Abstract:
Background: Metastatic cancer is associated with an invariably fatal outcome. However, DeltaRex-G, a tumor-targeted retrovector encoding a gene-edited dominant-negative CCNG1 inhibitor gene, has induced long term (>10 years) survival of patients with chemo-resistant metastatic pancreatic adenocarcinoma, malignant peripheral nerve sheath tumor, osteosarcoma, B-cell lymphoma, and breast carcinoma. Objective: To evaluate the level of CCNG1 expression in tumors as a potential biomarker for CCNG1 (Cyclin G1-blocking) inhibitor therapy. Methods: CCNG1 RNA expression levels that were previously measured as part of whole genome molecular profiling of tumors (TCGA, N=9161), adjacent “tissues” (TCGA, N=678) and GTEx normal tissues (N=7187) across 22 organ sites were analyzed. Differential expression of CCNG1 and Ki-67 in primary (N= 9161) vs metastatic (N= 393) tumors were also compared in primary (N=103) vs. metastatic (N=367) skin cancers (i.e., melanoma). Statistical Analysis: To detect systematically differential expression of CCNG1 and Ki-67 expression between populations (e.g. tumor vs. normal), unpaired Student's t-tests were performed. Results: Enhanced CCNG1 RNA and Cyclin G1 protein expression were noted in tumors compared to normal analogous counterparts, and CCNG1 expression correlated significantly with that of Ki-67. Moreover, CCNG1 expression tended to be higher than that of Ki-67 in metastatic vs primary tumors. Conclusions: Taken together with the emerging Cyclin G1 / Cdk / Myc / Mdm2 / p53 Axis governing Cancer Stem Cell Competence, this supportive data indicates: (1) CCNG1 expression is frequently enhanced in tumors when compared to their normal analogous counterparts, (2) CCNG1 and Ki-67 expressions are higher in metastatic vs primary tumors, (3) CCNG1 expression is significantly correlated with that of Ki-67, and (4) CCNG1 may actually be a stronger prognostic marker of stem cell competence, chemo-refractoriness, and EMT/metastasis than Ki-67. Phase 2 studies are planned to identify patients most likely to respond favorably to CCNG1 inhibitor therapy.

Abstract:
Background: The shoulder is one of the most frequent sites of musculoskeletal pain, incidence of shoulder pain in primary care patients is estimated to be 11.2 per 1000 per year. a considerable number of people with shoulder pain (41%) show persistent symptoms after 1 year. Comparing the exercise therapy and ultrasound therapy (UST) for impingement syndrome. Methodology: The study was conducted at OPD of MNR Hospital in Sangareddy. The study was conducted for a period of 3 weeks. A total number of 30 subjects were selected by simple random sampling after explained to all the participants and an informed consent was taken from each subject. 30 subjects were randomly divided into 2 Groups. Exercise therapy was given to Group 1 and UST was given to Group 2. Both the Groups received exercise therapy and ultrasonic therapy in shoulder impingement syndrome. Subjects were evaluated pre and post treatment for VAS (Visual Analogue Scale) score, SPADI SCORE (Shoulder Pain and disability index) shoulder ROM. Outcome measures: VAS scale, SPADI score. Results and Discussion: To test the significance of the mean difference of two groups, unpaired t test was done. It is statistically shown that there is some significant impact in the parameters VAS, SPADI and shoulder ROM. The results showed that, group-I had more significant improvement in all parameters than group –II. Conclusion: It is concluded that exercise therapy is better than ultrasonic therapy for subjects with shoulder impingement syndrome. KEY WORDS: Exercise Therapy, Ultrasonic Therapy, Shoulder Pain and disability index, Range of Motion.

Abstract:
Placental insufficiency, as well as its complication intrauterine chronic fetal hypoxia, is one of the important problems of modern perinatology. At the same time, its numbers have been increasing in recent years. Purpose - Evaluation of the effect of ozone therapy on the blood circulation of the mother and fetus, fetoplacental blood flow and the degree of adaptation of the fetus to hypoxia according to the results of cardiotocography, ultrasound and dopplerometry. Material and methods: 38 pregnant women who applied to the Department of Obstetrics and Gynecology of Clinic No. 1 of the Samara State Medical University with fetoplacental insufficiency were divided into two groups according to the type of treatment: patients who received complex treatment with ozone therapy, and patients who received standard medical procedures. Results: about a third of women in each group had a combined extragenital pathology. Women in both groups were statistically comparable when compared in terms of general clinical characteristics, basic anthropometric data, extragenital diseases and reproductive history. The most common complications of childbirth were premature rupture of amniotic fluid (4 in the 1st group - 22.2% and 5-25% in the 2nd group) and acute fetal hypoxia (3-16.7% and 3-15%). Conclusion: Thus, the indicators of maternal-fetal circulation, fetoplacental circulation in women in the ozone therapy group were more positive than in the 2nd group, and the level of fetal adaptability to hypoxia was higher.

Biomarker testing, particularly PD-L1, is now mandatory before treatments such as pembrolizumab (Keytruda) for lung, stomach, head and neck cancer, and atezolizumab (Tecentriq) for cervical and endometrial cancer cancer but PD-L1 is important for other cancers is uncertain and Despite progress, challenges in managing the side effects and high costs of IO persist, causing concern for the NHS and other healthcare systems. Furthermore, the IO pipeline includes promising CAR-T cell therapies and cancer therapies, although these present unique toxicity and cost challenges. Immunotherapies can be used as a delivery mechanism by attaching a monoclonal antibody to a chemotherapy drug to make an antibody drug conjugate (ADC). The antibody seeks out and hones in on a specific molecule on the tumor cell, bringing the chemotherapy with it.
