Abstract:
Although the term juvenile idiopathic arthritis (JIA) encompasses a heterogeneous group of diseases, they all share a common pathological hallmark: inflammation of the synovium. Highly activated T cells, monocytes, and neutrophils are attracted to the joint and secrete mediators that not only perpetuate inflammation but also may attenuate immune regulation. In the oligoarticular and polyarticular forms of JIA, which are thought to be autoimmune conditions, dysregulated adaptive immunity is a likely factor in disease pathogenesis; the nature of the interactions between T effector (Teff) cells and T regulatory cells (Treg) is probably a key factor in controlling disease progression. Factors that affect the frequency and function of Tregs and/or the sensitivity of Teffs to mechanisms of immune suppression will therefore impact on the disease course. In the systemic form of JIA, however, dysregulation of innate immune pathways appears more central to disease pathogenesis resulting in augmented levels of interleukins IL-1β‎, IL-6, and IL-18. In the end, a final, common pathological pathway in JIA is the activation of monocytes and neutrophils, which are the principal mediators of joint inflammation and damage. This is supported by the fact that the therapies that have targeted innate cytokine pathways have shown greater success in the treatment of JIA.

Abstract:
The role of adaptive immunity (i.e. the involvement of B and T lymphocytes) in the pathogenesis of axial spondyloarthritis has been investigated in both human disease and relevant animal models. Studies of B cell responses have not generally implicated an autoantibody in the disease, but there are abnormalities of antibody responses, particularly increased titres of antibodies to various gut bacteria. T cells are critical to the disease in animal models other than those where overexpression of a cytokine is engineered, suggesting that they are the drivers of the inflammatory response. There is convergent evidence from animal models, genetics in humans, and direct observation of human peripheral blood and joints to implicate T cells producing IL-17 under the influence of IL-23. These in turn may be responding to bacteria either in the gut or on the skin.

Starting with the equation of motion for the field operator ψ(x,t) of an interacting many-particle system, the n-particle Green’s function (Gn) equation of motion is developed, with interparticle interactions generating an infinite chain of equations coupling it to (n+1)- and (n−1)-particle Green’s functions (Gn+1 and Gn−1, respectively). Particularly important are the one-particle Green’s function equation with its coupling to the two-particle Green’s function and the two-particle Green’s function equation with its coupling to the three-particle Green’s function. To develop solutions, it is necessary to introduce non-correlation decoupling procedures involving the Hartree and Hartree-Fock approximations for G2 in the G1 equation; and a similar factorization “ansatz” for G3 in the G2 equation, resulting in the Sum of Ladder Diagrams integral equation for G2, with multiple Born iterates and finite collisional lifetimes. Similar treatment of the G11-equation for the joint propagation of one-electron and one-hole subject to mutual Coulomb attraction leads to bound electron-hole exciton states having a discrete hydrogen like spectrum of energy eigenstates. Its role in single-particle propagation is also discussed in terms of one-electron self-energy Σ‎ and the T-matrix

This chapter focuses on the role of Harry T. Burleigh's wife and family in his career as an “eminent baritone.” Due to his success in singing for the English royal and noble families, Burleigh returned to perform in England the following summer, but it also led to his wife's determination to create an identity distinct from her role as the wife of “the eminent baritone.” In fall 1909 Louise took their son Alston to England, where she placed him at Malden College for Boys just outside London. Then, assuming the stage name of Princess Redfeather, she “played in her own Indian Act in London music halls.” After the “real” Princess Redfeather, Princess Tsianina Redfeather, appeared and demanded that Louise must find another stage name, Louise became Ojibway Princess Nadonis, and later Princess Nadonis Shawa. This chapter considers Louise Alston Burleigh's separation from Harry and her decision to pursue a career as a performer in New York City, with particular emphasis on her American Indian presentations and her joint recitals.

Psoriasis is an immune-mediated inflammatory disease (IMID) characterized by skin inflammation, epidermal hyperplasia, increased risk of arthritis, and cardiovascular morbidity. Substantial evidence indicates that psoriasis is driven by abnormal interactions between cells of the innate and adaptive host defence systems, including keratinocytes, dendritic cells, and T-cells, resulting in a dysregulated immune response and markedly increased epidermal proliferation. The precise aetiology of psoriasis remains unknown. Here, we review how innate and adaptive host defence responses are regulated by genetic factors that modulate the overall risk of psoriasis and dictate whether the disease affects the skin and/or the joints. Specifically, we review the epidemiologic basis for considering psoriasis as a genodermatosis, summarize knowledge derived from linkage and association studies of cutaneous psoriasis (PsC) and psoriatic arthritis (PsA), and attempt to relate genetic and immunologic discoveries in a pathogenetic framework that may eventually allow prediction of the development of PsA in psoriatic individuals.

Abstract:
This chapter provides a brief summary of the immune pathogenesis of atherosclerosis, highlighting shared features with inflammatory pathways in rheumatoid arthritis (RA) described in detail in Chapter 25.4. RA constitutes a prototype autoimmune disease primarily affecting the joints but also the heart and vessels associated with increased cardiovascular mortality. Recent years have produced a wealth of novel insights into the diversity of immune cell types which either propagate or dampen inflammation in atherogenesis. Expansion of this inherent anti-inflammatory component carried by regulatory T cells may constitute a new therapeutic target to harness the progression of atherosclerotic cardiovascular disease. Among the various inflammatory mediators involved in RA pathology, cytokines (tumour necrosis factor-α‎ and interleukin-6) have gained major interest as therapeutic targets with approved therapies available. In light of the many common features in the pathogenesis of RA and atherosclerosis, these biologics are currently being evaluated in cardiovascular patients. The recently published CANTOS trial showed that IL-1 inhibition reduced adverse cardiovascular events in patients with coronary artery disease demonstrating that inflammation is a genuine therapeutic target. The near future will provide more information whether inflammation is a bona fide cardiovascular risk factor based on completion of several clinical trials using anti-inflammatory approaches in patients with both cardiovascular disease and rheumatoid arthritis.

We tend to think that reason guides technological development, that engineering expertise alone determines the final form an invention takes. But if you look closely enough at the history of any invention, says Pool, you will find that factors unrelated to engineering seem to have an almost equal impact. In his wide-ranging volume, he traces developments in nuclear energy, automobiles, light bulbs, commercial electricity, and personal computers, to reveal that the ultimate shape of a technology often has as much to do with outside and unforeseen forces. For instance, Pool explores the reasons why steam-powered cars lost out to internal combustion engines. He shows that the Stanley Steamer was in many ways superior to the Model T--it set a land speed record in 1906 of more than 127 miles per hour, it had no transmission (and no transmission headaches), and it was simpler (one Stanley engine had only twenty-two moving parts) and quieter than a gas engine--but the steamers were killed off by factors that had little or nothing to do with their engineering merits, including the Stanley twins' lack of business acumen and an outbreak of hoof-and-mouth disease. Pool illuminates other aspects of technology as well. He traces how seemingly minor decisions made early along the path of development can have profound consequences further down the road, and perhaps most important, he argues that with the increasing complexity of our technological advances--from nuclear reactors to genetic engineering--the number of things that can go wrong multiplies, making it increasingly difficult to engineer risk out of the equation. Citing such catastrophes as Bhopal, Three Mile Island, the Exxon Valdez, the Challenger, and Chernobyl, he argues that is it time to rethink our approach to technology. The days are gone when machines were solely a product of larger-than-life inventors and hard-working engineers. Increasingly, technology will be a joint effort, with its design shaped not only by engineers and executives but also psychologists, political scientists, management theorists, risk specialists, regulators and courts, and the general public.
