Abstract:
One of the leading cause of the decrease of treatment efficacy in patient with community-acquired infections in the intensive care department is a spread of antibiotic resistance in main causative agents. Among Gram-negative microorganisms Pseudomonas aeruginosa is one of the leading causative agents, that is related to health service. A review of foreign and domestic literature concerning tobramycin – systemic form use in the current medical practice in the conditions of increasing resistance of microorganisms to the most broad-spectrum antibiotics is presented in the article. Pharmacokinetic and pharmacodynamic characteristics of tobramycin are described, results of clinical trials, that demonstrate an efficacy of its use in the combination with other antibiotics in the life-threatening infections caused by Gram-negative microorganisms are presented in the article.

After ten years of practicing oral myofunctional therapy (MFT) in our office, we contacted 131 finished cases. Of the 131, ninety-eight responded to the invitation to come for a post-treatment check-up. Three basic questions were asked of these patients: 1. Can reflex patterns such as swallowing and breathing be corrected and stabilized permanently? Can this be achieved regardless of age of patient? 2. Does the success of the therapy endure over the years, or are there relapses following treatment deemed successful at its completion? 3. What are the patients' subjective perceptions of the program's effectiveness?

Background: Currently, photon therapy is mainly used for radiotherapy, however, the long-term side effects of photon therapy are sometimes reported, especially in children, including cognitive decline, mental decline, growth retardation, endocrine dysfunction and secondary tumor.Compared with photon radiotherapy, proton therapy is a novel method of radiation therapy, which reduces acute and late radiation damage and improves patients’ quality of life. Therefore, we evaluated proton therapy on medulloblastoma patients by meta-analysis in this study. Materials and Methods: A clinical randomized controlled trial was conducted by retrieving the main databases PubMed (Medline), EMbase, Cochrane Library, Chinese Biomedical Literature Database (CBM), Chinese Zhi-wang (CNKI), Wanfang Database, and VIP Database. We analyzed the literature which matched the quality criteria for the prognostic impact of proton and photon therapy on medulloblastoma patients. Results: Ten articles were included in this study. The overall survival (OS) rate and side effects were comprehensively analyzed. The results showed that proton radiotherapy significantly reduced the side effects and recurrence rate of tumor. Conclusion: Proton therapy could significantly reduce the side effects and recurrence rate of medulloblastoma in patients.

Background: Peripheral neuropathy can be caused by diabetes mellitus, nutritional deficiencies, entrapment or Carpal Tunnel Syndrome (CTS), and idiopathic.Objective: To determine the therapeutic efficacy of Vitamin B1, B6, and B12 forte in relieving symptoms of peripheral neuropathy.Methods: This was pre- and post-experimental study involving patients with moderate peripheral neuropathy (Toronto Clinical Neuropathy Score [TCNS] ≥6 and Michigan Neuropathy Screening Instrument [MNSI] ≥7) taken from outpatient neurological clinic of Dr. Kariadi Hospital, Semarang. The patient was prescribed Vit B1 100 mg, B6 100 mg, and B12 5000 mcg once daily for 2 months. Evaluation of the numerical pain rating scale in the form of VAS and Total Symptom Score (TSS) was conducted at the first and second month. The VAS score difference test was conducted with the Wilcoxon test and TSS with the Post Hoc test and considered significant if p <0.05.Results: There were 30 patients aged 18 - 65 years, consisted of 70% female and 30% male. The etiology of peripheral neuropathy were idiopathic (40%), CTS (26.7%), DM (23.3%), and HNP (10%). There were significant differences of the VAS scale and TSS at every evaluations.Conclusion: Administration of VitB1, B6, and B12 forte relieved symptoms of moderate peripheral neuropathy with improvement of VAS and TSS scores

Abstract:
The purpose of this article is to examine the potential application of sweat rituals to group counseling, adventure therapy, and other forms of group work by describing a theoretical model for how sweat rituals work and presenting the results of a randomized comparative outcome study on the efficacy of sweat therapy. The theoretical model proposes five factors that reciprocally interact to produce the positive effects of sweat rituals: cultural priming, exercise, self-regulation, metaphorical and contextual elements, and interpersonal factors. To investigate efficacy, 85 university students were randomly assigned to either a sweat condition (group counseling in a sauna), or a non-sweat condition (group counseling in a standard office setting) for six weekly sessions. Measures included the Critical Incidents Questionnaire, Therapeutic Factor Inventory, Exercise Induced Feeling Inventory, and the Subjective Exercise Experiences Scale. Results indicated that participants in the sweat condition perceived a greater availability of therapeutic factors, especially for group cohesion and interpersonal learning, and had better attendance, less attrition, and reported sessions to be more useful. Implications for future research and practice are discussed.

It has been a pleasure to assemble this month's issue of CNS Spectrums about cognitive-behavioral therapy (CBT) for anxiety disorders in which the successful treatments of five disorders are discussed. As is evident from the five papers in this issue, most patients with pathologic anxiety can be helped with available treatment, either short-term CBT or medication. Also apparent in these papers is that the work has not been completed: some patients do not benefit from the available treatments at all, and most who do respond still remain somewhat symptomatic.
