Abstract:
Uncertainty in Wastewater Treatment Design and Operation aims to facilitate the transition of the wastewater profession to the probabilistic use of simulators with the associated benefits of being better able to take advantage of opportunities and manage risk. There is a paradigm shift taking place in the design and operation of treatment plants in the water industry. The market is currently in transition to use modelling and simulation while still using conventional heuristic guidelines (safety factors). Key reasons for transition include: wastewater treatment simulation software advancements; stricter effluent requirements that cannot be designed for using traditional approaches, and increased pressure for more efficient designs (including energy efficiency, greenhouse gas emission control). There is increasing consensus among wastewater professionals that the performance of plants and the predictive power of their models (degree of uncertainty) is a critical component of plant design and operation. However, models and simulators used by designers and operators do not incorporate methods for the evaluation of uncertainty associated with each design. Thus, engineers often combine safety factors with simulation results in an arbitrary way based on designer ‘experience’. Furthermore, there is not an accepted methodology (outside modelling) that translates uncertainty to assumed opportunity or risk and how it is distributed among consultants/contractors and owners. Uncertainty in Wastewater Treatment Design and Operation documents how uncertainty, opportunity and risk are currently handled in the wastewater treatment practice by consultants, utilities and regulators. The book provides a useful set of terms and definitions relating to uncertainty and promotes an understanding of the issues and terms involved. It identifies the sources of uncertainty in different project phases and presents a critical review of the available methods. Real-world examples are selected to illustrate where and when sources of uncertainty are introduced and how models are implemented and used in design projects and in operational optimisation. Uncertainty in Wastewater Treatment Design and Operation defines the developments required to provide improved procedures and tools to implement uncertainty and risk evaluations in projects. Abstract:
Although uncertainty is a common element of patient care, it has largely been overlooked in research on evidence-based medicine. This book strives to correct this glaring omission. Applying the tools of economics to medical decision making, the book shows how uncertainty influences every stage, from risk analysis to treatment, and how this can be reasonably confronted. In the language of econometrics, uncertainty refers to the inadequacy of available evidence and knowledge to yield accurate information on outcomes. In the context of health care, a common example is a choice between periodic surveillance or aggressive treatment of patients at risk for a potential disease, such as women prone to breast cancer. While these choices make use of data analysis, the book demonstrates how statistical imprecision and identification problems often undermine clinical research and practice. Reviewing prevailing practices in contemporary medicine, the book discusses the controversy regarding whether clinicians should adhere to evidence-based guidelines or exercise their own judgment. It also critiques the wishful extrapolation of research findings from randomized trials to clinical practice. Exploring ways to make more sensible judgments with available data, to credibly use evidence, and to better train clinicians, the book helps practitioners and patients face uncertainties honestly. It concludes by examining patient care from a public health perspective and the management of uncertainty in drug approvals. The book explains why predictability in the field has been limited and furnishes criteria for more cogent steps forward.

Disorders of consciousness (DoCs) raise difficult and complex questions about the value of life for persons with impaired consciousness, the rights of persons unable to make medical decisions, and our social, medical, and ethical obligations to patients whose personhood has frequently been challenged and neglected. Recent neuroscientific discoveries have led to enhanced understanding of the heterogeneity of these disorders and patients, and focused renewed attention on the disturbingly high rate of misdiagnosis. This book examines the entanglement of epistemic and ethical uncertainty in DoCs and other medical contexts, and how they interact to create both epistemic and ethical risks. Philosopher and bioethicist L. Syd M Johnson pulls together multiple threads—the ontological mysteries of consciousness, medical uncertainty about unconsciousness, ableist bias, withdrawal of treatment in neurointensive care, and the rarely questioned view that consciousness is essential to personhood and moral status. Johnson challenges longstanding bioethical dogmas about DoC patients, and argues for an ethics of uncertainty for contexts where there is a need for decisive action in the presence of unavoidable uncertainty. The ethics of uncertainty refocuses ethical inquiry concerning persons with DoCs, placing less emphasis on their contested personhood, and more on inductive risk and uncertainty, on respect for autonomy, and especially on epistemic justice, and the duties of privileged epistemic agents. It is an approach with applications beyond brain injury, encouraging an expansive and humane approach that enables surrogate decision makers facing fraught, complex, risky choices to fulfill their obligations as moral and epistemic agents.

Clinicians and researchers who work with children and adolescents (i.e., youth) have long known of the value of collecting clinical reports from multiple informants. Along with youth self-reports, a number of significant others in lives of youth can provide reliable and valid reports about youth mental health problems. However, these informants’ reports often yield discrepant conclusions about both the presence of problems and treatment response, and only recently have controlled laboratory studies directly tested the idea that these discrepancies may yield valuable information on individual differences in youth’s clinical presentations. Consequently, clinicians and researchers continue to encounter uncertainty about constructing treatment response assessments. This chapter discusses key developmental considerations when constructing treatment response assessments; future research directions are highlighted for decreasing the uncertainty that clinicians and researchers currently face with selecting, using, and interpreting multi-informant assessments when treating youth.

Intolerance of uncertainty (IU) is a key cognitive construct in the maintenance of obsessive-compulsive disorder (OCD) symptoms. Whereas most individuals feel “certain-enough” that situations are relatively safe, those with OCD who have elevated IU have difficulty managing the feeling of not knowing “for sure” whether a feared outcome may occur. As a result, they engage in compulsive rituals (e.g., checking, reassurance seeking) with the aim of restoring a sense of certainty. Given the pervasiveness of uncertainty in daily life, these doubts and rituals can lead to heightened daily distress for individuals with OCD. Accordingly, the present chapter reviews the following: (a) a comprehensive definition of IU, (b) the conceptualization of IU as important in the development and maintenance of OCD across various symptom presentations, (c) the measurement of IU using both self-report and behavioral methods, and (d) recommendations for the consideration of IU in OCD treatment.

This chapter provides insight into practice and legislative changes that relates to advance decisions to refuse treatment and advance care planning (ACP) for end of life. It sets this in the context of the Medical Control Agency (MCA) which provides the legal framework In England and Wales. The chapter explore aspects of the impact of this legislation including Lasting Power of Attorney, and Deprivation of Liberty Safeguards, the role of the Office of the Public Guardian and Court of Protection. Case law examples are given relating to decisions on mental capacity and best interests decisions including life sustaining treatment. Also discussed are: ADRT and children; decisions relating to CPR; assisted suicide, legal challenges and reform. The chapter concludes with a reflection on dilemmas that persists. It offers positive suggestions to move forwards, postulating that ACP and advance decisions overall reduce uncertainty and increases the chances of achieving patient-centred outcomes.

What do states do with the opponents they have imprisoned? This chapter explores the treatment of prisoners as a way to understand political incarceration. For political prisoners, arbitrary and uncertain rules replace rights in the twentieth century. Prisoners see rules as capricious, but the capriciousness undermines prisoners’ sense of themselves and of their fate. Prison denies ordinary sensations and strips prisoners of their identity, even of their humanity. It instills uncertainty through the use of informers, purposeless labor, and torture. Examples in the chapter include Nazi Germany, Poland in the 1930s and in the Stalinist era, British Northern Ireland during the Troubles, and apartheid South Africa.

Clinicians need critical appraisal skills to weigh up the quality of the literature and to decide whether it has implications for their practice. Every belief in medicine is a construct created from the limited information available and carries a degree of uncertainty. A centralized approach to research identifies the highest priority areas of uncertainty so as to bring about the most improvement for the largest number of patients. This patient-centred model classifies studies into basic science, translational, and clinical research. Adequately powered mega-trials are required to bridge the translational gap between efficacy (does a treatment work?) and effectiveness (does a treatment work when applied in ordinary clinical practice?). There is a paucity of these in perioperative medicine.
