Full textThis work explored the adsorption of organic molecules to achieve change on surfaces. In this context it was proposed, the adsorption of organic molecules of halogenated derivatives of uracil, 5 Fluorouracil and 5 Chlorouracil, in monocrystalline substrate of HOPG (highly oriented pyrolytic graphite), sizzling was the technique used for construction of self-assembled monolayers (SAM). The atomic resolution of the STM (Scanning Tunneling Microscope) images of HOPG substrate prove its hexagonal structure and that their interatomic distance is 0.246 nm. While in the molecular resolution images of 5 Fluorouracil and of 5 Chlorouracil adsorbed on the HOPG substrate, is presented evidence that after in the adsorption some changes occur in the intermolecular distances in the structures and the positioning of these molecular films formed on the substrate because the molecular films behaved in a horizontal position and were stabilized laterally by hydrogen bonds when they are protonated, but when we performed the same procedure of adsorption for these organic molecules, only varying the pH of the environment, the change caused the deprotonation of the molecules, thus allowing one to simulate electrochemical environment and being able to extract information about the pKa of adsorbates using UV-Vis spectroscopy, and was also observed that these deprotonated molecular films behaved in a vertical position on the substrate.
Neste trabalho foi explorada a adsorção de moléculas orgânicas para realizar modificação em superfícies. Neste contexto foi proposta a adsorção das moléculas orgânicas de derivados halogenados do uracil, o 5 Fluorouracil e o 5 Clorouracil no, substrato monocristalino do HOPG (Grafite Pirolítico Altamente Orientado do inglês, Highly Oriented Pyrolitic Graphite), sendo usada a técnica de sizzling para construção de monocamadas automontadas (SAM Self-Assembled Monolayers). As imagens de STM (Scanning Tunneling Microscopy) de resolução atômica do substrato de HOPG comprovam sua estrutura hexagonal e sua distância interatômica que é de 0,246 nm. Já nas imagens de resolução molecular do 5 Fluorouracil e do 5 Clorouracil adsorvido no substrato do HOPG, são apresentadas evidências de que após a adsorção ocorrem mudanças nas distâncias intermoleculares, nas estruturas e no posicionamento desses filmes moleculares formados no substrato, pois os filmes moleculares se comportaram em um posicionamento horizontal e são estabilizados lateralmente por ligações de hidrogênio quando estão protonadas, mas quando foi realizado o mesmo procedimento de adsorção para essas moléculas orgânicas, só que variando o pH do meio, esta variação provocou a deprotonação das moléculas, assim permitindo simular um ambiente eletroquímico e sendo possível extrair informação sobre o pKa dos adsorbatos usando a espectroscopia UV-Vis, e foi observado também que estes filmes moleculares deprotonados se comportaram em um posicionamento vertical no substrato.

Oral mucositis (OM) is an important adverse effect in patients undergoing cancer treatment. Its pathophysiology is still not fully understood and causes discomfort and treatment interruptions, contributing to a worse prognosis. The relationship of the cyclooxygenase-2 (COX-2) pathway with the MO to chemotherapeutic agents is known, but the role of leukotrienes (another arachidonic acid metabolite and one of the actors in inflammatory processes of the gastrointestinal mucosa) is not determined. An experimental study of 5-fluorouracil induced OM in hamsters was conducted to investigate the participation of cysteinyl leukotrienes (cys-LT) in their pathophysiology. The study had two parts: 1st experiment: four control groups 1) 5-FU: animals submitted to oral mucositis; 2) SAL: animals not submitted to oral mucositis or normal control; 4) CLX (standard treatment group): animals with OM who received celecoxib (7.5 mg/kg/day, intraperitoneal) 1 hour before the administration of 5-fluorouracil, and daily until the tenth day) and three treatment groups: animals with OM and montelukast (MTK) cys-LT blockade (10 or 20 or 40 mg/kg/day/10 days; orally by gavage). The controls groups used were the same as those of the 1st experiment, except for the CLX group and the treatment group had blocked the leukotriene synthesis cascade by MK886 (inhibitor of 5-lipoxygenase synthesis) (3mg/kg/day; intraperitoneal) for 4 days, given 1 hour before 5-fluorouracil administrations and daily until the 4th experimental day. In both experiments the animals were euthanized on the 10th day and macroscopic analysis of the jugal mucosa was performed, as well as histopathological evaluation for hematoxylin-eosin, immunohistochemical evaluation for iNOS2, COX-2, IL1-β, TNF-α and IL10 for all experimental groups and dosage of myeloperoxidase for the animals of the 2nd experiment. Montelukast presented partial protection at a dose of 40mg/kg/day in the macroscopic evaluation, but in the other doses administered there was no protection. MK886 did not macroscopically protect the progression of OM by 5-fluorouracil. Myeloperoxidase activity was not reduced by the use of MK886 for four days. Montelukast or MK886 were unable to protect the animals from OM in the histopathological evaluation by hematoxylin-eosin, and unable to reduce the expression of iNOS2, COX-2 and IL1-β. But montelukast and MK886 decreased TNF-α expression at all doses used. Montelukast promotes a decrease in IL-10 expression at a dose of 40mg/kg/day. COX-2 blockade reduced the expression of iNOS2, COX-2, IL-1β and TNF-α, but was unable to induce decreased expression of IL10. Differing from the COX-2 pathway, the leukotriene pathway is not relevant in the pathophysiology of the 5-fluorouracil induced oral mucositis model. However, its effect on TNF-α suggests a modulating action specifically on this cytokine.
A mucosite oral (MO) é um importante efeito adverso em pacientes submetidos ao tratamento do câncer. Sua fisiopatologia ainda não está totalmente esclarecida e causa desconforto e interrupções do tratamento contribuindo para um pior prognóstico. A relação da via da cicloxigenase-2 (COX-2) com a MO por quimioterápicos é conhecida, mas o papel dos leucotrienos (outro metabólito do ácido araquidônico e um dos atores nos processos inflamatórios da mucosa gastrointestinal) não está determinado. Foi realizado estudo experimental de MO induzida por 5-fluorouracil (5-FU), em hamsters, para investigar a participação, dos cisteinil leucotrienos (cys-LT) na sua fisiopatologia. O estudo teve duas partes: 1° experimento: quatro grupos controles 1) 5-FU: animais submetidos à mucosite oral; 2) SAL: animais não submetidos à mucosite oral ou controle normal; 3) TM: animais submetidos apenas trauma mecânico (escoriações nas mucosas jugais) e 4) CLX (grupo tratamento padrão): animais com MO que receberam celecoxibe (7,5 mg/kg/dia; intra peritoneal) 1 hora antes das administrações de 5-FU e, diariamente até o décimo dia do experimento) e três grupos tratamento: animais submetidos a MO e bloqueio dos cys-LT por montelucaste (MTK) (10 ou 20 ou 40 mg/Kg/dia/10 dias; via oral por gavagem). 2o experimento: os grupos controles utilizados foram os mesmos do 1° experimento, com exceção do grupo CLX e grupo tratamento com bloqueio da cascata de síntese de leucotrienos por MK886 (inibidor da síntese da 5-lipoxigenase) (3mg/kg/dia; intraperitoneal) por 4 dias, administrado 1 hora antes das administrações de 5-fluorouracil e, diariamente, até o 4° dia experimental. Em ambos experimentos os animais foram eutanasiados no 10° dia e realizadas análises macroscópicas das mucosas jugais, além de avaliação histopatológica para hematoxilina-eosina, avaliação imunoistoquímica para iNOS2, COX-2, IL1-β, TNF-α e IL10 para todos os grupos experimentais e dosagem de mieloperoxidade para os animais do 2° experimento. Montelucaste apresentou uma proteção parcial na dose de 40mg/kg/dia na avaliação macroscópica, mas nas demais doses administradas não houve proteção. O MK886 não protegeu macroscopicamente a evolução da MO por 5-fluorouracil. A atividade da mieloperoxidase não foi reduzida pelo uso de MK886 por quatro dias. Montelucaste ou MK886 foram incapazes de proteger os animais da MO na avaliação histopatológica por hematoxilina-eosina, e incapaz de reduzir a expressão de iNOS2, COX-2 e IL1-β. Mas montelucaste e MK886 diminuíram a expressão de TNF-α em todas as doses utilizadas. Montelucaste promoveu a diminuição da expressão de IL10 na dose 40mg/kg/dia. O bloqueio da COX-2 reduziu a expressão de iNOS2, COX-2, IL1-β e TNF-α, mas foi incapaz de promover diminuição da expressão de IL10. Diferindo da via da COX-2, a via dos leucotrienos não é relevante na fisiopatologia no modelo de mucosite oral induzida por 5-flourouracil. Entretanto, o seu efeito sobre o TNF-α sugere uma ação moduladora especificamente sobre esta citocina.

Full textIn clinical practice, it is frequent to find parents of autistic children concerned about their other children being at risk of developing problems, due to social and communicational disabilities of the autistic child, or their severe behavioral problems. Parents are also unsure whether their home could be worsened to their other children, as they have to dedicate more time and energy to the child with mental disorder. The objective of this research was to evaluate the Quality of Life (QoL) in siblings of autistic patients, verifying if the 3 most compromised aspects in the autistic syndrome (communication, sociability and behavior) were also compromised to some level in these siblings, and if these aspects had an influence in their QoL. Casuistic included siblings of autistic patients (n=31) and, as a control group, siblings of patients with speech disorder (n=30). Criteria of inclusion (for both samples) were: age between 7 and 11 years old, regardless of sex, color or ethnic origin; negative scores for mental disorder in screening questionnaire; and attendance to regular school. Criteria of exclusion (for both samples) were: age not between 7 and 11 years old; antecedents of chronic clinical diseases, severe chronic disabilities (visual, hearing or motor) or of psychiatric disorders; antecedents of cognitive and/or intelligence disabilities; no attendance to regular school, or attendance to special education school. Instruments included questionnaires that evaluated QoL in objective and subjective ways. Results indicated that QoL in siblings of autistic patients was worse than in the control group, when assessed through subjective questionnaire (p=0.000); absence of differences between samples, when QoL was assessed through objective questionnaire (p=0.215); greater tendency towards mental disorder in the sample of siblings of autistic patients (p=0.001); higher frequency of speech disorders in the sample of siblings of autistic patients (p=0.026); and higher number of subjects with antecedents of delivery complications in the sample of siblings of autistic patients (p=0.001). There was evidence that the QoL of siblings of autistic patients was worse due to factors such as their personal dissatisfaction (related to autonomy, daily functions and family) and to the relation of these factors with communication, sociability and behavioral dysfunction (3 aspects present in autism). The hypothesis that QoL was compromised in children aged 7 to 11 by the presence of an autistic sibling was confirmed, and it was worse than that of siblings of children with speech disorders, besides being related to the 3 most compromised aspects in autism.

Full textWe have developed a novel macrolactonization technology. It is a domino process in which a strategically incorporated 5-amino oxazole serves as an internal traceless activator of the neighboring C-terminal carboxylic acid. No coupling reagent is required and the entire sequence is triggered by just a few equivalents of trifluoroacetic acid under very mild conditions. This new methodology has been successfully applied for the synthesis of cyclodepsipeptides and cyclic aminosugars. By combinaison with a three-component synthesis of 5-amino oxazole, a two-step synthesis of structurally complex cyclodepsipeptides, tripeptides and cyclic aminosugars from readily accessible starting materials is developed. An additional study of the mechanism of this domino process has also been provided

Full textCredit assessment is an important tool for the Swedish major banks in the role as capital intermediaries for companies. It can be based on the 5 C’s and used on several different decision levels. There is a practical and a theoretical problem in how the banks should organize the credit assessment since earlier research disagree regarding the prioritising in the credit assessment on different decision levels and what explains it. The main purpose of the study is then what the decision levels in the Swedish major banks prioritize in the credit assessment of companies, within the frame of the 5 C’s and which factors that explains that. With a qualitative method we have conducted interviews with managers and business advisors who represent the highest and the lowest decision levels on the local level, whilst secondary data in terms of the banks risk management reports have represented central level and the highest decision level. The result is that central level and the managers prioritize the capacity and the business advisors prioritize the character. Explaining factors is laws and credit policies which central level and managers sees as more controlling regarding prioritizing the capacity. Further on the rating consists of both financial ratios as well as subjective judgements. Another factor is the size on the company where business advisors handle smaller companies, which are easiest judged with base in the character.

Full textINTRODUCTION: New therapeutic approaches have made possible for children with serious diseases to live up until adolescence. The impact of a chronical disease in an adolescent affects its development and quality of life. OBJECTIVE: to analyze the perception of adolescents with chronical diseases of the interactions of their life conditions with the biopsychosocial peculiarities of the adolescence and its impact in their quality of life, indicating strategies utilized and evaluating resilience and quality of life scores.METHODS: the qualiquantitative, transversal and exploratory study involved 31 adolescents (12-18 years old), who have been having treatment of their chronical diseases in the pediatric specialties ambulatory of the Pediatrics Department of the Medical Sciences School of Santa Casa of São Paulo. The data collect involved: auto evaluation, sociodemographic questionnaires, WHOQOL-BREF, RS-14-scale and focus groups.RESULTS: the participants were 15 years old in average (±1,5), there were 20 adolescents female gender (64,5%) and other 11 (35,3%) male. Their perception of quality of life in their auto evaluation did not vary according to their gender, age, lost school year or degree of education of their parents. The lowest resilience scores were associated with school delay. The adolescents notice their limitations and try the social development through the process of sickening and treatment, generating ambiguous feelings like revolt and pain, contrasting with their auto-perception of the development of resilience. They value ambivalently the support network composed of family and friends. They highlight feelings of embarrassment due to their corporal image and their impacts on the self-stem. They appreciate leisure opportunities like sports, medias and music. They acknowledge their social relationships and their models among friends, family and partners. Basic activities like eating and sleeping were considered promoting factors of well-being. The adolescents with chronical disease have life projects, magical dreams and real plans that remit to future careers in health and care fields.CONCLUSIONS: In spite of their chronical diseases, the adolescents studied have dreams, desires and social behaviors expected of them in their age range. However, their condition impacts on their socialization and education. Those with lower resilience scores had less school delay. The adolescents develop social adaptive behaviors e count on their friends and family support. They wish and have conditions to participate of their therapeutic process

Assim, buscou-se avaliar uma possÃvel modulaÃÃo da via das guanilinas pelo enalapril no modelo de nefrectomia 5/6 (nx5/6). Utilizou-se ratos Wistar, machos, com peso entre 250-300g. Os animais foram divididos em 4 grupos (n=8): grupos controle sem tratamento ou tratado com enalapril (10mg/kg v.o.) (SHAM e SHAM+E) e grupos submetidos à nx 5/6 sem tratamento ou tratado com enalapril (10 mg/kg v.o.) (Nx e Nx+E). Ao final da 10 semana apÃs a cirurgia, foram determinados alguns marcadores de funÃÃo renal. As amostras de rim foram encaminhadas para anÃlise histolÃgica e avaliaÃÃo expressÃo de RNAm para Gn, UGn, PNA e dos receptores da guanilato ciclase de membrana, GC-A e GC-C e do receptor de clearence (NPR-C). No intestino, determinou-se a expressÃo de RNAm para Gn, UGn e G-C. Nx apresentou os nÃveis sÃricos de creatinina (Nx= 1.28  0.07; SHAM= 0.67  0.02 mg/dL), urÃia (Nx=108.0  5.57; SHAM=96.83  4.08 mg/dL), proteinÃria (Nx=129.10  13.87 SHAM=96.83  4.07; mg/24hrs) e FENa+ (Nx=3.552  0.56; SHAM=1.43  0.16) aumentados, e a TFG (Nx=0.44  0.10  0.04; SHAM=0.97  0.07 mL/min) diminuÃda. Nx+E, quando comparado a Nx, apresentou nÃveis reduzidos de creatinina (Nx+E= 0.97  0.08; Nx=1.28  0.07 mg/dL), de proteinÃria (Nx+E=31.94  6.46 Nx= 129.10  13.87 mg/24hrs) e da FENa+ (Nx+E= 2.02  0.28; Nx=3.55  0.56), alÃm elevar a TFG (Nx+E=0.70  0.08; Nx=0.44  0.10 mL/min). Nx apresentou aumento da expressÃo gÃnica intra-renal de Gn (Nx=13.92  5.13; SHAM=1.08  0.20), UGn (Nx=12.77  7.00; SHAM=1.04  0.13), GC-A (Nx=5.91  1.36; SHAM=1.06  0.17) e NPR-C (Nx=7.835  1.72; SHAM=1.15  0.27), e Nx+E teve genes reduzidos para UGn (Nx+E=0.10  0.03; Nx=1.75  0.96), GC-A (Nx+E=0.031  0.01; Nx=1.18  0.27) e NPR-C (Nx+E=0.03  0.01; Nx=1.08  0.24) quando comparados a Nx. No intestino, houve uma reduÃÃo da transcriÃÃo de GC-C (Nx=0,22Â0,04; SHAM=1.12  0.22) em Nx, e o enalapril aumentou os nÃveis de expressÃo deste gene (Nx+E=3.94  0.57; Nx=1.15  0.22). Em conjunto, estes dados sugerem uma hiperativaÃÃo na via das guanilinas na DRC, alÃm de modulaÃÃo dessa classe de peptÃdeos por parte da AngII.

A doença renal crônica (DRC) é caracterizada pela perda normalmente lenta, progressiva e irreversível da função renal. Sugere-se que, nesta patologia, a resposta natriurética do organismo à ingestão de sal e a expansão de volume encontra-se reduzida em consequência da lesão dos néfrons. Nx apresentou aumento da expressão gênica intra-renal de Gn (Nx=13.92 ± 5.13; SHAM=1.08 ± 0.20), UGn (Nx=12.77 ± 7.00; SHAM=1.04 ± 0.13), GC-A (Nx=5.91 ± 1.36; SHAM=1.06 ± 0.17) e NPR-C (Nx=7.835 ± 1.72; SHAM=1.15 ± 0.27), e Nx+E teve genes reduzidos para UGn (Nx+E=0.10 ± 0.03; Nx=1.75 ± 0.96), GC-A (Nx+E=0.031 ± 0.01; Nx=1.18 ± 0.27) e NPR-C (Nx+E=0.03 ± 0.01; Nx=1.08 ± 0.24) quando comparados a Nx. No intestino, houve uma redução da transcrição de GC-C (Nx=0,22±0,04; SHAM=1.12 ± 0.22) em Nx, e o enalapril aumentou os níveis de expressão deste gene (Nx+E=3.94 ± 0.57; Nx=1.15 ± 0.22). Em conjunto, estes dados sugerem uma hiperativação na via das guanilinas na DRC, além de modulação dessa classe de peptídeos por parte da AngII.

Full textIntroduction: Depression is a psychiatric disorder whose causes seem to involve chronic exposure to stressors. It is known that the dorsal hippocampus (DH) participates in the adaptation mechanism to aversive stimuli, once the facilitation of serotonergic neurotransmission in this structure reduces the effect of a stressor. The main source of serotonergic innervation to the HD is the median raphe nucleus (MRN). Aim: The aim of this study was to verify if the integrity of the MRN-HD pathway is essential for the antidepressant effects of chronic treatment with imipramine in animals with lesion on serotonergic neurons of MRN submitted to chronic and repeated stress. Materials and Methods: We used male Wistar rats, seven weeks old. The animals were submitted to stereotactic surgery for lesion (or not - control group) on MRN serotonergic neurons. Later, they were divided in groups according to the chronic drug treatment they would receive (Imipramine or Saline). All animals were subjected to chronic (7days/Two hours a day) or acute restraint stress (a single episode of restraint). On the eighth day the animals were tested in the Elevated Plus Maze (EPM). Results: When subjected to acute stress, animals with intact MRN and treated with imipramine showed a higher percentage of time spent in open arms (% TO) and the number of entries in open arms (% FO) when compared to control. Moreover, rats subjected to acute stress and chemically injured showed a greater exploration of open arms (% TO and % FO) when compared with the group without lesion. Imipramine was able to attenuate the effects of stress on animals subjected to chronic stress from both groups, with intact MRN and those with lesion. That was observed on a increase of % TO and % FO when compared with rats treated with saline. The injury on MNR in animals submitted to chronic stress decreased both %TO and % FO. Discussion: Our results showed that the lesion on serotonergic neurons of MNR dont interfere in the behavior of animals in the EPM when subjected to a single stress episode, but decreases the open arm exploration in rats exposed to chronic stress. This effect, however, was attenuated by chronic administration of imipramine. Conclusion: These results suggest that the integrity of the NMnR-HD pathway is not essential for the antidepressant effects Imipramine in the development of tolerance to repeated stress.

Full textIntroduction - Leprosy can affect the skin and peripheral nervous system and can lead to severe deformities. It is caused by Mycobacterium leprae, Bacillus acid-resistant, an intracellular parasite exclusively. Currently, leprosy is classified into two major groups: paucibacillary (less than five lesions) and multibacillary (more than five lesions). It is a public health problem, with Brazil among the countries with the highest endemicity in the world. The millennial morbidity of this disease is due to the fact that causes disabilities, sometimes irreversible, besides the stigma that accompanies it, compromising the quality of life. Several studies have been conducted regarding the quality of life in dermatological diseases such as psoriasis, atopic dermatitis, acne, genital herpes, leprosy, in order to try to assess the degree of impairment of physical well-being, psychosocial and daily routine activities as well as try to improve the doctor-patient relationship and health actions in order to minimize problems arising from each disease. Objective: To assess the actual quality of life for patients in group paucibacillary leprosy outpatients. Methods: application of the Dermatology Life Quality Index questionnaire and Short Form-36. The Dermatology Life Quality Index is a dermatology-specific questionnaire and consists of 10 questions which score ranges from zero to 30, with the maximum value (30) related to greater impairment of quality of life. The Short Form-36, more generic questionnaire covers eight domains (Physical Functioning, Role Physical, Pain, General Health, Vitality, Social, Emotional and Mental Health Aspects), distributed in 11 questions, each with varying number of items, totaling 36 items, which gives a score that can range from zero to 100, with the highest score (100) assigned to lower impairment of quality of life. These two questionnaires were applied in 49 patients with paucibacillary leprosy group. Results: Of 49 patients who participated in the study, 36 females and 13 males, most (63%) did not show quality of life compromising, according to the results obtained by the Dermatology Life Quality Index questionnaire. By applying the generic questionnaire SF - 36, the scores achieved in several domains also showed little decrease in quality of life in the studied group

Full textThe aim of this cross-sectional study was to establish a relationship between oral condition and its impact on quality of life in adults between 35 and 44 years-old (n=504) in Chapecó, State of Santa Catarina, Brazil. The Oral Impact on Daily Performances (OIDP), oral health condition, habits and care, self perception of health and socio-demographic aspects were collected during clinical examination and home interviews. Unconditional multiple logistic regression models were used in the analysis. Impact was found in a fifth of the population and was of low severity. Eating and cleaning teeth were the most affected performances. Households overcrowded, living in areas of poor conditions, being a woman, having sufered from orofacial pain in the last six months and individuals assessing their own health as reasonable or poor in the last month were independently associated with the impact

Full textThe onset of Systemic Lupus Erythematosus (SLE) is related to stressful and traumatic situations in patient lives, which suggests the psychological factor to trigger the disease. The Lupus Nephritis (LN) may be the first manifestation of SLE and is a major factor of morbidity and mortality, because it leads to kidney failure, requiring hemodialysis. Furthermore, the LN changes the routine of life and selfimage of patients, which makes the disease itself a traumatic event. Within this context, this study aims to: (i) verify the existence of life events associated with the outbreak of SLE, and how these patients understand their illness, and (ii) evaluate correlations between life events, status of SLE activity and socioeconomic profile. For the empirical research, we used a qualitative and quantitative approach, applying the following instruments: a semi-structured psychological interview; a psychiatric interview for recent life events and an instrumental for socioeconomic status. The sample was consisted of 43 subjects admitted to the Nephrology Department, at Hospital das Clinicas in Sao Paulo: 22 patients formed the case group, with LN and 21 formed the control group, with primary glomerular disease. The clinical and laboratory data were collected using the SLEDAI. The main results show that the occurrence of life events had no correlation with SLE activity. There was an association between negative life events and the outbreak of the disease in both groups, showing that negative events also precede and may be associated with the onset of other pathologies. Regarding the manner LN patients understand their illness, we see that they partially understand their disease, giving a negative meaning and associating an emotional situation at the beginning of illness. These are patients who perceive problems in their lives and need to deal with the disease giving importance to the treatment. The illness process causes suffering due to the changes and limitations the disease imposes, especially related to self-esteem, therefore, patients suffer emotionally and demand to be heard and supported. Thus, we conclude that the negative life events and/or trauma are related to the triggering of SLE both in an objective way as an event that involves changes in the external social environment, without taking into account the subjectivity of the subject as in a subjective singular life event, reported by the patient as a traumatic event. Based on this analysis, we suggest that patients with LN have particular psychological factors that operate both in the course of the disease, and in its triggering

Full textTraitement sensoriel précis de l'environnement est une fonction essentielle pour tout organisme de survivre. Afin d'accomplir ces neurones doivent être spécialement réglés pour répondre au mieux aux stimuli pertinents dans leur environnement. Un moyen efficace de coder efficacement l'information est pour les neurones à adapter leurs réponses à des stimuli provenant de différents contextes de comportement. Neuromodulateurs comme la sérotonine (5-HT) sont pensés pour aider à la médiation de ces adaptations. Le système 5 -HT a été bien conservé à travers l'évolution des vertébrés, ce qui suggère que sa fonction a également été conservée. En Apteronotus leptorhynchus, un des poissons électriques du genre gymnotiformes, 5-HT a déjà été démontré pour augmenter sensorielles pyramidales neurones réponses situées dans le lobe de la ligne-latérale électrosensoriel (LLE) aux stimuli provoqués par des congénères de même sexe en les rendant plus excitable in vivo. Remarquablement, l'application de la 5-HT in vitro a été démontré également de rendre ces neurones plus excitables à travers à la fois la régulation négative de courants potassiques sensibles au calcium (SK) et les courants potassique de type M. Cependant, la nature des récepteurs 5-HT présents dans les neurones pyramidaux est inconnue. En utilisant les expérimentés d'occlusion lors de l'enregistrement de neurones pyramidaux in vitro, nous montrons que l'antagoniste sélectif 5-HT2, ketanserin, a peut bloquer les effets de la 5-HT sur les cellules pyramidales quantifiées par des changements dans le taux de le potentiel d'action, fraction de le potentiel d'action dans une «bouffée», après-hyperpolarisation, et l'information mutuelle. Nos résultats montrent que la régulation négative des courants à la fois M et SK est probablement atteint par la même famille récepteur et ouvrir la voie à des études moléculaires futures comme l'hybridisation in situ ou l'immunohistochimie pour confirmer que les récepteurs 5-HT2 sont bien présents dans le LLE et permettent les effets de la 5-HT a observé à la fois in vitro et in vivo.

Full textA aplicação de força mecânica sobre o tecido ósseo resulta em remodelação, caracterizada pela reabsorção e deposição óssea em sítios específicos. Esses processos são regulados pela liberação de mediadores inflamatórios, dentre esses os metabólitos derivados do ácido araquidônico (AA). O AA, quando metabolizado pela via 5 lipoxigenase (5-LO) resulta na formação de leucotrieno B4 (LTB4) e cisteinil leucotrienos (CysLTs). Esses metabólitos têm sido amplamente estudados na resposta inflamatória e, mais recentemente, na remodelação óssea. Sendo assim, os objetivos deste trabalho foram: (i) desenvolver um modelo de remodelação óssea associada à movimentação dentária ortodôntica induzida por força mecânica; (ii) investigar o papel dos metabólitos da via 5-LO no processo de remodelação óssea, bem como no recrutamento e diferenciação de osteoclastos. Inicialmente, para definição da força mecânica ideal, camundongos C57BL/6 foram submetidos a uma força de intensidade entre 0,1 N e 0,5 N. A força ideal obtida, 0,35 N, foi aplicada nos demais grupos experimentais: C57BL/6 ou SV129 (Wild Type-WT); animais deficientes da enzima 5-LO (5-LO-/-), ou tratados com veículo (VH), zileuton (ZN) (inibidor da 5-LO) e montelucaste (MT) (antagonista do receptor de CysLTs). A quantidade movimentação dentária e o número de osteoclastos foram avaliados histomorfometricamente. A expressão de marcadores de remodelação óssea e citocinas foi avaliada em sítios de compressão e tensão do periodonto. O efeito dos metabólitos da via 5-LO na diferenciação de osteoclastos, bem como na expressão de TNF- foi avaliada in vitro após estímulo de células RAW 264.7 com LTB4 e LTD4. A força de 0,35 N promoveu movimentação dentária e recrutamento de osteoclastos sem induzir reabsorção radicular. A movimentação dentária e o número de osteoclastos estavam diminuídos na ausência ou inibição da enzima 5-LO e do receptor de CysLTs em comparação com os grupos WT e VH, respectivamente. Foi observada uma expressão diferenciada de citocinas e marcadores de remodelação óssea nos sítios de tensão e compressão. Catepsina K e o TNF- tiveram maior expressão nos sítios de compressão, enquanto OCN e RUNX2 no sítio de tensão. A expressão das moléculas TNF-, IL-10, RUNX2 e RANK estava diminuída nos grupos 5-LO-/- e MT em relação ao WT e ao VH, respectivamente. No grupo ZN a expressão reduzida de TNF-, IL-10 e RUNX2 foi observada em relação ao grupo VH. In vitro, LTB4 promoveu aumento da diferenciação de osteoclastos, enquanto LTB4 e LTD4 induziram a liberação de TNF-. Nossos resultados demonstram a contribuição dos metabólitos da via 5-LO, LTB4 e CysLTs, no processo de remodelação óssea por afetarem diretamente o recrutamento e diferenciação de osteoclastos.

Full textLeiomyoma (LMU) and leiomyosarcoma (LMSU) are uterine mesenchymal tumors with variable clinical behavior. Both are myometrial (MM) neoplasms that show the same pattern of cell differentiation, but with completely different clinical progression. To date, there are controversies about these neoplasias devolopement-related factors and a possible malignant transformation of a pre-existing leiomyoma. It was demonstrated that activation of the Sonic Hedgehog (SHH) signaling pathway is related to the development of several tumors, due to its activity in the cell proliferation and differentiation. The aim of this study was to evaluate the transcriptional and protein expression profile of the SHH pathway molecules in LMU and LMSU. It was evaluated 106 genes, by quantitative real time PCR, and seven proteins (SHH, PTCH 1, SMO, SUFU, GLI 1-3 and HHIP 1), by immunohistochemistry, in 176 samples (20 MM, 103 LMU - including 16 unconventional leiomyoma /LMAU and 37 LMSU). The results show that the gene and protein expression were similar for LMAU and LMSU samples. The gene expression of AXIN 2, FZD 2, CCND 1, FZD 6, ESR 1 and IFT 52 was associated with disease free survival; and FZD3, FZD 8 and WISP1 with cancer specific survival in patient with LMSU. No SMO, SUFU, GLI 1 and GLI 3 protein expression was observed in the MM samples. SHH and SUFU proteins were correlated with disease-free survival in patients with LMS. LMU, LMAU and LMSU samples displayed increased expression of all SHH proteins (except HHIP 1 and GLI 2). These data corroborate with the leiomyoma malignant transformation hypothesis. Moreover, some proteins evaluated have been used as therapeutic target for cancer patients. In the near future, new treatment strategies might be proposed for women with these neoplasias

Full textBackground: Recurrent vulvovaginal candidiasis (RVVC) affects about 138 million women annually worldwide. A frequent reason for gynecologist consults, RVVC is defined as four or more episodes of culture positive symptomatic episodes in a 12 month period. Vulvovaginal itching, irritation, pain, discharge and problems with their sexual and emotional relationships are frequent complaints. We evaluated the influence of RVVC on aspects of quality of life in affected women in comparison to a control group. METHODS: This study was conducted at University of Sao Paulo Hospital of Medicine between July 2016- December 2017. The study was approved by Ethical Committee of University Sao Paulo Medical School. The cross sectional study consisted of 100 women with RVVC and 101 epidemiologically healthy matched women with no history of RVVC. Each subject completed a selfadministrated validated World Health Organization Quality of Life Abbreviated Assessment (WHOQOL-bref) questionnaire, consisting of four domains (physical, psychological, social relations, environment) and composed of 26 questions. Internal consistency of responses was evaluated by Cronbach alpha. Data was analyzed by Chi square and student T test. RESULTS: The mean age of women with RVVC and controls was 34,0 years and 32,2 years, respectively. The mean WHOQOL-bref dimension scores for general health (59,7), life satisfaction (46,7), physical well being (54,5), social relations (51,5), psychological (56,8) and environment (43,5) for women with RVVC were all lower than in the control group: general health (71,3), life satisfaction (67,1), physical (68,5), social relations (69,1),psychological (65,8), and environment (57,4). Cronbach alpha coefficient was > 0.8 for general questions and > 0.65 for specific questions, indicating almost perfect and substantial internal consistency, respectively. In total, perception of quality of life and satisfaction with their health was greatly reduced in the RVVC group (p < 0.001). In the physical dominion, perception of pain, lack of energy, sleep problems, reduced daily activity, reliance on medications or treatments was increased in women with RVVC (p < 0.001). Psychologically, RVVC patients reported lowered affection, cognition, self-esteem and body image (p < 0.001). All aspects of social relations including sexual activity were also reduced (p < 0.001). Patients reported a less satisfactory home environment, lower financial resources, lower frequency of recreational activities and higher unemployment (p < 0.001). CONCLUSION: RVVC affects multiple aspects of affected women\'s well being. Women with this condition deserve serious attention from clinicians and research into susceptibility, prevention and treatment of this infection should receive much greater support

Full textPulmonary arterial hypertension (PAH) is a rare but insidious disease characterized by progressive pulmonary vascular remodelling with increased pulmonary vascular resistance ultimately leading to right ventricular failure e progressive dyspnea, leading to profound functional limitations, how is demonstrated by the six minute walk test. However, there is limited information about quality of life (QoL) in patients with PAH. The aim of this study was to evaluate the quality of life of PAH patients with the SF-36 generic questionnaire and compare the scores with the surrogate markers and identify the prognostic implication of this assessment. The results showed there was a good correlation with six minute walk test and functional class and there was a significant improvement in QoL after specific treatment. The physical component summary (PCS) of the SF36 QoLQ was significantly related to survival. Conclusion is that QoL is a useful tool for routine evaluation of PAH patients once it correlates with other surrogate markers, reflects treatment effect and predicts survival

Full textBIM – Building information modelling is used today by architects, structural engineers,contractors and property owners among others. BIM is a tool where a 3D-model of the projectcan be used as a source of information.When the 3D-model contains measurements/quantities and types of material it becomes apowerful tool when doing cost estimations/quantity takeoffs and production planning. The usageof BIM-models for these activities is a relatively new way of working and there are very fewstandards and proven methods. To have use of the BIM-model it also takes an implementation ofcost estimate- and planning programs that supports the use of BIM. Also, there are no standardsor guidelines for the configuration of the BIM-model.Research show that the use of BIM-models in quantity takeoff and production planning is bothtime-saving and more cost efficient then the use of the traditional ways where 2D-CAD drawingswith included descriptions is the only source of information. Earlier reports and master thesis’salso show how 5D-BIM is used today and how it can be implemented for companies with eitherindustrial- or more traditional concepts. 5D-BIM is a method where beyond the 3D-model, timeand cost are also included as dimensions 4, and 5.This report aims to evaluate under what conditions an implementation of 5D-BIM is possible fora contractor with a concept that includes both industrial- and traditional building, and alsoevaluate the possibilities and problems that occur during implementation. The use of BIM differsbetween industrial “off-site construction” and traditional “on site construction”. The author ofthis thesis therefor wants to study a company with a concept that includes both industrial andtraditional practices and also evaluate what information is requested in the BIM-model.An explorative case study has been conducted at a partially industrial contractor that produceshouses. The method 5D-BIM has been implemented and current workflow has been studied thrumodel configuration-, cost estimation to production planning. The study of the current workflowis the foundation for the implementation of 5D-BIM and the evaluation of § model configuration.The study shows how 5D-BIM as a method can be implemented for a company that has both onandoff site construction as a concept, and used to increase predictability in the projects.

Full textA therapeutic, comparative, prospective study was carried out to evaluate the minimally invasive posterior approach to total hip arthroplasty in relation to the traditional direct lateral approach. The study included 76 cases of primary total hip arthroplasty due to a diagnosis of primary or secondary osteoarthrosis. Patients were excluded from the study if they presented any type of coagulopathy and were using anticoagulants or platelet antiagregants; hypertension without medicinal control; any previous hip surgery; and sequelae of hip developmental dysplasia that might have led to an ectopic femoral head. The objective of our study was to evaluate the blood loss among patients for whom the minimally invasive posterior access is used. Regarding our patients\' ages, we observed a significant difference between the two groups: the mean age of the mini-incision group (50,1 years) was lower than the mean of the control group (56,8 years), p= 0.01. Lower total estimated bleeding was found in the mini-incision group (means of 1083,5 ml versus 1682,3 ml; p < 0.001) and lower intraoperative bleeding (means of 745,6 ml versus 1282,8 ml; p <0.001). There was difference in the need of allogenic transfusion between the two groups (8,8% versus 28,6%,p = 0.13). Our volumes of bleeding from the minimally invasive posterior approach were significantly higher than in the literature, but the mean quantity of transfused red cells and the proportion of transfused patients were both significantly lower than in the literature. Our final impression of the minimally invasive approach is positive with regard to lower blood loss, better clinical results after six months and a satisfactory alignment of the acetabular and femoral components.

Full textIntroduction: 3-hydroxy-3-methylglutaryl coenzime A reductase (HMGCR) is the rate-limiting enzyme of the mevalonate pathway, which generates isoprenoids for both post-translational modification of several proteins involved in cell growth and proliferation and biosynthesis of cholesterol. Therefore, in steroidogenic tissues (e. g. adrenal cortex), HMGCR may play a role in both growth control and steroidogenesis. HMGCR is overexpressed in several forms of human neoplasms incluiding adrenocortical tumors (ACTs), but its importance in the biology of ACTs is unknown. Objective: To assess the expression pattern of HMGCR, and other genes involved in cholesterol economy and steroidogenesis in pediatric and adult ACTs. To evaluate the impact of a HMGCR inhibitor lovastatin on proliferative pathway- MAPK - of NCI-H295A cells; to evaluate the effects of various mevalonate pathway inhibitors in NCI-H295A cells viability. Methods: We analyzed HMGCR, FDFT1, LDLR, SCARB1, StAR, TSPO, CYP11A1, CYP11B1, CYP17A1, CYP21A1 and HSD3B1 mRNA expression by real-time RT-PCR in ACTs [Adult tumors: 14 adenomas (ACA) and 11 carcinomas (ACC); Pediatric tumors: 13 adenomas (PAD) and 8 carcinomas (PAC)]. A commercial pool of normal human adrenal cortex poly A RNA was used as reference sample. We assess ERK phosphorylation after treatment with lovastatin through SDS-PAGE of intracellular extract followed by immunoblotting with specific antibodies. Action of mevalonate pathway inhibitors on cellular viability was assessed by colorimetric assay. Results: Most adult ACTs showed decreased expression of StAR, TSPO, CYP11A1, CYP11B1, CYP17A1, CYP21A1 and HSD3B1 and overexpression of HMGCR. In this group, the relative expressions of TSPO, StAR, CYP11B1, CYP21A1 and HSD3B1 were significantly lower in the ACCs (P<0.02). LDLR was not differentially expressed in most of adults ACTs, while FDFT1 and SCARB1 were hypoexpressed in this group, independently of biological behavior. Cluster analysis showed that adult ACCs form a group distinct from adrenocortical adenomas, with exception of one carcinoma sample. Regarding tumors of the pediatric group, no differences in the expression of evaluated genes were found between PAD and PAC. The expression of HMGCR and TSPO correlated negatively while a significantly positive pairwise correlation among StAR, CYP11A1, CYP11B1, CYP17A1, CYP21A1 e HSD3B1 was observed in adults ACTs. Lovastatin treatment leads a decreased ERK phosphorylation in NCI-H295A cells. Inhibition of HMGCR, farnesyl syntase and protein isoprenylation decreased drastically NCI-H295A viability unlike the specific block of cholesterol biosyntheses that did not have any effect on cell viability. Conclusion: The hypoexpression of genes related to steroidogenesis and (to a lesser degree) the overexpression of HMGCR characterizes ACCs. HMGCR overexpression cause an increased isoprenylation of proteins rather than cholesterol production for steroidogenesis or membrane cellular integrity since the key regulators involved in this process were downregulated and specific inhibition of cholesterol did not affect NCI-H295A viability. Isoprenylation of proteins is a crucial event for NCI-H295A viability and is involved in signaling cascades (e.g. Ras). Mevalonate pathway is a potential target for treatment of adrenocortical tumors

Full textIntroduction: Larynx cancer is the second most common malignant neoplasm in the cervicofacial segment. Epithelial cadherins (E-cadherins) together with catenins form the E-cadherin-catenin complex that acts on cell-to-cell adhesion. The loss of this molecule may lead to the reduction or even absence of E-cadherin expression in the cell membrane, cytoplasmic accumulation of beta-catenin and its translocation to the nucleus, contributing to carcinogenic events. Objective: To evaluate the expression of E-cadherin and beta-catenin in patients with laryngeal tumor in different locoregional situations. Methods: A study retrospective of 52 patients with glottic or supraglottic squamous cell carcinoma treated between 1998 and 2011 was conducted, evaluated according to the tumor localization site, the degree of histological differentiation, TNM stage and survival analysis. These data were confronted with the immunohistochemical expression of E-cadherin and ?-catenin. Results: We observed statistically significant association between the fall of E-cadherin expression and the supraglottic localization of the lesion, the presence of cervical metastasis, poorly differentiated tumors and locally advanced tumors when in glottic topography. Related to the expression of beta-catenin, statistical significance was also found for the presence of cervical metastasis and tumor of low differentiation with the decreased expression of this marker. Regarding survival analysis, the low expression of beta-catenin is related to worse overall survival and the reduction of expression of both markers to worse disease-free survival. Conclusion: The anomalous expression of the markers studied leads to a prognostic impact in order to provide tumors with greater local aggressiveness and presence of cervical metastasis

Full textIntroduction: Living kidney donors (LKDs) experience an abrupt decline in glomerular filtration rate (GFR). Mineral metabolism adaptations in early CKD are still debated and not well studied in LKDs. We prospectively studied acute and long term mineral metabolism adaptation of LKDs. Materials and Methods: We measured renal function and mineral metabolites longitudinally for 1 year (days (D) 1, 2, 14, 180, & 365 post-operatively) in 74 healthy individuals who underwent kidney live donation. Results: eGFR (MDRD) decreased to 59% of its baseline on day 2 and started to increase at day 3, to its maximum at day 360 (75.3±15.6 ml/min/1.73m2, p < 0.01) wile FGF23 increased from 60.6 (25th-75th percentile 19-81 RU/mL) at baseline to 111.0±144.6 (p < 0.01) on day 1 and keep higher than baseline throwout the study. PTH rose maximally on day 1 (64.9 ± 30.3pg/ml) and returned to its base line on D2 and did not change after that. Total serum Calcium levels decreased from 9,40±0,48 mg/dL to a nadir of 7.99±0,51 mg/dL on day 1 (p < 0.001). Serum Phosphate levels reached their nadir on day 2 (2.61±0,52 mg/dL; p < 0.01). At D14 total calcium and phosphate levels had returned to baseline, but phosphate levels returned down on D360 (3.36±0,52 mg/dL; p < 0.001). Phosphate excretion fraction (FePO4) increased from base line (11.4±5.2%) up to 15.2±8.1% until D360 (p < 0.001). Conclusions: Abrupt reduction in eGFR induces physiological increases in FGF23 and PTH, and decreases in serum Ca and Pi in the first week. The changes in FGF23 and Pi urinary fractional excretion of Pi remain modestly yet significantly different from baseline throughout the first year after nephrectomy. Wile Ca, PTH and Pi serum levels are not significantly different from the baseline

Full textVitamin D (VD) may be involved in the control of proliferation, differentiation and apoptosis of mammary cell lines exposed to high concentrations of the hormone. There is some evidence that women with breast cancer present lower serum level of 25(OH)D3 or 1,25(OH)2D3 compared to women without cancer. Moreover, little is known if serum concentration of VD can influence the development of mammary tumors in dogs and if the hormone may have chemopreventive effect by inhibiting the appearance of tumors or therapeutic effect, reducing the proliferation of malignant cells. Therefore, our goals were to compare the serum 25(OH)D3 level in animals with and without mammary tumor and to analyze 1,25(OH)2D3 effects in normal and tumoral canine mammary glands, using as a model the tissue culture. At first, 39 bitches with mammary tumor and 64 dogs without tumor (control), of which 50 were females and 14 were males were included. Animals in tumor group were older (median 108 months) compared to control group (median 36 months for males and 24 months for females). In control group, serum concentration was higher in males (32.5 ± 19.3 ng/mL) than in females (22.8 ± 9.6 ng/mL), but there was no difference when compared to tumor group (26.62 ± 14.25 ng/mL). Regarding diet, serum 25(OH)D3 level was higher in control bitches fed commercial pet food compared to those fed homemade and commercial pet food combined. However, there was no difference of serum 25(OH)D3 concentration, sun exposure and coat features. In tumor group, there was an inverse correlation between serum 25(OH)D3 and age, but there was no difference in 25(OH)D3 concentration among bitches with different histological type or clinical stage of the disease. Seventhy bitches diagnosed with mammary tumors had tumor and mammary samples collected, sliced and cultured. In tumor tissues, 50% were positive for estrogen receptor (over 10% of cells stained), and 44% were positive for HER-2 (HercepTest method). Vitamin D receptor (VDR) protein and genic expression was detected in normal and tumoral samples. Three patterns of VDR were detected by immunohistochemistry: I - localizated in epithelial and myoepithelial cells (more often in normal tissues), II - predominantly in myoepithelial cells (most common in tumor tissues) and III - predominantly in epithelial cells. Normal anmammary slices were treated with 1,25(OH)2D3 0.228 and 100nM concentrations (concentration physiological and pharmacological, respectively) and mammay tumor slices were treated with 1,25(OH)2D3 2 nM concentrations (drug concentration which does not induce hypercalcemia) and 100 nM, for 24 hours. VD tissue content measured by liquid chromatography was higher in samples exposed to high VD concentration, indicating penetration of the hormone in slices. VD treatment induced CYP24A1 expression, 27 to 158 fold depending on the concentration used, and indicating activation of the VD genomic pathway. This result also suggest that the tissue remains metabolically active in culture. However, no difference in gene expression of other target genes such as CYP27B1, genes involved in proliferation as CDKN1A and CDKN1B and genes involved in immunity, such as CD14. Calcitriol treatment at different concentrations did not induce apoptosis (protein expression of cleaved caspase-3) and did not alter proliferation in normal tissues (expression of protein Ki -67), but decreased proliferation in tumor tissues. No correlation was observed between the induction of apoptosis and reduction of proliferation with the protein expression patterns of VDR. In conclusion, no difference in serum 25(OH)D3 between bitches with mammary tumor and control animals was observed. In normal and tumoral mammary samples calcitriol physiologic concentration activated VD genomic pathway and in tumor samples calcitriol reduced the proliferation index (Ki-67)

Full textThe growing interest in studying quality of life within the neurosurgical field is noticeable, in as much to evaluate the disease impact and the treatment efficiency as to evaluate the physical and psychosocial impacts on those taken by the illness. Few are the studies found in the literature discussing this matter methodically, using instruments which enable us to embrace the composing dimensions of what we call quality of life. This study aimed mainly to evaluate the quality of life before and the development after three, six and twelve months from the meningeoma resection. A prospective study was carried out with 29 patients carrying benign meningeoma (17 women and 12 men) whose ages ranged from 28 and 76 years old. In order to evaluate the quality of life two instruments were used: the Nottingham Health Profile (NHP) and the Innsbruck Health Dimensions Questionnaire for Neurosurgical Patients (IHD-NS). Meaningful differences in the total score were found between NHP and IHD-NS scales when compared to the evaluation over the four moments. A relevant improvement was noticed in the following domains: Pain, Emotional Reactions, Physical Abilities, Sleepiness, Communication, Physical Condition, Independence and Psychological Condition. In the remaining dimensions domains (Autonomic Nervous System, Energy Level, Isolation and Social Interaction) we only observed that the symptoms were tending towards a reduction. In this study we could observe that the patients, who were submitted to the surgical intervention for the meningeoma resection, presented a relevant improvement in their quality of life, with greater intensity up to the first three months. It is worth noticing that between three and six months the patients situation revealed certain stabilization (for the impact caused by the increasing symptoms in the Emotional Reactions domain) and a new reduction of the symptoms took place up to twelve months from intervention. Considering the achieved results, it seems relevant to begin a preventive therapeutic procedure after three months from surgical intervention, in order to approach affective-emotional matters. The sample amplitude disabled us from making more specific analysis concerning age, school educational level, resection level and meningeoma location, such matters must be better exploited in further studies

Full textBACKGROUNG: Many of the health behaviors involved in the emergence of non - communicable chronic diseases are originated in childhood under their parents influence. The mother is the most involved person in education and on the child health care. Lifestyle (LS) is a social health determinant and never before was understood maternal LS on aspects of childhood nutrition. OBJECTIVE: Verify the association on maternal behavior and non- behavioral LS with nutritional aspects in preschool. METHODS: From January 2010 to December 2010, it was performed a cross sectional study with 255 of preschool mothers peers resident on five different sub-districts in São Paulo Southwest. A stratified random sample was selected on two layers, being first the drawn schools and then, the children. From the mother, was caught the age, economical status, education, marital status, behavioral and non- behavioral LS. From the child, the age, weight and height to nutritional status, waist circumference, sedentary behavior, food intake, through an alimentation frequency questionnaire to the studied age group were all taken information about. An extraction analysis factor was performed in order to common factors and principal components on non-behavioral LS and food frequency, respectively, with a subsequent group of mothers on the domain of LS and the children on extracted factors. The association was calculated using the chi-square test and logistic regression. RESULTS: All the mothers were grouped into three clusters LS: socio conscious, self-actualization and consumeristic. The children were grouped into two feeding clusters: minimally and upper processed. Children with minimally processed food were born from mothers with more socio conscious non- behavioral LS, while children with upper processed food were born from mothers with more consumeristic non-behavioral LS. No other association was found between the nutritional preschool characteristics and the kinds of behavioral maternal LS. Not eutrophic and central adiposity present, children had 99% and 92% higher chance, respectively, to be children from mothers with a high score in the modern domain. Children with sedentary behavior and upper processed alimentation had 113% and 84% higher chance, respectively, to be children from mothers belonging to the consumeristic cluster. CONCLUSION: Non- behavioral maternal LS is associated to nutritional preschool characteristics being the mothers living a consumeristic lifestyle tending to have negative influences on child nutrition

Full textINTRODUCTION: Hereditary angioedema (HAE) is a rare illness, caused by a deficiency of C1 esterase inhibitor (C1-INH), which manifests as recurring unexpected attacks of edema of the face, extremities, genitals, chest and gastrointestinal tract. Edemas in the upper airways can cause asphyxiation and death; while the attacks of abdominal pain can lead to unnecessary laparotomies. Due to its characteristics, HAE deeply affects the quality of life of those affected, physically as well as psychologically and socially. This study aims to evaluate the quality of life related to the health of these patients. METHODS: A generic instrument was applied 36 Item Short Form Health Survey Questionnaire (SF-36) to 35 patients over 15 years of age, with clinical and laboratory evidence or simply laboratory evidence of a deficiency of the C1 inhibitor, from four distinct regions of the country. In addition to this measure, a questionnaire containing personal data and clinical history of the patients in the last six months was applied. Based on this last questionnaire, the clinical severity of the HAE was evaluated through a score of six criteria. RESULTS: The patients were predominantly of female gender (71.4%), from urban areas (85.7%), with an average level of education (42.9%) and medium-low socio-economic level (57.1%). In relation to the evaluation of the QL by SF-36, ninety per-cent of the patients presented average SF-36 scores of below 70. The scores of the eight dimensions showed a variation from 51.03 to 75.95; the most negatively affected scales being vitality, social functioning and pain. The internal consistency of the evaluation was demonstrated by a Cronbach alpha coefficient of above 0.7. No correlation was found between the mean SF-36 scores and the parameters of gender, age, education, duration of the disease without diagnosis and clinical severity score, considered in this study. CONCLUSIONS: The quality of life of the patients with HAE is affected in several domains, however there is a need for studies with disease specific questionnaires to better characterize the damage caused by this disease to its sufferers. This information could be applied in health promotion measures which will have repercussions on the bio-psycho-social improvement of these patients

Methods: The World Health Organization quality-of-life assessment instrument (WHOQOL-100) was completed by 69 women with eating disorders (34 with anorexia nervosa, 26 with bulimia nervosa, and 9 with eating disorder not otherwise specified) and 69 healthy women. The Structured Clinical Interview for DSM-IV Axis I Disorders (SCID-I) was used for the diagnosis of psychiatric disorders. Results: Patients with eating disorders reported lower (worse) total quality- of-life scores and lower scores on all quality-of-life domains (physical, psychological, social relationships, environment, level of independence, and spirituality) compared with healthy patients, with the psychological domain being the most impaired. No significant differences in impairment of quality of life were found among eating disorders subtypes. The most prevalent psychiatric comorbidities were mood disorders, alcohol or substance abuse- related disorders, and anxiety disorders. Patients with anorexia nervosa and comorbid panic disorder had greater impairment in quality of life than patients with anorexia nervosa but no panic disorder. Most patients had multiple comorbidities and history of suicide attempts. Conclusions: Patients with eating disorders experience significantly greater impairment in quality of life on all domains, especially on the psychological domain, compared with healthy patients, probably because of peculiarities in eating disorders psychopathology. No difference in quality of life impairment was observed among eating disorders subtypes. Patients with anorexia nervosa and comorbid panic disorder should be carefully evaluated

Full textOBJECTIVES: To compare the quality of life of older kendo practitioners with physically active elderly non-practicing this fight and, secondly, to compare the functional aspects of these groups. METHODS: This is a controlled observational, cross-sectional design study. Since this is a topic not covered in the international literature, we opted for a convenience sample of 20 participants (10 in Kendo group and 10 in the control group). The statistical treatment chosen corresponded to non-parametric analysis with the Mann Whitney U test in order to compare the quantitative data and the chi-square test for comparison of qualitative data with 5% significance level. The groups were matched for age, and the main variable studied the exposure or not to kendo. The evaluation of quality of life was measured by the WHOQOL-bref instrument together with the WHOQOL-old. The body composition was measured in balance of bioimpedance (InBody230). The dynamic balance was measured on the force platform (Balance Master System, Neurocom International INC® Clackamas - USA). The strength of flexion and knee extension was measured in isokinetic (Biodex® Stystem 3 model Biodex Multi Joint System, BIODEX); the palm pressure strength was measured on the dynamometer portable manual (Jamar HS 5001). RESULTS: The quality of life of kendo group was statistically higher than the control group (p = 0.002), especially in Physical domain (p = 0.0001) and Environment (p = 0.004), as well as social participation facet (p = 0.001) and Activities Past, Present and Future (p = 0.01). The groups showed no statistical difference in the sociodemographic characteristics, body composition, physical activity level, strength and dynamic balance, but control group showed statistical difference in the sit-to-stand test (p = 0,03). CONCLUSION: The results suggest that kendo improves the quality of life of fighters, even when compared with a control group functionally superior

The use of zeolites in the cracking of hydrocarbons is limited because the molecules higher than 0.74 nm do not diffuse in their micropores. In the other hand, molecular sieves as MCM-41, MCM-48 ou SBA-15, whose diameters are up 10 nm for the two formers and up to 30 nm for the last, have low hydrothermal stability and catalytic activity. To overcome those limitations, many studies are devoted to prepare materials, which can combine the high activity of zeolites with the better accessibility of the mesoporous materials. Among the possible methods, the synthesis of zeolites in confined spaces (nanocasting) it is seem as a powerful alternative. In this method: 1) a mesoporous or non-porous silica is impregnated with an organic compound, which act as a carbon precursor, after being carbonized; 2) the porous carbon is subsequently obtained by treatment with hydrofluoric acid or alkaline hydroxides; 3) the porous of the carbon are filled with the zeolite synthesis gel, which is formed after crystallization; 4) finally, the mold of the carbon and the template of the zeolite microstructure are eliminated by calcination. In the present work, it was used the above described method for the synthesis of nanocasting ZSM-5 zeolites using porous carbon, which were prepared from non-porous or mesoporous silica moulds (MCM-41, MCM-48 e SBA-15). The XRD of the prepared zeolites did not show diffraction peaks in the low angle region (< 5°2θ), showing that the inverse replica of the mesoporous silicas was not obtained. However, the nitrogen sorption measurements evidenced the presence of mesoporous, which resulted from the carbon nanotubes occluded during the ZSM-5 crystals formation. The mesopores observed in the Znp, nanocast from non-porous silica, were attributed to the inter-crystalline spaces generated by the agglomeration of ZSM-5 nanocrystals. The mesoporous formation in the ZSM-5 zeolites during the crystallization in a confined space (nanocating) was considerately favored by the ageing of the synthesis gel at the laboratory temperature and without stirring. That procedure allows the preparation at 180 °C of ZSM-5 zeolites with high crystallinity and in a lower time. Nevertheless, as verified by 27Al-NMR, the aluminum was incorporated partially in the zeolite microstructure, remaining as alumina in the crystal surface, which leads to a lower activity in the n-decane cracking when compared with a conventional ZSM-5 zeolite with a similar Si/Al ratio.
A utilização de zeólitas no craqueamento de hidrocarbonetos é limitada devido a não permitir a difusão de moléculas de tamanho maior que o diâmetro de seus microporos (< 0,74 nm). Por outro lado, as peneiras moleculares mesoporosas, como a MCM-41, MCM-48 ou SBA-15 com diâmetro de poros de até 10 nm, no caso das duas primeiras, e de até 30 nm no caso da última, possuem baixas estabilidade hidrotérmica e atividade catalítica. Para superar essas restrições, estuda-se a preparação de materiais que combinem a elevada atividade catalítica das zeólitas com a melhor acessibilidade dos materiais mesoporosos. Dentre os métodos possíveis, a síntese de zeóltas em espaço confinado (nanomoldagem), surge como uma potencial alternativa. Nessa técnica: 1) uma silica mesoporosa ou não porosa é impregnada com um composto orgânico precursor de carbono, sendo o sólido posteriormente carbonizado; 2) o carbono poroso é obtido após dissolução da sílica com ácido fluorídrico ou hidróxidos alcalinos; 3) os poros do carbono são preenchidos com o gel de síntese da zeólita, a qual se forma após cristalização; 4) finalmente o molde de carbono e o agente direcionador da microestrutura são eliminados via calcinação. No presente trabalho, utilizou-se a técnica descrita para a síntese de zeólitas ZSM-5 nanomoldadas em carbonos porosos, os que foram obtidos utilizando como molde sílicas não porosas ou mesoporosas (MCM-41, MCM-48 e SBA-15). Os difratogramas de raios X das zeólitas preparadas não apresentaram picos de difração em ângulos menores que 5°(2θ), mostrando que o ordenamento das fases mesoporosas não foi replicado, porém, os resultados de fisissorção de nitrogênio indicaram a presença de mesoporos, sugerindo que nanotubos de carbono formados nos mesoporos dos moldes de sílica foram ocluídos nos cristais da ZSM-5. Os mesoporos observados nas zeólitas Znp, nanomoldadas em carbono obtido da sílica não porosa, foram atribuídos aos espaços intercristalinos resultantes do empacotamento de cristais nanométricos da zeólita ZSM-5. A formação de mesoporos nas zeólitas ZSM-5 durante o processo de cristalização em espaço confinado (nanomoldagem) foi consideravelmente favorecida pelo envelhecimento do gel de síntese à temperatura ambiente e sem aplicação de agitação. Esse procedimento permitiu a obtenção a 180 °C de zeólitas com alta cristalinidade e em menor tempo de cristalização. Entretanto, como verificado por 27Al-RMN, o alumínio foi incorporado parcialmente na estrutura da zeólita, permanecendo como alumina na superfície dos cristais, o que provocou menor atividade para o craqueamento de n-decano quando comparado com uma zeólita ZSM-5 convencional com relação Si/Al próxima.

Full textIntroduction: Only few studies assessed health-related quality of life (HRQOL) and quality-adjusted life years (QALY) of cancer patients admitted to intensive care units (ICU). We aimed to assess the long-term HRQOL and QALY of cancer patients admitted to ICUs. Methods: We conducted a prospective cohort study of cancer patients admitted to two ICUs from the state of São Paulo, Brazil. We assessed the HRQOL with the EQ-5D-3L before ICU admission, 15 days, 3, 6, 12 and 18 months. In addition, the vital status was assessed at 24 months. Survival was calculated with the Kaplan-Meier estimator and QALY with the adapted Zhao and Tsiatis estimator. Results: The mean age of the subjects was 61.6 ± 14.3 years, 42.5% were female and half were admitted after elective surgery. The mean Simplified Acute Physiology Score (SAPS) 3 was 47.4 ± 15.6. Survival at 12 and 18 months was 42.4% and 38.1%, respectively. The mean EQ5D utility measure before admission to the ICU was 0.47±0.43, at 15 days it was 0.41±0.44, at 90 days 0.56±0.42, at 6 months 0.60±0.41, at 12 months 0.67±0.35 and at 18 months 0.67±0.35. The probabilities for attaining 12 and 18 months of quality-adjusted survival were 30.1% and 19.1%, respectively. There were statistically significant differences in survival time and QALYs according to all assessed baseline characteristics (ICU admission after elective surgery, emergency surgery or medical admission; SAPS3; cancer extension; cancer status; previous surgery; previous chemotherapy; previous radiotherapy; performance status; and previous HRQOL). Only the previous HRQOL and performance status were associated with the HRQOL during the 18-month follow-up. Conclusion: Long-term HRQOL, survival and QALY expectancy of cancer patients admitted to the ICU are limited. Nevertheless, these clinical outcomes exhibit wide variability among patients and are associated with simple characteristics present at the time of ICU admission, which may help healthcare professionals estimate patients\' prognose

Full textMaternity experiences in Brazilian prison has been growing considering the increase of incarcerated women. This complex and adverse condition call upon researchers to deepen these experiences\' knowledge in order to make them socially visible. This research aims to apprehend and understand the experience of maternity in prison through everyday life and life trajectories of women who got out of prison. This is an exploratory, descriptive and reflexive, qualitative study. We used dialectical hermeneutics framework and constructed two oral histories, using the everyday life concept as main axis to analyze them. We identified ten categories in a temporal perspective of the collaborators trajectories. The results of the \"Pregnancy, birth and after birth\" experience show that pregnant women had different vulnerability conditions and risks for their physical integrity and the baby\'s as well. There was precarious access to health care and rights violation. In the experience of \"Maternity in prison\" we verified that when mother and baby stayed together in the institution, opposing the helplessness they lived, women developed solidarity practices as a mean of organization and resistance of the difficulties and material-affective privation they experimented. The experience of children care was perceived as pleasurable and exhausting at the same time due to the intensive and exclusive children care and to the sadness and hardness experienced in prison. Therefore, considering the precariousness and the rigid prison rules, women built strategies to optimize and guarantee the babies\' care and their self-care by adopting a submissive and resistance stances rotatively, and the recreation of their daily lives. The announced separation of mother-baby has permeated the women\'s ideals during the whole pregnancy and childcare period, anticipating the suffering caused by the separation moment. After delivering the children for their families, women developed singular ways to deal with the suffering and with the physical, emotional and financial load caused to their families. In the \"Life after prison\" period, the collaborators had difficulties in reestablishing the contact with their children and were uncertain about the consequences of their relation with them. The barriers, prejudice and precariousness of the access to the social policies and the support for social inclusion exacerbated it. As a result, women needed to act by themselves in order to have a life plan that would guarantee their children\'s future after imprisonment. We concluded that everyday life in prison revels to be a violator and standardizer of the maternal experience and its relation to the children. Hence, we verified that maternity experience was used as punishment for women, causing damage to their children that might be irreparable and that goes beyond the prison space-time. Considering the violations and suffering women experienced, they constructed spaces for recreating and resisting to the constraining everyday life in prison
