Ventricular assist devices (VADs) have become a cornerstone of therapy in the management of end-stage heart failure, both as a means of bridging to cardiac transplantation and as destination therapy for long-term quality of life improvement. Responding to medical emergencies in patients with VADs poses numerous challenges to rapid response team (RRT) providers. Managing these patients requires basic understanding of VAD function and physiology and the multitude of complications that follow their implantation. Most healthcare professionals lack exposure to VADs, and although it may seem daunting, this chapter will provide a systematic approach of how to identify, troubleshoot, diagnose, and manage VAD-associated complications and provide emergency care for the VAD patient.

Tax revenues in most African countries fall far short of what is feasible and necessary to finance basic human and economic development. Domestic tax revenue mobilization is called for, preferably in the form of a broad-based consumption tax, such as the VAT, which is less detrimental to economic growth than an income tax is. Most African VATs, however, are riddled with exemptions, exclusions, and zero rates on domestic goods and services. Hence, base-broadening rather than rate-increasing should be pursued if revenue is the goal. Base-broadening would also make the VATs less distortionary and complicated. The case for base-broadening is strengthened by the findings of incidence studies that show that the net result of fiscal systems can be equalizing if the revenue from broad-based VATs is used to finance in-kind transfers, such as healthcare and education. This highly informative and well-researched book makes the case for modernizing African VAT systems by posing the following questions. What is the role and design of a best-practice VAT? Do African VATs measure up to best practice and how productive are they of revenue? What is being done to mitigate the VAT’s regressive burden distribution? How should VAT be coordinated in regional economic communities? What are non-standard exemptions and how prevalent are they? What treatment is accorded public bodies, immovable property, financial services, and insurance? How are and should small businesses and farmers be treated? Answers to these questions are explored by reference to best practice elsewhere and on the basis of detailed analyses of country-specific VATs.

American society continues to be characterized by deep racial inequality that is a legacy of slavery and Jim Crow. What does justice demand in response? In this book, Andrew Valls argues that justice demands quite a lot—the United States has yet to fully reckon with its racial past, or to confront its ongoing legacies. Valls argues that liberal values and principles have far-reaching implications in the context of the deep injustices along racial lines in American society. In successive chapters, the book takes on such controversial issues as reparations, memorialization, the fate of black institutions and communities, affirmative action, residential segregation, the relation between racial inequality and the criminal justice system, and the intersection of race and public schools. In all of these contexts, Valls argues that liberal values of liberty and equality require profound changes in public policy and institutional arrangements in order to advance the cause of racial equality. Racial inequality will not go away on its own, Valls argues, and past and present injustices create an obligation to address it. But we must rethink some of the fundamental assumptions that shape mainstream approaches to the problem, particularly those that rely on integration as the primary route to racial equality.

Vasectomy is now well recognized worldwide as one of the safest and most effective contraceptive methods. A history should be taken and an examination should be carried out on every person requesting sterilization. Preoperative counselling should include alternative methods of contraception, complication and failure rates, and the need for postoperative semen analysis. There are no absolute contraindications to sterilization, provided that they make the request themselves, are of sound mind, and are not acting under external duress. Relative contraindications may be the absence of children, age <30 years, severe illness, no current relationship, and scrotal pain. Although various vasectomy techniques have been described, all share three essential steps: isolation of the vas deferens, delivery and interruption of the vas, and management of the vasal ends.

The Three Graces of Val-Kill changes the way we think about Eleanor Roosevelt. Emily Wilson examines what she calls the most formative period in Roosevelt's life, from 1922 to 1936, when she cultivated an intimate friendship with Marion Dickerman and Nancy Cook, who helped her build a cottage on the Val-Kill Creek in Hyde Park on the Roosevelt family land. VAR methods suggest that the monetary transmission mechanism may be weak and unreliable in low-income countries. But are structural VARs identified via short-run restrictions capable of detecting a transmission mechanism where one exists, under research conditions typical of these countries? Using small DSGEs as data-generating processes, the chapter assesses the impact on VAR-based inference of short data samples, measurement error, high-frequency supply shocks, and other features of the LIC environment. The impact of these features on finite-sample bias appears to be relatively modest when identification is valid—a strong caveat, especially in low-income countries. However, many of these features undermine the precision of estimated impulse responses to monetary policy shocks, and cumulatively they suggest that ‘insignificant’ results can be expected even when the underlying transmission mechanism is strong.

This chapter briefly reviews the challenges facing the empirical researcher in SSA, including scarce and inaccurate data, short policy regimes that make powerful inference difficult, and the lack of structural models to help interpret the data. It provides an overview of Chapters 4–6, which take three very different approaches to looking at these data: a broad search for cross-country stylized facts (Chapter 4), a detailed case study of a major monetary policy event (Chapter 5), and an examination of whether vector auto-regressions (VARs)—the workhorse empirical tool in this area—are likely to yield useful results in the SSA context (Chapter 6).

This chapter focuses on Ana Carolina's Mar de Rosas (Sea of Roses, 1977), Das Tripas Coração (Heart and Guts, 1982), and Sonho de Valsa (Dream Waltz, 1987). At a time when it was untenable to express her feminist views by way of a realist register, all three films develop a surrealist mode of expression. Indeed, Carolina's films adapt a surrealist mode of representation to critique repressive ideological constructions of femininity and seek the emancipation of the female psyche. Ultimately, her trilogy critiques those institutions and established beliefs through which presumably good, moral citizens are manufactured—the family, education, religion, romantic love, honoring the father, and the like—and reflects a desire for a new sociability and a new political system in which women are full, equal members. The gesture toward freedom in these films resonates with the second-wave women's movements and the larger struggle to escape a repressive authoritarian regime in the 1970s and 1980s.

The book puts forward a critical method for thinking about sentimentalism as a genre, mode, and political and affective outlook people have used to cultivate a sense of intimacy across distance—that is, an affective geography. Chapters intersperse theoretical reflections on the affective production of space with analyses of valleys that become vales of tears, heart-rending oratory, and weeping rock formations, as well as emplotments of narrative and continent in works by Harriet E. Wilson, Sarah Winnemucca Hopkins, Nathaniel Hawthorne, and others. Philosophers and rhetoricians regard grounds as necessary conditions for argumentation. This book takes grounds to be geopolitical, geoaffective, and geophysical, mapping them as alternately shaky, unsettling, and stabilizing. Circulating across bodies and surfaces, sympathy has enriched conditions for living at the same time that it has mercilessly enlisted some bodies and lives as the grounds for others’ well-being.

The book “Experiências Narradas: Relatos e vivências no espaço escolar” is the result of a collective formation process in the perspective of the Narrative Documentation of Pedagogical Experiences with a group of 16 Basic Education teachers, developed by the Research Group in Education, Narratives, and Teaching Experience in Secondary Education – Narratividades, of the Federal University of Vale do São Francisco – Univasf. With the narratives presented, it institutes teaching authorship and the dissemination of the experiential knowledge constituted in the daily life of urban and rural classrooms, of difference, and diversity. Each chapter is motivated by the following key question: How did I get here? Such a provocation seeks to interweave the several journeys made by each of these teachers, their challenges, victories, learnings, and inspirations. Bringing these teachers together in this publication aims to inspire many other professionals through important elements, actions, and life experiences as a permanent challenge in the professional field. Retrieving such elements, which result in several learnings within the formative process proposed, represents resistance and persistence in this union of pedagogical knowledge and teaching practices. Over the years there has been some controversy regarding the comparison between chimpanzees and bonobos. Whereas some authors have stressed their differences, others have stressed their similarities. One striking difference between wild chimpanzees and bonobos is tool use, especially in foraging contexts. While several chimpanzee populations possess tool kits formed by multiple tools (and their associated techniques) to exploit embedded resources, bonobos display no such tool specialization. However, studies in the laboratory have shown that bonobos are perfectly capable of using tools. In fact, several studies devoted to investigate the cognitive abilities underlying tool use have failed to detect any substantial differences between the two species. This chapter explores three aspects that could explain the difference between chimpanzees and bonobos in their propensity to use tools in the wild: socio-ecological factors, social versus technical cognition, and personality profiles. Au cours du temps, il y a eu beaucoup de controverse en relation aux comparaisons entres les chimpanzés et les bonobos. Alors que certains auteurs ont stressé les différences entre eux, d’autres ont stressé les similarités. Une grande différence entre les chipmanzés et les bonobos sauvages est l’utilisation des outils, spécialement en butinage. Tandis que plusieurs populations de chimpanzés possèdent des boîtes à outils diverses (et leur techniques respectives) pour exploiter les ressources, les bonobos ne montrent pas une spécialisation pareille. Cependant, les études en laboratoir ont montré que les bonobos sont capables d’utiliser des outils. En faite, plusieurs études des facultés cognitives dans l’utilisation des outils n’ont pas pu détecter de différences substantielles entre les deux espèces. Je vais explorer trois aspects qui pourraient expliquer les différences entre les chimpanzés et les bonobos en ce qui concerne leur tendance naturelle à utiliser les outils: facteurs socio-écologiques, cognition social vs. technique, et profils de personnalité.
