Abstract:
Although it is a vaccine-preventable disease, tetanus is frequently found in sub-Saharan Africa. Because of its rarity, this disease poses two problems for doctors of the 21st century: to make early diagnosis in order to refer patients to appropriate care structures, and to continue to ensure correct prevention of an affection that few current doctors have met. In it generalized form, the diagnosis of tetanus is easy, but when the beginning is localized, tetanus can be change with other diseases leading to a diagnostic wandering and a delay of adequate management. We report a case of tetanus in an 18-year-old male, who was received for neck pain and stiffness of the neck with a positive Kernig sign associated to fever since 48 hours. Initially treated for meningitis, the patient will develop at day 3 of hospitalization, the signs of generalized tetanus which led to evoked the diagnosis of tetanus. This diagnosis was reinforced by the notion of a rusty nail injury to the soles of the right foot 3 weeks before hospitalization and the absence of anti-tetanus vaccination. After using tetanus serum and vaccination with antibiotics (Metronidazole and Penicillin) and Diazepam in association with stripping of the wound under the foot, the patient was improved. This case illustrates that any neurological sign with the first trismus must evoke until proof of the contrary, a tetanus especially in case of association with a wound even if the immunization schedule is up to date.

Abstract:
Abstract In the nationwide debates in the build-up to the Ghana elections of December 2016, the New Patriotic Party, then in the opposition, claimed that 76,000 individuals were registered on both the Togolese and the Ghanaian voters’ registers, casting doubt on the citizenship status of voters who crossed the border from Togo to vote in Ghana. The issues that political parties continually raise about the voters’ register result in recurrent debates about identification documents and belonging. This article poses the underlying questions that many election analyses overlook: who is the electorate? Who decides who belongs to the nation? I argue that the criteria for belonging are neither those that are set in the law, nor those that seem to be suggested by political parties, but those that are decided at a local level where communities are the real gatekeepers of the vote. This article contributes to the literature on elections in Africa by highlighting the porosity of borders in a mobile world, not purely in terms of electoral outcomes, but in terms of broader issues about citizenship and belonging.

Abstract:
Introduction Identification of the recurrent laryngeal nerve during thyroid surgeries is recommended to preserve its functionl integrity. This study aims to determine the relationship between the recurrent laryngeal nerves and the inferior thyroid arteries during thyroidectomies, as well as any particularities and intraoperative complications. Materials and Methods Observational cross-sectional study of all culturally Togolese patients who underwent thyroidectomy with nerve dissection in the ENT department of Sylvanus Olympio University Hospital from 1 January 2013 to 30 June 2017, i.e. a period of 4 years 6 months. Results The sex ratio was 0.14 i.e., 7 women for 1 man and the average age of 41.82 years. Tissue nodules and multinodular goiters were the most operated lesions in 40.97% and 33.04%, respectively, with a plunging character in 17 cases. We had 306 cases of nerve dissection but the recurrent laryngeal nerve could not be found in 1 case on the right. It was globally retro-arterial (under the trunk and under the branches of the inferior thyroid artery) in 75.08% of cases with prevalence of the truncal, pre arterial situation in 16.07% and inter-arterial in 3.28% cases. The nerve was non-recurrent (type II pathway) in 1 case on the right. There was no recurrence injury. Parathyroid devascularization (5 cases) was autotransplanted. The surgeon's experience did not have a significant impact on the occurrence of intraoperative complications (p = 0.24). Conclusion This observational cross-sectional anatomical-surgical study supports the existing data on the relationships between the recurrent laryngeal nerve and the inferior thyroid artery by confirming their variability. Nevertheless, it was marked by the predominance of the retro arterial truncal situation of the nerve unlike the literature that most often reports a situation of the nerve under the arterial branches.

Abstract:
Medicinal plants are a precious heritage for humanity and especially for the majority of poor communities in developing countries who depend on them for primary health care and livelihoods. They are used in traditional practices against urinary tract infections without any scientific evidence for most of the species used. This work aims to have a summary of the literature on some medicinal plants used in the treatment of urinary tract infections. To do this, this study was carried out based on the different medicinal plants cited by herbalists in southern Togo against urinary tract infections. The names of these different plants have each been the subject of research with keywords in search engines such as Google Scholar, PubMed, FreefullPdf, and others to bring out the traditional use of these plants in the world. From all of the above, it appears that the medicinal plant species cited by actors of the Togolese pharmacopoeia are used to treat several diseases in the world. The organs of these plants, such as leaves, bark, and roots are used in the treatment of common illnesses such as typhoid fever, diarrhea, malaria, bronchitis, and hypofertility. Coughs, colds, skin diseases, toothache, gonorrhoea, viral infections, and many other diseases are treated with these plants. The plants are often combined with other plants in traditional medicine. They are used in the form of decoction, fumigation, herbal tea, and others. The type of disease treated by the plants changes from region to region and is also dependent on the culture of the region in the world. The use of medicinal plants is a very ancient practice. It is a very suitable alternative to modern medicine. However, there is a need to have more detailed studies on these plants, such as their toxicological effects.

Abstract:

Togo, this small West African nation, is still relative unknown, even in today's jet set world. The Western world is only now discovering the numerous advances Togo has made in it social and economic policies, but most of all in its political conjectures. After its Independence on April 27, 1960, Togo had barely begun its journey to democracy when the dictatorship of Gnassingbe Eyadema became the yoke of the people for over thirty-one years, on April 14th, 1967. The consequences of the stranglehold exercised by Gnassingbe was to shut the nation's cultural growth and cause the people to close in onto themselves and build a protective barrier between themselves and the rest of the world.

Yet, Togo had great beginnings. It was one of the pioneers of Sub-Saharan literature, publishing in 1929 one of the first true African novels still read today. In 1929, native son Felix Couchoro, was among the first Sub-Saharan authors to write a novel which gave agency to an African protagonist in a story set in Africa, with an African-themed plot, and with a conclusion that aimed at rethinking African society. Couchoro was the first to look deeply into his culture and the social identity of his nation. He brought forth suggestions that would help in Togo's growth and insure its successful battle for Independence.

In doing so, however, Couchoro also created great controversy around a subject which continues to plague not only Togelese people, but all Africans who feel pulled in two directions: preserving their authentic traditional customs while taking an active part in the modern world, through economic improvements as well as technological advances. In this dissertation, I will first study Couchoro's flagship novel which was the starting point of this quest for a modern identity, then analyze how subsequent Togolese writers have taken up Couchoro's legacy.
