Abstract:
AbstractIn this paper, we first show that the central height of the unit group of the integral group ring of a periodic group is at most 2. We then give a complete characterization of the n-centre of that unit group. The n-centre of the unit group is either the centre or the second centre (for n ≥ 2).

Abstract:
Objectives: There is emerging interest in models of care that focus on assessment and brief inpatient treatment (two to three days) including psychiatric emergency care centre units and short-stay units in Australia. We present the development of a functionally integrated Missenden Assessment Unit and six-bed short-stay unit in the new Professor Marie Bashir Centre at Royal Prince Alfred Hospital in inner-city Sydney. The focus was on collaboration between emergency, drug and alcohol and mental-health services in developing the short-stay unit and Missenden Assessment Unit with joint admission and resource use. We outline the models of care and findings from the 2016 evaluation following the initial two years of operation and consider ongoing challenges. Conclusion: The Missenden Assessment Unit provides an alternative point of presentation for mental-health drug and alcohol patients. The short-stay unit provides coordinated, therapeutic interventions. The Missenden Assessment Unit/short-stay unit reduced the burden of presentations to the emergency department while providing the opportunity for training and collaboration. Further refinement of the models of care should occur with policy development and via research.

Abstract:
Background: Neonatal tumours, occurring within 28 days of life, are associated with a favourable outcome in high-income countries. Limited data are available on neonatal tumours in low- and middle-income countries.
Aim: We aimed to create awareness of neonatal tumours in a middle-income country, by identifying the most common tumours diagnosed to determine whether there was a delay to presentation or diagnosis at the Paediatric Oncology unit, to identify possible areas of improvement and increase overall survival. Setting: Chris Hani Baragwanath Academic Hospital Paediatric Oncology unit.
Methods: A retrospective case series was performed collecting data on 60 neonates diagnosed with a tumour over a 25-year period (1 January 1988 to 31 December 2012). Descriptive statistical analysis using percentages and medians were used. Kaplan–Meier analysis was used to calculate survival.
Results: Germ cell tumours were identified as the most common neonatal tumours diagnosed 29/60 (48.3%), whereas malignant soft tissue tumours were the most common malignant tumours identified. The median delay to presentation was 7 days, the median diagnostic delay was 11 days and the median overall delay was 30 days. The overall 5-year survival rate was 67.3%, with an overall 5-year survival of 54.1% of neonates diagnosed with a malignant tumour.
Conclusion: A large percentage of neonates diagnosed with malignant tumours in highincome countries can be successfully treated and cured. Clinicians involved in the care of neonates need to be better acquainted with disease-specific signs of neonatal tumours to allow for early detection and referral to a Paediatric Oncology unit.

Abstract:
Introduction. The aim of this study was to examine the effect of surgeon relocation on lymph node (LN) retrieval in colorectal cancer (CRC) resection.Methods. The study population was 213 consecutive patients undergoing CRC resection by a single surgeon, at two units: unit one 110 operations (2002–2005) and unit two 103 (2005–2009). LN yields and case mix were compared.Results. Median LN harvests were significantly different between the two centres: unit 1: 13 nodes/patient and unit 2: 22 nodes/patient (). In unit one 42% of cases were LN positive and in unit two 48% (). There was no difference in case mix. Multivariate analysis identified unit () and pathologist () as independent predictors of harvest.Conclusions. A surgeon moving units can experience significantly different LN yield following CRC resection. Both units comply with national standards, but the “surgeon's results” at the two units appear to be pathologist dependent. This has implications for nodal harvest as a surrogate marker of surgical quality.

Abstract:
ObjectiveSecond transplant centre opinions (STCOs) for patients declined for liver transplantation are infrequent. We aimed to identify STCOs outcomes from a tertiary transplant centre.DesignReferrals between 2012 and 2020 to Birmingham Unit were reviewed. Incoming: all referrals from out-of-region centres were collated. Outgoing: patients not listed in Birmingham were reviewed to identify referrals for STCOs to the other UK centres (A–F).Results2535 patients were assessed for liver transplantation during the study period. Incoming: among 1751 referrals, 23 STCOs (17 unit A, 3 unit B, 1 unit C, 1 unit D and 1 unit E) were provided by Birmingham. Of the STCOs, 13/23 (57%) patients remained unsuitable for transplantation. Therefore, 10/23 (43%) underwent a second liver transplant assessment, of whom five (50%) were still deemed unsuitable, three (30%) listed (one transplanted) and two (20%) died preassessment. Outgoing: among 426 patients not listed, eight (1.8%) patients were referred for STCO (4 unit E, 2 unit B, 1 unit D, 1 unit A). Three (38%) were listed, two (25%) were assessed and declined, two (25%) were unsuitable for assessment and one (12.5%) died while waiting. Combining incoming and outgoing Birmingham STCOs (n=31), six (19%) of STCOs were listed in a second centre.ConclusionSecond transplant centre opinions are rare with the majority still deemed unsuitable for liver transplantation. This highlights potential resource implications especially when undergoing a full second formal assessment. A streamlined STCO process with sharing of investigations and use of telemedicine in appropriate patients may allow for greater transparency, quicker decision making and less use of labour-intensive resources.

Full text
Abstract:
Introduction: Toxic Epidermal Necrolysis is a drug-induced life-threatening systemic disease, characterized by extensive dermoepidermal detachment and mucositis. At least 95% of cases are believed to be drug-induced. SCORTEN is a scoring system used to stratify severity and predict mortality. Treatment demands immediate withdrawal of the causative drug and early transfer to a burn centre for specific and intensive care.Material and Methods: Authors have performed a retrospective study of 21 consecutive patients with SJS/ Toxic Epidermal Necrolysis admitted in the Burn Centre of Coimbra’s University Hospital, between January 1999 and December 2010, and have compared the actual mortality rate with that predicted by SCORTEN, in order to assess the predictive capacity of SCORTEN. Analysis of results andtreatment options were conducted. Data were analysed in SPSS 17.0®.Results: Thirteen females (61.9%) and 8 males (38.1%) were treated, mean age 55.6 ± 23.7 years and with a mean of 51% ± 22.4% epidermal detachment. The overall observed mortality rate was 47.6% and the one predicted by SCORTEN 42.2%. Immediate withdrawal of the causative drug and early transfer of the patient to our burn centre were the basis of treatment. Conclusion: Toxic Epidermal Necrolysis pathophysiology remains to be clarified and no specific treatment has unequivocally proven to be effective. SCORTEN seems to be an accurate scoring system for estimation of mortality rate.

Abstract:
Packing patterns, a new description of the limited number of possible arrangements of molecular building blocks in a unit cell, were assigned to many thousands of structures belonging to the space groups P21/c, P\bar 1, P212121, P21 and C2/c [Pidcock & Motherwell (2004). Cryst. Growth. Des. 4, 611–620]. The position of the molecular centre (in fractional coordinates) in the unit cell for these structures has been surveyed, with respect to the space group and the packing pattern. The results clearly show that the position at which the molecular centre is found in the unit cell is correlated with the packing pattern. The relationships between the orientation of the packing pattern in the unit cell and the symmetry operators of the space group are explored. Popular orientations of packing patterns within the unit cell are given.

Abstract:
This study aims to investigate the inbound open innovation and firm performance in different time periods across different organisations. Specifically, a conceptual model at firm level analysis was proposed based on literature review. The model was tested with structural equation modeling, using the empirical data which were collected from 60 research centre projects and 62 business unit projects in 2008. Results reveal the causal relationship between the implementation of inbound open innovation and the firm performance in both research centre and business unit. Comparative results showed that regarding research centre, increasing the frequency of inbound open innovation, efficient interal R&D activities and the higher degree of product newness make projects stable and efficient; consequently, the success of firm performance is achieved. As for the business unit, the frequently inbound open innovation practices are necessary for terms of generating efficiency and outcomes, and efficient inter R&D stargeties are significant conditioned by the degree of product newness, contributing to the firm performance as well.

Abstract:
Introduction: Multiple sclerosis (MS) is an immune-mediated neurodegenerative disease. Patients are commonly diagnosed when they are between 20 to 50 years and require life-long treatment. Appropriate treatment is complex and involves a wide range of professional disciplines and expertise. This multifariousness highlights the need for multidisciplinary MS care units that offer a comprehensive and well-managed treatment approach. Such care units would enable (1) time-efficient organization and coordination, (2) better inter-, intradisciplinary and patient communication, (3) neurologists, nurses and therapists specialized in MS and (4) formalized diagnostic workup procedures and protocols for the initiation and follow-up of disease-modifying therapies. Methods: The current single-centre pilot study evaluated such a multidisciplinary MS care unit by measuring patient satisfaction and quality of life prior to and after the reorganization of a Swiss clinical centre. Results: A significant improvement in patient satisfaction was seen after the reorganization of the Swiss clinical centre into an MS centre, and quality of life was maintained throughout. Conclusion: This study exemplifies the importance of MS-specialized care and encourages further longitudinal, large-scale multicentre studies.

AbstractThis newly discovered and excavated site defines an Upper Palaeolithic activity unit consisting of a roasting pit at the centre of an area 5m across. Although the main task was the processing of two mammoths, there were numerous other wild animals in the assemblage. The occupants used flint knives, made bone tools and modelled in baked clay – on which they left their fingerprints, along with imprints of reindeer hair and textiles. Pavlov VI offers an exemplary picture of the basic living unit that made up the settlement clusters of the Gravettian people in Central Europe.

Abstract:
Raw milk quality is one of the most important factor before processing of milk and milk products. It is the main factor which is responsible for deciding the quality of end products. The present study was carried out at the selected BMC centre of a FPO owned dairy unit in Chittoor district of Andhra Pradesh. The principal objective of the study was to draw a comparison between BMC standards of NDDB and standards of selected BMC centre. The findings of the study show that there are no major deviations except delay in opening and closing timings for milk collection. As a result preparation time is reduced hindering the efficiency of milk collection process. BMC unit is well-being running manually by operator and switch was being turned off once BMC centre milk temperature touches 40C and starts at 60C, due to this delay in manual cutoff the milk is being crystallized. As per NDDB standards auto mode is recommended so that the BMC switches off automatically once the milk temperature reaches 40C and restarts at 60C at BMC centre.

Abstract:
AbstractDay-stay surgery is an integral part of Otolaryngology. Many procedures which have traditionally been thought to require overnight stay can be carried out on a day care basis. We report our experience of treating 163 patients admitted for septal surgery as a day-case procedure. The paper summarizes the experience of two centres. One of these is a London Teaching Hospital, where surgery was performed through a dedicated day-case unit, and the other is a District General Hospital where patients were admitted to a day-case unit but had their surgery on a routine in-patient list.As the result of this study we conclude that day-stay septoplasty is associated with a low complication rate and is a safe and acceptable procedure provided that strict selection criteria are followed. The cost implications are discussed.

Abstract:
AbstractObjective:Dedicated otolaryngology high dependency units are uncommon. This paper reports the first experiences of such a facility in the UK, assessing reason for admission, duration of stay, occupancy rate and need for care escalation. The study sought to assess the presence of similar units in the UK.Methods:A retrospective review of high dependency unit admissions over an 18-month period and a national survey of otolaryngology departments in the UK were conducted to establish the overall presence and location of similar high dependency units. Results:A total of 128 patients were admitted during the study period, mainly following surgery and because of airway compromise. The average duration of stay was 2–3 days (range, 1–12 days). The occupancy rate was 31.7 per cent. No patients required their care to be escalated to the intensive care unit. Seven similar high dependency units were identified in the UK.Conclusion:The care provided prevented the need for escalation of care to an intensive care unit. This challenges the need for patient management on intensive care units following major surgery or airway compromise for those not requiring assisted ventilation. High dependency units similar to ours are not widespread.
