Abstract:
Toxicology questions frequently appear in the Intermediate FRCEM short-answer question (SAQ) paper and a general knowledge of poisoning and its associated management is required. This chapter covers the basic principles of poisoning and drug elimination. It also focuses on common toxidromes to aid diagnosis and their associated management. In addition, this chapter also provides detailed information and management for the commonest poisoning agents such as paracetamol, salicylates, tricyclic antidepressants, and carbon monoxide. Paracetamol overdose is one of the commonest presentations of poisoning to the emergency department, the treatment guidelines for which have recently changed. This chapter provides the basic pathophysiology of paracetamol toxicity and details the current management guidance.

Abstract:
To determine whether or not a contaminant has the potential to become a pollutant, its biological effects must first be established. The scientific discipline of toxicology considers the measurement of toxins, their mechanisms and the effects they have on the environment, its inhabitants and on human health. This chapter describes how contaminants behave in marine environments, how organisms are exposed to them and how they respond at the molecular, individual, population and ecosystem levels. Marine organisms exhibit a range of different responses and detoxification mechanisms that are used as the basis of approaches for measuring toxicity. The complex interactions of pollutants in the environment, their interactions with other toxicants and the large natural variability in toxic effects between species and individuals make this challenging. However, in order to regulate the production and discharge of pollutants it is critical that the impacts on ecosystems and ecosystem services are understood.

Abstract:
This chapter describes the basic principles of toxicology and their application to occupational and environmental health. Topics covered include pathways that toxic substances may take from sources in the environment to molecular targets in the cells of the body where toxic effects occur. These pathways include routes of exposure, absorption into the body, distribution to organs and tissues, metabolism, storage, and excretion. The various types of toxicological endpoints are discussed, along with the concepts of dose-response relationships, threshold doses, and the basis of interindividual differences and interspecies differences in response to exposure to toxic substances. The diversity of cellular and molecular mechanisms of toxicity, including enzyme induction and inhibition, oxidative stress, mutagenesis, carcinogenesis, and teratogenesis, are discussed and the chapter concludes with examples of practical applications in clinical evaluation and in toxicity testing.
