Abstract:
Providing career development services, through career counseling and assessment, is part of vocational rehabilitation programming. However, there is no applied evidence that such career development services are feasible or accepted among individuals with psychiatric disorders. We examined feasibility (acceptability, demand, and perceived need) of the Vocational Evaluation Center (VEC), one veterans affairs (VA) hospital’s method of career development services for veterans with psychiatric disorders. Demographics, referral source, and service utilization were analyzed among 90 veterans referred to the VEC. Qualitative analysis identified patterns to veterans’ reasons for seeking VEC services. Veterans referred to the VEC were predominately unemployed and disabled. Veterans tolerated the intervention well, with 16.7% dropping out. Reported needs for VEC services included (a) vocational uncertainty, (b) functional considerations in vocational planning, and (c) finding purpose. Veterans with psychiatric disorders want career development services. The VEC model appears feasible, well-tolerated, and aligned with consumers’ needs.

Abstract:
A new networking paradigm, Vehicular Edge Computing (VEC), has been introduced in recent years to the vehicular network to augment its computing capacity. The ultimate challenge to fulfill the requirements of both communication and computation is increasingly prominent, with the advent of ever-growing modern vehicular applications. With the breakthrough of VEC, service providers directly host services in close proximity to smart vehicles for reducing latency and improving quality of service (QoS). This paper illustrates the VEC architecture, coupled with the concept of the smart vehicle, its services, communication, and applications. Moreover, we categorized all the technical issues in the VEC architecture and reviewed all the relevant and latest solutions. We also shed some light and pinpoint future research challenges. This article not only enables naive readers to get a better understanding of this latest research field but also gives new directions in the field of VEC to the other researchers.

Abstract:
The rising importance of service for the global economy has resulted in a significant number of efforts to define and interpret its meaning. In this vein, this article identifies and organizes related literature following a holistic approach. The authors study service from different perspectives, both business-related and technical ones, e.g., marketing, operations management, and computer science. A total of 47 definitions of service are finally reviewed. They’ve found out: (1) How is service perceived and defined by different disciplines and (2) What are the common service species according to related literature. The authors observe that in the business realm service falls under either of the following two categories: service as a set of value-creating activities and service as a transformation. Due to ICT revolutionizing service provision, significant work has also been conducted in order to study technology-enabled services. The authors identified different types of technology-enabled services, namely e-services, SOA services and Web services. The main contribution of this article lies in the broadness, completeness and cross-disciplinarily of the literature covered, thus being able to stand as a reference source of knowledge for service analysts, engineers and practitioners.

Abstract:
Advancement in the information and communication technology make possible the development of e-government. The Indonesian government already grasped the opportunity to develop its own e-government system in providing public services. The obvious intent is to increase the quality of public service provision by the bureaucracy. This is hoped to be achieved by securing transparency and accountability of public officials. It is also to be expected that e-governance will be able to eradicate or at least decrease the possibility of corruptive behaviour. Unfortunately, fact speaks differently. Cases to be analysed here are: corruption committed by public officials working at the Investment and One Stop Service Office of the Bandung Municipality and those performed by public officials managing e-procurement services. In particular, this article shall discuss the issue how to develop e-government as to better at eradicating and preventing corruption by public officials.

Vehicular Edge Computing (VEC), based on the Edge Computing motivation and fundamentals, is a promising technology supporting Intelligent Transport Systems services, smart city applications, and urban computing. VEC can provide and manage computational resources closer to vehicles and end-users, providing access to services at lower latency and meeting the minimum execution requirements for each service type. This survey describes VEC’s concepts and technologies; we also present an overview of existing VEC architectures, discussing them and exemplifying them through layered designs. Besides, we describe the underlying vehicular communication in supporting resource allocation mechanisms. With the intent to overview the risks, breaches, and measures in VEC, we review related security approaches and methods. Finally, we conclude this survey work with an overview and study of VEC’s main challenges. Unlike other surveys in which they are focused on content caching and data offloading, this work proposes a taxonomy based on the architectures in which VEC serves as the central element. VEC supports such architectures in capturing and disseminating data and resources to offer services aimed at a smart city through their aggregation and the allocation in a secure manner.

Mobile cloud service is the most important traffic generator today, and high data rate services in this area are increasing. The Virtual Desktop Infrastructure (VDI) is a circuit-like service that is widely deployed owing to its security merits and the resource constraints of mobile devices. However, VDI requires a high and guaranteed virtually constant wireless data rate. Thus, we focus on the problems that have to be resolved in order for IEEE802.21 Media Independent Handover (MIH) to support a guaranteed wireless data rate and also enable it to offer optimized and maximized data rate support for mobile cloud services. We also consider a virtual machine (VM) migration scheme that is tightly coupled to mobile networks and can minimize the user's service delay and reduce the bandwidth requirements of the wired network. We then present a consistent control framework by extending the Global Environment for Network Innovations (GENI) control framework to provide a single integrated control framework for cloud computing and Future Internet compatibility. Finally, we develop an experimental testbed using a mobile cloud computing environment and evaluate the performance of the proposed scheme. We show that the proposed method can enhance the Quality of Experience (QoE) of mobile cloud computing services with increased data rate support and a single consistent control framework that is compatible with the GENI-based Future Internet architecture. The results of evaluations conducted indicate that our proposed method provides 24%–41% better data rate compared to conventional methods.

Ecosystem services are studied by using many different approaches and methods. Here we present a community approach with a combination of fieldwork methods and sociological surveys (questionnaire interviews and semi-structured interviews) in the assessment of mangrove ecosystem services. There were 198 households in 8 communes in 3 coastal districts of Nghe An province selected for the survey. We find that the basic mangrove ecosystem service values in the locality have identified by communities, including 17 types of services belonging to 4 groups: provisioning services, regulating services, supporting services and cultural services. The recognition and use of ecosystem services vary by location and occupation in the study area. The fishing group gives detailed information about the provisioning services, in contrast, the shrimp farming and agricultural production groups are interested in regulating and supporting services. According to space, mangroves in Quynh Luu district are assessed to have the highest potential for ecosystem services, followed by Dien Chau district, the lowest is Nghi Loc district. In addition to the potential for ecosystem services, mangroves are facing many threats of decline, placing responsibilities on managers and people in protecting and rationally using mangrove resources.

Borobudur Temple is one of the world cultural heritage owned by the Indonesian nation. The existence of this Borobudur temple has an impact for the surrounding community, one of which led to new tourist destinations. This study aims to analyze the tourism potential of the area around Borobudur Temple in the concept of sustainable tourism. This research was conducted by using life cycle assessment perspective on tourism activity in the district level, including: the primary inputs (resources), process and outputs. The data collected from one of the Village Economic Chambers in the districts surrounding the Borobudur Temple regions, named Borobudur VEC. The Borobudur VEC has provided two products and services: tempe mendoan and lodging services. Those two business products have different characteristics. The tempe mendoan business process requires more raw material than the lodging service business process. Based on the result, emission tempe mendoan production is more than from lodging services.

Objectives In light of evidence of national variability in service commissioning of varicose vein intervention, our aim was to evaluate the current state of primary care trust commissioning for all forms of varicose vein intervention in England. We also sought to clarify the extent to which access to endovenous and surgical varicose vein services is being restricted. Methods Under the Freedom of Information Act (2001), a structured email survey was sent to 108 primary care trusts in England. Trusts were asked how many elective endovenous laser therapy and open procedures were commissioned from 2008 to 2011 and they were asked to submit their commissioning policy for analysis. The ‘qualifying criteria’ expressed in each policy were analysed by theme and geographical region. Results Of 108 surveys, 95 (88%) were completed and returned. Of these, 91 (96%) stated that varicose vein interventions were actively commissioned. Eighty-eight (97%) of primary care trusts that commissioned varicose vein interventions stated that access was restricted. Qualifying criteria varied considerably between regions. Conclusions Access to varicose vein intervention appears to be restricted, with national variation in commissioning across England. This might have an impact on patient care and surgical training. We propose that a national decision be made about which varicose vein patients should be offered funding for treatment on the National Health Service.

Each year the Australian Journal of Primary Health devotes one issue to a special theme under the auspices of a guest editor. In 2006 the focus of the special issue will be on community-based care. Since the 1960s the aspiration to community-based care has become increasingly important for users of services as well as policy-makers. The evolution of community-based care has been influenced by deep currents of change in Western societies. Technical developments in areas such as dialysis technology and pharmaceuticals have made some kinds of complex care in community settings feasible. The evolution of market-based service frameworks and the "consumer society" have strengthened arguments that services should adapt to consumer aspirations by providing more services in communities familiar to their users. In a similar vein the drive for efficiency in public service provision has motivated policy-makers to explore ways of reducing the cost of services whilst striving to maintain quality. The social justice argument has been fundamental to deinstitutionalising people with complex needs.

Abstract:
The aim of this study was to describe the case report of a 33-year-old girl with portal vein thrombosis. J.S.D.F., 33 years old, female, previously healthy, with a personal and family history negative for thromboembolic phenomena, without comorbidities, started with severe abdominal pain in the mesogastric region, radiating to HCD and back, associated with vomiting. Smoking active for about 20 years and using OC for 2 years. In view of the symptoms, she sought emergency services, where the first suspicion would be pancreatitis. Initial tests plagued leukocytosis. GUS of the total abdomen with doppler and CT of the abdomen showed the presence of thrombus in the right portal vein, with no other alterations. Rocefin® and metronidazole as an initial measure in the service began in addition to full coagulation with Clexane®. It showed clinical and laboratory improvement, with a reduction of leukocytes. The patient remained hospitalized in a hospital for 22 days to investigate the etiological cause. After of hospitalization, the patient was released with medical discharge, with guidelines for seeking the hematology service. During the investigation consultations, the results of the requested tests were negative for the diagnostic suspicions of nocturnal paroxysmal hemoglobinuria and chronic myeloproliferative disease. After 6 months of follow-up, she was discharged from the hematology service because she did not present a suspected disease and classified the cause of thrombolytic event as idiopathic associated with the combined use of OC and smoking.

Abstract Abstract 4751 Introduction Subspecialist Thrombosis Consultation Services for the emergency department and hospital inpatients are not commonly offered, even in tertiary level care centers in Canada. Since 2006, the Jewish General Hospital, a McGill University teaching hospital in Montreal, Canada, has offered a subspecialist Thrombosis Consultation Service overseen by experienced Thrombosis Physicians. Additionally, a one year clinical Thrombosis Fellowship is offered to train physicians in Thrombosis medicine. In order to assist centers planning to start a subspecialist Thrombosis Consultation Service and to aid in the planning of future Thrombosis Fellowship curricula we performed a retrospective descriptive study of the Thrombosis Consultation Service at the Jewish General Hospital. Methods We reviewed all consultation requests to the Thrombosis Consultation Service at the Jewish General Hospital from July 2010 to June 2011. Copies of the completed written consultation sheets were reviewed in order to extract data on patient characteristics, reason for request, requesting service, and drugs and interventions suggested by the Thrombosis consultant. Results are presented descriptively. Results The total number of written consultation sheets reviewed was 504, corresponding to an average of 42 consults seen per month. Approximately 20 additional consultation sheets could not be retrieved. Most consultations were requested by Surgical services (27.8%, n= 140), the Emergency Department (27.4%, n=138) and Internal Medicine wards (25.0%, n=123). The remaining requests were from Oncology services (6%, n=30), the Intensive Care Unit (4.4%, n=22), Family Medicine (4%, n=20), Obstetrics (2.4%, n=12) and other clinical services (2.2%, n=11). In 1.4% (n=7) the requesting service was not recorded. The baseline characteristics of the patients are shown in Table 1. Forty nine percent (n=248) of the consultations were for assessment of a patient with acute thrombosis as the primary reason for the consultation. The distribution of the site of thrombosis included 33.% (n=83) with pulmonary embolus with or without deep vein thrombosis (DVT), 24.6% (n=61) with lower limb proximal DVT, 14.5% (n=36) with distal DVT, 8.5% (n=21) with unusual site DVT, 8.1% (n=20) with arterial thrombosis, 5.7% (n=14) with upper extremity DVT, 4.8% (n=12) with superficial vein thrombosis and 0.4% (n=1) with extracorporeal thrombosis of a dialyzer machine. Consultations to help manage bleeding associated with oral anticoagulant therapy (OAT) accounted for 6% (n=31) of consult requests. Other reasons for consultation included dosing of OAT (6.75%, n=34), anticoagulant bridging therapy (9.7%, n=49) and thromboprophylaxis (4.4%, n=22). Six percent (n=29) of the consultation requests were to exclude Heparin Induced Thrombocytopenia (HIT), of which a total of 4 cases were documented as confirmed HIT (0.8%). Conclusion The Thrombosis Consultation Service at the Jewish General Hospital received more than 500 consultation requests in a one year period between 2010 and 2011. These consultations were requested from a diverse array of clinical disciplines, involved adult patients of varying ages and pertained to both venous and arterial thrombotic disorders. The cases reviewed provided rich teaching material for fellows and resident trainees rounding on the service. Given the large volume of consultations requested, the emerging array of available anticoagulant drugs and the increasing complexity of cases reviewed (e.g. cancer patients, older patients with multiple comorbidities, bridging issues), there appears to be a growing need for specialized thrombosis services and formalized training of thrombosis medicine specialists. Abstract:
Purpose – The purpose of this paper is to investigate the perceptions and satisfaction between police officers and citizens on Kinmen Island (of Taiwan), using an analytical framework that combines five primary dimensions and 25 corresponding determinants. Design/methodology/approach – It considers recent developments in the application of performance measures and management to public and, particularly, police services. It goes on to assess comparative rural police performance which suggests that the application of targets could ultimately serve to provide the overall satisfaction with police services. Findings – The findings of the study suggest that perceptions and satisfaction of both citizens and police staff may result in the success of the policing management, and that police managers have to satisfy their citizens with a high level of service quality based on different localities. Practical implications – From managerial perspectives, police managers should consider both the service quality and customer satisfaction constructs as determinants of behavioural intentions, based on the fact that satisfaction can be a strategic key to maintain long term relationship with citizens as it is found of significant impact on the intentional behaviour. Originality/value – This research adds empirical support to this vein of literature and has identified the five main dimensions and the 25 sub-dimensions as important constructs for police service quality.

With the development of information technologies and digital solutions, desire of people to receive services electronically increases – whether it is a domestic or an electronic service offered by state or municipal authorities. In order to receive services of authorities remotely, a person needs to be identified electronically. The most important components of a person’s successful electronic identification are the fulfillment of both parties’ obligations on personal data security issues and the ability of the service provider to technically secure the person’s electronic identification. The Author of the paper has analysed types of person’s electronic identification, their characterisation and legal aspects, evaluating legal regulation, available literature and current court practice in electronic identification issues. The article is based on the author’s Master’s thesis, which explored the risks of electronic identification of a person and the issues of legal regulation.

Objectives Since 2009 the National Health Service (NHS) has been collecting patient-reported outcome measures (PROMs) following varicose vein interventions. The objective of this manuscript was to interrogate the one-year PROMs data with respect to varicose vein intervention and to discuss its potential impact on the provision of service. Methods We interrogated the one-year PROMs data with respect to potential impact on the provision of service. Results In total, 37,521 varicose vein operations were performed during the study period (1 April 2009–30 April 2010). A total of 15,808 preoperative questionnaires were completed and returned and 12,509 were linked to Hospital Episode Statistics episodes. A total of 8127 postoperative questionnaires were completed and linked to the preoperative questionnaires. For the EuroQuol (EQ) 5D questionnaire average preoperative score across the completed data-set was 0.773. The postoperative health gain was 0.094. Fifty-three percent of patients had improved postoperative scores, 33% reported no change, while 14% reported a reduction in their postoperative EQ-5D score. Pain/discomfort was the only domain where patients reported any negative symptoms; 72% (5390) reported pain/discomfort preoperatively and 37% (2804) postoperatively. In total, 62.6% (4685) reported no pain/discomfort following surgery A total of 7167 complete, linked and eligible data-sets were observed for the Aberdeen Varicose Vein Questionnaire (AVVQ). The average preoperative score was 18.75. The average postoperative score was 10.76. This represented a reduction in symptom scores of one-half following intervention. The lowest improvements were witnessed in patients with the lowest preoperative scores (least severe symptoms). Additional postoperative questions highlighted the improvements conferred from varicose vein surgery with 90.3% of patients reporting an improvement in their problems from varicose veins following surgery and 85% describing their operative results as excellent, very good or good. Conclusion These data have shown variable improvements following venous interventions. PROMs are likely to have significant implications for health care in the NHS on a number of levels including provision of funding and future planning of services.

Most existing shared automated mobility (SAM) services assume the door-to-door manner, i.e., the pickup and drop-off (PUDO) locations are the places requested by the customers (or demand-side). While some mobility services offer more affordable riding costs in exchange for a little walking effort from customers, their rationales and induced impacts (in terms of mobility and sustainability) from the system perspective are not clear. This study proposes a demand-side cooperative shared automated mobility (DC-SAM) service framework, aiming to fill this knowledge gap and to assess the mobility and sustainability impacts. The optimal ride matching problem is formulated and solved in an online manner through a micro-simulation model, Simulation of Urban Mobility (SUMO). The objective is to maximize the profit (considering both the revenue and cost) of the proposed SAM service, considering the constraints in seat capacities of shared automated vehicles (SAVs) and comfortable walking distance from the perspective of customers. A case study on a portion of a New York City (NYC) network with a pre-defined fleet size demonstrated the efficacy and promise of the proposed system. The results show that the proposed DC-SAM service can not only significantly reduce the SAV’s operating costs in terms of vehicle-miles traveled (VMT), vehicle-hours traveled (VHT), and vehicle energy consumption (VEC) by up to 53, 46 and 51%, respectively, but can also considerably improve the customer service by 30 and 56%, with regard to customer waiting time (CWT) and trip detour factor (TDF), compared to a heuristic service model. In addition, the demand-side cooperation strategy can bring about additional system-wide mobility and sustainability benefits in the range of 4–10%.

The deployment of modern applications, like massive Internet of Things (IoT), poses a combination of challenges that service providers need to overcome: high availability of the offered services, low latency, and low energy consumption. To overcome these challenges, service providers have been placing computing infrastructure close to the end users, at the edge of the network. In this vein, single board computer (SBC) clusters have gained attention due to their low cost, low energy consumption, and easy programmability. A subset of IoT applications requires the deployment of battery-powered SBCs, or clusters thereof. More recently, the deployment of services on SBC clusters has been automated through the use of containers. The management of these containers is performed by orchestration platforms, like Kubernetes. However, orchestration platforms do not consider remaining energy levels for their placement decisions and therefore are not optimized for energy-constrained environments. In this study, we propose a scheduler that is optimised for energy-constrained SBC clusters and operates within Kubernetes. Through comparison with the available schedulers we achieved 23% fewer event rejections, 83% less deadline violations, and approximately a 59% reduction of the consumed energy throughout the cluster.

The purpose of this article is to evaluate the quality of services offered by the Community College in Taibah University to its students and staff. The data are collected by questioning a sample of 281 respondents in the Community College at Taibah University during the 2018-2019 academic year. A descriptive analysis is used to define the variables of our study, and the principal component analysis (PCA) is applied to determine the contribution of each variable as a factor that affects the respondents’ preferences. The principal component analysis method captures the key variables of the staff satisfaction at the Community College in Taibah University regarding the services provided. The empirical findings of this study let the authors show that the student and staff satisfaction of the Community College in Taibah University is related to certain factors which constitute a necessity for respondents of both genders. These results serve as benchmarks to allow managers of the institution to improve the quality of their services.

Objectives This study aimed to assess the trends and regional variations in secondary care treatment of patients with varicose veins in National Health Service (NHS) England based on data published by the Hospital Episode Statistics which was freely and readily available to the public and health-care policy-makers. Methods Hospital Episode Statistics data for patients being treated for varicose veins, and UK Statistics Authority population estimates in all 28 Strategic Health Authorities (SHAs) in England from 2002 to 2006 were retrieved and analysed. Results Between 2002 and 2006 there was a 20% overall reduction (46,190–37,135) in the total number of varicose vein procedures performed in NHS England per year. The number of varicose vein procedures performed per 100,000 population per year varied significantly across the SHAs ( P < 0.0001). Similarly, significant regional variations were also noted in the frequency of primary procedures of greater and small saphenous vein ( P < 0.0001). During this time, injection sclerotherapy was only performed in 15 (53.6%) SHAs. The annual proportion of varicose vein procedures performed as daycases had increased from 56% to 64% during the period. Conclusion From 2002 to 2006 there was an overall reduction in the total number of varicose vein procedures performed in NHS England with major regional variations.

In vehicle-to-infrastructure (V2I) communications, various failures in the dynamic movement pose serious link interruptions. To study the continuous service quality for the V2I network when this issue happens, this paper proposes a survivability analysis and establishes the communication architecture for software-defined network (SDN)-based V2I communications. With the controllable advantages of SDN centralized management, the multi-path transmission control protocol is used to seamlessly switch the transmission information between the V2I links of each vehicle node. Specifically, according to the analysis of specific fault types for V2I links, the definitions of SDN-based V2I survivability is provided to establish the corresponding survivability mode. To further verify the survivability model, a full-state search is adopted by means of probability model checker PRISM. In addition, multi-directional probability and expected reward evaluation analyses are carried out from the point of view of time. The simulation results show that, with the failure of multiple V2I links, the network quality of service (QoS) correspondingly declines, but the network still survives, due to the multi-path transmission control protocol (MPTCP) action. Moreover, with a high fault repair rate, the service performance and survivability of the network is improved rapidly.

This article examines the next step in that direction—passage of Right to Service (RTS) Acts by a number of Indian States thereby providing its citizens the right to time-bound delivery of notified public services. These Acts not only empower citizens to make claims against the government if the rights are violated but also serve as a tool for the politicians and the senior bureaucrats to control lower bureaucracy. This article traces the genealogy of RTS Acts in Citizen’s Charter movement of the1990s in the UK and evaluates their progress and results with the help of various theories and concepts used for improving the public service delivery. How inept implementation has thwarted the promise of accountability inherent in these Acts will be seen in detail while piercing the veil of statistical data.

Aims: To compare the ability of vascular and general surgical services to abolish reflux in superficial venous system with aspecial reference to preoperative use of Doppler techniques. Materials and Methods: 68 lower limbs operated on for venous insufficiency based on either preoperative Duplex evaluation and Doppler marking in avascular surgical unit (Vascular Surgical Service, VSS: 33 limbs, clinical class C2–C6) or clinical findings in a general surgical unit (General Surgical Service, GSS: 35 limbs, clinical class C2–C4) were re-examined clinically and with duplex scanning for reflux some three years postoperatively. Results: Marked superficial or perforator vein reflux was observed in 27 of 68 (39.7 %) operated limbs, thirteen of which in VSS and fourteen in GSS. However, axial reflux at saphenofemoral or thigh level was observed significantly less in VSS compared to GSS (3 vs 13, p = 0, 006). Conclusion: Total ablation of any reflux appeared difficult irrespective of the preoperative assessment. Preoperative Duplex examination, however, aided in identifying and treating axial reflux at thigh level.

This service can only be enjoyed by those individuals who submit themselves to the (colonial) Civil Code. The introduction of HRM in the public sector was as a desire for governments to provide increase quality public services. In this vein, the need for efficient public sector organizations emerged. Moreover, reactions all over the world have centered on pinpointing the main business of government was to exposed the public sector to rivalry; and subcontracting aspect of service provision and support functions. The failure of old-fashioned controls had created customer/user choice. It was on these communities’ expectations that governments are becoming facilitator instead of straight provider of public services. However, the costs of accepting HRM practices and values were contended to have intended to expurgate employees’ benefits and wages. Also brought changes on the culture and structure in the public service accompanied by cutting back staff.

With the spread of connected vehicles (CVs), a growth of novel information services exploiting data transmitted by CVs is expected. Wireless communication systems, in particular in vehicular applications, operate with a varying level of transmission reliability, which may affect the quality of V2X-data-driven intelligent transport systems (ITS). Therefore, the performance of ITS should be evaluated in a variety of conditions and the configuration of parameters should be fine-tuned in a safe testbed, using computer simulations. A simple framework is presented, which couples VISSIM traffic simulation and OMNeT++ communication networks simulation in real time, enabling an assessment of the relationship between a communication reliability and transport service quality. A functionality of the framework is demonstrated by applying it to a scheme controlling signalized intersections while estimating traffic flows from the V2I data.

In the process of the writing of this article, it was established that the organizational and economic mechanism of a business security ensuring involves the use of various techniques, methods, and tools. Their combination is necessary to ensure a reliable security system and stable sustainable development of a business activity as a whole. It has been established that enterprises of different sizes, types, volumes of activity possess unequal organizational and financial capabilities. In addition, various is the level of risk under which business entities are engaged. However, the conclusion is definite that the implementation of the organizational and economic mechanism for the enterprise security ensuring is vital in financial and economic aspects and can be organized in different business areas in different ways. It is proved that the security mechanism action from the point of view of financial and economic aspect implies enterprise adaptation to innovations under the following conditions: the innovations are to be maximally ready for implementation; the motivation to implement them is the need to protect the enterprise interests; the probability of obtaining a positive result in the implementation of innovations should be high. It has been established that significant factors in the process of increasing the level of a business security are the financial and economic measures, which aim to cover losses and to eliminate the causes of their occurrence, to recover (preserve) the liquidity and solvency, to reduce the amount of debt, to improve the working capital structure, etc. It is proved that further research in the field of planning and implementation of tactical measures for ensuring the financial and economic security of business is needed. It is aimed at the assessment and analysis of the level of a business security and its dynamics, which will allow to determine the effectiveness of the implemented measures; an assessment of the efficiency of utilization of the enterprise financial potential, which is the main basis for the implementation of measures to ensure the organizational and economic mechanism of a business security. Since the implementation of organizational and economic tools into the security field often requires the allocation of significant financial resources and opportunities (financial potential), the substantiation of scientific recommendations for assessing cost-effectiveness become significant. It was found that the organizational and economic mechanism of a business security ensuring would effectively use corporate resources, create preconditions for achieving business targets, timely identify and optimize all possible hazards, threats, and risks of the enterprise activity in the conditions of acute competition and changing environment. It is recommended to propose the following measures to enhance business security: development and implementation of security system (structure) in business processes of the enterprise activity; improvement and optimization of the existing enterprise security service; business security outsourcing services; services of the enterprise personnel security; provision of services to provide economic and internal security of the enterprise; providing necessary data and information for the business functioning security insurances; checking (auditing) of existing security services of enterprises for their effective work and reliability; services for the enterprise security employees training (advanced training); personnel selection of professionally trained staff for the enterprise security service.

Abstract:
INTRODUCTION This study examines the impact of rationing varicose vein operations on operative training on a general surgical unit with a vascular interest. PATIENTS AND METHODS Log-books of middle-grade surgeons were analysed for 3-month periods before and after a decision by the local Primary Care Trust to ration varicose vein referrals. Number, intermediate equivalents and type of operations were recorded, whether they were general or vascular cases and whether the trainee had carried out or assisted with the operation. RESULTS There was a slight fall in the total number of operations in which the middle-grade surgeons were involved (208 to 186). There was a significant increase in general surgical cases with the fall in number of varicose vein operations (P < 0.0001).The fall in case-load and work-load operative training in vascular surgery was compensated by an increase in general surgical cases (P = 0.0003). This was largely due to increased number of hernia repairs (P = 0.0035). CONCLUSIONS From the point of operative training, a vascular unit in a district general hospital would not be sustainable following withdrawal of varicose vein services. However, this can be off-set by increasing general surgical case-load to fill the gap created.

Abstract OBJECTIVES Coronavirus disease 2019 is a new contagious disease that has spread rapidly across the world. It is associated with high mortality in those who develop respiratory complications and require admission to intensive care. Extracorporeal membrane oxygenation (ECMO) is a supportive therapy option for selected severely ill patients who deteriorate despite the best supportive care. During the coronavirus disease 2019 pandemic, extra demand led to staff reorganization; hence, cardiac surgery consultants joined the ECMO retrieval team. This article describes how we increased service provisions to adapt to the changes in activity and staffing. METHODS The data were collected from 16 March 2020 to 8 May 2020. The patients were referred through a dedicated Web-based referral portal to cope with increasing demand. The retrieval team attended the referring hospital, reviewed the patients and made the final decision to proceed with ECMO. RESULTS We reported 41 ECMO retrieval runs during this study period. Apart from staffing changes, other retrieval protocols were maintained. The preferred cannulation method for veno-venous ECMO was drainage via the femoral vein and return to the right internal jugular vein. There were no complications reported during cannulation or transport. CONCLUSIONS Staff reorganization in a crisis is of paramount importance. For those with precise transferrable skills, experience can be gained quickly with appropriate supervision. Therefore, the team members were selected based on skill mix rather than on roles that are more traditional. We have demonstrated that an ECMO retrieval service can be reorganized swiftly and successfully to cope with the sudden increase in demand by spending cardiac surgeons services to supplement the anaesthetic-intensivist roles.

Peer to peer lending has developed into a new investment measure providing solution for middle and small business enterprises in Indonesia, i.e., to obtain loan without collateral. However, the business enterprise providing the platform for peer to peer lending (websites or other apps), connecting would be lenders and borrowers, almost always puts into the service agreement a disclaimer stating that it would not be held liable in case of default. Using a juridical dogmatic approach, the author discusses the existence of such disclaimers from the viewpoint of consumer protection. The main argument put forward here is that the business enterprise providing this service cannot and should not shy away from bearing responsibility in protecting the consumer, i.e. those that provide loans.

Digitalization has enabled infrastructure and cities to be “smarter”; the use of physical space and energy, the transmission of information, the management of users, assets and processes, the operation of businesses and companies have been progressively digitalized. The main challenges of a Smart City is its definition, scope and interconnections; there are different approaches to Smart City implementations that vary from collaborative multidisciplinary environments, the addition of Information and Communications Technology (ICT) within its physical fabric to the use of Big Data for higher abstraction decisions. This paper presents the concept of Digital as a Service (DaaS), where any complete digitalization can be implemented independently of its associated physical infrastructure in a Cloud environment; DasS would enable an interoperable Virtual Digital Infrastructure (VDI). In addition, this paper reviews the current Digital Systems, Transmission Networks, Servers and Management Systems. The next Industrial Revolution will be founded on Artificial Intelligence that will entirely replace humans by taking production and management decisions based on the Internet of Things (IoT), the Cloud, BlockChain, Big Data, Virtual Reality and the combination of digital and real infrastructure or city. Digital as a Service would be its enabler by providing the entire interconnection, integration and virtualization of its Space, Services and Structure (3S).

The past decade has witnessed the emergence of Vehicular Ad-hoc Networks (VANET), specializing from the well-known Mobile Ad Hoc Networks (MANET) to Vehicle-to-Vehicle (V2V) and Vehicle-to-Infrastructure (V2I) wireless communications. While the original motivation for Vehicular Networks was to promote traffic safety, recently it has become increasingly obvious that Vehicular Networks open new vistas for Internet access, providing weather or road condition, parking availability, distributed gaming, and advertisement. In previous papers [27,28], we introduced Cooperation as a Service (CaaS); a new service-oriented solution which enables improved and new services for the road users and an optimized use of the road network through vehicle's cooperation and vehicle-to-vehicle communications. The current paper is an extension of the first ones; it describes an improved version of CaaS and provides its full implementation details and simulation results. CaaS structures the network into clusters, and uses Content Based Routing (CBR) for intra-cluster communications and DTN (Delay–and disruption-Tolerant Network) routing for inter-cluster communications. To show the feasibility of our approach, we implemented and tested CaaS using Opnet modeler software package. Simulation results prove the correctness of our protocol and indicate that CaaS achieves higher performance as compared to an Epidemic approach.

✓ The authors present a retrospective analysis of the management of deep vein thrombosis (DVT) and pulmonary embolism (PE) in neurosurgical patients at the Massachusetts General Hospital from January, 1978, through June, 1982. There were 44 cases of DVT and 13 cases of PE. Management modalities included observation only, femoral vein ligation, inferior vena cava clipping, transvenous placement of an inferior vena cava filter or umbrella, and anticoagulation therapy. Six (75%) of eight patients with symptomatic DVT who were managed by observation alone had subsequent pulmonary emboli, and three (38%) died. Femoral vein ligation was followed by PE in one of four cases and led to significant leg swelling in two others. Neither observation alone nor femoral vein ligation can be recommended as routine management options. Partial inferior vena cava interruption with a De Weese clip, Kim-Ray Greenfield filter, or Mobin-Uddin umbrella all successfully prevented pulmonary emboli. The major problem associated with these methods was leg edema, which occurred in 47% of patients with clip placement, 25% with filter placement, and 21% with a Mobin-Uddin umbrella. Anticoagulation therapy was associated with a complication rate of 29% and a mortality rate of 15%. Fatal PE and paradoxical hypercoagulability with gangrene of a lower extremity were the causes of death. In one patient, hemorrhage into a glioblastoma occurred following discontinuation of anticoagulation therapy when the coagulation parameters were normal. The authors conclude that: 1) management with observation alone of patients with symptomatic DVT places the patient at risk for the development of life-threatening pulmonary emboli; 2) the safety and timing of therapeutic anticoagulation in postoperative neurosurgical patients or patients with tumors is unclear; and 3) partial interruption of the inferior vena cava with a transvenous filter successfully prevents PE and may represent a safer alternative to anticoagulation therapy.
