In the textbook, in section I, the norms and standards of the official business style, genre templates, rules for preparing documents, and the basics of business ethics are set out in a simple, accessible form. It highlights aspects of business communication that, despite their importance, are not reflected in manuals on similar topics. Chapter 12 describes patient monitoring in the setting of addiction or addiction risk, and pain management, to include drug testing. The advantages, disadvantages, and types of available testing for drug use are described, with a discussion of their limitations. Body fluids and tissues for sampling include hair, blood, saliva, sweat, and urine; all yield information regarding drug use within unique limitations and at different levels of convenience. The test methodology imposes a need to know respective sensitivities and specificities, particularly in a forensic setting. Breath testing (alcohol) is not within the scope of this chapter. Recommended frequencies of urine testing are addressed. The utility of other forms of monitoring (self-report, collateral sources, PDMP) is assessed in the final section. Two tables are provided, (1) urine drug detection times, and (2) sources of false positive tests in urine immunoassay studies. Supplemental information sources are cited in a text box.

A clinical trial is a research study that assigns people or groups to different interventions and compares the impact of these on health outcomes. This chapter examines the design and delivery of clinical trials in anaesthesia and perioperative medicine covering the issues outlined below. The features of a high-quality clinical trial include well-defined inclusion and exclusion criteria, a control group, randomization, and blinding. Outcome measures may be broadly divided into counting the number of people who experience an outcome and taking measurements on people. The outcome measures selected for a clinical trial reflect the purpose of the study and may include ‘true’ clinical measures such as major postoperative complications or surrogate measures such as the results of a biochemical test. Outcome measures may be combined in a composite outcome. Assessment of health-related quality of life using a tool such as the SF-36 questionnaire is an important aspect of many clinical trials in its own right and also informs the economic analyses that may be embedded in a trial. Determining the number for recruits needed for a clinical trial requires both clinical and statistical insight and judgement. The analysis of a clinical trial requires a similarly sophisticated approach that takes into account the objectives of the study and balances the need for appropriate subgroup analyses with the risk of false-positive results. The safe and effective management of a clinical trial requires rigorous organizational discipline and an understanding of the ethical and regulatory structures that govern clinical research.

Physicians have examined urine for over 6000 years. Urine microscopy was first employed to diagnose kidney disease in the seventeenth century and remains an indispensable tool. The value of urinalysis for diagnosis and management of renal and genitourinary disease is well accepted. Urinalysis aids in the diagnosis of renal disease especially in cases when a renal biopsy is not immediately available or is contraindicated. It is most informative when done by the treating physician with knowledge of the clinical context. Inspection is done by eye. Routine chemical analysis is done by dipstick but urine microscopy is essential for it may reveal abnormalities even when chemical evaluation is normal. Dysmorphic red cells, red cell casts, white blood cells, renal cells, and specific crystals may be diagnostically important. Urinalysis and microscopy can narrow the differential diagnosis faster than many more complex tests are able to.

Abstract:
In the introduction to this chapter and interwoven throughout the text is the message that qualitative research begins and ends in writing, which in this case means that research design is a beginning point for that writing. This chapter is composed of three major sections that illustrate how team start-up is critical to how the writing will proceed down the line. The first section—Team Formation—provides detailed information on issues to consider in establishing the team in a manner that will be most beneficial to the conduct of qualitative research. The second section—Research Design and Project Organization—discusses early writing tasks, establishing a project management system, and the importance of linking all of this to a data archiving plan. Digital tools are discussed in some depth. The third section—Caring: Internalized and Externalized—suggests a novel approach to the issue of ethics and team management.

The practice of emergency medicine has changed significantly over the past several decades. It has evolved into a separate and distinct specialty with a unique knowledge base and training program. As the specialty has evolved, so too has the complexity of patients being cared for in the emergency department (ED), as well as the range of tests and treatments being performed. For many of these patients, the acute management of pain and anxiety is an essential component of their ED care and fundamental to the performance of diagnostic and therapeutic interventions. Consequently, possessing an arsenal of anesthesia techniques is essential to the safe and effective practice of modern emergency medicine. This chapter reviews the provision of such anesthesia in the ED setting.

Complaining that he gets too many emails, he wants their team to start communicating through text messages. But he forgets that mobile service plans are fairly expensive for many of his employees. What happens in the “Flicked Her a Nickel” story sets the stage for how employees, managers, organizations, friends, and family negotiate for control over mobile communication. This chapter introduces a focus on how communication happens around mobile devices and shows how this research contributes to three fields: mobile communication, organizational communication, and management information systems. It introduces key terms, previews the chapters, and teases the reader with a few of the unexpected findings.

Ascertaining an individual’s history of alcohol consumption is an important component in the proper treatment of accidental trauma or acute or chronic illness, as well as for matters of public health and safety, legal issues, insurance coverage, and the management of and recovery from hazardous/harmful levels of alcohol consumption. Although self-report of alcohol consumption in both research and clinical settings represents the most common mode of assessment, there is long-standing interest in developing objective measures of alcohol consumption that do not rely on the ability or willingness of a person to truthfully report consumption. Biologic diagnostic tests or biomarkers can provide information on current and past quantity and frequency of alcohol consumption. This chapter discusses and evaluates many of the biomarker candidates that have been investigated and provides insights into future searches for optimal diagnostic tools to provide biologic evidence of duration, quantity, and frequency of individual alcohol consumption. We have included a limited discussion of biomarkers for assessing cannabis use since cannabis and alcohol use many times are a concomitant feature of intoxication.

Conservation physiology is a rapidly expanding, multi-disciplinary field that uses physiological tools to characterize and solve conservation problems. This text provides a consolidated overview of the scope, purpose, and goals of conservation physiology, with a focus on animals. It outlines the major avenues by which conservation physiology is contributing to the monitoring, management, and restoration of animal populations and defines opportunities for growth in the field. Thoroughly updated, referenced and interspersed with case examples, the book reviews how to get the most out exercise testing, without and with ancillary imaging. In addition to evaluation of ST segment depression, other powerful tools to detect ischemia and forecast the future are reviewed to increase the diagnostic accuracy and prognostic ability of exercise testing. The recognition and significance of exercise induced arrhythmias and conduction defects are examined. When to convert to pharmacologic stress or add ancillary imaging, including myocardial perfusion imaging, echocardiography, coronary calcium scoring, and magnetic reference imaging are reviewed. The use of stress testing in the management of obstructive and non-obstructive coronary artery disease (CAD), heart failure, cardiac rehabilitation, peripheral vascular disease, congenital heart and other cardiovascular diseases (CVD) is examined. Options to optimize the diagnostic capabilities of exercise and other diagnostic testing for women are highlighted. Strategic use of exercise testing in the face of a decreasing burden of CAD in the developed world, as well as the opportunity to rely on exercise testing as the first test to evaluate CVD in the developing world, are reviewed. The fundamentals of exercise physiology and myocardial ischemia that serve as the foundation for exercise testing in health and disease are explained.
