Abstract:
In 1980, young, healthy women in the United States suddenly began to get sick and even die. The unexpected link to these deaths was superabsorbent tampons. Thousands of women used them during their menstrual periods, signaling the potential for a large-scale outbreak. Toxic Shock: A Social History traces the emergence of this new illness of toxic shock syndrome (TSS) and its relationship to tampon technology. This multifaceted history engages microbiology, design and innovation, journalism and mass communication, product liability, and federal policy and regulation. The broad scope captures the various approaches that contributed to defining meaning about the emergent illness. Vostral argues that tampon-related TSS was a paradigm shift in the way that illness manifests. No longer was an infection necessarily the origin of disease, or a faulty product the direct cause of injury. Together, a new pathway to an illness formed, in which a supposedly inert tampon became interactive, and a bacterium once in equilibrium grew dominant and produced toxins. Toxic Shock: A Social History makes a case for understanding tampon-related TSS as the result of biocatalytic activity between technology and bacterium. Moreover, though women were the primary consumers of tampons, the bacterium became the unintended users. This unusual disease process challenged standard approaches to public health, required women to evaluate technological risk, and currently serves as a harbinger about other internal medical devices used and worn within the human body.

Noninvasive magnetic and electrical stimulation of cerebral cortex is an evolving field. The most widely used variant, transcranial electrical stimulation (TES), is routinely used for intraoperative monitoring. Transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS) are emerging as clinical and experimental tools. TMS has gained wide acceptance in extraoperative functional cortical mapping. TES and TMS rely on pulsatile stimulation with electrical current intensities sufficient to trigger action potentials within the stimulated cortical volume. tDCS, in contrast, is based on neuromodulatory effects of very-low-amplitude direct current conducted through the scalp. tDCS and TMS, particularly when applied in repetitive trains, can modulate cortical excitability for prolonged periods and thus are either in active clinical use or in advanced stages of clinical trials for common neurological and psychiatric disorders such as major depression and epilepsy. This chapter summarizes physiologic principles of transcranial stimulation and clinical applications of these techniques.

Toxic infectious exfoliative conditions include staphylococcal toxic shock syndrome (TSS), streptococcal toxic shock syndrome (STSS), and staphylococcal scalded skin syndrome (SSSS). All three are mediated by bacterial toxin production and are considerations in the differential diagnosis of a febrile, hypotensive patient with a rash. Meningococcemia is potentially fatal and extremely contagious with a short incubation period. Disseminated gonococcal infection (DGI) presents with tenosynovitis, dermatitis, and polyarthralgias without purulent arthritis or with purulent arthritis but without skin lesions. Ecthyma gangrenosum (EG) is a cutaneous manifestation of Pseudomonas aeruginosa infection. Rocky Mountain Spotted Fever (RMSF) is caused by Rickettsia rickettsii, most commonly transmitted by the American dog tick. Patients present with nonspecific symptoms, such as fever, headache, myalgias, arthralgias, nausea, vomiting, and abdominal pain. Finally, vibrio vulnificus is a gram-negative bacterium that causes serious wound infections, sepsis, and diarrhea in patients exposed to shellfish or marine water.

Two stimulation approaches developed for selectively exciting descending motor pathways are, transcranial electrical (TES) and transcranial magnetic (TMS) stimulation. This article highlights the comparison between electrical and magnetic transcranial stimulation. Magnetic stimulation is relatively painless; therefore it is the more preferred technique. The article reviews the use of TMS for monitoring the functional integrity of the descending motor systems during surgery and discusses the potential role of TMS in the preoperative period for conscious patients planning to undergo neurosurgical procedures involving the cerebral cortex. Selective monitoring of spinal cord motor function involves acquisition of TMS-induced epidural and/or myogenic responses. As patients are generally given anesthesia before spinal cord surgeries, this article discusses the effect of general anesthetic agents on the myogenic responses.

This article discusses recent developments relating to the so-called topological superconductors (TSCs), which have a full pairing gap in the bulk and gapless surface states consisting of Majorana fermions (MFs). It first provides a background on topological superconductivity as a novel quantum state of matter before turning to topological insulators (TIs) and superconducting heterostructures, with particular emphasis on the vortices of such materials and the Majorana mode within a vortex. It also considers proposals for realizing TSCs by proximity effects through TI/SC heterostructures as well as experimental efforts to fabricate artificial TSCs using nanowires, superconducting junctions, and ferromagnetic atomic chains on superconductors.

Abstract:
The total survey error (TSE) approach is a useful schema for organizing the planning and evaluation of surveys. It classifies the several possible types of errors in surveys, including in respondent selection, response accuracy, and survey administration. While the goal is to minimize these errors, the TSE approach emphasizes that this must be done within limitations imposed by several constraints: the cost of minimizing each type of error, the time requirements for the survey, and ethical standards. In addition to survey errors and constraints, there are several survey effects for which there are no error-free solutions; the size of these effects can be studied even though they cannot be eliminated. The total survey quality (TSQ) approach further emphasizes the need for survey organizations to maximize the quality of the product they deliver to their clients, within the context of TSE tradeoffs between survey errors and costs.
