The first four chapters cover the reasons why a CPET may be requested, pre-test assessment, supervision of the test to ensure that it is safe, and the practicalities of the test itself. Subsequent chapters look in detail at the key CPET measurements of heart rate, ventilation, oxygen uptake, and carbon dioxide output. There are chapters on the parameters that can be derived from these basic measurements: ventilatory equivalents, oxygen pulse, and the respiratory exchange ratio. Further chapters show how the anaerobic threshold and respiratory compensation point are obtained, and how they can be used to interpret the physiological response to exercise. The role of monitoring oxygen saturation and flow–volume loops during the test is described. The third section of the book has chapters on presentation of results, interpretative strategies, and summaries of classical CPET patterns seen in different diseases. The final chapters consider the role of preoperative CPET testing and how to prescribe exercise. All chapters have pointers to further resources.

Urodynamic studies encompass several tests to investigate the hydrodynamics of the lower urinary tract during bladder storage/filling and voiding. These tests include bladder diaries, free uroflowmetry, and post-void residuals, filling cystometry, voiding pressure/flow studies, urethral pressure profiles, leak point pressures, videourodynamics, ambulatory urodynamics, and electromyography. The tests are performed after formulating a urodynamics question to objectively observe lower urinary tract function and dysfunction with the idea of choosing an appropriate treatment for the pathology. Invasive urodynamic tests require appropriate training, as per the joint statement on minimum standards for urodynamic practice in the United Kingdom, and should be performed according to the International Continence Society good urodynamics practice guidelines. It is also important to be able to interpret the traces appropriately and troubleshoot any problems occurring during the test.

Recently, a number of point-of-care tests for evaluation of platelet function and the efficacy of antiplatelet therapy has been investigated. Thrombophilia contributes to the risk of thrombosis, and a battery of complex assays is required to identify all thrombophilias. Disseminated intravascular coagulation is characterized by microthrombosis and clinical bleeding. A scoring system for overt disseminated intravascular coagulation provides a five-step diagnostic algorithm. The cornerstone of the management of disseminated intravascular coagulation is treatment of the underlying triggering condition. Heparin-induced thrombocytopenia is an adverse immunological effect of heparin therapy. Besides thrombocytopenia, the major clinical consequence of heparin-induced thrombocytopenia is an increased risk of thrombosis. The diagnosis is based on clinical symptoms and detection of platelet-activating heparin-induced thrombocytopenia antibodies. When heparin-induced thrombocytopenia is strongly suspected, it is recommended to stop heparin treatment, investigate for heparin-induced thrombocytopenia antibodies, and initiate non-heparin anticoagulant treatment.

The ACI (accusativus cum infinitivo) in Modern German is governed almost exclusively by perception verbs. For genuine OHG, the same can be said. In MHG and ENHG authors began to experiment with other verb classes as potentially governing ACIs, probably influenced by Latin, but this usage never made its way in ‘normal’ grammar. The tenacity of the exclusive association of ACI with perception verbs hints at an analysis in which the logical subject of the ACI is a constituent on its own, the predicate part of the ACI being a separate constituent. Other tests, e.g. tests for constituency, point in the same direction. This is different from Latin; here the ACI as a whole counts as constituent and can therefore as a whole function as direct object. The structural difference might account for the fact that the syntactic loan of an extended usage of the ACI never came to fruition.

Low back pain (LBP) is pain arising from the structures of the lumbar spine, including joints, discs, connective tissue, and nerves. Symptoms include pain and, muscle tightness or stiffness, with or without referral of pain to the legs. Most (95%) of LBP is managed in primary care; the rest is managed in secondary care. LBP affects nearly everyone at some point in their life and has an annual prevalence of around 40%. It is less common in children but from age 16 onwards the point prevalence for all age groups is around 25%. Many experience milder persisting symptoms interspersed with exacerbations, and 7% of adults have persisting LBP, which restricts function. This chapter discusses LBP, including definitions, differential diagnosis, context, approach to diagnosis, specific clues to the diagnosis, key diagnostic tests, treatment and therapy, prognosis, and how to handle uncertainty in the diagnosis of the symptom.

Haemostasis is a dynamic process to stop bleeding after vessel wall damage. Platelets form a platelet plug via activation, adherence, and aggregation processes. The coagulation proteins are activated one-by-one, cascading towards fibrin polymerization, a process controlled by thrombin generation. Fibrinolysis is the process responsible for fibrin mesh degradation, which is also controlled by thrombin. Besides procoagulant proteins, anticoagulant proteins maintain a balance in the haemostatic system. Measuring platelet count and function can be done as part of the monitoring of haemostasis, while coagulation times are measured to assess the coagulation proteins. Degradation products of fibrin and lysis times give information about fibrinolysis. Point-of-care monitoring provides simple, rapid bedside testing for platelets and for whole blood using viscoelasticity properties. In trauma-induced coagulopathy (TIC) platelet counts and coagulation times are still common practice to evaluate haemostasis, but point-of-care measurements are being used more and more. Medication interfering with haemostasis is frequently used in intensive care unit patients. Each (group of) drug(s) has its own monitoring tests either based on classical or novel techniques.

Microbiology laboratories play an important role in antimicrobial stewardship at the level of individual patients and the population as a whole. When empiric therapy has been started, rapid results can lead to earlier targeted treatment. Accumulated results of susceptibility tests can be analysed and used to generate local or national guidelines on empiric treatment and prophylaxis. Several methods can be used to determine microbial identity and antimicrobial susceptibility, including traditional culture-based methods and newer molecular methods such as matrix-assisted laser desorption ionization time-of-flight (MALDI-TOF) mass spectrometry, polymerase chain reaction (PCR), and whole-genome sequencing. These methods and potential advantages are reviewed. Before results are reported, expert rules are applied and results edited. At this point the laboratory can influence prescribing practices.

Invasive fungal disease can present without localization or obvious target organ involvement. These disseminated mycoses occur predominantly in patients who are immunocompromised, particularly from haematological malignancy and HIV. Candidiasis and aspergillosis are the commonest forms of disseminated fungal infection worldwide, but an increasing number of non-Candida yeasts and non-Aspergillus moulds have emerged as important causes of invasive disease in recent years. Endemic fungi such as Histoplasma capsulatum are important causes of invasive disease within limited geographic regions. Fever is the commonest manifestation of disseminated fungal infection, but other clinical features such as cutaneous manifestations may point to a specific diagnosis. Definitive diagnosis relies on the detection of fungi in tissue or blood, but serological tests can augment diagnosis in some infections. Mortality from disseminated fungal disease is high and prompt initiation of antifungal therapy—where invasive disease is suspected—is essential.

In addition to their geographical setting, ‘Mediterranean’ romances feature themes such as sea voyages, merchants, pirates, mutable identities, and the changes of fortune occasioned by the hazards of maritime travel. Floire et Blancheflor, first attested in French c.1150 and subsequently translated into many languages, provides the focal point for a discussion of medieval romance that draws inspiration from Peregrine Horden and Nicholas Purcell’s 2000 study, The Corrupting Sea. The second part of the chapter tests the longue durée of the Mediterranean thematic by examining the Hellenistic romance Callirhoe. The close parallels between the two texts, corresponding to Mikhail Bakhtin’s description of the Greek novel of adventure, also allows an assessment of their divergences as reflections of the shift from a late antique to a high medieval context.

This book examines scholarship, best practice methodologies, and examples of policy and practice from various professional fields in education and psychology to illuminate the elevated emphasis on test fairness in the 2014 Standards for Educational and Psychological Testing. Together, the chapters provide a survey of critical and current issues with a view to broadening and contextualizing the fairness guidelines for different types of tests, test takers, and testing contexts. Researchers and practitioners from school psychology, clinical/counseling psychology, industrial/organizational psychology, and education will find the content useful in thinking more acutely about fairness in testing in their work. The book also has chapters that address implications for policy makers, and, in some cases, the public. These discussions are offered as a starting point for future scholarship on the theoretical, empirical, and applied aspects of fairness in testing particularly given the ever-increasing importance of addressing equity in testing.

How can genocide be witnessed through imaginative literature? How can the Holocaust affect readers who were not there? Reading the work of major figures such as Elie Wiesel, Paul Celan, Avrom Sutzkever, Ida Fink, Wladyslaw Szlengel, Itzhak Katzenelson, and Czeslaw Milosz, Poesis in Extremis poses fundamental questions about how prose and poetry are written under extreme conditions, either in real time or immediately after the Holocaust. Framed by discussion of truth in fiction and fiction as truth, with Wiesel’s French literary memoir Night as an entry point, this innovative book explores the blurred boundary of fact and fiction in Holocaust literature in order to ask to what extent poetry and fiction can serve as testimony and what might be the criteria for literary witnessing. Wartime writing in particular tests the limits of “poesis in extremis” when poets faced their own annihilation and wrote in the hope that their words, like a message in a bottle, would somehow reach readers.
