A cenoura é uma hortaliça de elevado valor nutritivo, sendo provavelmente umas das principais fontes de β-caroteno, no entanto a queima das folhas (Alternaria dauci) tem comprometido a cultura em decorrência da severa incidência foliar. Diante desse aspecto, o objetivo do trabalho foi avaliar a reação dos seguintes genótipos à queima das folhas: Alvorada calibrada média; Brasília; Brazlândia, Ferracini, Flakee; Nantes; Nantes Express e Suprema calibrada média nas condições da Região do Alto Vale do Itajaí/SC. Os dados de severidade foram integralizados pela área abaixo da curva de progresso da doença (AACPD) e junto com a severidade final e da produtividade foram comparadas pelo teste Scott-Knott 5%. Não houve diferença na AACPD entre os genótipos e a severidade final foi estaticamente superior nos genótipos Nantes (57,3%) e Nantes Express (53,5%). A produtividade foi superior no Brasília e inferior no Nantes Express com 112.743 e 50.054 Kg.ha-1 respectivamente nas condições do Alto Vale do Itajaí/SC.

In the 19th century, von Bismarck, the Chancellor of Germany, made an extraordinary announcement. He declared 65 to be the age of retirement, when a pension could be received. It was a somewhat cynical exercise, since it was meant to keep the veterans of the France-Prussian War quiet. It was thought they might not rebel if they got a pension. Nevertheless, since most people in those days lived to only 50, there was not much of a demand on the Exchequer. This 65-year-old cutoff is amazingly still recognized, though some sensible countries have chosen to ignore it. Demography, biological aging, personal vanity, and a host of other factors have made it a nonsense. Yet, for the time being, we still yield to it. It represents something symbolic, if not real. It is the next to last hurdle in life. It raises the specter of whether retirement is a predeath experience.

Objective.The Centers for Disease Control and Prevention implemented new surveillance definitions for ventilator-associated events (VAEs) in January 2013. We describe the epidemiology, attributable morbidity, and attributable mortality of VAEs.Design.Retrospective cohort study.Setting.Academic tertiary care center.Patients.All patients initiated on mechanical ventilation between January 1, 2006, and December 31, 2011.Methods.We calculated and compared VAE hazard ratios, antibiotic exposures, microbiology, attributable morbidity, and attributable mortality for all VAE tiers.Results.Among 20,356 episodes of mechanical ventilation, there were 1,141 (5.6%) ventilator-associated condition (VAC) events, 431 (2.1%) infection-related ventilator-associated complications (IVACs), 139 (0.7%) possible pneumonias, and 127 (0.6%) probable pneumonias. VAC hazard rates were highest in medical, surgical, and thoracic units and lowest in cardiac and neuroscience units. The median number of days to VAC onset was 6 (interquartile range, 4–11). The proportion of IVACs to VACs ranged from 29% in medical units to 42% in surgical units. Patients with probable pneumonia were more likely to be prescribed nafcillin, ceftazidime, and fluroquinolones compared with patients with possible pneumonia or IVAC-alone. The most frequendy isolated organisms were Staphylococcus aureus (29%), Pseudomonas aeruginosa (14%), and Enterobacter species (7.9%). Compared with matched controls, VAEs were associated with more days to extubation (relative rate, 3.12 [95% confidence interval (CI), 2.96–3.29]), more days to hospital discharge (relative rate, 1.46 [95% CI, 1.37–1.55]), and higher hospital mortality risk (odds ratio, 1.98 [95% CI, 1.60–2.44]).Conclusions.VAEs are common and morbid. Prevention strategies targeting VAEs are needed.

Virus-Associated Hemophagocytic Syndrome (VAHS) is a severe hematological disorder related to some viral infections. It is an illness characterized by persistent fever, pancytopenia, splenomegaly, hyperferritinemia and, the most important, hemophagocytosis observed in the bone marrow, liver and/or lymph nodes. VAHS associated with hepatitis A virus infection is rarely described, despite the high incidence of this viral infection in the population in general. There is no consensus in the literature regarding the optimal treatment of VAHS. In this article the clinical features, presumed pathogenesis, diagnostic criteria and treatment of VAHS are discussed, including description of cases of VAHS related to hepatitis A virus infection found in the medical literature.

In a 6-week feeding trial, Vernonia amygdaliana leaf meat (VaLM) was tested as a dietary ingredient in broiler starter diets. The diets comprised VaLM included at Og (diet 1), 50g per kg basal diet (incorporated at the expense of groundnut cake and wheat ofsals - diets 2 and 3 respectively) and 100g per kg basal diet (incorporated at the expense of groundnut cake -- diet 4). During the study, parameters such as characteristics, blood parameters, histopathology of some organs (liver, kidney, lungs, heat, gizzard and pancreas) and carcass characteristics were investigated. Birds fed on VaLM - based diets performed poorly, the effects of the diet being more pronounced on those fed diet 4. Addition of VaLM to poultry feed significantly (P<0.05) reduced feed intake and weight gain of experimental birds. Blood cellular constituents decreased significantly (p<0.05) in birds fed VaLM-based diets and there were serious histopathological changes within the liver, kidney, lungs, heart, gizzard and pancreas.

Despite the global growth in the use of Voter Advice Applications (VAAs), which advise users on how similar their own policy views are to the policy positions of the political parties, there have been few field experiments that isolate the causal effects of VAA use on party support. Nor has there been much investigation of how VAAs may help to ameliorate ethnically based voting divisions by refocusing voter attention on other issues. This article draws on evidence from a field experiment in the deeply divided context of Northern Ireland. We find that at the individual level party preferences are somewhat more closely related to voter ideology after the provision of advice. Yet, at the aggregate level, we find no evidence that advice leads to weaker ethno-national structuring of party support. These results suggest that while receiving advice from VAAs has some impact on users’ party preferences, there is no observable overall impact on support levels for the ethno-national blocs in Northern Ireland.

Valdecoxib (VALD) selective electrodes based on VALD-5,6-diamino-2-thiouracil hydrochloride (DTUH) (VALD-DTUH) ion pair in PVC matrix membrane were constructed. The plasticizers used were dibutylphthalate (DBP) (Electrode B) tri-n-butylphosphate (TBP) (electrode C), or dioctylphthalate (DOP) (electrode) A. The electrodes gave the linear range between 6.0×10−6-17.9×10−3, 2.5×10−5–8.3×10−3M, M and 2.5×10−6–2.0×10−2M respectively. The slopes for linear range ranged from 39.29 to 58.76mV/decade with correlation coefficients lying between 0.997, 0.989 and 0.999 respectively. The best detection limit was 1.50×10−6M for the electrode based on DOP. Selectivity coefficients of VALD related to a number of interfering cation and some organic compounds and excipients were studied using matched potential method (MPM(, and there were negligible interference caused by most of the investigated species. The pH and life time of the electrodes were also studied. The direct determination of VALD shows an average recovery of (100.08-100.61 and 99.75-101.00)% and a mean relative standard deviation of (2.70-0.61 and 2.39-0.56)% for sensor A and B respectively. The results obtained by determination of VALD in tablets using the proposed sensors which comparable favorably with those obtained by spectrophotometric method. Validation of the method shows the suitability of the electrodes for the determination of VALD in pharmaceutical formulation.

Heart disease is the leading cause of death in men and women around the world and it is a significant problem that requires remedy. Heart failure is where the heart is not pumping blood around the body as effectively as it should. Importantly, heart failure does not mean that the heart has stopped working, but it does require support in order to get it working better. Over the past few decades, there have been many innovations to help address the problems with heart failure, including developing ventricular assist devices (VADs). A VAD is a mechanical pump that is placed inside the body to support heart function and blood flow in heart patients. It works by taking blood from a lower chamber in the heart and pumping it to the body and vital organs, in the same way that a healthy heart would. Dr Wataru Hijikata is an expert in mechanical engineering and technology with a focus on healthcare outcomes. He explains that the first generation of VADs was a pulsatile flow type, but because of the complicated mechanical structure, the size of them was large and they had low durability. He notes that the second generation VAD was a continuous flow type. Because the blood flow was generated by the rotation of an impeller, the mechanical structure was simple and the size was small. However, the bearings to support the rotating impeller led to low durability. 'Third generation VADs are continuous flow types with contactless support of the impeller by magnetic bearing or hydrodynamic bearings,' Hijikata outlines. Owing to the contactless support technology, the durability of VADs has been enhanced and the size of the device is small. 'However, while there has been significant progress over the years, VADs are still far from perfect and there is much room for improvement,' he concludes. With that in mind, a team of researchers based at the Department of Mechanical Engineering in the Tokyo Institute of Technology, Japan, is investigating a means of improving VADs.

Ventricular assist devices (VADs) implanted in human body produce different levels of shear stress due to mechanical structure, causing damage for erythrocytes. The study is focusing on the morphology changes of erythrocytes causing by continue flow VADs. This study used a Hemodynamic Shearing Device (Thermo Electron Corporation) to expose erythrocytes to shear stress produced by VADs. Then free hemoglobin of plasma is measured and blood smears are respectively made to count the number of abnormal erythrocytes. The results show that the correlation coefficient of the percent of abnormal erythrocytes (PAE) and shear stress is 0.725 (p=0.027, <0.05). After blood shearing experiments the number of abnormal erythrocytes has increased with shear stress under constant exposure time, and both shear stress and exposure time contribute to morphology changes in erythrocytes. The conclusion indicates that low shear stress (lower than 450 Dynes/cm2) damage of erythrocyte is an important factor in the application of VADs.

Abstract Visceral artery aneurysms (VAAs) are rare and affect the celiac artery, superior mesenteric artery, and inferior mesenteric artery, and their branches. The natural history of VAAs is not well understood as they are often asymptomatic and found incidentally; however, they carry a risk of rupture that can result in death from hemorrhage in the peritoneal cavity, retroperitoneal space, or gastrointestinal tract. Recent advances in imaging technology and its availability allow us to diagnose all types of VAA. VAAs can be treated by open surgery, laparoscopic surgery, endovascular therapy, or a hybrid approach. However, there are still no specific indications for the treatment of VAAs, and the best strategy depends on the anatomical location of the aneurysm as well as the clinical presentation of the patient. This article reviews the literature on the etiology, clinical features, diagnosis, and anatomic characteristics of each type of VAA and discusses the current options for their treatment and management.

This paper presents general aspects of the water budget (including the latest available figures from its components), followed by some characteristics of the rainfall along the Paraiba do Sul watershed. The regional water budget theme is then discussed, taking into account the actual and future rainfall in the region. Finally, reflections and discussions regarding the water security theme (represented by the water transposition from the Paraiba do Sul watershed to the Cantareira´s system) are discussed, as the latter represents the water demand of São Paulo City. The future scenarios of the water availability are estimated using a climatic simulation model.

AbstractA new aspect of electoral campaigns in Europe, and increasingly elsewhere as well, has been the proliferation of the online voter information tools, widely known in the political science community as Voting Advice Applications (VAAs). By accessing VAAs, users are provided with information about the degree of congruence between their policy preferences and those of different parties or candidates. Although the exact mechanisms have not been rigorously investigated, a series of studies across European countries, such as Finland, Germany, Italy, the Netherlands, and Switzerland have demonstrated a link between the use of VAAs and electoral turnout. The aim of this paper is to contribute to this growing literature by analyzing previously untapped data from Greece, extending the empirical literature to a country where VAA effects have not been investigated before. The analysis indicates that the effect of VAAs in Greece is marginal to non-existent while there seems to be no evidence of the hypothesized information mechanism which purportedly drives such effects. The paper concludes with suggestions that future studies of VAA effects on turnout can address in their design.

Background Acute ischemic stroke (AIS) more frequently develops in patients with intracranial vertebral artery dissection (VAD) than extracranial VAD, and is associated with possible poor clinical outcomes. The aim of this study is to compare high-resolution magnetic resonance imaging (HR-MRI) findings and clinical features of VAD with and without AIS. Methods Twenty-nine lesions from 27 patients (15 male and 12 female patients; age range = 28–73 years) who underwent diffusion MRI and 3T HR-MRI within seven days were included. We classified VAD according to the presence of AIS lesions on diffusion MRI. Clinical features and HR-MRI findings (angiographic patterns, presence of double lumen sign, dissecting flap, posterior inferior cerebellar artery involvement, remodeling index, length of affected vessels, T1-signal intensity, area of intramural hematoma, and grades and patterns of vessel wall enhancement) were evaluated. Results Thirteen VADs with AIS and 16 without AIS were included. There were no significant differences in the clinical parameters (sex, age, risk factors, symptoms). More VADs with AIS presented as a steno-occlusive pattern than VADs without AIS. More VADs without AIS presented with aneurysmal dilation, larger mean remodeling index and longer mean length than VADs with AIS. Presence of intramural hematoma, T1-iso-signal intensity of intramural hematoma and contrast enhancement were significantly more common in VADs with AIS than without AIS. Conclusions Our study showed some differences in HR-MRI comparing intracranial VAD patients with and without AIS. Differing findings may facilitate a better understanding of intracranial VAD and risk assessment of AIS in these patients.

Abstract Although second-generation tyrosine kinase inhibitors (TKIs) show superiority in achieving deep molecular responses in chronic myeloid leukemia in chronic phase (CML-CP) compared with imatinib, the differing adverse effect (AE) profiles need consideration when deciding the best drug for individual patients. Long-term data from randomized trials of nilotinib demonstrate an increased risk of vascular AEs (VAEs) compared with other TKIs, although the natural history of these events in response to dose modifications or cessation has not been fully characterized. We retrospectively reviewed the incidence of nilotinib-associated AEs in 220 patients with CML-CP at 17 Australian institutions. Overall, AEs of any grade were reported in 95 patients (43%) and prompted nilotinib cessation in 46 (21%). VAEs occurred in 26 patients (12%), with an incidence of 4.1 events per 100 patient-years. Multivariate analysis identified age (P = .022) and dyslipidemia (P = .007) as independent variables for their development. There was 1 fatal first VAE, whereas the remaining patients either continued nilotinib (14 patients) or stopped it immediately (11 patients). Recurrent VAEs were associated with ongoing therapy in 7 of 14 who continued (with 2 fatal VAEs) vs 1 of 11 who discontinued (P = .04). Nineteen of the 23 evaluable patients surviving a VAE ultimately stopped nilotinib, of whom 14 received an alternative TKI. Dose reduction or cessation because of VAEs did not adversely affect maintenance of major molecular response. These findings demonstrate that in contrast to other AEs, VAEs are ideally managed with nilotinib cessation because of the increased risk of additional events with its ongoing use.

Valsa canker, caused by the fungus Valsa mali, is one of the most destructive diseases of apple in the primary production areas of China and other East Asian countries. Currently, there are no effective control methods for this disease. We investigated the occurrence of Valsa canker in 24 apple orchards in Shaanxi Province in concert with foliar nutrient analysis, and found that there was a significant negative correlation of leaf potassium (K) content with incidence and severity of Valsa canker. Fertilization experiments showed that increasing tree K content enhanced resistance to pathogen colonization and establishment. Apple trees with leaf K content greater than 1.30% exhibited almost complete resistance to Valsa mali. Field trials demonstrated that increasing K fertilization could significantly reduce disease incidence. Improved management of tree nutrition, especially K content, could effectively control the occurrence and development of Valsa canker.

Abstract BACKGROUND Vertebral artery dissections (VADs) are a common cause of stroke in young patients and can result in various secondary effects, including pseudoaneurysm formation. OBJECTIVE To identify differences in predisposing factors and outcomes for VADs with and without concomitant pseudoaneurysms. METHODS We retrospective chart reviewed patients who presented to our institution at the time of VAD with at least a 3-mo clinical follow-up. Demographics, VAD characteristics, treatment, and outcomes represented as modified Rankin scale (mRS) scores were collected. RESULTS Of 310 patients with a VAD included in this study, 301 patients had an identified pseudoaneurysm status, with 54 pseudoaneurysm-associated VADs and 247 VADs not associated with pseudoaneurysm. VAD patients with associated pseudoaneurysms were more likely to be female (P &lt; .004), have bilateral VADs (P &lt; .001), and have fewer vertebral artery segments affected (P = .018), and less likely to have stroke (P &lt; .008) or occlusion of the vertebral artery (P &lt; .001). There was no difference in the proportion of patients treated with antiplatelet agents (P = .12) or anticoagulants (P = .27) between the groups. VAD patients with associated pseudoaneurysms were more likely to have a higher mRS at 3-mo follow-up (P = .044) but not discharge (P = .18) or last follow-up (P = .05). VAD patients with pseudoaneurysms were equally likely to have resolution of occlusion (P = .40) and stenosis (P = .19). CONCLUSION Demographics and clinical and radiological characteristics of VADs associated with pseudoaneurysms are different from those without associated pseudoaneurysms. Vertebral artery dissections with concomitant pseudoaneurysms are neither associated with worse functional nor radiographic outcomes.

The Southwest region of Pará state, Brazil, imports 100% of all onion consumed from other regions of the country, which generates market dependence and raises the product's costs. The present study investigated parameters related to germination, agronomic performance and postharvest characteristics of five onion cultivars at different planting densities. The experiment was conducted in two stages. In the first, in a greenhouse, the germination kinetics was evaluated. In the second, in the field, the seedlings were transplanted into beds. The experimental design was in complete randomized blocks in a 5 x 3 factorial scheme with three replications. The treatments consisted of the combination of five cultivars (Mata Hari, Dulciana, Cimarron, NUN 1205-F1 and Vale Ouro IPA 11) and three line spacing (20, 30 and 40 cm) 8 cm between plants. As for the germination rate (GR), the results allowed to highlight the cvs. Cimarron and Vale Ouro IPA 11 with an 80.55% and 77.77% of GR, respectively. There was a significant difference for total bulb productivity, highlighting Vale Ouro IPA 11 in the 8 x 20 cm spacing. For the bulb diameter, cv. Vale Ouro IPA 11 obtained the best results in all classes. In the average weight of the bulbs, cv. Vale Ouro IPA 11 in the 8 x 40 cm spacing was the best treatment with 60.77g. As for total soluble solids, cv. Vale Ouro IPA 11 presented °Brix higher than the others, being significant at 1%. These results demonstrate that it is possible to produce quality onions in the Southwest region of Pará, which would reduce prices, since all commercialized onions come from regions outside the state.

Wolframite has been the principal product of the mine although several other metals of economic interest are known to occur there. The silver content of the ores was attributed by earlier workers to the presence of tetrahedrite and schapbachite, whilst this work shows matildite, pavonite, acanthite, and argyrodite to be present, together with Ag-bearing galena. The textures and compositions of these phases are described with preliminary results of fluid inclusion studies.

The results are: (1) There is a significant influence of the product innovation variable on the decision to become a foreign exchange customer; (2) There is no significant effect of location variables on the decision to become a foreign exchange customer; (3) There is a significant influence of promotion variables on the customer's decision to buy and sell foreign currencies.

Background/Aims: Autophagy is a lysosomal degradation pathway that is essential for cellular survival, differentiation, and homeostasis. Sirtuin 1 (SIRT1), a NAD+-dependent deacetylase, plays a pivotal role in modulation of autophagy. Recent studies found that autophagy was involved in the regulation of inflammatory response. In this study, we aimed to determine the effect of SIRT1 on autophagy and inflammation, and whether autophagy can regulate the inflammatory response in vascular adventitial fibroblasts (VAFs). Methods: Cell autophagy was evaluated by fluorescence microscope and transmission electron microscopy. The expression of protein and mRNA were determined by Western blot analysis and real time-PCR. The production of cytokine was detected by ELISA. Results: TNF-α induced autophagy and increased SIRT1 expression in VAFs. SIRT1 activator resveratrol enhanced TNF-α-induced VAF autophagy. In contrast, SIRT1 knockdown attenuated VAF autophagy. Both the Akt inhibitor MK2206 and mTOR inhibitor rapamycin further increased TNF-α-induced VAF autophagy. Furthermore, SIRT1 knockdown increased Akt phosphorylation and inhibited the autophagy in VAFs. However, MK2206 attenuated the effect of SIRT1 knockdown on VAF autophagy. In addition, ingenuity pathway analysis showed that there is a relationship between cell autophagy and inflammation. We found that SIRT1 knockdown increased the expression of NLRP3 and interleukin (IL)-6 and promoted the production of IL-1β in VAFs. Further study showed that autophagy activation decreased the expression of NLRP3 and IL-6 and inhibited the production of IL-1β, whereas autophagy inhibition increased the inflammatory response of VAFs. More importantly, our study showed that autophagy was involved in the degradation of NLRP3 through the autophagy-lysosome pathway. Conclusion: SIRT1 not only regulates VAF autophagy through the Akt/mTOR signaling pathway but also suppresses the inflammatory response of VAFs through autophagy.

77 Background: Early-stage breast cancer, such as DCIS, may force to complete removal of the mammary gland because of its wide spread in the breast. For such cases, skin-sparing mastectomy (SSM) should be recommended. However, the conventional oncoplastic surgery leaves some ugly scars on the breast due to large skin incisions. By applying the endoscopic video-assisted breast surgery (VABS), we can provide more aesthetic surgery with single periareolar port approach, leaving only a small and inconspicuous scar on the breast. Methods: VABS of SSM consists of periareolar skin incision, dissection of whole mammary gland, sentinel node (SN) biopsy, and axillary node (AN) dissection. After SSM, the breast reconstruction was performed by the delayed or simultaneous method. We use the implant for simultaneous reconstruction. SN biopsy was performed endoscopically with only 1 cm incision in the axilla which was marked with 3D-CT lymphography prior to surgery. For SSM, we made a skin incision of 2.5 - 3.5 cm incision at the foot-side edge of the areola, to preserve blood flow around the areola. We cut across the gland near the nipple and quickly checked the transected stump by fast frozen section. Subcutaneous detachment was applied to the entire gland. Back of the gland was peeled just above the pectoralis major muscle fascia. The liberated gland was removed through the port with a plastic bag. Results: We performed VABS on 300 patients since 2001, and SSM on 30 patients. Tumor size is 1.6 cm on average. Age is 57.2 years. 23 nipples were preserved. 17 cases were DCIS. 4 were invasive lobular carcinoma. 9 were invasive ductal carcinoma. 2 cases had SN metastasis. One case had AN metastasis. Between VABS and the conventional surgery, there was no difference in the amount of blood loss and operation time. Postoperative aesthetics was particularly good after simultaneous reconstruction. They had less sensory disturbance and high satisfaction. After 38 months, there have been no distant metastases, no postoperative deaths, and one local recurrence). Conclusions: VABS SSM with periareolar skin incision approach makes better aesthetic results.

Background Ventricular assist devices (VADs) are increasingly being implanted in children, yet there is little literature to guide anesthetic management for these procedures. Aims To describe the pediatric population presenting for VAD implantation and the anesthetic management these patients receive. To compare (a) children under and over 12 months of age and (b) children with and without congenital heart disease. Methods Retrospective review of patients aged 0 to 17 years who underwent VAD implantation at a single center between 2014 and 2019. Results Seventy-seven VADs were implanted in 68 patients (46 left VADs, 24 biventricular VADs, 6 right VADs, and 1 univentricular VAD). One procedure was abandoned. Preoperatively, 20 (26%) patients were supported with extracorporeal membrane oxygenation and 57 (73%) patients were ventilated. Intraoperative donor blood products were required in 74 (95%) cases. Postimplantation inotropic support was required in 66 (85%) cases overall and 46 (100%) patients receiving a left VAD. Infants under 12 months were more likely to require preoperative extracorporeal membrane oxygenation (42% vs 19%), have femoral venous access (54% vs 28%), receive an intraoperative vasoconstrictor (42% vs 24%), and have delayed sternal closure (63 vs 22%). Mortality was higher in patients under 12 months (25% vs 19%) and in patients with congenital heart disease (25% vs 20%). Conclusions Children undergoing VAD implantation require high levels of preoperative organ support, high-dose intraoperative inotropic support, and high-volume blood transfusion. Children under 12 months and those with congenital heart disease are particularly challenging for anesthesiologists and have worse overall outcomes.

Objective.To identify macrophage activation syndrome (MAS) in patients with systemic juvenile idiopathic arthritis (sJIA) undergoing tocilizumab (TCZ) treatment, and to confirm laboratory marker changes and responses to treatment in patients with MAS receiving TCZ.Methods.In Japan, 394 patients with sJIA were registered in an all-patient registry surveillance of TCZ as of January 15, 2012. TCZ (8 mg/kg) was administered every 2 weeks to patients with sJIA. MAS, hemophagocytic lymphohistiocytosis, or Epstein-Barr virus–associated hemophagocytic syndrome (EB-VAHS) was reported in 23 of these patients (25 events). The Safety Evaluation Committee of Tocilizumab for JIA reviewed these cases and clinically evaluated the data and laboratory findings using their own therapeutic experience. Events were categorized into 4 groups: definitive MAS, probable MAS, EB-VAHS, and non-MAS.Results.The committee’s review revealed 3 events of definitive MAS in 3 patients, 12 events of probable MAS in 11 patients, 2 events of EB-VAHS in 2 patients, and 8 events of non-MAS in 8 patients. There were 2 patients who developed 2 events: 2 events in 1 patient were classified into definitive MAS and probable MAS, and 2 events in another patient were classified into probable MAS. In patients with definitive or probable MAS, common clinical manifestations and laboratory findings of MAS were observed. Changes in laboratory data observed in patients with EB-VAHS were similar to those observed in patients with MAS.Conclusion.These results suggest that the clinical/laboratory features in the course of MAS appear to be similar among patients regardless of whether TCZ is administered. Similarities in the pathophysiological background of MAS and EB-VAHS were also suggested.

ABSTRACT In 2015, the Fundão dam of the company Samarco (controlled by Vale) collapsed, causing a huge wave of mud, causing 19 deaths and becoming the biggest environmental disaster in Brazil. As early as 2019, a new dam rupture took place, with Vale as the responsible company, with around 270 fatalities. Both disasters brought numerous social, environmental and financial problems. This study aims to analyze the impact of environmental disaster of Brumadinho and Mariana on Vale and Samarco Financial Statements, specifically the problems related to provisions, contingencies and environmental processes. The method employed with the case study was a content analysis, and data collection was carried out from Financial Statements, Reference Form and the company’s press channel over a 10-year period from 2010 to 2019. Among the main results, we observed that in 2015, the Mariana dam collapse may explain the significant increase in the coming years regarding deposits and provisions for environmental actions filed against Samarco and Vale. In 2019, it is observed that the amount moved to judicial deposits increased 45 times compared to the previous year. Considering this fact, there is a need for studies that contribute to the transparency of actions for the victims, communities affected by the tragedy, disregard of those involved and the environment, since this is not the first disaster caused by this company.

Since 1992, the Vale of Glamorgan Festival (founded in 1969 by composer John Metcalf) has focused exclusively on living composers, aiming to reach a wider audience than that traditionally associated with new music. In Metcalf's words: ‘We're not here for … the new music profession; of course, composers, ensembles and publishers are an important part of our audience, but … new music needs to go beyond that now’. However, Vale programmes do not lack challenge or stylistic diversity; alongside more recognized figures, the Festival showcases hitherto little-known composers and explores a variety of music from smaller countries and/or younger traditions. Premières are a key feature of the programming. This year, the Festival was extended to run from 9–18 May, adding a second concert to the customary single contribution from the BBC National Orchestra of Wales. Across a range of platforms, there were 19 premières of music by 10 composers. The following offers some of the highlights.

Introduction: The fine-needle aspiration has a significant role in assessing the malignancy risk of thyroid nodules. There is uncertainty regarding the value of repeat fine-needle aspiration in benign nodules. This study aims to evaluate the concordance of results in consecutive fine-needle aspiration and to study the relevance of repetition in benign results.Material and Methods: Retrospective study of the 4800 thyroid nodules fine-needle aspiration held in Instituto de Patologia e Imunologia Molecular da Universidade do Porto between January 1, 2014 and May 2, 2016. Of the initial sample, we selected the repeated fine-needle aspiration on the same nodule.Results: The first fine-needle aspiration result of the 309 nodules underwent revaluation was non-diagnostic in 103 (33.3%), benign in 120 (38.8%) and atypia/follicular lesion of undetermined significance in 86 (27.8%). The agreement between the first and second fine-needle aspiration was significantly higher in cases with an initial benign result (benign: 85.8%, non-diagnostic: 27.2% and atypia/ follicular lesion of undetermined significance: 17.4%, p < 0.005). The fine-needle aspiration repeating motifs in initially benign nodules (n = 78) were repetition suggestion in 58, nodule growth in 17 and suspicious ultrasonographic features in 3.Discussion: The fine-needle aspiration repetition in nodules with initial non-diagnostic and atypia/follicular lesion of undetermined significance result changed the initial diagnosis in a significant proportion of patients, modifying their therapeutic approach. The high concordance of results in initially benign nodules makes fine-needle aspiration repetition not cost-effective in most cases.Conclusion: The fine-needle aspiration should be repeated when the initial cytology result is non-diagnostic or atypia/follicular lesion of undetermined significance.

At present, there is no consensus regarding the optimal timing or type of neurosurgical procedure to best treat PPHVD. Conflicting data exist regarding the relative risks and benefits of two commonly used temporizing neurosurgical procedures (TNPs), ventricular access devices ([VADs] or ventricular reservoirs) versus ventriculosubgaleal (VSG) shunts. This study was designed to address this issue. Methods This is a single-center, 10-year retrospective review of all preterm infants admitted to the St. Louis Children's Hospital neonatal intensive care unit (NICU) with Papile Grade III–IV IVH. The development of PPHVD and the requirement for and type of TNP were recorded. Rates of TNP complication, ventriculoperitoneal (VP) shunt implantation, shunt infection, and mortality rates were used to compare the efficacy and limitations of each TNP type. Results Over this 10-year interval, 325 preterm infants with Grade III–IV IVH were identified, with trends showing an increasing number of affected infants annually, and an increasing number of TNPs were required annually. Ninety-five (29.2%) of the 325 infants underwent a TNP for PPHVD (65 VADs, 30 VSG shunts). The rate of permanent VP shunt implantation for all TNPs was 72.6% (69 of 95 infants). Forty-nine (75.4%) of the 65 infants treated with VADs and 20 (66.7%) of the 30 treated with VSG shunts required VP shunts (p = 0.38). There was no statistical difference between VAD or VSG shunt with regard to TNP-related infection (p = 0.57), need for TNP revision (p = 0.16), subsequent shunt infection (p = 0.77), shunt revision rate (p = 0.58), or mortality rate (p = 0.24). Conclusions Rates of IVH and PPHVD observed at the authors' center have increased over time. In contrast to recent literature, the results from the current study did not demonstrate a difference in complication rate or requirement for permanent VP shunt placement between VADs and VSG shunts. Definitive conclusions will require a larger, prospective trial.

In the validation cohort, there were 1,234 admissions, 431 ventilated patients, 2,604 ventilator days, and 56 VAEs. With manual surveillance, in the development cohort, sensitivity was 40%, specificity was 98%, and PPV was 70%. In the validation cohort, sensitivity was 71%, specificity was 98%, and PPV was 87%. With automated surveillance, in the development cohort, sensitivity was 100%, specificity was 100%, and PPV was 100%. In the validation cohort, sensitivity was 85%, specificity was 99%, and PPV was 100%. Manual surveillance detection errors included missed detections, misclassifications, and false detections.CONCLUSIONSManual surveillance is vulnerable to human error. The breast reconstruction was made by filling absorbable oxydized cellulose. The postoperative aesthetic results were evaluated by ABNSW. Results: BCS was performed on 286 patients (26 after preoperative chemotherapy) and skin-sparing mastectomy on 14. There was no serious complication after surgery. Surgical margin was minimally positive in 2. The original shapes of the breast were preserved well. The follow-up is 126 months at maximum and 74 months on average. There is 3 locoregional recurrences and 14 distant metastases. 5-year survival rate is 97.3%. With regard to TARM, The skin incision only in the axilla made better looks and shapes of the breast. It could be applied for tumors in any area of the breast without tumor nipple extension. The reconstruction with oxidized cellulose needs no excessive detachment of the skin beyond the surgical margin. The postoperative esthetic results were excellent and better. Over the last 20 years interpretive approaches within Iron Age studies in Britain have moved from the national to the regional. This was an important development which challenged the notion that a unified, British, Iron Age ever existed. However, whilst this approach has allowed regional histories to be told in their own right, there has been far too much focus on ‘key’ areas such as Wessex and Yorkshire. Our knowledge of the ‘gaps’ in-between these regions is uneven across the country and seriously distorts our understanding of the period. This situation is particularly acute in Wales where there is a paucity of very large material and structural assemblages. As with many ‘in-between’ areas, developer-funded archaeology has increased the baseline dataset, although the interpretations of those data have not developed in parallel. This paper will demonstrate that, to more fully understand the integrated regional composition of Iron Age Britain, we must give detailed consideration to the evidence from these ‘gaps’. By bringing together for the first time all of the available aerial photographic, chronological, faunal, palaeobotanical, and excavated data in one of these ‘gap’ areas, southern Glamorgan, this paper will show that through the careful analysis of the available evidence we are able to gain an understanding of different areas’ distinctive regional characters and move beyond our over-reliance on a small number of key regions.

Vertebral artery dissection (VAD) is a rare cause of ischemic stroke in young patients. The largely nonspecific symptoms and delayed presentation pose a serious diagnostic challenge. Medical management with either anticoagulation or antiplatelet therapy is recommended, but there are no reports of successful dual therapy. We report a case of spontaneous bilateral vertebral artery dissections (VADs) treated with both anticoagulation and antiplatelet therapy and a literature review on clinical presentation and the current medical and surgical management options. A 37-year-old healthy female presented to the emergency department with worsening neck pain and headache for two weeks despite over-the-counter medication, block therapy, yoga, and deep tissue neck massage. She denied any trauma but admitted to multiple roller coaster rides over the past few months. CT angiography was concerning for VADs, and MRI brain revealed multiple strokes in the left posterior inferior cerebellar artery (PICA) territory. Cerebral arteriography confirmed the diagnosis of VADs. The patient was initiated on warfarin, along with atorvastatin and aspirin. She was discharged home with no complications and followed up with neurology as an outpatient. MR angiography after three months revealed complete resolution of the dissection. The patient did not report any bleeding complications from dual therapy.

Denne artikkelen presenterer resultater fra en spørreundersøkelse om grunnskolelæreres kompetanse i musikk og deres valg av undervisningsinnhold i musikkfaget i grunnskolen. Dermed bidrar undersøkelsen til å beskrive hva lærere i musikk kan og hva de vektlegger i sin undervisning, og slik også sammenhengen mellom kompetanse og valg av undervisningsinnhold. Utvalget er 135 lærere som underviser musikk på tredje, sjette og niende trinn i fem kommuner på Østlandet. Datamaterialet analyseres gjennom deskriptiv statistikk og variansanalyser (ANOVA). Resultatene viser at de fleste lærere i musikk er allmennlærere med lite formell kompetanse i musikk fra høyere utdanning. Lærere på lave trinn har vesentlig mindre formell kompetanse enn lærere på høyere trinn, og de deltar også i mindre grad i uformell musikalsk læring gjennom musikkpraksis utenfor skolen. Det er også forskjeller i valg av undervisningsinnhold, og disse forskjellene kan ikke forklares av læreplanens beskrivelse av de ulike trinnene alene. Det ser ut til å være klare sammenhenger mellom hva lærere kan og hva de velger å vektlegge i sin undervisning. Likevel er studiens kanskje viktigste bidrag at den antyder at forskjeller i læreres musikkundervisningspraksis påvirkes av flere forhold, og kompetanse (formell og uformell), læreplanforståelse, profesjonell identitet og kjønn ser ut til å være sentrale faktorer. Thus, the survey presents a description of what teachers know, and what they emphasize in their music teaching, and the correspondence between these factors. The sample comprises 135 teachers teaching music in grades three, six and nine in five municipalities. Data is analyzed using descriptive statistics and analysis of variance (ANOVA). The results suggest that most teachers are generalist teachers with sparse musical training from higher education – teachers in lower grades less than teachers in higher grades. The lower grade teachers participate as well to a lesser degree in informal music practices outside school. There are several differences between grade three, six and nine teachers’ choice of educational content, and these differences cannot be explained by curricular differences alone. There seems instead to be systematic correspondence between teachers’ competence and educational content. The results suggest, however, that the differences regarding educational content are affected by an intricate interplay between several factors, including competence, curricular issues, professional identity and gender.Keywords: music, teachers, competence, educational content, gender

A three-dimensional study of the vegetational history of Seamer Carr from the Devensian Late-glacial to the Atlantic period has been made with the aid of six sites in the main basin. Five of these are close to the margins of the organic deposits and the sixth is from deep basin deposits. The Godwin zonation scheme has been used to allow a direct comparison with the survey of Walker and Godwin (1954). A composite section through the basin deposits, based on the detailed contour plan described in Part 1, shows that, pre-Zone IV, the open water margin abutted the dry land, and that during Zone IV reedswamp and carr gradually filled in the edges of the lake. Large expanses of fen carr occupied Seamer Carr during Zone V, but a rise in water level during Zone VI was responsible for the area reverting to reedswamp. At the Boreal/Atlantic transition, coincident with the rise in the alder pollen curve, there is a thick band of highly humified, acid peat, full of microscopic pieces of charcoal. Analysis through this band at one of the specially selected sites on East Island produced evidence of local disturbance, seemingly anthropogenic, at the wetland margin. There were two other special sites in West Embayment that were sampled for pollen analysis. The first was located over a radiocarbon dated Zone II organic deposit, sealed beneath a Zone III calcareous sand layer. The second was located in a late Mesolithic organic mud radiocarbon dated to c. 8000 BP and containing an assemblage of microliths of late Mesolithic type, identified tentatively as a composite tool (A. David pers. comm.).

Category: Sports Introduction/Purpose: It is well established that elevated vertical loadrates on footstrike are a risk factor for developing running injuries. Overstriding, or increased foot and tibial angles at footstrike, has been theorized to increase loading rate. Thus, it is often suggested that runners shorten their stride to reduce their injury risk. However, the relationship between landing alignment and loadrates has not been well established in current literature. Thus, we aimed to investigate the relationship between sagittal plane foot angle (FA) and tibial angle (TA) with vertical loadrates in both healthy and injured forefoot (FFS) and rearfoot strike (RFS) runners. It was hypothesized that as FA and TA increased, loadrate would increase for all runners. Methods: This is an ongoing study with 52 healthy runners (35 RFS, 17 FFS) and 24 injured runners (14 RFS, 10 FFS) for a total of 76 runners (51 M, 25 F; age34.3±11.4 yrs). Vertical average loadrate (VALR) and vertical instantaneous loadrate (VILR) were obtained while running at 2.68 m/s on an instrumented treadmill. All runners reported 0/10 pain during the assessment. Sagittal plane FA and TA at footstrike were measured from video recording using an open-source program. Positive FA designated RFS. Positive TA was defined as ankle anterior to the knee. Between-group differences were evaluated using paired two-tailed t-tests. Correlation coefficients (r) were computed for FA and TA with VALR and VILR (p=0.05; trend: p =0.10). Results: Healthy RFS - FA and TA were negatively correlated with VALR and VILR. Injured RFS – There was a trend toward negative correlation between TA and both VALR and VILR. Healthy FFS – TA was negatively correlated with both loadrates. Injured FFS – There were no significant correlations. Correlation coefficients, slopes, and p values are presented in Table 1. VALR and VILR were higher in RFS vs FFS runners (56±20 vs 40±10, 69±25 vs 54±12 BW/s, p<0.01), but were not significantly different for healthy vs injured runners (52±19 vs 56±26, 63±21 vs 67±27 BW/s, p=0.4). Conclusion: In contrast to current thought, preliminary results suggest that increasing FA and TA at footstrike are associated with decreasing vertical loadrates. This relationship was strongest for FA in the healthy RFS runners and weakest for FA in both the healthy and injured FFS runners. With increased FA, load attenuation may be due to increased eccentric activity of the tibialis anterior as well as increased knee flexion excursion and eccentric quadriceps activity with increased TA.

RESUMODurante a vida útil das edificações, observa-se um processo de degradação decorrente de vários fatores. Prédios históricos, isto é, aqueles que fazem parte da história cultural do município, devem ter uma atenção especial no seu restauro, o que significa que devem passar por um processo de recuperação no qual as características originais sejam preservadas. O presente estudo tem o objetivo de identificar as manifestações patológicas encontradas em uma edificação histórica situada no bairro Lomba Grande, na cidade de Novo Hamburgo, RS, conhecida popularmente como “Casa da Lomba”. Para a identificação das manifestações patológicas encontradas nessa edificação histórica em estudo, realizaram-se vistorias in loco e, posteriormente, preencheram-se fichas cadastrais com o intuito de identificar tais manifestações, bem como um modo de recuperá-las. Na referida edificação, pode-se identificar, como manifestações patológicas mais recorrentes, trincas, descolamentos e bolores. Sendo esta uma edificação histórica, as indicações de tratamento para tais manifestações devem ser realizadas por meio de técnicas menos invasivas, visando manter as características originais da edificação. Observam-se, com maior atenção, as fissuras que ainda podem estar ativas, devendo, assim, ser feita a análise com auxílio de gesso, lâmina de vidro ou fissurômetro e, também, verificar possíveis recalques de fundação que podem ainda estar ocorrendo, para evitar que as manifestações voltem a ocorrer. Faz-se necessário um cuidado maior com o restauro, por se tratar de uma edificação com valor histórico, devendo todo o tratamento ser voltado à recuperação.Palavras-chave: Manifestações patológicas. Historic buildings, that is, those that are part of the municipality’s cultural history, must pay special attention to their restoration, which means that they must undergo a recovery process in which the original features are preserved. This study aims to identify the pathological manifestations found in a historic building located in the Lomba Grande neighborhood, in the city of Novo Hamburgo, RS, popularly known as “Casa da Lomba”. In order to identify the pathological manifestations found in this historic building under study, on-site inspections were carried out and, subsequently, registration forms were filled out in order to identify such manifestations, as well as a way to recover them. In that building, it is possible to identify, as the most recurrent pathological manifestations, cracks, detachments and molds. This being a historic building, the indications of treatment for such manifestations must be carried out using less invasive techniques, aiming at maintaining the original characteristics of the building. We observe, with greater attention, the cracks that may still be active, therefore, the analysis should be made with the aid of plaster, glass slide or fissurometer and, also, to check possible settlements of foundation that may still be occurring, for prevent manifestations from reoccurring. The Upper Jurassic Kimmeridge Clay Formation (KCF) underlies much of the Vale of Pickering where it is almost wholly concealed by the Cretaceous Speeton Clay Formation and Quaternary deposits. There are few KCF inland or coastal exposures in Yorkshire with the result that the succession was stratigraphically poorly known until the 1970s oil crisis when the British Geological Survey drilled continuously cored boreholes at Marton and Reighton to examine the formation as a possible source of hydrocarbons. These were supplemented in 1987 by continuously cored boreholes drilled at Marton, Reighton, Ebberston and Flixborough by the Institut Français du Pétrole for hydrocarbons research. Taken together, the boreholes have enabled the lithological, palaeontological, geochemical and geophysical characters of the full thickness of the formation to be examined. Comparison of the KCF successions proved in Yorkshire with those in the adjacent North Sea, the East Midlands and the Dorset coast type area, shows marked variations in thickness related to penecontemporaneous faulting. However, there are only minor variations in the lithologies and faunas at any particular stratigraphical level. This appears to be due to a combination of Milankovitch-driven climatic fluctuations and pulsed variations in sea level which combined to produce similar depositional conditions throughout the English KCF at any one time. The chronostratigraphical classification of the KCF developed in southern England has therefore been shown to be applicable to the Yorkshire outcrop and the southern North Sea. The changes in sea level may be eustatic rather than regional events, but there is insufficient palaeontological evidence to enable them to be correlated with confidence with those of the standard Jurassic sea-level curve.

Based on the study, the following conclusions were made: the periods of creativity of Reza Vali and their boundaries were established, a two-stage periodization of the composer's work was proposed and developed; the date of the beginning of the first and second periods in creativity has been clarified; the names of two periods of Reza Vali's creativity are proposed and substantiated; the stylistic features typical for the first and second periods of Reza Valiʼs creativity are revealed, a namely, the first Pittsburgh period of the composer's creativeness (from the 1978 to the 2000) and the second Pittsburgh period of the musician's creativity (from 2000 to the present).

Abstract:
A qualitative needs analysis, undertaken with 33 older Italian-Australians living in Ascot Vale, identified a number of language related difficulties that had actual or potential health consequences. Indeed health services utilisation was a problem for most participants and although they all attended a general practitioner, there was usually little or no contact with agencies that provide home care, support or health enhancing information, either because of a lack of awareness on the part of participants or because they believed the services were linguistically (or culturally) inappropriate. Language support, at best, was provided by family members because the health interpreter service was rarely used. The findings suggest a lack of structural and practitioner commitment to addressing the language needs of LOTE immigrants, especially: the current financial and administrative disincentives to community based practitioners using the health interpreter services; an apparent lack of commitment on the part of health workers to use the health interpreter services; and a failure to develop mechanisms for providing them with health related information. It appears that the equity debates of the 1970s that focused on language barriers should be revisited, because the findings of this study suggest language competence is a necessary prerequisite to these older participants effectively accessing health related information or services, or participating, either individually or collectively, in the planning or implementation of their health care.

Abstract:
The purpose of this study is to investigate and determine the recognition of employee competence in companies in Eastern Indonesia. This study used a causal-quantitative design with positivism paradigm. This study was conducted at PT Vale Indonesia, PT Freeport Indonesia and PT Newmont (PT Amman Mineral) which apply BNSP certificate of competence. This study was conducted for three months. The population in this study were the employees of PT Vale Indonesia, PT Freeport Indonesia and PT Newmont (PT Amman Mineral) with a total of 3.424 employees. The sample of this research was 400 employees. The sampling technique used was the systematic random sampling. There is a positive and significant effect between competence acknowledgment and the self-concept on the intrinsic motivation. The intrinsic motivation variable is the variable that mediates the effect of competence acknowledgment and self-concept on job satisfaction. Job satisfaction has a positive and significant effect on employee performance.
