Abstract:
[Truncated abstract] Approximately 35% of women worldwide have stress incontinence, which is defined as involuntary leakage of urine on effort, exertion, or on sneezing and coughing. There are various surgical techniques for stress incontinence; however, minimally invasive operations are increasingly being chosen by surgeons and their patients. Of these procedures, tension-free vaginal tape (TVT) has a cure rate of approximately 90% and is now perceived as the standard technique for stress incontinence. Reported complications of TVT include arterial laceration, bladder perforation, bowel perforation, de novo urgency, dyspareunia, excessive blood loss, haematoma, nerve injuries, urethral erosion, urge incontinence, urinary tract infection, vascular injury, vaginal mesh erosion, voiding dysfunction and death. Suprapubic urethral support sling (SPARC) is a very similar minimally invasive operation and early indications suggested that the success rate for treating stress incontinence was expected to be identical or better than those obtained with the earlier TVT approach, with possibly fewer adverse perioperative events. Our trial sought to establish equivalence between TVT and SPARC in relation to short-term complications and efficacy. OBJECTIVES The primary outcome was bladder perforation. Secondary outcomes were blood loss, voiding difficulty, urgency, and cure of stress incontinence symptoms. METHOD A randomised controlled one-sided equivalence trial (RCT) was conducted in Perth, Western Australia during 2003 and 2004 by researchers in the School of Population Health, University of Western Australia (UWA) and King Edward Memorial Hospital (KEMH). Patients were recruited from the public Urology/Urogynaecology Clinic at the primary women's hospital and the consultant surgeons' private practices. ... However, acute urinary retention requiring a return to theatre to loosen the tape (TVT 0%, SPARC 6.5%; OR: [infinity], 95% CL: 2.2, [infinity]; p=0.002) and subjective short-term cure (TVT 87.1%, SPARC 76.5%; OR: 2.07, 95% CL: 1.13, 3.81; p=0.03) were statistically significantly different. CONCLUSIONS The results are consistent with clinical equivalence between TVT and SPARC in relation to the incidence of bladder perforation. No statistically significant difference was found between TVT and SPARC in blood loss, urgency or short-term objective cure of stress incontinence at the six week post-discharge visit to the surgeon. However, the tapes were more difficult to adjust correctly in SPARC procedures and a statistically significant number of patients required a return to theatre for loosening of the tape (TVT 0/147, 0% and SPARC 10/154, 6.5%, p=0.002). Compared with SPARC, TVT was statistically significantly higher for subjective short-term cure. In ii relation to vaginal mesh erosion, TVT was lower than SPARC, though not statistically significantly. Overall, voiding difficulty (loosening of the tape), urgency and vaginal mesh erosion were the most important clinical problems. This randomised controlled trial demonstrates the importance of testing new devices which appear to be similar, but which may have clinically relevant differences. A follow up study to assess the long-term efficacy of tension-free vaginal tape and suprapubic urethral support sling and associated complications is planned.

Inventory management is one of critical aspects of successful management. Inventory was typically classified into raw materials, component parts, spare parts, work-in-process, sub-assemblies, and finished goods. Among these inventory types, characteristics as well as demand patterns of spare part items are absolutely the most variant and complicated of all. In equipment spare parts, the most important characteristic was the “high difference in variations of demand intervals”, which leading to complicated inventory management. This study focused on the spare parts inventory management of large equipments. In addition to the original characteristics of spare parts, the most important characteristic was the variety of items. Thus, In recent years, many scholars invested a lot of time and effort in spare parts inventory management and developed many strategies of spare parts’ inventory management. However, most of the methods were complex, and couldn’t be successfully implemented in the industry and still remained on the theoretical stage. In this study, we established an inventory classification model based on the characteristic of spare parts. Four classification schemes were applied and spare parts were classified into eight categories. An inventory management policy for each item category is also suggested. The proposed model does not require complex for obtaining policy parameter values. Finally, using the spare parts of TFT-LCD industrial process equipments as an example, the performance of the proposed model is evaluated. The result showed that it not only reduced inventory levels but also labor cost.

Spare parts inventories are not the final products to be sold customers and the policy which govern spare parts inventories are different from WIP and some other inventories. Spare parts of TFT-LCD equipment have characteristics of multi-items, high difference of usage and high difference of prices; hence managers usually use different inventory policy according to the usage and price of spare parts to govern spare parts inventories in practice. For those fast-moving items, most literatures assume that the demand is Normal distribution. It will overlook the relation between the demand of spare parts and malfunction of equipment. In this study, we develop a simple inventory model for the fast-moving spare parts under the continuous (s,Q) inventory policy, lead-time constant and demand follows compound Poisson distribution . Then, we find the optimal reorder point and the reorder quantities that make total cost minimal. The study result shows: (1). Average demand quantities affect reorder point, reorder quantity and total cost directly. (2). Lead time reduction does not mean the total cost reduction, but can help to reduce inventory level and reorder quantities. (3). The shortage cost increasing will lead the reorder point up and reorder quantities down respectively. Therefore, when the ordering cost is low and the shortage cost is high, the optimal purchasing decision will close to (S-1,S), that means lot-for-lot policy.

Static electricity is a natural phenomenon. Nearly all materials produce the static electricity; hence it could exist in household, factory and environment. For example, people combing the hair and mechanical operations create the statically electric spark. Therefore, almost everyone has been experienced the static electricity. Generally, the locations of the organic solvent operation and product/storage of the flammable materials in the factory easily caused the fire and exploration amongst the static electricity-caused hazardous places. Thus, the control of the statically electric spark is a principal topic among others. This thesis adopted the gel preparation room of a TFT-LCD-produced factory as a research example to investigate the possible controls in the area of organic solvent operation of this room. This research also used a novel risk assay, Hazard and Operability Analysis, to dissect the procedures during the gel preparation. Accompany with a higher risk assay to improve the drawbacks and to decrease its risk level and the incidence of the hazard.

This study employed the material handling facilities in a TFT-LCD factory as a case study. We screened the set of critical parts for the material handling system from the historical data from the procurement and maintenance logs. Then, we conduct fitting analysis on the number of parts used for maintenance, the time between failures of the part to get the probability distribution of failure events for the critical parts. We formulated the corresponding simulation model for the material handling system based on the results of fitting analysis on the part consumption, the time between failures. Then, we used an enumeration approach to test different combinations of inventory levels of spare parts using the simulations, recorded the number of events in which the breakdown of the system lasts more than 8 hours (in short, BMT8). We pick the best combination of inventory levels of spare parts that meets the required conditions BMT8 is no more than 5 times with the minimum inventory holding cost. The framework and results of the proposed system simulation could serve an important support and reference on the decision of inventory levels of spare parts for the case company.

Abstract:
Logistics, getting the right product in the right place at the right time to the right customer, is one of the most important functions in large companies. Toyota is one of the world's leading companies in many aspects of successful business practice, particularly in logistics. The techniques developed in the company since the 1950s provide a competitive advantage to Toyota and provide efficiency in many business functions with supply chain management. For that reason, it is imperative to understand how lean logistics practices are applied in the Toyota Boshoku Turkey(TBT). This chapter examines practical logistics applications in TBT, one of the suppliers of Toyota located outside of Japan. In addition to theoretical research, it is also important that practical applications in enterprises such as a Toyota plant contribute to the literature. Consequently, as a case study, discussion and explanation of logistics and supply chains of TBT will spark reader interest.

Abstract:
On the day that the June 2006 issue of Wired magazine was released, the publication’s technology director searched the web for the word crowdsourcing, the subject of an article by contributing writer Jeff Howe. He took a screenshot of what he found, a total of three brief mentions, and forwarded it to the author, advising that Howe save it as a “historical document.” Howe didn’t have to wait long to see history in action. Within nine days Google was returning 182,000 hits. Nor was it a fleeting fad. Three years later the number had multiplied to 1,620,000, with regular appearances in the mainstream media, from the Washington Post to Fox News, where crowdsourcing was averaging two hundred new mentions each month. There’s a simple explanation for the neologism’s success. Howe had detected a trend and given it a word. The backstory, which Howe posted on his personal blog, crowdsourcing.com, supports this notion: In January Wired asked me to give a sort of “reporter’s notebook” style presentation to some executives. I had recently been looking into common threads behind the ways advertising agencies, TV networks and newspapers were leveraging user-generated content, and picked that for my topic. Later that day I called my editor at Wired, Mark Robinson, and told him I thought there was a broader story that other journalists were missing, ie, that users weren’t just making dumbpet-trick movies, but were poised to contribute in significant and measurable ways in a disparate array of industries. In what Howe characterizes as “a fit of back-and-forth wordplay,” he and Robinson came up with a term that riffed off the title of a business book popular at the time, James Surowiecki’s The Wisdom of Crowds, while also suggesting opensource software and corporate outsourcing. When “The Rise of Crowdsourcing” was finally published six months later, the last of these three roots was explicitly evoked in the teaser: “Remember outsourcing? Sending jobs to India and China is so 2003. The new pool of cheap labor: everyday people using their spare cycles to create content, solve problems, even do corporate R & D.”

Abstract:
The aim of this study is the joint optimization of the transportation and spare parts inventory policies in a reverse logistics (RL) network designed for End of Life (EOL) television (TV) recyling. Besides recycling, Printed Circuit Boards (PCBs) recovered from EOL TVs are used to meet the spare PCB demand. In order to model this RL network with its disassembly, transportation and spare parts inventory related aspects, a discrete event simulation (DES) model has been developed in detail using Arena simulation software. Next, Arena OptQuest has been used to propose optimum number and size of trucks together with the optimum reorder (s) and order quantity (Q) levels for the spare PCBs based on the minimization of total cost which includes inventory holding, PCB recovery, new PCB acquisition, truck amortization and operating costs.

Abstract:
This paper proposes a novel master-slave control strategy for coordination of throttle, wastegate and supercharger actuators in an electrically twincharged engine in order to guarantee efficient boost control during transients, while at steady state a throttle-wastegate coordination provides minimum engine backpressure hence engine efficiency elevation. The benefits and challenges associated with Low Pressure Exhaust Gas Recirculation (LP-EGR) in a baseline turbocharged engine, including improved engine efficiency, mainly due to better combustion phasing, and sluggish engine response to a torque demand due to slowed down air path dynamics were studied and quantified in [1]. Hence in this paper an electrical Eaton TVS roots type supercharger at high pressure side of the turbocharger compressor (TC compressor) is added to the baseline turbocharged engine and the performance of the proposed controller in the presence of LP-EGR, which is a more demanding condition, is evaluated and compared to the turbocharged engine. One dimensional (1D) crankangle resolved engine simulations show that the proposed master-slave control strategy can effectively improve the transient response of the twincharged engine, making it comparable to naturally aspirated engines, while the consumed electrical energy during transients can be recovered from the decreased fuel consumption due to LP-EGR conditions at steady state in approximately 1 second. Finally, a simple controller is developed to bypass the TC compressor and maximize the engine feeding charge during the transients in order to avoid TC compressor choking and achieve faster response.

NGK Spark Plug Co., Ltd. has been developing various silicon nitride materials, and the technology for fabricating components for ceramic gas turbines (CGT) using theses materials. We are supplying silicon nitride material components for the project to develop 300 kW class CGT for co-generation in Japan. EC-152 was developed for components that require high strength at high temperature, such as turbine blades and turbine nozzles. In order to adapt the increasing of the turbine inlet temperature (TIT) up to 1,350 °C in accordance with the project goals, we developed two silicon nitride materials with further unproved properties: ST-1 and ST-2. ST-1 has a higher strength than EC-152 and is suitable for first stage turbine blades and power turbine blades. ST-2 has higher oxidation resistance than EC-152 and is suitable for power turbine nozzles. In this paper, we report on the properties of these materials, and present the results of evaluations of these materials when they are actually used for CGT components such as first stage turbine blades and power turbine nozzles.

Abstract:
Circular structures are frequently found in offshore industrial application, such as risers, umbilicals, spars, and TLP platforms. Theses structures are frequently subjected to vortex induced vibration. Sometimes, they are also subjected to forced vibration. In the present paper, the forced vibration of a circular cylinder is investigated by the numerical solution of the Reynolds Average Navier-Stokes (RANS) equations. An upwind and Total Variation Diminishing (TVD) conservative scheme is used to solve the governing equations written in curvilinear coordinates. The k–ε turbulence model is used to simulate the turbulent flow in the wake of the body, when necessary. The cylinder is forced to oscillate only in the transverse direction of the mean flow with low Reynolds number and low amplitude ratio. The numerical results of the lift and drag coefficients were compared with numerical data obtained from Benevenutti and Silvestrini [1] and Meneghini and Bearman [2] to validate the code for forced vibration. The numerical results indicate that the implemented code is able to reproduce the experimental data capturing quite well the lock-in boundary, and results of practical interest are obtained, such as mean drag, RMS lift and lock-in range and.

Particle impact testing is one of those evaluations. Materials used in CGT engine are exposed in high speed gas flow, and impact damage of these materials is considered to be a concern. We tested ST-1 in the particle impact test. In this test, we observed fracture modes, and estimated the critical impact velocity. This paper summarizes the development of silicon nitride components, and the result of evaluations of these silicon nitride materials.
