ObjectiveTo report the results of a global survey on cataract practice patterns related to preoperative, intraoperative and postoperative care, surgical setting and personnel allocation.Methods and analysisAn online 28 questions survey was sent to 240 ophthalmologists asking to describe prevailing trends in their institutions across 38 countries and 5 continents. Questions inquired country, institution, surgical volume and setting, anaesthesia, preoperative and intraoperative examination and postsurgical care. Statistical analysis used crosstabs lambda statistics for non-parametric nominal variables. P value less than 0.05 was considered statistically significant.Results209/240 (87%) ophthalmologists responded: 38% representing public hospitals, 36% private practices and 26% academic sites; overall surgical volume was between 241 700 and 410 500 cataracts per year. There was a significant correlation between type of institution and surgical volume. Complete results available in online (https://freeonlinesurveys.com/r/W6BcLLxy).ConclusionCataract surgery related patterns of perioperative care showed significant difference among respondents, regardless to type of institution, surgical volume and country. Many evidence-based procedures are unevenly practiced around the world and some widespread and expensive habits lack solid scientific evidence while consuming enormous amount of resources both monetary and human. There is a need to reach consensus and share evidence-based practice patterns.

Abstract:
Purpose: As compared to other countries, the practice of General Surgery in Pakistan has gone through a series of important development over the last 30 years, despite major variable training models. There increasing concern is that the current teaching of General Surgery residency does not hold out enough skills that are essential to practice the scope of General Surgery in Pakistan. Procedure: Executed by a National Force on the General Surgery Future, this project directed to advance guidance on the favorable configuration of Training on General Surgery in Multan. A consecutive series of studies based on evidence and a survey was introduced to inform these guidelines. Results: The general results of a procedure in any study show the complexity of the practice in General Surgery. 1 Location matters a lot for a surgeon to practice surgery 2 General surgery accompanies a strong grip for the overall clinical competence as well 3 Today general surgeons experience a subspecialized training too 4 Writing a proper report and guidelines for proper future surgical goals is also a part of training for General surgeons today To be a general surgeon in the 21st century, these are the four recommendations that are necessary to maintain. Conclusion: This project mainly refers to the importance of location for fresh general surgery graduates, emphasizing the impact of variable location in contrast with the implementation of the same objectives of the surgery. The results of this study were sent to the Nishtar College to review the requirements of training and the work on a more precise and comfortable working scheme. Hence, we ensured that the GSRTP General Surgery Residency and Training Programs could work for the wider range of practice all around Pakistan for better training practices.

Ethical behaviour is integral to medical practice. Society rightfully trusts surgeons to have ethical standards above those of the wider community. By and large the profession has repaid that societal trust by upholding such standards. However, the growing influence of social media on marketing and self-promotion, and the inappropriate prioritisation of entrepreneurial business practices, means we are now experiencing a shift away from patient-centered decision-making to outcomes that favour surgeons.

Abstract:
Objective: The objective of this study was to evaluate current percutaneous foot surgery practice among Brazilian specialists. Methods: A cross-sectional observational study was conducted, surveying members of the Brazilian Foot and Ankle Society (ABTPé) by emailing electronic questionnaires in 2017 and 2019. The information requested included demographic data and the profile of their practice with relation to percutaneous foot surgery. A total of 74 participants completed the survey questionnaire in 2017 and 82 in 2019 (response rates of 15 and 14% respectively). Results: A total of 49 participants in 2017 (65.33%) and 57 in 2019 (69.51%) were performing percutaneous foot surgery. Among respondents who were not performing percutaneous foot surgery, 15 in 2017 (57.69%) and 10 in 2019 (40%) stated that they believed in the method, but had not been trained to perform it. Exclusively spinal anesthesia was used by 19 surgeons in 2017 (38.77%) and 23 in 2019 (40.35%). When correcting Hallux Valgus, 13 surgeons in 2017 (26.53%) and 3 in 2019 (5%) did not use any type of fixation. The most frequently reported complication was poor reduction in both periods, reported by 36 (73.46%) participants in 2017 and 39 (68.42%) in 2019. Conclusion: In Brazil, a lack of specific training is one factor that limits the practice of percutaneous foot surgery. The technique is used by a greater number of younger surgeons. The most frequent complication is poor reduction and there is a growing trend to employee fixation hardware. Level of Evidence V; Expert Opinion.
