Abstract:
Regional autonomy refers to the authority that regulates the community's interests or acts as a regulator in areas where the federal government empowers local governments to manage their own government. This research technique utilizes five book sources in order to learn about regional autonomy, its various forms, and legal foundations. Community engagement in governance, particularly at the village level, is one of the foundations of regional autonomy that requires attention in this regard. Concerning the village government system's role in development implementation, many village development initiatives have been conceived and determined primarily on considerations and techniques from above, without involving the community being developed.

Abstract:
Abstract Introduction: Despite recent advances in the treatment of osteoarthritis (OA), few studies have evaluated the longitudinal effect of physical modalities in functional capacity in patients with knee OA. Thereby, since the physical components and pain can affect the functional performance of daily activities, the effect of these treatment’s form is still to be established. Objective: Evaluate the effectiveness of therapeutic ultrasound, electrical stimulation and phototherapy in the functional performance, in patients with knee osteoarthritis. Methods: Articles present in the PubMed, Lilacs, SciELO and PEDro’s databases were evaluated. The used keywords were “pulsed ultrasound therapy”, “ultrasound therapy”, “electric stimulation” and “low level laser therapy” in combination with “knee osteoarthritis”. Were included in this presented review, randomized clinical studies using ultrasound, electrical and laser stimulation in subjects with knee osteoarthritis. To evaluate the methodological quality of the selected studies, was used the PEDro’s scale. The dependent variables of the study were: pain, physical function, joint stiffness, life quality and functional performance. Results: 268 studies were found, of these, 41 studies met eligibility criteria and were classified for analysis in full. The used methodology in the studies varied widely, however, in most cases there was improvement in functional performance of individuals with knee OA, with the use of physical modalities, for the pulsed ultrasound, continuous ultrasound, electrical stimulation and laser resourses. Conclusion: The physical modalities used in the studies demonstrated improvement in functional performance of individuals with knee OA.

Abstract:
Background: the nurse scheduling problem (NSP) is a serious and complex challenge in the medical industry. Effective, efficient, and high-quality services must be carried out by nurses in hospitals.Purpose: the goal of the systematic review was to find out how nurses have a positive impact on shift schedules on professional nurses in hospitals. Methods: employed the PRISMA approach. Electronic search engines from Proquest, Science Direct, and Ebsco were used to locate pertinent publications. Only English-language publications released between January 1, 2019 and December 31, 2021, had an inpatient hospital focus. Results: after identifying 265 references initially, duplicates were eliminated, and then authors from 6 pertinent papers were chosen. Overall, nurses reported that 12-hour shifts had a favorable impact on retention, including "the ability to retain experienced staff." Most nurses are flexible, don't get too tired, and will respond to patient requests like "better handoff" and "improvement of morale", Conclusion: Given that shift nurses are supposed to work about 37 hours per week and that 12-hour shifts are popular (i.e., three shifts of 12-hours). Nurse managers may think about taking action to assist nurses in reducing potential job stress brought on by the need to work shifts in order to maintain patient care. More investigation is required.

Abstract:
Abstract Introduction: The instrumental practice for a long time, the high performance level, the strict technique and the specific shape of each musical instrument can take musicians to overcome their physiological limits, giving a high prevalence of musculoskeletal injuries. Objective: Investigate the prevalence of temporomandibular joint disorder and neck pain in musicians. Methods: Between August and September 2015 were reviewed five databases: LILACS, SciELO, Medline / PubMed, Scopus and Web of Science. The articles were read and evaluated by the criteria of the Strengthening the Reporting of Observational Studies in Epidemiology (STROBE), items, that obtained a percentage above of 50 percent, were considered in the analysis of this work. Results: 15 articles attended the inclusion criteria. Among all musicians the prevalence of TMJ pain ranged from 10 - 81% and the prevalence of neck pain ranged from 29 - 80%. Conclusion: In this study was observed that the musicians showed both, temporomandibular joint disorders and neck pain, watching a high prevalence especially in violinists and the horn players. In the risk factors identified in the literature for the emergence of painful symptoms in musicians, stand out the biomechanical factors involved in maintaining anti-physiologic postures.

Shepherd, Emily Sarah. "Closing Research Gaps for Cerebral Palsy Prevention and Magnesium Sulphate Neuroprotection." Thesis, 2020. http://hdl.handle.net/2440/126062.

Full text
Abstract:
Background: Cerebral palsy (CP) is the leading cause of physical disability in childhood. Despite emerging evidence that the prevalence of CP has begun to decline, approximately one in 500 babies continue to be affected worldwide. While causes and risk factors for CP are well established, potential preventive interventions are under-researched. Aims: 1. To summarise and interpret the evidence regarding antenatal and intrapartum interventions for preventing CP. 2. To summarise and interpret the evidence regarding neonatal interventions for preventing CP. 3. To link data from a maternal perinatal randomised controlled trial (RCT) with a nationwide CP register to identify children with CP. 4. To assess whether antenatal magnesium sulphate is associated with perinatal death or other adverse neonatal outcomes. Methods: To achieve the above aims, the following methodologies were employed: 1. An overview of Cochrane reviews regarding antenatal and intrapartum interventions for CP prevention. 2. An overview of Cochrane reviews regarding neonatal interventions for CP prevention. 3. A de-identified linkage of Australasian Collaborative Trial of Magnesium Sulphate (ACTOMgSO4) and Australian Cerebral Palsy Register (ACPR) data. 4. A systematic review of RCTs and non-randomised studies assessing antenatal magnesium sulphate, perinatal death and other adverse neonatal outcomes. Results: 1. The overview of antenatal and intrapartum interventions included 15 Cochrane reviews, with CP data from 27 RCTs (32,490 children). Magnesium sulphate for women at risk of very preterm birth for fetal neuroprotection reduced CP risk (high-quality evidence). CP risk was probably increased (moderate-quality evidence, 2 reviews), probably not changed (moderate-quality evidence, 1 review), or unclear (low- to very low-quality evidence, 11 reviews) with other interventions assessed. 2. The overview of neonatal interventions included 43 Cochrane reviews, with CP data from 96 RCTs (15,885 children). Therapeutic hypothermia in late preterm or term neonates with hypoxic-ischaemic encephalopathy reduced CP risk (high-quality evidence), and prophylactic methylxanthines for endotracheal extubation in preterm neonates probably reduced CP risk (moderate-quality evidence). CP risk was probably increased (moderate-quality evidence, 2 reviews), probably not changed (moderate-quality evidence, 5 reviews), or unclear (low- to very low-quality evidence, 26 reviews) with other interventions assessed. 3. Linkage of data from 913 ACTOMgSO4 children (born 1996-2000) and the ACPR was achieved. Differences in ACTOMgSO4 (at 2 years) and ACPR (up to 5 years) CP diagnoses were identified; attributed to limitations in CP diagnostic methods, and register under-ascertainment in this era. 4. The systematic review of adverse neonatal outcomes included 40 RCTs (19,265 women and their babies), 138 non-randomised studies, and 19 case reports. Perinatal death was not increased with antenatal magnesium sulphate in RCTs. RCTs showed no clear increased risks of other adverse neonatal outcomes; non-randomised studies identified a limited number of outcomes necessitating further evaluation. Conclusions: Antenatal magnesium sulphate for fetal neuroprotection in women at risk of very preterm birth, and therapeutic hypothermia in late preterm or term neonates with hypoxic-ischaemic encephalopathy reduce the risk of CP. There is an urgent need for further research regarding the effects of other identified interventions on CP, and on strategies to assess CP following maternal perinatal RCTs.
