Abstract:

Abstract:
Abstract Background The “Undetectable equals Untransmittable (U=U)” HIV prevention campaign is a cornerstone of HIV prevention. However, there are few recommendations to guide patients and providers in U=U implementation and limited data on risk factors for viral rebound among persons eligible for U=U. Methods We conducted a retrospective multi-center study using data from the CNICS HIV research network to identify risk factors for viral rebound among persons with established viral suppression [two viral loads (VL) and all VLs of &lt; 200 copies/ul within a one-year period (U=U eligible)]. Demographics, patient-reported outcomes, and longitudinal clinical data from 21,359 persons with HIV were analyzed. To include missing data in the analysis, they were treated as a separate category. The primary outcome of viral rebound was defined as any VL &gt; 200 copies/ul within two years after U=U eligibility. A univariable logistic regression model was conducted to identify predictors of viral rebound. Significant variables (p&lt; 0.05) were included in a multivariable logistic regression model. Predictive values of individual variables were captured by adjusted odds ratios (aORs). Results From 2011-2019, 12,150 patients met criteria for U=U eligibility and had two years of follow up data. The median age was 46 (IQR: 38-53); 68% male; 51% were white, 39% black. 1544 (13%) experienced viral rebound during follow-up. Forest plot summaries of univariable and multivariable logistic regression models are in Figures 1&2. In multivariable analysis, Black race (aOR=1.56, p&lt; 0.001); MSM-IDU risk (aOR=1.38, p=0.006); lower QoL score (aOR=1.49, p=0.005); poorer ART adherence (aOR=1.84, p&lt; 0.001); duration of lifetime ART [aOR=1.47 (10+yrs), = 1.37 (5-10 yrs); and = 1.28 (2-5 yrs), p&lt; 0.001]; use of InSTIs after eligibility (aOR=1.60, p&lt; 0.001); current smoker (aOR=1.49, p&lt; 0.001), current amphetamine (aOR=1.83, p&lt; 0.001) or cocaine use (aOR=1.46, p=0.012), were associated with viral rebound. In both analyses, older age was protective against viral rebound. Figure 1. Summary of Univariate Logistic Regression Model Figure 2. Summary of Multivariable Logistic Regression Model Conclusion We identified multiple risk factors for viral rebound among PWH with viral suppression. Further research is needed to identify synergistic risk factors that increase probability of viral rebound to inform optimal implementation of U=U. Abstract:
Abstract OBJECTIVE: We present a quantitative comparison of radiosurgery treatments for cavernous sinus tumors using the Leksell gamma knife Model U versus the Model C with automatic positioning system (APS) (Elekta Instruments, Norcross, GA). METHODS: At our medical center from August 1994 through May 2000, the Model U was used to treat 96 patients (37 men [39%] and 59 women [61%]; median age, 54.5 yr) with benign cavernous sinus tumors: 43 meningiomas (45%), 48 pituitary tumors (50%), and 5 others (5%). From June 2000 through April 2002, the Model C with APS treated 45 patients (20 men [44%] and 25 women [56%]; median age, 51.4 yr) with 15 meningiomas (33%), 29 pituitary tumors (65%), and 1 schwannoma (2%). The two groups had similar treated tumor volumes (Model U mean, 4.3 cm3; Model C mean, 4.2 cm3), equivalent tumor distances from critical structures (optic nerve, chiasm, and pons), comparable distributions in Sekhar tumor grades, and the same median prescribed dose of 15 Gy to the 50% isodose line at the tumor periphery. All planning and treatments were performed by the same radiosurgery team to minimize dosage to adjacent critical tissues and to optimize conformity index. RESULTS: Analysis of multiple treatment parameters showed that the Model C plans were superior. Model C treatments had an improved conformity index (Model U mean, 1.7; Model C mean, 1.6; P &lt; 0.02) and a lower underdosed tumor volume (Model U mean, 0.4 cm3; Model C mean, 0.1 cm3; P &lt; 0.004). The total treated volume and the excess treated volume were similar. The Model C group had a reduction in optic chiasm dose (Model C mean dose, 3.8 Gy; Model U mean dose, 5.3 Gy; P &lt; 0.0001). The average number of isocenters was slightly higher for the Model C group (6.7 versus 6 for the Model U), but with a lower mean number of collimator sizes (1 versus 2 for the Model U). Model C plans required a mean of 93 fewer plugs per treatment, thus contributing to an estimated 67.6 minutes saved per treatment session. CONCLUSION: Comparison of radiosurgery treatments using the Leksell gamma knife Model U versus the Model C with APS was performed by quantitative analysis of treatment parameters on a cohort of benign cavernous sinus tumors. Treatment plans using the Model C resulted in better tumor coverage (improved conformity, less underdosed tumor volume) and decreased optic chiasm dose. An estimated average of 1 hour was saved per treatment when using the Model C with APS.

Abstract:
We consider a two-dimensional ruin problem where the surplus process of business lines is modelled by a two-dimensional correlated Brownian motion with drift. We study the ruin function P ( u ) for the component-wise ruin (that is both business lines are ruined in an infinite-time horizon), where u is the same initial capital for each line. We measure the goodness of the business by analysing the adjustment coefficient, that is the limit of - ln P ( u ) / u as u tends to infinity, which depends essentially on the correlation ρ of the two surplus processes. In order to work out the adjustment coefficient we solve a two-layer optimization problem.

Abstract:
The attractive (U < 0) and repulsive (U > 0) Hubbard models have been studied using the Fermi liquid perturbation approach. The attractive Hubbard model (U < 0) is an adequate model for 3 He , an incompressible and strongly paramagnetic liquid [Formula: see text], [Formula: see text] for |U|N F = 0.9) with a pseudogap in the charge response. A pairing instability and superfluidity for U < 0 exists in the spin channel only: spin-triplet with l = 0, or spin-singlet with l = 1 (p-wave orbital state j = s + l = 1), where l is the orbital momentum of a pair. The repulsive Hubbard model (U > 0) represents a highly compressible and nearly antiferromagnetic liquid [Formula: see text], [Formula: see text] for UN F = 0.9) with a pseudogap in the spin response. However, for U > 0 a pairing instability and superconductivity exist in the charge channel only: spin-singlet with l = 0 (s-wave), or with l = 2 (d-wave) in the case of an anisotropic Fermi surface.

AbstractThis paper is concerned with the following type of quasilinear elliptic equations in{\mathbb{R}^{N}}involving thep-Laplacian and critical growth:-\Delta_{p}u+V(|x|)|u|^{p-2}u-\Delta_{p}(|u|^{2})u=\lambda|u|^{q-2}u+|u|^{2p^{% *}-2}u,which arises as a model in mathematical physics, where{2<p<N},{p^{*}=\frac{Np}{N-p}}. For any given integer{k\geq 0}, by using change of variables and minimization arguments, we obtain, under some additional assumptions onpandq, a radial sign-changing nodal solution with{k+1}nodal domains. Since the critical exponent appears and the lower order term (obtained by a transformation) may change sign, we shall use delicate arguments.

Abstract:
The amount of photopolymer material consumed during the three-dimensional (3D) printing of a dental model varies with the volume and internal structure of the modeling data. This study analyzed how the internal structure and the presence of a cross-arch plate influence the accuracy of a 3D printed dental model. The model was designed with a U-shaped arch and the palate removed (Group U) or a cross-arch plate attached to the palate area (Group P), and the internal structure was divided into five types. The trueness and precision were analyzed for accuracy comparisons of the 3D printed models. Two-way ANOVA of the trueness revealed that the accuracy was 135.2 ± 26.3 µm (mean ± SD) in Group U and 85.6 ± 13.1 µm in Group P. Regarding the internal structure, the accuracy was 143.1 ± 46.8 µm in the 1.5 mm-thick shell group, which improved to 111.1 ± 31.9 µm and 106.7 ± 26.3 µm in the roughly filled and fully filled models, respectively. The precision was 70.3 ± 19.1 µm in Group U and 65.0 ± 8.8 µm in Group P. The results of this study suggest that a cross-arch plate is necessary for the accurate production of a model using 3D printing regardless of its internal structure. In Group U, the error during the printing process was higher for the hollowed models.

Data-driven machine learning has become ubiquitous. A marketplace for machine learning models connects data owners and model buyers, and can dramatically facilitate data-driven machine learning applications. In this paper, we take a formal data marketplace perspective and propose the first en<u> D </u>-to-end mod <u>e</u> l m <u>a</u> rketp <u>l</u> ace with diff <u>e</u> rential p <u>r</u> ivacy ( Dealer ) towards answering the following questions: How to formulate data owners' compensation functions and model buyers' price functions? How can the broker determine prices for a set of models to maximize the revenue with arbitrage-free guarantee, and train a set of models with maximum Shapley coverage given a manufacturing budget to remain competitive ? For the former, we propose compensation function for each data owner based on Shapley value and privacy sensitivity, and price function for each model buyer based on Shapley coverage sensitivity and noise sensitivity. Both privacy sensitivity and noise sensitivity are measured by the level of differential privacy. For the latter, we formulate two optimization problems for model pricing and model training, and propose efficient dynamic programming algorithms. Experiment results on the real chess dataset and synthetic datasets justify the design of Dealer and verify the efficiency and effectiveness of the proposed algorithms.

<p><strong>Cilj</strong>. In this paper we provide a rigorous asymptotic analysis of a phase-field model used to simulate pressure-driven fracture propagation in poro-elastic media. More precisely, assuming a given pressure p ∈ W 1,∞ (Ω) we show that functionals of the form $$ E(\vec{u})={\int }_{\mathrm{\Omega }} e(\vec{u}):\mathbb{C}e(\vec{u})+p\nabla \cdot \vec{u}+\left\langle \nabla p,\vec{u}\right\rangle\enspace \mathrm{d}x+{\mathcal{H}}^{n-1}({J}_{\vec{u}}),\enspace \vec{u}\in \mathrm{G}{SBD}(\mathrm{\Omega })\cap {L}^1(\mathrm{\Omega };{\mathbb{R}}^n) $$ can be approximated in terms of Γ-convergence by a sequence of phase-field functionals, which are suitable for numerical simulations. The Γ-convergence result is complemented by a numerical example where the phase-field model is implemented using a Discontinuous Galerkin Discretization.

Abstract:
SummaryTo investigate the potential role of tissue-type plasminogen activator (t-PA) or urokinase-type plasminogen activator (u-PA) in development of adipose tissue, we have used a nutritionally induced obesity model in t-PA (t-PA−/−) and u-PA (u-PA−/−) deficient mice. Five week old male wild-type (WT), t-PA−/− or u-PA−/− mice (n = 9 to 16) were fed a high fat diet (HFD, 42% fat). After 16 weeks of HFD, the body weight of t-PA−/− mice was significantly higher than that of WT mice (48 ± 1.1 g vs. 39 ± 2.2 g, p = 0.004). The total weight of the isolated subcutaneous (sc) fat deposit was higher in t-PA−/− than in WT mice (2.4 ± 0.22 g vs. 1.2 ± 0.29 g, p = 0.002), accompanied with higher adipocyte diameters (80 ± 1.7 µm vs. 61 ± 4.7 µm, p < 0.01). These differences were not observed in the intra-abdominal fat deposit. The number of stroma cells in both adipose tissue territories was increased in t-PA−/− as compared to WT mice (2.0 ± 0.13 vs. 1.5 ± 0.10 p = 0.02 and 3.0 ± 0.17 vs 1.6 ± 0.17, p = 0.0001, stroma cells/ adipocytes in sc and intra-abdominal tissue, respectively), partly as a result of an increased number of endothelial cells (192 ± 9 vs. 154 ± 18 p = 0.06 and 108 ± 13 vs. 69 ± 8 p = 0.04 CD31 stained/adipocyte area). In contrast the weight gain and adipose tissue development in u-PA−/− mice was not different from that in WT mice. These data suggest that t-PA but not u-PA plays a role in adipose tissue development.

Abstract:
To investigate the neuroprotective effects of pretreatment with Fe3O4 methylprednisolone plus calpain inhibitor against spinal cord ischemia-reperfusion injury (SCI) in rats. A total of 18 male Sprague Dawley rats were randomly divided into sham operation, model, and pretreatment + SCI groups (receiving 25 mg/kg Fe3O4 methylprednisolone plus calpain inhibitor) (n = 6 each). The SCI model was established by clamping the left renal inferior abdominal aorta. Hindlimb motor function was evaluated 6, 12, 24, and 48 hours after reperfusion. At 48 hours after reperfusion, spinal cord tissues were collected for histopathological examination, measurement of oxidation product accumulation (malondialdehyde, MDA), and antioxidant enzyme activity assays [superoxide dismutase (SOD), catalase (CAT), and glutathione peroxidase (GSH-PX)]. Motor function scores were compared using non-parametric rank sum (Kruskal-Wallis) test, and mean tissue MDA content and antioxidant enzyme activities were compared using one-way analysis of variance. Compared with the sham operation group, the model group showed significantly increased MDA content (2.95 vs. 5.90 g mg; P = 0.023); significantly increased GSH-Px activity (0.31 vs. 0.78 U/mg; P = 0.001); significantly decreased SOD (16.03 vs. 8.37 U/mg; P = 0.001) and CAT (1.24 vs. 0.54 U/mg; P = 0.002) activities; and significantly decreased hindlimb motor function score at 6 hours (21 to 15.5; P =0.001). Compared with the model group, the pretreatment + SCI group showed significantly decreased MDA content (5.90 vs. 3.27 mg; P = 0.0355); significantly decreased GSH-Px activity (0.78 vs. 0.36 U/mg; P = 0.001); and significantly increased SOD (8.37 vs. 14.79 U/mg; P = 0.001) and CAT (0.54 vs. 0.89 U/mg; P = 0.043) activities. Histopathological changes were significantly less severe in the pretreatment + SCI group than in the model and sham operation groups (P < 0.05). Pretreatment with Fe3O4 methylprednisolone plus calpain inhibitor can prevent SCI by improving the balance between oxidation and antioxidant systems.

Abstract:
Orthopedic suture, as an implantable surgical device for skeletal and soft tissue connection, is vital in tendon or ligament injury repair. Resorbable therapy approaches exhibit excellent biocompatibility in the field of suture materials but lack a long-term fixation effect in orthopedic treatment. Herein, this study focused on a series of partially absorbable orthopedic sutures, which were composed of absorbable polycaprolactone (PCL) multifilament and non-absorbable ultra-high molecular weight polyethylene (UHMWPE) multifilament. Comprehensive in vitro mechanical evaluations were conducted to probe the relationship between material composition and mechanical properties of the sutures. The results showed that the partially absorbable sutures, especially P/U = 50/50 and P/U = 25/75, exhibited significant improvements in mechanical properties compared to single-material sutures. The tensile strength of P/U = 50/50 and P/U = 25/75 was 180.99 and 210.91 N, respectively, which was about two times higher than that of absorbable PCL suture P/U = 100/0 (62.42 N). Furthermore, their suture-to-suture friction force was 1.89 times and 2.51 times that of non-absorbable UHMWPE suture P/U = 0/100, respectively, which guaranteed good knot security. Compared with the clinically used orthopedic suture Ethibond (110 N), P/U = 50/50 and P/U = 25/75 also presented superior tensile properties. Notably, P/U = 50/50 and P/U = 25/75 had similar tensile curves to that of the native tendon/ligament, which might be beneficial to tissue healing. Moreover, the R2 of Eyring's model to simulate the creep curves of each suture was higher than 0.99, which indicated that Eyring's model could be used in predicting the long-term creep behavior of the sutures.

<p><strong>Cilj. Abstract:
The aim of this paper is to investigate the model of the figure of the hero in the plays of Anton Pavlovich Chekhov. In analyzing the model of the figure of the hero, four of the most significant plays by A. P. Chekhov were analyzed in consultation with the literature on the poetics of 20th century drama in order to gain a clearer understanding of how the dramatic world of characters was constructed. Specifically, the focus is on four dramas in four acts: "Three Sisters", "Ivanov", "Seagull" and "Uncle Vanya". For the sake of placing love in the key context of this work, the subject of research is not the drama "Cherry." The paper is entitled thesis on the functioning and construction of characters in dramas based on the principle of irony, and thus sets the goal of examining and proving the title thesis. In doing so, it is very interesting to deal with such a model of hero placed in dramatic situations from the perspective of A. P. Chekhov, a writer who writes his characters as a writer-analyst.

Abstract:
Objective: The aim of this experimental study was to examine the effect of the antioxidant drug “U-74389G” on a rat model using an ischemia reperfusion protocol. The effect of U-74389G was studied biochemically by measuring mean blood creatinine levels. Materials and Methods: Forty rats were used in the study. Creatinine levels were measured at 60 min of reperfusion (groups A and C) or at 120 min of reperfusion (groups B and D), where groups A and B were controls and groups C and D received U-74389G administration. Results: U-74389G administration significantly decreased the predicted creatinine levels by 21.02 ± 5.06% (p = 0.0001). Reperfusion time non-significantly increased the predicted creatinine levels by 4.20 ± 6.12% (p = 0.4103). However, U-74389G administration and reperfusion time together produced a significant combined effect in decreasing the predicted creatinine levels by 11.69 ± 3.16% (p = 0.0005). Conclusion: Independent of reperfusion time, U-74389G administration significantly decreased the creatinine levels in an ischemic rat model. This study demonstrates that short-term U-74389G administration improves renal function by increasing creatinine excretion.

Here f : [0, ∞) → R is a continuous function. This model arises in the studies of population biology of one species with u representing the concentration of the species. We discuss the existence of positive solution when f satisfies certain additional conditions. We use the method of sub- and super-solutions to establish our results.

Abstract:
An analytic pair of dimension n and center V is a pair (V, M) where M is a complex manifold of (complex) dimension n and V ⊂ M is a closed totally real analytic submanifold of dimension n. To an analytic pair (V, M) we associate the class [Formula: see text] of the functions u : M → [0, π/4] which are plurisubharmonic in M and such that u(p) = 0 for each p ∈ V. If [Formula: see text] admits a maximal function u, the triple (V, M, u) is said to be a maximal plurisubharmonic model. After defining a pseudo-metric EV, M on the center V of an analytic pair (V, M) we prove (see Theorem 4.1, Theorem 5.1) that maximal plurisubharmonic models provide a natural generalization of the Monge–Ampère models introduced by Lempert and Szöke in [18].

Abstract:
The maximum sustained swimming speeds (Ums) for large (0.45 m long) and small (0.15 m) Atlantic salmon were respectively 0.91ms−1 and 0.54ms−1. Video and cin6 films of fish swimming close to Ums were analysed to obtain variables required for the application of two hydrodynamic models, those of Lighthill and Yates, to determine the mean thrust (T) and mean power output (P) at these swimming speeds (U) close to Ums. A large fish (‘Salmon’) and a small fish (‘Smolt’) were selected for analysis. For salmon using Lighthill's model, T=0.30N and P=0.26W, and using Yates' model, T=0.28N and P=0.25W (U/=0.87ms−1=0.96Ums). For smolt using Lighthill's model, T=0.0052N and P=0.0019W, and using Yates' model, T=0.0065 N and P=0.0024W (U=0.37ms−1=0.69Ums). The power output for smolt swimming at 0.69Ums was corrected to that required to swim at Ums, giving P=0.0059W (Lighthill's model) and P=0.0074W (Yates' model). At Ums it was assumed that all the red muscle was used. Two fish were selected from each size group and cross-sectioned to estimate their red muscle masses. Using a maximum mass-specific power output of 5–8 W kg− for slow red muscle fibres allowed us to calculate that the large and small fish have a power output capacity of 0.125–0.3 W and 0.007–0.019 W, respectively. The power output values at Ums derived from the different approaches for the large (0.25–0.26 W) and small (0.0059–0.0074 W) salmon agree closely. Effects of scaling are discussed.

Abstract:
For thousands of years, it has been widely believed that walnut is a kind of nut that has benefits for the human body. Walnut oil, accounting for about 70% of walnut, mainly consists of polyunsaturated fatty acids. To investigate the effect of walnut oil on memory impairment in mice, scopolamine (3 mg/kg body weight/d) was used to establish the animal model during Morris Water Maze (MWM) tests. Walnut oil was administrated orally at 10 mL/kg body weight/d for 8 consecutive weeks. The results showed that walnut oil treatment ameliorated the behavior of the memory-impaired mice in the MWM test. Additionally, walnut oil obviously inhibited acetylcholinesterase activity (1.26 ± 0.12 U/mg prot) (p = 0.013) and increased choline acetyltransferase activity (129.75 ± 6.76 U/mg tissue wet weight) in the brains of scopolamine-treated mice (p = 0.024), suggesting that walnut oil could prevent cholinergic function damage in mice brains. Furthermore, walnut oil remarkably prevented the decrease in total superoxide dismutase activity (93.30 ± 5.50 U/mg prot) (p = 0.006) and glutathione content (110.45 ± 17.70 mg/g prot) (p = 0.047) and the increase of malondialdehyde content (13.79 ± 0.96 nmol/mg prot) (p = 0.001) in the brain of scopolamine-treated mice, indicating that walnut oil could inhibit oxidative stress in the brain of mice. Furthermore, walnut oil prevented histological changes of neurons in hippocampal CA1 and CA3 regions induced by scopolamine. These findings indicate that walnut oil could prevent memory impairment in mice, which might be a potential way for the prevention of memory dysfunctions.

Abstract:
Background Life-threatening bleeding requires prompt reversal of the anticoagulant effects of factor Xa inhibitors. This study investigated the effectiveness of four-factor prothrombin complex concentrate in treating trauma-related hemorrhage with rivaroxaban-anticoagulation in a pig polytrauma model. This study also tested the hypothesis that the combined use of a low dose of prothrombin complex concentrate plus tranexamic acid and fibrinogen concentrate could improve its subtherapeutic effects. Methods Trauma (blunt liver injury and bilateral femur fractures) was induced in 48 anesthetized male pigs after 30 min of rivaroxaban infusion (1 mg/kg). Animals in the first part of the study received prothrombin complex concentrate (12.5, 25, and 50 U/kg). In the second part, animals were treated with 12.5 U/kg prothrombin complex concentrate plus tranexamic acid or plus tranexamic acid and fibrinogen concentrate. The primary endpoint was total blood loss postinjury. The secondary endpoints (panel of coagulation parameters and thrombin generation) were monitored for 240 min posttrauma or until death. Results The first part of the study showed that blood loss was significantly lower in the 25 U/kg prothrombin complex concentrate (1,541 ± 269 ml) and 50 U/kg prothrombin complex concentrate (1,464 ± 108 ml) compared with control (3,313 ± 634 ml), and 12.5 U/kg prothrombin complex concentrate (2,671 ± 334 ml, all P &lt; 0.0001). In the second part of the study, blood loss was significantly less in the 12.5 U/kg prothrombin complex concentrate plus tranexamic acid and fibrinogen concentrate (1,836 ± 556 ml, P &lt; 0.001) compared with 12.5 U/kg prothrombin complex concentrate plus tranexamic acid (2,910 ± 856 ml), and there were no early deaths in the 25 U/kg prothrombin complex concentrate, 50 U/kg prothrombin complex concentrate, and 12.5 U/kg prothrombin complex concentrate plus tranexamic acid and fibrinogen concentrate groups. Histopathologic analyses postmortem showed no adverse events. Conclusions Prothrombin complex concentrate effectively reduced blood loss, restored hemostasis, and balanced thrombin generation. A multimodal hemostatic approach using tranexamic acid plus fibrinogen concentrate enhanced the effect of low doses of prothrombin complex concentrate, potentially reducing the prothrombin complex concentrate doses required for effective bleeding control. Abstract:
To investigate the mechanism responsible for underperfusion of working skeletal muscle in heart failure, we measured systemic and femoral bed hemodynamics during treadmill exercise and gracilis muscle resistance during contraction frequencies of 1-9 Hz in 8 dogs ventricularly paced at 260 beats/min for 3 wk to induce heart failure (HF) and in 9 control dogs. At peak treadmill exercise (4 mph, 10%), HF dogs had reduced cardiac outputs (control: 297 +/- 42 vs. HF: 212 +/- 16 ml X min-1 X kg-1) and femoral bed flows (control: 352 +/- 112 vs. HF: 229 +/- 95 ml/min) and elevated arterial lactates [control: 1.7 +/- 0.7 vs. HF: 4.1 +/- 0.7 mM (all P less than 0.04)], consistent with skeletal muscle underperfusion. This muscle underperfusion was associated with reduced mean arteriovenous pressure gradients [control: 119 +/- 12 vs. HF: 91 +/- 9 mmHg (P less than 0.001)] but with normal systemic vascular [control: 21 +/- 6 vs. HF: 23 +/- 5 U (P = NS)] and femoral bed resistances [control: 3.5 +/- 1.6 vs. HF: 4.4 +/- 2.3 X 10(2) U (P = NS)]. Both groups also had similar gracilis muscle minimal resistance during exercise (control: 2.0 +/- 1.1 vs. HF: 1.9 +/- 0.9 X 10(3) U/100 g) and following maximal vasodilation (control: 2.0 +/- 1.0 vs. HF: 2.1 +/- 1.0 X 10(3) U/100 g). These results suggest that 1) short-term rapid ventricular pacing in the dog produces a model of low output HF resembling HF in humans, and 2) skeletal muscle underperfusion in this model is due to a reduced muscle arteriovenous pressure gradient and not to impaired skeletal muscle arteriolar vasodilation.

Abstract:
Background Inflammation is characterized by leukocyte recruitment. Macrophages and neutrophils contribute to tissue damage and organ dysfunction. Modulating leukocyte invasion can protect from these adverse effects. Leukocyte recruitment critically depends on the urokinase-type plasminogen activator receptor (u-PAR). We here use a novel technique to longitudinally quantify cell trafficking in inflammatory models in live animals. Methods Near-infrared fluorophore-labeled leukocytes were adoptively transferred to mice with thioglycollate peritonitis to study leukocyte trafficking to sites of inflammation. Macrophage and neutrophil trafficking was followed with three-dimensional fluorescence-mediated-tomography. u-PAR-/- and wild-type macrophage recruitment was studied by cross-over adoptive cell transfer to elucidate the role of leukocytic versus u-PAR expressed on other cells. Endotoxic shock-induced pulmonary inflammation was used to study u-PARs role for pulmonary neutrophil recruitment. Results Mice experiencing peritonitis showed a significant increase in mean fluorescence intensity because of enhanced macrophage (315%, n=9-10), P&lt;0.05) or neutrophil (194%, n=6, P&lt;0.02) recruitment. Fluorescence-mediated-tomography uncovered a macrophage recruitment defect in the peritonitis model for u-PAR-/- mice (147% of baseline) compared with control mice (335% of baseline, n=8-9, P&lt;0.05). When u-PAR-/--macrophages were transferred to wild-type mice fluorescence intensity increased to 145% while wild-type macrophage transfer into u-PAR-/- resulted in 192% increase compared with baseline (n=6, P&lt;0.05). Reduced neutrophil recruitment in pulmonary inflammation in u-PAR-/- mice was accompanied by improved pulmonary gas exchange. Conclusion Using noninvasive in vivo fluorescence-mediated tomography to image leukocyte recruitment in inflammatory mouse models, we describe a novel macrophage recruitment defect in u-PAR-/- mice. Targeting u-PAR for modulation of leukocyte recruitment is a promising therapeutic strategy to ameliorate leukocyte induced tissue damage.

Abstract:
Allogeneic blood transfusion is associated with increased morbidity and mortality. The authors evaluated the affect of blood transfusion, independent of injury severity on mortality. The authors conducted a retrospective review of all patients, age ≥18 years with blunt injury admitted to their Level 2 trauma center from 1994 to 2004 by query of the NTRACS trauma registry. Initial systolic blood pressure and heart rate determined the shock index. Logistic regression was used to model the affect of blood transfusion on mortality. Transfusion requirements were categorized as follows: A, 0 U; B, 1 to 2 U; C, 3 to 5 U; D, ≥6 U blood. In this sample of 8215 blunt trauma patients, 324 patients received blood transfusion. Mortality rates between the transfused and nontransfused groups were 15.12 per cent and 1.84 per cent ( P < 0.000) respectively. In the logistic regression model, transfusion category B did not have a significant affect on the odds of death ( P = 0.176); the affect of transfusing 3 to 5 U and ≥6 U had a mortality odds ratio of 3.22 ( P = 0.002) and 4.87 ( P = 0.000) respectively. Transfusing ≥2U blood was strongly associated with mortality in this blunt trauma population. There must be a continuous attempt to limit blood transfusion when feasible and physiologically appropriate.

Abstract:
A mathematical model was used to investigate how concentrated urine is produced within the medullary cones of the quail kidney. Model simulations were consistent with a concentrating mechanism based on single-solute countercurrent multiplication and on NaCl cycling from ascending to descending limbs of loops of Henle. The model predicted a urine-to-plasma (U/P) osmolality ratio of ∼2.26, a value consistent with maximum avian U/P osmolality ratios. Active NaCl transport from descending limb prebend thick segments contributed 70% of concentrating capability. NaCl entry and water extraction provided 80 and 20%, respectively, of the concentrating effect in descending limb flow. Parameter studies indicated that urine osmolality is sensitive to the rate of fluid entry into descending limbs and collecting ducts at the cone base. Parameter studies also indicated that the energetic cost of concentrating urine is sensitive to loop of Henle population as a function of medullary depth: as the fraction of loops reaching the cone tip increased above anatomic values, urine osmolality increased only marginally, and, ultimately, urine osmolality decreased.

Abstract:
<p class="zhengwen">Because local weather conditions in moderate climates are changing constantly, heat transfer specifications of substrate and vegetation in vegetated green roofs also change accordingly. Nevertheless, it is assumed that vegetated green roofs can have a positive effect on the thermal performance of construction in winter conditions. Is there proof from scientific research and field testing for this assumption? To answer this question, research is conducted with the main research question: Which parameters defining thermal performance for vegetated green-roof construction for a moderate winter climate like that in the Netherlands can be determined from existing literature, and how do these parameters influence thermal performance? Literature research was executed on monitoring and testing of thermal specifications of vegetated green roofs. Models with physical parameters on vegetated green roofs were studied and compared. The first goal was to make a list of all physical parameters and corresponding variables valid in the Dutch moderate-winter climate. None of the models that were found in the literature seemed to cover all physical processes. These models use parameters and variables to calculate the overall u-value of substrate and vegetation. Nevertheless, one nearly complete model was used for a sensitivity test on variables. Maximum and minimum values of variables were calculated in the model to determine the influence on the outcome in terms of u-value. From this analysis, a distinction could be made between variables influencing the u-value strongly and other variables influencing the outcome weakly.</p><p class="zhengwen">The modelling showed that three variables were influencing the model calculations moderately strongly and therefore the thermal performance of the vegetated green-roof substrate and vegetation. These variables are not consistent with parameters modeling or calculating u-value in constructions. This finding means that contribution to thermal insulation by extensive vegetated green-roof substrate and vegetation in terms of u-value would be negligible. Only a small theoretical contribution to thermal insulation can be argued from weak variables. To be sure about this small theoretical contribution to the u-value of the roof construction, this u-value was used as input for energy-use calculations for residential buildings. These calculations show that such a small increase of the u-value leads to no visible reduction in energy use for heating in winter conditions. The contribution is negligible compared to the influence of the u-value from extra insulation under the roof.</p>For vegetated green roofs in such moderate winter climates as in the Netherlands, additional u-value will have to be proven specifically, because the modelling shows that, in general, no contribution to thermal insulation can be expected.

Abstract:
The shoot of Urtica dioica is used in several cultures as a vegetable or herb. However, not much has been studied about the potential of this plant when consumed as a whole food/vegetable rather than an extract for dietary supplements. In a 12-week dietary intervention study, we tested the effect of U. dioica vegetable on high fat diet induced obesity and insulin resistance in C57BL/6J mice. Mice were fed ad libitum with isocaloric diets containing 10% fat or 45% fat with or without U. dioica. The diet supplemented with U. dioica attenuated high fat diet induced weight gain (p < 0.005; n = 9), fat accumulation in adipose tissue (p < 0.005; n = 9), and whole-body insulin resistance (HOMA-IR index) (p < 0.001; n = 9). Analysis of gene expression in skeletal muscle showed no effect on the constituents of the insulin signaling pathway (AKT, IRS proteins, PI3K, GLUT4, and insulin receptor). Notable genes that impact lipid or glucose metabolism and whose expression was changed by U. dioica include fasting induced adipocyte factor (FIAF) in adipose and skeletal muscle, peroxisome proliferator-activated receptor-α (Ppar-α) and forkhead box protein (FOXO1) in muscle and liver, and Carnitine palmitoyltransferase I (Cpt1) in liver (p < 0.01). We conclude that U. dioica vegetable protects against diet induced obesity through mechanisms involving lipid accumulation and glucose metabolism in skeletal muscle, liver, and adipose tissue.

Abstract:
Spontaneous potential well-logging is an important technique in petroleum exploitation. To make the corresponding log interpretation chart, it is supposed that the geometrical structure of the formation, the resistivity in each subdomain, and the spontaneous potential difference on each interface are all known; then in the direct problem, the spontaneous potential u = u(r, z) satisfies an elliptic boundary value problem with jump conditions on interfaces. At the joint points A and B of the interfaces (figure 2), the jumps of the spontaneous potential do not, in general, satisfy the compatibility condition. It turns out that it is impossible to find a piecewise H1 solution to the problem, and the standard finite element method cannot be applied to get an approximate solution. In this paper, by means of a method of removing the singularities at A and B, it is proved that the problem admits a unique weak solution that is piecewise W1,p for any fixed p with 1 ≤ p <2. Moreover, based on this method a numerical scheme is suggested, and some numerical examples and some conclusions of practical interest are given. The techniques used in this paper will find a wider applicability in other problems.

Abstract:
627 Background: Thirty percent of patients with localized clear cell renal cell carcinoma (ccRCC) will ultimately develop recurrence (local or metastatic) after nephrectomy. Current risk stratification systems still misclassify patients. We have developed a novel classification integrating cytogenetic findings to better stratify the risk of recurrence and overall survival (OS) after surgery for localized ccRCC. Methods: A total of 646 patients from UCLA with ccRCC and tumor cytogenetic analysis, were included in this study. After a selection of histologic parameters using logistic regression and cytogenetic parameters using principal component analysis a CHAID decision tree and Kaplan Meier analysis were used to build the UCLA Histo-Genetic Risk Classification (U-HGRC). Survival analyses of the model were validated on two random samples of 323 patients. Recurrence was defined as any local recurrence or development of new metastasis after surgery. Results: The T-stage, tumor size, presence of sarcomatoid features, gain of chromosome 5q, loss 10q, or loss X/Y were used to stratify the risk of recurrence of ccRCC into three U-HGRC groups of low (1), intermediate (2) or high-risk (3). After a mean follow-up of 55 months, risk of recurrence (HR = 2.44, p = .001 for U-HGRC 2; HR = 9.90, p < .0001 for U-HGRC 3), disease-free survival (DFS) (Log-rank p < .0001), risk of death (HR = 1.72, p = .033 for U-HGRC 2; HR = 4.74, p < .0001 for U-HGRC 3) and OS (Log-rank p < .0001) were significantly different between groups. These findings were validated on two random samples. For high-risk group, median DFS and OS were 2.7 and 6.3 years, respectively. The 5-year risks of recurrence for U-HGRC group 1, 2 and 3 were 9%, 25% and 62%, respectively. The AUC of the model was significantly improved comparing to the current UISS system (0.72 for U-HGRC vs 0.65 for UISS, p = .008) with an accuracy of 82.8% for the U-HGRC high-risk group. Conclusions: The U-HGRC, which integrates genomic alterations with clinical and pathologic features, allowed a better stratification of recurrence risk and overall survival that could help to select appropriate patients for surveillance and adjuvant therapy protocols.

Abstract:
✓ The efficacy of U-74006F and U-78517F in the treatment of blood-tumor barrier permeability and tumor-associated neurological dysfunction was evaluated in a brain-tumor model in rats. U-74006F is a 21-aminosteroid and U-78517F is a 2-methylamino chroman. Rats with stereotactically implanted Walker 256 tumors were treated with methylprednisolone, U-74006F, U-78517F, or vehicle (0.05 N HCl) on Days 6 through 10 following implantation. Neurological function and vascular permeability were assessed on Day 10. Methylprednisolone and U-74006F were equally effective at preventing neurological dysfunction compared to the control group (p < 0.01); U-78517F was slightly less effective than U-74006F and methylprednisolone but was significantly better than vehicle in preventing neurological dysfunction. Delivery of methylprednisolone resulted in a significant decrease in tumor vascular permeability (p < 0.006) while U-74006F and U-78517F had no effect on permeability. This suggests that U-74006F and U-78517F prevented tumor-associated neurological dysfunction by a mechanism other than decreasing permeability in tumor capillaries, and that U-74006F or U-78517F could prove useful in the treatment of brain tumors.

Abstract:
A two-dimensional model has been developed to predict the temperature field and melt pool shape during blown powder laser cladding. A finite volume method has been used to solve the two-dimensional energy equation for a body moving relative to a stationary laser heat source. A boundary fitted coordinate system has been employed as the built-up surface geometry is complex. Important dimensionless groups representing laser parameters power ( P) and scanning speed ( U) and the process parameters built-up height and average dilution are identified. The effect of laser parameters on process parameters is investigated. For cladding P/U should always be kept low (less than 1) to reduce the average dilution, and in order to achieve the desired clad height the power level P should be controlled.
