Image-guided celiac plexus neurolysis can provide substantial and long-lasting pain relief in patients suffering from malignant pain from upper abdominal viscera. When performed by experienced hands, celiac plexus neurolysis also appears to be a relatively safe procedure with a limited side effect profile. Multiple imaging modalities are available for this procedure, though no single approach has systematically been proven superior in terms of efficacy or side effect profile. Each imaging guidance modality has advantages and disadvantages. Given the ability to visualize soft-tissue structures, CT guidance is recommended over fluoroscopy when intentionally transgressing into the retroperitoneum for celiac plexus neurolysis. It is also recommended in those patients with complicated anatomy, where anatomic distortion may complicate successful celiac blockade. However, in the patient without significant tumor burden involving the celiac axis and/or pancreatic body/tail, the fluoroscopy-guided retrocrural approach has been demonstrated to be efficacious, and complications are exceedingly rare.

Underwater navigation in turbid water for exploration in catastrophic conditions or navigation in confined unstructured environments is still a challenge for robotics. In these conditions, neither vision nor sonar can be used. Pursuing a bio-inspired approach in robotics, one can seek solutions in nature to solve this difficult problem. Several hundred fish species in families Gymnotidae and Mormyridae have developed an original sense well adapted to this situation: the electric sense. Gnathonemus petersii first polarizes its body with respect to an electric organ discharge located at the base of its tail and generates a dipolar electric field in its near surroundings. Then, using many transcutaneous electro-receptors distributed along its body, the fish “measures” the distortion of the electric field and infers an image of its surroundings. Understanding and implementing this bio-inspired sense offers the opportunity to enhance the navigation abilities of our underwater robots in confined spaces bathed by turbid waters.

The complexity of pain and addiction is a challenging clinical problem to address. Given these facts, an interface has been created in this chapter of 2 phenomenal approaches to pain focused in both the psychiatric and mechanistic pain models. The result is a well-rounded and comprehensive view on how to approach pain in the integrative format for patients with addiction. When to consider different conventional and integrative modalities is reviewed including their evidence base. The role of personality, pain perception, and cognitions are all examined. The full array of integrative approaches including mind-body interventions, guided imagery, CBT, hypnosis, spirituality, mindfulness and postural techniques, manipulation, yoga, Tai chi and TCM are all discussed.

From the earliest skeletal complement comprising violin, tabla and harmonium in silent films to the dynamic and expansive orchestral colour, timbre and styles of the twenty-first century, Tamil film orchestral sonorities have drawn global attention to the ‘Kollywood sound’. Music directors have adopted a cross-cultural approach to musical composition, enriching film background scores and song interludes while establishing film music’s distinctiveness as a musical genre. Of all the sub-periods in which European medieval history has been divided over time, the later middle ages is possibly the one on which the burden of past and current grand narratives weighs the most. Its chronological and geopolitical boundaries are in fact shaped by a heavy narrative of decline or transition, and consequently this period is often interpreted through the lenses of previous or following developments, becoming in turn the tail-end of the ‘feudal’, ‘communal’, ‘imperial versus papal’ era or the announcement of modernity. There is therefore an urgent need to revise and rewrite the story of the later Middle Ages, and in order to do so, to forge new critical and technical vocabularies not derived from the study of other periods. By adopting a conscious approach towards temporal and spatial variety, and by breaking the traditional and unitary narrative of decline and transition into one of many changes and continuities, this book charts the principal developments of late medieval Europe while opening up to different political cultures and societies, throwing new light on older concepts, and revealing analogies and differences with other geopolitical contexts. Including maps, illustrations, a detailed chronology and a rich range of reading suggestions, this book aims at providing a first introduction to a very complex, dynamic, and fascinating period for Europe and beyond.

Structural and functional changes in the aging lung and body predispose the older patient to pulmonary diseases such as asthma, chronic obstructive pulmonary disease, and chronic dyspnea. Pulmonary diseases of the aging population have a negative impact on morbidity and mortality and quality of life and increase the burden on health care costs. Pharmacological treatment remains the standard of care, but polypharmacy in the older patients can be an issue and has been associated with negative consequences. Integrative or nonpharmacological approaches can be used in conjunction with conventional therapy and can decrease dependence on pharmacological drugs. Integrative approaches include nutrition, breathing exercises, yoga, acupuncture, use of botanicals and supplements, tai chi, and mind–body therapy. This chapter examines the evidence for integrative or nonpharmacological approaches and provides recommendations for their use in pulmonary diseases of older adults.

This chapter is a complement to Chapter 15, concentrating on the non-pharmacological approaches to chronic pain. It features a discussion on the utility of mind-body therapies, psychosocial treatments, and technology-based therapies in the context of recovery through 12-Step programs and other mutual support groups. Such settings are commonly poorly receptive to medication management of either pain or addiction; the availability of other approaches can bridge the gap, leading to effective management of both. The therapies discussed include “movement” therapies, such as internal qi gong, tai chi, yoga, and martial arts. Healing touch, reiki, external qi gong, and acupuncture are examples of “energy” therapies, requiring an intercessor. Among the psychosocial treatments are motivational interviewing, cognitive restructuring, cognitive behavioral therapy, acceptance-based cognitive therapy, operant training, hypnosis, relaxation training, and mindfulness/meditation. Addressed as procedures are massage, chiropractic and osteopathic manipulations, trans-epidermal nerve stimulation (TENS), and transcranial magnetic stimulation (TMS).

This article focuses on flexible Bayes regression of epidemiologic data involving pregnancy outcomes. It first provides an overview of finite mixture models and nonparametric Bayes methods before discussing some of the possibilities focusing on gestational age at delivery, DDE and age data from the Longnecker et al. (2001) study. More specifically, it examines how risk of premature delivery is impacted by maternal exposure to the pesticide DDT. The results showcase the use of Bayesian analysis in epidemiological studies that collect continuous health outcomes data, and in which the scientific and clinical interest typically focuses on the relationships between exposures and risks of an abnormal response, corresponding to an observation in the tails of the distribution. The article also highlights the limitations of current standard approaches that can be overcome by means of Bayesian analysis using density regression, mixtures and nonparametric models, as developed and applied in this pregnancy outcome study.

Genetic conditions affecting the skeleton and supporting structures are individually rare and heterogeneous. This chapter presents an approach to assessing patients with suspected skeletal dysplasia, osteogenesis imperfecta, Marfan syndrome, and Ehlers–Danlos syndrome. Skeletal dysplasias are caused by abnormalities of bone growth and modelling; the commonest non-lethal type is achondroplasia, with an incidence of 1/10 000 to 1/30 000. The typical presentation of osteogenesis imperfecta is with multiple fractures, sometimes prenatally. There may be associated short stature, bone deformity, dentogenesis imperfecta, blue sclera, and hearing loss. Most patients with osteogenesis imperfecta have mutations in COL1A1 or COL1A2. Marfan syndrome is a connective tissue disease with a pattern of symptoms related to the presence of fibrillin in tissues. Typically, affected individuals are of tall, thin stature, with long fingers and toes (arachnodactyly), a pectus deformity, and scoliosis. Between 66% and 91% of individuals with Marfan syndrome have a mutation in fibrillin-1 (FBN1; locus: 15q21). All forms of Ehlers–Danlos syndrome present with variable thinning and fragility of skin, leading to easy bruising and poor scar formation. There is skin and joint laxity. In severe forms, blood vessels and internal organs are affected.

The problem with the definition of inertia was solved, in the simple case of free point particles, by Tait, who introduced the concept of inertial frame. Tait’s solution would have satisfied Leibniz’ request that inertia be determined dynamically, however it only works in the absence of interactions between the material bodies. Later Mach posed again the question of the origin of inertia, suggesting the idea that it should be dynamical, which was later dubbed ‘Mach’s principle’. Moreover Mach criticized also Newton’s absolute time, and introduced the basic idea of temporal relationalism, i.e. that time should be a concept that is abstracted from change and has no independent existence. This idea is at the basis of SD and many other relational approaches to physics. This chapter concludes with the Barbour–Bertotti formulation of Mach’s principle, which they called ‘Mach–Poincaré Principle’. This formulation removes the vagueness of Mach’s original idea, and puts the principle into a precise mathematical form, which is one of the basic axioms of SD.

La ricerca qui condotta si concentra sul tema del pregiudizio effettivo, per verificare se e come sia possibile adottare, nel sistema italiano, un simile parametro quando ci si appresti a stabilire se un atto processuale sia invalido. L’operazione appare a prima vista delicata, e aperta a forti critiche, aprendo il varco a modelli, sistemi di ragionamento e approcci distanti da quelli che fanno parte della nostra tradizione culturale. Si potrebbe sostenere, in senso radicalmente contrario, che compito della dottrina sia principalmente riaffermare i principi e metodi consolidati, senza accettare compromessi – spesso discutibili sul piano sistematico – con le evoluzioni proposte dalla casistica. Il rischio, tuttavia, e quello di finire per subire una trasformazione epocale invece di provare a governarla. Se non si vuole accettare d’esser relegati al solo ruolo di passiva testimonianza, occorre dunque provare a interferire con i mutamenti in atto, cercando di innestarvi razionalità: del resto, questa dovrebbe essere, da sempre, la ragione fondativa della dottrina, nell’ambito delle scienze sociali.
