This chapter describes the different modalities for assessment of systolic function by transthoracic echocardiography. Firstly, the basic principles of physiology and the determinants of left ventricular (LV) performance are considered, followed by a systematic appraisal of the methodologies for global LV systolic function assessment. Starting with M-mode echocardiography, passing through the traditional two-dimensional echocardiography evaluation to three-dimensional echocardiography approaches, main strengths and limitations are described. Power Doppler usefulness, regarding stroke volume calculations and dP/dt measurement are summarily explained, taking into consideration the usual pitfalls found in daily practice. There is a section dedicated to regional systolic function evaluation, with the recommendations for standardized LV division and differential characteristics of wall motion abnormalities. Additionally, more recent approaches with tissue Doppler imaging and strain analyses for global and regional LV function assessment are described. Finally, a section is dedicated to right ventricle systolic function which describes all modalities of evaluation.

Neuroimaging provides a way of examining the structure and function of the brain in life. Cystic fibrosis (CF) is the most common genetic autosomal recessive disease of the Caucasian race, generally leading to death in early adulthood.1 The frequency of the gene carrier (heterozygote) is 1:20–25 in Caucasian populations, 1:2000 in African-Americans, and practically non-existent in Asian populations. The disease occurs in about 1 in every 2500 life births of the white population. Mean survival has risen from 8.4 years in 1969 to 32 years in 2000 due to improvements in treatment. The genetic defect causes a pathological electrolyte transport through the cell membranes by a defective chloride channel membrane transport protein [cystic fibrosis transmembrane conductance regulator (CFTR)]. With respect to the function, this affects mainly the exocrine glands of secretory cells, sinuses, lungs, pancreas, liver, and the reproductive tract of the human body leading to a highly viscous, water-depleted secretion. The secretion cannot leave the glands and in consequence causes local inflammation and destruction of various organs. The main symptoms include chronic inflammatory pulmonary disease with a progressive loss of lung function, exocrine and sometimes endocrine pancreas insufficiency, and an excessive salt loss through the sweat glands.1 A summary of the signs and symptoms of CF will be given with a special emphasis on the effect of exercise performance and capacity.

This chapter describes the origin of the human lineage within the evolution of the hominoids, which raises the difficult issue of how to integrate the evolution of dentition and terrestrial locomotion. The hypothesis of the adaptation to the open savanna by bipedalism is explored. Abstract:
The introduction sets up the scope and framework of the volume as a whole. Over the past three decades, the study of the film soundtrack has developed into a rich scholarly discipline, characterized by diverse approaches and methodologies. As an object of theoretical focus, the voice has helped correct the long-standing ocularcentrism of film theory discourse. Yet, the voice as a narrow concept is itself problematic in that it limits our understanding of how it functions among the various components of the soundtrack. Understood as part of an integrated soundtrack—that is, a soundtrack where the various components have a sense of being planned or composed and where sound design and music are blended into a kind of conceptual unity—the voice assumes a somewhat different role and allows for a more complex and interpretively richer conceptual framework. This volume aims to reconsider and broaden our notion of what the voice as a concept can do for studies of film music and sound. The introduction explores theoretical concerns relating to film dialogue, vococentrism, the spectacle of the singing voice, film music’s commentative function as voice, as well as the voice of various cinematic authorship and production. It concludes with a brief summary of each chapter in the volume. Considering these many different conceptions of “voice” can provide new perspectives on how we consider the relationship between voice and cinema, broadly defined.

Abstract:
This chapter provides a summary for the volume as a whole and discusses prospects for a cognitive grammar of music going forward. The first portion of the chapter reviews the main questions explored over the course of the book and the answers provided by the theoretical framework that has been set out. The second portion of the chapter sets out three ways a cognitive grammar of music might be furthered: first, through empirical research on processes of categorization, memory function, and analogies involving dynamic processes, all of which are connected with musical understanding; second, through an exploration of the relationship between the familiar constructs of music theory (like intervals, scales, and chords) and the sonic analogs for dynamic processes fundamental to musical grammar; and, third, through a continued investigation into the ways musical materials shape humans’ cultural interactions.
