Objectives: This opinion paper considers co-morbid borderline personality disorder (BPD) and eating disorders and proposes that a trans-diagnostic approach looking at presenting problems and treatment approaches may be of value. Conclusions: Trans-diagnostic elements can be identified across the two syndromes. Trans-diagnostic elements appear to include some shared aetiological factors as well as some shared syndromal components. The shared syndromal components include problems with affect, interpersonal problems, problems with self-concept and impulsivity. The pathoplastic effects of culture, attitudes to fatness and the neurobiology of starvation and/or restriction of intake are crucial to integrate into the understanding of co-morbidity of these two disorders. Trans-diagnostic approaches to treatment lead to trans-diagnostic improvements across the two disorders.

Abstract:
Q Fever is a zoonotic disease of significant animal and public health concern, caused by Coxiella burnetii (C. burnetii), an obligate intracellular bacterium. This study was done to evaluate the diagnostic sensitivity (DSe) and diagnostic specificity (DSp) of three diagnostic methods to diagnose C. burnetii infection in cattle and buffaloes in Punjab, India: an indirect ELISA method applied in serum samples and a trans-Polymerase Chain Reaction (trans-PCR) technique applied in milk samples and genital swabs, using a Bayesian latent class analysis. Conditional independence was assumed between the tests, given (i) the different biological principle of ELISA and trans-PCR and (ii) the fact that the trans-PCR was performed on different tissues. The ELISA method in the serum samples showed the highest DSe of 0.97 (95% Probability Intervals (PIs): 0.93; 0.99) compared to the trans-PCR method applied in milk samples 0.76 (0.63; 0.87) and genital swabs 0.73 (0.58; 0.85). The DSps of all tests were high, with trans-PCR in genital swabs recording the highest DSp of 0.99 (0.98; 1), while the DSp of trans-PCR in milk samples and ELISA in serum samples were 0.97 (0.95; 0.99) and 0.95 (0.93; 0.97) respectively. The study results show that none of the applied tests are perfect, therefore, a testing regimen based on the diagnostic characteristic of the tests may be considered for diagnosis of C. burnetii.

Progress in understanding the underlying neurobiology of obsessive-compulsive disorder (OCD) has stalled in part because of the considerable problem of heterogeneity within this diagnostic category, and homogeneity across other putatively discrete, diagnostic categories. As psychiatry begins to recognize the shortcomings of a purely symptom-based psychiatric nosology, new data-driven approaches have begun to be utilized with the goal of solving these problems: specifically, identifying trans-diagnostic aspects of clinical phenomenology based on their association with neurobiological processes. In this review, we describe key methodological approaches to understanding OCD from this perspective and highlight the candidate traits that have already been identified as a result of these early endeavours. We discuss how important inferences can be made from pre-existing case-control studies as well as showcasing newer methods that rely on large general population datasets to refine and validate psychiatric phenotypes. As exemplars, we take ‘compulsivity’ and ‘anxiety’, putatively trans-diagnostic symptom dimensions that are linked to well-defined neurobiological mechanisms, goal-directed learning and error-related negativity, respectively. We argue that the identification of biologically valid, more homogeneous, dimensions such as these provides renewed optimism for identifying reliable genetic contributions to OCD and other disorders, improving animal models and critically, provides a path towards a future of more targeted psychiatric treatments.

Objective. To probe into the early diagnosis value of transfer coefficient ( K trans ) and rate constant ( K ep ) of the dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) hemodynamic parameters in patients with hepatocellular carcinoma (HCC). Methods. Fifty patients with HCC diagnosed in our hospital from May 2017 to May 2020 were studied retrospectively as the patient group (PG), and 45 cases with benign liver nodules diagnosed in our hospital during the same period were collected as the control group (CG). K trans , K ep , and AFP levels were compared between PG and CG. The diagnostic value of K trans and K ep in HCC were observed, and their correlations with patient clinical data were analyzed. The diagnostic value of K trans and K ep combined with AFP in clinical staging, differentiation degree, and distant metastasis was compared. Results. K trans , K ep , and AFP were notably higher in cases than in controls ( P < 0.05 ), indicating their high diagnostic value in HCC. K trans and K ep present elevated expression in HCC patients with clinical stage III-IV, low differentiation, and distant metastasis ( P < 0.05 ). Furthermore, it is found that the combined detection with either K trans or K ep can improve the clinical diagnostic value of AFP in the clinical stage and differentiation of HCC. However, the combined diagnosis showed little effect in raising the sensitivity of AFP in distant metastasis. Conclusion. DCE-MRI hemodynamic parameters K trans and K ep have high clinical value in early diagnosis and differentiation of HCC patients.

Abstract:
Objectives The heterogeneity and comorbidity of major mental disorders presenting in adolescents and young adults has fostered calls for trans-diagnostic research. This study examines early expressions of psychopathology and risk and trans-diagnostic caseness in a community cohort of twins and non-twin siblings. Methods Using data from the Brisbane Longitudinal Twin Study, we estimated median number of self-rated psychiatric symptoms, prevalence of subthreshold syndromes, family history of mood and/or psychotic disorders, and likelihood of subsequent trans-diagnostic caseness (individuals meeting diagnostic criteria for mood and/or psychotic syndromes). Next, we used cross-validated Chi-Square Automatic Interaction Detector (CHAID) analyses to identify the nature and relative importance of individual self-rated symptoms that predicted trans-diagnostic caseness. We examined the positive and negative predictive values (PPV; NPV) and accuracy of all classifications (Area under the Curve and 95% confidence intervals: AUC; 95% CI). Results Of 1815 participants (Female 1050, 58%; mean age 26.40), more than one in four met caseness criteria for a mood and/or psychotic disorder. Examination of individual factors indicated that the AUC was highest for subthreshold syndromes, followed by family history then self-rated psychiatric symptoms, and that NPV always exceeded PPV for caseness. In contrast, the CHAID analysis (adjusted for age, sex, twin status) generated a classification tree comprising six trans-diagnostic symptoms. Whilst the contribution of two symptoms (need for sleep; physical activity) to the model was more difficult to interpret, CHAID analysis indicated that four self-rated symptoms (sadness; feeling overwhelmed; impaired concentration; paranoia) offered the best discrimination between cases and non-cases. These four symptoms showed different associations with family history status. Conclusions The findings need replication in independent cohorts. However, the use of CHAID might provide a means of identifying specific subsets of trans-diagnostic symptoms representing clinical phenotypes that predict transition to caseness in individuals at risk of onset of major mental disorders.

Purpose: The systematic review aimed to review and evaluate constructs and measures of emotion regulation (ER) in the psychosis spectrum population literature. The empirical study aimed to examine the feasibility and acceptability of a trans-diagnostic emotion regulation (ER) group developed for an acute inpatient setting. Methods: The literature was systematically searched for research related to the measurement of emotion regulation (ER) in a psychosis spectrum population. A mixed method design was employed to assess acceptability and feasibility of a six session ER skills group delivered in an acute mental health inpatient setting. The group intervention was developed and piloted over a 5 month period. The mixed method design included a multiple single case series design and qualitative exit interviews, conducted with eight participants. Results: 24 papers met criteria for inclusion in the systematic review. 15 different self-report tools were identified as measures of ER strategies in this review. Descriptive data from the empirical study indicated high attendance and low attrition rates. Group level analysis identified large effect sizes for change in ER skills. Case series data indicated that sustained change, on at least one measured variable, occurred for four participants. Qualitative themes triangulate findings related to acceptability of the group, change in ER strategies and increased emotional acceptance. Conclusions: ER conceptualisation is variable in the literature reviewed, where the understanding of how ER and psychosis are linked is limited. The emphasis on the literature reviewed is on cognitive strategies of ER. The pilot study indicates that the intervention is feasible and acceptable, with preliminary evidence identifying potential clinical benefits. The challenges in evaluating interventions in an acute inpatient environment are discussed.

The New Guinea region is linguistically the most complex on earth: as many languages as in the Americas are spoken there. The typological diversity of Papuan languages is also great, though underestimated because of a tendency to survey data from languages of the Trans New Guinea family, the largest and most widespread. Its languages have provided a misleading picture of a ‘typical’ Papuan language, including the typological category of polysynthesis. Due to the generally low to moderately agglutinating structure of Trans New Guinea languages, the degree and range of polysynthesis in New Guinea has been under-recognized. By taking four parameters, head marking, verbal pronominal agreement affixes (polypersonalism), incorporation, and clause linkage by parataxis as diagnostic of polysynthesis, this chapter explores its range and degree across several Papuan language families. It argues that polysynthesis is a cluster of features a language can have to a greater or lesser degree.

Recanalization of chronic venous occlusions is often difficult. Sharp recanalization of occluded central veins was first described in 1996, with subsequent further variations reported in the literature. The needle recanalization of chronic venous total occlusions (NRCVTO) technique may be employed where standard initial techniques have failed. Initial efforts to cross the occlusion should always begin with an angled or straight 0.035-in. glidewire together with a 4 Fr diagnostic catheter. The NRCVTO technique employs the use of a trans-septal needle together with a 0.014-in. guidewire. A target is then provided in the vessel distal to the occlusion in the form of a 4 Fr 10-mm snare, which is usually placed via a transfemoral or, less commonly, transhepatic approach.

This chapter gives a broad overview of the “habit hypothesis” of obsessive-compulsive disorder (OCD). Most patients with OCD recognize that becoming trapped in seemingly never-ending streams of repetitive ritualistic behaviors defies reason. Importantly, this recognition is not enough to put a halt to these behaviors. It has been proposed that these compulsions are “bad habits”: that external cues trigger an urge to perform a familiar response, which the patient cannot resist. The chapter presents the basics of what habits are, and how they relate to what we call “goal-directed control” over action. Next, an in-depth analysis of a series of empirical investigations that tested this hypothesis will be presented. In the final section, the habit hypothesis of OCD will be put into the broader context of “compulsivity” as a putative trans-diagnostic trait that is relevant for many psychiatric disorders.

AbstractLatinos have higher incidence rates of cervical, gall bladder, liver, and gastric cancer, and higher mortality rates for six cancer sites than US Whites. This review chapter focuses on Latino cancer disparities, how the exposome can be applied to understanding Latino cancer disparities, and how environmental exposures lead to alterations in key biological pathways at the cellular, molecular, and system level, helping to explain the increased risk for population level cancer disparities among Latinos. An exposome-wide association study (ExWAS) approach is proposed as a novel conceptual framework to assess the role of multiple chemical and non-chemical exposures in the cause and progression of cancer among Latinos across the life course. Also discussed is how this strategy could be exploited by using biomarkers of susceptibility, exposure, and effect; and how a trans-omics approach, using recent advances in genomics, epigenomics, transcriptomics, metabolomics, proteomics, and lipidomics, could be used to deploy new biomarkers that serve both prognostic and diagnostic purposes. Also outlined are the knowledge gaps and scope for future studies in this area with implications for public health and policy interventions.

Depending on the definition of the disease and diagnostic criteria mitral valve prolapse (MVP) is one of the commonest structural abnormalities of the heart. The condition is characterized by myxoid degeneration of the mitral valve and appears to be more common in females. Trans-thoracic echocardiography is the primary diagnostic tool for diagnosing MVP and provides information about the structure and function of the valve, but also allows comprehensive evaluation of the subvalvular complex. Additional trans-oesophageal echocardiography and 3D echocardiography provide excellent further assessment of the mitral valve complex. The vast majority of patients have a relatively benign natural history. However, a small proportion may develop severe mitral regurgitation due to degenerative disease or chordal rupture, infective endocarditis, embolic cerebrovascular accident, supraventricular and ventricular arrhythmias, and sudden cardiac death. Athletic training has the potential for expediting the degenerative process and a propensity for arrhythmias or even sudden death.

Abstract:
Chapter 19 discusses invasive cardiology procedures performed in the cardiac catheterization laboratory (CCL), the electrophysiology laboratory (EPL), and the transesophageal echocardiography laboratory, and which have become the purview of anesthesiologists. Common CCL procedures include the following: diagnostic cardiac catheterizations, percutaneous coronary interventions, peripheral vascular diagnostic and therapeutic procedures, implantation of percutaneous left ventricular assist devices, placement of septal occlusion devices, and percutaneous valve repair or replacement procedures. Common EPL procedures consist of EP studies, atrial and ventricular radiofrequency (RF) ablation procedures, implantation and removal of pacing and cardioverter defibrillator devices, and electrical cardioversion. TEE procedures include trans-esophageal echocardiography (TEE) and combined TEE/direct current cardioversions (DCCV). All of these procedures may require the involvement of anesthesiologists if the patient has significant comorbidities or if the procedure requires that the patient be absolutely still and/or asleep.

Abstract:
Several parameters have been proposed to assess the physiologic significance of coronary lesions invasively. The purpose of the present study is to determine the effect of heart rate and percentage area stenosis on Fractional Flow Reserve (FFR), which is the ratio of mean pressure distal to stenosis over the mean proximal (aortic) pressure and Pressure Drop Coefficient (CDP), ratio of trans-stenotic pressure gradient to distal dynamic pressure. In-vivo experiments were performed on three Yorkshire pigs, to achieve the objective. It was found that increase in heart rate does have a significant effect on the coronary diagnostic parameters with increase in area stenosis.

Abstract:
Optimization of cut-off values for diagnostic parameters in determining stenosis severity depends on coupling functional (hemodynamic) and anatomical data. In this study, we sought to investigate the functional diagnostic parameter (CDP; the ratio of trans-stenotic pressure drop to distal dynamic pressure) by correlating with FFR (ratio of distal to proximal pressure of a stenotic section) and CFR (ratio of hyperemic blood flow velocity to that of resting condition). A 0.014-inch dual sensor (pressure and Doppler velocity) guidewire was used in 9 patients to obtain pressure drop and velocity at hyperemia. Functional index CDP was assessed from measured hyperemic pressure drop and velocity. CDP correlated linearly and significantly with FFR and CFR. Indicating that, measurements of CDP can provide a better assessment of stenosis severity.

Invasive guide-wire measurements are used to assess coronary lesion severity under clinical settings. The objective of the present research is to determine the influence of heart rate (HR) and contractility (CY) on fractional flow reserve (FFR; the ratio of distal pressure to proximal pressure at a stenotic section) and pressure drop coefficient (CDPe; the ratio of trans-stenotic pressure drop to distal dynamic pressure). In-vivo experiments were performed on eight Yorkshire pigs, to evaluate the diagnostic parameters for the conditions “CY<1100 mmHg/sec” and “CY>1100 mmHg/sec,” and for the conditions “HR<110 bpm” and “HR>110 bpm”. It was found that in the presence of normal microvasculature the measured coronary diagnostic parameters (FFR and CDPe) have a significant mean difference for variation in contractility (0.59±0.04 to 0.89±0.045 for FFR and 121.63±18 to 23.53±18 for CDPe). The variation in HR has no significant effect on FFR and CDPe (0.72±0.048 to 0.74±0.048 and 54±20 to 53±20 respectively).
