Full textPhysical activity and sedentary behaviors are major health determinants and are being surveyed worldwide. Objective: The aim of this thesis was to contribute to the surveillance and measurement of physical activity and sedentary behaviors. Method: This thesis includes four studies. The first study analyzes and discusses the present situation of French national surveillance studies. The second study presents the results from the first French report card on physical activity for children and adolescents. The third study discusses the validity and reliability properties of the French version of the Global Physical Activity Questionnaire (GPAQ). The fourth study examines the content of questionnaires measuring sedentary behaviors. Results: Studies 1 and 2 reported measurement issues impairing the overall data quality, inter-study comparisons and survey of changes over time. The study of GPAQ reported limited but acceptable reliability and validity for the measurement of physical activity and sedentary time in France. Finally, the systematic review on sedentary behaviors questionnaires reported large differences in the population targeted (ex: adults, youth, elderly), recall frame (ex: previous day, last 7 days, last month), the number of item (from 1 to more than 100), and the sedentary behaviors characteristics measured (from only sitting time to up to 27 sedentary behaviors). Conclusion: The work realized during this thesis allows the formulation of recommendations to improve the surveillance of physical activity and sedentary behaviors in France

Full textState surveillance of the poor has a long history. It begins several centuries ago, more precisely with the creation of modern nation-states. According to social theorists, the strengthening of the state’s surveillance capabilities results from both mass bureaucracy and democracy. It also stems from people’s desire to assert their civil rights. This thesis examines state supervision from a welfare recipient point of view. It aims to identify recipients’ rationales and practices of resistance. We interviewed 19 participants from both cities of Montreal and Quebec City. Results suggest the extent of resistance at work: two different resistance rationales, as well as six tactical moves emerged from our analysis.

Full textThe management of missing values is a common and widespread problem in epidemiology. The most common technique used restricts the data analysis to subjects with complete information on variables of interest, which can reducesubstantially statistical power and precision and may also result in biased estimates.This thesis investigates the application of multiple imputation methods to manage missing values in epidemiological studies and surveillance systems for infectious diseases. Study designs to which multiple imputation was applied were diverse: a risk analysis of HIV transmission through blood transfusion, a case-control study on risk factors for ampylobacter infection, and a capture-recapture study to estimate the number of new HIV diagnoses among children. We then performed multiple imputation analysis on data of a surveillance system for chronic hepatitis C (HCV) to assess risk factors of severe liver disease among HCV infected patients who reported drug use. Within this study on HCV, we proposedguidelines to apply a sensitivity analysis in order to test the multiple imputation underlying hypotheses. Finally, we describe how we elaborated and applied an ongoing multiple imputation process of the French national HIV surveillance database, evaluated and attempted to validate multiple imputation procedures.Based on these practical applications, we worked out a strategy to handle missing data in surveillance data base, including the thorough examination of the incomplete database, the building of the imputation model, and the procedure to validate imputation models and examine underlying multiple imputation hypotheses

Full textIn the wings of the deportation from french territory, between surveillance and assistance : Ethnographic exploration in a French Immigration Detention CentreIn a context of increasing control of irregular immigration and the tight management of borders, in what manner, according to what terms, by which directives and which applications, does the incarceration of foreigners pending to be expulsed from French territory participate in the preparation of this expulsion? This thesis deals with the management of the confinement of foreigners in a French Immigration Detention Centre, according to two central aspects, monitoring and care, and she focuses, through the practices of police and health, on the role of this imprisonment in the implementation of expulsion. It fits in the anthropology of the State and the police work and in those of the sociology of immigration and of the places of confinement. It shows how different forms of directives, specific to the confinement of foreigners, are embodied in agents whose practices are the result both of direct applications of these guidelines and especially their facilities, specific to the management of contradictions, arising from this place. This thesis focuses first on police practices fixed around surveillance which, under the management of contradictions, proper to the retention, becomes by a shift to a job of expectation and anticipation, in forms ranging from expectantly looking for information to local strategies of constraint and conditioning. It then focuses on the practices of health, by showing how the confinement and deportation issues come to shape them, in a profound way, to the point that they are involved in the process of selection and differentiation. Finally she deals with this comprehensive preparatory work, to the effects of the relationship between the police and health professionals, in the management of an incarcerated population, held by the threat of expulsion. This thesis renders, in sum, different practices, and different political and moral positions of the agents that are working in a control system, administered by the State, that are faced in their work to contradictions and moral conflicts. This work is the fruit of an ethnographic investigation conducted within a French Immigration Detention Centre and targeted interviews and observations with actors belonging to places on which the French Immigration Detention Centre is dependent

Full textLes EDS constituent un bon indicateur du niveau des épidémies à l'échelle national et régional, voir infrarégional grâce au recours à des techniques d'analyse spatiale en composantes d'échelles. Mais leur fréquence est inadaptée pour mesurer les évolutions à court terme. La surveillance sentinelle, estimateur local des ordres de grandeur, peut être un indicateur de tendances sous certaines conditions. Cependant, si la situation actuelle commence à être mieux connue, une mesure réelle des dynamiques est encore hors de notre portée.

Full textThe objective of this work was to analyze abuse, dependence and diversion of psychoactive medicines in real settings using jointly different indicators issued from mixed datasources in order to present a synthetic vision. The datasources used in this work are issued from the tools developed by the Centres for Evaluation and Information on Pharmacodependency (CEIP). They allow to measure directly drug abuse with specific populations of dependent patients or under opiate treatment (OPPIDUM (Observation of the Illicit Psychotropic Products or Diverted from their Medicinal Use) survey)). These tools also allow to measure the diversion via the measure of the phenomenon of “doctor shopping” (overlapping of prescriptions) and the measure of the number of patients presenting a deviant behaviour from general health insurance databases; then they measure diversion through falsified prescriptions presented at pharmacies (the OSIAP (Forged prescriptions indicating potential abuse) survey).This multisources approach has been firstly applied to analyse abuse and diversion of clonazepam (1st publication). This study has highlighted the emerging problem of diversion of clonazepam, after flunitrazepam and has also illustrated the difficulty of analysing with consistency the information gathered by these different datasources. A good system for controlling drug diversion and abuse has to allow analysing trends. We have so proposed a classification method aiming at revealing profile of subjects with deviant behaviour to use it on an evolutive manner so as to study diversion of methylphenidate on a four year period (2nd publication). This classification method has then been applied jointly with a method measuring the “doctor shopping” to analyse diversion of High Dosage Buprenorphine (HDB) (3rd publication). This study has revealed an important problem of diversion of HDB, has also demonstrated that the two methods were globally concordant and has allowed to evaluate their advantages for the controlling of the abuse and diversion of prescription drugs. These two last methods have then been analysed jointly with data from the OPPIDUM and OSIAP surveys to allow to study and compare diversion of benzodiazepine drugs (4th publication) and opioids drugs (5th publication). This multisource approach allows to limit biases linked to each method seen individually. Our work points out the relevance of such a multisources system to estimate the abuse of a prescription drug and to compare it with the other substances. Nevertheless, the development of such a system applied to the domain of the drug dependency is relatively new, and requires improvements concerning the integration of the other sources of data and the methodology used to join and synthetize the information obtained. Finally, such a system "multi-sources” has the potential to exist and to make a real contribution to the domain of the drug dependency in France

Full textThe problem of deportation or deportees from West Africa during the colonial era is not sufficiently addressed by French- and even English-speaking African historiography, which has focused more on wars, resistances and their different forms. In doing so, a reality of a part of colonial history remains more or less unknown. That is why we would like to study the following theme, which has been and remains of burning topicality: "The deportees of Senegambia and Sudan: between resistance and repression in a colonial space from 1840 to 1946". The actors in this story of the deportees are emblematic figures and/or simple anonymous, who wanted to defend the land of their ancestors, direct the destinies of their peoples, fight for the maintenance of African values and traditions. The history of "these soldiers of refusal" – namely religious leaders, fighters in the service of Islam and ancestral values or beliefs and political leaders to which are added the mentally insane, social bandits and delinquents, men of the press, supporters and/or followers of leaders and even Senegalese riflemen – deserves to be examined. This thesis is part of the questions of a colonial history attentive to the issues of repression and the maintenance of order. Faced with the manifest refusal of the leaders of troops or creators of emotions to resign themselves to the colonial diktat, the response given by the colonial authorities was, among other things, to deport/imprison them, to house arrest, to prohibit them from staying, to cut them off all forms of communication, any contact with their entourage and thus put them out of harm's way. In many cases, it was a form of imprisonment, which leads us to the study of the prison environment that reveals the forms of avoidance, the living conditions of the deportees, the architecture related to security issues, etc. The application of this technique of repression, part of the logic of security policies, was a way of slowing down the momentum of the leaders and annihilating all colonial resistance. The study we wish to conduct aims above all to identify the decisive place of deportation in the system of colonial repression, in the maintenance of security order, political control, control of people and spaces, for the exploitation of colonies. The chronological framework that this work attempts to illuminate goes from 1840 to 1946, a pivotal period in colonial history in West Africa, particularly in Senegambia and Sudan, in that it is marked by rapid transformations at all levels (political, economic, social and cultural). Was deportation so fundamental, so necessary for the realization of the colonial project, the maintenance of security order? To what extent did the deportees constitute a real obstacle, an obstacle to the establishment and imposition of colonial power? What was the role of law enforcement actors in the deportation process? This thesis explores major themes such as the contexts of deportation, the abuse of power by colonial administrators, indigénat and indigenous justice, the motivations of deportation, the multiple responses of indigenous people, their arrest and deportation, the place of agents/actors (army, gendarmerie and colonial police) in maintaining, restoring and/or protecting stability and the politico-economic consequences of such a "technique of power"
