Abstract:
Abstract Sirolimus (SIR), an inhibitor of mammalian target of rapamycin (mTOR), is an immunosuppressive agent that is synergistic with CNI to prevent or treat GVHD in patients (pts) undergoing allo-HST (Cutler, Biol Blood Marrow Transplant. 2004;10:328). Transplant associated microangiopathy (TAM) is a multi-factorial complication of allo- HST, associated with CNI. Recently, the addition of SIR to CNIs was reported to result in a higher than expected (10.8%) incidence of TAM (Cutler et al. Biol Blood Marrow Transplant. 2005; 11:551). We analyzed the incidence and clinical characteristics of TAM in 2 groups of pediatric patients (pts): the prevention group (GP): pts who received SIR and tacrolimus (TAC) for prevention; and the treatment group (TG): pts treated with SIR and a CNI (TAC or cyclosporine) for exacerbation of GVHD. Methods: A retrospective chart review of pts with allo-HST who received SIR/TAC for either prevention or treatment of GVHD. Demographic data, clinical course, occurrence of GVHD and TAC and SIR levels were recorded. TAM was defined as serum creatinine (SCr) of ≥ 50% above baseline, elevated LDH levels (>X2 upper limit of normal), presence of schistocytes or persistence of nucleated red blood cells, and prolonged or progressive thrombocytopenia. For PG the risk period was defined between days −1 to +60 of HSC. The risk period for the TG was defined from the first day of SIR/TAC until day +30 after the last dose was given. Results: Records of 45 pts who were treated with SIR/TAC were reviewed. The median age of 21 patients in PG was 9.1 yr (2.9–22); male gender, n=13. The median age in 24 TG pts was14.7 yr (3.3–20.6); male gender, n= 11. Conditioning regimens consisted of TBI based regimens (64%), or chemotherapy only (36%). Diagnoses included ALL (40%), AML (27 %), NHL (9%), Fanconi’s (4 %), and other (9%). The median follow up for the surviving patients is 28.6 months. TAM criteria were met in 15 ( 30%) patients who received SIR/TAC, 5 PG pts (23.8%) and 10 TG pts ( 41.6%). Two PG patients died from multi-organ failure (MOF) related to other reasons (VOD, n=1 and multi-organism infection, n=1). Both pts had hemolytic uremic syndrome (HUS), possibly contributing to MOF. One patient developed seizures secondary to thrombotic thrombocytopenic purpura (TTP) as a complication of TAM. TAC and SIR levels were within the desired range (WDR, (≤10 ng/ml, for each) in this patient and signs and symptoms of TAM and TTP resolved after discontinuation of both TAC and SIR. In 2 patients TAM findings were asymptomatic, without progression to HUS or TTP. SIR/TAC levels in these patients were all WDR. Among 10 TG pts that met TAM criteria, 2 pts progressed to HUS and one developed severe TTP. All 3 patients with complicated TAM died (HUS, n=1, MOF, n=1, and idiopathic pneumonitis, n=1). Two of the pts with laboratory, but not clinically apparent TAM died (invasive fungal infection, n=1, IP n=1). In all TG pts with TAM either TAC (n=7) or SIR (n=6) levels exceeded WDR. To identify possible risk factors for TAM, the following parameters were analyzed: age, conditioning regimen, disease type, degree of HLA match. In the PG, only organ failure or high-grade GVHD could be identified as risk factors. In the TG, TAC or SIR levels exceeding WDR, in addition to ac GVHD or chronic GVHD present in all patients, were associated with an increased risk for clinical significant TAM. Conclusion: Organ damage, either by acute GVHD or from other reasons may contribute to development of clinically significant TAM in pts undergoing allo-HST treated with SIR plus CNI. Monitoring TAC or SIR levels and early diagnosis of TAM may prove to be critical in patients with progressive organ damage of GVHD.

Abstract:
Background. A large number of pelvic injuries are seriously unstable, with mortality rates reaching 19%. Approximately 60% of pelvic injuries are related to the posterior pelvic ring. However, the selection of a fixation method for a posterior pelvic ring injury remains a challenging problem for orthopedic surgeons. The aim of the present study is to investigate the biomechanical performance of five different fixation approaches for posterior pelvic ring injury and thus provide guidance on the choice of treatment approach in a clinical setting. Methods. A finite element (FE) model, including the L3-L5 lumbar vertebrae, sacrum, and full pelvis, was created from CT images of a healthy adult. Tile B and Tile C types of pelvic fractures were created in the model. Five different fixation methods for fixing the posterior ring injury (PRI) were simulated: TA1 (conservative treatment), TA2 (S1 screw fixation), TA3 (S1 + S2 screw fixation), TA4 (plate fixation), and TA5 (modified triangular osteosynthesis). Based on the fixation status (fixed or nonfixed) of the anterior ring and the fixation method for PRI, 20 different FE models were created. An upright standing loading scenario was simulated, and the resultant displacements at the sacroiliac joint were compared between different models. Results. When TA5 was applied, the resultant displacements at the sacroiliac joint were the smallest (1.5 mm, 1.6 mm, 1.6 mm, and 1.7 mm) for all the injury cases. The displacements induced by TA3 and TA2 were similar to those induced by TA5. TA4 led to larger displacements at the sacroiliac joint (2.3 mm, 2.4 mm, 4.8 mm, and 4.9 mm), and TA1 was the worst case (3.1 mm, 3.2 mm, 6.3 mm, and 6.5 mm). Conclusions. The best internal fixation method for PRI is the triangular osteosynthesis approach (TA5), followed by S1 + S2 screw fixation (TA3), S1 screw fixation (TA2), and plate fixation (TA4).

Abstract SOX8, SOX9 and SOX10 compose the SOXE transcription factor group. They govern cell fate and differentiation in many lineages, and mutations impairing their activity cause severe diseases, including campomelic dysplasia (SOX9), sex determination disorders (SOX8 and SOX9) and Waardenburg-Shah syndrome (SOX10). However, incomplete knowledge of their modes of action limits disease understanding. We here uncover that the proteins share a bipartite transactivation mechanism, whereby a transactivation domain in the middle of the proteins (TAM) synergizes with a C-terminal one (TAC). TAM comprises amphipathic α-helices predicted to form a protein-binding pocket and overlapping with minimal transactivation motifs (9-aa-TAD) described in many transcription factors. One 9-aa-TAD sequence includes an evolutionarily conserved and functionally required EΦ[D/E]QYΦ motif. SOXF proteins (SOX7, SOX17 and SOX18) contain an identical motif, suggesting evolution from a common ancestor already harboring this motif, whereas TAC and other transactivating SOX proteins feature only remotely related motifs. Missense variants in this SOXE/SOXF-specific motif are rare in control individuals, but have been detected in cancers, supporting its importance in development and physiology. By deepening understanding of mechanisms underlying the central transactivation function of SOXE proteins, these findings should help further decipher molecular networks essential for development and health and dysregulated in diseases.

Abstract The mechanisms of tumor rejection were investigated by using a therapeutic model system involving treatment of C57BL/6J (B6) mice bearing the syngeneic MCA/76-9 or the unrelated MCA/76-64 sarcomas with cytoxan and tumor-sensitized T lymphocytes. Separated tumor-associated T lymphocytes (TAL) and tumor-associated macrophages (TAM) isolated from the regressing tumors 8 to 10 days after combination therapy expressed relatively specific cytotoxicity in vitro, whereas the unseparated tumor-associated cells (TAC), consisting of a mixture of TAL and TAM, expressed nonspecific cytotoxicity. TAM-mediated cytotoxicity was not dependent on the presence of TAL, as shown by T cell depletion of TAM or TAC cultures with the use of monoclonal anti-Thy-1 or anti-Lyt-2 antibody and complement. In contrast, the nonspecific cytotoxicity was dependent on the presence of T cells. In vivo assays using the Winn test failed to confirm certain aspects of the in vitro data. Without exception, the TAC inhibited tumor growth in an immunologically specific manner, having no effect on the growth of the unrelated B6 sarcoma. T cell depletion completely abrogated in vivo cytotoxicity. Specificity of tumor growth inhibition was confirmed in a bystander experiment in which TAC were mixed with both tumor cell types and were injected into recipient B6 mice. Tumors grew under these conditions, but the tumor that grew consisted only of those tumor cells toward which TAC cytotoxicity was not specifically directed. A bioassay indicated that the specifically immune antitumor effects at the site of regression were initiated between days 3 and 7 after combination therapy. By days 7 and 9, few tumorigenic stem cells could be detected at the tumor site. However, T cell depletion of the TAC isolated on days 8 to 10 resulted in enhanced tumor growth when the depleted TAC were injected into recipient mice. The conclusions reached were that tumor rejection was absolutely dependent on T cell participation at the tumor site, and that if TAM were involved, they required the presence of TAL and did not express nonspecific antitumor cytotoxicity. Indeed, the accelerated tumor growth seen in the absence of TAL suggested the possibility that TAM were growth stimulatory.

In 2012, federal legislation introduced a requirement for formal transportation asset management (TAM) in transportation agencies. The law specifically requires agencies to develop TAM plans and implement TAM programs in their decision-making processes. The history of national policy development for (TAM) and research in policy implementation and organizational theory have shown that program sustainment is not easily achieved. Often, agencies can respond to this kind of legislative mandate with ineffective efforts to achieve legitimacy that reduce the likelihood for the program to be sustained in the long-term. This presents a challenge because without sustainment, the benefits of TAM, which are mostly long-term, may not be fully realized. The objective of this work was to develop a conceptual framework to guide transportation agencies to review their TAM implementation practices and identify opportunities to enhance long-term program sustainment. The conceptual basis for the framework comes from a synthesis of transportation, policy and program implementation, and change management literature, supported by insight from a panel of practitioner and academic experts. The TAM Implementation Review Framework (TIRF) incorporates seven themes of implementation factors that can promote sustainment of TAM practices impacting the social, organizational, and programmatic elements of implementation. The TIRF was applied in case studies to review the TAM implementation processes of three Departments of Transportation (DOTs) resulting in different kinds of information on how implementation activities address factors related to program sustainment. These results can steer future implementation activities in DOTs towards increased probabilities of achieving long-term program sustainment. The primary contributions of this work lie in the development of the conceptual framework and approach to enhance TAM implementation by emphasizing the people and organizational elements of TAM, alongside with the technical. In practice, the TIRF offers agencies a review and planning tool to support TAM implementation decision making and to promote program sustainment. The output is being proposed as a foundational element in the development of an evidence-based catalog of TAM implementation strategies. This can facilitate inter-agency knowledge sharing by providing a platform for systematic documentation of implementation experiences to support broader adoption of strategies that can contribute to increased program sustainment.

Les travaux exposes dans cette these porte essentiellement sur la modelisation, l'estimation parametrique, et la commande d'un bras manipulateur a quatre degres de liberte, le tam. Nous presentons les modeles geometriques, cinematiques et dynamiques (directs et inverses) obtenus pour le bras tam, respectivement avec ou sans outil. Nous rappelons ensuite les notions de parametrisation minimale et d'identifiabilite structurelle, puis la condition d'excitation persistante des signaux, en soulignant ses variations suivant le type d'algorithme d'estimation choisi (recursif ou non, avec formulation matricielle ou sequentielle); nous faisons apparaitre les simplifications importantes qu'apporte la procedure sequentielle dans la recherche de trajectoires excitantes. Nous developpons un nouvel algorithme recursif et sequentiel, utilise pour l'estimation des parametres minimaux et de frottements du tam. Sensibilises au probleme de complexite du modele dynamique des robots rigides, nous avons mis au point une methode de simplification originale et efficace, basee sur un critere energetique et tenant compte des proprietes fondamentales de la mecanique a verifier. Nous proposons alors une methodologie pour l'estimation et la reduction du modele dynamique, incluse dans un logiciel tres convivial, nomme projet, developpe sur station sun. Une etude experimentale realisee en implantant des lois de commande du type computed torque a parametres fixes (avec modele complet ou reduit), comparees a des lois classiques de type pid, montre clairement la superiorite de la commande dynamique qui permet d'effectuer des taches delicates comme l'insertion d'outils dans la plaque tubulaire du generateur de vapeur

Abstract:
The complexity of electronic system is increasing rapidly and many of the electronic systems   are   embedded   systems   implemented   as   system-on-chip   (SoC).   This increasing  complexity  of  SoC  leads  to  longer  test  application  time  (TAT).  One approach  to  reduce  the  TAT  is  to  perform  tests  to  several  cores  in  parallel,  which requests transporting test data in parallel instead of sequentially. In IEEE Std. 1500, it supports parallel test mode by incorporating a user-defined, parallel   test   access   mechanism   (TAM)   to   speed   up   the   testing   process.   The user-defined  TAM  means  the  detail  of  TAM  design  is  excluded  from  standard  and decided by system integrator. Therefore, we propose a customized TAM structure and two approaches to guarantee full-spatial-parallelism under a bus width limit, and aim to  minimize  the  total  number  of  wire  connections.  In  order  to  know  how  close  to optimal  solution  our  solutions  are,  we  implement  a  Simulated  Annealing  (SA) algorithm to do the comparison. The  experimental  results  of  the  two  proposed  approaches  based  on  benchmark ISCAS’89  and  ITC’02  show  the  parallelism  can  be  guaranteed  by  our  approaches while using only a few wire connections per pin, and the execution times of them are shorter compared with the SA algorithm.

As empresas frequentemente buscam moldar a evolução das aplicações tecnológicas e estão cada vez mais utilizando chatbots e inteligência artificial para se comunicar com os clientes, resolver dúvidas, problemas de atendimento, procurar informações, comprar produtos e serviços, reservar hotéis, fazer perguntas às empresas, entre outros. Sendo assim, o objetivo desta pesquisa consiste em explorar diferentes variáveis externas e do Modelo TAM para ver se influenciam o consumidor em relação ao uso dos chatbots pelas empresas. Para coletar os dados quantitativos foi realizado um questionário online e 235 respostas foram analisadas com o programa SPSS. De 16 hipóteses, obteve-se 12 hipóteses confirmadas e aceitas, sendo possível afirmar que os consumidores estão dispostos a adotar e utilizar chatbots para se comunicar com as empresas.
Companies often seek to shape the evolution of technology applications and are increasingly using chatbots and artificial intelligence to communicate with customers, solve questions, service issues, look for information, buy products and services, book hotels, ask business questions. Therefore, the objective of this research is to explore different external variables and the Model TAM that influence the consumer in relation to the use of chatbots by companies. To collect the quantitative data, an online questionnaire was performed and 235 responses were analyzed with the SPSS program. Vertinant skirtingą temperatūrą grunte, gaunami šilumos nuostoliai per atitvarą besiribojančią su gruntu yra taip pat skirtingi. Mokslininkai grunto temperatūrą vertina dviem būdais: pirmuoju atveju, kai gruntas veikiamas adiabatine apkrova tam tikrame gylyje ir antruoju, kai tam tikrame gylyje užsidedama temperatūros apkrova. Tai skatina tirti ir analizuoti abu šiuos grunto vertinimo metodus ir išsiaiškinti, kuris yra labiau tinkamas Lietuvos sąlygomis bei koks ši- lumos praradimas atitvaroje besiribojančioje su gruntu skirtumas galimas tarp šių metodų. Šiame darbe nagrinėjamos temperatūros konstrukcijose besiribojančiose su gruntu naudojantis THERM skaitmeninio modeliavimo programa. Gauti rezultatai parodė, jog antruoju skaitmeninio modeliavimo atveju užsidavus temperatūrinę apkrovą tam tikrame gylyje, gauta grunto temperatūra ir įšalo gylis artimas Lietuvos galiojančioje statybinėje klimatologijoje RSN 156-94 „Statybinė klimatologija “.

Introduction: Approximately two-thirds of all breast cancer patients overexpress hormonal receptors and are treated with endocrine therapy, being tamoxifen (TAM) the standard treatment. However, many of the initial responders to TAM as first-line experience relapse. Several mechanisms have been proposed to explain the occurrence of acquired TAM resistance. Previous studies showed that estrogen receptor β (ERβ) expression is associated with better response to tamoxifen treatment, as the co-expression of ERα and ERβ is associated with TAM antiproliferative effects. Moreover, there is a growing interest about the cross talk between ERs and ErbB family in response to endocrine therapy. Objectives: The aim of the present study was to evaluate the expression of ERβ and the relation of ERβ with ERα and ErbB family in response to TAM treatment and in TAM resistant cells. Methods: ERβ expression was analyzed in two different databases of breast cancer patients. The mRNA levels of ER, HER receptors and PTEN and MAPK signal pathways were measured after TAM treatment, in TAM resistance cells and in cells silenced for ER genes. The cellular viability was also measured after TAM treatment, in TAM resistance cells and in cells silenced for ER genes. Results: Breast cancer patients presented reduced ERβ expression, and the ERα-positive breast cancer subtypes presented lower ERβ levels when compared to ERα-negative breast cancer subtypes. Cells expressing moderate levels of ERβ presented a better response to TAM treatment. Downregulation of ERs induced by TAM treatment was accompanied by an increase in ErbB2 and ErbB3, MAPK3 mRNA levels and increased PTEN levels. TAM–resistant cells expressed decreased ERs, PTEN and MAPK3 mRNA levels and increased EGFR, ErbB3 and ErbB4 levels. In accordance to the resistance finding, cells silenced for ERβ presented decreased MAPK3 and cells silenced for ERα showed an increased EGFR. Conclusions: These results provide additional data indicating the importance of ERβ as a possible biomarker of endocrine resistance and highlighted the interaction between ERα and EGFR as a mechanism of TAM resistance.

Ecological models provide crucial tools for informing many aspects of ecosystem restoration and management, ranging from increasing understanding of complex ecological functions to prioritizing restoration sites and quantifying benefits for project reporting. The diversity of ecosystem types and restoration objectives often precludes the use of existing models; as such, model development is commonly required to inform restoration decision-making. Index-based habitat models are a common approach for assessing ecosystem condition. These models relate habitat quality to species’ distributions. Habitat suitability (quality) typically ranges on a scale from 0 to 1. Habitat models have been developed to assess habitat suitability for specific taxa, communities, or ecosystem functions. Restoration-project timelines often require that these models be developed rapidly and in conjunction with many external stakeholders or partners. Here, the Toolkit for interActive Modeling (TAM) is proposed as a platform for rapidly developing index-based models, particularly for US Army Corps of Engineers’ (USACE) ecosystem-restoration or mitigation planning processes. The TAM is a consistent quantitative framework that allows for development of a generic platform for index-based model development.

Salgının yayılımı açısından en kritik kamusal alanlardan biri olan okullarda sürecin nasıl yürütüleceği ve okullar açıkken salgının ne derece kontrol altında tutulabileceği konusu toplumun her kesiminde merak ve endişe uyandırmaktadır. Bu minvalde elinizdeki bu çalışmanın amacı Covid-19 salgını sürecinde kapalı kalan okulların yeni eğitim öğretim dönemine ne kadar hazır olduklarının değerlendirilmesi ve bu bağlamda politika önerilerinin ortaya konmasıdır. Bu kapsamda ilk olarak okulların mevcut fiziksel koşullarının sağlık önlemleri açısından yeterliği ele alınmıştır. Okulların tam kapasite ile yüz yüze yeniden açılması sürecinde yöneticileri, öğretmenleri, öğrencileri ve velileri ilgilendiren mevcut ve muhtemel problemler değerlendirilmiş ve çözüm önerilerine değinilmiştir. Community and neighborhood organizations are in the process of renewing the organizational culture, considering technological environments in the way of training, and advancing communally, being competitive in adaptation and learning, creating new solutions, promoting change, and altering the status quo, based on the advancement of technology over the last few years, currently applied in most organizations. The decisive factor is the ability of true leaders to appropriate the Technological Acceptance Model –TAM– principles, participating in programs and projects, adopting new technologies from the different actors involved, contributing to the welfare of each community. There is, however, a relative resistance to the use of technology as support in community management, due to the generational differences in leaders and dignitaries, according to collected reports in this study, in relation to the age range of dignitaries –Generation X and Baby Boomers predominate–. They present a challenge to digital inclusion with difficulties related to age, cognitive, sensory, difficulty in developing skills, and abilities required in Digital Technologies, necessary to face new scenarios post-pandemic and, in general, the need to use technological facilities.

In February 2020 the FSA published two reports which produced new estimates of foodborne norovirus cases. These were the ‘Norovirus Attribution Study’ (NoVAS study) (O’Brien et al., 2020) and the accompanying internal FSA technical review ‘Technical Report: Review of Quantitative Risk Assessment of foodborne norovirus transmission’ (NoVAS model review), (Food Standards Agency, 2020). The NoVAS study produced a Quantitative Microbiological Risk Assessment model (QMRA) to estimate foodborne norovirus. The NoVAS model review considered the impact of using alternative assumptions and other data sources on these estimates. From these two pieces of work, a revised estimate of foodborne norovirus was produced. The FSA has therefore updated its estimates of annual foodborne disease to include these new results and also to take account of more recent data related to other pathogens. The estimates produced include: •Estimates of GP presentations and hospital admissions for foodbornenorovirus based on the new estimates of cases. The NoVAS study onlyproduced estimates for cases. •Estimates of foodborne cases, GP presentations and hospital admissions for12 other pathogens •Estimates of unattributed cases of foodborne disease •Estimates of total foodborne disease from all pathogens Previous estimates An FSA funded research project ‘The second study of infectious intestinal disease in the community’, published in 2012 and referred to as the IID2 study (Tam et al., 2012), estimated that there were 17 million cases of infectious intestinal disease (IID) in 2009. These include illness caused by all sources, not just food. Of these 17 million cases, around 40% (around 7 million) could be attributed to 13 known pathogens. These pathogens included norovirus. The remaining 60% of cases (equivalent to 10 million cases) were unattributed cases. These are cases where the causal pathogen is unknown. Reasons for this include the causal pathogen was not tested for, the test was not sensitive enough to detect the causal pathogen or the pathogen is unknown to science. A second project ‘Costed extension to the second study of infectious intestinal disease in the community’, published in 2014 and known as IID2 extension (Tam, Larose and O’Brien, 2014), estimated that there were 566,000 cases of foodborne disease per year caused by the same 13 known pathogens. Although a proportion of the unattributed cases would also be due to food, no estimate was provided for this in the IID2 extension. New estimates We estimate that there were 2.4 million cases of foodborne disease in the UK in 2018 (95% credible intervals 1.8 million to 3.1 million), with 222,000 GP presentations (95% Cred. Int. 150,000 to 322,000) and 16,400 hospital admissions (95% Cred. Int. 11,200 to 26,000). Of the estimated 2.4 million cases, 0.9 million (95% Cred. Int. 0.7 million to 1.2 million) were from the 13 known pathogens included in the IID2 extension and 1.4 million1 (95% Cred. Int. 1.0 million to 2.0 million) for unattributed cases. Norovirus was the pathogen with the largest estimate with 383,000 cases a year. However, this estimate is within the 95% credible interval for Campylobacter of 127,000 to 571,000. The pathogen with the next highest number of cases was Clostridium perfringens with 85,000 (95% Cred. Int. 32,000 to 225,000). While the methodology used in the NoVAS study does not lend itself to producing credible intervals for cases of norovirus, this does not mean that there is no uncertainty in these estimates. There were a number of parameters used in the NoVAS study which, while based on the best science currently available, were acknowledged to have uncertain values. Sensitivity analysis undertaken as part of the study showed that changes to the values of these parameters could make big differences to the overall estimates. Campylobacter was estimated to have the most GP presentations with 43,000 (95% Cred. Int. 19,000 to 76,000) followed by norovirus with 17,000 (95% Cred. Int. 11,000 to 26,000) and Clostridium perfringens with 13,000 (95% Cred. Int. 6,000 to 29,000). For hospital admissions Campylobacter was estimated to have 3,500 (95% Cred. Int. 1,400 to 7,600), followed by norovirus 2,200 (95% Cred. Int. 1,500 to 3,100) and Salmonella with 2,100 admissions (95% Cred. Int. 400 to 9,900). As many of these credible intervals overlap, any ranking needs to be undertaken with caution. While the estimates provided in this report are for 2018 the methodology described can be applied to future years.
