Sammanfattning

Tinnitus är idag ett folkhälsoproblem i Sverige som drabbar allt fler. Det finns i dagsläget inget definitivt botemedel men det finns behandlingsmetoder som syftar till att på olika sätt lindra tinnitus. En av dessa metoder är Tinnitus Retraining Therapy (TRT) som grundar sig i den neurofysiologiska modellen. TRT består av två delar; rådgivning och ljudterapi. I ljudterapin tillämpas vanligtvis en ljudstimulator som tillför ett bredbandigt brus på svag nivå i patientens öra.

Syftet

Syftet med uppsatsen är att redogöra för effekten av behandlingsmetoden Tinnitus Retraining Therapy.

Metod

Metoden har omfattat granskning av artiklar som sökts via Örebro Universitets databaser samt via relevanta referenslistor i arbeten, böcker och artiklar. Urvalet har innefattat tre steg. Första steget var att välja ut artiklar publicerade i vetenskapliga tidskrifter genom att granska artiklarnas titel. Andra steget var att granska de artiklar som valdes ut i steg ett genom att läsa deras sammanfattningssida för att se om innehållet var relevant för arbetets syfte. Tredje steget var att utförligt läsa de artiklar som valts ut i steg två.

Resultat och diskussion

Resultatet och diskussionen visar att det är svårt att säga huruvida TRT är effektivt. För detta skulle en mer omfattande liknande studie vara nödvändig.

Abstract:
Tinnitus och ljudöverkänslighet är två vanliga hörselproblem. Men vad är det som ligger bakom problemen? Vad finns det för hjälp till någon som är drabbad? Jag ville lära mig mer om detta ämne dels för att jag lider av både tinnitus och ljudöverkänslighet själv sen flera år tillbaka men också för att jag är lärare. Jag kan visa och berätta för mina elever på kulturskolan vad som händer om man inte använder öronproppar när man repar. Jag ville också få fram hjälp och tips till de som lider av dessa problem. Därför skickade jag ut en enkät för att få reda på olika personers tips. Men jag fick tyvärr inget svar.. Då provade jag en självhjälpsbok och ett tips som jag tyckte var extra bra står med under resultatdelen. Jag är nöjd med mitt resultat trots att det blev väldigt annorlunda emot vad jag tänkt mig.

Abstract:
Subjective tinnitus is the perception of sound despite the absence of an external stimulus. This challenging sensory event affects millions of people per year. There currently is no cure for tinnitus, but there have been many different options researched to help patients manage its effects, albeit with varying efficacy. In a viral internet video, the Beating the Heavenly Drum maneuver, found in the Eastern practice of Qigong, was said to eliminate the perception of tinnitus for the participants. In this current study, the Beating the Heavenly Drum maneuver was compared to a sham maneuver and evaluated for effectiveness in relieving tinnitus. To be included in this study, participants had experienced tinnitus for at least 6 months and were not currently receiving other tinnitus care. Exclusionary criteria included a diagnosis of Post-Traumatic Stress Disorder, Traumatic Brain Injury, any neurological condition, whiplash, neck injury, or severe anxiety or depression, as determined by the Hospital Anxiety and Depression Scale. Participants completed a tinnitus case history form, Tinnitus Functional Index (TFI), Tinnitus Handicap Index (THI), and Visual Analog Scale (VAS) ranking the annoyance of their tinnitus. Participants were assigned to groups in an alternating fashion, with odd identifiers in Group 1 and even identifiers in Group 2. In Group 2, participants received the experimental maneuver (Beating the Heavenly Drum) during the first session and the sham maneuver (circles rubbed at the base of the skull) during the second. In Group 1, participants received a sham maneuver during the first session and the experimental maneuver during the second session. Within 48 hours after each session, participants completed the TFI, THI, and rated the annoyance of their tinnitus on a scale of 0 to 10 (keeping the same parameters from the VAS) via phone call. Of the twelve participants, two reported that the experimental maneuver, Qigong, was effective for tinnitus relief; while five reported the sham condition relieved their tinnitus. The remaining five participants stated that neither maneuver altered their tinnitus sensation. Across all participants, there were no significant difference scores on the THI, and only one significant difference score on the TFI. While participants reported some change in their tinnitus with either maneuver, none of the questionnaire measures corroborated their subjective report. Additionally, the sham maneuver was perceived as more effective than the experimental maneuver. The Qigong maneuver was not effective for tinnitus relief in this study. It should be noted that a limitation to this study is the small sample size. While this Qigong maneuver did not relieve tinnitus, there are other Internet tinnitus “cures” that should be investigated for their effectiveness in tinnitus relief. With the rise of the Internet and more patients searching for “quick fixes” to tinnitus, it is vital that clinicians provide appropriate education and research to best help patients manage their tinnitus.

Abstract:
Tinnitus affects 10-15% of the population and causes a diminished quality of life for 1-3% of people. Without a clear understanding of the mechanisms that generate and sustain tinnitus, treatment plans can only attempt to manage the problem rather than address the underlying causes of tinnitus. As 85% of tinnitus sufferers have hearing loss, tinnitus was originally considered to be a problem of the ear. Moreover the remaining tinnitus sufferers are postulated to have hidden hearing loss. Subsequent research has found that tinnitus is a problem extending beyond the ear. Experimental findings have identified tinnitus related activity throughout the auditory system. It is unclear how this activity is created and how it leads to the perception of a phantom sound. This thesis aims to identify the factors involved in the development of tinnitus related activity and the manifestation of a phantom sound; computational models of the auditory system are developed to address this primary objective. In this thesis, a biologically inspired model of the auditory periphery is created, called the peripheral model, which emulates tinnitus related activity in the auditory brainstem and accurately models hidden hearing loss in line with empirical data. The peripheral model is extended to model correlates of tinnitus associated with the thalamocortical network. Finally a perceptual model of tinnitus is developed using a Linear Mixed Effects (LME) approach to show how tinnitus related activity leads to the perception of a phantom sound. The outcomes from the development of the peripheral model include an accurate model of cochlear synaptopathy to replicate hidden hearing loss as well as the finding that hidden hearing loss can instigate adaptive changes that result in tinni­tus related activity. Extending the peripheral model to include the thalamocortical network led to the discovery that tinnitus requires changes to both the bottom-up and top-down signals in the auditory system. The result provides a significant step forward towards understanding the mechanisms underpinning tinnitus related activity. The peripheral model and the result from the investigation of the thalamocortical network provide the underpinning basis upon which to develop the perceptual model of tinnitus using a LME approach. The LME model provides a state of the art perceptual model of tinnitus that accurately models characteristics of tinnitus such as pitch and loudness. The results provide a significant advance­ment in the understanding of tinnitus generation and the evidence to motivate and direct further studies, which could lead to improved treatment methods for this condition.

Abstract:
Tinnitus is a highly prevalent health condition creating moderate or severe interference on mood, sleep and daily functioning for a group of those affected. The aims of this thesis were 1) to explore the role of acceptance and psychological flexibility in understanding tinnitus interference both experimentally and with a longitudinal design 2) to evaluate the immediate and long-term outcomes of an acceptance based behaviour therapy (Acceptance and Commitment Therapy; ACT) in the treatment of people with tinnitus and, 3) to investigate the relationship between treatment outcome and processes assumed to be the active ingredients of treatment (acceptance and cognitive defusion). Study I (n=47) was an experiment comparing the impact of acceptance to that of thought suppression or a neutral instruction on the ability to maintain attention on an imagery task. Results indicated that participants could benefit from an acceptance strategy when performing the task. Study II (n=47) was a longitudinal trial studying the mediating role of acceptance on the relationship between tinnitus interference at baseline and tinnitus interference, anxiety, life quality, and depression at a seven-month follow-up. Full mediation was found for life quality and depression, and partial mediation for tinnitus interference. Study IV (n=64) was a randomised controlled trial evaluating the immediate and long-term effects of ACT in comparison to those of Tinnitus Retraining Therapy (TRT) and to a wait list control. Results showed that ACT had large immediate effects on tinnitus interference in comparison to wait list, and medium long-term effects in comparison to TRT. Results were also seen on secondary outcome. Self-reported tinnitus acceptance significantly mediated the immediate outcome of ACT. Study III (n=24) was a process study where the video recorded sessions of ACT from study IV were observed and rated with regard to client behaviour. Results showed that in-session acceptance and defusion behaviours rated early in therapy were predictors of sustained positive treatment effects of ACT. These associations continued to be substantial even when controlling for the prior improvement in outcome. This whereas prior symptom change could not predict process variables rated late in therapy. Participants in all trials were chronic tinnitus patients, mainly from different departments of audiology. These findings implicate that 1) acceptance and psychological flexibility may contribute to the understanding of tinnitus interference 2) ACT can reduce tinnitus interference in a group of normal hearing tinnitus patients and 3) acceptance and cognitive defusion are important processes in ACT, related to outcome.

Denna avhandlings syften var att 1) utforska den roll acceptans och psykologisk flexibilitet har för förståelsen av tinnitusbesvär såväl experimentellt som med en longitudinell design 2) utvärdera de omedelbara och långsiktiga effekterna av en acceptansinriktad beteendeterapi (Acceptance and Commitment Therapy; ACT) vid behandling av personer med tinnitus samt 3) undersöka relationen mellan behandlingsutfall och processer som antas utgöra behandlingens aktiva komponenter (acceptans och kognitiv defusion). Studie I (n=47) var ett experiment där man jämförde förmågan att bibehålla uppmärksamheten på en mental bild vid tre olika betingelser: acceptans, tankesuppression eller en neutral betingelse. Resultaten indikerade att deltagarna som slumpats till acceptans var hjälpta av denna strategi i genomförandet av uppgiften. Studie II (n=47) var en självrapportstudie med longitudinell design där det undersöktes om acceptans fungerade som en mediator i sambandet mellan tinnitusbesvär i baslinjen och livskvalitet, depression, ångest och tinnitusbesvär vid en uppföljning efter sju månader. För variablerna livskvalitet och depression visade resultaten en fullständig mediation och för tinnitusbesvär en partiell mediation. Studie IV (n=64) var en randomiserad kontrollerad prövning av de omedelbara och långsiktiga effekterna av ACT i jämförelse med Tinnitus Retraining Therapy (TRT) och en väntelistekontrollgrupp. Resultaten visade att ACT hade en stor omedelbar effekt på tinnitusbesvär i jämförelse med väntelistan och en medelstor effekt i jämförelse med TRT. Även sekundära utfallsmått visade på effekter. Utfallet i ACT medierades av självrapporterad acceptans av tinnitus. Studie III (n=24) studerade processer i ACT-behandlingen genom observation och skattning av klientbeteenden under de videoinspelade sessionerna. Resultaten visade att klienternas acceptans- och defusionbeteende under sessioner tidigt i terapin predicerade det långsiktiga utfallet i behandlingen. Dessa samband kvarstod även när man kontrollerade för symtomförbättring fram till den skattade sessionen. Detta medan tidigare symtomförbättring inte predicerade klientbeteende senare i behandling. Deltagare i samtliga studier var patienter med kronisk tinnitus, huvudsakligen rekryterade från reguljär hörselvård. Resultaten från dessa studier indikerar att 1) acceptans och psykologisk flexibilitet kan bidra till förståelsen av tinnitusbesvär 2) ACT kan minska tinnitusbesvär hos en grupp normalhörande patienter 3) acceptans och kognitiv defusion är viktiga processer i ACT vilka är relaterade till behandlingutfallet.

Abstract:
Objectives: Creative approaches to improve access to evidence-based tinnitus interventions are required. The purpose of this thesis was to address this need by developing an Internet-based cognitive behavioural therapy intervention (iCBT) specifically for those with tinnitus in the United Kingdom (UK). A unique aspect was providing audiological, instead of psychological guidance for those undertaking the intervention. Further objectives were to evaluate whether audiologist-guided iCBT could reduce tinnitus distress and tinnitus-associated comorbidities in a UK population. Method: Initially an innovative iCBT tinnitus intervention adapted for a UK population was developed. The intervention was assessed for functionality and acceptability by both tinnitus professionals (n = 5) and adults with tinnitus (n = 29). A three-phase audiologist-guided clinical trial followed to evaluate feasibility (n = 39), efficacy (n = 146) and effectiveness (n = 92). In addition, the longer-term outcomes and unwanted effects were investigated (n = 104). A process evaluation was conducted parallel to the efficacy trial. Standardised self-reported assessment measures for tinnitus distress, insomnia, anxiety, depression, hearing disability hyperacusis, cognitive failures and life satisfaction were included. Results: The developed intervention was rated as acceptable by both professionals and adults experiencing tinnitus. In Phase I, feasibility was established in terms of recruitment potential, attrition and adherence rates. In Phase II, efficacy was evident as undertaking iCBT led to a significant reduction in tinnitus distress and tinnitus-related comorbidities (insomnia, depression, hyperacusis, cognitive failures and increase in life satisfaction). These results remained stable up to 1 year post-intervention. Unwanted treatment effects were reported by 11% of participants. Process evaluation identified intervention aspects that facilitated and hampered the outcomes obtained. Phase III results were comparable regardless of receiving iCBT or individualised face-to-face care. Conclusion: An acceptable iCBT tinnitus intervention was developed for a UK population. Original research has been undertaken, which has indicated the acceptability, feasibility, efficacy and effectiveness of audiologist-guided iCBT in reducing tinnitus distress and tinnitus-associated comorbidities in a UK population.

Abstract:
Tinnitus is defined as an auditory stimulus that is unrelated to external stimulation. There are many theories as to what causes tinnitus, therefore, there are many treatment options for tinnitus. This paper attempts to increase the audiologist's knowledge of the etiology, as well as, the most appropriate treatment for tinnitus. There are two types of tinnitus, objective and subjective. Subjective tinnitus is more common, although it is more difficult to treat than objective tinnitus. There are many theories as to what causes tinnitus. Several disorders that have tinnitus as a symptom, such as, Meniere's disease, acoustic neuroma, and dysfunction of serotonin levels, are discussed. Before treatment of tinnitus, the patient must undergo a medical and audiologic evaluation. Tests of tinnitus pitch, loudness, residual masking, and minimal masking are included. The implications of these tests on treatment are also discussed. There are many treatment options available for tinnitus, such as, electrical stimulation, medications, stress and psychological therapy, tinnitus maskers, and hearing aids. This paper focuses on mainly the treatments that are most feasible for an audiologist. In addition, included is an empirical study that was conducted to examine the effects of hearing aids and circuit type on tinnitus relief. To conclude, this paper will summarize the steps to follow in order to manage a patient that exhibits tinnitus. Although there are some treatments that seem to be more appropriate for an audiologist to utilize, (i.e., hearing aids, maskers, and Tinnitus Retraining Therapy, none have been proven to be effective in every patient. Research is still needed in this area.

Formål: Undersøke hvordan tinnitusrammede opplever å være plaget av tinnitus. I forlengelsen av denne hovedproblemstillingen undersøkes om de ulike behandlingsmetodene som er brukt av informantene er like effektive, og hva de oppgir som grunn til at de ikke har blitt behandlet. Til sist undersøkes sammenhengen i informantgruppen mellom tinnitusplage og redusert sosial-, fysisk- og mental yteevne. Metode: Det ble gjennomført en kvantitativ undersøkelse bestående av 757 tinnitusplagede. Informantene var enten medlemmer i Hørselshemmedes landsforbund (HLF) som hadde registrert seg med tinnitus, eller deltakere på kurs eller behandling ved ulike offentlige eller private behandlingsinstitusjoner. Undersøkelsen har et deskriptivt og ikke – eksperimentelt design. Surveyen er inspirert av det helserelaterte spørreskjemaet Short Form Health Survey 36 versjon 1.0 (SF 36 v 1.0) og tilpasset denne undersøkelsen. Spørreskjemaet bestod både i en elektronisk og manuell form. Resultater: Analysene viser at de tinnitusrammede informantene er noe mer sykemeldt enn landsgjennomsnittet. Nær halvparten av de respondentene som plages med tinnitus hele tiden, har problemer med å gjennomføre arbeid eller andre daglige gjøremål, og det er signifikant forskjell mellom de som plages daglig og de som plages hele tiden i forhold til om de har problemer med å gjennomføre arbeid eller andre gjøremål. Resultatene tyder også på at det er signifikant forskjell mellom de som plages daglig og de som plages hele tiden i forhold til hvordan de tror helsen vil utvikle seg. Resultatene viser at det er vesentlig for å oppnå effekt av behandlingen at tinntiusrammede benytter Tinnitus habitueringsterapi (THT). Analysene tyder på at informasjonen om behandling av tinnitus er mangelfull både i forhold til tinnitusrammede og i forhold til helsepersonell, noe som er foruroligende med henblikk på at tinnitusrammede har rett til prioritert helsehjelp. Det er ikke signifikant forskjell mellom de som plages daglig og de som plages hele tiden med hensyn til fysisk funksjonsevne og mental helse, derimot er det signifikant forskjell mellom plagegrad og sosial yteevne, noe som tyder på at det er sammenheng mellom hvor mye man plages av tinnitus og muligheten for sosial omgang.
