Abstract:
Conflict in marital relationships affects couple intimacy and satisfaction impacting on the couple system, the family system and wider social systems. Negative conflict is associated with breakdown in intimacy, marital dissatisfaction, and is seen to be a prominent cause of marital failure. As conflict is a process, it has the possibility of change and development over time, including moving towards a process whereby disagreements can be worked out with mutually acceptable solutions. Theoretical frameworks for conflict and marital counselling were reviewed and the systems-interactionist theory and a systemic-based counselling approach chosen for the purposes of this study. It was hypothesised that a systemic-based counselling approach would lower the levels of conflict occurring within a marital relationship. The principal objective of this study was to provide a more rigorous investigation than previous studies of client perceptions of the effectiveness of a systems-based counselling approach for marital counselling. Three case studies were conducted, with embedded units of analysis, within the context of a therapeutic process which drew on current theories of systemic counselling. Conflict was measured and operationalised as overt behavioural conflict, communication of negative affect, frequency of disagreements, intensity of disagreements, desire to change the other partner, the desire for change, and evidence of, and frequency of, positive conflict. As the study was based on the client's perceptions of change, multi-methods of selfreports were employed. As none of the known measurements met the needs of this study, a questionnaire known as the QCR was devised specifically for the pre- and posttests. The QCR was designed to measure: any increase or decrease in positive conflict; changes to both the intensity and frequency in negative conflict; and perceptions of the desire for change. The effectiveness of the counselling approach was measured by comparing the results of the pre-and post-tests with the continual self-reports and the self-reports at the long term follow-up interviews. An analysis of the self-reports focus on: the desire for change; the effectiveness of a systems based counselling approach in lowering negative conflict; and the usefulness of the QCR and other tools as measures. The systems-based-counselling approach, proved from the clients' perspective, to be: highly effective in case one; mostly effective in case two; and ineffective in case three as one partner aborted the counselling process. The systems-based-counselling approach had variable success in decreasing the intensity of negative conflict but was mostly successful in decreasing the frequency of negative conflict. This approach was not largely successful in reporting an increase in the frequency of positive conflict. The study revealed evidence of a relationship between commitment to the marital relationship and negative conflict. It also adds to the debate about the appropriateness of a systems-based counselling approach in violent relationships. In judging the clients' perceptions of the effectiveness of the systems-based counselling approach multi-measurements employed including the lengthy recording of the therapeutic sessions. It is claimed that this thesis offers a more rigorous methodology than anecdotal evidence previously used in the reporting of systemic counselling cases.

Abstract:
This study examined the effectiveness of a systemic cybernetic counselling approach with 3 females with bulimia nervosa. Bulimia nervosa is a relatively recent diagnosed condition (1980). Thus, little is known about the efficacy of different treatment approaches. The systemic cybernetic counselling approach (White; de Shazer) which informs family therapy represents a potentially powerful form of treatment in that it allows a complex construction of the problem. The research design employed a multiple (3) single case study approach with embedded units of analyses. The 3 women, aged 17 to 27, were seen over a 2-3 month period for 4-8 one hour sessions. Predicted patterns of non-equivalent dependent variables were compared with empirically based patterns over time. Continuous (including pre-, post-treatment and long-term follow up) assessment of frequency of bingeing/purging was established as well as ratings on other dependent variables - psychometric measures (Eating Disorders Inventory, Beck Depression Inventory, Coopersmith Self Esteem Inventory), affective self reports and reports by others. Results show that all three clients eliminated bingeing/purging by post-treatment. Two clients maintained this improvement on all dependent measures at long-term follow up. The most marked improvements were associated with the least severe pre-treatment scores. In the case where the whole family attended counselling sessions, the number of sessions was reduced. On the basis of the results, systemic cybernetic counselling procedures hold promise for the successful treatment of bulimia nervosa.

Abstract:
The current models that we use in systemic family therapy came out of office/clinic-based practice. To date, there is no model specifically orientated to systemic family therapy in the home. As a systemic family therapist, I argue that non-traditional approaches may need to be considered; and that systemic family therapy models should come closer to reflecting discourses that have shades of global influences. My interest in the area emerged from a position of ignorance – making assumptions that the tools used in the clinic could easily be colonised into a family’s home – but I found that the models often used in the clinic do not necessarily transfer easily into the home. an adaptation of a systematic review was conducted that undermined the notion that therapists are ‘knowing’ with particular skill and competency to work in the home. I ask the question: How do I improve upon my systemic family therapy practice to work in families’ homes? African oral traditional ideas (AOTI) are broadly explored to consider the notion of self and bodily feelings as a source of knowledge. Through the use of AOTI I created an approach known as Seselelame, foregrounding a new practice stemming from ideas that are not home grown within the systemic family therapy perspectives, to support my practice within the home. the inquiry offers the following contribution of new knowledge to family systemic therapy: conceptualization of a method (Seselelame) that incorporates the idea of self in the context of awareness of feelings in the body; a method that incorporates African oral traditional ideas and thus expanded the traditional Western view of family/systemic therapy; contextualization of the significance of home as a source of knowledge; the Seselelame model was used as an analytical tool alongside a systemic constructionist analytical model to compare and contrast the data produced. The findings conclude that the inquiry has implications for the practice and teaching of systemic family therapy, which will eventually be published once the thesis is completed.

Abstract:
This thesis explores a dialogical approach – in relation to supervision, therapy and research. I have as supervisor inquired into my relationship with groups of supervisees who were training to become family therapists or systemic practitioners. Through my doctoral portfolio, I speak from within my practice and I show in some detail the micro processes in relational encounters which help dialogue to evolve. I also address grand narratives about what it means to be a human being, and show how perceiving a human being as dialogical has extensive and governing consequences for how we think about a person’s movements in the world, how we think about them as person, in relation to other people, and how we understand problems, and approach problem solving. My research has been a doing, an experiencing and a creation of knowing in a reflexive flow. My research philosophy, mode of approaching my practice as therapist and supervisor (and as a person in the world) has reflexively been created through my being in practice. I show how an embodied belief in fluidity and complexity, enables me as supervisor to contribute to a space in the context of supervision which welcomes the freedom of a kind of orientation which is open towards situated, emerging, novel and provisional understanding. By attending to here-and-now interactions, becoming answerable in the moment and by embracing intuition, ambiguity and relational compassion, we have been able to welcome risk-taking and improvisation. This mode of dialogical supervision demonstrates a willingness to spontaneously dive into the uniqueness of every new encounter and every new movement. I see this as the poetics of the dialogical meeting. I have experienced how this space has opened up quite unexpected aspects of the supervisees’ experiences and has served as an incitement for them to question different aspects of their relationship to life. This has reflexively created a certain spirit and atmosphere that has invited us all to be bolder in our sharing and exploration of our lives, practice and our ideas. This thesis makes a contribution concerning: how we can be with people in ways that opens up more understanding and creates a sense of belonging and liberation; challenging and transgressively exploring discursive boundaries which attempt to define and fix what research is, what therapy is, what supervision is, and welcomes the infinity of opportunities and possibilities life may offer us. Thus I suggest that it may become significant for the profession to review the usefulness and legitimacy of distinct categorization between therapy and supervision. Through my choice of genre I offer the reader a possibility to respond emotionally as well as intellectually to my writing. I believe the way I have chosen to re-present my research through a mix of genre and evocative texts not ‘frozen’ findings, permits and anticipates novel ways of going on in relation to research in a manner that I don’t believe have been described in this way before within the community of family therapy and systemic practice.

Abstract:
Although I was trained as a nurse, I became curious about the views of psychiatric nursing staff as to what they feel would create a clinical learning environment for them. This was as a result of having my dearly beloved father admitted to a medical ward, and being on the receiving side of care. This created a fear in me that surprised me. I was scared at how my father would be treated if I was not present to witness his care. As a nurse, strangely, I felt that I could not trust the nurses to provide safe clinical care for/with my father. I began to feel concerned about the competencies of nurses as I observed the care that was being offered to him. My observation and experience was that the ward environment generated an atmosphere that needed a form of nursing care that was collaborative and transparent where the hand-over between professionals communicated the needs of the patients in their care – from making sure that patients’ bedding is comfortable, to checking whether they are trying to communicate something, to being sure that their medication has been properly given. This aroused my curiosity as to whether nursing staff themselves had views as to what is needed to have, or to create, an environment that sustained their original urge to take up nursing in the first place. I thus became curious to investigate the views and experiences of both qualified and non-qualified psychiatric nurses with the aim of improving the clinical learning environment within acute adult inpatient wards, as well as secure adult and male adolescent mental health inpatient wards, as these wards raise crucial issues to do with control, power, seclusion, rights and responsibilities, issues that are not easy for nurses to learn to deal with in their classroom training. For this study I interviewed sixteen staff members of wards within the National Health Service and the private sector. The staff varied in their experiences and qualification, from qualified mental health nurses to non-qualified nurses. My findings show that: (1) Nurses often felt the ward organisation had hindered their learning through the way in which it worked to organise them. (2) Nurses would have liked to experience a different kind of learning. However they were not sure in what way or how they would like the learning experience to be. (3) The expert nurses were able to work in a competent manner despite the sense of the organisation organising their practices, as they were able to sense which of the limited number of organisational possibilities were open to them so that their choices allowed for their practices to be learning experiences as well as providing sound clinical care. (4) Learning dialogues happen in contexts where nurses feel supported and where the episode of care in which they are engaged is also supported by a team approach and resourceful pulling together of skills and abilities. (5) There was a lack of space(s) for the nurses to use for reflection. (6) Nurses also expressed the need for supervision after an episode or critical event had taken place. (7) Throughout all the areas I inquired into, what was strongly echoed was that the psychiatric nurses all felt that they needed a voice within the organisation and its hierarchy of team structures within these wards. There was a felt sense that the nurses wanted and at times needed more expert nurses working in the teams. Overall, I was struck by the abilities which were brought to the forefront as the nurses shared their views and experiences of how they felt organised by the organisation. They were able to explore the factors that they felt would improve the quality of care that nurses provide and were able to share what they believe will help them in co-creating standards for how the clinical environment could become a learning environment for the nurses.

Abstract:
This document is an invite to join me on a journey that follows the path of bereaved children, adolescents and their families who attended seven bereavement groups within a mental health setting. This thesis sets out to illustrate how families hold and maintain grief within the family system and how effective a bereavement group is as a form of therapeutic intervention. The bereavement group is a platform where families bring their private stories into a public domain and talk about the concerns that they have seen in their families since the bereavement. As an insider/outsider researcher I am of the opinion that by talking, listening and sharing their stories with other families with similar experiences within the groups, families can begin to think about what could be done differently if they want something to change within their family system. By sharing, families learn how to go with their grief without their loved ones and begin to create new narratives about the next part of their journey. The group can provide an opportunity for families to hear the ‘Untold’ stories and begin to create new narratives within their family system. The bereavement group also acts as a lens that allows me to look into my practice as a family therapist within a social constructionist framework and make new meaning of the stories that families bring within the bereavement groups. A tapestry is created from the complex diverse stories of grief that are interwoven with each family that attends the bereavement group. Each family brings their own pattern of bereavement and creates new patterns as their experience is shared with other families. Data is taken from the conversations at the assessment, treatment and follow up stages to highlight what difficulties the families have when there is bereavement. This is to ascertain what factors may be contributing to holding and maintaining the grief in the family and whether a bereavement group is effective in bringing the issues out for all family members to discuss. Different methods are used to deconstruct the different themes and unpick the ‘Told’ stories. At the end of this journey my hope is that there is more awareness about the effect of bereavement and how it shows itself in children’s mental health and how the family system can be affected.

Chapter I starts with the concept of childhood and traces the history of child care and examines the present situation of pastoral care and counselling. The importance and necessity of pastoral care and counselling of children is emphasized. Chapter 2 locates pastoral care and counselling of children under the umbrella of Practical Theolob'Y· It proposes an interdisciplinary model and argues th<:t pastoral care is a theological issue. In chapter 3 the ancient Israelite society is examined 11s an example of a therapeutic environment for growing children. Her.~ the family provided a social structure through which children experienced a strong sense of belonging, security, love and self-identity. In the family circle God's covenantal love was conveyed by means of storyte.lling. Children have their own world and language which differ in many ways from the adult world. Chapter 4 explores the personal world and language of children and gives an exposition of the different developmental stages between the ages of six and twelve. Chapter 5 deals with the world of children, the family and the immediate environment of growing children. Healing in pastoral care is exercised through faith care. The research proposes the storytelling method as a most effective vehicle to convey God's love to the child. Chapier six explains the necessity for a storytelling technique through which the horizon of the child's environment merges with the horizon of God's unfailing love and grace.

Abstract:
In the South African public health care system, HIV Counselling and Testing (HCT) has become a function that is routinely entrusted to lay counsellors. These counsellors are expected to educate clients about HIV and AIDS, encourage them to be tested and convince them to change risky behavioural practices. They have to convey the clients’ test results and assist those who test HIV-positive and their families to cope with the psychological challenges associated with the diagnosis. The counsellors occupy the front line of HIV and AIDS service delivery, even though they are not formally employed in the health care system. They only have basic training and are not adequately remunerated. The counsellors are confronted with psychological and structural stressors in their work. Psychological stressors include the impact of emotionally challenging work, the lack of appropriate training, debriefing and supervision. Because they are not formally employed in the health care system, there is a lack of formal supervision or channels to discuss their frustrations. This research focuses on the stressors which HIV counsellors experience, how they cope, and the impact it has on their psychological well-being. As part of the mixed methods approach 50 HCT counsellors working at the City of Tshwane clinics completed the Bar-On Emotional Quotient Inventory, the Maslach Burnout Inventory for Human Services Survey (MBI- HSS), the Centre for Epidemiologic Studies Depression scale (CES-D), and the Brief COPE scale to assess their psychological well-being. In addition, they participated in focus group discussions. EQ-i scores indicated that counsellors reported below average emotional skills, with the overall group score of (88.76). Scores that indicated average emotional skills were Self-Regard (101), Interpersonal Relationships (100.12), and Impulse Control (102.66). Scores that indicated low emotional skills were Independence (86.66), Self-Actualization (88.28), and Reality Testing (83.94). Although they reported high levels of Emotional Exhaustion (27.66), they also have a sense of high Personal Accomplishment (38.64) (MBI-HSS). Counsellors reported an overall CES-D score which was indicative of mild depression (26.08). Counsellors used mostly positive coping skills that included religion, planning, and direct action in coping with stressors in their lives. In a regression analysis with depression as the dependent variable, there was a positive relationship between depression and depersonalization and a negative relationship with positive or active coping and assertiveness. It was concluded that counsellors experienced some depression, emotional exhaustion, and lower than average levels of emotional well-being. Despite that, they reported positive ways of coping and high levels of personal accomplishment. Counsellors were motivated by their sense of altruism, compassion towards their clients, the positive changes they see in client’s lives as well as the reciprocal relationships they have formed with their clients. Counsellors thus have strengths to cope with the high level of stressors and challenges related to their work. It can be concluded that their state of mental health is in line with Keyes’ proposal that mental health forms a continuum. They fluctuate between mental well-being and mental ill-being. However, it is necessary to assist HCT counsellors to develop their emotional capacities to enable them to enhance their ability to counsel their clients effectively.

Abstract:
Harmful drinking adversely affects the drinkers’ health, social, and occupational functioning, leads to immense societal costs, and causes a significant burden on global health. Despite this, policy makers, and consequently health systems, are disproportionately focused on providing care for dependent drinking, the more severe end of the alcohol use disorders spectrum. Consequently there is a large treatment gap for harmful drinking, especially in lowS and middleSincome countries where only a small proportion of those with harmful drinking receive appropriate psychosocial care. Two major barriers to providing care for harmful drinking are the shortage of adequately skilled specialist human resources and the lack of contextually appropriate psychosocial treatments suitable for low resource settings. The aim of my thesis is to describe the systematic process used to develop Counselling for Alcohol Problems (CAP), a brief psychosocial intervention for delivery by lay counsellors in routine primary care settings to men with harmful drinking in India. In my thesis I have described each step of the intervention development process which is broadly divided three sequential stages viz (i) identifying potential psychosocial intervention strategiesZ (ii) developing a theoretical framework for the new interventionZ and (iii) evaluating the acceptability, feasibility, and preliminary impact of the new intervention. These three stages have nine steps that utilise a range of quantitative and qualitative participatory research methods to achieve the aims of the study. The outcome of the intervention development process is CAP a threeSphase treatment delivered over one to four sessions based on a Motivational Interviewing stance and includes other strategies as follows: assessment and personalised feedback, family engagement, drink refusal skills, skills to address drinking urges, problemSsolving skills and handling difficult emotions, and relapse prevention and management. To conclude, the outputs of the intervention development process were a) a contextually appropriate brief psychosocial intervention for harmful drinking designed to overcome specialist manpower shortages as it can be delivered by lay counsellors in primary care, and b) a structured framework to guide the development of contextually appropriate psychosocial interventions in low resource settings.

Sammanfattning

Syftet med denna uppsats var att ge en bild av hur ett antal studie- och karriärvägledare inom universitet/högskola förhåller sig till begreppet breddad rekrytering.

Uppsatsen ville också visa vad en högskola och ett universitet har för strategier för att bredda sin rekrytering och om man medvetet använder sig av studie- och karriärvägledning för detta ändamål.

Metoden var kvalitativ och resultaten byggde på intervjuer av tio studievägledare fördelade på en högskola och ett universitet, samt två personer som arbetar övergripande med breddad rekrytering på dessa lärosäten. Det som framkom är att studievägledarna överlag anser att breddad rekrytering är en viktig uppgift som ingår i deras yrkesverksamhet, vidare att vägledningen är mer psykologiskt än socialt inriktad, på så sätt att vägledarna fokuserar mer på psykologiska aspekter som t ex studentens intressen, värderingar och förmågor snarare än den sociala kontext studenten befinner sig i t ex klasstillhörighet eller genus. Det finns inslag av processtänkande men fortfarande mycket av "trait and factor" tänkande. Högskolan/ universitetet anser att studievägledning är en viktig verksamhet och använder sig delvis av den för att bredda sin rekrytering, men kunskapen om vad vägledning är och kan användas till känns delvis outvecklad.

En framtidsvision är att vägledning har mer av samhällsförbättrande karaktär. Vägledaren skulle då vara en person som uppmuntrar till val utanför normen: Gränsöverskridande val.


 

Abstract

The purpose of this thesis was to give a picture of how a number of student counsellors at the university look on their commission in the perspective of widening recruitment.

The thesis also showed what strategies the universities have to widen their recruitment and of how to use counselling for this purpose.

The method was qualitative and the result was founded on interviews with ten student counsellors from two different universities and from two persons who work overall with widening recruitment and diversity. This study shows that student counsellors regard widening recruitment as an important task in their work. It emerged under the interviews that counselling is more psychological than social, which means that the counsellors focus more on factors like interests, values and competence rather than social context with factors like social class and gender. There are elements of trait and factor thinking as well as a great deal of process thinking. The university regards student counselling as an important activity and is using it for widening recruitment, but the knowledge of  it and its use is somewhat undeveloped.

När specialpedagogen använder handledning som verktyg söker hon åstadkomma förändring genom att ge pedagogerna råd. Fokus i samtalet riktas framförallt på barnet och på pedagogens arbete. Specialpedagogen kan ses som en slags expert på området barn i svårigheter och den pedagogiska praktiken. När specialpedagogen använder handledning som rum för reflektion söker hon åstadkomma förändring genom att ställa frågor för att stimulera pedagogens tänkande och stödja hennes utforskande av hur problemet kan förstås. Specialpedagogen kan då ses som en slags expert på att föra reflekterande samtal och samtalets fokus riktas huvudsakligen mot pedagogen. Specialpedagogernas repertoar innehåller därmed såväl förmåga att handleda genom att ge pedagogerna råd med utgångspunkt i det enskilda barnet som att stimulera pedagogernas reflekterande över aspekter av sitt arbete.

Avhandlings syfte är att undersöka hur elva specialpedagoger beskriver och genomför handledning med pedagoger i förskola och grundskola. Intresset riktas mot samtalens innehåll och mot de påverkansprocesser som äger rum i samtalet. Handledningssamtalen studeras ur specialpedagogens perspektiv.

Specialpedagogers handledningssamtal kan bidra till att vidga synen på objektet för den specialpedagogiska verksamheten såväl som befästa ett individperspektiv på området. I denna studie dominerar användningen av handledning som verktyg. Utmaningen förefaller ligga i att söka skapa villkor som även gynnar användningen av handledning som rum för reflektion.

This dissertation is a report on research into the development and evaluation of a career assessment and counselling procedure that falls under the aegis of the constructivist, narrative approach: My Career Chapter: A Dialogical Autobiography. My Career Chapter enables an individual to construct a holistic understanding of his or her career. The procedure facilitates an individual writing and reflecting on an autobiographical account of his or her career that is contextualised amidst systems of career influences. The resulting autobiographical text can be used in career counselling, including co-constructive dialogue between client and counsellor. The literature underpinning the research project is described with a wide-ranging discussion of issues that critically pertain to the research endeavour and essentially provide a primary base for the work. Two theoretical frameworks that exemplify constructivism in vocational psychology underpin the research: the Systems Theory Framework and the Theory of Career Construction. From the base of those two theoretical frameworks, narrative career counselling is explicated and exemplars are described. The Theory of Dialogical Self is introduced to inform the design of My Career Chapter and, ultimately, the theory and practice of narrative career counselling. The research is predominantly positioned within a paradigm of constructivism/interpretivism and the results of the studies are collectively interpreted accordingly; but postpositivism and critical ideological paradigms are present in a secondary form due to the mixture of research methods used in the project as a whole. Six empirical studies investigate the experience of My Career Chapter from the perspective of the developer, the counsellor-user, and the client-user; each explicated with two studies respectively. Research methods include autoethnography for the developer's experience, interpretative phenomenological analysis and focus group for the counsellor-users' experience, and quasi-experiment and interpretative phenomenological analysis for the client-users' experience. The studies of the developer's experience of My Career Chapter comprehensively explicate how and why the procedure was developed and emphasise the importance of reflexive science and practice. Crucially, the autoethnographies revealed a nexus of theory-practice-person which underpins the production of My Career Chapter, and critically influences the entire research project. The studies involving counsellor-users affirmed My Career Chapter's alignment with recommendations for the development and application of qualitative career assessment and counselling procedures. These studies also raised questions pertaining to the characteristics of client-users that may mediate the efficacy of the procedure (e.g., age, language ability). Studies of client-users firstly support the conclusion that My Career Chapter is a safe career assessment and counselling procedure, with minimal attendant risk of inducing psychological harm or distress. The procedure was experienced as being helpful as a tool for personal reflection, through its theoretically-derived processes of facilitating clients writing, reading, and hearing and talking their autobiographical manuscripts through in the interpretation phase. There are four dimensions of significance associated with this research project. Firstly, the divide between theory and practice has indeed been much lamented in vocational psychology and counselling psychology. Thus, the overall significance of the research reported upon in this dissertation is significant because it attempts to bring theory and practice together through a reflexive and theoretically informed research process into a career assessment and counselling procedure. Secondly, the research and development process produced a new career assessment and counselling product which will add to the limited range of techniques that fall under the aegis of constructivist career assessment and counselling broadly, and the narrative approach specifically. My Career Chapter complements other procedures. Thirdly, two of the research methods used in the project (viz., autoethnography and interpretative phenomenological analysis) demonstrated their potential as additional qualitative methods for research within vocational psychology. Finally, the research process has enabled the articulation of the Theory of Dialogical Self—from another branch of psychology—into the extant corpus of literature on career development theory and practice.

This research report is a narrative on my encounter with sport psychology. I used the metaphors of “touring” and “journal” to guide the construction of this narrative. Before embarking on my tour with sport psychology, I introduced myself as researcher (chapter 2, journal entry I) and stated the research problem, question and goal(s)(chapter 3, journal entry II). I then went on a tour with sport psychology. I visited a sport psychology museum to learn more about the history of sport psychology (chapter 4, journal entry III) and a sport psychology library to learn more about the field of sport psychology (chapter 5, journal entry IV). I also attended a sport psychology conference to become better acquainted with postgraduate sport psychology training in Psychology Departments (chapter 6, journal entry V). I also visited a sport psychology department at a university in Australia, to witness the delivery of a postgraduate training programme in sport psychology (chapter 7, journal entry VI), as well as a sport centre in South Africa and a sport institute in Australia, to see how and the extent to which sport psychology was practised (chapter 8, journal entry VII). After my tour with sport psychology I took what I had learnt and adapted the MA (Counselling Psychology) programme at the University of Pretoria, South Africa. This led to the programme having three core modules and two elective modules. The core modules are fundamental psychology, counselling psychology and career psychology. The elective modules are: community psychology and sport psychology. The sport psychology module consists of the following courses: sport psychology, sport management, sport sociology and exercise psychology. The Professional Board of Psychology at the Health Professions Council of South Africa approved this module at the end of 2003 (chapter 9, journal entry VIII). Finally, I also looked at the influence of my experience with sport psychology on my identity as a psychologist (chapter 10, journal entry IX).

This study focuses on educational psychology students’ self-efficacy beliefs in career counselling. The research was conducted in the Department of Educational Psychology of the University of Pretoria. An Interactive Qualitative Analysis (IQA) as described by Northcutt and McCoy (2004) was done. The constituency comprised seven students who have provided career counselling to at least one client. The IQA approach to research postulate that the ideal in qualitative research is to create a process that invite participants to produce data while minimizing the effect of the research process on the content. With the IQA approach the participants in the research have a remarkable degree of freedom and are empowered to generate and analyze their own data with minimal external influence. The role of the researcher in this process is to facilitate the process and to teach participants the process while guiding them to generate and analyze their own data. The central assumption in IQA, namely that the people closest to the phenomenon are in the best position to report about the phenomenon was confirmed by the researcher. The main findings of the study indicate that self-efficacy develops through formal and personal development. The identification of possible areas of growth and how this is communicated by the supervisor is fundamental for both formal and personal development processes of students. A positive cooperative developmental approach during feedback sessions is conducive to the growth of the career counsellor. The importance of experiential learning and modelling as a means to develop self-efficacy was confirmed by the research participants. The reduction of anxiety with the increase of self-efficacy was reported as a very important benefit of increased self-efficacy.

Literature shows that cataracts are the leading cause of blindness globally and nationally. Retinal detachment has also been a substantial problem both globally and nationally. Both of these conditions are prevalent in patients of 50 years and older. The treatment for both conditions is for surgery to be performed. In the Western Cape the three leading hospitals do not have ophthalmic pre-operative and post-operative protocols.Review question:What are the best practices to manage pre-operative and post-operative nursing care in patients waiting for cataract and retinal detachment surgery? bjectives:1. To determine the best practice in pre-operative and post-operative care in patients who have undergone cataract and/or retinal detachment surgery regarding: health education offered by nurses, counselling to prevent psychological effects, and positioning to prevent physical complications. 2. To develop a framework based on systematic reviews for pre-operative and post-operative ophthalmic nursing care in South Africa. Methodology: A systematic review using the guide by the Centre for Reviews and Dissemination was done, and studies were identified by searching various electronic databases and visually scanning reference lists from the relevant studies. Studies that were included were evidence-based. All study types were considered and the studies were selected based on the title and, where available, the abstract. These were then assessed against the inclusion criteria. A narrative synthesis was used. Finally the evidence was summarised and a framework was drawn up, focusing on pre-operative and post-operative nursing care for cataract and retinal detachment surgery

The study investigates counselling interventions used by staff at the Centre for Guidance and Counselling (CGC) from 1977 to 2006. Both location and situation of the University of Zululand (Unizul) in the rural north coast of KwaZulu-Natal, where poverty and political intolerance once reigned, presents unique challenges to students and staff, particularly the Student Representative Council (SRC) and Management. A number of significant discoveries were made as revealed by research techniques used i.e. narratives, selected qualitative and quantitative research methods and case studies. After the 1976 national riots and communication breakdown between students and management, the University established a department to serve as a communication conduit between the two structures (Bengu, 2005). It is with that background that the Student Services Department (SSD) was conceived at Unizul. In spite of interruptions and difficulties from one period to another, there is evidence of hard work from previous headships. Unforeseen circumstances in each case prevented formal hand over. It is only recently, in 2003 that Unizul began to make provisions for change management strategy. For the purposes of this study a survey on student experiences on campus life was conducted. From randomly selected members of the university community it is concluded that, all is not well with student life at Unizul. Further, attitudes of students and staff in Student Services Department (SSD) were explored as they impact on service delivery (Ludeman, 1998; 1997; Worthen, 1987). The genera! view is that problems of de-motivation and academic indifference, political intolerance and domination, mob spirit and group-think are excessive between and amongst students. Student politics rules student affairs. Executive appointments in sports, societies and clubs are dominated by political placements of individuals that serve interests of political groupings. Over the years, the operations of the SRC, Student Parliament and Senate, All Sports and Student Housing Committees have taken political stands. By accommodating and sometimes nursing political squabbles of students, management and staff are perceived to be aligned to one of two main political rivals in the country. SSD is faced with an ongoing responsibility of diffusing tensions to establish a healthy working relationship amongst students, and also between student governing structures and management. The University is faced with challenges of normalizing conditions to enable effective teaching and learning to take place. A systematic, multi-pronged approach to deal with problems of political intolerance and domination, poverty and academic indifference is recommended. As problems will not vanish on their own, a need for an effective, well-thought management strategy and psychological intervention are strongly recommended.

Integrating psychology and spirituality to open up discussion on spiritual identity and its effects on the whole person was birthed in a counselling context, as individuals sought answers for various problems, having an effect on their psychological and spiritual well-being. The body, soul and spirit approach unfolded, as we explored their identity, more specifically, their spiritual identity, thus integrating psychology and spirituality. The aim was to explore how the discovery of their spiritual identity had an effect on them holistically. I chose a qualitative research design because my research questions required the collection and analysis of rich, in-depth data regarding participants’ psychological and spiritual journeys (Phipps, & Vorster, 2011; Ryan, 2006). My data collection method was twofold, using both in-depth interviews (narrative storytelling), as the initial stage for the individual to tell their story and the effects on their psychological and spiritual well-being, as well as semi-structured interviews (brainstorming), as the second stage in the research. The objective was to see what effects the problem had on them as a whole person. I used two stages of data analysis to reach this objective namely a collaborative deconstruction technique, together with the individual as the first stage and secondly a thematic analysis to interpret the main messages, patterns that repeated, as well as the highlights, having an effect on them as a whole. The results confirmed that individuals “discover” their spiritual identity when their self-identity reaches a limit of coping with problems and have more positive effects on them as whole persons. The significance of the research is that it has contributed to a more integrated counselling approach, within psychology, for counsellors and psychologists, to explore spiritual identity with the individual. Through the integration of seeing individuals as whole beings, including a spiritual dimension, awareness was created within the counselling context of the value of seeing individuals in a more integrative and holistic manner. Such a psycho-spiritual integrative approach is more relevant in the field of counselling in journeying with individuals in wholeness and affecting their dimensions of body, soul and spirit positively in the context of identity.

The thesis deals with the use of Omaha system in lactation counselling. At present time the care of newborns realized through general practitioners is minimal. Therefore, the connection of community nursing related to shorter time of hospitalization of both mother and her newborn child is vital. Lactation counselling is closely connected to the obligatory documentation of provided care. Omaha system is considered as a suitable standardized terminology for community nursing. The thesis dealt with the analysis of Omaha system, lactation counselling, the issue of breastfeeding as a whole and the legislation related to nursing documentation in the theoretical part. The main aim of the research is the implementation of Omaha system in lactation counselling. Three main goals were set in the thesis. The first one was to map the differences between the care provided by lactation counsellors with and without medical qualification. The second goal was to describe the way of administration of breastfeeding mothers by the counsellors. The third goal was to elaborate and evaluate the documentation according to Omaha system in the field of lactation counselling provided by counselors with a qualification. To achieve the goals of this thesis quantitative-qualitative method of data collection was used. The method of questioning through a non-standardized questionnaire was selected for quantitative research. The qualitative part of the research was based on the use of semi-structured in-depth interview method. The third part of the research is represented by the content analysis of Omaha system. The qualitative part of the research was processed with open coding method. The research question was answered through the analysis of the interviews. The respondents in this research are represented by lactation counselors. The first research file is represented by counselors both with and without qualification. The second research file includes counselors with medical qualification only. The questionnaire research was realized through an electronic questionnaire. The interviews were conducted with six lactation counselors with a medical qualification. The questionnaire research shows differences in the counselling provided by the counselors with and without a qualification which supports H1. The official documentation for counselling is used by a few respondents and mainly by the ones with a qualification. The good thing is that most counselors record data about mothers and breastfeeding. Next, it seems that the unofficial documentation of counselors includes problems and solutions description. The results show that H2 has not been supported. However, it is good that almost half of the respondents are interested in using a standardized documentation for counselling. Next, the counselors with a qualification do not use standardized taxonomy of nursing problems. The results show that only two counselors with a qualification have a contract with an insurance company. Most of them do not cooperate with a podiatrist or gynecologist. The most troubling issues for them are lack of time and missing contracts with an insurance company. Most of them think that longer hospitalization of mothers does not have a better effect on the quality of breastfeeding. Moreover, the interviews with the counselors with a medical qualification show that the newly created documentation of Omaha system has been processed thoroughly and clearly. They consider it more time demanding, which could be a problem because of the lack of time caused by the financial situation of mothers or because of insurance companies. The output of tis thesis is a newly created documentation for lactation counselors and lactation centers based on Omaha system which allows a more effective way of providing lactation counselling.

The professional development of a career counselling intern on a satellite campus at a distance education institution was facilitated with the use of a heutagogic learning strategy. The heutagogic learning process was recorded by reflective writing based on Kolb's experiential learning model. This research is a disquisition of the reflective dataset. The research is an autoethnographic case study in the constructionist paradigm with a creative analysis process. During the research process features about the benefits of heutagogic learning in the professional development of the career counselling intern were critically constructed. The findings of the research are captured in memescapes showing mindset changes and mental transformations on patterns which describe the theory-praxis gap, diversity, wellness, the macro-ethic. The recommendations include the hope that these findings will feed through to inform future career counselling internships in the IOP field.

One of the goals of cognitive rehabilitation following traumatic brain injury is to help people perform everyday tasks. However, options for the rigorous assessment of everyday cognitive effectiveness after rehabilitation are limited. Performance on neuropsychological tests is only moderately correlated with everyday functioning, while previous measures of everyday functioning include only fairly general estimates of overall cognitive functioning. The aim of the current study was to develop an ecologically valid measure that captured a number of subdomains of executive functioning, using systematic behavioural observation of an everyday task. The initial phase of the research involved identifying an everyday task that was sufficiently complex to ensure that executive functioning was utilised in the completion of the task. Participants with traumatic brain injury were then asked to prepare chocolate brownies, using a recipe provided, and a hot drink. Participants were allowed to use any compensatory strategy to help complete the task. Participant performance was directly observed by an examiner and videotaped for subsequent inter-rater reliability. Two independent raters assessed nine components of executive functioning. During this phase, the examiner manuals were modified improving inter-rater reliability. The final version of the measure was then trialled with participants with and without traumatic brain injury. Final inter-rater reliability indicated the approach had merit. Significant and moderate correlations were found between traditional measures of executive functioning and the everyday task. This study employed systematic behavioural observation to obtain fine-grained information regarding a person’s cognitive functioning. With further development, this approach may prove useful for targeting and monitoring specific functional difficulties during cognitive rehabilitation.

The research study investigates the trauma experienced by Metrorail train drivers. An Employee Assistance Programme (EAP) is a worksite-based programme that assists in dealing with employee’s problems that impact on their job performance and overall production. Trauma awareness and management through therapy are necessary components and contribute to the effective operation of EAP. The study was conducted within the framework of a survey. Face-to-face scheduled interviews were designed and conducted on employees, i.e. Metrorail train drivers, after their respective supervisors had given permission. From the feedback on these interviews, it was apparent that train drivers need the help of EAP services, maybe through psychological counselling and motivation. Witnessing suicides by people on rail tracks apparently is a common occurrence to Metrorail train drivers. These accidents leave them emotionally scarred and haunted for almost their entire lives. EAP services will embrace other elements influencing the performance and occupational well-being of these train drivers. These elements include performance incentives, fringe benefits, security, and conducive train settings.

Training in psychotherapy needs to involve more than merely skills and techniques. The field of psychotherapy needs to involve more than the application of formulas in a modernistic framework. This study proposes an approach to training in psychotherapy where the emphasis is on the client and the therapeutic relationship. Psychotherapy training should be an ongoing process and this study aims to facilitate and enhance that process for students. The study combines basic therapeutic skills and interpersonal psychotherapy to form a training programme referred to as the Basic Relational Skills Training (BRST) programme. The first part of the study provides a historical perspective of basic therapeutic skills, followed by a review of the relevant literature. The second part of the study constitutes a possible format for a training manual for the proposed BRST programme. The researcher hopes that the proposed BRST programme be utilised by trainers to assess the programme's efficacy.

OBJECTIVE/ METHOD The study examined implementation challenges faced by primary health care workers within the Ekurhuleni Metropolitan Municipality in Gauteng South Africa. Data collection was based on semi-structured interviews carried out on a purposive sample (n=19) of frontline clinicians working within the district as primary health care practitioners. RESULTS Participants confirmed that work within the primary health care service disproportionately focussed on curative and rehabilitative functions of their roles with little prioritisation of preventive and promotive interventions. Primary identified reasons included, institutional culture that prioritised short-term curative approaches. Clinicians also cited a range of other organisational barriers, such as – poor strategic planning, and a lack of understanding of health promotion and illness prevention. CONCLUSIONS Although the challenges that exist in implementing primary health care are clearly understood, clinicians perceive the solutions for these as being within the control of policy makers and those with power within the organisation.

The aim of this study was to describe the nurses‘ perception of termination of pregnancy and her perception of her conduct towards clients that request termination of pregnancy guided by Kings‘ Systems Approach and Goal Attainment Theory. A non-experimental quantitative descriptive correlational design was used. A non-probability purposive sampling method was used consisting of institutions in the Western Cape, available to participate in this study, which deliver termination of pregnancy services, gov ernment health care institutions (21) and Marie Stopes clinics (3). Non-probability convenience sampling was used consisting of registered nurses who are working in termination of pregnancy units in these facilities (N=82). Data were collected through the administration of a self-designed questionnaire. The main findings of the study indicate that respondents (personal system) appear to feel positive about the work they are doing. Formal improvement and development are however not utilised optimally. Regarding the interpersonal system, respondents also show a positive orientation to both the woman opting for termination of pregnancy and the foetus. The potential towards transaction and ultimately goal attainment reside within this system. The respondents are also positive about legislation, employers and policies (social system) relating to termination of pregnancy. This orientation creates a milieu that is conducive to transactions, interactions and goal attainment.The main recommendation is that three systems as proposed by King be continuously integrated within the termination of pregnancy setup through, amongst other measures: values clarification, management support, in service education, the inclusion of termination of pregnancy in undergraduate programmes, focus groups and the like.
Die doel van hierdie studie was om die verpleegkundige se persepsie van terminasie van swangerskap en haar persepsie van haar houding en gedrag teenoor kliёnte wat terminasie van swangerskap versoek, aan die hand van King se Sisteembenadering en Doelwitbereikingsteorie te ondersoek en te beskryf. `n Nie-eksperimentele kwantitatiewe beskrywende korrelasie navorsingsontwerp was gebruik. `n Nie-waarskynlike doelbewuste-steekproefneming van die staatsgesondheidsorg-instansies wat terminasie van swangerskapdienste aanbied, is geneem en wat beskikbaar vir hierdie studie was (21) asook die Marie Stopesdienste (3), in die Wes-Kaap. `n Nie-waarskynlike gerieflikheidsteekproefneming is van verpleegkundiges geneem wat swangerskappe in hierdie afdelings termineer of moet termineer (N=82). Data is met behulp van `n selfontwerpte vraelys ingesamel. Die hoofbevindings van die studie dui daarop dat respondente (persoonlike sisteem) positief voel oor hul werk. Formele ontwikkelingshulpbronne word egter nie optimaal deur die sisteem benut nie. Wat die interpersoonlike sisteem betref, toon respondente ook ‗n positiewe oriëntasie tot beide die vrou wat terminasie van swangerskap aanvra en die fetus. Die potensiaal tot transaksie-sluiting en uiteindelike doelwitbereiking is binne hierdie sisteem geleë. Die respondente is ook positief oor wetgewing en die werkgewer en beleid (sosiale siteem) ten opsigte van terminasie van swangerskap. Hierdie oriëntering laat ‗n milieu wat bevorderlik is vir transaksie-sluiting, interaksie en doelwitbereiking. Die hoofaanbeveling is dat voortdurend gepoog moet word na die integrasie van King se drie sisteme binne die terminasie van swangerskapafdelings deur onder meer: waarde-uitklaringswerkswinkels, ondersteuning van bestuur, indiensopleiding, die insluiting van terminasie van swangerskap in voorgraadse kurrikula, fokusgroepe en so meer.

Abstract:
The purpose of this study was to analyse the support systems for refugees in Southern Africa with specific reference to the Republic of Botswana. Qualitative framework as described by Lofland and Lofland (1984), Schensus and Schensus (1992) was used to conduct the investigation. Interviews were conducted with thirty refugees who currently living in Botswana as a refugee or asylum seeker. Focus group discussion was also held with twenty-six refugee workers. Interview findings were derived using Glaser and Straus' (1976) and Van Maanen, (1979) constant comparative method of qualitative analysis and were grouped into four major categories. Among the most significant findings were that the subjects agreed that on paper and by design, there are structures for providing the different services to refugees but refugees are not provided with adequate services. The second finding is that the support systems for refugees in Botswana are more focused on the provision of material support with little attention given to the psychosocial needs of the refugees. The third finding is that the Botswana government withheld some of the Articles of the 1951 UN refugee Convention, which deal with the socio-economic rights of refugees in Botswana. The fourth finding is that refugee workers need specialised training to enable them to address a wide rage of psychosocial issues affecting refugees. Last major finding is that there is no established clear system of service delivery in the participating agencies. The researcher concluded that because of trauma and stress experienced by refugees and refugee workers, there is a need to improve on the psychosocial support provided to refugees and refugee workers in Botswana by improving the knowledge and skills of refugee workers and promoting refugee participation. The researcher recommends two urgent actions that should be taken. First, the refugee management in Botswana need to improve on its service quality control mechanism, including evaluating its legal and operational framework. Second, psychosocial components need to be integrated into every aspect of the refugee programmes. This will support recovery for the many traumatised refugees and refugee workers in Botswana.

Dazu wurden Fokusgruppendiskussionen mit Teams aus der ambulanten Palliativversorgung in Nordhessen durchgeführt und diese nach ihren Erfahrungen und Beobachtungen gefragt. Theoretische Grundlagen zur Spiritualität im Kontext der Palliativversorgung bilden den Rahmen zur Auswertung der empirischen Masterarbeit mittels qualitativer Inhaltsanalyse. Der Befund zeigt, dass das Erkennen von spirituellen Bedürfnissen bei Patienten die Teammitglieder mehrheitlich vor Herausforderungen stellt und multiple Verunsicherung sowie Probleme im Umgang bestehen. Außerdem prägen persönliche Überzeugungen ihre Wahrnehmung maßgeblich. Die beobachtete Spiritualität ist überwiegend kirchlich geprägt und beruht auf christlichen Elementen wie Gebete, Glauben oder Begleitung durch PfarrerInnen. Teammitglieder sprechen zudem Begegnungen, „Dasein“ und (Kirchen)Musik eine spirituelle Dimension zu. Es gibt auch Patienten, die keine spirituellen Bedürfnisse haben oder derartiges ablehnen. Theoretical foundations of spirituality and palliative care formed the framework for the evaluation of empirical research by using qualitative content analysis. Findings show that recognition of spiritual needs in patients poses a challenge to the majority of team members, as well as insecurities and problems in dealing with them. The observed spirituality is predominantly ecclesiastical and based on Christian elements such as prayers, faith and accompaniment by pastors. Team members address encounters, "being" and music as spiritual dimensions. There are also patients who have no spiritual needs. In future, practical theology could serve as a needs-based provision of competences, so that employees of health systems can be empowered to perceive, reflect on and adequately respond to the spiritual needs of their patients.

Students in Technical and Vocational Education and Training Colleges (TVET) in South Africa face a number of challenges including psychological problems. These challenges have a strong bearing on the academic success of the students. The aim of the study was to examine the effectiveness of psychological support services rendered to students at Majuba TVET College. Interviews were recorded verbatim and transcribed. Thematic data analysis was used to analyse the data. The themes used were derived from the research objectives and these were further divided into categories and sub-categories. The study found that the institution does provide psychological support services to the learner. The forms of psychological support include career counselling, pre-counselling, referral system, on-going counselling and peer counselling and mentoring. Improved academic performance, improved student attendance student retention and improved college certification rate are some of the benefits that can be realised from the effective utilisation of psychological support by the students. Various challenges hinder the effective utilisation of these psychological support services including, lack of qualified personnel, being understaffed, poor referral system, conflicting roles of staff, inadequate on-site counselling, low-prioritisation of psychological support by college management and limited access time. The participants highlighted the need for the institution to establish linkages with key stakeholders such as other TVET Colleges so as to have a platform for sharing vital information on psychological support. The participants were of the view that more qualified and registered staff should be hired to bolster the provision of psychological support to students. The referral system should be improved and the participants advocated for the services to be accessible around the clock. College management should show commitment to students’ welfare by prioritising and recognising student psychological support as pivotal to students’ academic success.
