Full textO principal objetivo dessa tese foi analisar as flutuações de longo-prazo na distribuição e abundância de ovos e larvas de Engraulias anchoita, uma espécie de peixe de importância econômica e ecológica, na Plataforma Continental Sudeste do Brasil (PCSE). Nós analisamos amostras e dados abióticos de dezoito cruzeiros oceanográficos realizados durante o fim da primavera e o começo do verão de 1974 a 2010. Dois estoques distintos foram identificados com base no tamanho dos ovos, um predominante e com menor tamanho e outro de maior tamanho ao sul da PCSE. Através de \"krigagem\" indicativa, foram identificadas áreas de desova ocasional (como ao norte de Florianópolis e a área ao largo da baía de Santos) e áreas em que a desova foi evitada (como em frente à Ilha de São Sebastião e ao Sistema Costeiro Cananéia-Iguape). Usando modelos inflacionados de zeros, os fatores espaciais (diferentes áreas e profundidades amostradas) foram relacionados com a probabilidade de se amostrar falso zero, enquanto os fatores temporais e oceanográficos (diferentes anos e temperatura) foram relacionados com a abundância de ovos e larvas. Apresentamos também uma metodologia mais rápida e mais eficiente para identificar os ovos de E. anchoita, comparamos as amostragens realizadas com duas malhagens diferentes e analisamos variações sazonais do tamanho dos ovos capturados. Assim, nossos resultados poderão auxiliar estudos futuros e também no manejo pesqueiro da espécie em questão, ainda não explorada comercialmente na área de estudo.

Full textSingle-organ respiratory failure defines chronic respiratory failure. Obesity hypoventilation syndrome is the main cause of chronic respiratory failure and occurs in 4 to 5% of obese patients. Chronic respiratory failure is also the end-stage evolution of chronic obstructive pulmonary disease that has a prevalence of 6 to 8% in the adult population. The incidence of these diseases increases so does the incidence of chronic respiratory failure. In this thesis, we will evaluate novel diagnostic and therapeutic modalities that could improve the care of patients with chronic respiratory failure. Regarding diagnostic modalities, we have seen that evaluating the work of breathing with surface parasternal electromyography was an independent prognostic marker in patients with chronic obstructive pulmonary disease. We have also seen that it was a relevant tool to predict the clinicalefficacy and compliance to home non-invasive ventilation. Regarding therapeutic modalities, we have shown that the use of a semi-automatic mode of non-invasive ventilation had the same efficacy of a standard mode with a shorter length of stay for its setup. We have shown the relevance and feasibility of the use of high-flow oxygen therapy in the home setting whilst it was only used in intensive care units. Finally, we have shown the benefits of continuous positive airway pressure during exertion in patients with tracheobronchomalacia. Regarding patients’ follow-up, we have shown that the use of data from built-in software could predict the onset of a severe exacerbation of chronic obstructive pulmonary disease. However, we also show that the implementation of tele-medicine in patients with chronic respiratory failure cannot be included in daily clinical practice yet. In this thesis, we have identified novel physiological tools, novel ways to administer treatments and novel follow-up tools that can improve the management of patients with chronic respiratory failure

Full textPulmonary rehabilitation (PR) is recommended in the management of subjects with ventilatory impairment to improve their quality of life. Although a large body of evidence support its use, only few subjects benefit from it and the optimal training modality has not been determined yet. In this context, the use of new and existing tools to optimize access as well as the effects of the program are major developments that deserve to be studied. As part of this thesis, we sought to explore these two major issues (1) by considering a rehabilitation model relocated outside the PR centres while assessing the obstacles to this model and (2) exploring the effectiveness of different add-on to PR in further optimizing the benefits of the program. In the first part, we have shown, through several retrospective studies and an original prospective multicentre contribution, that the six-minute stepper test can be used to prescribe endurance training, particularly for those patients with a mild to moderate chronic obstructive pulmonary disease (COPD), but not to prescribe muscle strengthening. Furthermore, we have shown in a cohort of 105 subjects referred for PR that the use of a remote tele monitoring device was feasible, valid and widely accepted. Finally, we explored the prevalence of cognitive dysfunction, another systemic impairment of COPD that could compromise the relocation of the program, and showed that it was a very common condition (around 75% of the subjects) but that it could improve following PR and did not seem to influence the use of a remote tele monitoring device. In the second part, we evaluated the effects of different add-on used to potentiate the benefits of the PR program. In a cross-over study of 21 COPD patients, we showed that non-invasive ventilation did not improve endurance exercise capacity due to technological limitation of the ventilator. Through a multicentre randomized controlled study carried out in 73 patients with severe to very severe COPD, we have shown that neuromuscular electrical stimulation at home, performed in addition to a PR program, did not provide further benefits on quality of life or exercise capacity. Finally, through a randomized cross-over double-blind study carried out in 10 patients, we were unable to show the effectiveness of transcutaneous nerve electrical stimulation in improving their endurance exercise capacity. Finally, in a last part, we highlighted the research currently carried out in our laboratory following the original contributions described during this thesis, as well as new area of research in order to pursue the themes explored. Thus, two systematic reviews and meta-analysis (the first about nasal high flow therapy in subjects with stable COPD and the second about the use of advanced telehealth technologies to deliver PR) will serve as a basis for future research

In Taiwan, due to the trends of population aging, chronic shifting of morbidity pattern, disability as the main stream of health problems, and the care tending to be complicated with long-term custody, it has formed a big and spade burden to the society and the country. The study be focused on the operation & practice of elderly care of one local hospital settings in eastern Taiwan, having organized and analyzed their databank retrospectively over their sociodemography, health status or background [co-morbidity index by Quan(CCI-Q) & 16 morbid conditions accelerating or pre-matured aging decay], certified status of the disabled, and their long-term care (custody) needs(LTCN; ADLs & IADLs), for some description, correlation and regression ultimately, concurrently reflecting the suitability of current scheme in Taiwan. The result showed, among the elderly, age, female, low education, urbanization, and presence of care-worker(s), all well-correlated with disability or LTCNs(IADLs rather than ADLs), and those with higher CCI-Q, and with 16 unique morbidities accelerating or pre-matured aging decay also well-correlated (with disability or LTCNs, IADLs rather than ADLs), health status or background has been moderately negative correlated with ADLs indices(rs=-0.420;rs=-0.458) and IADLs marks(rs=-0.351;rs=-0.397).Hence, if chronic morbid conditions could be handled pertinently with generalized summation/aggregation and well listing during medical workup, it would be helpful to set up the global disability components or LTCNs(ADLs/IADLs) status. Moreover, under total overall regression, it showed the significant parameter of urbanizing(β=15.784), followed by living alone(β=14.994);rather, taking into certified disability with leveling for regression, the significant parameter shifted to extremely/severe disability dominating inversely(β=-26.917), followed by urbanizing(β=16.666). Nevertheless, the impact or prediction from other sociodemographic or health variables had less significant relatively. Thru’ stepwise regression, focusing on ADLs, sociodemography could explain 55.8% of variance(p<0.001), rather, upto 60.2%(p<0.001) adding with health status;certified disability leveling might upgrade to 67.8% (p<0.001). Nevertheless, focusing on IADLs, sociodemography could explain 54.3%(p<0.001);adding with health status, upto 57.6%(p<0.001);more with certified the leveling, more upto 66.8%. The study results showed that LTCN and certified disability positioning were mutually interacted, esp. with extremely/severe level ones. Both the elderly with LTCNs and certified disabilities should be unhooked somewhere properly, and might be taken either in those with duality in nature. There had been various areas with different urbanization levels in Taiwan, the study here could showed it just in an instance. However, currently in Taiwan, it seemed those with higher disability or LTCNs leveling might stay in urbanized zones, and with care-worker (i.e. good service utilization);conversely, those with under urbanized areas, living alone, undernourished(i.e. insufficient service utilization), those with lower disability or LTCNs leveling staying in sub-urbanized or undernourished zones. And certainly, still no definite or clear causal relationship or dependence linkage can be elucidated. Thus, originally it has been infeasible or with difficult to attain or implement the goal of “age in place” somewhere, and rather, mandatorily with strong reviewing and well-discussion on/to need/supply and resources allocation with uncertain-basis to LTC policy/tactics contemporarily.

Over 98.04% of energy consumed in Taiwan are imported from overseas, wherein the total oil imports 48.52%, coal accounted for 29.2%, as can be seen the main source of electricity is the thermal power generation; However, air pollution caused by thermal power is seriously affecting people's health. On the other hand electricity is a no way to store energy, the demand for electricity depending on time, season, and user load change. In order to meet demand of electricity user, the electricity industry to set various power equipment to sustain a stable supply of adequate electricity. And electricity users in the use of electricity are easy charge exorbitant fines because the situation is not clear electricity usage, but also makes the electricity industry equipment damage. It will be necessary to actively demand forecast, to reduce the risk of fines and ensure electricity industry equipment life. To achieve this objective in this paper collected 18331 rows in 15 minute data in an office building of Taiwan, between September 2014 and September 2015. Through fuzzy set calculate membership function to fuzzifier feature, amplification of the immune network of artificial immune system and use collaborative filtering find similarity neighbor calculate adjustment value to forecast Short-Term Electricity. The method proposed in this study to predict the short-term electricity demand in Taiwan still on good performance.

According to the latest data of demographical yearbook from the Ministry of Interior, it showed the elder people of over 65 years have 2,832,839 aged people in the end of 2015 Taiwan includes 12.08% of 23,445,534 on the total population, and gets up to the World Health Organization (WHO) demonstrated that the old people''s rate exceeds population 7% of the countries are called and melted in the society at advanced age over 65 (i.e. aging society); it could be seen Taiwan was inclining to aging society. In the aging society, most of vulnerable elders’ personal sense of security had showed their need for medical caring, living care, relaxations, spiritual life, and so forth. Followed by the increase of elderly population, the demand of long-term caring and the organization with elderly care have been raised for aged people. The purpose of this study was to explore the professional management in strategy of advertising was involved in Internet advertisement; moreover, how to take the marketing project into account when it popularizes a set of marketing element and expect the project would be a reference to draft a strategy of the provision of older-person-centered. By virtue of participating in concerning the elder people of the community, it is helpful to realize Taiwan’s present situation of the organization with elderly care. This study utilized practical operation methods to analyze that the effects of engaging in service management for non-profit organization (NPO). The result could assist the elder people to understand the long-term care center’s customization and service. Long-term care centers with elder people utilized the advisement on the Internet and a set of marketing involved in the community of elder-care center. The centers believe that it is useful for elder people toward acquiring online information to put the methods into the community of elder-care center. The community elder care plays an important value in the entire society. By means of the long-term with elder care center’s advertising in the Internet and a set of marketing, this center provides elder people with online information in order to realizing its customization and service. Eventually, the aged people are satisfied with the service from long-term care centers.

Full textThe goal of this study was to investigate the reference frames used in perceptual encoding and storage of visual motion information. In our experiments, observers viewed multiple moving objects and reported the direction of motion of a randomly selected item. Using a vector-decomposition technique, we computed performance during smooth pursuit with respect to a spatiotopic (nonretinotopic) and to a retinotopic component and compared them with performance during fixation, which served as the baseline. For the stimulus encoding stage, which precedes memory, we found that the reference frame depends on the stimulus set size. For a single moving target, the spatiotopic reference frame had the most significant contribution with some additional contribution from the retinotopic reference frame. When the number of items increased (Set Sizes 3 to 7), the spatiotopic reference frame was able to account for the performance. Finally, when the number of items became larger than 7, the distinction between reference frames vanished. We interpret this finding as a switch to a more abstract nonmetric encoding of motion direction. We found that the retinotopic reference frame was not used in memory. Taken together with other studies, our results suggest that, whereas a retinotopic reference frame may be employed for controlling eye movements, perception and memory use primarily nonretinotopic reference frames. Furthermore, the use of nonretinotopic reference frames appears to be capacity limited. In the case of complex stimuli, the visual system may use perceptual grouping in order to simplify the complexity of stimuli or resort to a nonmetric abstract coding of motion information.
