths and tested their validity on deaths occurring at hospitals in Tanzania (n=315), Ethiopia (n=249) and Ghana (n=232). Hospital records of adult deaths occurring at study hospitals from June 1993 to April 1995 were collected prospectively. VA interviews were conducted by trained non-medical interviewers. Caused of death from VA data were reached by a panel of three physicians and by a computerised algorithm. The validity of VA was assessed by comparing the VA diagnoses with hospital diagnoses. Specificity of VA fell below 95% only for few common causes of adult death. Sensitivity and kappa of VA for all common causes of adult death were low and this suggests that the accuracy of VA at the individual level is low. However, the misclassification of causes of death was bi-directional and the number of false positive and false negative diagnosis for most common causes of adult death tend to be similar. Thus there was robust agreement between the true and VA estimates of cause specific mortality fractions of common causes of adult death and VA is useful for assessing cause specific mortality fractions of common causes of adult death.

Enligt Arbetsmiljölagen har arbetsgivaren en skyldighet att tillämpa nödvändiga åtgärder för att förebygga eventuella olycksfall eller komplikationer som kan påverka arbetstagarens hälsa.Syfte: Syftet med denna litteraturstudie var att sammanställa vad verbal misshandel riktat mot sjuksköterskan från patient och dess anhöriga kunde ge för effekter på sjuksköterskan och omvårdnadsarbetet.Metod: Litteraturstudie av kvalitativ ansats innehållandes 10 artiklar. Databassökning gjordes i databaserna CINAHL, PsycINFO och PubMed. Innehållsanalys av artiklarna tillämpades enligt Forsberg & Wengström.Resultat: Fyra teman presenteras i resultatet: Avsaknad av effekter, Emotionella effekter på sjuksköterskan samt Effekter på omvårdnadsarbetet.Konklusion: Verbal misshandel bidrar till en ohälsosam arbetsmiljö som medför negativa effekter både för sjuksköterskan och för patienten. Background: Violence is a recurring problem, especially among health professionals which have a considerable high risk of exposure. ICN has reported and described how verbal abuse has the same level of affect and trauma as physical abuse. According to the labour legislation the employer has a binding obligation to take necessary measures to prevent accidents and complications that may have an adverse effect on the health of the employee.Aim: The purpose of this literature review was to compile the effects of verbal abuse towards the nurse by patient and relatives, and how it affected the nursing care. Method: Literature review with a qualitative approach containing 10 articles. The database searches were made in CINAHL, PsycINFO and PubMed. The articles were analyzed according to a content analysis by Forsberg & Wengström.Result: Four themes are presented in the result; Absence of effects, Emotional effects on the nurse, effects on the nursing care. Conclusion: Verbal abuse contributes to an unhealthy work environment which involves negative effects both regarding the nurse as well as the patient. Introduktion: Bemötande och kommunikation är centrala delar inom vården. Under ett möte avgör kommunikationen till stor del behandlingens utfall. Denna studie gjordes i Kenya. Kenya är ett utvecklingsland där fysioterapi är ett lågprioriterat yrke jämfört med andra vårdyrken. Inom landet finns det både positiva och negativa aspekter inom yrket som kan påverka patientens tillfredsställelse av mötet. En negativ faktor är brist på resurser och en positiv att patienterna känner sig omhändertagna och sedda av vårdgivarna. Kommunikation utgörs av både verbal och icke-verbal kommunikation vilka är viktiga för att åstadkomma en god relation i ett möte mellan fysioterapeut och patient.  Syfte: Syftet med studien var att undersöka hur mötet sker mellan fysioterapeut och patient på ett lokalt sjukhus i Kenya. Metod: För att besvara studiens syfte och frågeställningar gjordes en observationsstudie. Utifrån ett observationsprotokoll gjordes datainsamlingen. Författarna befann sig på ett lokalt sjukhus i Kenya under nio dagar. Deltagarna som inkluderades var fyra fysioterapeuter och patienter från 10-års ålder med besvär från rörelseapparaten. Resultat: 22 möten inkluderades. Observationerna resulterade i att mötet mellan fysioterapeut och patient i Kenya var patientcentrerat. Fysioterapeuterna använde sig av verbal kommunikation i form av dialog och lugn samtalston. De stämde av och var förklarande. Inom den icke verbala kommunikation förekom faktorer som ögonkontakt, vänd mot patient och aktivt lyssnande. Fysioterapeuterna visade även ett intresse samt skrattade med patienterna. Konklusion: Studien visar att trots sparsamma förutsättningar och minimal utrustning kan ett gott resultat uppnås vid fokus på bemötande.

I varje kultur spelar kommunikationen en stor roll. Kommunikation är en process som både sändare och mottagare deltar i och omfattar såväl verbal som icke- verbala delar. En person kan visa känslor och attityder med kroppsliga uttryck om han eller hon inte kan använda språket. När det finns en brist i förståelsen mellan patient och vårdare kan detta påverka relationen mellan vårdare och patient och därmed också vården av patienten. Syftet med denna litteraturstudie är att beskriva faktorer som kan vara hindrande i kommunikationen mellan vårdare och patient med annan kulturell bakgrund, utifrån vårdare och patient perspektiv. Uppsatsen är litteraturbaserad och vetenskapliga artiklar har använts. Sex artiklar har analyserats. I resultatet framkom tre teman; Språk som barriär, brist i kunskap om andra kulturer, patientens kulturella bakgrund. I diskussionen diskuterar författaren sina fynd och drar slutsatsen att vårdgivare behöver mer kunskap och kontinuerlig information om patienternas kulturella bakgrund. De behöver också reflektera över hur detta kan påverka vårdrelationen.

The Deep Brain Stimulation (DBS) for the treatment of Parkinson's disease (PD) involves the implantation of four microelectrodes in the subthalamic nucleus (STN). Although usually safe from a cognitive point of view, the SNT DBS has been linked to a decrease in verbal fluency (VF) in nearly all studies comparing preoperative to postoperative period. The decline can be attributed to surgical effects, but the real contributions of the stimulation frequency effects are not known. This research aims to investigate the impact of the frequency of SNT DBS in the performance of VF in patients with PD, comparing the VF and the motor performance at low frequency (60 Hz) and high-frequency (130 Hz), assessing the quality of life and correlating the demographic factors such as age and educational level and reading and writing habits with VF. It is a randomized double-blind experimental study. The initial sample consisted of 25 individuals, but 5 were excluded for not meeting the inclusion criteria. Thus, 20 subjects were included in the sample, from 30 to 75 years of age (56.7 ? 10.7), with average schooling 10.1 (? 5.2), and 16 were males and 4 females. Participants were evaluated with phonetic and categorical VF tasks, motor scale (UPDRS III), quality of life questionnaire (PDQ-39) and reading and writing habits. The evaluation of motor and VF was assessed after one hour from the frequency setting for high (130 Hz) and low (60 Hz), the frequency adjustment order being randomized. Patients with PD who underwent SNT DBS showed a decrease in phonemic VF in high frequency. There was no correlation regarding age and educational level of participants when compared to VF. The high frequency (130 Hz) of the SNT DBS caused a decline in VF and there was no correlation with socio-demographic factors such as age and educational level.

Bakgrund:  Kommunikationen mellan sjuksköterskorna och patienterna spelar en avgörande roll för att patienterna ska kunna förstå samt uppleva delaktighet kring sin vård. Som legitimerad sjuksköterska finns ett ansvar att kommunikation sker på ett respektfullt, lyhört och empatiskt sätt gentemot patienterna för bästa möjliga vård. Sjuksköterskornas förmåga att kommunicera vid informationsgivning är en förutsättning för en personcentrerad vård. Syfte: Studiens syfte var att beskriva patienters upplevelser av sjuksköterskors verbala kommunikation i omvårdnadshandlingar. Metod: En litteraturöversikt med induktiv ansats baserad på nio vetenskapliga artiklar med kvalitativ metod. Vid analys av data användes en kvalitativ innehållsanalys beskriven av Graneheim och Lundman. Resultat: Resultatet visar två huvudkategorier; främjande kommunikation och icke främjande kommunikation. Under huvudkategorin främjande kommunikation visas underkategorierna individanpassad kommunikation och vikten av en god vårdrelation. Under huvudkategorin icke främjande kommunikation bildades underkategorierna otydlig kommunikation och uppgiftsorienterad kommunikation. Slutsats: Kommunikation är en viktig del i omvårdnadsarbetet och det sker dagligen otaliga möten mellan sjuksköterskor och patienter inom vård- och omsorg. För att patienterna ska uppleva kommunikationen som främjande är det viktigt att sjuksköterskorna arbetar på ett personcentrerat sätt och individanpassar kommunikationen. Med en god kommunikation känner sig patienterna sedda och förstådda.
