Baloh, Robert W. Politzer’s Otology Clinic and the Discovery of the Caloric Test. Oxford University Press, 2016. http://dx.doi.org/10.1093/med/9780190600129.003.0008.

Full text
Abstract:
Robert Bárány began his training in Adam Politzer’s Otology Clinic at the University of Vienna in October 1903 after completing his surgical training at the Vienna General Hospital. During his training, Bárány became friends with Gustav Alexander, who already had been offered a position in Politzer’s clinic. Alexander stimulated Bárány’s interest in the vestibular apparatus of the inner ear and was influential in helping Bárány obtain his appointment in Politzer’s clinic. It was well known in Politzer’s clinic that one had to be extremely careful regarding the temperature of the water used to irrigate the ear canals in removing cerumen, otherwise the patient would become dizzy. Bárány discovered the mechanism of this caloric reaction and eventually received the Nobel Prize in Medicine. Bárány’s findings were considered to be pivotal in clinical otology. His colleagues at the clinic were less magnanimous in their praise of Bárány’s accomplishments and questioned his integrity.

Abstract:
The existence of armed conflict is the most fundamental prerequisite for the exercise of jurisdiction over war crimes. This chapter probes the characterization of armed conflict in the case-law of the ICC. It shows that the ICC has relied heavily on the jurisprudence of the ICTY concerning the conceptualization of non-international armed conflict (e.g. Tadić) and internationalization of prima facie internal armed conflict based on the overall control test. It argues that maintaining the integrity of armed conflict as a concept of international humanitarian law is one of the greatest longer-term challenges facing the Court.

Abramovitch, Amitai. Neuropsychological Function in OCD. Edited by Christopher Pittenger. Oxford University Press, 2017. http://dx.doi.org/10.1093/med/9780190228163.003.0015.

Full text
Abstract:
This chapter reviews the neuropsychological literature in adult and pediatric OCD, and then reviews the limitations, current controversies, and caveats in this area. Characterization of neuropsychological deficits associated with psychological problems has the potential to integrate neurobiological and psychopathological research. The cognitive neuropsychology of Obsessive-Compulsive Disorder (OCD) has been extensively studied over the past three decades. This impressive body of literature indicates that individuals diagnosed with OCD tend to exhibit moderate underperformance on neuropsychological tests in most, although not all, cognitive domains. However, neuropsychological research in OCD has been notoriously inconsistent. Moreover, the presence of broad though modest deficits, rather than large discrete ones, raises serious challenges for attempts to integrate neuropsychological constructs into neurobiological and psychological models of OCD.

Full text
Abstract:
Although causality is rarely discussed in texts on decision-making, decisions often depend on causal knowledge and causal reasoning. This chapter reviews what is known about how people integrate causal considerations into their choice processes. It first introduces causal decision theory, a normative theory of choice based on the idea that rational decision-making requires considering the causal structure underlying a decision problem. It then provides an overview of empirical studies that explore how causal assumptions influence choice and test predictions derived from causal decision theory. Next it reviews three descriptive theories that integrate causal thinking into decision-making, each in a different way: the causal model theory of choice, the story model of decision-making, and attribution theory. It discusses commonalities and differences between the theories and the role of causality in other decision-making theories. It concludes by noting challenges that lie ahead for research on the role of causal reasoning in decision-making.

Lopez, Berenice, and Patrick J. Twomey. Biochemical investigation of rheumatic diseases. Oxford University Press, 2013. http://dx.doi.org/10.1093/med/9780199642489.003.0062.

Full text
Abstract:
It is important for rheumatologists to have an understanding of biochemical tests including an awareness of their limitations. The biological variability of an analyte both within and between individuals, the limitations of the measurement technology, the sensitivity of laboratory internal quality control and external quality assurance procedures, as well as interlaboratory variations in practices including sample collection procedures, may all impact on the interpretation of a result. Biochemical tests are often requested to monitor organ-specific dysfunction arising as an adverse consequence of pharmacotherapy or as a component of a systemic rheumatic disease, although dysfunction may also reflect infection or coincidental pathology. Patients with rheumatic diseases are at high risk of renal and hepatic disease. Serum creatinine and its derivative estimated glomerular filtration rate (eGFR) are the most readily available surrogate markers of GFR and are used to assess renal impairment and monitor its course. However, the use of creatinine alone lacks sensitivity and a substantial loss of function must occur before creatinine levels are increased. Additional biochemical screening for kidney damage can be performed by assessment of glomerular integrity, including proteinuria or albuminuria and haematuria. A wide spectrum of rheumatic diseases can affect the liver with various degrees of involvement and hepatic pathology. These often present with cholestatic or hepatitic biochemical profiles. The medical management of rheumatic diseases also involves medications that are hepatotoxic, and routine monitoring of liver function is recommended. This approach is not problem-free and may be improved by quantitative determinations of non-invasive markers of liver fibrosis in the future. Together with imaging techniques, biochemical tests play an important role in the assessment and differential diagnosis of metabolic bone disease.

Full text
Abstract:
Part of the “What Do I Do Now?: Emergency Medicine” series, Legal and Ethical Issues in Emergency Medicine uses a case-based approach to cover common and important topics in the legal and ethical dilemmas that surface in the practice of emergency medicine. Each unique case draws upon the four well-established principles of bioethics: beneficence, non-maleficence, respect for autonomy, and justice. Other ethical principles, such as honesty and personal integrity, are also addressed. Chapters are rounded out by key points to remember and selected references for further reading. Legal and Ethical Issues in Emergency Medicine addresses a wide range of topics including HIPPA and confidentiality, advance directives, suicidal patients, refusal of care, expert witness testimony, and more. This book is an engaging collection of thought-provoking cases which clinicians can utilize when they encounter difficult situations in the emergency department. The volume is also a self-assessment tool that tests the reader’s ability to answer the question, “What do I do now?”

Weston, Maureen A. The Regulation of Doping in U.S. and International Sports. Edited by Michael A. McCann. Oxford University Press, 2017. http://dx.doi.org/10.1093/oxfordhb/9780190465957.013.4.

Full text
Abstract:
This chapter examines the regulation of doping in U.S. professional and amateur sport, as well as the anti-doping regime for Olympic and international sport. Doping is generally defined as the presence, use or attempted use, trafficking, possession, or administration of any prohibited substance or method—as well as evading, refusing, or failing to submit to sample collection; to file whereabouts, information, or missed tests; or to tampering or attempting to tamper with any part of doping control. Doping in sport jeopardizes the health and safety of athletes, undermines the fairness and integrity of sport competition, and is outright cheating. The “war” against doping in sport is truly global, yet the regulatory schemes vary depending on the applicable sport governance authority. Doping in sport can be addressed effectively and fairly by looking at the practical, political, legal, and ethical concerns related to doping, doping control, fair process, privacy, and impact on sport.

O’Brien, Barbara, and Catherine M. Grosso. Jury Selection in the Post-Batson Era. Oxford University Press, 2018. http://dx.doi.org/10.1093/oso/9780190658113.003.0002.

Full text
Abstract:
In Batson v. Kentucky (1986), the US Supreme Court sought to eradicate racial discrimination in jury selection by prohibiting the exercise of peremptory strikes based on race. This chapter reviews the evidence that Batson has failed to protect jurors from race-based strikes and the reasons for this failure. The test for establishing racial discrimination set forth in Batson suffers from design flaws that make its enforcement difficult given common psychological mechanisms at work in the decision-making process and which may be exacerbated by the jury selection process itself. Batson seeks to remedy only intentional discrimination. Moreover, its capacity to ensure diverse juries is limited by the stages of jury selection that precede its application. Enforcing Batson effectively is critical to the system’s integrity, but no simple solutions exist to remedy the stubborn persistence of racial bias in jury selection. All of this suggests that measures are needed to strengthen Batson’s protections.

Steinmo, Sven. Historical Institutionalism and Experimental Methods. Edited by Orfeo Fioretos, Tulia G. Falleti, and Adam Sheingate. Oxford University Press, 2016. http://dx.doi.org/10.1093/oxfordhb/9780199662814.013.6.

Full text
Abstract:
Although a core insight of historical institutionalism (HI) is that history affects actors’ beliefs, values and preferences, it is difficult to test these propositions directly. This chapter argues that one way of testing HI theories is to integrate some of the methods and techniques of experimental social science. Using experimental methods, historical institutionalism can better explain how specific institutional structures, decision-making processes, and historical contexts frame individual choices and shape the broader ecology of political decisions. A combination of diverse research traditions and methodologies can illuminate the dynamic relationships between ideas, interests and institutions that yield variation in policies and preferences across cultures and over time.

Full text
Abstract:
Policy dissemination research is focused on understanding and addressing these barriers and can be defined as: the study of the targeted distribution of scientific evidence to policymakers to understand how to promote the adoption and sustainment of evidence-based policies. Policy dissemination research studies can be classified as audience research studies—which are formative assessments of policymakers’ knowledge, attitudes, and uses of research evidence and policy contexts—and intervention studies, which test the effectiveness of different policymaker-focused dissemination strategies. Outcomes of policy dissemination research studies include self-report policymaker research utilization, self-report policymaker support for evidence-based policies, and observed policymaker research utilization. There is also a need to grow the field of policy implementation research and integrate theories, frameworks, and methods across the fields of public administration research, political science, and implementation science. Among the topics covered in this book, policy dissemination research is among the least developed.

Full text
Abstract:
The First World War quintessentially defined the future of ‘minorities’ on the Continent of Europe. Even before representatives met at Versailles, a number of determined national movements had engaged in bloody campaigns bent on independence or unification with their ethnic kin. Still larger numbers of ‘small peoples’ endeavoured to integrate themselves into the norms and cultures of their home states. Likewise, governments of the era were often compelled to make peace with the demographic, political, and economic realities that “national” minority populations. The arrival of refugees and migrants represented a test for provincial communities and administrations seeking to settle the meaning of integration and national belonging. The opportunity to deport, resettle, or even liquidate populations provided a tool for states seeking to resolve national questions in a capricious manner. Even with the creation of the League of Nations, minorities in Europe regularly suffered at the hands of majoritarian governments.

Abstract:
As conclusion to theHandbook, this chapter reflects on the ways in which Chinese literary studies can and does inform the broader fields of literary studies and the humanities as such. In the past decades, Chinese literary studies has been experiencing a double perspectival shift: on one hand it has extended and expanded the scope of the field with ever more complex definitions of “Chineseness,” on the other, it has striven to integrate itself into broader intercultural, global, and comparative frameworks. From this vantage point, the chapter critically probes the role Chinese literary studies plays within world literary, comparative, and area studies approaches. Instead of constituting merely another object of world-literary theories formulated elsewhere, or an exceptional test case for cultural comparison, Chinese literature—as the chapters in theHandbookpropose—can be read as a rich reservoir of models that formulate new methodologies and inspire new insights for literary and cultural study as such in dialogue and contestation with existing local, regional, national, intercultural concepts and frameworks.

Gorman, Sara E., and Jack M. Gorman. Denying to the Grave. Oxford University Press, 2016. http://dx.doi.org/10.1093/oso/9780199396603.001.0001.

Full text
Abstract:
Why do some parents refuse to vaccinate their children? Why do some people keep guns at home, despite scientific evidence of risk to their family members? And why do people use antibiotics for illnesses they cannot possibly alleviate? When it comes to health, many people insist that science is wrong, that the evidence is incomplete, and that unidentified hazards lurk everywhere. In Denying to the Grave, Gorman and Gorman, a father-daughter team, explore the psychology of health science denial. Using several examples of such denial as test cases, they propose six key principles that may lead individuals to reject "accepted" health-related wisdom: the charismatic leader; fear of complexity; confirmation bias and the internet; fear of corporate and government conspiracies; causality and filling the ignorance gap; and the nature of risk prediction. The authors argue that the health sciences are especially vulnerable to our innate resistance to integrate new concepts with pre-existing beliefs. This psychological difficulty of incorporating new information is on the cutting edge of neuroscience research, as scientists continue to identify brain responses to new information that reveal deep-seated, innate discomfort with changing our minds. Denying to the Grave explores risk theory and how people make decisions about what is best for them and their loved ones, in an effort to better understand how people think when faced with significant health decisions. This book points the way to a new and important understanding of how science should be conveyed to the public in order to save lives with existing knowledge and technology.

Lachniet, Matthew S., and Juan Pablo Bernal-Uruchurtu. AD 550–600 Collapse at Teotihuacan. Oxford University Press, 2017. http://dx.doi.org/10.1093/oso/9780199329199.003.0006.

Full text
Abstract:
We analyze a 2400-year rainfall reconstruction from an ultra-high-resolution absolutely-dated stalagmite (JX-6) from southwestern Mexico (Lachniet et al., 2012). Oxygen isotope variations correlate strongly to rainfall amount in the Mexico City area since 1870 CE, and for the wider southwestern Mexico region since 1948, allowing us to quantitatively reconstruct rainfall variability for the Basin of Mexico and Sierra Madre del Sur for the past 2400 years. Because oxygen isotopes integrate rainfall variations over broad geographic regions, our data suggest substantial variations in Mesoamerican monsoon strength over the past two millennia. As a result of low age uncertainties (≤ 11 yr), our stalagmite paleoclimate reconstruction allows us to place robust ages on past rainfall variations with a resolution an order of magnitude more precise than archeological dates associated with societal change. We relate our new rainfall reconstruction to the sequence of events at Teotihuacan (Millon, 1967; Cowgill, 2015a) and to other pre-Colombian civilizations in Mesoamerica. We observe a centuries long drying trend that culminated in peak drought conditions in ca. 750 CE related to a weakening monsoon, which may have been a stressor on Mesoamerican societies. Teotihuacan is an ideal location to test for links between climate change and society, because it was located in a semi-arid highland valley with limited permanent water sources, which relied upon spring fed irrigation to ensure a reliable maize harvest (Sanders, 1977). The city of Teotihuacan was one of the largest Mesoamerican cities, which apparently reached population sizes of 80,000 to 100,000 inhabitants by AD 300 (Cowgill, 1997; 2015a). Following the “Great Fire”, which dates approximately to AD 550, population decreased to lower levels and many buildings were abandoned (Cowgill, 2015). Because of the apparent reliance on rainwater capture (Linn é, 2003) and spring-fed agriculture in the Teotihuacan valley to ensure food security and drinking water, food production and domestic water supplies should have been sensitive to rainfall variations that recharge the surficial aquifer that sustained spring discharge prior recent groundwater extraction.
