Abstract:
Vigorous and prolonged physical exercise can produce a range of urinary abnormalities which would normally be considered alarming. They include haematuria, haemoglobinuria, the appearance in urine of red cells in urine, some fragmented in a ‘glomerular’ manner, red cell cast formation, and proteinuria. A variety of names have been given to these syndromes, including march haematuria and march haemoglobinuria. Mostly these changes seem benign and self-limiting. Rarely they are associated with acute kidney injury but this is often in the context of other renal insults, extreme dehydration, or hyperpyrexic conditions. Vigorous exercise is also commonly associated with various electrolyte changes related to both over- and under-hydration. These can complicate assessment. Transient proteinuria in the absence of haematuria appears to be a physiological response to even short-term exercise, its degree related to the intensity of the exercise. Causation of these syndromes is mixed and not fully explained. There is good evidence for physical trauma to red cells being a significant part, but this cannot explain the appearance of glomerular red cells and red cell casts. Exercise-related changes mostly resolve within less than a day, and almost all by 72 hours.

Abstract:
Influenza viruses are a significant cause of morbidity and mortality globally, resulting in severe illness in 3-5 million people and death in up to 500,000 during epidemic years. In March 2009, a novel H1N1 virus emerged in Mexico, spreading rapidly around the globe and achieving pandemic status within 3 months. Although it is now generally considered that the 2009 pandemic resulted in mild disease in most individuals, serious complications still occurred, with 12,000 deaths by mid-February 2010 in the United States alone. Risk factors for severe disease included asthma, cardiac disease, immunosuppression, pregnancy, diabetes mellitus, and obesity. The chapter outlines the case of a young pregnant female who presented with an influenza-like illness and subsequently developed acute respiratory distress syndrome requiring extracorporeal membrane oxygenation. The origins, presentation, diagnosis, complications, and management of pandemic influenza are discussed, in addition to a summary of the pulmonary physiology and pathology of pregnancy.

Abstract:
Enteral nutrition (EN) is an integral part of the patient care in the intensive care unit (ICU) in order to maintain gut integrity, to modulate stress and the systemic immune response, and to attenuate disease severity. The timing of commencing EN in critically-ill patients depends on patient status and should be initiated as soon as the patient is stabilized. The energy and protein targets should be estimated and the feed prescription should match the nutritional target. The rate of EN dose increment or the addition of supplemental parenteral nutrition (PN) to reach the nutritional target is still debatable and ranges between 3 days (ESPEN approach) and up to 8 days (ASPEN approach). Micronutrients should be supplemented to all patients. The role of pharmaconutrition is controversial due to recent negative trials, but the use of EN with supplemental omega-3 and GLA for acute respiratory distress syndrome patients is still advocated by ESPEN and ASPEN guidelines.

Phosphatase and Tensin homolog deleted on chromosome 10 (PTEN) is a gene encoding an intracellular signaling molecule. PTEN was originally discovered as the gene responsible for a subset of familial hamartoma (tumor) syndromes associated with increased risk for certain cancers (Nelen et al., 1997) and as a gene often mutated in human cancers and tumor cell lines (Li et al., 1997; Steck et al., 1997). More recently, mutations in PTEN have been linked genetically to the clinical phenotype of autism or developmental delay with macrocephaly (Boccone et al., 2006; Butler et al., 2005; Buxbaum et al., 2007; Goffin, Hoefsloot, Bosgoed, Swillen, & Fryns, 2001; Herman, Butter, et al., 2007; McBride et al., 2010; Orrico et al., 2009; Stein, Elias, Saenz, Pickler, & Reynolds, 2010; Varga, Pastore, Prior, Herman, & McBride, 2009; Zori, Marsh, Graham, Marliss, & Eng, 1998). This chapter examines the role of PTEN in intracellular signaling, the link between PTEN signaling pathways and other autism-related genes and signaling pathways, the genetic relationship between PTEN and autism, model systems in which effects of Pten deletion on the brain have been studied, and promising preclinical data identifying therapeutic targets for patients with autism/macrocephaly associated with PTEN mutations.
