Abstract:
Disruption is about radical change—why it happens and how. Drawing on case studies ranging from the fourth century AD through the twentieth century, we look at how long-established systems of government and thought are challenged, how new institutions are created, and new ideas become powerful. While paying attention to the underlying political, intellectual, economic, and environmental sources of social disruption, we will see that no matter what similarities there might be between forces that shake different societies, these underlying factors do not dictate specific outcomes. The human actors are ultimately the most important; their decisions drive the conclusions that we see over time. Through our case studies, we can explore successful and unsuccessful decision making, and the emergence of the ideas that conditioned human actions. We’ll explore the development of Islam and of Christian doctrine, of constitutional thought, of socialism, and social Darwinism. We’ll look at how these ideas, all of them emerging on the fringes of society, became central. We’ll also have our eyes set on whether the sorts of disruptive forces we’ve seen in the past are present at this time. We’ll look at the issues confronting the liberal democracies that have been the dominant political/economic forces on our planet in the last half century and see how they have come under stress in the last few decades. And we will look at the possibility that we’re facing a new period of disruption and at what we can learn from the past about how change can be constructive rather than destructive.

This chapter covers the mechanism, classification, presenting signs, and basic principles of management for aortic dissection. Aortic disruption occurs when a tear develops in the aortic intima with passage of blood into the new space between the intima and media. A hypothetical patient presenting with signs of dissection is presented first, followed by sections covering classification systems of dissection, risk factors for dissection, and basic principles of management. Acute stabilization, diagnostic imaging, definitive management, and long-term follow up of patient management are discussed. In addition the dissection location and its implication on principles of management and outcome are covered. The chapter concludes with some discussion questions covering frequently encountered aspects of management touched on in the chapter.

The neuroendocrine system is the interface between the endocrine and nervous systems. This chapter presents an overview of the neuroendocrine system and endogenous hormones, with a primary focus on the hypothalamic-pituitary-gonadal (HPG) axis, the hypothalamic-pituitary-adrenal (HPA) axis and the hypothalamic-pituitary-thyroid axis (HPT). The importance of impacts of exogenous compounds, both natural and man-made, on the neuroendocrine system is discussed, with a focus on endocrine-disruptive actions of plant-derived phytoestrogens and the role of the aryl hydrocarbon receptor as an environmental sensor. The impacts of EDCs on feed-forward and negative feedback regulation of neuroendocrine functions, including those mediated by estrogen, androgen, and thyroid pathways, as well as other less studied pathways of hormonal signaling that involve disruption of neurosteroids, peptide hormones, and adrenal hormone signaling are also presented.

In recent years, the United Kingdom's Home Office has started using automated systems to make immigration decisions. These systems promise faster, more accurate, and cheaper decision-making, but in practice they have exposed people to distress, disruption, and even deportation. This book identifies a pattern of risky experimentation with automated systems in the Home Office. It analyses three recent case studies including: a voice recognition system used to detect fraud in English-language testing; an algorithm for identifying ‘risky’ visa applications; and automated decision-making in the EU Settlement Scheme. The book argues that a precautionary approach is essential to ensure that society benefits from government automation without exposing individuals to unacceptable risks.

This chapter turns to the regulation of work in the on-demand economy. To the extent that ‘disruption’ has become tech-speak for breaking the law, the time has come to ‘disrupt the disruptors’. On-demand gigs, tasks, and rides are work, rather than entrepreneurship—and should be recognized as such. This means that the industry needs to be brought within the scope of employment law. Legal systems across the world have learned to respond to employers’ attempts to mischaracterize work as independent entrepreneurship by focusing on the reality of the underlying relationship instead; in more complex multilateral scenarios, a further question crops up as to who should be the responsible employer. The time has come to rethink substantive employment law norms, from portable ratings to higher wages for non-guaranteed hours.

Many factors contribute to sleep disruption in critically ill patients. Sleep is a complex process, with broad effects on diverse physiologic systems. Environmental factors, such as light exposure, noise from diverse sources, and sleep interruptions related to patient care, have all received considerable investigational attention. Critical illness can affect elements involved in sleep initiation and maintenance. The various modes of mechanical ventilation may have different effects on sleep fragmentation and on the propensity to cause central apnoeas, thereby potentially prolonging the time on the ventilator. Pharmacologic agents, especially sedatives, can directly affect sleep architecture and may contribute to the incidence of intensive care unit delirium. Additional research is needed on the biological effects of critical illness on sleep, how sleep disruption affects systemic physiology and outcomes, and how these interactions can be modulated for therapeutic purposes.
