Abstract:
Therapy is not simply a domain or form of medical practice, but also a metaphor for and a performance of medicine, of its functions and status, of its distinctive mode of action upon the world. This article examines medical treatment or therapy (in Russianlechenie), as concept and practice, in what came to be known in Russia as defectology (defektologiia) – the discipline and occupation concerned with the study and care of children with developmental pathologies, disabilities and special needs. Defectology formed an impure, occupationally ambiguous, therapeutic field, which emerged between different types of expertise in the niche populated by children considered ‘difficult to cure’, ‘difficult to teach’, and ‘difficult to discipline’. The article follows the multiple genealogy of defectological therapeutics in the medical, pedagogical and juridical domains, across the late tsarist and early Soviet eras. It argues that the distinctiveness of defectological therapeutics emerged from the tensions between its biomedical, sociopedagogical and moral-juridical framings, resulting in ambiguous hybrid forms, in which medical treatment strategically interlaced with education or upbringing, on the one hand, and moral correction, on the other.

Babywearing is defined as the act or practice of keeping an infant close to an adult’s torso by using a supporting device that straps to the front of the adult’s body (Merriam-Webster, n.d.). The practice of babywearing as an adjunct to therapy is likely to be beneficial to children and caregivers. Although research on babywearing is limited, the therapeutic benefits of “skin-to-skin care” or “kangaroo care” have been empirically established. Building on this research, this article attempts to raise awareness about babywearing by elucidating the likely therapeutic benefits for children with disabilities or special needs and areas for future research.

Summary Saliva as a microfluidic system offers numerous advantages for both general and oral health diagnostic and therapeutic procedures since its assembly is quick, stress-free, inexpensive and non-invasive. Moreover, saliva is frequently referred to as a mirror of the body due to the fact that it can reflect the physiological and pathological state of the body. More than a decade ago the term “Salivaomics” has been introduced with the aim of emphasizing the development of research, knowledge and applications of five salivary constituents: proteome, transcriptome, micro-RNA, metabolome, and microbiome. Contemporary oral health care delivery in pediatric and special care dentistry is focused toward the development of new diagnostic and therapeutical procedures that are essentially noninvasive due to common issue of intolerability to invasive procedures among these patients, with the possibility of increasing participation rates. Besides the criteria of being easily and non-invasive collected, there are additional standards that should be met before routine application in everyday clinical practice; the existence of specific biomarkers for a disease, and ability of having its biomarkers detected using present-day equipment. For example, there are recent suggestions that a salivary RNA panel could objectively differentiate children with autism spectrum disorder from their neurotypical peers. In addition, due to the ease of the administration, the oral cavity is an attractive site for the drug delivery systems development because through this route it is possible to realize mucosal and transmucosal, systemic effect. All these contemporary advances extended the salivary diagnostic approach from the oral to general health pointing towards a promising future of salivary diagnostics for personalized medicine devices.

The legal provision of valeological education of the future teacher is regulated by several national and international documents according to the hierarchical principle. Ukraine acquires rights, duties, and, most importantly, opportunities in the development of the authentic valeological space at all levels of political and social activity, gradually entering the world social, medical, psychological, and valeological community. In the state documents on education, considerable attention is paid to the implementation of a social request, a social order related to the search for new forms of training of a specialist in the educational and recreational field, who should perfectly possess the main and related professions, can solve the tasks of training specialists in conditions of competitiveness, integrativity, and intensity of activity. Now in our state, several laws stimulate the introduction of valeological education for the general masses of the population. Although in the Ukrainian legal field valeological issues are solved dualistically (as medical and extra medical, therapeutic and preventive, special and general), now there is every opportunity to make valeological culture a publicly available component of universal humanity. Naturally, the subject of reflection of various forms of social consciousness at different times was a person, his spiritual and/or physical perfection. So, within the framework of philosophy, as a joint foremother of the humanities and, to a large extent, the natural sciences, psychological, pedagogical, and medical theories of human health and existence developed.

The aim of the research was to explore how 55 Swedish independent schools, directed at (or limited to) students in need of special support (SNSS), describe their organisation, work and visions. The empirical data of the research consisted of the schools’ website presentations, which were processed and analysed in consecutive steps. The results showed that the students’ complicated school- and life situations were often combined with disabilities mainly in the neuropsychiatric field. The majority of the schools (76%) practiced both schooling and methods for treatment and care, differentiating their role from the mainstream track. Neuropsychiatric and psychological perspectives had a significant influence, reflected in how the schools describe their daily routines, therapeutic methods of treatment and access to specific categories of staff. Small groups, individual instruction and competent staff were described as specific features. Teaching content and didactic aspects were seldom highlighted. The focus on the websites was on socialisation and subjectification while qualification, i.e. knowledge development, had a more limited role. The study points to a need for further research exploring daily pedagogical practice in more depth and calls for a greater focus on student perspectives. Consequences for learning contexts are discussed in the concluding part of the article. The specialist role, the independent schools in the present study tended to take on are most urgent issues to discuss in an educational context striving for equity and inclusive learning environments. This article will consider the use of therapy television – specifically the self-help television program The Dr. Phil Show – as a locus of government. Specifically, I will examine the ways in which ethics are addressed as biopolitical problems of the self through the often disciplinary instruction of the therapist. In this respect The Dr. Phil Show is representative of a shift in the talk show genre away from the tabloid model to a pedagogical model. Self-help talk shows are increasingly concerned with the cultivation of the soul, the production of truth and the discipline of the body. I demonstrate this by analyzing a series of Dr. Phil Show episodes centered on the confession and obesity, respectively. I emphasize the connection between TV expertise – here embodied in the discourse of the expert/therapist Dr. Phil McGraw – and neo-liberal goals requiring subjects to both care, and take responsibility, for themselves.

This article explores the discourse of care at The Retreat, York by examining its birth within moral treatment, Quakerism and the birth of asylums. I develop this further by linking these discourses into subsequent movements, namely group analysis and the therapeutic community movement. Through problematizing this history, I offer a new register for considering care in communities and groups by suggesting the use of the pedagogical concept of ‘figured worlds’. Background This study was conducted to explore the role of clinical pharmacists in providing medicine and therapeutic information service in oncology care setting. Methods It was a prospective study conducted for a period of three years after implementation of medicine and therapeutic information service as an integral part of oncology pharmacy services. The medicine and therapeutic information queries were received during ward rounds, at ambulatory care and via telephone by clinical pharmacists. All the medicine and therapeutic information requests were reviewed and answered to the concerned requester(s). Answered medicine and therapeutic information requests were electronically documented in the hospital drug information database and analyzed further. Results A total of 484 medicine and therapeutic information requests were received by clinical pharmacists during period of August 2013 to June 2016. Majority of medicine and therapeutic information queries were requested by radiation oncologists (27.2%) followed by medical oncologists (26.4%), general physicians (14.04%), resident medical officers (11.7%), ambulatory care nurses (8.6%), in-patient nurses (5.1%) and patients and care takers (6.6%). Majority of the medicine and therapeutic information queries were asked for the purpose of improving patient care (48.3%) followed by to update knowledge (30.9%) and training sessions to nurses (6.6%). The most common categories of medicine and therapeutic information were adverse drug reactions and its management (21.4%) followed by dosage adjustments of chemotherapy and biologicals (15.5%), supportive care related (14.6%), contraindications (14%), drug–drug interactions (11.9%), management of co-morbidities (7.8%), chemotherapy selection in special populations (4.5%). Conclusion The provision of medicine and therapeutic information was found to be useful in providing medicine information to improve patient care and to update knowledge of health care professionals at the study hospital.

Prolonged bed rest is common in critically ill patients, and therapeutic positioning is important to prevent further complications and to improve patient outcomes. Nurses use therapeutic positioning to prevent complications of immobility. This article reviews therapeutic positions including stationary positions (supine, semirecumbent with head of bed elevation, lateral, and prone) and active repositioning (manual, continuous lateral rotation, and kinetic therapy). The physiological rationale and current evidence for each position are described. Applicable evidence-based practice guidelines are summarized. Special considerations for therapeutic positioning of critically ill obese and elderly patients are also discussed.

Diabetic foot ulcers (DFUs) are the fastest growing chronic complication of diabetes mellitus, with more than 400 million people diagnosed globally, and the condition is responsible for lower extremity amputation in 85% of people affected, leading to high-cost hospital care and increased mortality risk. Neuropathy and peripheral arterial disease trigger deformities or trauma, and aggravating factors such as infection and edema are the etiological factors for the development of DFUs. DFUs require identifying the etiology and assessing the co-morbidities to provide the correct therapeutic approach, essential to reducing lower-extremity amputation risk. This review focuses on the current treatment strategies for DFUs with a special emphasis on tissue engineering techniques and regenerative medicine that collectively target all components of chronic wound pathology.

Organization of care for patients with glomerulonephritis is a rather urgent problem today, due to the high incidence rate and the tendency to progression of renal dysfunction. When discussing treatment issues, it is necessary to pay attention to the fulfillment of three basic requirements for patients, which include normalization of the life regime (in particular, the exclusion of night work, avoiding stressful situations and heavy physical activity when having high blood pressure), the implementation of certain dietary recommendations and prolonged drug treatment [3]. Diseases of the kidneys as the main organ, the affection of which leads to a deterioration of excretory function, require a special diet with a restriction of a number of products. Against the background of glomerulonephritis disruption of the process of glomerular filtration, which in some cases leads to the development of renal failure, it is necessary to reduce the intake of food substances that can have a negative effect on the body — alcohol, smoked meats, spicy, salty foods, preserves, sausages, foods with high protein and salt content. Moreover, one should also take control of the amount of fluid consumed and ensure compliance with the frequency and regularity of food intake.

Abstract The current debate on our healthcare system has focused primarily on the cost of care. Because of drastically rising costs and their burden on our economy, government and the private sector have developed many approaches to reduce these costs. Managed care, special contracting arrangements, and government fiat have all been used to stem the tide of rising costs--with variable success. We propose that the primary goal of healthcare is the provision of patient benefit, and we describe a model that calculates an expected benefit in terms of survival and quality of life. We have applied this model to a cohort of patients undergoing coronary angiography to determine the distributions of benefit. Furthermore, we describe a reimbursement strategy that relates the expected therapeutic benefit to the reimbursement received for that therapy--the greater the benefit, the greater the reimbursement. The future of our healthcare system lies in keeping the patient at the center of the debate on the delicate balance between optimal care and societal cost.

<p class="Standard"><strong></strong><em>Dental caries is currently one of the major public health problems, given its high incidence among 6-12-year-old children. This age group of children is considered a priority group, due to the transitional period of the replacement of deciduous teeth. This article intends to present a ludic-pedagogical instrument for oral health education, targeted at these children, based on the learning of problems related to oral health through a story narrative and associated pictograms. By means of a health education manual with several pictorial representations of dentistry clinical acts, we intended to imagetically reinforce the therapeutic adherence of children to Paediatric Dentistry as well as oral health prevention care, which are considered determinant factors for oral health success amongst children. The choice of a handbook format for this purpose was considered a health education pedagogical strategy with added value to the Paediatric Dentistry appointment setting, granting patients an active and leading role in their therapeutic path. The handbook can also be of use to younger children, through parental storytelling, establishing a dyadic communication between parents, educators and professionals.</em></p>

The transition of teenagers with special needs to young adulthood is a complex period for the children and their families. This transition is especially difficult when it comes to securing needed oral health care. Residents of “lockdown” dementia units, also referred to as “Special Care Units” of skilled nursing facilities, constitute a population of rapidly escalating needs. These entail rising demands for speech-language pathology services to treat and manage symptoms of dementia. This article recounts an exploratory investigation of rehabilitation sessions with an elderly resident of a Special Care Unit, using a new computer-based program targeting cognitive-communicative capacities. Preliminary results suggest that this resident with moderate dementia achieved a higher degree of functional recovery and superior quality of life than would have been possible with more traditional therapeutic approaches alone. An iPad-based software platform was used to administer tasks to train attention, working memory, and executive functions. The resident demonstrated significant gains in task performance that were coupled with increased independence and safety, enhanced participation in non-computerized therapeutic tasks, adaptation to surroundings, and reduction of negative behaviors. The resident's improved cognitive-communicative performance was sufficient to warrant a transfer to a long-term care wing within the same facility. This proof of concept demonstration invites formulation of testable hypotheses, which should be pursued in future research on optimizing interventions for institutionalized people with dementia using leading-edge computerized therapies.

Sorafenib is the only systemic therapy to demonstrate a significant survival benefit over supportive care in robust randomised controlled trials for advanced hepatocellular carcinoma (HCC). In the context of an intense search for prognostic and predictive factors for response and efficacy of different systemic therapies (including sorafenib), a number of molecular targets have been identified, paving new avenues for potential therapeutic opportunities. Such molecular targets include somatostatin receptor (SSTR)-related alterations. In this review, we provide an overview of the various considerations relating to SSTRs as potentially novel prognostic and predictive biomarkers for HCC with special emphasis on the therapeutic potential of somatostatin analogues in HCC management.

Background An interprofessional and cross-cultural pedagogical project in community health for students in nursing, social work, anthropology and medicine at the end of the bachelor’s degree begun in 2014. After a rural context fieldwork in several Santal villages of West Bengal (India), students had to conduct a research project, based on a community-health topic. Aims This paper describes how such a pedagogical project, introducing students to ethnographic research, can initiate new ways of thinking for possible future health interventions in rural communities. Methods An inductive approach based on ethnography was used during the fieldwork, including observations, interviews, focus groups and local documentation. Results Our observations led to the finding that actions in rural health cannot be initiated without: promoting an interprofessional/interdisciplinary perspective and a culture of complexity and reflectivity; considering local populations in transition and not in a fixed homogenous situation; understanding more than imposing; taking into account local disease classification and local pragmatic solutions; considering the dialogue between bio-medicine and therapeutic pluralism; considering local perceptions and practices; considering care itineraries/pathways; and finally being conscious of our apostolic function. Conclusion Our interprofessional pedagogical project promotes a bottom-up approach in dialogue with a global health vision.

While the performance of percutaneous coronary interventions remains the domain of interventional cardiologists, the management of these patients before, during, and after the procedure is in the domain of general cardiologists, internists and primary care physicians. Therefore, for optimal patient care it is crucial that all engaged physicians should understand the procedural risks, complications and optimal treatment strategy before, during and after the procedure. Before a percutaenous coronary intervention, patients with known allergies to iodinated contrast dye should be pretreated with oral corticosteroids and H1-receptor blockers. Diabetic patients as well as patients with renal failure need special care. Hydration is crucial for patients with renal insufficiency in order to minimise the risk of contrast nephropathy. Metformin therapy should be discontinued before the procedure in patients with renal failure in order to avoid lactic acidosis, and it should be reinstituted after the procedure only when normal serum creatine level is confirmed. Double antiplatelet therapy (aspirin plus clopidogrel) should be initiated at least six hours before the procedure. While aspirin therapy after the procedure is life long, the duration of clopidogrel therapy depends on the type of implanted stent (in patients with bare stents implanted clopidogrel should be taken at least 3 - 4 weeks post procedural, and in patients with drug-eluting stents implanted clopidogrel should be taken at least 6 - 12 months after the procedure due to in-stent restenosis prevention). Patients who experience typical anginal pain in a period of one to eight month after percutaneous coronary revascularization are likely to have restenosis, and they should be reevaluated with stress echocardiography and/or repeated coronary angiography.

Abstract The AMA Guides to the Evaluation of Permanent Impairment, (AMA Guides) specifies that impairment evaluation may be conducted by a treating or nontreating physician. A treating physician who is knowledgeable about the use of the AMA Guides may be the appropriate professional to evaluate impairment, but in cases that involve pain, disability, and medicolegal (forensic) issues, a truly independent medical examiner typically is most appropriate. All specialties share the same elevated risk of iatrogenesis when treating and evaluating/forensic roles are mixed because mixing compromises the quality of care and threatens the viability of the therapeutic relationship. Further, all treating specialties share the same bias toward offering treatment for most complaints—rather than engaging in the type of cautious skepticism that is required for competent impairment evaluation and other forensic duties. Finally, no treating clinician, regardless of specialty, can offer allegiance to judicial and administrative decision-makers. Treating clinicians often find themselves in a position in which they would be cutting off a source of their own income if they were to offer opinions that the clinical presentation is not valid, work related, or injury related, nor is the patient in need of further treatment. Such a financial conflict of interest can be eliminated only by referral to independent examiners and restricting treating clinicians from becoming involved in such decisions.

Introduction. Thyroid tumors are a borderline pathology approached equally by endocrine surgeons, ENT surgeons, thorax surgeons or general surgeons. The cooperation with the endocrinologist is mandatory in preoperative balance sheet, but also in postoperative treatment. The large size of a thyroid tumor (over 10 cm) causes a big surgical risk and a special preoperative preparation. Material and method. We present two cases of giant thyroid tumors: an anaplastic carcinoma developed on a papillary carcinoma and a medullar carcinoma. We present our therapeutic options for every single case, including the surgical treatment in mixed team with thorax surgeon‚ “long time hard surgery”. In one case we performed intubation on bronchoscope to avoid tracheotomy, and in the other case we made preoperative embolization. Results. The immediate therapeutic result depends on team work with thoracic surgeon, with the endocrinologist and intensive care specialist. The final therapeutic result depends on complex oncological treatment, but also on clinical and endocrine supervision. Discussions. The decision to operate a giant thyroid tumor had to put in balance risks and benefits maybe more than in other types of head and neck tumors because of the major intraoperative risk of bleeding and of bilateral injury of recurrent nerves. Conclusions. The surgical indication must be assessed judiciously. Preoperative (endocrine and general) complete clinical balance is indispensable. The surgical intervention has to be well prepared by both patient and surgeon. Effective therapeutic result depends on the interdisciplinary collaboration and on patient compliance to treatment.

People with disabilities may experience negative relation between their own personal potentials compared to environmental expectations and potentials of healthy peers in terms of functional participation and activity limitations. Therefore, they usually need an individualized dental treatment plan. Dentists have an important role in maintaining and improving oral health in this vulnerable group. Having in mind the United Nation's Declaration on Human Rights, patients with disabilities have human rights to achieve equal health outcomes as their healthy peers. Therefore, all preventive, prophylactic, and therapeutic interventions need to be carefully planned. In addition to precise medical history, the dentist should also have basic psychological knowledge to adjust the approach to patient's needs. Improving the oral health of patients with disabilities involves a primary, secondary, or tertiary level of oral health care, depending on patient's abilities and needs. The team work and a multidisciplinary approach, with the cooperation of experts of different profiles and specialties, is the only approach that gives satisfactory results.

Актуальность работы определяется потребностью практического здравоохранения в оптимизации диагностики фактического питания и коррекции пищевых рационов у пациентов всех профилей. Статья посвящена разработке программного приложения для управления коррекцией фактического питания и массы тела в практическом здравоохранении. При реализации были использованы обзор аналогов, статистические данные, современные средства программирования, логичная и понятная архитектура. Разработанный нами программный продукт позволяет оценивать: фактическое питание, качество режима, ритма и привычек питания; физическую активность и условия труда, возрастные и гендерные особенности; индивидуальные физиологические потребности в пищевых веществах и энергии; соматическую патологию; формировать отчет по каждому пациенту, группам по нозологиям, полу, возрасту; предлагать семидневное меню с возможностью адаптации под индивидуальность пациента, выдавать индивидуализированное семидневное меню, хранить все результаты в базе данных. Применение программного комплекса оценки и коррекции питания, разработанного нами, позволяет оптимизировать процесс назначения и индивидуализации лечебного диетического и лечебного профилактического питания Chronic non-communicable diseases represent the largest group of diseases of modern man, make the greatest contribution to the overall mortality and disability of the population. Comorbid and polymorbid pathology occupies a special place in scientific medicine and practical health care and needs further comprehensive study. The article is devoted to the study and analysis of the prevalence of metabolic disorders, nutritional status and risk factors for chronic non-communicable diseases in patients with comorbid and polymorbid somatic pathology. We used survey methods using special questionnaires, a comprehensive method for assessing nutritional status, and statistical methods for processing the data obtained using computer programs. In the sample of patients with comorbid chronic non-communicable diseases, there was a high prevalence of violations of nutritional status, metabolism, and risk factors for chronic non-communicable diseases. In the presence of polymorbidity, the changes were significantly more pronounced. Close correlations between the individual parameters studied were revealed. The results of the study should be taken into account when conducting further scientific research, as well as in practical medicine within the framework of a three-level and three-stage system of organizing national health care, including when providing medical care for medical rehabilitation

Introduction: A life-threatening illness can cause the involvement of family members and the imposition of psychological and physical stress on them. Certainly, the family is a very valuable resource in patient care and plays an important role in maintaining the emotional support and patient’s recovery. The aim of this study was to explain the family members’ supporting behaviors of the patient admitted to the cardiac special units. Methods: This qualitative study was performed in the cardiac special units in Isfahan. The number of participants was 20, including 5 nurses, 8 family members, and 7 patients. The data were collected through interview and observation by purposive sampling. Then, the data were analyzed by Graneheim and Lundman’s qualitative content analysis method. The study lasted 12 months. Results: Data analysis showed that family’s support can be classified into three levels, including support by the therapeutic alliance (attempts to console and reassure, restoration of selfesteem, diminishing patient’s insensitivity, commitment to the patient, and visiting the patient ), participatory information (obtaining reliable information from the nurse, active role in providing meaningful information about the patient’s prognosis), practical and instrumental support (searching for economic support resources, providing the patient with the necessary equipment, trying to do the right care taking into account the family culture). Conclusion: Understanding family’s supportive behaviors can help improve counseling and planning for quality care of patients admitted to the cardiac care units (CCUs).

Research background. In the spectrum of psychotherapeutic interventions, artistic therapies such as painting, clay modelling, music, dance therapy, as well as poetry and bibliotherapy have become an essential part of psychosocial treatment in a wide variety of medical and therapeutic indications. Artistic therapies enable patients to express stressful emotions in a protected environment, to regulate their own experiences, to learn about resources and to better process the psychologically restrictive consequences of the illness. Empirical evidence of efficacy is still lacking in many fields of Arts Therapies. However, recent, partially controlled randomized studies on art therapy in oncology show a significant reduction in anxiety and depression, a lesser degree of experiencing stress, improved perception of health, and improved disease management. A case Vignette shows how sustainable artistic experiences can have a therapeutic effect and how significant they can become for the individual client. Conclusion. Art therapy, especially in the field of oncology and palliative care, offers special opportunities for therapeutic accompaniment, through which central objectives can be met in the context of the overall treatment. It requires further practical and scientific development work as well as good public relations in order to draw attention to the potential of art therapy and to further validate and establish it as a fixed therapeutic component in oncology and palliative medicine, both inpatient and outpatient.

Underlying mechanisms such as neuroplasticity help to understand the multifaceted effects of music in pre- and perinatal care. Individual conditions and features of the mother and her child have to be taken into account and music interventions to be harmonized with complementary perinatal programs.

The attention of neurologists, neurosurgeons, intensive care physicians has been attracted recently by the new PICS (Post Intensive Care Syndrome) symptom complex (PIC) or PIC syndrome — Post Intensive Care Syndrome. One of the most severe options for PIT syndrome is critical illness polymyoneuropathy (CIP). Polyneuropathy (Critical illness polyneuropathies, or CIP) and myopathy (Critical illness myopathies, or CIM) are common complications of critical care. Several syndromes of muscle weakness are combined under the term «Intensive care unit-acquired weakness» or ICUAW. Respiratory neuropathy is a special case of PMCS, where respiratory failure is associated with damage to the neuromuscular apparatus of external respiration. The clinical consequence of respiratory neuropathy is an unsuccessful weaning from ventilator and a long stay of patients in ICU. This systematic review of the literature is an analysis of publications devoted to the main pathogenetic mechanisms of the development of CIP and respiratory neuropathy, diagnostic methods, new therapeutic approaches to the treatment of ICU patients with respiratory neuropathy. The special attention is given to the problem of acute muscle wasting, diagnosis and correction of proteinenergy metabolism disorders in patients with respiratory neuropathy.

The variation in the practice of DHCA has not been adequately characterized and may offer multiple therapeutic opportunities for outcome enhancement and reduction of procedural risk. The International Aortic Arch Study Group (IAASSG) was recently organized to investigate an optimal neuroprotection during the thoracic aortic reconstruction.

Abstract:
This article describes the nurse practitioner (NP) led management of a possible nosocomial exposure of tuberculosis (TB) in a level 3 NICU in Toronto, Canada. 26 babies, premature and term, were identified as being at risk and multiple emergency clinics were set up to diagnose possible infection, prescribe window prophylaxis, and monitor for adverse effects to the medication. The NICU NPs were chosen to organize, co-ordinate, and manage these emergency clinics because of their skills in leadership, diagnosing, therapeutic management, and client relationship building. The clinic was able to achieve 100 percent follow up with each at risk baby, from initial assessment through to completion of window prophylaxis with negative tuberculin skin test. Some insight in to the decision making surrounding possible TB outbreak management is provided, and special considerations around therapeutic management specific to this population are discussed. This incident demonstrates how nurse practitioners can be utilized to provide high quality care, across multiple clinical situations, to meet the needs of the health care system.

<p class="06IsiAbstrak"><span lang="EN-GB">This article discusses the development of psychotherapy conducted at an Islamic boarding school. The study of the dynamics of students with special characteristics in Islamic boarding schools is still very limited. This article aims to reveal psychotherapy in the handling of students with special needs at Bait As-Shuffah An Nahdliyah, an Islamic boarding school in Rembang. The subject is a student with special needs and this student receives therapeutic care because of the need for intensive care and to explore further the student’s problems and conditions. Data obtained through observation and interviews. Through a qualitative descriptive analysis approach, the results show that Bait As-Shuffah An Nahdliyah has applied psychotherapy in a psychological and also an Islamic approach, especially in a psychosufistic approach. This shows that there is a development in Islamic boarding school's assistance models that refers to combining the application of interventions with psychological concepts and the Sufism concepts.</span></p>
