Abstract:
Abstract Given its peculiarities, terminality highlights the need for an individualization of the therapeutic plan and integrality of assistance in health care. This article analyzed 23 scientific publications with thematics related to terminality and palliative care and discussed the therapeutic approach to the terminally ill patient and the incorporation of different integral practices in health. We sought to evidence that recognizing the characteristics of terminality makes it possible to establish the adequate prognostic study and implement a plan of care that supplies the necessities of the terminally ill patient, with care based of bioethical principles, respecting the will and particularities of the individual. We conclude that the palliative care constitutes an important instrument in managing biopsychosocial and spiritual angst of terminally ill people, by making ample assistance in care possible, promoting dignity, minimizing suffering, and bettering the quality of life of these patients and their families.

Abstract:
The leading questions about delivery of terminal care have shifted somewhat over recent years. Not long ago the major questions were a) Is there a need for palliative care services? b) Does hospice care make a difference to the well-being of the patient and family?, and c) Is hospice care less costly than conventional care? Although continued investigation is needed into these questions, another set of issues has started to take precedence: the ability of various hospice programs to survive financially, the contributions of various models of hospice care, the impact of Medicare legislation on the hospice movement, the demonstration of an unique hospice intervention, the integration of spiritual care into hospice programs, and the role of hospice in the care of individuals with AIDS. These topics are examined, with reference to existing studies where available, and suggestions for future research are given.

Abstract:
This paper describes provision for dying patients in the Shetland Isles, both in the author's practice – which serves 2,750 people, living in a 15-mile radius of Scalloway – and also the area as a whole, with about 23,000 people living on the 17 inhabited islands of the Shetland group. The possibilities for improving care are considered and existing models for hospice-type services are examined – and found to have little relevance to an area which is remote and has a small population. The importance of good symptom control and good nursing care is noted, and ways to improve these are put forward. The family doctor, who has adequate training and expertise in terminal care, is seen to have a unique opportunity for looking after both the dying patient and his family. This is particularly relevant in an area far from specialist services, because such patients are rarely more than 30 minutes by car from their family doctor's surgery.
