BackgroundUndergraduate medical education and postgraduate foundation training are largely secondary care based. General practice trainees also spend nearly half of their training in hospital rotations. Little is known about factors that support effective transition into general practice specialty training or belongingness experiences throughout training.AimTo explore the reported experiences of general practice trainee transition into general practice, training in hospital settings, and views of the future.MethodSemi-structured interviews with 18 purposively selected trainees plus observation and stakeholder discussions by a visiting Sri Lankan general practice trainee attending the vocational training scheme in a participant observer role.ResultsMastering core skills of general practice, undergraduate and early experience in general practice during specialty training, and general practice trainer guidance and role modelling facilitated transition. A reduced sense of belongingness during hospital rotations impacted on training and work. Building bridging social connections, personal agency initiatives to bring general practice relevance into hospital training, and secondary care affiliative behaviours were adaptive strategies. Allocation to more general practice relevant duties was thought to create value within the hospital team. General practice trainees are thriving with the support of general practice trainers and colleagues, vocational training scheme, and good work/life balance. International graduates require additional support in specific areas.ConclusionAdequate support towards transition into general practice and fostering belongingness in hospital settings is important due to the wider impact on training, patient care, and primary/secondary care integration. We propose a theoretical explanation based on Wenger’s social learning model, which may have useful practical implications.

Aims and MethodAll psychiatry trainees and supervisors on the Southeast Scotland scheme were invited to complete a questionnaire about the regularity, responsibility, structure, content and value of supervision.ResultsSignificantly more supervisors (87%) than trainees (69%) reported regular supervision. Some trainees still find it difficult to obtain regular supervision. Although it is seen as a joint responsibility, there is uncertainty about the role and responsibility of each trainee and supervisor. Most trainees and supervisors feel that supervision is useful, but supervisors are likely to rate their quality of supervision better than their trainees. Guidelines for the structure, content and boundaries of supervision might be useful. Supervision is viewed as useful for discussing clinical management, including the trainee's own case-load.Clinical ImplicationsTraining in the use of supervision should be available to all trainees and supervisors. Regular supervision should be a priority, and it is a joint responsibility to ensure that it happens. There should be greater accountability to the College and Trusts. Discussion of the trainee's clinical case-load during supervision is a necessary part of training and the supervision process.

The semi-structured training programme is outlined and benefits and challenges to both trainer and trainee are discussed. The patients generally welcomed the scheme. We consider that the programme could provide a model for similar training initiatives in the future.

ObjectiveTo determine quantitatively if a positive association exists between the mentoring of junior doctors and better training outcomes in postgraduate medical training within the UK.DesignObservational study.Participants117 trainees from the East of England Deanery (non-mentored group) and the recently established Royal College of Physicians (RCP) Mentoring scheme (mentored group) who were core medical trainees (CMTs) between 2015 and 2017 completed an online survey. Trainees who received mentoring at the start of higher specialty training, incomplete responses and trainees who were a part of both the East of England deanery and RCP Mentoring scheme were excluded leaving 85 trainees in the non-mentored arm and 25 trainees in the mentored arm. Responses from a total of 110 trainees were analysed.Main outcome measuresPass rates of the various components of the Membership of the Royal College of Physicians (MRCP) (UK) examination (MRCP Part 1, MRCP Part 2 Written and MRCP Part 2 PACES), pass rates at the Annual Review of Competency Progression (ARCP), trainee involvement in significant events, clinical incidents or complaints and trainee feedback on career progression and confidence.ResultsMentored trainees reported higher pass rates of the MRCP Part 1 exam versus non-mentored trainees (84.0% vs 42.4%, p<0.01). Mentored international medical graduates (IMGs) reported higher pass rates than non-mentored IMGs in the MRCP Part 2 Written exam (71.4% vs 24.0%, p<0.05). ARCP pass rates in mentored trainees were observed to be higher than non-mentored trainees (95.8% vs 69.9%, p<0.05). Rates of involvement in significant events, clinical incidents and complaints in both groups did not show any statistical difference. Mentored trainees reported higher confidence and career progression.ConclusionsA positive association is observed between the mentoring of CMTs and better training outcomes. Further studies are needed to investigate the causative effects of mentoring in postgraduate medical training within the UK.

Aims and MethodFollowing the introduction of a new research mentoring process, a survey was conducted of Wessex specialist registrars' views on research training.ResultsOf 34 respondents, 26 (76.5%) had agreed a plan with a research mentor and 31 (91.2%) were making good use of their research day for specified purposes. Of 24 trainees undertaking research, 21 (87.5%) felt adequately supported, 20 (83.3%) were interested in their research, 16 (66.7%) enjoyed it and 21 (87.5%) gained research knowledge/skills.Clinical ImplicationsAgreeing a research training plan with a mentor, coupled with regularly reviewed support and supervision through a newly developed mentoring process, was associated with high levels of trainee satisfaction. Compared with a previous survey, these results indicate considerable improvement in research training.

Poussés par les démarches internationales qui visent à développer les systèmes de transports intelligents, les opérateurs de transport ferroviaires s'appuient de plus en plus sur les nouvelles technologies de la communication pour optimiser les coûts d'exploitation mais aussi pour moderniser leur image et rendre les transports collectifs plus attractifs. Dans ce contexte, nous avons proposé une architecture de transmission originale combinant un lien terrestre et un lien satellite. Nous avons caractérisé les contraintes engendrées par la prise en compte de la vitesse dans l'architecture globale du système. Une étude théorique des canaux de propagation ferroviaires et des différents modèles existants tenant compte de la vitesse, a été effectuée et nous a permis de choisir un modèle de canal adapté à l'application. Ensuite, une étude des performances de la modulation OFDM dans un environnent ferroviaire a été réalisée par simulations numériques. Cette technique a été retenue pour ses propriétés de robustesse vis-à-vis de la sélectivité en fréquence du canal de propagation. Afin d'améliorer les performances de cette modulation, une comparaison des performances de plusieurs techniques de codage correcteur d'erreurs, d'estimation et d'égalisation du canal est proposée. Trois familles de codes ont été retenues: les codes convolutifs, les codes de Reed Solomon et les Turbo codes. Une chaîne de simulation complète met en œuvre ces différentes techniques. Dans la dernière partie du document, certains de ces algorithmes sont intégrés dans une chaîne de transmission réelle. Les difficultés et les premiers résultats des essais sont décrits
With the rapid growth of digital communication in recent years, the need for high data rate transmission is growing. This increase has opened a new dimension to future wireless communications whose ultimate goal is to provide universal personal and multimedia communication without regard to mobility or location. Pushed by these internationals trends, railways operators rely more and more on the new information and communication technologies to optimize exploitation and maintenance costs but also to enhance the friendliness, comfort and security feeling of public transport by offering new services to passengers while travelling. In this work we investigated a transmission architecture using a terrestrial link in conjunction with a satellite link in order to enable high data rate transmission in the case of high speed trains. We first addressed the channel propagation characterization and impairments and we gave an overview of some existing channel simulators that take into account the mobile speed effect. After an introductive overview of the OFDM modulation principles that allow to reach a higher spectral efficiency, we dealt with channel estimation and synchronization issues for this kind of systems, especially with the problem of inter-carriers interferences due to frequency offsets induced by the mobile speed. We also dealt with channel coding such as Convolutionnal, Reed Solomon and Turbo Codes and we outlined an improvement of OFDM system performances using channel coding in the case of high mobile speed. Finally, the algorithms chosen in the theoretical study in a time varying channel are implemented and tested in a real transmission chain

Background: The global human resources for health crisis has affected Uganda deeply as is evidenced by grossly inadequate medical doctor to population ratios. Strategies to increase training and retention initiatives have been identified as the most promising ways to address the problem. In Uganda, the dual track tuition policy of higher education (called the Private Students Scheme or PSS) at the University of Makerere was initiated in the academic year 1993/94, to boost student intake and to supplement university revenue. However, the impact of this scheme on the enrolment and graduation of medical students at this University is unknown. Aim: This study aimed to assess the effect of the PSS on enrolment, time to completion, attrition and number of graduated medical students at Makerere University Medical School after (post-) the Private Students Scheme (PSS). Study design: A quantitative cross-sectional descriptive study based on a retrospective review of enrolment and graduation records of medical students was conducted comparing records of students enrolled five years before and after the privatisation scheme. Numbers enrolled, attrition rates, time to completion and graduation numbers were analysed. Results: There were 895 students enrolled in the study period, 612 (72.2%) males and 236 (27.6%) females. Pre- and post-PSS periods had 401 and 494 enrolments respectively (a net increase of 93 students). During the post-PSS period, 447 (90.5%) government sponsored students were enrolled - 351 (71.1%) males and 143 (28.9% females); in the same period, 47 (9.5%) private students were enrolled, 30 (63.8%) male and 17 (36.2%) female. Graduation rates for the entire study period were 96% (859), which represented 44% (378) in the pre-PSS and 56% (481) in the post-PSS periods. Private students contributed 8.9% (43) of the graduates 9in the post-PSS period. The majority of students (90.4%) graduated in five years. Thirty four students (3.8%) dropped out in the entire period, constituting significantly more in the pre-PSS - 22 (5.5%) than in the post PSS-period - 12 (2.4%). Males were more likely to drop out: 31 males did so (4.4%) compared with 3 (1.2%) females. In the post-PSS period, males made up 83.3% (10/12) of the attrition rate. Nine of them were government sponsored while three were private students. Conclusions: The PSS resulted in a 10% increase in enrolments when compared to the pre-PSS period. Furthermore the number of private medical student enrolments contributed 8.9% of the total graduations indicating that PSS succeeded in increasing the number of medical doctors graduated at MUSM. More males than females enrolled across all the years which might indicate a tendency for females to pursue non-medical professions which should be discouraged. Attrition of students was low which is encouraging but the finding that males were more likely to drop out than females deserves attention.

Cette thèse porte sur la résolution numérique du modèle multi-dimensionnel d'écoulement cisaillé en eau peu profonde. Dans le cas d'un mouvement unidimensionnel, ces équations coïncident avec les équations de la dynamique de gaz pour un choix particulier de l'équation d'état. Dans le cas multi-dimensionnel, le système est complètement différent du modèle de la dynamique de gaz. Il s'agit d'un système EDP hyperbolique 2D non-conservatif qui rappelle un modèle de turbulence barotrope. Le modèle comporte trois types d'ondes correspondant à la propagation des ondes de surface, des ondes de cisaillement et à celle de la discontinuité de contact. Nous présentons dans le cas 2D un schéma numérique basé sur une nouvelle approche de ``splitting" pour les systèmes d'équations non-conservatives. Chaque sous-système ne contient qu'une seule famille d'ondes: ondes de surface ou ondes de cisaillement, et discontinuité de contact. La précision d'une telle approche est testée sur des solutions exactes 2D décrivant l'écoulement lorsque la vitesse est linéaire par rapport aux variables spatiales, ainsi que sur des solutions décrivant des trains de rouleaux 1D. Finalement, nous modélisons un ressaut hydraulique circulaire formé dans un écoulement convergent radial d'eau. Les résultats numériques obtenus sont clairement similaires à ceux obtenus expérimentalement: oscillations du ressaut et son rotation avec formation du point singulier. L'ensemble des validations proposées dans ce manuscrit démontre les aptitudes du modèle et de la méthode numérique pour la résolution des problèmes complexes d'écoulements cisaillés en eau peu profonde multidimensionnels
This thesis is devoted to the numerical modelling of multi-dimensional shear shallow water flows. In 1D case, the corresponding equations coincide with the equations describing non--isentropic gas flows with a special equation of state. However, in the multi-D case, the system differs significantly from the gas dynamics model. This is a 2D hyperbolic non-conservative system of equations which is reminiscent of a generic Reynolds averaged model of barotropic turbulent flows. The model has three families of characteristics corresponding to the propagation of surface waves, shear waves and average flow (contact characteristics). First, we show the ability of the one-dimensional conservative shear shallow water model to predict the formation of roll-waves from unstable initial data. The stability of roll waves is also studied.Second, we present in 2D case a new numerical scheme based on a splitting approach for non-conservative systems of equations. Each split subsystem contains only one family of waves (either surface or shear waves) and contact characteristics. The accuracy of such an approach is tested on exact 2D solutions describing the flow where the velocity is linear with respect to the space variables, and on the solutions describing 1D roll waves. Finally, we model a circular hydraulic jump formed in a convergent radial flow of water. Obtained numerical results are qualitatively similar to those observed experimentally: oscillation of the hydraulic jump and its rotation with formation of a singular point. These validations demonstrate the capability of the model and numerical method to solve challenging multi--dimensional problems of shear shallow water flows

La modification du périmètre des régions dans le cadre de la réforme territoriale de 2015 conduit à mobiliser le concept de cohérence territoriale. En effet, dans la région Grand Est, les pratiques territoriales et les représentations spatiales n’ont pas évolué malgré la nécessité de faire émerger un territoire de vie commun. La région reste ainsi marquée par la mitoyenneté de trois ensembles (Alsace, Lorraine, Champagne-Ardenne) fonctionnant selon une dynamique propre à chacun. La mise en réseau de ces entités représente un moyen d’assurer un fonctionnement transversal plus cohérent et vecteur d’un usage plus important du réseau ferré régional. Cette mise en réseau peut être portée par le réseau ferré lui-même, un des principaux instruments de cohérence territoriale à travers sa double capacité à supporter les mobilités et à conditionner leur émergence. Face aux limites que présente le réseau classique pour répondre aux enjeux de déplacements à l’échelle d’une région élargie, le réseau à grande vitesse réserve quant à lui des opportunités d’optimisation à travers le déploiement de services régionaux à grande vitesse. Un outil d’aide à la décision est développé afin d’étudier les évolutions du réseau et des services ferroviaires permettant de répondre à ces enjeux. Une approche par l’accessibilité horaire des territoires a pour but la simulation de scénarios préalablement définis dans un volet prospectif
The modification of the perimeter of the French regions as part of the last territorial reform of 2015 leads to enlist the concept of territorial coherence. In the Greater East region, territorial practices and spatial representations have not changed despite the need to bring out a common living area. The Greater East region remains marked by the juxtaposition of three entities (Alsace, Lorraine, Champagne-Ardenne), each of them operating with a specific dynamic.The networking of these entities represents a means of ensuring a transversal functioning, which could be a vector for a greater use of the regional railway network. This networking can be carried out by the rail network itself: one of the main instruments of territorial coherence through its dual capacity to support mobility and to condition their emergence.Faced with the limitations of the conventional network to meet the challenges of travel on the scale of an enlarged region, the high-speed network reserves opportunities for optimization through the deployment of regional high-speed services. A decision-support tool is developed in order to highlight the improvements of the network and the rail services needed to meet these challenges. An approach by the schedule accessibility of the territories which considers the train schedules will enable the modelling of scenarios previously defined within a prospective approach

AbstractIn this paper, we propose a safe reinforcement learning approach to synthesize deep neural network (DNN) controllers for nonlinear systems subject to safety constraints. The proposed approach employs an iterative scheme where a learner and a verifier interact to synthesize safe DNN controllers. The learner trains a DNN controller via deep reinforcement learning, and the verifier certifies the learned controller through computing a maximal safe initial region and its corresponding barrier certificate, based on polynomial abstraction and bilinear matrix inequalities solving. Compared with the existing verification-in-the-loop synthesis methods, our iterative framework is a sequential synthesis scheme of controllers and barrier certificates, which can learn safe controllers with adaptive barrier certificates rather than user-defined ones. We implement the tool SRLBC and evaluate its performance over a set of benchmark examples. The experimental results demonstrate that our approach efficiently synthesizes safe DNN controllers even for a nonlinear system with dimension up to 12.

AbstractMachine learning has been proved to be feasible and reasonable in architectural field by extensive researches recently, whereas its potential is far from being tapped. Previous studies show that the training of GAN by labelling can enable a computer to grasp interrelationship of spatial elements and logical relationship between spatial elements and boundary. This study set the learning object as layout of private gardens in southern Yangtze with higher complexity. Chinese scholars usually analyse private garden layout based on their observation and experience. In this paper, based on Pix2Pix model, we enable a computer to generate private garden layout plan for given site conditions by learning classic cases of traditional Chinese private gardens. Through the experiment, taking Lingering garden as example, we continuously adjust the labelling method to improve learning effect. The finally trained model can quickly generate private garden layout and aid designers to complete scheme design with private garden element corpus. In addition, the working process of training GAN enables us to discover and verify some private garden layout rules that have not been paid attention to.

This study investigated processes in novice–expert consultation meetings in an organizational context to identify ‘what’ is done ‘how’ by novices and expert in consultation discourses. A conceptual model was developed for studying novice–expert design discourses at a fine-resolution level. An empirical study was performed at Rolls-Royce Aerospace Engineering. In total 7 audio-records were captured of meetings between trainees (novices) and expert designers, which occurred over the course of 3 trainee teams’ design projects. Relations were investigated between two coding schemes, namely the activity coding scheme and the conversational flow coding scheme. It was found that certain activities in the meeting were more often performed by either novices or experts, whereas other activities were more often performed collaboratively. Based on the results, implications for design engineering practitioners were derived and suggestions for further research are provided.

We propose a new dynamic row/column management algorithm for the schedule of freight trains in a single/double track railway system. While many works have already been devoted to train scheduling, previously published optimization models all suffer from scalability issues. Moreover, very few of them consider the number of alternate tracks in the sidings for train meets, as well as the delay incurred by trains that take sidings. We propose a non time-indexed model, which takes into account such constraints, and we design a solution scheme with iterative additions/removals of constraints/variables in order to remain with a manageable sized mathematical program, while still ensuring convergence to an optimal solution. Numerical results are presented on data instances of Canada Pacific Railway. We evaluate the performance of the optimization model, and the sensitivity of the train schedules to the length of the operation hours, the length of the trains, and the departure times.

Recent researches have suggested that the predictive accuracy of neural network may contend with its adversarial robustness. This presents challenges in designing effective regularization schemes that also provide strong adversarial robustness. Revisiting Vicinal Risk Minimization (VRM) as a unifying regularization principle, we propose Adversarial Labelling of Perturbed Samples (ALPS) as a regularization scheme that aims at improving the generalization ability and adversarial robustness of the trained model. ALPS trains neural networks with synthetic samples formed by perturbing each authentic input sample towards another one along with an adversarially assigned label. The ALPS regularization objective is formulated as a min-max problem, in which the outer problem is minimizing an upper-bound of the VRM loss, and the inner problem is L1-ball constrained adversarial labelling on perturbed sample. The analytic solution to the induced inner maximization problem is elegantly derived, which enables computational efficiency. Experiments on the SVHN, CIFAR-10, CIFAR-100 and Tiny-ImageNet datasets show that the ALPS has a state-of-the-art regularization performance while also serving as an effective adversarial training scheme.

For multi-channel speech recognition, speech enhancement techniques such as denoising or dereverberation are conventionally applied as a front-end processor. Deep learning-based front-ends using such techniques require aligned clean and noisy speech pairs which are generally obtained via data simulation. Recently, several joint optimization techniques have been proposed to train the front-end without parallel data within an end-to-end automatic speech recognition (ASR) scheme. However, the ASR objective is sub-optimal and insufficient for fully training the front-end, which still leaves room for improvement. In this paper, we propose a novel approach which incorporates flow-based density estimation for the robust front-end using non-parallel clean and noisy speech. Experimental results on the CHiME-4 dataset show that the proposed method outperforms the conventional techniques where the front-end is trained only with ASR objective.

Abstract:
The orientation of constructed levee embankments relative to alluvial swales is a useful measure for identifying regions susceptible to backward erosion piping (BEP). This research was conducted to create an automated, efficient process to classify patterns and orientations of swales within the Lower Mississippi Valley (LMV) to support levee risk assessments. Two machine learning algorithms are used to train the classification models: a convolutional neural network and a U-net. The resulting workflow can identify linear topographic features but is unable to reliably differentiate swales from other features, such as the levee structure and riverbanks. Further tuning of training data or manual identification of regions of interest could yield significantly better results. The workflow also provides an orientation to each linear feature to support subsequent analyses of position relative to levee alignments. While the individual models fall short of immediate applicability, the procedure provides a feasible, automated scheme to assist in swale classification and characterization within mature alluvial valley systems similar to LMV.
