Abstract:

Objective: Present the clinical and functional outcome of the first five ankle arthroplasties performed in our department. Methods: Five patients (five feet) with secondary arthrosis of the ankle underwent ankle arthroplasty with the Taric® prosthesis. Average age at the time of surgery was 42 years. Follow-up time averaged 43 months. We recorded the immediate postoperative complications. For clinical evaluation we used the VAS pain scale, the AOFAS hindfoot and ankle scale, and the Foot Function Index. We measured the leg-foot range of motion. On the radiographs, we measured the alpha, beta, and gamma angles as well as sagittal balance. We sought to identify the presence of posterior osteophytes (bone spurs). Results: We observed a case of medial malleolus fracture and a case of superficial suture dehiscence. In two cases, we considered the final outcome to be poor. The mean VAS of pain before surgery was 8.8 points while the postoperative score was 5.4 points. On the AOFAS scale, the average preoperative score was 52 points and the score at final evaluation, 70 points. We noticed a 10º increase in leg-foot range of motion. Conclusion: The assessment of the first patients undergoing ankle arthroplasty with the Taric® prosthesis yielded outcomes considered poor in 40% of cases. However, in those with an outcome considered satisfactory, there was an improvement in pain and an increase in leg-foot range of motion. In all patients, we noticed the formation of a posterior osteophyte alongside the posterior cortex of the distal tibia. Level of Evidence IV; Therapeutic Studies; Case Series.

Abstract:

Stereo differentiated asymmetric syntheses have been achieved by S-indoline derivations. Diels-Alder cycloadditions of S-indoline chiral acrylamides with cyclopentadiene proceed with high diastereofacial selectivity, giving either endo-R or endo-S products depending on Lewis acid and the structures of chiral dienophiles. Diastereo- and enantio-selective pinacol coupling reactions of chiral α-ketoamides mediated by samarium diiodide afforded extremely high diastereoselectivities. Enantiopure (S,S)- or (R,R)-2,3-dialkyltar-taric acid and derivatives can be synthesized for the first time depending on the structure of α-ketoamides.

Abstract:

ZusammenfassungDas differenzialtherapeutische Konzept in den fortgeschrittenen Larsen-Stadien 4 und 5 der rheumatischen Sprung-gelenksarthritis umfasst neben der Arthrodese auch die prothetische Versorgung des oberen Sprunggelenks. Bedingt durch die kritischen Ergebnisse mit zementiert verankerten Sprung-gelenksprothesen der ersten Generation wurde die Endopro-these lange Zeit als experimentell eingestuft und die Arthrodese als Methode der Wahl empfohlen. Die Nachteile der Sprung-gelenksarthrodese liegen jedoch in der langen Ruhigstellung, der erhöhten mechanischen Belastung der subtalaren Gelenke und dem Pseudarthroserisiko. Die rekonstruktive Versorgung mit einer Sprunggelenksendoprothese ermöglicht dem Rheumapatienten im Vergleich zur Fusion eine wesentlich schnellere Remobilisierung mit Erhalt der Abrollvorgänge und Korrekturmöglichkeiten im Sprunggelenk. Die Entwicklung der Sprunggelenksendoprothetik beginnt Anfang der 1970er-Jahre mit der Einführung der Zweikomponenten-Prothesen. Nach anfänglich optimistischen Resultaten zeigten diese Mo-delle im mittelfristigen Verlauf nicht akzeptable Revisionsraten zwischen 20 und 40 %. Büchel und Pappas mit der B.P.-Pro-these und H. Kofoed mit der S.T.A.R.®-Prothese inaugurierten das Prinzip der modernen meniskustragenden Dreikomponentenprothese. Zwischen der anatomisch geformten Taluskappe und einer planen Tibiabasisplatte bewegt sich – frei zur Tibia, geführt durch die Formgebung der Taluskomponente – ein Polyethylengleitkern. Die mittelfristigen Ergebnisse der aktuellen Dreikomponentenprothesen sind sehr ermutigend. In den vergangenen 15 Jahren wurden entscheidende Fortschritte im Hinblick auf Design und Technik der Sprunggelenksprothetik erreicht. Unsere Patienten können, nach einer gut implantier-ten Prothese, über einen mittelfristigen Zeitraum von acht bis zehn Jahren mit einer schmerzfreien Funktion des operierten Sprunggelenks rechnen. Trotz der einheitlich guten klinischen Ergebnisse mit den modernen Sprunggelenksimplantaten sind längst nicht alle Probleme gelöst. Basierend auf den Erfahrun-gen mit verschiedenen Prothesensystemen hat die Autorengruppe die TARIC®-Prothese entwickelt. Die ersten Ergebnisse sind sehr vielversprechend und wir hoffen, auch weiterhin über gleichwertige mittel- und langfristige Resultate berichten zu können.

Abstract:

Mandatory Sistem Verifikasi Legalitas Kayu (SVLK) menjadi non tarif barrier yang dibuat oleh pemerintah. Seluruh produk kayu tidak dapat menembus semua pasar ekspor jika tidak memiliki sertifikat legalitas kayu dan dokumen v-legal. Deregulasi menimbulkan kerisauan bagi industri furnitur kayu. Tarik ulur regulasi menunjukkan ketidakkonsistenan pemerintah atas regulasi yang dibuatnya. Akibatnya, selama tujuh tahun ditetapkan kebijakan ini belum memberikan dampak signifikan bagi peningkatan daya saing industri furnitur kayu. Oleh sebab itu, diperlukan transformasi regulasi yang lebih implementatif dan sesuai dengan kebutuhan industri. Hasil kajian menyatakan bahwa pemerintah perlu melakukan evaluasi implementasi SVLK secara menyeluruh dengan melibatkan industri dan seluruh stakeholder kehutanan. Pemerintah juga harus melakukan deregulasi antara lain dengan cara menurunkan standar biaya sertifikasi dan menetapkan standar maksimum tarif sertifikasi yang diberlakukan lembaga verifikasi (LV). Mandatory SVLK khusus pasar Uni Eropa dan niche market yang dilandasi dengan voluntary partnership agreement (VPA) seperti halnya yang dilakukan dengan Uni Eropa.

Abstract:

Tamansari Water Castle is a sub-tourist attraction located in the Keraton Yogyakarta complex, this tourist attraction is famous for its heritage buildings and also for its water palace. Since it was opened to tourists on April 1, 1974 many tourists visited this tourist attraction, both domestic and foreign tourists. Since the opening of this tourist attraction for the first time, which has not yet drawn retribution rates to tourists, there have been several times the rate of retribution increases to enter this tourist attraction. So there needs to be research on the feasibility of increasing the levy rate. In this regard, the author tries to examine the "Studi Kelayakan Peningkatan Tarif Retribusi di Daya Tarik Wisata Tamansari Water Castle". The research method used in this study is quantitative research with comparative matrix analysis techniques, to analyze the feasibility of increasing levy rates in the tourist attraction of Tamansari Water Castle. Sources of data from primary and secondary data, data collection techniques using observation, interviews and literature studies, and interview instruments in the form of interview guidelines. Determination of informants at Tamansari Water Castle in this study using purposive sampling technique, so as to obtain accurate data about the feasibility of increasing levy rates in the tourist attraction of Tamansari Water Castle. Abstract:

Peningkatan kualitas pelayanan angkutan umum masal berbasis jalan (TransJakarta) merupakan kebijakan yang dilakukan pemerintah Kota Jakarta untuk mengatasi kemacetan. Sebagai salah satu penerapan manajemen kebutuhan transportasi yang berdampak tarik, kebijakan tersebut perlu didukung oleh ketersediaan fasilitaspark and ride yang memenuhi kriteria. Penelitian ini bertujuan untuk menganalisis kriteria apa saja yang menjadi prioritas pada sebuah fasilitas park and ride. Melalui metode analisis Analytical Hierarchy Process (AHP) dihasilkan suatu kesimpulan bahwa pengembangan fasilitaspark and rideharus memprioritaskan variabel kemudahan bagi pengguna jasa (0,182), aksesibilitas (0,153), dan lokasi/penempatan (0,128). Pengembangan fasilitaspark and ride sebagai pendukung TransJakarta juga perlu mengutamakan beberapa kriteria yaitu berfungsi sebagai bagian dari sistem transportasi intermoda sekaligus sebuah fasilitas, terletak pada wilayah sub urban (tidak harus terletak pada terminal tipe A), letak jalan masuk/keluar menghindari konflik dengan pejalan kaki, pola parkir menyesuaikan dengan daya tampung, tarif terpadu dengan TransJakarta, tersedia petugas penjaga dan petugas tiket, tersedia jalur pejalan kaki menuju halte bus, terkoneksi dengan layanan moda lain, dan memberi dampak positif terhadap wilayah sekitarnya.

Imbalan atas PNBP Royalti Paten telah menjadi polemik selama bertahun-tahun pada inventor yang bernaung pada lembaga/institusi penelitian milik negara. Salah satu efek yang menjadi perhatian adalah hilangnya potensi temuan-temuan yang berefek pada perkembangan ekonomi dalam negeri karena para inventor lebih tertarik untuk "menjual" temuan dimaksud ke pihak luar dibawah tangan. Pemberian Imbalan merupakan pengembangan sistem pengakuan dan penghargaan di bidang HKI, pemberian insentif akan berbanding lurus dengan pengembangan inovasi berbasis IPTEK yang bermanfaat secara ekonomi bagi bangsa yang pada akhirnya akan meningkatkan penerimaan negara bukan pajak. Kajian ini menyimpulkan bahwa berdasarkan pertimbangan dimaksud maka imbalan perlu diposisikan sebagai kebijakan tarik ulur bergantung pada situasi dan kondisi yang berlaku. Memperhatikan segala risiko atas potensi kehilangan bagi negara yang lebih besar maka kebijakan pemberian imbalan merupakan kebijakan yang layak direkomendasikan. Agar kebijakan ini dapat berjalan, perlu disiapkan 3 buah produk hukum seperti disebutkan dalam surat DJA Nomor S-1765/AG/2012 tanggal 09 Juli 2012, yakni: PP Tarif, KMK Persetujuan Penggunaan, dan PMK Standar Biaya mengenai Imbalan.

Abstract:

This research aims at exploring tourist attraction and the potentials of Selogriyo Temple and its area in Magelang District. The method used is qualitative method. The types of data are primary data and secondary data. The primary data are obtained from observation and interview. The secondary data are obtained from literature review or relevant various literature study and then the results are used as the supporting data for the primary data. The results of this research show that the attraction of Selogriyo Temple and its area located in rice field terracing, local residents activity, and art show. Abstract:

AbstrakSalah satu kelengkapan dalam pelaksanaan tata cara adat, upacara adat yang ada pada masyarakat melinting adalah adanya sebuah tarian adat melinting. Pada awalnya tarian ini memang merupakan sebuah tarian sakral, namun dalam perkembangannya tarian ini banyak mengalami perubahan dan perkembangan, baik dalam segi gerak, kostum, maupun kesakralannya. Penelitian ini bertujuan mendeskripsikan dan mengungkap tari melinting beserta filosofi yang mendasarinya. Pengumpulan data dilakukan dengan melaksanakan wawancara mendalam, pengamatan (observasi), dan studi kepustakaan. Ternyata tari melinting memiliki makna yang mendalam, tidak hanya gerakan tarinya yang indah, namun juga filosofi yang ada di balik tarian tersebut, seperti tentang kearifan tradisional masyarakat, falsafah hidup, serta dinamika kehidupan masyarakat Melinting khususnya dan masyarakat Lampung Timur pada umumnya. Kata melinting mempunyai makna yang sangat luas, merupakan nama daerah (kecamatan), nama jenis tumbuhan, nama tarian, dan juga nama dari sebuah wilayah adat dari Keratuan Melinting yang meliputi daerah Labuhan Maringgai, Gunung Pelindung, Wana. Di masa lalu wilayah adat melinting mencakup daerah yang lebih luas lagi, ke utara sampai dengan pantai di Labuhan Maringgai dan ke selatan sampai ke daerah Tegineneng. AbstractOne of the complement during the ceremonial customs in Melinting society is Melinting dance. In the beginning, this dance is sacred, however during the time their movement, customs, or sacred has changed and developed. This research has a purpose to describe and reveal of Melinting dance with their philosophy. The procedure for collecting the data with an in depth interview, observation, and library research. In result, Melinting dance has a deep meaning. It is not only about beautiful dance movement, but also the philosophy of wisdom, life, also talking about the dynamics of melinting and East Lampung society. Word of Melinting has a broader meaning, such as name of an area (district), plantation, dancing, and the territory of Melinting crown including Labuan Maringgai, Gunung Pelindung, Wana. In the past, the melintingregion included a wider area, to the north reach Labuan Maringgau beach, and to the south reach Tegineneng area.

Pembiayaan kesehatan dapat menentukan kualitas pelayanan kesehatan yang diberikan, di Kabupaten Karawang ditemukan bahwa kasus preeklampsia dan perdarahan masih menjadi dua penyebab utama kematian ibu, selain itu periode kematian mayoritas terjadi pada masa nifas (50%), penelitian ini bertujuan untuk melakukan kajian lebih lanjut apakah tarif asuhan kebidanan pada masa nifas dan bayi abru lahir yang telah ditetapkan oleh BPJS Kesehatan dapat menutup seluruh pelayanan yang diberikan oleh bidan, Penelitian ini merupakan penelitian analitik komparatif membandingkan tarif 2 kelompok tidak berpasangan yaitu tarif pelayanan nifas dan bayi baru lahir non BPJS Kesehatan diperkotaan dan dipedesaan yang kemudian dibandingkan dengan standar tarif yang telah ditetapkan oleh BPJS Kesehatan. Pengumpulan data dilakukan melalui survei langsung pada bidan yang dijadikan responden penelitian, dengan jumlah sampel sebanyak 31 bidan setiap kelompok. Hasil penelitian didapatkan adanya perbedaan tarif asuhan pasca salin antara tarif di pedesaan dan BPJS serta antara tarif di perkotaan dan tarif BPJS (P<0.05) dan perbedaan tarif asuhan bayi baru lahir antara tarif di pedesaan dan BPJS (p<0,05) serta antara tarif di perkotaan dan tarif BPJS (p<0,05). Adapun nilai tengah tarif yang berlaku pada umumnya sebesar Rp.50.000 baik untuk kunjungan nifas maupun bayi baru lahir, sehingga standarisasi tarif kunjungan nifas dan bayi yang ditetapkan BPJS Kesehatan masih jauh dibawah nilai tengah tersebut, sehingga pemerintah sudah saatnya untuk meningkatkan standar tarif BPJS yang telah ditetapkan tersebut. Abstract:

Angkutan umum merupakan salah satu moda transportasi yang masih menjadi pilihan masyarakat di Purbalingga. Satu diantaranya adalah PO. Teguh Rahayu yang melayani rute Terminal Purbalingga - Bobotsari. Penentuan besaran tarif angkutan membutuhkan penanganan dan kebijakan yang arif. Karena harus dapat menjembatani kepentingan pengguna jasa dan pengelola jasa angkutan umum. Maka dari itu dilakukan penelitian mengenai tarif berdasarkan Biaya Operasional Kendaraan untuk mengetahui tarif yang tepat. Metode pengumpulan data dengan melakukan survey penumpang dan wawancara terhadap Operator PO. Teguh Rahayu untuk mengetahui biaya operasional kendaraan micro bus. Didapatkan hasil BOK rata - rata sebesar Rp. 5.906,91/ Orang-trip. Untuk perhitungan tarif maksimal diperoleh sebesar Rp 6.302,04 dan perhitungan tarif minimal diperoleh Rp 3.428,35. Namun tarif yang ada di lapangan dimana tarif ditetapkan adalah Rp. 5000/orang-trip, untuk satu kali perjalanan dengan jarak tempuh 11 Km. Berdasarkan perhitungan tarif Bus PO. Teguh Rahayu trayek terminal Purbalingga – Bobotsari didapat harga tarif lapangan mendekati posisi tarif perhitungan biaya operasional kendaraan. Hal ini menunjukan hasil analisis tarif berdasarkan biaya operasional kendaraan dengan tarif sebenarnya masih sesuai.
