Abstract:
Esta tese discute a organização tecnológica do cuidado em saúde à luz das políticas de saúde bucal. Metodologicamente estruturou-se em um histórico das políticas de saúde bucal no Brasil desde a década de 1950 aos dias atuais, na vigência da Política Nacional de Saúde Bucal (PNSB, 2004). A abordagem das políticas procedeu-se sob teoria de M. Foucault na obra Arqueologia do Saber (1997). O estudo buscou apreender qual saber operante, (Mendes Gonçalves 1979, 1994) tem organizado a prática neste campo. Utilizou-se de revisão de documentos normativos do SUS e de publicações do período de 2000 a 2012. Procurou-se desvelar, a partir da organização tecnológica (categoria de análise) como a atual política aborda as necessidades de saúde da população e quais ferramentas/instrumentos/tecnologias são oferecidas para o cuidado em saúde bucal. O SUS busca tem buscado a mudança de modelos de atenção e de organização do trabalho que transformem a prática de assistência odontológica (ineficaz, baixa cobertura, monopolista, baixa resolubilidade, mal distribuída geográfica e socialmente), por modelos voltados à promoção da saúde. No entanto, de acordo com levantamento de artigos sobre a condução da atual PNSB no cotidiano dos serviços públicos de saúde, destacou um modus operandi ainda calcado na pragmática odontologia, plena de conflitos e contradições. A abordagem de conceitos como promoção da saúde, integralidade e cuidado em saúde trouxeram a este estudo uma luminosidade, uma clareza ao olhar a prática em saúde bucal. O histórico das Políticas em Saúde Bucal foi fundamental para compor a análise da organização tecnológica e os processos de trabalho, pois realçaram que o fio condutor das práticas tem sido realizado, de forma linear, por abordagens ainda muito centradas no conhecimento biológico do adoecimento. Para que de fato um novo modelo como propõe a PNSB transforme a prática em saúde bucal com novos arranjos tecnológicos no processo de trabalho, outras formas de vínculo e comprometimento devem ser almejadas. É necessário melhorar a formação e a prática profissional com a adição de novos saberes que, por sua vez, irão exigir novos padrões cognitivos e culturais. Há que se qualificar a escuta sobre os padecimentos do paciente, ir além do diagnóstico de sinais e sintomas, apreender as subjetividades produzidas na condição do adoecimento. Talvez estes processos consigam legitimar a saúde bucal como um dos componentes da saúde em uma expressão ampliada: a da qualidade de vida.
This thesis discusses the technological organization of health care in light of the dental health policies. Methodologically structured in a history of oral health policies in Brazil since the 1950s to today, in the presence of the National Policy Oral Health (PNSB, 2004). The approach of the policies proceeded up under the theory of M. Foucault\'s work on the Archaeology of Knowledge (1997). The study aimed to discover which operating knowledge, (Mendes Gonçalves 1979, 1994) has organized practice in this field. We used the review of regulatory documents and publications SUS from 2000 to 2012. We sought to reveal, from the technological organization (analysis category) as the current policy sees the health needs of the population and what tools / instruments / technologies are offered to oral health care. The SUS search has sought to change care models and work organizations that transform the practice of dental assisting (ineffective, low coverage, monopolistic, low resolution, poorly distributed geographically and socially), for models aimed at health promotion. However, according to a survey of articles about the conduction of current PNSB in the routine of public health services, said a modus operandi still supported in the pragmatic dentistry, full of conflicts and contradictions. The approach of concepts such as: health promotion, and integrality health care have brought a luminosity to this study, a clarity look at at the practice in oral health. The historic Policy Oral Health was fundamental to compose the analysis of technological organization and work processes, as highlighted that the guiding line of practices has been performed, linearly, by approaches still very centered on biological knowledge of the illness. For a really new model as proposed by the PNSB turn the practice of oral health with new technological arrangements in the work process, other forms of ties and compromise to be desired. It is necessary to improve training and professional practice by adding new knowledge which will require new cultural and cognitive standards. It must be qualify the listening about the sufferings of the patient, to go beyond of the diagnosis of signs and symptoms, to capture subjectivities produced in the condition of illness. Perhaps these processes are able to legitimize the oral health as a component of health in a larger expression: a quality of life.

In this diploma thesis two steel frame objects of a waste dump in Úholičky are solved. The aim is to resolve the chosen parts of a construction-technological project. The main topics for selected technological phases are transportation, bill of quantities, technological regulations, construction organization’s principles, time schedule, machinery, control and testing plan, work safety and worker’s health protection. These topics are prepared for the technological phases of ground works, fundamental contruction, upper load-bearing structure, construction of the floor and sheathing of the object. O Brasil tem experimentado transformações na composição etária e no perfil epidemiológico da população, com alterações relevantes no quadro de morbi-mortalidade. Diante deste contexto, aumenta o interesse dos estudiosos sobre as conseqüências sociais e econômicas das doenças crônicas não transmissíveis e a busca por novos métodos para analisar o perfil dessas doenças e formas de alocar, com eficiência, os recursos em saúde visando à promoção da saúde da população. Os objetivos deste estudo, de delineamento metodológico, foram traduzir, adaptar culturalmente para o Brasil o instrumento Health and Work Performance Questionnaire (HPQ) e avaliar as propriedades psicométricas da versão brasileira em enfermeiros. O HPQ é um instrumento adotado pela Organização Mundial da Saúde que permite a coleta de informações sobre condições crônicas de saúde e avaliar o impacto destas condições no desempenho do trabalho, pela análise do absenteísmo, presenteísmo e a ocorrência de acidentes de trabalho. O processo de adaptação seguiu o Protocolo de Tradução da Organização Mundial da Saúde. Os dados para a avaliação das propriedades psicométricas foram coletados no período de julho a agosto de 2011, em um hospital público e de ensino do interior do Estado de São Paulo, por meio da aplicação da versão adaptada do HPQ, denominada \"Questionário Saúde e Desempenho no Trabalho\" (HPQ Brasil), em 100 enfermeiros.. As propriedades psicométricas analisadas foram: a validade de face e conteúdo (grupo de especialistas), a confiabilidade pelo Alfa de Cronbach e a estabilidade por teste-reteste. O nível de significância adotado foi de 0,05. Os resultados demonstraram que a amostra estudada foi composta por enfermeiros que possuíam as seguintes características; 89% sujeitos do sexo feminino, com média de idade de 33,9 anos, 51% solteiros e 67% com formação de pós graduação. Na avaliação das propriedades psicométricas, a consistência interna da versão adaptada do HPQ, alfa de cronbach foi de 0,94 para a seção A e 0,86 para a seção B do instrumento. Na análise das concordâncias da estabilidade teste-reteste, as concordâncias foram positivas e estastisticamente significantes. Desta forma, a versão adaptada do HPQ mostrou-se valida e confiável na amostra estudada. O HPQ - Brasil poderá ser usado para acompanhar a prevalência de problemas de saúde entre trabalhadores, as taxas de tratamento destes problemas e os seus efeitos no desempenho no trabalho, além de oferecer as instituições de saúde informações fundamentais para o planejamento de programas de melhoria da saúde e da produtividade dos trabalhadores e de redução dos custos com as Doenças Crônicas Não-Trasmissíveis aspectos que denotam a relevância do estudo ora apresentado.
Brazil has undergone a process of transition with changes in age composition and epidemiological profile of its population which has caused a significant impact on morbidity and mortality. In light of that there is a growing interest among experts in social and economic consequences of chronic noncommunicable diseases (NCDs) and new approaches to assess the profile of these diseases and to effectively allocate resources for health promotion. This study aimed to translate into Brazilian Portuguese and culturally adapt the World Health Organization (WHO) Health and Work Performance Questionnaire (HPQ) and evaluate its psychometric properties among Brazilian nurses. The HPQ was developed by the WHO and can measure the impact of chronic illnesses on work performance through the analysis of absenteeism, presenteeism and work-related injuries. The adaptation process followed the WHO Translation Protocol. The psychometric properties of the Brazilian version of the questionnaire (Health and Work Performance Questionnaire, HPQ-Brazil) were evaluated in a sample of 100 nurses from a public teaching hospital in the state of São Paulo, southeastern Brazil, between July and August 2011. The psychometric properties analyzed included face validity and content validity (expert group), reliability by Cronbach\'s alpha, and test-retest stability. A 5% level of significance was set. The study sample comprised nurses with the following characteristics: 89% were female, mean age 33.9 years; 51% were single and 67% had graduate training. The internal consistency of the adapted version of HPQ using Cronbach\'s alpha was 0.94 for the instrument\'s section A and 0.86 for section B. There were statistically significant positive concordances in the analysis of test-retest stability. Thus, the adapted version of the HPQ proved valid and reliable in the sample studied. The HPQ-Brazil can be used to assess the prevalence of health conditions and treatment rates and their impact on work performance among workers. In addition it can provide input for planning strategies to improve worker\'s health and productivity and reduce NCD-related costs.

The aim of thediploma thesis is to make the detailed building-technological project of microbrewery Vysočina. It is concentrated on the principles of building organization what makes construction facility, the calculation of electric power and water supply, calculation of the costs of operating the construction site and its graphic representation for chosen construction phases. I also make the project of the optimal machine composition. After that I compile the budget and costing of the microbrewery, the time and financial plan and the evaluation of main sources. I technologically solve the phase of upper shell construction made of reinforced pre-cast construction for building object of the microbrewery Vysočina. I apply the other ways of assembly of upper shell construction and show their evaluation in terms of price and time construction. I make the inspection and test plan, environmental aspects, the source register of working risk, heat-engineering review of building case and fire safety solutions.

Abstract:
Statement of Problem: Despite significant advancements in occupational health and safety in recent decades, injury rates in commercial construction remain high. New programs that address the complexity of the construction work environment are needed to keep workers healthy and safe. Methods: The first step of this dissertation was to explore associations between organizational programs and policies, as measured by a Contractor Safety Assessment Program (CSAP) score, and worker safety climate scores. Next, a safety communication and recognition program was developed and piloted. It was evaluated through a mixed methods approach in a randomized controlled trial. Primary outcome measures included safety climate, awareness, communication, and teambuilding. Additionally, the dynamic nature of the construction site was quantified through an analysis of the determinants of length of stay of construction workers on the worksite. Results: Correlations between CSAP scores and safety climate scores were weak at best, thus highlighting a gap in communication between management and workers. The B-SAFE program, a safety communication and recognition program was developed to meet this gap. It used data from safety inspection scores to provide feedback to workers on hazards and controls, and provided a reward when the site met a pre-determined safety inspection threshold (a measure that was fair, consistent, attainable and fair). In the final program design, the whole site was treated as the unit of analysis. B-SAFE led to many positive changes, including a statistically significant increase in safety climate scores of 2.29 points (p-value=0.012), when adjusting for time-varying parameters and worker characteristics. Workers at the B-SAFE sites noted increased levels of safety awareness, communication, and teamwork, when compared to control sites. The composition of workers on-site at any given month changed by approximately 50%, and the length of stay on-site was associated with race/ethnicity, union status, title, trade, and musculoskeletal pain (p-values<0.05). Conclusions: The construction work environment is dynamic, with over half of the population on-site changing each month. This makes applying and evaluating traditional worksite based interventions challenging. Interventions like B-SAFE that are developed to address the complexities can have a positive impact on site safety measures.

Abstract:
The research study focuses on how social work Approved Mental Health Professionals (AMHPs) use the law in practice. AMHPs in England and Wales have statutory powers under the Mental Health Act 1983 (MHA) to detain people in hospital for assessment and/or treatment. The stakes in this area of law and social work are high: practitioners deal with important issues concerning individual liberty that have profound implications in relation to the power of the state to intervene in the lives of citizens, where notions of autonomy, protection, coercion and care sit in tension. The study explores the relationship between law and social work practice by interpreting meanings contained in case stories told by social work AMHPs about recent Mental Health Act assessments that they undertook. Eleven social work AMHPs, purposively selected from three different local authorities in England, participated in the study, which used qualitative in-depth interviews to collect data about using the law in circumstances where compulsory admission to hospital was a possibility. The use of case stories encouraged participants to provide a rich description of events as they unfolded over time. The data were analysed using Framework analysis (Ritchie and Spencer 1994). Computer Assisted Qualitative Data Analysis in the form of NVIVO was utilized to manage the data, and to support data analysis. Five themes are presented in the findings chapter: understanding the referral situation; understanding the individual; understanding the situation causing concern; community versus containment, and relationships and resources. The study contributes to knowledge by illuminating how the use of law in practice is an inherently socio-relational undertaking, involving embodied practice. Bourdieu's (1977) concept of habitus is used to make sense of participants' accounts of the action that unfolds when they use the law. A further contribution is made to knowledge on legal literacy in social work, where there is little empirical research focusing on how social workers use the law, and still less on how mental health social workers use the law to consider compulsory powers under mental health legislation. The organisational factors impacting on how participants relate to the law are outlined and discussed drawing on legal consciousness theory (Ewick and Sibley 1998; Sibley 2005), together with an account of how participants adapt to this, drawing on street level bureaucracy (Lipsky 1990). The thesis explores the distinction in practice between medical and social perspectives occupied by AMHPs when they use the law in circumstances where compulsory admission to psychiatric hospital is a possibility. The study findings suggest that AMHPs' perspectives are holistic and social and can be understood as occupying a socio-medical-juridical perspective. The most important factor in the decision to use compulsory powers in mental health law to detain a person involves the AMHP taking a wide perspective in terms of their understanding of the individual that is relational to the understanding of others, and understanding the person in their environment in relation to how they relate to others. The thesis outlines that the social and family situation of the person assessed, combined with views of others, and particularly the impact of risk on others, is the most influential factor in the decision to detain. This leads to the further argument that notwithstanding a holistic and social perspective, this does not necessarily lead to less coercive interventions. Medical and social perspectives thus often lead to the same conclusions in relation to decisions to use the law to detain.

Abstract:
Children in foster care are at high risk of experiencing mental health problems and tackling this issue is a key priority. Previous research suggests that the transition from primary to secondary school can be particularly challenging, as well-being declines and mental health problems increase in early adolescence. However, there is insufficient understanding of variations in the well-being and mental health of this group of children, and particularly the role played by their social interactions, relationships, and psychological attributes. This thesis includes three papers reporting on a programme of empirical research conducted to address this gap in knowledge and better understand the risk and protective factors, particularly in the peer context, for changes in mental health and well-being. The first paper focuses on current provision and reports the findings from a national survey of Virtual Schools that support the education of children in care. The second paper presents the findings of a longitudinal study with children not in care (aged 10-13 years), to test our conceptual model in the general population. This demonstrated that peer factors predict changes in mental health problems and well-being over and above parental and other adult support. The third paper presents findings from a longitudinal study of children in foster care (aged 10-14 years), to test these key pathways in our focus population. This revealed a pattern of differentiated links from peer and adult support to mental health and well-being, and identified self-efficacy as a key longitudinal predictor of change, especially when moderated by peer relationship quality. The thesis demonstrates the importance of supportive relationships with both adults and peers for the mental health and well-being of children in care. This has important implications for future work where social activities and relationship quality with peers should be considered as potential protective factors, especially in school settings.

Abstract:
Advances in medical technology have altered the need for certain types of surgery to be performed in traditional inpatient hospital settings. Less invasive surgical procedures allow a growing number of medical treatments to take place on an outpatient basis. Hospitals face growing competition from ambulatory surgery centers (ASCs). The competitive threats posed by ASCs are important, given that inpatient surgery has been the cornerstone of hospital services for over a century. Additional research is needed to understand how surgical volume shifts between and within acute care general hospitals (ACGHs) and ASCs. This study investigates how medical technology within the hospital industry is changing medical services delivery. The main purposes of this study are to (1) test Clayton M. Christensen’s theory of disruptive innovation in health care, and (2) examine the effects of disruptive innovation on appendectomy, cholecystectomy, and bariatric surgery (ACBS) utilization. Disruptive innovation theory contends that advanced technology combined with innovative business models—located outside of traditional product markets or delivery systems—will produce simplified, quality products and services at lower costs with broader accessibility. Consequently, new markets will emerge, and conventional industry leaders will experience a loss of market share to “non-traditional” new entrants into the marketplace. The underlying assumption of this work is that ASCs (innovative business models) have adopted laparoscopy (innovative technology) and their unification has initiated disruptive innovation within the hospital industry. The disruptive effects have spawned shifts in surgical volumes from open to laparoscopic procedures, from inpatient to ambulatory settings, and from hospitals to ASCs. The research hypothesizes that: (1) there will be larger increases in the percentage of laparoscopic ACBS performed than open ACBS procedures; (2) ambulatory ACBS will experience larger percent increases than inpatient ACBS procedures; and (3) ASCs will experience larger percent increases than ACGHs. The study tracks the utilization of open, laparoscopic, inpatient and ambulatory ACBS. The research questions that guide the inquiry are: 1. How has ACBS utilization changed over this time? 2. Do ACGHs and ASCs differ in the utilization of ACBS? 3. How do states differ in the utilization of ACBS? 4. Do study findings support disruptive innovation theory in the hospital industry? The quantitative study employs a panel design using hospital discharge data from 2004 and 2009. The unit of analysis is the facility. The sampling frame is comprised of ACGHs and ASCs in Florida and Wisconsin. The study employs exploratory and confirmatory data analysis. This work finds that disruptive innovation theory is an effective model for assessing the hospital industry. The model provides a useful framework for analyzing the interplay between ACGHs and ASCs. While study findings did not support the stated hypotheses, the impact of government interventions into the competitive marketplace supports the claims of disruptive innovation theory. Regulations that intervened in the hospital industry facilitated interactions between ASCs and ACGHs, reducing the number of ASCs performing ACBS and altering the trajectory of ACBS volume by shifting surgeries from ASCs to ACGHs.

The phenomenon under study was organizational change. A component of this research was also to explore the possibility of change fatigue in nurses’ discourse. Change is inevitable, and increasingly rapid and continuous in health care as organizations strive to adapt, improve and innovate in response to external pressures. These pressures challenge hospitals to strive for patient safety, quality assurance and provision of exceptional family centred care. Attending to these pressures require time, energy and money. Rapid and continuous change creates a push/pull relationship between innovation and budget. New technologies require extra resources however, simultaneous restructuring and optimization efforts see hospitals decreasing available resources. This creates a challenging workplace for nurses who must engage in organizational change activities with limited resources. Organizational change challenges health care providers in a variety of ways because it restructures how and when patient care delivery is provided, changing ways in which nurses must carry out their work. Little research has been done regarding the impact of rapid and continuous organizational change for frontline health care providers, most notably, nurses. In this study a critical hermeneutic design was applied. Guided by the theoretical framework of critical management studies, the researcher explored concepts of organizational change, experience of change, change fatigue, and power and voice. The setting was an urban pediatric teaching hospital located in eastern Ontario. The researcher sought breadth, depth, complexity and richness of data in understanding the experience of organizational change, which supported a decision to seek a sample size of ten to fifteen participants. Thick description commenced at fourteen participants. Face to face interviews were conducted using open-ended questions to understand nurses’ experiences of change. Brown and Gilligan’s Voice-centred relational method of data analysis was used – a multi-levelled analysis exploring the concept of voice in relation to self, other, culture, society and history. Rapid and continuous organizational change in the workplace profoundly impacted nurses’ work, their relationships to the self, other, culture, society and history. Nurses recognized that many change initiatives reflected an ideological shift in health care that supported a culture of service, whilst sacrificing a culture of care. A culture of service prioritized cost-savings and efficiency, which saw nurses lose the time and resources required to provide quality, safe care. Nurses felt morally responsible to uphold a culture of care, which proved challenging, and at times unobtainable. The inability to provide quality, safe care resulted in a multitude of negative emotional repercussions, which fostered moral distress. Nurses exhibited elements of change fatigue, further contributing to feelings of voicelessness and powerlessness within their workplace. Organizational change must be re-conceptualized in ways that ensure change initiatives uphold institutional integrity and better support the provision of morally authentic nursing practice. Health care organizations should place nurses at the forefront of planning, implementation and evaluation of change initiatives in order to alleviate the many negative experiences of organizational change noted in this study.

This study examines the science/policy interactions in global health science and technology governance. It focuses on the institutional design of organizations that sit at the interface of science and policy, conceptualizing them as Boundary Organizations (BOs). The analysis considers how the institutional design of BOs affect boundary-work. The study examines two case studies, UNESCO’s International Bioethics Committee and the WHO’s Strategic Advisory Group of Experts on Immunization. The study examines the ways in which boundary-work is carried out and finds that the concept of a BO demonstrates an institutionalization of science/policy interactions and the analysis of these two cases show that there are different ways that boundary-work is practiced as a function of the design of BOs.

Organisationen har en betydande roll i medarbetarens livspussel. Det finns mycket att vinna för arbetsgivaren genom att hantera problem och finna lösningar som passar den anställde. Studier av hälsofrämjande interventioner på arbetsplatsen visar att bäst resultat uppnås om interventionen riktar sig samtidigt till både individen och organisationen. Syftet med studien var att beskriva kommunanställda chefers uppfattningar av hur en interventionsstudie, som chefernas medarbetare deltagit i, påverkat arbetsplatsen. Studien syftade även till att söka förståelse för chefernas vidare tankar kring hälsofrämjande arbete på organisationsnivå utifrån ett perspektiv där hela vardagen inkluderas. Metoden som användes vid datainsamlingen var individuella intervjuer. Det genomfördes sex intervjuer med chefer inom socialförvaltningen i en kommun. Resultatet av studien visade att cheferna var positivt inställda till interventionsstudien, men cheferna märkte inte någon förändring på arbetsplatsen eller på deltagande medarbetare. De ansåg att chefen var ytterst ansvarig för hälsa på arbetsplatsen och individen ytterst ansvarig för sin personliga hälsa. Slutsatsen från studien visade att chefens hälsosyn påverkade i vilken utsträckning organisationen kunde tillgodogöra sig kunskap från hälsointerventionen. Chefens syn på vilka faktorer som innefattas i arbetshälsa kan genom kompetensutveckling breddas och därmed skapa bättre förutsättningar för en mer hälsosam arbetsmiljö. Litteraturgenomgången pekar på att en gemensam grundsyn i organisationen och strävan mot en balans i vardagen för medarbetaren är viktigt i det hälsofrämjande arbetet på arbetsplatsen. En positiv effekt av en intervention som fokuserar på livspusslet är att det kan leda till mer hållbar personal som stannar längre, vilket kan bidra till att organisationen blir mer konkurrenskraftig och attraherar ny personal.
The organization has a significant role in an employee’s work-life balance. There is much to gain for the employer by addressing problems and finding solutions that fit the employee. Studies of health promotion interventions at the workplace show that the best results are achieved if the intervention focuses simultaneously on both the individual and the organization. The aim of the study was to describe municipal employed managerial perceptions of an intervention study, that managers employees participated in, affected the workplace. The study also aimed to seek understanding of managers further thoughts on health promotion at an organizational level from a perspective where everyday life was included. The method used for data gathering was six individual interviews with managers in the social department in a municipality. The results of the study showed that managers were in favor of the intervention study, but the managers did not notice any change at the workplace or on participating employees. They thought that the manager was responsible for health at the workplace and that individuals were responsible for their own health. The conclusion from our study showed that managers attitude towards work-related health affects to which extent the organization can assimilate knowledge from a health intervention. The managers opinion of which factors are included in work-related health can be broadened through education and create better conditions for a healthier work environment. The literature review suggests that a shared ethos of the organization and the pursuit of work-life balance for employees is an important step in health promotion at the workplace. A positive effect of work-life balance interventions are more sustainable staff who stay longer, which makes the organization become more competitive and attractive to new staff.

This research study investigates the concept of workplace information literacy (IL) theoretically and empirically, focusing on the connections between information literacy and knowledge management (KM). This dissertation examines the relevance and applicability of current IL frameworks in a workplace environment by means of a review of the literature, a review of NHS Scotland documentation on its KM initiatives, and a phenomenographic study undertaken with frontline staff at NHS24, a nurse-led, 24/7 service of NHS Scotland that provides over-the-phone consultation and health information. For that study, a working definition of IL as ‘effective information use’ was employed. The concept of information literacy has been developed mainly within librarianship, researched mainly within educational contexts and focused on individual competence in information use. While its application to workplace environments has been assumed, comparatively little research has been done into workplace situations. On the other hand, the concept of knowledge management is directed at a wider organizational level. However, while there is a clear focus in the KM literature on the value of information and its importance for organizations, little attention has been paid to the theoretical and empirical developments of Library and Information Science (LIS) relative to information behaviour and effective information use. The findings of this research identified limitations in the current IL frameworks, notably the lack of consideration for people’s exchanges of knowledge and information and of the social sense making that influences information interpretation and application. The findings endorse views of learning and information use grounded in socio-constructive perspectives and a consideration of context as situated practice. The conclusions suggest the need for more collaboration between studies of IL and information behaviour, and for LIS research to focus more on workplace studies and knowledge management.

A pesquisa, que fundamentou este estudo, foi desenvolvida através da psicodinâmica do trabalho com a participação de trabalhadores convidados aos encontros coletivos, para expressão, análise e interpretação dos efeitos da organização, dos processos e das relações estabelecidas no trabalho em relação à vivência do prazer/sofrimento. Os pressupostos da metodologia adotada possibilitaram a ampliação dos processos comunicacionais dos coletivos, mediante a circulação da palavra e a compreensão e interpretação da realidade desses trabalhadores, proporcionando uma análise intencional e mais precisa, das condições e dos processos de trabalho, para além de queixas, buscando identificar os processos de produção de saúde em seu ambiente laboral com todas as suas peculiaridades. Constata-se que a realidade do trabalho, em particular, o da enfermagem, determina modos de subjetivação, com suas peculiaridades, à medida em que o trabalhador tenta manter sua saúde mental em meio à complexidade de relações e ações específicas no desempenho de suas atribuições. Os trabalhadores da UE do HCR ao enfrentarem o contexto complexo para o exercício de suas atividades assistenciais, envolvem-se nas demandas por competências profissionais próprias aos cuidados dos usuários com traumas, em situação de emergência, com expectativa de procedimentos de qualidade e resolutividade, e também, submentem-se às condições materiais, institucionais e/ou organizacionais nem sempre favoráveis. Os trabalhadores da UE do HCR ao enfrentarem o contexto complexo para o exercício de suas atividades assistenciais, envolvem-se nas demandas por competências profissionais próprias aos cuidados dos usuários com traumas, em situação de emergência, com expectativa de procedimentos de qualidade e resolutividade, e também, submentem-se às condições materiais, institucionais e/ou organizacionais nem sempre favoráveis. Além da demanda do domínio de tecnologias de atuação nas equipes e atendimento dos usuários, aos trabalhadores, também é exigido novos modos de participação e condução dos processos num tempo/espaço institucional em que convivem modelos contraditórios de produção da saúde, o antigo e hegemônico, hospitalocêntrico, médico centrado e o proposto pelas diretrizes do SUS, reforçado pela PNH. O conflito entre esse modo de organização do trabalho e a subjetividade do trabalhador é gerador de sofrimento psíquico. Neste contexto é que, individual e em equipe, os trabalhadores refletem os efeitos ou ressentem os impactos sobre a sua saúde mental, provocando dor / sofrimento / doença. Ao identificar os fatores promotores de sofrimento, os trabalhadores constroem estratégias coletivas de defesa, eliminando, camuflando ou mesmo, minimizando-os. Tanto enfrentam as adversidades do seu trabalho, opondo-se, quanto fazem subsistir espaços de produção de prazer/saúde. A necessidade de se discorrer sobre saúde/doença - prazer/sofrimento vem assumindo diferentes conotações, acompanhadas e/ou regidas pelas configurações sociais, políticas e econômicas que dão sentido às diversas práticas laborais, desde criativas, engenhosas, prazerosas, quanto sofridas, alienantes, estressantes e adoecedoras. Todas, por sua vez, inscrevem-se na cultura e caracterizam, de alguma forma, o mal estar contemporâneo.
This thesis examines dynamics of pleasure/suffering of nursing workers from the Emergency Public Hospital Cristo Redentor of Porto Alegre. The search was developed through psychodynamic of work with participation of invited workers to the group meetings- to expression, analysis and interpretation of an organizing proposal, processes and established relationships at work in relation of experience of pleasure/suffering. That methodological assumption makes able the magnification of communicational processes of groups, under the circulation of words, understanding and real interpretation of their job, providing an analysis more specific and intentional to even make complaints of conditions and processes of work looking for identify the production health processes at their workplace with all peculiarities. Notes the reality of work, this case the nursing, determines subjective modes. The extent that worker tries to keep their mental health just on the middle of complexity of relations and specific actions in performing their duties. The workers of UE (Emergency Hospital) and HCR (Hospital Cristo Redentor) when they face a complex context to exercise their welfare activities getting involved in demands of their own professional competence being watched by trauma users, most of them in hard situation and hoping for good and quality assist, also, the workers has to face bad conditions like materials, institutional and organizational. Besides the demand from the field of performance technology teams and service to users and workers, also is required new ways of participation and conduction of proceedings at a time/space institutional which contradictory models of health production coexist, the ancient and hegemonic, „‟hospital centric‟‟, means a doctor focused and proposed by SUS (Unique System of Health) guidelines and supported by PNH. The conflict between the way of organization of work and subjectivity of worker is the cause of psychic suffering. At these same contexts the individual or group could consider effect about their own mental health, causing pain/suffering/illness. To identify what is causing such a pain, the worker has to build strategies and advocacy groups, deleting, hiding or minimizing that. They stand up to tribulation at their job being against even of pleasure/health production. The need to discourse about health/illness – pleasure/suffering has taking over different connotations ruled by social configurations, political and economic, which gives sense to many of workplace practical. Since it is such creative, ingenious, pleasurable, such it is at the same time alienating, stressful and sicken. All of them are introduced on culture and characterized, somehow, the contemporary malaise.

Abstract:

Ongoing structural and financial changes in the health care sector have resulted in increased risks for ethical dilemmas and moral distress. It is purported that increased ethical competence will help staff manage ethical dilemmas and hence decrease moral distress. To enhance ethical competence several approaches may be used – theoretical education, and methods focusing on reflection and decision-making abilities.

Ethics rounds are a widespread systematic method hypothesized to improve ethical competence, nurture a reflective climate, and help in ethical decision-making. Despite its popularity, its effects on moral distress have hitherto never been evaluated in a controlled study.

The purpose of this thesis was to evaluate the impact of an intervention, including ethics rounds; the hypothesis being that the intervention would decrease perceived moral distress. An additional aim was exploring the concept of moral distress in various health care establishments, including pharmacies.

Focus groups were conducted to explore the concept of moral distress. To evaluate the intervention a scale assessing staff-perceived moral distress was designed, validated, and implemented.

Results showed that moral distress is evident in diverse health care settings. Some factors associated with this were lack of resources, conflicts of interest, and rules that are incompatible with practice. An expanded definition of moral distress was presented.

The training program was much appreciated by participants. However, no significant effects on perceived moral distress were found. Reasons could be that the intervention was too short or otherwise ineffective, there is no association between ethical competence and moral distress, the assessment scale was not sensitive enough, or management was not sufficiently involved.

There is a need to further refine the various aspects of ethical dilemmas in clinical settings, and to evaluate the most efficient means to enhance skills for dealing with ethical dilemmas, for the benefit of staff, patients, institutions, and society.

Įvertinti terminalinės būklės pacientus prižiūrinčių slaugytojų emocinius sunkumus bei jų kontrolės galimybes darbe. Tyrimo uždaviniai. 1. Išsiaiškinti terminalin��s būklės pacientus prižiūrinčių slaugytojų profesinius motyvus ir lūkesčius. 2. Išanalizuoti su darbu susijusius slaugytojų emocinius sunkumus bei jų įveikos būdus. 3. Atrenkant skirtingų tipų institucijose dirbančius respondentus siekta atspindėti galimą požiūrių įvairovę. Duomenys buvo renkami 2008 m. sausio – vasario mėnesiais. Rezultatai. Tyrimas parodė, kad skirtingose terminalinės būklės pacientų slaugos institucijose dirbančios slaugytojos savo kasdieninėje veikloje patiria panašius sunkumus, neigiamai veikiančius jų savijautą ir sveikatą. While improving information technologies, automation, mechanization, rates of production and competition accelerates, information flow grows, labouring man‘s responsibility increases. All this grows intellectual, nervous and emotional tension and raises professional stress. One of the most difficult outcomes of enduring stress for tenders is health disorder. Objective of research. Evaluate tenders‘ nursing patients in terminal state emotional difficulties and their control possibilities in work. Goals of research. 1. Ascertain tenders‘ nursing patients in terminal state professional motives and suspenses. 2. Traverse emotional difficulties connected with tenders‘ work and their defeat modes. 3. Evaluate meaning of work organization for tenders‘ feelings. Methodology of research. O estudo parte do reconhecimento da crescente disputa por materiais recicláveis no atual estágio de alargamento das formas desregulamentadas, degradadas e precarizadas de trabalho, condicionadas pelo imperativo da sobrevivência. Situa o trabalho intensivo da pré-reciclagem no circuito inferior da economia urbana e discute o incalculável custo humano deste trabalho que alimenta as indústrias recicladoras e traz enaltecidos benefícios ambientais à coletividade. Partindo do entrecruzamento da Economia Solidária e do campo de Saúde do Trabalhador, procurou-se identificar as potencialidades e limites da organização do trabalho cotidiano pelos trabalhadores da Cooperativa de Reciclagem de Matéria-Prima de Embu (Coopermape) e suas repercussões à saúde dos trabalhadores. O trabalho de campo permitiu reconhecer a complexidade do contínuo replanejamento do trabalho cotidiano e as dificuldades de construção da autogestão para compatibilizar a logística e o faturamento capazes de assegurar as expectativas de retirada mensal dos cooperados e, ao mesmo tempo, construir um processo horizontal de tomada de decisão que considere também a atenção às condições de trabalho e saúde. Na Coopermape, esse desafio é potencializado ainda por limitações impostas pelas constantes oscilações e condicionalidades do mercado da reciclagem e pelas pressões sofridas pela prefeitura de Embu. Num ambiente em que estão em jogo pesadas forças e interesses divergentes aos da cooperativa, o aprendizado do grupo se faz \"a duras penas\". Apesar da configuração exógena do processo de trabalho, a experiência de campo mostrou que existe uma margem residual, mas importante, de decisão dos cooperados sobre o processo cotidiano de trabalho. As experiências de reconfiguração de algumas etapas de trabalho e atitudes preventivas identificadas indicam que em algumas situações os cooperados conseguem compatibilizar a agilidade do processo e a minimização da sobrecarga e desgaste os trabalhadores. Ainda que a maioria destas pequenas mudanças não tenha sido promovida com a intencionalidade deliberada e explícita de poupar a saúde dos trabalhadores, elas permitem alimentar esperanças de reorganização do trabalho a favor dos trabalhadores.
The study begins by recognizing the increasing struggle for recycling material at the actual period of extension of unregulated, decreased and precarious work occupations, determinated by mandatory survival. Associates the intensive work made before the recycling process to low urban economy circuit and discusses the incommensurable human price of the this work that feeds recycling industry and brings exalt environment profities to society. Leaving from the crossing of Solidarity Economy and Workers\' Health field, the objective was to identify the possibilities and limits of work organization conducted by Cooperativa de Reciclagem de Matéria-Prima de Embu (Coopermape)\' workers and its effects to workers\' health. The empiric work allowed recognizing the complexity of day-by-day continuous replanning of work and the difficulties to build workers\' self-management. These difficulties are connected to the need to consider both the logistics and invoicing able to assure the workers expected rewarding and the construction of an even decision making process that also considers the regard to work and health condition. At Coopermape, this challenge is even bigger because of the limits imposed by the constant variation and conditionality of recycling trade and by Embu\'s city hall pressure. On a surrounding involving heavy power and disagreeing interests in relation to the ones of the cooperative, the learning takes a high cost. Although the exogenous conformation of work process, the empiric experience showed that there is a remaining, but important, border of decision of the day-by-day work process by the workers. The experiences of changing conformation at some work stages and provident attitudes demonstrates that in some situations the cooperative\' workers can consider the process agility and the reducing of workload and workers consuming. Though the majority of these little changes were not encouraged with the premeditated and explicit intention of guarding workers\' health, they allow feeding hopes of work rearrangement in favor of workers.

Darbo tikslas - įvertinti Šiaulių miesto ir Joniškio rajono sveikatos priežiūros darbuotojų nuomonę apie stacionarinių sveikatos priežiūros paslaugų organizavimą ir darbo aplinką. Darbo uždaviniai: 1. Nustatyti gydytojų ir slaugytojų nuomonę apie stacionarinių sveikatos priežiūros įstaigų darbo aplinkos ypatumus. 2. Ištirti ir palyginti sveikatos priežiūros darbuotojų nuomonę apie sveikatos priežiūros įstaigų darbo aplinką miesto ir rajono ligoninėse. 3. Įvertinti stacionarinių sveikatos priežiūros paslaugų organizavimo ypatumus ir problemas sveikatos priežiūros įstaigose. Tyrimo metodika: Vienmomentinis kiekybinis tyrimas atliktas 2012 m. kovo-rugsėjo mėnesiais. Tyrime buvo apklausti 497 respondentai: 111 i6 Joniškio ir 386 iš Šiaulių (atsako dažnis – 95,6 proc.). Tyrimui atlikti buvo panaudotas originalus klausimynas. Statistinė analizė buvo atlikta taikant SPSS programinės įrangos paketą. Tyrimo rezultatai: Vertinant respondentų darbo sąlygas nustatyta, kad daugiau nei pusė (53,6 proc.) savo darbo sąlygas vertina vidutiniškai, 39,0 proc. labai gerai ir gerai ir 7,4 proc. respondentų labai blogai ir blogai. Dauguma (91,3 proc.) respondentų savo darbo sąlygas apibūdino kaip lydimas emocinės/psichologinės įtampos, 61,4 proc. darbas lydimas fizinės įtampos, pusė - darbo priemonių stygių bei darbą su dezinfekuojančiomis medžiagomis, trečdalis - pažymėjo darbą ankštose patalpose, ketvirtadalis – darbą blogai vėdinamose patalpose, o 18 proc. respondentų paminėjo asmens apsaugos... The aim - to evaluate the Šiauliai city and Joniškis district hospitals health care workers opinion on inpatient health care organization and the work environment. The objectives: 1. To determine the opinion of doctors and nurses about the inpatient health care work environment. 2. To investigate and compare the health care staff views on the health care work environment for city and district hospitals. 3. To assess and evaluate of inpatient health care organization characteristics and problems of health care facilities. Methods: A cross-sectional quantitative study carried out in 2012 March-September. The study surveyed 497 respondents: 111 from Joniškis and 386 from Šiauliai (response rate - 95.6%). The original survey questionnaire was used. Statistical analysis was performed using SPSS software package. The results. Evaluation of the working conditions of the respondents found that more than half (53.6%) their working conditions the average, 39.0% very well and good, and 7.4% very bad and bad. Abstract:
Studies have shown that emergency mental health trauma (EMHT) services can significantly reduce the long-term effects of trauma after a disaster. However, rural municipalities may find they do not have the capacity to create such a service, or may not realize that their disaster planning includes no provision for emergency mental health care. Such was the case in a rural island community in the state of Washington, where, in 2014, several residents initiated a discussion that helped to identify the community's lack of EMHT services. This project, framed by action research and based on collaboration theory, sought to advance the potential for the community's 21 resident social workers to address this issue collaboratively. Accordingly, the project's research question asked how social workers on south Whidbey Island perceived the issue of a rural EMHT service in their community. Data consisted of responses from 8 participants who completed mailed questionnaires and participated in brief telephone interviews. Descriptive coding analysis of the data confirmed a nearly universal lack of knowledge about an EMHT service, a clear perception of the need for such a service, and a unanimous commitment from the respondents to participate in addressing this problem. Such collaborative activity is expected to have a positive impact on the micro, mezzo, and macro levels of social work practice in south Whidbey, as well as on the community itself, not only in spearheading a dialogue about EMHT but also in activating a group of social workers who had no prior association.

This research identifies, characterizes and analyses the dimensions of work, health and environment of agriculturist women from Sagrado Coração de Jesus/Vila do Engenho community located in the city of Itacoatiara, Amazonas. The goal is to identify different places, processes and organizations regarding the work of women, in its multiple activities, that are incorporated, in the day-to-day help, and also to identify health risks referenced by the worker in the day-to-day work. This qualitative research through the theorical criticism perspective in the sociohistorical approach, dialogues with the psychodynamics of work theories. These theories opt for the speech and listening as privileged access to subjective experiences. In order to understand the work of women in the agriculture, it is important to trace an access between its geographical, psychosocial and health aspects. This research uses the Nucleus of Meaning Analysis (NMA), inspired and adapted from the Category-based Content Analysis, that privileges the words, the subjective experiences analysis and the studies of their meanings. The results show that in Vila do Engenho, histories are connected to the cooperative dynamic with the protagonism of women at work experiences, in the organization and participation in the community. Contradictions and current conditions in which they live, through their own reflection as people, and through the action of the subject that tends to transform material conditions that generated such contradictions, constitute the work dynamic in which they live. As results, it can be confirmed that through their histories, work is determining in the health/disease process of the agriculturist women, in which working is to make a work-help relation. It is possible to observe that for these women, it is not so clear of which function performed daily can be identified as help and work. The role of the woman as a mere helper disappears when the relations of work become complex. On the other hand, when these relations are less complex, the idea of helping the husband is better considered. It is revealed an invisible work that extrapolates "housewifing". There are situations and places that increase the level of exploration of this work force, house - association - agro-industry - plantation, associated by the woman as much as their proper duties. In the agriculture work, it can be observed that the women acquire pain due to bad posture, physical effort and to mechanical pression on determined body parts, like the lower and the upper limbs, heat and solar radiation exposure for long periods of time and agrochemicals exposure, and also the ones related to organization and psychosocial factors of each job, associated to common work overload in all places of Vila. The public space permits the speech, that can be heard and demands actions, favoring women to question the place they are given, and the strenghtening of the worker identity, inserted in the reproductive and productive spaces of agriculture in the rural environment of the Amazon.
A pesquisa identifica, caracteriza e analisa as dimensões de trabalho, ambiente e saúde de mulheres agricultoras da comunidade Sagrado Coração de Jesus/Vila do Engenho, no Município de Itacoatiara, AM. Tem como objetivo identificar os diferentes lugares, processo e organização do trabalho da mulher, em suas múltiplas atividades, incorporadas, muitas vezes, no cotidiano da ajuda, assim como agravos e riscos à saúde referenciados pela trabalhadora no cotidiano do trabalho. A pesquisa do tipo qualitativa na perspectiva teórico crítico de abordagem sócio-histórico, faz interlocução com o aporte teórico da psicodinâmica do trabalho, que opta pelo espaço da fala e da escuta como acesso privilegiado das vivências subjetivas. A fim de compreender o trabalho da mulher na agricultura, traça-se um acesso entre os aspectos geográficos, psicossociais e de saúde. Para o alcance das perspectivas de compreensão e interpretação, este estudo trabalha com a Análise dos Núcleos de Sentido (ANS), inspirada e adaptada na Análise de Conteúdo Categorial, que privilegia a palavra, a análise das vivências subjetivas e a busca do seu significado. Os resultados revelam que na Vila do Engenho, as histórias são interligadas à dinâmica da cooperativa, com protagonismo das mulheres no cenário das trajetórias de trabalho, nos processos de organização e de participação na comunidade, constitui a dinâmica de vida e trabalho onde estas vivem as contradições e as condições não superadas que a geram de um modo, por meio da reflexão delas enquanto sujeitos, e, de outro, pela ação do sujeito que busca transformar as condições materiais que geraram tais contradições. Como resultado confirma-se que no trançado de histórias, o trabalho é determinante no processo saúde/doença das trabalhadoras agrícolas, onde trabalhar é fazer relação do trabalho-ajuda. Observa-se que para elas, há falta de clareza acerca de quais as funções desempenhadas no seu cotidiano que podem ser identificadas por si e pelo outro como ajuda e trabalho. Tem-se que quando relações de trabalho tornam-se complexas, desaparece o papel da mulher como ajudante. Por outro lado, quando essas relações são menos complexas, aparece mais claramente a ideia da ajuda ao marido, da ajuda à renda da família. Frente ao trabalho prescrito nas híbridas atividades e lugares, revela-se uma sobrecarga, um trabalho invisível que extrapola o "quintal da casa". Existem cotidianos e lugares que aumentam o grau de exploração dessa força de trabalho, potencializando agravos, fatores de riscos, e acidentes invisíveis, na engrenagem do trabalho, casa – associação – agroindústria – plantio, tão associados pela trabalhadora como a parte que lhes cabe. No trabalho na agricultura, observa-se que as trabalhadoras evidenciam dores decorrentes de posturas inadequadas por tempo prolongado, ao esforço físico, e à pressão mecânica sobre determinados segmentos do corpo, como os membros superiores e inferiores, exposição às radiações solares e ao calor por longos períodos e exposição a agrotóxico, assim como relacionados aos fatores organizacionais e fatores psicossociais de cada trabalho, associado a sobrecarga de trabalho comum em todos os lugares de trabalho na Vila. O espaço público possibilita a fala, que se faz escuta e reflete ação, favorecendo a mulher o questionamento do lugar que lhe é designado e o fortalecimento da identidade de trabalhadora, inserida nos espaços reprodutivo e produtivo da agricultura no ambiente rural na Amazônia.

A problematizaÃÃo surgiu diante de relatos e a percepÃÃo de uma possÃvel relaÃÃo entre casos de sofrimento psÃquico, diante do cotidiano de trabalho desses profissionais. Para o inÃcio da pesquisa foram realizados estudos teÃricos dos temas trabalho, sofrimento psÃquico, Psicopatologia e PsicodinÃmica, OrganizaÃÃes familiares e lideranÃa. Partiu-se de pressupostos que apontavam as caracterÃsticas das organizaÃÃes familiares, com suas prÃticas de gestÃo peculiares, como desencadeadoras de sofrimento psÃquico e que mesmo diante de estratÃgias defensivas levariam a afastamentos e somatizaÃÃes. A pesquisa teve metodologia qualitativa, de natureza descritiva e exploratÃria, buscando conhecer a realidade da organizaÃÃo familiar e investigar a relaÃÃo entre as vivÃncias de prazer e sofrimento no trabalho dos lÃderes diante da gestÃo de uma organizaÃÃo familiar, dando-lhes oportunidade de expor o conteÃdo do trabalho, as percepÃÃes que atravessam o cotidiano vivenciado e a relaÃÃo prazer e sofrimento com o trabalho realizado. Foram realizadas entrevistas e grupos focais, com objetivo de levantar dados que pudessem identificar a existÃncia e o tipo de manifestaÃÃo de sofrimento psÃquico dos lÃderes; descrever as caracterÃsticas da gestÃo; analisar a relaÃÃo dessa gestÃo com a organizaÃÃo familiar e relacionar o trabalho dos lÃderes com seu sofrimento psÃquico, bem como suas estratÃgias de defesa para lidar com esse sofrimento. This study deals with issues related to work and mental health relating them to everyday leaders in family organizations. The questioning came before reports and the perception of a possible link between cases of psychological distress before the daily work of these professionals. Top of research were carried out theoretical studies labor issues, psychological distress, psychopathology and psychodynamics, family organizations and leadership. It started from assumptions that pointed the features of family organizations, with their peculiar management practices, such as psychological distress and triggering even before defensive strategies would lead to absenteeism and somatization. The research was qualitative methodology, descriptive and exploratory, seeking to know the reality of family organization and investigate the relationship between pleasure and suffering experiences in the work of the leaders on the management of an organization family, giving them the opportunity to expose the content of the work, perceptions crossing the experienced daily life and the relationship with pleasure and suffering the work done. Interviews and focus groups were conducted in order to collect data that could identify the existence and type of psychological distress manifestation of the leaders; describe management features; analyze the relationship of this management and family organization and relate the work of the leaders with their psychological distress and their defense strategies to deal with this suffering. After the content analysis found five categories: Work - vicissitudes and crossings, pointing work as occupation of time beyond the organization of space and experiences that interfere in the family environment and leisure; Threshold between pleasure and pain, noting that there is a threshold between the situations of pleasure and suffering in the work of the leaders; Charge of life - ease and difficulty before the people management experience; Contexts of family organization, with its peculiarities, reinforcing the existence of limitation in the management of the leader and defense strategies that are used to not getting sick. The obtained speech brought the symbolic experience determined by contextualized scenario, before the central theme suffering and pleasure in work, pleasure and suffering characterizing the leadership experiences in the peculiar context of researched family organization

RESUMO O objeto deste estudo é o acolhimento do usuário em uma unidade da rede básica de serviços públicos de saúde de Ribeirão Preto. Este constituise em um processo, ocorrendo em quaisquer das etapas do processo de trabalho, cuja responsabilidade cabe aos trabalhadores. Sua efetivação implica no estabelecimento de relações humanas, no uso das tecnologias disponíveis para a solução dos problemas, estando estreitamente relacionada ao desenvolvimento de autonomia dos usuários em relação à sua vida e com a conformação do modelo assistencial. Dentre os objetivos buscou-se identificar e analisar os aspectos que o compõem bem como identificar e analisar como os trabalhadores efetuam-no e compreendem seus componentes no processo de trabalho. Utilizou-se como técnicas de investigação a observação participante, tomando-se o fluxograma analisador do modelo de atenção como organizador desta, acompanhando-se usuários adultos da clínica médica e pronto atendimento e entrevistas semiestruturadas com trabalhadores que participaram desses atendimentos. Identificou-se como componentes do acolhimento: a forma como se produz o atendimento, o autogoverno dos trabalhadores, a mútua representação entre os envolvidos, a objetivação do problema/necessidade, as linhas de fuga da estruturação do trabalho, a plasticidade no uso das tecnologias, o preparo para a relação, a comunicação, a escuta, o espaço-tempo que lhe é destinado e a responsabilização pelo trabalho. Verificou-se que o trabalhador não se apercebe produtor do acolhimento, efetuando o atendimento como um procedimento técnico externo a ele.
SUMMARY The objective of this study is the welcoming reception of the user at a basic health unit of the public health service of Ribeirão Preto. It consists of a process, which occurs in any of the stages of the work process, being a responsibility of the workers. Its effectiveness implies in the establishment of human relations, in the use of technologies available for problems solution, having a close relation to the development of the users autonomy in relation to their lives and the assistencial pattern conformation. The aims were to identify and analyse the aspects which took part of it and identify and analyse how the workers did it and how they understand its components in the work process. As research technique, the participative observation was used, taking the flow chart analyser of the attention pattern as its organizer, following the adult users of the clinician and emergency and semi-structured interviews with the workers which participated in these appointments. As components of the reception we identified: the way the assistance is produced, the self-management of the workers, the mutual representation between those envolved, the objectiveness of the problem/need, the escape lines from the structure, plasticity in the use of technologies, the preparation for the relation, the communication, the listening, the space-time destined to it and the responsibilization from the work. It was noticed that the worker doesnt realize himself as producer of the welcoming reception, doing the assistance as a technical procedure external to him.

Objeto de estudo: repercussões psicofísicas no trabalhador de enfermagem decorrentes da atuação em ressuscitações cardiopulmonares em enfermarias de clínica médica. Objetivos: identificar os fatores facilitadores e dificultadores enfrentados pelos trabalhadores de enfermagem na atuação em ressuscitação cardiopulmonar, discutir os sentimentos que emergem desses trabalhadores nesta atuação, analisar as repercussões psicofísicas na saúde destes trabalhadores decorrentes da atuação em ressuscitação cardiopulmonar. Estudo qualitativo, descritivo, fundamentado no método dialético e com referencial teórico da Psicodinâmica do Trabalho. Desenvolvido em quatro enfermarias de clínica médica de um hospital universitário da rede pública do município do Rio de Janeiro, tendo como sujeitos cinco enfermeiros e doze auxiliares de enfermagem. Utilizou-se como instrumento de coleta de dados a entrevista semiestruturada e as informações coletadas foram tratadas e analisadas à luz do método de análise de conteúdo, o qual possibilitou o surgimento de três categorias: a organização do trabalho e seus reflexos sobre a atuação dos trabalhadores de Enfermagem em ressuscitação cardiopulmonar, vivências referentes a esta atuação e implicações na saúde destes trabalhadores decorrentes desta atuação. Os resultados apontaram para um significativo distanciamento entre o trabalho prescrito e o real. Esse distanciamento gerava nos sujeitos uma percepção marcadamente de dificuldades, as quais conduziram a relatos de muito mais sofrimento do que de prazer. Verificou-se que o sentimento de prazer emergia quando os sujeitos aludiam à recuperação do cliente, à sensação do dever cumprido e quando eles conseguiam vencer as dificuldades que perpassavam este atendimento. Diante das situações dificultadoras, os trabalhadores referiram angústia, irritabilidade, nervosismo, ansiedade, frustração, isto é, sentimentos negativos e prejudiciais para a saúde do trabalhador. Várias repercussões psicofísicas foram sentidas pelos trabalhadores decorrentes do atendimento na parada cardiorrespiratória, e as que remetem as da dimensão psicológica foram as mais mencionadas, as quais foram identificadas como: sentimento de perda, de sentir-se desrespeitado e impotente para transpor as dificuldades, tristeza, ansiedade, agonia, tensão, dificuldade no enfrentamento da morte e medo de errar no procedimento. No entanto, os sujeitos também citaram tensão na região cervical, rigidez muscular, cefaléia e hipertensão, como repercussões ligadas à dimensão física. Além dessas, houve referência de dores nas mãos, nas pernas e lombalgia, decorrentes do exercício laboral em condições não favoráveis às necessidades ergonômicas para realização desta atividade. A partir dos resultados apreendidos, considera-se que existe a necessidade de discutir, avaliar e intervir no processo de trabalho a fim de que a atividade de RCP seja mais bem operacionalizada, com menos ou nenhum impacto negativo na saúde do trabalhador de enfermagem. A mudança no processo laboral deve envolver a esfera gerencial e a assistencial, em nível multidisciplinar, privilegiando inclusive o saber prático dos trabalhadores. Recomenda-se ainda a formação e a manutenção de grupos de reflexão com os profissionais de saúde, em especial os da enfermagem, para ajudá-los a lidar com a morte e compreendê-la como um segmento do processo de trabalho em saúde.
Aim of the study: psychophysical repercussions of nursing workers resulting from their practice in cardiopulmonary resuscitation (CPR) among medical in-patients. Objectives: to identify the facilitating and difficulty factors faced by nursing workers practice in CPR, discuss the feelings that emerge from the workers in their practice and analyze the psychophysical repercussions on the workers' health arising from such practice. Qualitative, descriptive study, based on the dialectical method and theoretical reference to the Psychodynamics of Work. The study was developed in four medical wards of a public university hospital in the city of Rio de Janeiro, which had five nurses and twelve nursing assistants as subjects. Data collection instrument chosen was a semi-structured interview, whose information was analysed and dealt with under the light of content analysis approach, resulting in the creation of three categories: the organization of work and its reflexes on the nursing workers practice as regards cardiopulmonary resuscitation, experiences in relation to their practice and the implications on the health of the workers resulting from their practice. The results indicated a significant distancing between the prescribed and the real work, which created in the subjects involved a clear perception of difficulties that resulted in reports that stressed much more on suffering than on pleasure. It was also verified that the feeling of pleasure emerged when the subjects mentioned the recovery of the patient, in the sense of accomplished duty and when they managed to overcome the difficulties involved in that attendance. When facing difficult situations, the workers referred to anguish, irritability, nervousness, anxiety, frustration, that is, negative feelings which are harmful to the worker's health. Various psychophysical repercussions were felt by the workers stemming from the attendance of cardiorespiratory arrests and the ones that have a psychological dimension were the most mentioned, which were identified as: feeling of loss, of being disrespected and impotent to surpass the difficulties, sadness, anxiety, agony, tension, difficulty in facing death and the fear of making mistakes in the procedures. Besides, the subjects also mentioned feeling tense in the cervical region, muscular stiffness, headache and high blood pressure as repercussions linked to the physical dimension. There was also reference to feeling pain in the hands, legs and back, caused by the laborial exercise in conditions not so favorable to the ergonomic necessities for the realization of this activity. From the obtained results, we consider that there is a need to discuss, evaluate and intervene in the working process so that CPR can be better operationalized, with less or no negative impact on the wellbeing of the nursing worker. The change in the working process must involve both the managerial and assistential spheres, on a multidisciplinary level, privileging the practical knowledge of the workers as well. We also recommend the formation and maintenance of reflection groups among health care professionals, especially in Nursing so as to help them better deal with death and acknowledge it as part of the process in Health Care.

Syftet med denna kvalitativa studie var att förstå och sammanfatta hur första linjens chefer (FLC) i en vårdorganisation upplever sin egen psykosociala arbetsmiljö, vad de tycker fungerar bra, vilka utmaningar de ser, och vilket stöd de behöver för att kunna förbättra den egna psykosociala arbetsmiljön. Jag genomförde semi-strukturerade intervjuer för att samla in data och två klassiska modeller inom psykosocial arbetsmiljöforskning: krav-kontroll-stöd och ansträngning-belöning, har fungerat som teoretiska tolkningsramar. Tematisk analys användes för att analysera data och identifiera teman för resultatet. Resultatet visade på sju teman som respondenterna upplevde påverkade deras egen psykosociala arbetsmiljö: chefsrollen, arbetsbelastning, styrning av den dagliga verksamheten, påverkan/beslutsutrymme, information/kommunikation, arbetsrelationer/samarbete och stödfunktioner. Några utmaningar för FLC är: orimligt höga krav, stora arbetsgrupper, bemanningsproblem, tidskrävande administration, brist på långsiktig planering och förbättringsarbete, mycket övertid och behov av mer stöd för att minska arbetsbelastningen. En närliggande tolkning av resultatet är att flera delar behöver förändras för att förbättra FLCs psykosociala arbetsmiljö och förutsättningarna för chefsuppdraget.
The purpose of this qualitative study was to understand and summarize how first-line managers (FLM’s) in a healthcare organization is experiencing their own psychosocial work environment, what they think works well, what challenges they see, and what support they need to improve their own psychosocial work environment. I conducted semi-structured interviews to collect data and two classic models in the psychosocial work environment research: demand-control-support and effort-reward, have served as theoretical frameworks of interpretation. Thematic analysis was used to analyze data and identify themes for the result. The result showed seven themes that the respondents experienced affected their own psychosocial work environment: the manager role, workload, control of daily operations, impact/decision latitude, information/communication, working relationships/cooperation and support functions. Some challenges for FLM are: unreasonable high demands, large workgroups, challenges with staffing, time-consuming administration, lack of long-term planning and improvement work, a lot of overtime and more support needed to reduce workload. An interpretation of the result is that the organization needs to change several parts to improve FLM’s psychosocial work environment and conditions for the manger role.

La presente tesi, attraverso l’utilizzo di diverse fonti di dati, sia longitudinali che trasversali, contribuisce alla letteratura esistente in materia di lavoro precario, invecchiamento e discriminazione di genere, fornendo evidenza empirica riguardo le conseguenze in termini di salute e benessere della precarietà, del pensionamento e della leadership femminile nel mercato del lavoro. Il primo capitolo esamina la relazione esistente fra insicurezza sul lavoro, prospettive di reimpiego e disagio psicologico, utilizzando dati cross-country provenienti dalle European Working Conditions Surveys del 2010. Il secondo capitolo è dedicato alla stima dell’effetto causale del pensionamento sull’utilizzo dei servizi sanitari in 10 paesi Europei nel periodo 2004-2013. In particolare, il capitolo approfondisce il tema della riduzione del costo opportunità del tempo libero dopo il pensionamento, che può dare luogo ad aumenti improvvisi nell’utilizzo dei servizi sanitari. Il terzo capitolo, infine, studia la relazione esistente tra leadership femminile, pratiche organizzative a livello aziendale e discriminazione di genere per 30 paesi Europei, considerati nel periodo 1995-2010.
The present dissertation, using both longitudinal and cross-sectional data from different sources, contributes to existing literature on precarious employment, aging and gender discrimination providing empirical evidence on the health and wellbeing outcomes of work-related insecurity, retirement and female leadership across European countries. Chapter 1 examines the relationship among perceived job insecurity, employability and psychological distress in Europe, using cross-country data from the 2010 European Working Conditions Surveys. Chapter 2, using SHARE data (from 2004 to 2013) for 10 European countries, is devoted to the analysis of the (causal) effect of retirement on health care utilization. In particular, it explores the existence of a discontinuous change in health investment at the time of retirement, as suggested by the theory, asking whether this ``puzzling'' jump is associated with the drop in the opportunity cost of time induced by retirement. Chapter 3 investigates the association between female leadership, work organization practices and perceived gender discrimination within firms, using EWCS data for 30 European countries for the period 1995-2010.

Investigou-se a presença de assédio moral no trabalho e suas inter-relações com dimensão psicossocial, bem-estar e adaptação psicossocial positiva. As bases teóricas compreenderam: Abordagem Ecológica, Saúde Mental e Trabalho e Psicologia Positiva. No Estudo I, participaram 598 bancários, que responderam questionário biosociodemográfico e laboral, Questionário de Atos Negativos, Job Content Questionnaire, Medidas de Bem-Estar no Trabalho e Escala de Adaptação Psicossocial Positiva. Os dados foram submetidos a estatísticas descritivas, exploratórias e inferenciais. No Estudo II, realizaram-se 4 estudos de casos, e os dados submetidos à análise de conteúdo. Houve alta prevalência de assédio moral e associação com baixo controle, altas demandas, baixo apoio e alta insegurança no emprego. Os níveis de bem-estar no trabalho foram menores para as vítimas. Diferentes fontes de dados (incidência, correlações, preditores e vivências concretas de trabalhadores) indicaram transições ecológicas entre os papéis de vítima, testemunha e agressor e natureza principalmente organizacional do fenômeno.
The aim of this study was to investigate workplace bullying and its relations to psychosocial dimension, well-being and positive psychosocial adaptation. Theoretical basis comprehended: Ecological Theory, Work and Mental Health and Positive Psychology. In the first study, 598 banking workers answered demographic and labor questionnaire, Negative Acts Questionnaire, Job Content Questionnaire, Measurements of Well-Being at Work and Positive Psychosocial Adaptation scale. Data were submitted to descriptive, exploratory and inferential statistics. In the second study, 4 multiple case studies were conducted. Data were submitted to qualitative content analysis. The prevalence of workplace bullying was high. It was observed association with low control, high demands, low social support and high job insecurity. The taxes of well-being at work were lower for the victims. Different data resources (incidence, correlations, predictors and concrete experiences of worker) indicate ecological transitions among victims, observers and perpetrators, as well as a mainly organizational nature of workplace bullying.

Resumo: As transformações pelas quais tem passado o campo da saúde pública no Brasil têm trazido reflexos na organização do trabalho. Nessa perspectiva, o Programa Saúde da Família (PSF), enquanto uma das medidas para tornar realidade as mudanças preconizadas desde a regulamentação do Sistema Único de Saúde (SUS), constituiu-se como cenário da pesquisa. O objetivo da pesquisa foi identificar a relação entre o sofrimento psíquico do trabalhador e a organização do trabalho em uma equipe do PSF. A fundamentação teórica possibilitou o entendimento do sofrimento psíquico numa organização do trabalho permeada pela lógica capitalista, representada pelo materialismo histórico. Além disso, destacou-se o caráter subjetivo do elemento sofrimento psíquico, frente a organização do trabalho, valendo-se da Psicodinâmica do Trabalho, da Psicologia Social e da Saúde Coletiva. A observação participante e entrevista não-estruturada foram utilizadas para o levantamento dos dados, numa abordagem de pesquisa qualitativa. As entrevistas foram gravadas e transcritas. A população constituiu-se de todos os componentes de uma equipe da Unidade Saúde da Família (USF) de um município de médio porte do interior do estado de São Paulo, totalizando quinze trabalhadores. Na análise de conteúdo do material coletado foram identificadas três temáticas gerais: a) implicações de ordem pessoal; b) o cotidiano de trabalho de atenção; e c) a infra-estrutura institucional. Essa análise mostrou implicações entre dificuldades estruturais e funcionais na organização do trabalho e o sofrimento psíquico do trabalhador. Algumas indicações disso aparecem como desânimo, angústia, conflito na equipe, falta de rede de apoio, acúmulo de atividades, absorção dos problemas do usuário para si mesmo, desvalorização profissional, sobrecarga de trabalho e impotência frente à dificuldade de dar conta da demanda...
Abstract: The Brazilian public health service took some changes and theses transformations have been caused reflections on work organization. With this perspective, the Family Health Program (PSF - Programa Saúde da Família) became like research scenario, while one of forms to come true changes determinated since regulation of Single Healthcare System (SUS - Sistema Único de Saúde ). The aim of this study was to identify the relation between worker psychic suffering and the work organization in one team of PSF. The theorical base became possible the understanding of psychic suffering in one work organization permeated on capitalist logic represented by historic materialism. Besides, the subjective character of psychic suffering element was evidenced about work organization using Work Psychology, Social Psychology and Collective Health. The participative observation and unstructured interview were used on data survey, in qualitative research boarding. The interviews were record and transcript. The population constituted all team elements of Family Health Point (USF - Unidade Saúde da Família) from a medium port city on interior of São Paulo State, totalizing 15 workers. Content analysis was realized through establishment of three general thematics: a) personal implications; b) attention work quotidian; and, c) institutional substructure. This analysis showed implications among structural and functional difficulties on organization of work and psychic suffering of worker. Some indications about this appears like discouragement, anguish, team conflict, absence of net of support, accumulation of activities, self absorption of user problems, professional devaluation, excess of work and impotence about difficulty to get finish the work. Este trabalho buscou investigar as estratégias que os/as professores/as constroem para enfrentar as adversidades do cotidiano escolar. As abordagens fenomenológica e hermenêutica foram a inspiração metodológica da pesquisa. O estudo, que ocorreu numa escola da rede pública municipal de Salvador, com professores de 1ª a 8ª série do ensino fundamental, considerou a organização do trabalho pedagógico como elemento atrator de promoção e/ou agravar a saúde do professorado. Compreendo que a ação docente tem algumas especificidades em relação a outras categorias profissionais, discuti o exercício da profissão como uma atividade que demanda certas especificidades das organizações escolares. Entender as estratégias construídas por professores/as exigia uma discussão sobre as adversidades que eles/as enfrentam diante da docência. Essas adversidades podem ser decorrentes das relações construídas no espaço escolar e do conteúdo da ação docente. Sobre as estratégias os/as professores/as as constroem como um recurso que visa, tanto poupar energia e se desgastar menos, como encontrar, de modo criativo alternativas para enfrentar os problemas da escola. A pesquisa aponta para a necessidade de se construir uma organização do trabalho pedagógico que considere a coletividade como intrínseca à educação e à ação docente. Finalizo, destacando que as estratégias defensivas,ou de fuga, levam a uma banalização do processo educativo, enquanto as de enfrentamento ou criativas, representam um esforço no sentido de transformar a adversidade em criatividade, buscando sentido para a atividade pedagógica.

Uppsatsen syftade till att beskriva upplevelser bland individer som tidigare har medverkat i Rapatac och nuvarande medarbetare på Rapatac gällande vad ett medverkande i ungdomsorganisationen kan innebära för individernas skolsituation, sociala nätverk och hälsa. Fyra individer som har medverkat i Rapatac och två medarbetare intervjuades i en kvalitativ semistrukturerad intervjustudie. Teorier som användes i studien var den utvecklingsekologiska systemteorin och teorin om KASAM. Resultatet visade att upplevelserna av medverkandet gällande skolsituation, socialt nätverk och hälsa liknar varandra. Generellt upplevde intervjupersonerna att medverkandet i Rapatac har bidragit till positiv förändring. I vilken utsträckning individerna upplevde att medverkandet förändrade deras skolsituation, sociala nätverk och hälsa skilde sig. I relation till den tidigare forskningen inom området uppvisade studiens resultat ett flertal likheter.
The aim of the study was to describe experiences among individuals who previously have participated in Rapatac and current co-workers at Rapatac regarding what a participation in the youth organization can mean for the individuals school situation, social network and health. Four individuals who have participated in Rapatac and two co-workers were interviewed in a qualitative semi-structured interview study. Theories used in the study were the ecological systems theory and the SOC theory. The result indicated that the experiences of the participation regarding school situation, social network and health are similar to each other. In general, the interviewees experienced that the participation in Rapatac has contributed to positive change. The extent to which the individuals experienced that the participation changed their school situation, social network and health differed. In relation to previous research within the field the results of the study showed several similarities.

Aim: The aim of the study was to describe the experience of working environment in open plan office within IT- sector. Background: Previously was the most common office model cell office, open plan office is now the most common. A good working environment in theise two office types needs different attributes. A theory to explain relationship between health and unhealth is Karasek and Theorells demand- control- supportmodel. Method: A qualitative interview study was performed to gain a deeper understanding of the experience of working in open plan office within IT- sector. Ten informants from two different organizations were chosen. The result has been analyzed with qualitative content analysis. Results: The analyses resulted in three themes and six sub-themes. The result showed that there were both positive and negative aspects of working environment in open plan office. Positive aspect was mentioned as good communication and good social interaction while high sound level and disturbing working situation was mentioned as negative aspects. Conclusion: The result shows that there is many aspects to take in consideration white work in open plan office. To have a good working environment there is need to have a deeper understanding from the organization how the open plan office affect the individual.
Syfte: Syftet med studien var att undersöka upplevelsen av arbetsmiljö i öppna kontor inom IT-sektorn. Bakgrund: Tidigare var den vanligast förekommande kontorsmodellen att sitta i enskilt cellkontor numera har öppna kontor blivit den vanligast förekommande arbetsplatsmodellen. Förutsättningen för god arbetsmiljö ser helt olika ut i dessa två kontorsmodeller. I öppna kontor är Karasek och Theorells Krav- kontroll- stödmodell en användbar teori för att tydliggöra förhållandet mellan hälsa och ohälsa på arbetsplatsen. Metod: En kvalitativ intervjustudie genomfördes för att få en djupare förståelse för upplevelsen av att arbeta i öppna kontor inom IT-sektorn. Tio informanter från två olika verksamheter intervjuades och materialet analyserades sedan med kvalitativa innehållsanalys. Resultat: Analysen resulterade i tre huvudkategorier och sex underkategorier. Det framkom både fördelar och nackdelar i arbetsmiljön på öppna kontor. Bland annat god kommunikation och det sociala samspelet lyftes som fördelar medan ljudnivån och brist på ostörd arbetsmiljö ansågs negativt. Konklusion: Studiens resultat visar att det finns många olika aspekter att ta hänsyn till vid arbete i öppna kontor. För att få en god arbetsmiljö krävs förståelse från verksamhetschefer och medarbetare hur det öppna kontoret påverkar individen ur både fysiskt och psykiskt perspektiv

After Lithuania regained its independence, many of the country's unsolved social problems soon became apparent. These problems illustrated the need for a properly trained labor force of social workers equipped to deal with them, particularly in the field of health care. It soon became clear that effective health care prevention was every bit as important as diagnosing and curing disease. A very important component of such health care is the nursing care and social support that patients receive, but the provision of such care is still insufficient in Lithuania . Patients that have lost their independence and are seriously ill must receive adequate care and support.

Darbo tikslas. Įvertinti burnos higienistų, dirbančių Kauno miesto privačiose ir viešosiose odontologijos klinikose, darbo organizavimo ypatumus. Tyrimo metodika. Atlikta momentinė anoniminė Kauno miesto burnos higienistų anketinė apklausa. Išdalinta 70 anketų: 56 dirbantiems privačiose ir 14 viešosiose odontologijos klinikose. Anketas užpildė ir grąžino 45 privačiose (atsako dažnis 80,3 proc.) ir 14 viešosiose (atsako dažnis 100 proc.) klinikose dirbantys burnos higienistai. Statistinė duomenų analizė buvo atliekama naudojant SPSS 17 for Windows programinį paketą. Hipotezėms tikrinti buvo naudojama aprašomoji statistika bei Chi kvadrato (χ2 ), t ir Mann-Whitney kriterijai. Rezultatai. Dažniausiai odontologijos klinikose atliekamos burnos higienistų procedūros: burnos higienos mokymas (100 proc.), dantų akmenų šalinimas ultragarsu (100 proc.) bei rankiniais instrumentais (93,2 proc.). Dažniausiai aptarnaujami jauni (19-35 metų) bei vidutinio amžiaus (36-55 metų) pacientai. Rečiausiai – vaikai ar paaugliai iki 18 metų (22,8 proc.), neįgalūs pacientai. Privačiose įstaigose dirbantys burnos higienistai dažniau nei viešosiose dirba dar ir odontologo asistentu (atitinkamai 62,2 ir 21,4 proc.). Viešose įstaigose dažniau nei privačiose dirba mokslinį darbą (14,3 proc) arba dėstytojauja (2,4 proc.). Beveik absoliuti dauguma (93,2 proc.) respondentų linkę manyti, kad geriau burnos higienistu dirbti privačioje įstaigoje dėl geresnių darbo sąlygų, darbo vietos organizavimo (87,3 ir 63,6... Aim of the study. To evaluate the dental hygienists work organization peculiarities in private and public dental clinics in Kaunas city. Methods. An anonymous questionnaire have been completed in private and public dental clinics in Kaunas city. In total 70 questionnaires were distributed to employed dental hygienists: 56 in private and 14 in public dental clinics in Kaunas. Questionnaires filled out and returned: 45 dental hygienists employed in private (response rate 80,3 %) and 14 in public dental clinics (response rate 100 %). The statistical analysis was performed using SPSS version 17 for Windows statistical software . Hypotheses were checked using descriptive statistics and Chi square (χ2), t and Mann-Whitney criterions. Results. Most frequently dental hygienists performed oral treatments in dental clinics such as oral hygiene education and training (100 %), scaling (100 %) and calculus removal using hand instruments (93.2 % ). Most dental hygienist served young (19-35 years) and middle-aged (36-55 years) patients. Least - working with children or adolescents under 18 years old (22.8 %), with disabled patients. Dental hygienists in private dental institutions more than in public works as a dental assistant (respectively 62.2 and 21.4 %) Meanwhile, in public dental institutions dental hygienist more than in private work in research work (14.3% ) and as academics (2.4 %). Förändring är en del av livet, likaså lärande och hälsa, vilket rimligtvis borde innebära att dessa processer påverkar varandra. Denna studie handlar om hur medarbetare inom en kommunal enhet, vars huvudsakliga uppdrag är att arbeta hälsofrämjande gentemot seniorer, beskriver och upplever dessa tre processer utifrån deras arbetsplatskontext. Syfte: Syftet med studien var därför att utifrån ett hälsopedagogiskt perspektiv undersöka hur medarbetarna beskrev förändring och lärande på enheten samt hur de upplevde att dessa processer påverkade deras hälsa. Metod: Studien utgick från en kvalitativ ansats där fyra fokusgruppsintervjuer användes som datainsamlingsmetod. Urvalet bestod av 12 medarbetare som intervjuades i grupper om fyra. Resultatet analyserades utifrån en kvalitativ innehållsanalys. Resultat: Medarbetarna beskrev att dagens seniorer är mer aktiva samt att arbetet kräver digital kompetens och omvärldsbevakning. De beskrev också att lärandet på enheten främjas av samarbete och en tillåtande atmosfär. Medarbetarna upplevde att deras hälsa främjas av att alla (organisation, chef och medarbetare) tar ansvar för en god arbetsmiljö. Slutsats: Processerna förändring, lärande och hälsa ställer höga krav på en tydlig organisationsstruktur och ett hälsofrämjande ledarskap. För att hantera förändring, samtidigt som lärande och hälsa främjas, är även goda relationer och strategier för dialog och reflektion viktigt.
Change is a part of life, as is learning and health, which should reasonably mean that these processes affect each other. This study is about how employees within a municipal unit, whose main task is to work to promote health towards seniors, describe and experience these three processes based on their workplace context. Aim: The aim of the study was to examine from a health pedagogical perspective how the employees described change and learning at the unit and how they felt that these processes affected their health. Method: The study was based on a qualitative approach where four focus group interviews were used as a data collection method. The sample consisted of 12 employees who were interviewed in groups of four. The results were analyzed on the basis of a qualitative content analysis. Results: The employees described that today's seniors are more active and that the work requires digital competence and external monitoring. They also described that learning at the unit is promoted by cooperation and a permissive atmosphere. The employees felt that their health was promoted by everyone (organization, manager and employees) taking responsibility for a good working environment. Conclusion: The processes of change, learning and health place high demands on a sharply defined organizational structure and health-promoting leadership. Good relationships and strategies for dialogue and reflection are also important in managing change while at the same time promoting learning and health.

Abstract:

This essay’s focus lies on the collaboration that takes place in hospitals and called a care-plan (vårdplanering) between the two organisations: Health-care (landsting) and social-care (kommun). According to the Swedish-law are these organizations responsible for the care of elderly individuals in Sweden. The aim of this essay has been to study closely this collaboration in order to see which partner has the most influencing power.

This essay is of a qualitative character, which means that the results that are presented are based on six individual interviews with employees from the two organizations. The employees chosen for this essay all have job experiences from the collaboration that takes place during care-plans in hospitals.

One of the main results of this essay has been that the two organizations are not equal collaboration-partners. All the interviewed individuals talked highly about the importance of collaboration while in reality they end up in an subliminal battle against each other. The main conclusion of this essay is that the focus of a care-plan meeting appears to be on gaining the most power, in order to influence the outcome of a care-plan, rather than giving priority to the elderly individual’s needs and concerns.

Abstract:
Given the importance of entrepreneurship to individuals and to society, and given that the individual has come to be seen as not only an important function of social and economic processes but also as an important end of these processes, the link between entrepreneurship and well-being requires further investigation. To that end, this thesis provides a solid framework for understanding and measuring well-being in entrepreneurship, and a comprehensive theoretical and empirical examination of the intimate relationship between these two phenomena. Some new and very interesting results were produced: • Active engagement in the creation of new business activities leads to greater well-being by granting the freedom to self-organize work and working relationships, which satisfies individual’s needs for autonomy, competence, and relatedness. • Entrepreneurial engagement is detrimental to mental health during its early inception in planning and deliberation, which may explain why many fail to transition from thought to action in entrepreneurship. • The presence of a discrepancy between extrinsic life goals and motives is just as strong a predictor of well-being in entrepreneurship as the presence of strong intrinsic life goals and motives. • Entrepreneurial Well-Being, a context-specific state of mental wellness derived from engagement in entrepreneurship, uniquely contributes to entrepreneurs’ overall well-being and is associated with a range of cognitive and motivational processes crucial to entrepreneurship. The reasons for these findings, and the mechanisms involved, are explained at length. My hope is that this work will usher on the adoption of a more integrated approach to assessing and optimizing entrepreneurial performance in terms of both business outcomes and mental health outcomes, as well as offer insights relevant to well-being and its promotion on a more general level.

Em organizações que operam segundo a lógica de serviço há uma mudança estratégica, com o deslocamento da produção de um produto para um valor. Seguindo esta dinâmica, surgiram na Saúde Pública propostas de modelos de atenção alternativos ao hegemônico, centrado em procedimentos e equipamentos. O presente estudo analisou o modelo da Estratégia de Saúde da Família, cuja proposta centra-se nas necessidades do usuário e no vínculo usuário-equipe multiprofissional, tendo como objetivo investigar como a organização e as condições do trabalho influenciam na utilização de recursos imateriais pelas equipes. Consistiu em um estudo de caso realizado junto a oito equipes de saúde da família do município de Caraguatatuba/SP. A metodologia compreendeu observação direta e realização de grupos focais com os profissionais das equipes. A análise abrangeu a categorização dos temas mais relevantes, em especial aqueles que se relacionavam ao uso e desenvolvimento dos recursos imateriais. Os resultados indicaram que, embora os profissionais valorizassem os aspectos relacionais, o processo de trabalho das equipes encontrava-se centrado na produção de procedimentos e informações quantitativos dos atendimentos, não incorporadas às práticas do cuidado. Os recursos imateriais, bem como seus resultados, não encontravam uma forma sistematizada de avaliação. E, dessa forma, enfrentavam desafios para serem apropriados e desenvolvidos como conhecimento pela organização.
Organizations operating according to the service logic imply a strategic change, the shifting of production from that of a product to that of a value. Following such dynamics, new models of attention emerged in the Public Health System, purporting alternatives to the hegemonic model.This study analyzes the model adopted by the Strategy of Family Health, which is focused on the users\' necessities and the bonds between those former and the multi-professional team. Its aim was to investigate how work organization and conditions influence the teams\' utilization of non-material resources. To such purpose a case study was carried out in the city of Caraguatatuba, São Paulo state, Brazil.The methodology comprehended direct observation and focal groups with the teams\' professionals. The analysis covered the categorization of the subjects considered more relevant, especially those concerning the utilization and development of non-material resources. The results pointed out that although relational aspects were valued by the professionals, the teams\' work process was mainly focused on the production of proceedings and information concerning the number of patients assisted, apart from the practices of care.There was no systematic evaluation of the nonmaterial resources and their results, which, therefore, could not be easily appropriated and developed as a knowledge by the organization.

This thesis presents the results of a study in Chemical Industry of the State of Goiás S/A (IQUEGO), aiming to diagnose the occurrence of accidents in the workplace, in the period from 2000 to 2007. The problem discussed concerns the relations between the conditions and organization of work as being facilitators of the occurrence of accidents from the assumption that, knowing the risk factors involved in labor activities in the pharmaceutical industry, you can see a strategy aimed at reducing the current rate of accidents at work. First, an analysis was made of the profile of the employee victim, type of activity carried out in business, time distribution of accidents, body parts affected and causes of accidents and injuries. We tried to verify the relationship between the accident and profile of affected workers and the working conditions of the area where they work in accordance with the personnel management policies adopted in the company. The method adopted for the research was the case study. Data were collected from a compilation of accidents reported through communication of Occupational Accidents (CAT), available in the collection of documentary industry. The research shows that the productive sector has a range of diversity of jobs and tasks. Organized in two shifts, the pace of work is intense, with continuous and repetitive movements of body parts, forced postures, intense rotations of trunk and head and legs of inflections, which makes the environment conducive to accidents. Under health and safety of workers, found that the organization adopts a policy not very well formalized, but supports the activities prevention ongoing, as the recommendations of Good Manufacturing Practices (GMP) and the legal concepts. The findings point to the need of the pharmaceutical industry, field of research, preventive care and has the standard of management, that invests in health and safety, improving working conditions, preserving the dignity of their workers.
Esta dissertação apresenta os resultados de uma pesquisa realizada na Indústria Química do Estado de Goiás S/A (IQUEGO), com o objetivo de diagnosticar a ocorrência de acidentes no ambiente de trabalho, no período de 2000 a 2007. A problemática discutida refere-se às relações entre as condições e a organização do trabalho como elementos facilitadores da ocorrência de acidentes, a partir do pressuposto de que, conhecendo os fatores de riscos inerentes às atividades laborais da indústria farmacêutica, pode-se vislumbrar uma estratégia voltada à redução dos índices de acidentes no trabalho. Em primeiro plano, procedeu-se a análise do perfil do trabalhador vitimado, tipo de atividade exercida na empresa, distribuição temporal dos acidentes, partes do corpo atingidas e causas dos acidentes e das lesões. Buscou-se verificar a relação entre os acidentes e o perfil dos trabalhadores vitimados, bem como as condições de trabalho da área em que atuam, de conformidade com as políticas de gestão de pessoal adotadas na empresa. O método adotado para a investigação foi o estudo de caso. Os dados foram coletados dos registros dos acidentes relatados por meio de Comunicação de Acidente de Trabalho (CAT), disponíveis no acervo documental da indústria. A pesquisa revelou que o setor produtivo possui uma gama de diversidade de postos de trabalho e tarefas executadas. Organizado em dois turnos, o ritmo de trabalho é intenso, com movimentação contínua e repetitiva de partes do corpo, posturas forçadas, rotações intensas de tronco e cabeça e flexões de pernas, o que torna esse ambiente propício à ocorrência de acidentes. No âmbito da saúde e segurança do trabalhador, constatou-se que a organização não adota uma política própria de prevenção devidamente formalizada, mas apóia as atividades prevencionistas em curso, conforme as recomendações das Boas Práticas de Fabricação (BPF) e as concepções legais sobre o assunto. As conclusões apontam para a necessidade de a indústria farmacêutica, campo da pesquisa, ter o cuidado preventivo como norma de gestão, investir em saúde e segurança, no sentido de melhorar as condições de trabalho, preservando a saúde e a dignidade de seus trabalhadores.

Cette thèse vise à comprendre l’absentéisme des femmes et des hommes dans un métier mixte. Nous nous sommes interrogés sur la manière dont un métier devenu progressivement mixte peut générer des inégalités de santé entre les femmes et les hommes en raison de son organisation du travail, de son évolution économique ainsi que de ses éléments relatifs au contexte psychosocial. Pour faire cela, nous avons étudié les causes de l’absentéisme différencié des facteurs et des factrices. Afin de comprendre cet écart, nous avons mobilisé des techniques d’observations, d’entretiens, d’enquêtes emploi du temps et de diagnostic photo pour mettre en exergue les expositions auxquelles les femmes peuvent être confrontées dans un métier mixte. Nos résultats nous amènent à identifier plusieurs causes permettant d’expliquer cet absentéisme différencié : une organisation du travail pensée sur des normes masculines en raison d’un héritage historique, une évolution de l’activité en partie liée à la baisse du courrier et à la hausse des colis qui peut être plus défavorable pour les femmes, un matériel conçu sur des normes masculines qui n’est pas complètement adaptés aux femmes et un rapport au travail différent pour les femmes et les hommes. Cette thèse met en évidence le fait que la prise en compte de la variable sexe pour comprendre le travail des femmes permet d’améliorer les conditions de travail de tout le monde. De nombreux éléments ont pu en outre être révélés grâce à la prise en compte de cette variable
The objective of this thesis is to understand men and women absenteeism in the same profession. We questioned how same jobs can potentially generate inequalities in health conditions between men and women due to work organization, economic development and the psychosocial context. To do this, we have studied the causes of absenteeism of post-men and post-women. In order to understand this difference, we used different methods to highlight exposures that women may face in the same job : field observations and interviews, photographic diagnosis and time-use surveys. These different methods show some elements which may explain the different occupational health outcomes of women compared to the same job than men do : work organization built around male norms due to historic heritage, a declining physical mail volumes and increase in packages which can be unfavorable for women, working materials were conceived on male norms are not adapted to women and relationship to work was different for men and women. This thesis revealed that taking care of gender in order to understand women’s work allows to improve work conditions for everyone. Numerous elements have been highlighted in order to taking gender into account
