Abstract:
Aim: This study aimed to evaluate the inferior alveolar nerve block, that is, the Halstead technique, Clark and Holmes technique, Gow Gates technique, and Sargenti technique, for mandibular anesthesia. Methodology: This prospective, double-blinded, in-vivo study was conducted amongst 100 patients, requiring mandibular anesthesia. These patients were divided into four groups. Parameters assessed were time required for appearance of subjective and objective symptoms and signs, positive aspiration, need for supplementary anesthesia, and ease of administration. Results: The means for subjective symptoms for the four techniques, that is, Halstead technique, Clark and Holmes technique, Gow Gates technique, and Sargenti technique, were 78.44, 120.76, 176.6, and 203.08, respectively. The means for objectives symptoms for the four techniques, that is, Halstead technique, Clark and Holmes technique, Gow Gates technique, and Sargenti technique, were 110.6, 269.8, 287.48, and 154.08, respectively. Halstead technique had statistically significant (P < 0.05) faster objective signs than all the other techniques. Supplementary block if required was noted for all four techniques. Conclusion: The Clark and Holmes technique showed maximum complications, while Gow Gates technique was most difficult to administer. The Angelo Sargenti technique gave good results, same as standard Halstead technique.

Abstract:
Background: Since the introduction of successful thyroidectomy no change occurred in the technique but with the innovation of sutureless surgery, sutureless thyroidectomy becomes a competitor for the conventional technique. In this study we tried to analyze the outcomes of each.Methods: This was a prospective cross armed uncontrolled randomized study included 65 patients suffering from non-malignant goiter who underwent total thyroidectomy, admitted to Sohag University Hospital from September 2017 to October 2018. Patients were randomly allocated into two groups; Group (S) managed with sutureless thyroidectomy, and Group (C) managed with conventional knot tying technique. The data of the patients were gathered and analyzed considering the operative and post-operative outcomes. All patients were followed-up for 6 -12 months.Results: We found a significantly shorter operative time (p=0.001), also the amount of intra-operative blood loss was highly significant less in Group S (p=0.001). There was higher incidence of post-operative pain in Group C (p<0.001), post-operative drainage volume was highly significantly less in Group S (p=0.001). Regarding post-operative complications; the incidence of recurrent laryngeal nerve paralysis (RLNP) was insignificant between both groups (p=0.38), in the same time the incidence post-operative transient hypocalcaemia was significantly more in Group C (P=0.025). The overall post-operative complication rate was significantly more in Group C (p=0.002). The duration of hospital stay was highly significant shorter in Group S (p=0.0001).Conclusions: The use of Harmonic FOCUS® is beneficial for both patient and surgeon; it is efficient and a trustable safe substitute to the conventional knot tying technique.

Category: Midfoot/Forefoot Introduction/Purpose: Sesamoidectomy is most often indicated for sesamoid fractures. The procedure may also be a useful option for cases of recalcitrant sesamoiditis, osteochondritis dissecans, and osteoarthritis. In instances of concomitant arthritis of the first metatarsophalangeal joint and the sesamoids, sesamoidectomy may be performed in tandem with first metatarsophalangeal joint fusion. This carries the additional benefit of harvesting bone graft from the excised sesamoid to include in the fusion construct. Though sesamoidectomy is commonly performed through a medial or plantar approach, neither of these approaches are suited to fusion of the first metatarsophalangeal joint. Sesamoidectomy can be performed effectively through a dorsal approach and is the preferred technique when performed in conjunction with first metatarsophalangeal joint fusion. Methods: A dorsal midline incision is made over the first metatarsophalangeal joint. Care is taken to protect the extensor hallucis longus tendon while dissection proceeds to the joint capsule. The capsule is incised and released so that the first metatarsophalangeal joint can be sufficiently mobilized. Next, the opposing joint surfaces are prepared, increasing the mobility of the joint and exposure of surrounding structures. Results: (technique, continued) A sharp retractor is used to elevate the metatarsal head. Hyperflexion of the metatarsophalangeal joint may increase visualization of the sesamoids. A fresh knife is used to tease the sesamoid away from the flexor hallucis longus tendon, beginning at one edge of the sesamoid. The bone is carefully elevated off the tendon while dissection proceeds around the perimeter. Once the sesamoid has been freed and excised, the tendon is examined for injury. The excised sesamoid can then be cleared of soft tissue and cartilage and morselized for bone graft. Conclusion: The dorsal approach is effective for sesamoidectomy and efficient when performed as an adjunct to first metatarsophalangeal joint fusion.

Abstract:
Problems of female subfertility are a current challenge. Implant failure of the embryo after an assisted human reproduction technique is undesirable and unpleasant, and strategies to improve this implant rate are welcome. One of these techniques is endometrial scratching. We performed a literature review by searching in the PubMed and Cochrane databases for relevant information regarding the technique of endometrial scratching. The technique of endometrial scratching is not complex, but choosing an optimal period in the menstrual cycle seems to produce the desired effects for the successful implantation of the embryo. Pregnancy rates obtained through assisted human reproduction techniques have been improved due to endometrial scratching techniques.

Abstract:
This paper proposes a hybrid technique for data security. The computational model of the technique is grounded on both the non-linearity of neural network manipulations and the effective distortion operations. To accomplish this, a two-layer feedforward neural network is trained for each plaintext block. The first layer encodes the symbols of the input block, making the resulting ciphertext highly uncorrelated with the input block. The second layer reverses the impact of the first layer by generating weights that are used to restore the original plaintext block from the ciphered one. The distortion stage imposes further confusion on the ciphertext by applying a set of distortion and substitution operations whose functionality is fully controlled by random numbers generated by a key-based random number generator. This hybridization between these two stages (neural network stage and distortion stage) yields a very elusive technique that produces ciphertext with the maximum confusion. Furthermore, the proposed technique goes a step further by embedding a recurrent neural network that works in parallel with the first layer of the neural network to generate a digital signature for each input block. This signature is used to maintain the integrity of the block. The proposed method, therefore, not only ensures the confidentiality of the information but also equally maintains its integrity. The effectiveness of the proposed technique is proven through a set of rigorous randomness testing.
